Abstract

ObjectivesEndometrial carcinoma mortality is mainly caused by recurrent disease, and various immunohistochemical markers to predict recurrences have been studied. Loss of the estrogen receptor (ER) and progesterone receptor (PR) and the presence of the L1 cell adhesion molecule (L1CAM) are promising markers, but their combined value has not been studied.Materials and MethodsExpression of ER, PR, and L1CAM was immunohistochemically determined in 293 endometrial carcinomas from 11 collaborating European Network for Individualized Treatment of Endometrial Cancer centers. Estrogen receptor, PR, or L1CAM staining was considered positive or negative when expressed by greater than or equal to 10% or less than 10% of the tumor cells, respectively. The association between these markers and clinicopathological markers, and their combined value in predicting survival were calculated, both in the entire cohort and in a selected groups of stage I endometrioid and low-risk stage I endometrioid carcinomas.ResultsEstrogen receptor and PR were negative in 19% and 28% of the cases, respectively, and L1CAM was positive in 18%. All 3 were associated with advanced stage, high-grade, nonendometrioid histology, lymphovascular space invasion (LVSI), and reduced disease-free survival. Only advanced stage, loss of PR, and LVSI were associated with reduced disease-free survival in multivariate analysis. A prognostic model including these 3 markers was superior to 1 including only the 3 immunohistochemical markers, which was superior to the traditional model. In both the stage I endometrioid and the low-risk stage I endometrioid groups, only loss of PR was associated with reduced disease-free survival.ConclusionsLoss of ER and PR, and the presence of L1CAM are associated with high risk characteristics, and loss of PR is the strongest predictor of recurrent disease. Although a combination of these 3 markers is slightly superior to the traditional histological markers, a prognostic model including stage, PR expression, and LVSI is the most promising model in the identification of high risk carcinomas. In the stage I endometrioid carcinomas, PR immunohistochemistry appears to be of additional value in predicting recurrences.

Abstract

Endometrial carcinoma is the most frequent genital tract malignancy. The first symptom (guide-symptom) is usually metrorrhagia; however, in around 10% of cases it is not. In contrast, osseous metastases are infrequent in endometrial cancer. The bones of the pelvis and lower extremities are those most frequently involved in disseminated metastatic diseases. In these cases, endometrial cancer is usually high grade (Grade III). Case report: 56-year-old woman who presented right inguinal pain. The X-ray showed a lithic lesion in the right ischium. A histopathological study demonstrated a metastatic lesion, suspected to be endometrial cancer. The computer tomography scan revealed a uterine mass and a second lithic lesion in the right tibia. The patient received chemotherapy (carboplatin and paclitaxel), and the bone lesions were irradiated. The patient is still alive 18 months after the diagnosis. This case emphasizes the importance of considering an endometrial primary tumor when evaluating bone metastasis of unknown primary cancer.

Abstract

Objective To determine the prevalence of human papillomavirus (HPV) in urine samples from women with high-grade cervical lesions. Secondary objectives are to identify the influence of socio-demographic factors and the different genotypes with urinary HPV positivity. Study design 75 women with a positive biopsy for CIN2+ were included in the study from October 2010 to July 2011. A sample of urine was collected immediately before conization at the outpatient clinic. We analyzed the presence of HPV using a PCR technique. Results The mean age of the patients was 34.8 years (range 24 to 61). All patients had histological CIN2+, of whom 54.67% had CIN3. The prevalence of HPV in urine test was 58.82% in CIN2 population versus 78.05% in CIN3 patients (p 0.072). 31 different genotypes were found. The most frequent HPV genotype was 16-HPV, which was identified in 58% of women with positive HPV-DNA in urine samples. No demographic characteristics were significantly associated to urinary HPV prevalence. Conclusion Most of the patients with CIN2+ showed positive results for urine HPV test. The prevalence of positive urinary HPV test was higher for patients with CIN3. HPV urine detection could be considered as an acceptable option for high-risk population who skip regular screening programs.

Abstract

Background MET signaling has a role in gliomagenesis and glioma stem cell maintenance and midkine (ALK ligand) promotes resistance of glioma cells to anticancer therapies. Crizotinib is an ALK and c-MET inhibitor with a preclinical rationale to be tested in newly diagnosed GB. Methods Elegible patients received crizotinib in addition to standard RT and TMZ and then adjuvant TMZ. Maintenance treatment with crizotinib beyond 6 TMZ cycles was allowed. The primary objective was safety evaluation. Secondary objectives included efficacy (progression free survival (PFS) and overall survival (OS)) and an exploratory biomarker analysis. PFS and OS were estimated with Kaplan–Meier method. The results of the dose-escalation cohort (DE) have been reported previously. 250 mg/d was the crizotinib dose selected for the expansion cohort (EC). We report here safety and efficacy for the whole cohort. Results 38 patients (pts) were enrolled, 37 evaluable for safety and 36 for efficacy (12 included in DE). Median age 52 years (33-76). 44%were male. Median KPS 90%, Barthel 100%. 44% were MGMT methylated and 3 pts had IDH1/2 mutation. Most common related adverse events (AE) (all grades) included: nausea (67.6%), asthenia (62.2%), transaminase elevation (40.5%), neutropenia (32.4%) thrombocytopenia (29.7%), diarrhea (29.7%), anorexia (29.7%), vomiting (27%) and constipation (24.3%). In the EC 8/25 pts (32%) presented grade ≥3 AEs (transaminase elevation, thrombocytopenia). 97.2% finished concomitant therapy. 94.4% initiated adjuvant treatment, 67.7% completed 6 TMZ cycles. 18 pts (50%) started maintenance therapy. 8 pts are still on treatment. At the time of this analysis 24 pts have progressed and 1 died without progression. Median follow up was 13.7 months (m), median PFS was 10.78m (95% CI, 7.61-13.94), with 6 month PFS and 12 month PFS of 71.6% and 40.2% respectively. Median OS was 31.4 m(95% CI, 12.64-50.10) with 12month OS of 78.9% and 24month OS of 56.6%. Conclusions In this phase Ib study addition of crizotinib to standard RT and TMZ was safe and resulted in highly promising efficacy for newly diagnosed GB, deserving further investigation. Abstract

Optimal identification of endometrial carcinoma (EC) patients at high risk of recurrence is currently lacking. Hypoxia is an important feature of aggressive EC leading to activation of hypoxic and angiogenetic target genes. The present study investigates the prognostic role of hypoxia and angiogenesis in EC. Data and tissues were used from 11 collaborating European Network for Individualized Treatment of Endometrial Cancer (ENITEC) centers. Tumor slides were stained for CAIX as a hypoxic marker and CD-34 for assessment of microvessel density (MVD) as a marker for angiogenesis. Complete slides were digitalized and analyzed using ImageJ software after exclusion of areas without tumor. A cutoff of 1% for the fraction of CAIX positive tumor cells was used. The MVD was assessed according to the Weidner method with the median as cutoff. Correlations with disease-specific survival (DSS), disease-free survival (DFS) and distant disease-free survival (DDFS) were calculated using Cox regression analysis. Sixty-three (16.4%) of 385 ECs showed positive CAIX-expression with high vascular density. Multivariable analysis showed that ECs with combined positive CAIX-expression and high vascular density had a reduced DSS (hazard ratio [HR] 3.71, p = 0.002) and DDFS (HR 2.68, p = 0.009) and a trend for reduced DFS (HR 1.87, p = 0.054). Multivariable analyses with CAIX-expression and vascular density as separate markers, showed that both were independent prognostic markers as well. This study found an impaired DSS and DDFS in ECs with positive CAIX-expression and high vascular density. Differential adjuvant treatment might be indicated for these ECs. Abstract

The aim of the study was to find morphological changes in the feto-placental unit due to prenatal exposure to drugs of abuse. A blind histomorphometric study was performed using 225 placentas. Based on meconium testing, the fetuses were classified as exposed or unexposed to opiates, cocaine, cannabis or alcohol. To establish prenatal tobacco exposure, cotinine in cord blood was analyzed. At the microscopic level a non statistically significant reduction of placental vascularization was observed in cocaine, opiates and alcohol using mothers. In addition, alcohol-consuming mothers did not present with larger placental vessel diameter than controls. Prenatal use of cocaine and tobacco was associated with a decrease in newborn weight and length. Furthermore, tobacco use was associated with a higher rate of previous abortions. In conclusion, placentas from mothers using tobacco, cocaine, opiates or alcohol during pregnancy present vasculature changes that may explain the adverse perinatal outcomes in their newborns.

Abstract

Resumen Introduccion La neoplasia intraepitelial anal es una lesion precursora del carcinoma escamoso anal. Se considera poblacion en riesgo de padecer esta lesion a los pacientes con condilomas anogenitales, historia previa de displasia de cervix, infeccion por VIH y en general los pacientes con infeccion por el VPH. El objetivo de este estudio es analizar los resultados de la aplicacion de un protocolo diagnostico de neoplasia intraepitelial anal en poblacion de riesgo mediante el empleo de citologia anal. Material y metodo El protocolo diagnostico de neoplasia intraepitelial anal consistio en realizar una anamnesis estructurada, exploracion fisica y citologia anal, la cual se interpreto mediante los criterios de Bethesda. En este estudio observacional de corte transversal se analizan los resultados de diagnostico de neoplasia intraepitelial anal y su asociacion con factores de riesgo. Resultados Se incluyo a 64 pacientes en los que se diagnosticaron 25 alteraciones citologicas: 9 alteraciones citologicas de significado incierto o ASCUS, 15 casos de neoplasia intraepitelial anal de bajo grado y 1 de alto grado. Al relacionar la presencia de alteraciones en la citologia anal con los factores de riesgo conocidos, no hubo asociacion estadisticamente significativa con la presencia de condilomas (p=0,22), infeccion por VPH de alto riesgo (p=0,84), infeccion por VIH (p=0,98) o tabaquismo (p=0,14). Conclusiones La aplicacion de un protocolo de deteccion de neoplasia intraepitelial anal en poblacion de riesgo ha permitido detectar un 25% de pacientes con lesiones precursoras de carcinoma anal.

Abstract

Introduction Anal intraepithelial neoplasia is a precursor condition of squamous anal carcinoma. The groups at risk of this lesion are patients with anogenital condylomata, cervical dysplasia, human immunodeficiency virus infection and, in general, patients with HPV infection. The aim of this study was to analyse the results of a diagnostics protocol of anal intraepithelial neoplasia in high risk population using anal cytology. Patients and method The protocol is based on a visit in the outpatient department, clinical interview, physical examination, and anal cytology evaluated by Bethesda criteria. The cross-sectional observational study was designed to study the anal smear results and their relationship with risk factors. Results A total of 64 patients were included from January 2005 to December 2006. In the overall series, 25 patients have been diagnosed with abnormal anal cytology: 9 atypical squamous cells of undetermined significance (ASCUS), 15 low-grade, and 1 highgrade squamous intraepithelial lesions. There were no significant associations between abnormal cytology results and the presence of anal condyloma ( P = .22). Neither were there statistical associations found with high risk-HPV infection ( P = .84), HIV infection ( P = .98), or tobacco use ( P = .14). Conclusions Our diagnostic protocol of anal intraepithelial neoplasia revealed 25% of patients with pre-invasive lesions of squamous anal cancer.

Contains fulltext : 165692.pdf (Publisher’s version ) (Open Access) BACKGROUND: Identification of aggressive endometrioid endometrial carcinomas (EECs) and non-endometrioid carcinomas (NEECs) is essential to improve outcome. L1 cell adhesion molecule (L1CAM) expression is a strong prognostic marker in stage I EECs, but less is known about L1CAM expression in advanced-stage EECs and NEECs. This study analyses L1CAM expression in a clinically representative cohort of endometrial carcinomas. METHODS: The expression of L1CAM was immunohistochemically determined in 1199 endometrial carcinomas, treated at one of the European Network for Individualized Treatment of Endometrial Cancer (ENITEC) centres. Staining was considered positive when >10% of the tumour cells expressed L1CAM. The association between L1CAM expression and several clincopathological characteristics and disease outcome was calculated. RESULTS: In all, L1CAM was expressed in 10% of the 935 stage I EECs, 18% of the 160 advanced stage EECs, and 75% of the 104 NEECs. The expression of L1CAM was associated with advanced stage, nodal involvement, high tumour grade, non-endometrioid histology, lymphovascular space invasion, and distant recurrences in all cases, and with reduced survival in the EECs, but not in the NEECs. CONCLUSIONS: The expression of L1CAM is a strong predictor of poor outcome in EECs, but not NEECs. It is strongly associated with non-endometrioid histology and distant spread, and could improve the postoperative selection of high-risk endometrial carcinomas. The value of L1CAM expression in the preoperative selection of high-risk endometrial carcinomas should be studied.

10. Helicobacter pylori y cáncer gástrico: relación entre el subtipo histológico y la localización del tumor

Abstract

Resumen Introduccion La infeccion por Helicobacter pylori (HP) se ha implicado en la patogenia del adenocarcinoma gastrico. Se han publicado resultados contradictorios respecto a la infeccion por HP y su relacion tanto con el subtipo histologico como con la localizacion del tumor; hay pocos datos al respecto en nuestro medio. El objetivo del estudio ha sido la evaluacion de estos aspectos en nuestra serie de pacientes con adenocarcinoma gastrico. Material y metodo Evaluacion retrospectiva de todos los pacientes diagnosticados de neoplasia gastrica en el Hospital del Mar de Barcelona en el periodo comprendido entre 1995 y 2001. El subtipo histologico se ha establecido basandonos en la clasificacion de Lauren. Las muestras de tejido se obtuvieron a partir de la pieza de reseccion o de las biopsias realizadas con endoscopia. La infeccion por HP se determino histologicamente mediante tinciones con hematoxilinaeosina, tricromico de Masson y Giemsa. Resultados En el periodo del estudio se diagnosticaron 304 neoplasias gastricas: 275 (90,4%) adenocarcinomas, 22 (7,2%) linfomas, 3 (1,0%) leiomiosarcomas, 2 (0,7%) tumores del estroma gastrointestinal (GIST) degenerados y 2 (0,7%) sarcomas de Kaposi. En relacion con los pacientes con adenocarcinoma, la edad media en el momento del diagnostico fue de 69 anos y el sexo predominante varon (62%). En antro se localizo el 48,1% de las neoplasias; en cuerpo, el 23,7%, y en el fundus, el 19,1% (el 13,6% en el periodo 1994-1997 y el 25,4% en el periodo 1998-2001; p = 0,018). Se ha observado un patron intestinal en el 56% de los casos, difuso en el 28% e indeterminado en el 16%. En el 69% de los casos se confirmo la infeccion por HP (el 68% en el subtipo intestinal, el 69% en el difuso y el 69% en el indeterminado; p = 0,84), y de manera significativa en la mayoria de los adenocarcinomas distales respecto a los proximales (el 73,6 frente al 48,6%; p Conclusiones No se han observado diferencias entre el tipo histologico de adenocarcinoma y la infeccion por HP. En los ultimos anos se ha constatado un incremento en los adenocarcinomas de localizacion fundica; en estos tumores hay una menor asociacion con la infeccion por HP.

Abstract

OBJECTIVE Our aim was to determine whether the Ki-67 immunostaining pattern, present on diagnosis of cervical intraepithelial neoplasia (CIN), predicts the change from low-grade to high-grade CIN over a 2-year period after diagnosis. MATERIALS AND METHODS Of 59 cervical biopsy samples from 59 patients diagnosed as having cervical CIN, 35 were diagnosed as CIN 1 and 24 were diagnosed as CIN 2 or CIN 3. The Ki-67 immunostain showed immunopositive cells in the upper two thirds of the epithelium in all specimens. Two hundred nuclei were counted in 25 high-power fields in each specimen, including all of the epithelial layers, to determine the mean number of Ki-67-positive cells. In situ hybridization was used to demonstrate and type human papillomavirus. The chi test, Fisher exact test, Student t test, one-way analysis of variance, and Tukey test were used for statistical analysis, with significance set at p < .05. RESULTS The mean Ki-67 labeling index for CIN 1, CIN 2, CIN 3, and CIN 2,3 were, respectively, 32.5%, 43.2%, 53.2%, and 47.8%. The statistical study showed significant differences between CIN 1 versus CIN 2, CIN 1 versus CIN 3, and CIN 1 versus CIN 2,3. For CIN 1, the mean Ki-67 labeling index was 32.8% when the lesion disappeared and was 34.6% for persisting lesions. There was no statistically significant difference. CONCLUSIONS Ki-67 labeling index did not predict persisting CIN 1.

Abstract

Twelve adult New Zealand white rabbits were randomly divided into two groups. In group 1, 30% of the central mass of the right patellar tendon was removed. In group 2, 60% was removed. The left knees served as controls. The animals were killed 1 year later. The patella-patellar tendon-tibial tuberosity units of all knees were studied using histological and morphometric analysis. In both groups, the tendons had lengthened. Lengthening average was 2.50 mm in group 1 and 8.17 mm in group 2. In both groups, histology revealed poor alignment of the collagen fibres and high cellularity, although the findings in group 1 were nearer the normal histological pattern. The results suggest that removal of significant portions of the patellar tendon leads to lengthening of the resulting tendon. In clinical practice, it seems prudent to pay attention to the dimensions of the patellar tendon when harvesting a graft.

Abstract

Endometrial carcinoma is the most common gynaecological malignancy in the western world and the most frequent among infiltrating tumours of the female genital tract. Despite the characterisation of molecular events associated with the development of endometrial carcinoma, those associated with the early steps of infiltration and invasion in endometrial cancer are less known. Deep myometrial invasion correlates with more undifferentiated tumours, lymph-vascular invasion, node affectation and decreased global survival. In this review we present an overview of the molecular pathology of myometrial infiltration that defines the initial steps of invasion in endometrial cancer. Down-regulation of E-cadherin as a main player of epithelial to mesenchymal transition, as well as modifications on other molecules involved in cell-cell contacts, render cells with a migratory phenotype. In addition, altered signalling pathways and transcription factors associate with myometrial invasion, histologic grade and metastasis. Fil: Abal, M.. Abstract

The betel nut is commonly used as a drug by Asian populations. A high prevalence of adverse pregnancy outcomes has been reported in women who chewed betel quid during gestation. The hypothesis that chronic exposure of the fetus to arecoline (the principal alkaloid of the areca nut) is the cause was investigated in a clinical observational study on six newborns from Asian mothers who chewed betel nut during pregnancy.

Abstract

Objective: to determine the prevalence of Helicobacter pylori infection in patients having undergone gastrectomy for non-neoplastic disease who later developed gastric stump cancer. Material and methods: retrospective study of all patients with partial gastrectomy for non-malignant peptic disease who were submitted to an endoscopic exploration between 1995 and 2001. A comparison was made of major clinical and histological characteristics, and the presence of Helicobacter pylori among patients with and without gastric cancer in the stomach remnant. Results: a total of 73 patients were studied in this period. Fifteen patients (20.5%) had remnant-stump gastric cancer. All but one were adenocarcinomas (71% intestinal and 29% diffuse, respectively). The average time between diagnosis of gastric cancer and previous gastrectomy was 32 (14-48) years. There was a higher detection rate of Helicobacter pylori in patients with cancer in the gastric remnant (100 vs. 81.5%, respectively, p < 0.07). No relationship was seen between type of gastric reconstruction (Billroth I or II) and rate of Helicobacter pylori detection. Conclusions: Helicobacter pylori infection is frequent in patients with previous gastrectomy for non-neoplastic disease. The results of the study suggest that Helicobacter pylori infection may play a role in gastric stump cancer.

21. 3. Cavernomatosis múltiple ¿Una cuestión de diagnóstico?

Abstract

La cavernomatosis multiple se presenta con una frecuencia alrededor del 20%, segun los metodos radiologicos empleados para su diagnostico en las diferentes series. Un 75% de estos casos son familiares, habiendose encontrado una penetrancia radiologica mayor que la clinica, ya que hasta un 60% pueden ser portadores asintomaticos. La literatura recoge los casos de unas 50 familias, 5 de ellas de Espana. Los cavernomas pueden encontrarse a lo largo de todo el sistema nervioso central, tanto intracranealmente como en la medula espinal, en otros organos y sistemas (higado, rinon), y asociados a lesiones cutaneas. Se trata de lesiones dinamicas, variables en el tiempo, con potencial de crecer, sangrar, calcificarse e incluso surgir de novo. Los casos familiares presentan cierta tendencia a presentar hemorragias sintomaticas mas pronto y con mayor frecuencia que los aislados, por lo que debe insistirse en su diagnostico y seguimiento. Presentamos tres casos de cavernomatosis cerebral multiple, dos de ellos con probada extension familiar. Una de las familias presentaba lesiones cutaneas asociadas. El estudio con la secuencia adecuada de RMN demostro la presencia de multiples lesiones no diagnosticadas por otros metodos y ha permitido comprobar la variabilidad de las lesiones en el tiempo de seguimiento. Planteamos el problema de la indicacion terapeutica en estos casos.

Abstract

Con motivo de presentar la casuistica de los TC (tumor carcinoide) recopilados por los autores, se efectua una revision del tema, recordando su potencial de malignidad, su forma de presentacion clinica y radiologica, sus caracteristicas anatomo-patologicas, histoquimicas y hormonales y su vision bajo el microscopio electronico. Se destaca la ocasional dificultad para efectuar el diagnostico diferencial con el carcinoma indiferenciado de celula pequena y los problemas terapeutico-pronostico que comporta. Se hace enfasis en el tratamiento quirurgico de estos tumores con una reseccion reglada lo mas economica posible como tratamiento de eleccion, la larga supervivencia de los mismos despues de la intervencion y el acompanamiento ocasional de diversos sindromes hormonales, entre los que destaca el sindrome carcinoide
