1. Research funding for male reproductive health and infertility in the UK and USA [2016 – 2019]

Abstract: TitleResearch Funding for Male Reproductive Health and Infertility in the UK and USA [2016 – 2019]Study questionWhat is the research funding for male reproductive health and infertility in the UK and US between 2016 to 2019?Summary answerThe average funding for a research project in male reproductive health and infertility was not significantly different to that for female-based projects (£653,733 in the UK and $779,707 in the US). However, only 0.07% and 0.83% of government funds from NIHR (UK) and NICHD (USA) was granted for male reproductive health, respectively.What is known alreadyThere is a marked paucity of data on research funding for male reproductive health.Study design, size, durationExamined government databases over a total 4-year period from January 2016 to December 2019.Participants/materials, setting, methodsInformation on the funding provided to male-based and female-based research was collected using public accessed web-databases from the UKRI-GTR, the NIHR’s Open Data Summary, and the US’ NIH RePORT. Funded projects that began research activity between January 2016 to December 2019 were recorded, along with their grant and project details. Strict inclusion-exclusion criteria were followed for both UK and US data with a primary research focus of male infertility, reproductive health and disorders, and contraception development. Funding support was divided into three research groups: male-based, female-based, and not-specified research. Between the 4-year period, the UK is divided into 5 funding periods, starting from 2015/16 to 2019/20, and the US is divided into 5 fiscal years, from 2016 to 2020.Main results and the role of chanceBetween January 2016 to December 2019, UK agencies awarded a total of £11,767,190 to 18 projects for male-based research and £29,850,945 to 40 projects for female-based research. There was no statistically significant difference in funding average between the two research groups (P=0.56, W=392). The US NIH funded 76 projects totaling $59,257,746 for male-based research and 99 projects totaling $83,272,898 for female-based research. There was no statistically significant difference in funding average between the two groups (P=0.83, W=3834).Limitations, reasons for cautionThe findings of this study cannot be used to generalize and reflect global funding trends towards infertility and reproductive health as the data collected followed a narrow funding timeframe from government agencies and only two countries. Other funding sources such as charities, industry and major philanthropic organizations were not evaluated.Wider implications of the findingsThis is the first study examining funding granted by main government research agencies from the UK and US for male reproductive health. Abstract: Coronary revascularization is one of the most studied types of interventions in cardiology, but there is no consensus among specialists about the indications for its implementation in patients with chronic coronary syndromes (CCS). The data of recently completed clinical trials on the role of revascularization in CCS clearly contradict the current Guidelines, emphasizing the high effectiveness of modern conservative therapy. This paper discusses the main recommendations of the most significant American and European Guidelines on myocardial revascularization, and also analyzes the appropriateness of revascularization to improve the prognosis and symptoms in chronic coronary syndromes in view of the new research data, primarily the ISCHEMIA study (NCT01471522). Its strengths and limitations are discussed in detail. The data on the expediency of revacularization in CCS, obtained after the completion of ISCHEMIA and its potential significance, as well as subgroup analyses of ISCHEMIA, including in the most important ‘problem’ subgroups (3-vessel disease, proximal LAD disease, severe ischemia on stress test, etc.) are discussed. The paper also discusses the important achievements in modern drug therapy of chronic coronary syndromes, primarily antithrombotic therapy. The data of the COMPASS study (NCT01776424) are discussed, based on which the addition of a second antithrombotic drug – rivaroxaban in a small dose (2.5 mg BID) – is recommended for patients with CCS without atrial fibrillation who have high-risk characteristics. Indications the administration of dual antithrombotic therapy to patients with CCS, comparative results of its various regimens in relation to the prevention of cardiovascular complications, the risk of bleeding and the net clinical effect are given.

3. Parameters of arterial stiffness in hypertensive patients with and without subclinical carotid atherosclerosis

Abstract: Objective. To assess the relationship between arterial stiffness parameters in hypertensive patients with and without atherosclerotic lesions.Design and methods. We included 127 subjects who were divided into 3 groups: patients with hypertension (HTN) without atherosclerosis (n = 42); patients with HTN and subclinical atherosclerosis (SА) (n = 52) and control group which consisted of individuals without HTN, SA, or coronary artery disease (n = 33). All groups matched by age and gender. All subjects underwent following examinations: ultrasonography of extracranial segments of carotid arteries, 24-hour blood pressure monitoring with the assessment of arterial stiffness parameters.Results. In subjects with HTN compared to controls, pulse wave velocity in aorta (PWVao) was significantly higher (11,3 ± 1,5; 12,3 ± 1,8 vs 10,4 ± 1,3 m/s; p < 0,05), as well as pulse pressure (PP) (46,4 ± 9,8; 45,6 ± 10,6 vs 39,9 ± 6,5 mmHg; p < 0,05), central pulse pressure (PPао) (35,5 ± 8,5; 34,9 ± 8,5 vs 30,9 ± 5,4 mmHg; p < 0,05), and arterial stiffness index (ASI) (141 (127, 159); 139 (128,5, 160,5) vs 126 (118, 138) mmHg; p < 0,05). In subjects with HTN and SA, PWVao was significantly higher compared to other groups (p < 0,05). No significant difference in augmentation index was found (–32,5 (–45, –12); –22 (–36, –12); –37 (–50, –17); p = 0,25). Аmbulatory arterial stiffness index was higher in controls (0,5 ± 0,2) compared to HTN group (0,4 ± 0,2; p = 0,05), while HTN and SA group did not differ significantly (0,5 ± 0,2; p = 0,3). PWVao above 11,15 m/s is associated with 4,3 (2,3–8,2) times higher rate of atherosclerosis plaque detection.Conclusions. In HTN patients, arterial stiffness is changed compared to healthy individuals. PWVao above 11,15 m/s is associated with 4,3 (2,3–8,2) times higher rate of atherosclerosis plaque detection. In patients with HTN and SA arterial stiffness is higher, which might have additional predictive value in risk stratification.

Abstract: BACKGROUND: Among patients with stable coronary disease and moderate or severe ischemia, whether clinical outcomes are better in those who receive an invasive intervention plus medical therapy than in those who receive medical therapy alone is uncertain. METHODS: We randomly assigned 5179 patients with moderate or severe ischemia to an initial invasive strategy (angiography and revascularization when feasible) and medical therapy or to an initial conservative strategy of medical therapy alone and angiography if medical therapy failed. The primary outcome was a composite of death from cardiovascular causes, myocardial infarction, or hospitalization for unstable angina, heart failure, or resuscitated cardiac arrest. A key secondary outcome was death from cardiovascular causes or myocardial infarction. RESULTS: Over a median of 3.2 years, 318 primary outcome events occurred in the invasive-strategy group and 352 occurred in the conservative-strategy group. At 6 months, the cumulative event rate was 5.3% in the invasive-strategy group and 3.4% in the conservative-strategy group (difference, 1.9 percentage points; 95% confidence interval [CI], 0.8 to 3.0); at 5 years, the cumulative event rate was 16.4% and 18.2%, respectively (difference, -1.8 percentage points; 95% CI, -4.7 to 1.0). Results were similar with respect to the key secondary outcome. The incidence of the primary outcome was sensitive to the definition of myocardial infarction; a secondary analysis yielded more procedural myocardial infarctions of uncertain clinical importance. There were 145 deaths in the invasive-strategy group and 144 deaths in the conservative-strategy group (hazard ratio, 1.05; 95% CI, 0.83 to 1.32). CONCLUSIONS: Among patients with stable coronary disease and moderate or severe ischemia, we did not find evidence that an initial invasive strategy, as compared with an initial conservative strategy, reduced the risk of ischemic cardiovascular events or death from any cause over a median of 3.2 years. The trial findings were sensitive to the definition of myocardial infarction that was used. 5. Optimization of periodontal disease diagnosis by the results of clinical laboratory tests

Abstract: Relevance. The authors have established that the microbiological and local risk factors prevail in changing the clinical condition of the periodontium. Aim – сlinical and diagnosis argumentation of the gingival tissue condition according to the criteria of the New International Classification of Periodontal and Peri-implant Diseases and Conditions and Proceedings of the 2017 World Workshop jointly held together by the American Association of Periodontology (AAP) and the European Federation of Periodontology (EFP).Materials and methods. Clinical and laboratory assessment of 105 young patients was conducted. Three patient groups were formed according to the detected risk factor and according to the data of the New International Classification of Periodontal and Peri-implant Diseases and Conditions. The main group consisted of 70 (66.6%) patients with diagnosed chronic plaque-induced gingivitis (33.3%) and initial periodontitis (33.3%) (mild chronic periodontitis). The control group comprised 35 patients with clinically healthy gingiva on an intact periodontium (71.1%) and reduced periodontium (22.9%). Periodontal pathogens as a risk factor were assessed by PCR using DNA-express commercial sets (Liteh, LLC, scientific manufacturing company, Russia). Cytology of the gingival crevicular fluid impression smears stained by Romanovsky-Giemsa method was performed.Results. Changes in the hygiene and periodontal indices were revealed on full dental examination. PCR detected low or critical number of periodontal pathogens in the studied samples. Neutrophilic leukocytes, histiocytes and epithelial cells were present in the impression smears, polymorphonyclear neutrophils significantly increased and macrophages, histiocytes, epithelial cells appeared; macrophages decreased.Conclusion. Full dental examination and laboratory tests revealed the following clinical conditions: clinically healthy gingiva on an intact periodontium, clinically healthy gingiva on a reduced periodontium, plaque-induced gingivitis, stage I periodontitis – initial periodontitis, which corresponded to the New International Classification of Periodontal and Peri-Implant Diseases and Conditions and Proceedings of the 2017 World Workshop held by the American Academy of Periodontology (AAP) and the European Federation of Periodontology (EFP).

The composition of supragingival and subgingival dental plaque, gingival and oral fluid, dentogingival or periodontal pocket, are a valuable diagnostic medium for non-invasive diagnosis of inflammatory periodontal diseases, since they contain a wide variety of microorganisms, many of which are markers. Non-invasive diagnosis risk factors, in particular, the presence of periodontal pathogenic microflora, provided for the study of various biotopes of the oral cavity — plaque, oral and gingival fluids, the composition of the dental pocket by applying a high-quality PCR diagnostic method using commercial DNA-express kits (LLC NPF «Litech», Russia). Results. A comparative study of markers of the development of periodontal diseases using a qualitative version of the PCR test system in samples of subgingival plaque and dental pocket contents of individuals with initial stage I periodontitis showed 3.1 times greater prevalence of the representative of the yellow complex — Aggregatibacter actinomycetemcomitans, 1.4 times more often than the representative of the red complex P. gingivalis. In the studied material of supragingival plaque, gingival and oral fluid of individuals with gingivitis induced by dental biofilm, the representative of the yellow complex Aggregatibacter actinomycetemcomitans prevails 2.6 times more often than representatives of the red complex — P. gingivalis and Tannerella forsythia, 1.4 times the orange complexFusobacterium nucleatum. In samples of supragingival plaque in individuals with clinically healthy gums with loss of periodontal tissue, representatives of the microflora that form orange, red and yellow complexes most often predominate. Conclusion. The study of supragingival and subgingival plaque, gingival and oral fluid, and the dentogingival pocket using a PCR test system allows timely detection of markers of the development of inflammatory diseases, regardless of clinical condition of periodontal disease.

Abstract: This paper examines one of the literary techniques in J. R.R. Tolkien’s novel The Lord of the Rings that creates “the depth effect”: texts’ insertion. The article’s author analyzes an insert text from the second part of the prologue (“The Herblore of the Shire”) and its integration in the main plot of the novel, and also looks into interchanges between the main text and insert ones, and Meriadoc’s role as one of the secondary narrators. The Lord of the Rings is full of insert texts of various genres and “The Herblore of the Shire” is merely one of them, but it shows that such texts are not isolated and notlimited to single occasions. Throughout the novel they serve as quotations that link different plot episodes and organize them around the image of the secondary narrator. According to Tolkien’s creative position that he repeatedly stated in his letters, we can say that he carefully contemplated the ways of involving his reader in his fantastic world; that is why it is important to describe narrative and compositional techniques of the novel that served the author’s purpose. Abstract: The features of the chemical composition of formation waters of the Upper Permian, Lower and Middle Triassic aquifers within individual structures of the Malozemelsk-Kolguev monocline of the Timan-Pechora oil and gas bearing basin are examined. It is shown that the aquifers that, despite their remoteness from the recharge areas, had experienced repeated processes of dipping and uplifting of the territory, preserved the infiltration waters, but significantly transformed ones. To identify their genetic profile and determine the relationship with the processes of catagenetic changes, hydrochemical coefficients and indices of water saturation of calcium carbonate and calcium sulfate are calculated. It is established that the examined infiltration waters are characterized by high aggressiveness, dissolution ability, leaching and removal of main minerals. It is pointed out that the consequence of these processes is the formation of secondary void space and the creation of high capacitive and filtration properties of rocks. The universality of geochemical interaction between water and rocks in thermodynamically open geological systems is emphasized and it is confirmed by individual geological examples that the heterogeneity of the hydrochemical field can act as a cause of screening zones of oil and gas accumulation, as well as provide appropriate conditions for localization of hydrocarbon fluids.

This article describes the experience of organizing such training for children in the 4th quarter of the 2019/2020 academic year. It considers all stages of implementation of distance learning, such as solution of legal issues regulating the activities of an educational institution; effective construction of the educational process, considering the important relationship "student-teacher"; the right choice of information technology that meets the capabilities and limited skills of students, and has a user-friendly interface. The experience of distance collaboration between a teacher, a student and his parents is also presented, which made it possible to build effective teaching and achieve a high-quality education.

Abstract: Along with the increasing prevalence of type 1 diabetes among children and adolescents, there is a growth of rare variants of the disease — type 2 diabetes and monogenic forms of diabetes. Objective. To study the characteristics of clinical course of diabetes mellitus in patients under 18 years, depending on the type of disease. Children characteristics and research methods. The researchers compared age, gender, and clinical and laboratory characteristics in the debut of the pathology in 43 patients with an atypical manifestation of type 1 diabetes mellitus. Results. Despite the non-standard clinical manifestations of the disease, 62.8% of the patients in this cohort account for children with type 1 diabetes mellitus, 23.3% — for patients with MODY diabetes (23.3%), and 13.9% — for type 2 diabetes mellitus. There were no clinical and laboratory markers (due to insufficient number of patients), such as: burdened heredity, patient weight, level of residual secretion of insulin, glycemia, glycated hemoglobin at the moment of manifestation, which could be used as a guideline for the differential diagnosis. This fact justifies the usage of the entire range of laboratory and molecular genetic studies to verily the diagnosis.

Abstract: Currently the oncological mortality takes the second place globally, the leading cause being cardiovascular diseases. The statistics of malignant neoplasms is rather negative all over the world. 10 million of cases of oncological disorders are diagnosed annually; this means that 27 million people fall sick with oncological diseases annually. It was established in 2019 that there are 14 million people suffering from oncological diseases, 8.2 million of these die. WHO anticipates that in 20 years’ time the malignant neoplasm incidence statistics will be on an increase as the number of new cases will reach 20 million, 12 million out of which will die. Regardless of such formidable figures medicine does not stand still; keeping up with the times, the science attempts to develop cutting edge methods of treating malignant tumours. As a result, the treatment of malignant neoplasms is continuing to improve. However, the number of side effects is also growing, thus requiring research attention. Therefore, the significance of the impact that oncological drugs have on a patient’s body is becoming more and more urgent for further discussion. While current tumour treatment methods involving drugs such as tyrosine kinase inhibitors, anthracycline chemotherapy and immunotherapy protocols are effective for the treatment of various forms of cancer, these drugs affect the DNA replication process thus resulting in endothelial dysfunction and nonspecific immune response. This causes cardiotoxic side effects. Cardiotoxicity, in its turn, is a notion that includes various adverse events involving the cardiovascular system of oncological patients receiving drug treatment. Cardiotoxicity may develop during treatment or following its completion.

Abstract: Gnaphalium uliginosum L. (Asteraceae) is widely used in phytotherapy and has a potential for agricultural utilization. The ethanolic extract was obtained from the maceration of air-dried plants, and the extract was then filtered and concentrated using a rotary evaporator. The essential oil was pressed from freshly harvested and crushed, aerial part of plants. The chemical compositions of ethanolic extract and essential oil of G. uliginosum L. (EOG) were investigated using gas chromatography/mass spectrometry (GC/MS). Nine constituents accounted for 77.3 % of the total detected components were determined at the ethanolic extract with a high proportion of sterols 42.8%, fatty acids 24.1 %, triterpenes 14.4 %. Twenty constituents were identified in essential oil of G. uliginosum L.; with α-pinene and estragole accounting for 54.0 %. The ethanolic extract was found to have a moderate antimicrobial activity (MIC values varied from 125 to 500 μg/mL), for S. aureus, B. cereus and A. solani significant activity (less than 62.5 μg/mL). The essential oil was found to have a moderate (MICs 500-1000 μg/mL) and strong activities (31.3-250 μg/mL). The antioxidant activity was evaluated using chemiluminescent assay. The ethanolic extract of G. uliginosum L. acted as a medium-strength antioxidant at concentrations of less than 0.1 mg/mL. The essential oil made be regarded as a weak strength antioxidant.

Abstract: The need to create favorable conditions in schools to support gifted and talented students is due to the social order and humanistic paradigm of modern education. This article deals with the problem of developing personal qualities of students in the framework of pedagogical support for students with signs of giftedness in the modern school. The article reveals the results of the study of personal development of students accompanied by teachers in the process of designing and implementing individual educational routes at the main stage of general education. The leading method of studying this problem is the analysis of the results of the survey of teachers and schoolchildren in order to determine changes in the manifestation and development of personal qualities of students with signs of giftedness, accompanied by teachers, in the process of implementing their individual educational path using the model of the educational route developed by the authors of the article. The summary of the research presented in the article is the development and testing the structural-functional model of individual educational route for students with signs of giftedness, the use of which contributes to the manifestation and development of personal qualities of students required for the development of their potential and achieving educational outcomes. The materials of the article are of practical value for teachers when planning individual work at school.

Abstract: We report that two widely-used drugs for erectile dysfunction, tadalafil and vardenafil, trigger bone gain in mice through a combination of anabolic and antiresorptive actions on the skeleton. Both drugs were found to enhance osteoblastic bone formation in vivo using a unique gene footprint and to inhibit osteoclast formation. The target enzyme, phosphodiesterase 5A (PDE5A), was found to be expressed in mouse and human bone as well as in specific brain regions, namely the locus coeruleus, raphe pallidus, and paraventricular nucleus of the hypothalamus. Localization of PDE5A in sympathetic neurons was confirmed by coimmunolabeling with dopamine β-hydroxylase, as well as by retrograde bone-brain tracing using a sympathetic nerve-specific pseudorabies virus, PRV152. Both drugs elicited an antianabolic sympathetic imprint in osteoblasts, but with net bone gain. Unlike in humans, in whom vardenafil is more potent than tadalafil, the relative potencies were reversed with respect to their osteoprotective actions in mice. Structural modeling revealed a higher binding energy of tadalafil to mouse PDE5A compared with vardenafil, due to steric clashes of vardenafil with a single methionine residue at position 806 in mouse PDE5A. Collectively, our findings suggest that a balance between peripheral and central actions of PDE5A inhibitors on bone formation together with their antiresorptive actions specify the osteoprotective action of PDE5A blockade.

Abstract: The current state of oil fields in Western Siberia is characterized by a massive occurrence of deposits in the late stage of development, accompanied by high water cut of well production while maintaining significant residual oil reserves. This encourages oil companies to resort to large-scale introduction of methods of limiting water inflows and enhanced oil recovery. This primarily applies to the technologies of the leveling of the injectivity profile in the injection wells. Another feature of the current state of the oil industry is the massive implementation of measures to intensify oil production. This is optimization of operating modes of producing wells by lowering the bottomhole pressure below the gas saturation pressure, stimulation of wells by hydraulic fracturing and bottomhole formation treatments. When choosing wells for the development of such activities, the question of their influence on the rate of water production is usually not considered. At the same time, in practice, a large number of cases of growth of water cut in well production after geological and technical measures in them to accelerate fluid withdrawals, as well as cases of a decrease in the rate of watering, are noted in practice. Another important feature of the current state of the oil industry is the massive development of deposits of hard-to-recover oil reserves. First of all, this applies to deposits with low-permeability reservoirs, on which stimulation of wells with fracturing is massively applied. It also concerns waterfowl deposits of oil with massive watering of wells by means of stabbed overflows. The task of reducing the rate of decline in production and the recovery of residual stocks requires the use of new technologies. At the same time, the preferred technologies are those that do not require capital expenditure for implementation. Such flow-diverting technologies include the injection of crosslinked polymer compositions based on polymers of a series of acrylamide and cross-linkers of polyvalent metal salts.

Abstract: The article discusses the formation of a new approach to the statutory regulation of 3D printing, an uncommon object of civil matters. Creating a product with the help of such technology not only by an entrepreneur but also by any person, can in practice cause some issues in protecting all participants’ rights during the 3D printing process. 3D printing is regarded as a complex object, so its characteristics are determined. Additionally, possible ways of resolving legislative gaps are proposed, such as: defining the concept of the manufacturer, supplier, seller, and consumer of 3D printed products; developing their rights and responsibilities; establishing quality requirements for 3D printed elements; and adopting clear rules on liability and consumer protection. Abstract: Alcoholic liver disease is a huge medical and social problem that leads to liver fibrosis, cirrhosis, and hepatocellular carcinoma. Unfortunately, the last stages of the disease do not have efficient treatment, except liver transplantation, and require development of new therapeutical approaches. Transplantation of stem cells might be the most promising approach. In our research, we studied transplantation of autologous hematopoietic stem cells (HSC) into the celiac trunk of patients with alcoholic liver cirrhosis. In this article, we pay particular attention to proliferation and apoptosis—two fundamental processes, which determine the fate of regeneration. Liver biopsy specimens before treatment, 3 and 12 months after transplantation of HSC, were stained immunohistochemically with antibodies against PCNA and Bcl-2. The results showed that treatment was safe and effective, hepatocytes increased proliferation, and inflammatory cells decreased antiapoptotic activity, signifying improvement in liver regeneration. However, effect of treatment after 12 months decreases and requires repeated HSC transplantation.

Abstract: Background: The beginning of a new era of direct acting antivirals sets up its own rules, that is, to achieve the highest efficacy with the shortest duration of treatment. It is assumed that the use of the first generation of direct acting antivirals, similarly to interferon-free regimens, would allow for personalization of approaches to their prescriptions. Aim: To identify the most important parameters that can predict the greatest efficacy of triple antiviral therapy of 12 week duration in patients with chronic hepatitis C genotype 1. Materials and methods: The study included 204 patients with chronic hepatitis C virus (HCV) genotype 1 at an early stage of liver disease (METAVIR score F0-F2), who were either treatment-naive or had a history of relapse after standard of care antiviral therapy. In addition to routine work-up, all patients were screened for IL28B polymorphism; in the course of the treatment viral kinetics was assessed by an ultrasensitive polymerase chain reaction (PCR) (with lower limit of quantification of 12 IU/ml). Duration of the triple therapy (pegylated interferon-α2a, ribavirin and telaprevir) was reduced to 12 weeks if a rapid virological response was achieved; otherwise the patients continued their treatment in according with guidelines. Results: A complete rapid virological response was achieved in 174 patients (81.6%), in whom the duration of triple therapy was 12 weeks. According to the protocol, 25 patients with a partial rapid virological response continued their standard antiviral therapy for 12 weeks more. In those who achieved a rapid virological response, there was an association between IL28B-CC genotype at rs12979860 and maintenance of zero viremia at 12 weeks after termination of antiviral therapy (r = 0.38, p < 0.001). In all such patients there was a stable virological response at 12 weeks of the follow-up. Monitoring of viral load after 14 days of antiviral treatment was not predictive of its success. The preliminary results of a shortened (12 week) course of triple telaprevir-based viral therapy allowed to identify the most significant parameters of 100% efficacy, i.e., absence of the virus in blood at 12 weeks after termination of antiviral therapy. Conclusion: A 12 week course of triple telaprevir-based combination therapy is an optimal regimen for achievement of a stable virological response after 12 weeks of the follow-up in treatment-naive patients with HCV genotype 1 or with a relapse after previous conventional antiviral treatment, who have IL28B – CC polymorphism, are at an early stage of liver disease and who achieve a rapid complete virological response confirmed by a highly sensitive PCR.

Abstract: On the example of a typical residential multi-apartment building, a feasibility study was carried out on the choice of energy-saving measures for the thermal insulation of facades. The decision to increase the energy efficiency of the building was made on the basis of calculating the loss of thermal energy through the external walls. Based on the parameters of the heating period, capital costs for additional thermal insulation of facades and calculated values of operating costs for heating, the optimum thickness of the additional layer of insulation is determined, in which the payback period assumes a minimum value.

Abstract: Peripheral arterial diseases are characterized by a progressing tissue ischemia which results in the invalidization of patients. The aim of our research was to study the morphological effects of autologous peripheral blood stem cells intramuscular injection into patients with peripheral arterial disease. Peripheral blood stem cells were transplanted intramuscularly into a 48-year-old male patient with peripheral arterial disease stage IIb by Fontaine. The biopsies of his gastrocnemius muscle were taken before the stem cells were transplanted and 3 months after transplantation. These biopsies were stained with H&E and also with antibodies against CD34, myogenin, caspase 3, and bcl-2. Immunohistochemical study results showed an increase of capillary density of 32.7 % (P = 0.005). In muscular biopsies obtained before therapy, we identified single myogenin+ myosatellite cells, while 3 months after transplantation we detected the presence of cells with myogenin-positive nuclei and multinucleated myotubes. We also observed the formation of young myogenin+ muscle fibers with central nuclei. There was no significant difference in the expression of caspase-3 before transplantation and 3 months after transplantation. An increased number of bcl-2+ myosatellite cells, myotubes, and muscle fibers were detected after transplantation. The patient’s ankle-brachial index increased by 13.56 % (0.59 before and 0.67 3 months post transplantation). The patient’s pain-free walking distance by 89.97 % (from 59.56 to 113.77 m). Control arteriograms showed the formation of new collaterals. Transplanted autologous peripheral blood stem cells stimulated the formation of new capillaries, the activation of myosatellite cells and bcl-2 expression in muscle fibers.

Abstract: Indications for coronary revascularization in patients with stable coronary heart disease (CHD), presented in the current international guidelines are largely based on the clinical trials conducted sometimes more than 30 years ago. ISCHEMIA is the major multicenter international randomized trial intended to answer the question about the optimal treatment strategy in stable coronary artery disease at the present time. Purpose. To analyze the most important baseline characteristics of patients enrolled in the ISCHEMIA study in one Russian site. Methods. The principal inclusion criteria are a positive stress test (in our center, exercise stress echocardiography) of at least moderate risk and obstructive coronary heart disease confirmed by coronary computed tomography angiography (CCTA). Main exclusion criteria are a significant stenosis of the left main coronary artery by CCTA, clinical progression/destabilization of the ischemic heart disease, angina of IV CCS class, left ventricular ejection fraction (EF) 80% -beta blockers, >80% - ACE inhibitors/ARBs. ^ey are at well controlled resting heart rate ( 5 METs, the median ischemic area was 4.0 (3.5-5.5) segments, in 71% the ischemia in the left anterior descendent artery region was provoked. The ratio of interventional to surgical revascularization in the invasive group was 1.2, indicating anatomically diffuse CHD in 45% of these patients. Conclusion. In ISCHEMIA pts enrolled in our center, the main RFs, except the lipid-related, were well controlled at baseline; the lipid-related RFs improved after 6 months. Ischemic symptoms were mild or moderate, but the variables of the stress test and low PCI:CABG ratio indicate the study pts are moderate or high risk pts.

Abstract: The article describes possibilities of computer modeling of fire resistance for structures in high-rise buildings, which include new fire-resistant plaster compositions. As an example of program complex, “Sofistik” (Germany) was chosen for the calculation of reinforced concrete structures of tier buildings. The characteristics of fire protection compounds and their regulation, which are usually used, also are shown. According to the new code “Highrise buildings and complexes. Fire safety requirements”, limits of fire resistance for walls of not less than 240 minutes. Skyscraper “Lakhta center” with a height of 462 m and limits of fire resistance of load-bearing reinforced concrete structures 180 and 240 minutes is built up in Russia. The article investigates the means of fire protection in the business center. Moreover, modern form of cladding in Mada Residences building (Dubai, UAE) is concerned to make some conclusions about future fire resistant solutions.

Abstract: Execution of thermal insulation for enclosing structures of residential properties is a sophisticated part of the thermal protection. The choice of insulation materials for external walls is one of the main ways to increase the performance of heat insulation. This article contains thermo-technical calculations of two enclosing structures due to conditions of Saint-Petersburg. According to the results, the thickness of the insulating layer and a total wall thickness of the exterior walls composed of concrete, insulation and sand-cement rendering are smaller than the same characteristics of wall construction of hollow tile, insulation and sand-cement rendering.

Abstract: Although Russia’s WTO-accession is foreseeable, there are still deficits in the literature on its concrete effects on the sectoral and regional levels. In this paper we analyse effects on Russia’s chemical and petrochemical sectors (CPS), which are, unlike for example its oil and mineral gas industry, rarely in the focus of public attention. On the basis of an extensive examination of these sectors’ current condition, we expect serious problems for them to emerge in the nearer future, the perhaps most serious and most puzzling problem being under-investment.
