Every year, the U.S. economy loses more than $411 billion because of work performance reduction, injuries, and traffic accidents caused by microsleep. To mitigate microsleeps consequences, an unobtrusive, reliable, and socially acceptable microsleep detection solution throughout the day, every day is required. Unfortunately, existing solutions do not meet these requirements. In this paper, we propose WAKE, a novel behind-the-ear wearable device for microsleep detection. By monitoring biosignals from the brain, eye movements, facial muscle contractions, and sweat gland activities from behind the user's ears, WAKE can detect microsleep with a high temporal resolution. We introduce a Three-fold Cascaded Amplifying (3CA) technique to tame the motion artifacts and environmental noises for capturing high fidelity signals. Through our prototyping, we show that WAKE can suppress motion and environmental noise in real-time by 9.74-19.47 dB while walking, driving, or staying in different environments ensuring that the biosignals are captured reliably. We evaluated WAKE using gold-standard devices on 19 sleep-deprived and narcoleptic subjects. The Leave-One-Subject-Out Cross-Validation results show the feasibility of WAKE in microsleep detection on an unseen subject with average precision and recall of 76% and 85%, respectively.

Given the prevalence of depression worldwide and its major impact on society, several studies employed artificial intelligence modelling to automatically detect and assess depression. However, interpretation of these models and cues are rarely discussed in detail in the AI community, but have received increased attention lately. In this study, we aim to analyse the commonly selected features using a proposed framework of several feature selection methods and their effect on the classification results, which will provide an interpretation of the depression detection model. The developed framework aggregates and selects the most promising features for modelling depression detection from 38 feature selection algorithms of different categories. Using three real-world depression datasets, 902 behavioural cues were extracted from speech behaviour, speech prosody, eye movement and head pose. To verify the generalisability of the proposed framework, we applied the entire process to depression datasets individually and when combined. The results from the proposed framework showed that speech behaviour features (e.g. pauses) are the most distinctive features of the depression detection model. From the speech prosody modality, the strongest feature groups were F0, HNR, formants, and MFCC, while for the eye activity modality they were left-right eye movement and gaze direction, and for the head modality it was yaw head movement. Modelling depression detection using the selected features (even though there are only 9 features) outperformed using all features in all the individual and combined datasets. Our feature selection framework did not only provide an interpretation of the model, but was also able to produce a higher accuracy of depression detection with a small number of features in varied datasets. This could help to reduce the processing time needed to extract features and creating the model.

BACKGROUND AND OBJECTIVES Bilateral subthalamic nucleus deep brain stimulation (STN DBS) in Parkinson's disease (PD) can have detrimental effects on eye movement inhibitory control. To investigate this detrimental effect of bilateral STN DBS, we examined the effects of manipulating STN DBS amplitude on inhibitory control during the antisaccade task. The prosaccade error rate during the antisaccade task, that is, directional errors, was indicative of impaired inhibitory control. We hypothesized that as stimulation amplitude increased, the prosaccade error rate would increase. MATERIALS AND METHODS Ten participants with bilateral STN DBS completed the antisaccade task on six different stimulation amplitudes (including zero amplitude) after a 12-hour overnight withdrawal from antiparkinsonian medication. Electroencephalogram (EEG) has been widely used for the detection of anxiety because of its ability to reflect the functional activities of the brain. However, EEG alone may not provide precision in the detection of anxiety because other emotional disorders usually trigger the same changes in brain function. To discover effective diagnostic indicators and to achieve more precise anxiety detection, we integrate eye movement information into EEG and divide the features into groups according to their respective characteristics. We use group sparse canonical correlation analysis (GSCCA) to investigate group structure information among EEG and eye movement features and obtain an effective fusion representation of EEG and eye movement to achieve more precise detection of anxiety mood. Experimental results from 45 anxious subjects and 47 normal controls showed that GSCCA could be effectively used to explore the correlation between EEG features within different scalp regions and eye movement features from several aspects. Visual behaviors, including saccades and fixation, are more linearly related to the power spectrum of EEG on the scalp area corresponding to the visual region of the brain. The ultimate fusion representation achieved an optimal classification accuracy of 82.70% with SVM on the gamma band of EEG.

BACKGROUND: Sleep staging is an important part of sleep research. Traditional automatic sleep staging based on machine learning requires extensive feature extraction and selection. OBJECTIVE: This paper proposed a deep learning algorithm without feature extraction based on one-dimensional convolutional neural network and long short-term memory. METHODS: The algorithm can automatically divide sleep into 5 phases including awake period, non-rapid eye movement sleep period (N1 [Formula: see text] N3) and rapid eye movement using the electroencephalogram signals. The raw signal was processed by the wavelet transform. Then, the processed signal was directly input into the deep learning algorithm to obtain the staging result. RESULTS: The accuracy of staging is 93.47% using the Fpz-Cz electroencephalogram signal. When using the Fpz-Cz and electroencephalogram signal, the algorithm can obtain the highest accuracy of 94.15%. CONCLUSION: These results show that this algorithm is suitable for different physiological signals and can realize end-to-end automatic sleep staging without any manual feature extraction.

Cerebrospinal fluid (CSF) plays an important role in the clearance of metabolic waste products from the brain, yet the driving forces of CSF movement are not fully understood. It is commonly believed that CSF movement is facilitated by blood vessel wall movements (i.e., hemodynamic oscillations) in the brain. A coherent pattern of low frequency hemodynamic oscillations and CSF movement was recently found during non-rapid eye movement (NREM) sleep via functional MRI. However, questions remain regarding 1) the explanation of coupling between hemodynamic oscillations and CSF movement from fMRI signals; 2) the existence of the coupling during wakefulness; 3) the direction of CSF movement. In this resting state fMRI study, we proposed a mechanical model to explain the coupling between hemodynamics and CSF movement through the lens of fMRI. We found that the observed delays between hemodynamics and CSF movement match those predicted by the model. Moreover, by conducting separate fMRI scans of the brain and neck, we confirmed the low frequency CSF movement at the fourth ventricle is bidirectional. Our finding also demonstrates that CSF movement is facilitated by hemodynamic oscillations mainly in the low frequency range, even when the individual is awake.

Contemporary human-machine-interfaces (HMIs) employ a wide range of human expressions to provide assistive support to the elderly and disabled population. Based on the disability type, expressions conveyed in terms of eye movements are often found to provide the most efficient way of communication. Nowadays, standard Electroencephalogram (EEG) based arrangements, used to analyze neurological states are also being adopted for the detection of eye movements. Although, a majority of the EEG-based state-of-the-arts researches either detects eye-movements in a lesser direction or uses a higher feature dimension with limited classification accuracy. In this study, a robust, simple and automated algorithm is proposed that uses the analysis of the EEG signal to classify six different types of eye movement. The algorithm uses discrete wavelet transform (DWT) on the EEG signals acquired from six different leads to eliminate a wide range of noise and artefacts. Then, two features per lead are extracted from the reconstructed wavelet coefficients and combined to form a binary feature map. Finally, a unique feature obtained from the calculated weighted sum of the binary map is used to classify six types of eye movements via a threshold-based technique. The algorithm presents high average accuracy (Acc), sensitivity (Se), specificity (Sp) of 95.85%, 95.83% and 95.83% respectively, using a single feature value only. Compared to other state-of-the-art methods, the adopted simple methodologies and the obtained results indicate the immense potential of the proposed algorithm to be implemented in personal assistive applications.

Sleep stage scoring is important to determine sleep structure in preclinical and clinical research. The aim of this study was to develop an automatic sleep stage classification system for mice with a new deep neural network algorithm. For the purpose of base feature extraction, wake-sleep and rapid eye movement (REM) and non- rapid eye movement (NREM) models were developed by extracting defining features from mouse-derived electromyogram (EMG) and electroencephalogram (EEG) signals, respectively. The wake-sleep model and REM-NREM sleep model were integrated into three different algorithms including a rule-based integration approach, an ensemble stacking approach, and a multimodal with fine-tuning approach. The deep learning algorithm assessing sleep stages in animal experiments by the multimodal with fine-tuning approach showed high potential for increasing accuracy in sleep stage scoring in mice and promoting sleep research.

Parkinson's disease (PD) is a neurodegenerative disease that includes motor impairments, such as tremor, bradykinesia, and postural instability. Although eye movement deficits are commonly found in saccade and pursuit tasks, preservation of oculomotor function has also been reported. Here we investigate specific task and stimulus conditions under which oculomotor function in PD is preserved. Sixteen PD patients and 18 healthy, age-matched controls completed a battery of movement tasks that included stationary or moving targets eliciting reactive or deliberate eye movements: pro-saccades, anti-saccades, visually guided pursuit, and rapid go/no-go manual interception. Compared with controls, patients demonstrated systematic impairments in tasks with stationary targets: pro-saccades were hypometric and anti-saccades were incorrectly initiated toward the cued target in ∼35% of trials compared with 14% errors in controls. In patients, task errors were linked to short latency saccades, indicating abnormalities in inhibitory control. However, patients' eye movements in response to dynamic targets were relatively preserved. PD patients were able to track and predict a disappearing moving target and make quick go/no-go decisions as accurately as controls. Patients' interceptive hand movements were slower on average but initiated earlier, indicating adaptive processes to compensate for motor slowing. We conclude that PD patients demonstrate stimulus and task dependency of oculomotor impairments, and we propose that preservation of eye and hand movement function in PD is linked to a separate functional pathway through the superior colliculus-brainstem loop that bypasses the fronto-basal ganglia network. Our results demonstrate that studying oculomotor and hand movement function in PD can support disease diagnosis and further our understanding of disease progression and dynamics.SIGNIFICANCE STATEMENTEye movements are a promising clinical tool to aid in the diagnosis of movement disorders and to monitor disease progression. Although Parkinson's disease (PD) patients show some oculomotor abnormalities, it is not clear whether previously described eye movement impairments are task-specific. We assessed eye movements in PD under different visual (stationary vs moving targets) and movement (reactive vs deliberate) conditions. We demonstrate that PD patients are able to accurately track moving objects but make inaccurate eye movements toward stationary targets. The preservation of eye movements toward dynamic stimuli might enable patients to accurately act on the predicted motion path of the moving target. These results can inform the development of tools for the rehabilitation or maintenance of functional performance.

Physically salient objects are thought to attract attention in natural scenes. However, research has shown that meaning maps, which capture the spatial distribution of semantically informative scene features, trump physical saliency in predicting the pattern of eye moments in natural scene viewing. Meaning maps even predict the fastest eye movements, suggesting that the brain extracts the spatial distribution of potentially meaningful scene regions very rapidly. To test this hypothesis, we applied representational similarity analysis to ERP data. The ERPs were obtained from human participants (N= 32, male and female) who viewed a series of 50 different natural scenes while performing a modified 1-back task. For each scene, we obtained a physical saliency map from a computational model and a meaning map from crowd-sourced ratings. We then used representational similarity analysis to assess the extent to which the representational geometry of physical saliency maps and meaning maps can predict the representational geometry of the neural response (the ERP scalp distribution) at each moment in time following scene onset. We found that a link between physical saliency and the ERPs emerged first (∼78 ms after stimulus onset), with a link to semantic informativeness emerging soon afterward (∼87 ms after stimulus onset). These findings are in line with previous evidence indicating that saliency is computed rapidly, while also indicating that information related to the spatial distribution of semantically informative scene elements is computed shortly thereafter, early enough to potentially exert an influence on eye movements.SIGNIFICANCE STATEMENTAttention may be attracted by physically salient objects, such as flashing lights, but humans must also be able to direct their attention to meaningful parts of scenes. Understanding how we direct attention to meaningful scene regions will be important for developing treatments for disorders of attention and for designing roadways, cockpits, and computer user interfaces. Information about saliency appears to be extracted rapidly by the brain, but little is known about the mechanisms that determine the locations of meaningful information. To address this gap, we showed people photographs of real-world scenes and measured brain activity. We found that information related to the locations of meaningful scene elements was extracted rapidly, shortly after the emergence of saliency-related information.

Not all websites have fulfilled basic principles such as accessible and easy to use (usable). There are several measuring methods that already exist. Background Preliminary evidence indicates that non–rapid eye movement (NREM) sleep is implicated in enhancing working memory (WM) performance across days in healthy individuals. While REM sleep has been implicated in other forms of memory, its role in WM remains unclear. Further, the relationship between sleep changes and WM improvement is largely unknown in posttraumatic stress disorder (PTSD). Examining the relationship between changes in sleep and WM improvement in healthy participants and participants with PTSD may inform cognitive enhancement strategies and intervention targets. Methods Repeated assessments of WM and overnight measurement of NREM and REM sleep parameters were performed in 79 participants (participants with PTSD: n = 33) during a 48-hour laboratory stay. Relationships between sleep parameter changes, WM performance changes, and clinical characteristics were analyzed in PTSD and healthy groups. Results A between-night enhancement in both NREM and REM sleep parameters in frontoparietal areas predicted across-day better WM performance in healthy participants, particularly in those with improved performance. In contrast, in participants with PTSD, an enhancement of these sleep parameters predicted a worse WM performance and was also associated with more PTSD-related sleep disturbances. Conclusions This study shows that higher sleep activity in frontoparietal areas leads to enhanced WM performance in healthy individuals, whereas in individuals with PTSD, it likely reflects the presence of sleep disturbances that interfere with WM improvement. Interventions focused on addressing sleep disturbances could therefore ameliorate cognitive impairments in individuals with PTSD.

BACKGROUND: In clinical practice, eye movements can provide an early diagnostic marker for early onset ataxia (EOA). However, quantitative oculomotor assessment is not included in the most frequently used and age-validated ataxia rating scale in children, the Scale for the Assessment and Rating of Ataxia (SARA). We aimed to investigate the applicability of semi-quantitative eye movement assessment by the International Cooperative Ataxia Rating Scale (ICARSOCM) and Ocular Motion Score (OMS7-10) complementary to SARA measurements in children.METHODS: In 52 typically developing children (aged 4-16 years; n = 4 per year of age), three independent assessors scored saccadic eye movements and ocular pursuit according to the ICARSOCM and matching parameters from the OMS7-10. For ICARSOCM, we determined 1) construct validity for coordinated eye movements by correlation with OMS7-10, ICARSEYE-HAND-COORDINATION and SARA subscale scores, 2) agreement percentage and inter-rater agreement (Fleiss Kappa) and 3) age-dependency.RESULTS: Spearman's rank correlations of ICARSOCM with OMS7-10 and ICARS- and SARA subscales were moderate to fair (all p < .001). Inter-rater agreement of ICARS-OCM was 80.8%; (Fleiss Kappa: 0.411). ICARSOCM scores revealed a similar exponentially decreasing association with age as the other SARA (sub)scores, reaching a plateau at 10 years of age.INTERPRETATION: ICARSOCM has a valid construct for the measurement of coordinated eye movement performance and is reliably assessable in children. ICARSOCM reveals a similar age-dependent relationship as the other ataxia subscales, reflecting the physiological maturation of the cerebellum. In children, these data may implicate that ICARSOCM can reliably contribute to coordination assessment, complementary to the SARA subscales.

STUDY OBJECTIVES: Research evaluating the influence of rapid eye movement suppressing antidepressants (REMS-AD) on multiple sleep latency test (MSLT) results and the value of performing actigraphy prior to this test in children and adolescents is lacking. We examined the impact of REMS-AD and actigraphy parameters on mean sleep latency (MSL) and sleep-onset REM episodes (SOREMs) on MSLT in a pediatric clinical sample. METHODS: This was a retrospective chart review at a quarternary referral center. We identified 164 MSLTs conducted in patients aged less than 18 years between 2014 and 2017. Correlations between REMS-AD, self-reported sleep duration, actigraphy parameters, and each of the outcomes (MSL and SOREMs) were examined. Regression analyses accounting for clinical characteristics were performed. RESULTS: Mean age of the sample was 11.9 ± 4.19 years, 62% were female, 28 (17%) were on REMS-AD (48% of whom were able to discontinue these medications prior to MSLT), and mean pediatric daytime sleepiness score was 21.7 ± 6.1. MSL was 11.27 ± 5.77 min and mean number of SOREMs 0.55 ± 1.04. Patients on a REMS-AD at initial assessment had fewer SOREMs compared to those not taking these medications (0.17 ± 0.19 vs 0.62 ± 0.09; P = .04); no difference was noted in MSL (10.36 ± 1.10 vs 11.47 ± 0.50; P = .36). Increased time in bed on actigraphy correlated with a longer MSL and fewer SOREMs (r = .23; P = .04 and r = .316; P = .004, respectively). Following regression analyses, use of REMS-AD continued to remain associated with fewer SOREMs; greater time in bed on actigraphy, but not self-reported sleep duration, was associated with a longer MSL (all P < .05). CONCLUSIONS: Clinicians should account for the use of REMS-AD and utilize actigraphy to determine time in bed while interpreting the results of a pediatric MSLT. Saccadic eye movements (saccades) disrupt the continuous flow of visual information, yet our perception of the visual world remains uninterrupted. Here we assess the representation of the visual scene across saccades from single-trial spike trains of extrastriate visual areas, using a combined electrophysiology and statistical modeling approach. Using a model-based decoder we generate a high temporal resolution readout of visual information, and identify the specific changes in neurons’ spatiotemporal sensitivity that underly an integrated perisaccadic representation of visual space. Our results show that by maintaining a memory of the visual scene, extrastriate neurons produce an uninterrupted representation of the visual world. Extrastriate neurons exhibit a late response enhancement close to the time of saccade onset, which preserves the latest pre-saccadic information until the post-saccadic flow of retinal information resumes. These results show how our brain exploits available information to maintain a representation of the scene while visual inputs are disrupted., A late enhancement of the perisaccadic neural response may exist in extrastriate areas. Here the authors show this preserves pre-saccadic information until the post-saccadic information is received, maintaining an integrated representation of the visual scene across saccadic eye movements.

Background Spontaneity is a unique feature of the nervous system. One of the fundamentally critical and recognized forms of spontaneous motor activity is witnessed in the visuomotor system. Microsaccades, the miniature spontaneous eye movements, are critical for the visual perception. We hypothesized that microsaccades follow specific temporal patterns that are modulated by the basal ganglia output. Methods We used high-resolution video-oculography to capture microsaccades in 48 subjects (31 healthy and 17 with Parkinson's disease) when subjects were asked to hold their gaze on a straight-ahead target projected on white background. We analyzed spontaneous discharge patterns of microsaccades. Results The first analysis considering coefficient of variation in intersaccadic interval distribution demonstrated that microsaccades in Parkinson's disease are more dispersed than the control group. The second analysis scrutinized microsaccades' temporal variability and revealed 3 distinct occurrence patterns: regular rhythmic, clustered, and randomly occurring following a Poisson-like process. The regular pattern was relatively more common in Parkinson's disease. Subthalamic DBS modulated this temporal pattern. The amount of change in the temporal variability depended on the DBS-induced volume of tissue activation and its overlap with the subthalamic nucleus. The third analysis determined the autocorrelations of microsaccades within 2-second time windows. We found that Parkinson's disease altered local temporal organization in microsaccade generation, and DBS had a modulatory effect. Conclusion The microsaccades occur in 3 temporal patterns. The basal ganglia are one of the modulators of the microsaccade spontaneity.

Sleep is a crucial component of life, characterized by global behavioral and neurochemical inhibition of brain activity. The gold-standard measurement of sleep is polysomnography (PSG). It assesses several organic functions during sleep, embracing brain activity, eyes movements, muscular pattern, and heart rhythm. Despite motor function being reduced during all sleep stages, particular muscular groups remain active, including those involved in respiratory or eye movement control. Polysomnography depends on this residual muscle activity to achieve reliable sleep recordings. Any interference with mechanisms dictating this behavior might impact electrogenic patterns in PSG, therefore, affecting clinical conclusions. There is a lack of knowledge about the association between myofunctional patterns during sleep and the widely prescribed cosmetic procedure of botulinum neurotoxin (BoNT). Considering aesthetical purposes, BoNT injection is often used to minimize facial wrinkles, through a reduction on the tonus of target muscles via cholinergic pathways. This review aimed to raise two important and clinically pertinent questions: (1) would the muscular hypotonicity caused by treatment with BoNT affect PSG patterns?; (2) could this change the way sleep looks like after BoNT-based cosmetic treatment challenging the effective use of standard PSG scoring criteria? A critical review of the literature currently available in the PubMed electronic database was undertaken. This review confirmed the lack of data regarding the possible consequences of the cosmetic use of BoNT on PSG reading and interpretation. We propose putative mechanisms and pathways by which this treatment could have these effects with clinically relevant impact on diagnosis of sleep disturbances. Even though there is little evidence on the relationship between cosmetic use of BoNT and substantial changes on sleep related motor control, it is reasonable to hypothesize that this may impact PSG patterns and its interpretation. As the use of this drug is rising, as well as there is an increase on the prevalence of sleep disturbances needing PSG diagnosis, this should be a relevant subject warranting future research.

Video quality assessment (VQA) algorithms play an significant role in many fields of video analysis. To improve the accuracy of video quality evaluation, many scholars start by imitating viewers’ subjective feelings to make the algorithm more suitable for the subjective feelings of the video viewers. On the other hand, many scholars are also working to reduce some unnecessary time costs caused by frame-by-frame analysis. To achieve this goal, it also caused a lot of unnecessary time costs. To solve these problems, this paper proposes an improved algorithm based on danmaku analysis. First of all, through the analysis of eye movement data, we find that the concentrated expression of the subjective emotion of the video viewers contains a strong direct relationship with the quality of the video, and the danmaku is an element of such subjective emotion. Then we improve the existing video quality assessment algorithm, analyze the significant role of danmaku in subjective emotion, extract specific keyframes, and get the corresponding objective score as the result. The experimental results show that the algorithm improves the efficiency of time optimization, and from the Pearson correlation coefficient (PCC), the results are more fit for viewers’ subjective feelings.

Background Melanopsin retinal ganglion cell (mRGC)-mediated pupillary light reflex (PLR) abnormalities have been documented in several neurodegenerative disorders including Parkinson's disease. Overall, isolated rapid eye movement (REM) sleep behavior disorder (iRBD) represents the strongest prodromal risk factor for impending α-synucleinopathies. Objectives To quantitatively compare PLR and mRGC-mediated contribution to PLR in 16 iRBD patients and 16 healthy controls. Methods iRBD and controls underwent extensive neuro-ophthalmological evaluation and chromatic pupillometry. In iRBD, PLR metrics were correlated with clinical variables and with additional biomarkers including REM atonia index (RAI), DaTscan, and presence of phosphorylated-α-synuclein (p-α-syn) deposition in skin biopsy. Results We documented higher baseline pupil diameter and decreased rod-transient PLR amplitude in iRBD patients compared to controls. PLR rod-contribution correlated with RAI. Moreover, only iRBD patients with evidence of p-α-syn deposition at skin biopsy showed reduced PLR amplitude compared to controls. Conclusion The observed PLR abnormalities in iRBD might be considered as potential biomarkers for the risk stratification of phenoconversion of the disease. © 2021 The Authors. Introduction Second-language learning (SLL) depends on distinct functional-neuroanatomical systems including procedural and declarative long-term memory. Characteristic features of rapid eye movement (REM) and non-REM sleep such as rapid eye movements and sleep spindles are electrophysiological markers of cognitively complex procedural and declarative memory consolidation, respectively. In adults, grammatical learning depends at first on declarative memory (“early SLL”) then shifts to procedural memory with experience (“late SLL”). However, it is unknown if the shift from declarative to procedural memory in early vs late SLL is supported by sleep. Here, we hypothesized that increases in sleep spindle characteristics would be associated with early SLL, whereas increases in REM activity (eg, density and EEG theta-band activity time-locked to rapid eye movements) would be associated with late SLL. Methods Eight Anglophone (English first language) participants completed four polysomnographic recordings throughout an intensive 6-week French immersion course. Sleep spindle data and electroencephalographic spectral power time-locked to rapid eye movements were extracted from parietal temporal electrodes. Results As predicted, improvements in French proficiency were associated with changes in spindles during early SLL. Furthermore, we observed increased event-related theta power time-locked to rapid eye movements during late SLL compared with early SLL. The increases in theta power were significantly correlated with improvements in French proficiency. Discussion This supports the notion that sleep spindles are involved in early SLL when grammar depends on declarative memory, whereas cortical theta activity time-locked to rapid eye movements is involved in late SLL when grammar depends on procedural memory.

Face recognition systems are widely used for target recognition and identity authentication, such as automated teller machines, mobile phones, and entrance guard systems. However, face recognition systems are vulnerable to presentation attacks, such as photo, replay, and 3D mask attacks. In particular, silicone mask attacks pose a greater threat to face recognition systems because high-quality silicone masks do living properties. To promote the development of face anti-spoofing detection algorithms for silicone mask attacks, this paper constructs a Silicone Mask Face Motion Video Dataset (SMFMVD) containing 200 real face videos and 200 silicone mask face videos. These videos include different facial motions collected from 20 subjects. Moreover, inspired by the observation that the silicone mask face’s facial movement is not so natural as the real face, we propose a novel silicone mask face anti-spoofing detection method based on visual saliency and facial motion characteristics. Specifically, we compute the visual saliency map of a given face image by simulating two kinds of eye movement behaviors, namely “gaze” and “saccade”. Then, we propose a saliency-weighted histogram of local binary pattern operator to extract facial texture features in spatial domain and a saliency-guided histogram of oriented optical flow operator to extract facial motion features in temporal domain. Finally, the support vector machine is used to fuse two groups of facial features to distinguish real and spoof faces. Extensive experiments on public and self-built datasets show its superiority over the state-of-the-art methods.

The evaluation index of camouflage patterns is important in the field of military application. It is the goal that researchers have always pursued to make the computable evaluation indicators more in line with the human visual mechanism. In order to make the evaluation method more computationally intelligent, a Multi-Feature Camouflage Fused Index (MF-CFI) is proposed based on the comparison of grayscale, color and texture features between the target and the background. In order to verify the effectiveness of the proposed index, eye movement experiments are conducted to compare the proposed index with existing indexes including Universal Image Quality Index (UIQI), Camouflage Similarity Index (CSI) and Structural Similarity (SSIM). Twenty-four different simulated targets are designed in a grassland background, 28 observers participate in the experiment and record the eye movement data during the observation process. The results show that the highest Pearson correlation coefficient is observed between MF-CFI and the eye movement data, both in the designed digital camouflage patterns and large-spot camouflage patterns. Since MF-CFI is more in line with the detection law of camouflage targets in human visual perception, the proposed index can be used for the comparison and parameter optimization of camouflage design algorithms.

Objective We clarified the clinical and mechanistic significance of physiological modulations of high-frequency broadband cortical activity associated with spontaneous saccadic eye movements during a resting state. Methods We studied 30 patients who underwent epilepsy surgery following extraoperative electrocorticography and electrooculography recordings. We determined whether high-gamma activity at 70–110 Hz preceding saccade onset would predict upcoming ocular behaviors. We assessed how accurately the model incorporating saccade-related high-gamma modulations would localize the primary visual cortex defined by electrical stimulation. Results The dynamic atlas demonstrated transient high-gamma suppression in the striatal cortex before saccade onset and high-gamma augmentation subsequently involving the widespread posterior brain regions. More intense striatal high-gamma suppression predicted the upcoming saccade directed to the ipsilateral side and lasting longer in duration. The bagged-tree-ensemble model demonstrated that intense saccade-related high-gamma modulations localized the visual cortex with an accuracy of 95%. Conclusions We successfully animated the neural dynamics supporting saccadic suppression, a principal mechanism minimizing the perception of blurred vision during rapid eye movements. The primary visual cortex per se may prepare actively in advance for massive image motion expected during upcoming prolonged saccades. Significance Measuring saccade-related electrocorticographic signals may help localize the visual cortex and avoid misperceiving physiological high-frequency activity as epileptogenic.

I list a summary of the major clinical observations of SVIN in patients with total unilateral vestibular loss (TUVL) and show how basic results from neurophysiology can explain these clinical observations. The account integrates results from single neuron recordings of identified semicircular canal and otolith afferent neurons in guinea pigs in response to low frequency skull vibration with evidence of the eye movement response in cats to selective semicircular canal stimulation (both individual and combined) and a simple model of nystagmus generation to show how these results explain most of the major characteristics of SVIN.

Objective Eye tracking technology has been employed in assessing ocular motor and vestibular function following vestibular and neurologic conditions, including traumatic brain injury (TBI). Assessments include tests that provide visual and motion (rotation) stimuli while recording horizontal, vertical, and torsional eye movements. While some of these tests have shown diagnostic promise in previous studies, their use in clinical practice is limited by the lack of normative data. The goal of this study was to construct normative reference ranges to be used when comparing patients' results. Methods Optokinetic response, subjective visual horizontal and vertical, and rotation tests were administered to male and female volunteers, ages 18‐45, who were free from neurological, vestibular disorders, or other head injuries. Tests were administered using either a rotatory chair or a portable virtual reality‐like goggle equipped with video‐oculography. Results Reference values for eye movements in response to different patterns of stimuli were analyzed from 290 to 449 participants. Analysis of gender (self‐reported) or age when grouped as pediatric (late adolescent; 18‐21 years of age) and adult (21‐45 years of age) revealed no effects on the test metrics. Data were pooled and presented for each test metric as the 95% reference interval (RI) with 90% confidence intervals (CI) on upper and lower limits of the RI. Conclusions This normative database can serve as a tool to aid in diagnosis, treatment, and/or rehabilitation protocols for vestibular and neurological conditions, including mild TBI (mTBI). Although neuroimaging studies of collision (COLL) sport athletes demonstrate alterations in brain structure and function from pre- to post-season, reliable tools to detect behavioral/cognitive change relevant to functional networks associated with participation in collision sports are lacking. This study evaluated the use of eye-movement testing to detect change in cognitive and sensorimotor processing among male club collegiate athletes after one season of participation in collision sports of variable exposure. We predicted that COLL (High Dose [hockey], n = 8; Low Dose [rugby], n = 9) would demonstrate longer reaction times (antisaccade and memory-guided saccade [MGS] latencies), increased inhibitory errors (antisaccade error rate), and poorer spatial working memory (MGS spatial accuracy) at post-season, relative to pre-season, whereas non-collision collegiate athletes (NON-COLL; n = 17) would remain stable. We also predicted that whereas eye-movement performance would detect pre- to post-season change, ImPACT (Immediate Post-Concussion Assessment and Cognitive Test) performance would remain stable. Our data showed that NON-COLL had shorter (improved performance) post- versus pre-season antisaccade and MGS latencies, whereas COLL groups showed stable, longer, or attenuated reduction in latency (ps ≤ 0.001). Multiple sleep-onset rapid eye movement periods (SOREMPs) are involved in the pathophysiology of narcolepsy, but it is not clear whether the lack of multiple SOREMPs is associated with the pathophysiology of idiopathic hypersomnia or not. We examined the significance of multiple SOREMPs in patients with pathological sleep prolongation. Participants were consecutive patients complaining of unexplained sleepiness and agreed to a 3-day-sleep studies; 24 h polysomnography (PSG) followed by standard PSG and multiple sleep latency test (MSLT). Forty-one (26 females, 21.9 ± 8.1 years old, BMI 20.4 ± 2.3 kg/m2) of 54 eligible patients without other sleep pathologies showed pathological sleep prolongation. We subdivided them into those with and without multiple SOREMPs on MSLT and compared clinical and PSG variables between groups. Six of 41 (14.6%) patients showed multiple SOREMPs on MSLT. There were almost no differences in sleep variables between those with and without multiple SOREMPs. We only found shorter mean sleep latency on MSLT and more REM cycles on 24 h PSG in those with multiple SOREMPs (adjusted p = 0.016 and 0.031). The frequencies of REM-related phenomena and clinical symptoms related to idiopathic hypersomnia were not different between groups. Our results indicated that patients with pathological sleep prolongation had the same clinical profiles regardless of the status of SOREMPs, suggesting the absence of multiple SOREMPs, prerequisite for the diagnosis of idiopathic hypersomnia, is not a specific feature of pathological sleep prolongation. Confirmation of sleep prolongation alone could be a diagnostic tool for idiopathic hypersomnia.

This study examined the visual characteristics of drivers with color weakness to improve their safety while driving. Significantly affected by the traffic environment, drivers with color weakness are not able to recognize traffic lights rapidly and accurately, which endangers traffic safety. In the first part of the research, through a static visual recognition test of color vision using the orthogonal method, the study explored the influence of light intensity, visual recognition distance, and color weakness type on the perception of traffic light colors by participants with color weakness. In the second part, a dynamic visual recognition test of color vision was conducted through simulating the driving environment of urban roads. Eye movement indexes between participants with color weakness and those with normal vision were analyzed by means of different vehicle speeds and time periods. The results indicated that the type of color weakness was the dominant factor affecting visual recognition of traffic lights. The eye movements of participants with deuteranomaly were close to those of people with normal vision, whereas the eye movement index of those with protanomaly and dyserythrochloropsia were significantly different. Distraction, slower responses, and higher color recognition error rates for the traffic lights were major characteristics—all representing risks that increase at night. To reduce the probability of road traffic injury, the driving safety of people with color weakness should be addressed.

Introduction: Covid-19 pandemic disease and its infections caused may be associated with bacterial and fungal co-infections. Case Presentation: A 61- year-old male patient, after Covid-19 disease admitted in hospital with the complaints of headache, right eye swelling, watering of right eyes, loss of vision since 2 days, also has history of loss of appetite, no history of febrile illness. Patient having history of diabetic mellitus but it was under control. As per physical examination left eye’s vision was reduced but eye movement was present. In right eye there was swelling of periorbital region, redness, watering of eyes was present and perception of light was negative due to damaged optic nerve. Loss of vision was found, eye movement was not present. Patient having mucormycosis fungal infection which was confirmed on culture test. All blood investigations were done. All reports were normal except white blood cells (WBC) were raised with 13400cu.mm. Random blood sugar (RBS) 250 mg%. Without delay medical treatment was started and patient was kept under strict observation.Spo2 were 97%, pulse rate 78 beats/ min. Conclusion: Here we would like to bring into notice that, Medical team members should know about the danger of secondary invasive fungal infection in a patient after Covid 19 disease. This type of cases can be prevented by using hygienic technique while handling of patient and proper standard treatment. It is necessary to prevent the spread of infection and its mortality and morbidity. Hence, medical practitioners should be aware of early clinical manifestation of the disease.

Objective: Recent research has suggested that a type of atypical eye movement, called square wave jerks, together with anxiety, may distinguish individuals with anorexia nervosa from those without anorexia nervosa and may represent a biomarker and endophenotype for the illness. The aim of this study was to identify the presence of this proposed marker in individuals currently with anorexia nervosa relative to healthy controls, and to identify the state independence and heritability of this putative marker by exploring whether it also exists in individuals who are weight-restored from anorexia nervosa and first-degree relatives (i.e. sisters of people with anorexia nervosa). Methods: Data from 80 female participants (20/group: current anorexia nervosa, weight-restored from anorexia nervosa, sisters of people with anorexia nervosa and healthy controls) were analysed. Square wave jerk rate was acquired during a fixation task, and anxiety was measured with the State Trait Anxiety Inventory. Results: Current anorexia nervosa, weight-restored from anorexia nervosa and sisters of people with anorexia nervosa groups made significantly more square wave jerks than healthy controls, but did not differ from one another. Square wave jerk rate and anxiety were found to discriminate groups with exceptionally high accuracy (current anorexia nervosa vs healthy control = 92.5%; weight-restored from anorexia nervosa vs healthy control = 77.5%; sisters of people with anorexia nervosa vs healthy control = 77.5%; p Conclusion: The combination of square wave jerk rate and anxiety was found to be a promising two-element marker for anorexia nervosa, and has the potential to be used as a biomarker or endophenotype to identify people at risk of anorexia nervosa and inform future treatments.

Abnormalities in cranial motor nerve development cause paralytic strabismus syndromes, collectively referred to as congenital cranial dysinnervation disorders, in which patients cannot fully move their eyes. These disorders can arise through one of two mechanisms: ( a) defective motor neuron specification, usually by loss of a transcription factor necessary for brainstem patterning, or ( b) axon growth and guidance abnormalities of the oculomotor, trochlear, and abducens nerves. This review focuses on our current understanding of axon guidance mechanisms in the cranial motor nerves and how disease-causing mutations disrupt axon targeting. Abnormalities of axon growth and guidance are often limited to a single nerve or subdivision, even when the causative gene is ubiquitously expressed. Additionally, when one nerve is absent, its normal target muscles attract other motor neurons. Study of these disorders highlights the complexities of axon guidance and how each population of neurons uses a unique but overlapping set of axon guidance pathways.

In healthy vision, the fovea provides high acuity and serves as the locus for fixation achieved through saccadic eye movements. Bilateral loss of the foveal regions in both eyes causes individuals to adopt an eccentric locus for fixation. This review deals with the eye movement consequences of the loss of the foveal oculomotor reference and the ability of individuals to use an eccentric fixation locus as the new oculomotor reference. Eye movements are an integral part of everyday activities, such as reading, searching for an item of interest, eye–hand coordination, navigation, or tracking an approaching car. We consider how these tasks are impacted by the need to use an eccentric locus for fixation and as a reference for eye movements, specifically saccadic and smooth pursuit eye movements.

PURPOSE: Dynamic text presentation methods may improve reading ability in patients with central vision loss (CVL) by eliminating the need for accurate eye movements. We compared rapid serial visual presentation (RSVP) and horizontal scrolling text presentation (scrolling) on reading rate and reading acuity in CVL observers and normally-sighted controls with simulated CVL (simCVL). METHODS: CVL observers’ (n = 11) central scotomas and preferred retinal loci (PRL) for each eye were determined with MAIA microperimetry and fixation analysis. SimCVL controls (n = 16) used 4° inferior eccentric viewing, enforced with an Eyelink eye-tracker. Observers read aloud 4-word phrases randomly drawn from the MNREAD sentences. Six font sizes (0.50–1.30 logMAR) were tested with the better near acuity eye and both eyes of CVL observers. Three font sizes (0.50–1.00 logMAR) were tested binocularly in simCVL controls. Text presentation duration of each word for RSVP or drift speed for scrolling was varied to determine reading rate, defined as 50% of words read correctly. In a subset of CVL observers (n = 7), relationships between PRL eccentricity, reading threshold and rate were explored. RESULTS: SimCVL controls demonstrated significantly faster reading rates for RSVP than scrolling text (p < 0.0001), and there was a significant main effect of font size (p < 0.0001). CVL patients demonstrated no significant differences in binocular reading rate between font sizes (p = 0.12) and text presentation (p = 0.25). Similar results were seen under monocular conditions. Reading acuity for RSVP and scrolling worsened with increasing PRL eccentricity (μ = 4.5°, p = 0.07). RSVP reading rate decreased significantly with increasing eccentricity (p = 0.02). CONCLUSIONS: Consistent with previous work, reading acuity worsened with increasing PRL eccentricity. RSVP and scrolling text presentations significantly affected reading rate in simCVL, but not in CVL observers, suggesting that simCVL results may not generalise to pathological CVL.

Objective: This study aims to investigate the elements of wayfinding in indoor complex healthcare environments. Background: The study replicates Ghamari and Pati’s 2018 study to identify the environmental attributes that attract eye fixation during wayfinding by objectively tracking eye movements and fixation as healthy subjects navigate through a complex, unfamiliar indoor healthcare setting. The study addressed what do people look at while navigating in unfamiliar healthcare environments? What are the relative time periods of eye fixations on different visual environmental elements of the healthcare-designed environments? And what role do visual environmental attributes in healthcare facilities, such as configuration, color, art, directories, maps, furniture, and so on, play during the wayfinding process. Method: Twenty-four adults in different genders and various age groups participated in this study and navigated five routes with different degrees of difficulty. The sequence of the destinations in this study was randomized. The data were collected by tracking gaze fixations while human subjects navigated an indoor complex healthcare environment. Results: The findings show that identifying signs (29.1%), informative signs (20.8%), and architectural features (11.3%) constituted the most frequent elements attracting gaze, substantially more than the other classes of information. Four types of signage (identifying signs, informative signs, directional, and safety/regulatory signs) were accounted for 62.3% of the total gaze fixation time. Conclusions: The comparison of the ordered list based on frequencies and time of eye fixations on various elements developed in Ghamari and Pati’s study shows a vast degree of similarities.

Biomedical technology has now been widely adopted as a means of monitoring the human body in real-time. For example, to detect eye movement. In the medical world, eye movement can be used to determine the type of disease. With the application of human-machine interface (HMI) technology, eyeball movement can be developed in the robotics industry as robot navigation. For example, by moving the eyeball left and right, the robot can interpret the eye signal to move left and right. The interaction between the eyeball movement and the robot is of particular concern in this study. This study aimed to design a measuring instrument for eye movement detection using Electrooculography (EOG) techniques to move a wheeled robot. The EOG measuring instrument consisting of an instrument differential amplifier, a low pass filter, and a high pass filter has been applied in this research. The signal generator technique on EOG is carried out by placing electrodes on three sides of the face, namely forehead (G), left horizontal (H-), right horizontal (H +). The experimental results showed a significant difference between the left and right eye movement amplitude signals. This amplitude is used to classify the movement of the robot wheel towards the left and right. The process of sending robot signals and EOG measuring instruments uses Bluetooth HC-05 serial communication. Based on the research results, it is proven that the robot manages to move left and right according to the eyeball movement.

Western populations are ageing. With age comes an increased risk of mild cognitive impairment (MCI) and fragility that leads to higher fatal car crashes. This study develops a driving simulation paradigm that seeks to detect unsafe drivers, particularly among older drivers with MCI. The paradigm includes repeated urban intersection crossings at three difficulty levels while eye movements are tracked. The internal validity of this part of the paradigm was tested with young adults ( N = 7). Results indicated that the simulator tests elicited unsafe driving behaviors that varied across difficulty and avoided ceiling and floor effects. Eye movement metrics associated with cognitive load also varied with difficulty and predicted safe crosses. The strongest predictors were gaze transition entropy, gaze variability, and pupil size entropy. These findings indicate internal validity of the tests. Future research should test the external validity of this paradigm with a larger, more diverse sample.

A 76-year-old incarcerated man presented with word-finding difficulties and underwent a routine EEG. Shortly after reaching stage N2 sleep, EEG showed brisk lateral eye movements with concurrent sawtooth waves (figure), indicating sleep-onset REM period (SOREMP). Normal REM sleep is reached 90–120 minutes after sleep onset. SOREMP is an abnormal EEG finding thought to be suggestive of narcolepsy. However, it may also be present in 3.9% of the general population1 and is normal in infants. It is also associated with severe sleep deprivation, obstructive sleep apnea, and alcohol or medication (selective serotonin reuptake inhibitors, serotonin and norepinephrine reuptake inhibitors, tricyclic antidepressants, monoamine oxidase inhibitors) withdrawal. In the inpatient setting, it is usually caused by sleep deprivation.2

Background and ObjectivesLesions of the cerebellar flocculus cause enduring downbeat nystagmus (DBN) with unrelenting oscillopsia. Unlike most patients with DBN, the flocculus is structurally spared in nonalcoholic Wernicke encephalopathy (nWE) with chronic DBN. The objective was to study the effects of alcohol in nWE.MethodsWe recorded eye movements of a unique patient with nWE under controlled alcohol consumption who said his oscillopsia disappeared with a few drinks of alcohol.ResultsHis DBN was markedly diminished by alcohol (by 77.4%), although he remained alert with normal saccades.DiscussionThis striking observation may be caused by the differential effect of alcohol on the perihypoglossal complex and the paramedian tract neurons, which control the level of activity in the flocculus, with opposite (inhibition and excitation, respectively) effects. The finding suggests new ideas about the treatment and pathophysiology of DBN with a structurally intact cerebellum.

Experts have a remarkable capability of locating, perceptually organizing, identifying, and categorizing objects in images specific to their domains of expertise. In this article, we present a hierarchical probabilistic framework to discover the stereotypical and idiosyncratic viewing behaviors exhibited with expertise-specific groups. Through these patterned eye movement behaviors we are able to elicit the domain-specific knowledge and perceptual skills from the subjects whose eye movements are recorded during diagnostic reasoning processes on medical images. Analyzing experts' eye movement patterns provides us insight into cognitive strategies exploited to solve complex perceptual reasoning tasks. An experiment was conducted to collect both eye movement and verbal narrative data from three groups of subjects with different levels or no medical training (eleven board-certified dermatologists, four dermatologists in training and thirteen undergraduates) while they were examining and describing 50 photographic dermatological images. We use a hidden Markov model to describe each subject's eye movement sequence combined with hierarchical stochastic processes to capture and differentiate the discovered eye movement patterns shared by multiple subjects within and among the three groups. Independent experts' annotations of diagnostic conceptual units of thought in the transcribed verbal narratives are time-aligned with discovered eye movement patterns to help interpret the patterns' meanings. By mapping eye movement patterns to thought units, we uncover the relationships between visual and linguistic elements of their reasoning and perceptual processes, and show the manner in which these subjects varied their behaviors while parsing the images. We also show that inferred eye movement patterns characterize groups of similar temporal and spatial properties, and specify a subset of distinctive eye movement patterns which are commonly exhibited across multiple images. Based on the combinations of the occurrences of these eye movement patterns, we are able to categorize the images from the perspective of experts' viewing strategies in a novel way. In each category, images share similar lesion distributions and configurations. Our results show that modeling with multi-modal data, representative of physicians' diagnostic viewing behaviors and thought processes, is feasible and informative to gain insights into physicians' cognitive strategies, as well as medical image understanding.

A concussion occurs when a direct or indirect force is transmitted to the brain, causing a change in brain function. Given that approximately half the brain circuits are involved in vision and the control of eye movements, a concussion frequently results in visual symptoms. Ophthalmic abnormalities are helpful in the assessment of acute concussion, identified by rapid automized naming tasks and eye movement assessments. In particular, convergence, eye-tracking and the vestibular-ocular motor screening tool may be used. For patients suffering from post-concussion syndrome more than three months from the original injury, abnormalities may be found in convergence, accommodation and smooth pursuit. Orthoptic exercises are useful rehabilitation tools to allow patients to return to school, work and recreation. This article provides a brief overview of concussion as it relates to vision and ophthalmic practice.

Purpose of review Rapid eye movement (REM) obstructive sleep apnoea (OSA) is a stage-specific OSA, in which obstructive events occur primarily during REM sleep. This review discusses REM-OSA definitions, its cardiometabolic correlates, associated comorbidities and treatment, and addresses diagnostic ambiguities and therapeutic pitfalls. Recent findings Current evidence indicates that REM-OSA is prevalent among younger age groups and women and is independently associated with cardiometabolic complications, particularly hypertension, metabolic complications such as insulin resistance and metabolic syndrome. However, currently, there is no consensus on the accepted diagnostic criteria for REM-OSA. Available data suggest that adherence to positive airway pressure (PAP) therapy in patients with REM-OSA is suboptimal. Moreover, the currently accepted criteria for good adherence to PAP therapy of 4 h/night, 70% of the days may not be suitable for REM-OSA, as it will not cover most of the REM sleep periods. In addition, further research is needed to assess the impact of REM-OSA treatment on cardiometabolic outcomes. Summary Patients with REM-OSA are at an increased risk of cardiometabolic complications. A high index of suspicion is needed to diagnose this disorder, and close follow-up is required to enhance adherence to therapy.

The hippocampus is essential for spatial learning and memory. To assess learning we used contextual fear conditioning (cFC), where animals learn to associate a place with aversive events like foot-shocks. Candidate memory mechanisms for cFC are long-term potentiation and long-term depression, but there is little direct evidence of them operating in the hippocampus in vivo following cFC. Also, little is known about the behavioral state changes induced by cFC. To address these issues, we recorded local field potentials in freely behaving mice by stimulating in the left dorsal CA1 region and recording in the right dorsal CA1 region. Synaptic strength in the commissural pathway was monitored by measuring field excitatory postsynaptic potentials (fEPSPs) before and after cFC. After cFC, the commissural pathway’s synaptic strength was potentiated. Although recordings occurred during the wake phase of the light/dark cycle, the mice slept more in the post-conditioning period than in the pre-conditioning period. Relative to awake periods, in non-rapid eye movement (NREM) sleep the fEPSPs were larger in both pre- and post-conditioning periods. We also found a significant negative correlation between the animal’s speed and fEPSP size. Therefore, to avoid confounds in the fEFSP potentiation estimates, we controlled for speed-related and sleep-related fEPSP changes and still found that cFC induced long-term potentiation, but no significant long-term depression. Synaptic strength changes were not found in the control group that simply explored the fear-conditioning chamber, indicating that exploration of the novel place did not produce the measurable effects caused by cFC. These results show that following cFC, the CA1 commissural pathway is potentiated, likely contributing to the functional integration of the left and right hippocampi in fear memory consolidation. In addition, the cFC paradigm produces significant changes in an animal’s behavioral state, which are observable as proximal changes in sleep patterns.

For automated driving at SAE level 3 or lower, driver performance in responding to takeover requests (TORs) is decisive in providing system safety. A driver state monitoring system that can predict a driver’s performance in a TOR event will facilitate a safer control transition from vehicle to driver. This experimental study investigated whether driver eye-movement measured before a TOR can predict driving performance in a subsequent TOR event. We recruited participants (N = 36) to obtain realistic results in a real-vehicle study. In the experiment, drivers rode in an automated vehicle on a test track for about 32 min, and a critical TOR event occurred at the end of the drive. Eye movements were measured by a camera-based driver monitoring system, and five measures were extracted from the last 2-min epoch prior to the TOR event. The correlations between each eye-movement measure and driver reaction time were examined, and a multiple regression model was built using a stepwise procedure. The results showed that longer reaction time could be significantly predicted by a smaller number of large saccades, a greater number of medium saccades, and lower saccadic velocity. The implications of these relationships are consistent with previous studies. The present real-vehicle study can provide insights to the automotive industry in the search for a safer and more flexible interface between the automated vehicle and the driver.

Modern radiotherapy techniques achieve highly conformal target doses while avoiding organs-at-risk such as the lens. The magnitude of eye movement and its influence on CSI target- and avoidance volumes are unclear. We aimed to evaluate the movement-range of lenses and optic nerves and its influence on dose distribution of several planning techniques. Methods Ten volunteers underwent MRI scans in various gaze directions (neutral, left, right, cranial, caudal). Lenses, orbital optic nerves, optic discs and CSI target volumes were delineated. 36-Gy cranial irradiation plans were constructed on synthetic CT images in neutral gaze, with Volumetric Modulated Arc Therapy, pencil-beam scanning proton therapy, and 3D-conventional photons. Movement-amplitudes of lenses and optic discs were analyzed, and influence of gaze direction on lens and orbital optic nerve dose distribution. Results Mean eye structures’ shift from neutral position was greatest in caudal gaze; −5.8±1.2 mm (±SD) for lenses and 7.0±2.0 mm for optic discs. In 3D-conventional plans, caudal gaze decreased Mean Lens Dose (MLD). In VMAT and proton plans, eye movements mainly increased MLD and diminished D98 orbital optic nerve (D98OON) coverage; mean MLD increased up to 5.5 Gy [total ΔMLD range −8.1 to 10.0 Gy], and mean D98OON decreased up to 3.3 Gy [total ΔD98OON range −13.6 to 1.2 Gy]. VMAT plans optimized for optic disc Internal Target Volume and lens Planning organ-at-Risk Volume resulted in higher MLD over gaze directions. D98OON became ≥95% of prescribed dose over 95/100 evaluated gaze directions, while all-gaze bilateral D98OON significantly changed in 1 of 10 volunteers. To investigate the trend of restoration of postoperative eye movements after orbital blowout fractures by analyzing the percentage of Hess area ratio (HAR%). This was a retrospective study of patients who underwent orbital blow out reconstruction surgery within 28-days post injury. Eye movements were measured preoperatively and 3, 6, and 12 months post operatively by Hess chart. Orbital fractures were classified into three shapes: trap-door fracture with muscle entrapment, trap-door fracture with incarcerated tissue, and depressed fracture. Correlations between fracture shapes, regions, reconstruction implant, age and HAR% were analyzed. This study involved 125 eyes (64 right eyes and 61 left eyes) of 125 isolated orbital fracture cases (95 males, 30 females, mean age: 27.2, range 4-85 years old). Of the total 125 cases, 96 patients had orbital floor fractures, 18 had orbital medial wall fractures, and 11 had combined orbital medial wall and floor fractures. Three had trap-door fractures with muscle entrapment, 42 had trap-door fractures with incarcerated tissue, and 80 had orbital depressed fractures. The overall mean HAR% improved significantly from 76.0 pre-operatively to 95.5 post-12 months (P < 0.01). The mean HAR% in orbital floor fracture improved significantly from 76.7 pre-operatively to 92.9 and 94.7 at 3, 6 months respectively. There was a non-significant improvement in the HAR% in medial wall fractures from 83.2 pre-operatively to 89.5 at 3 months and a significant improvement to 93.2 at 6 months (p < 0.05). Orbital fractures were reconstructed with either Unsintered hydroxyapatite particles/poly l-lactide composite sheet (u-HA/PLLA composite sheet) (91 patients), a silicone silastic sheet (20 patients) a combination of sheets (7 patients) or without an implant (7 patients). SIGNIFICANCE Visual performance is affected least by a 15° radial aperture surrounded by peripheral myopic defocus. This finding has important applications for spectacle and contact lens designs and myopia control optimization. PURPOSE The purpose of this study was to assess the effect of clear central apertures of different diameters with a defocused retinal periphery, using a range of visual performance tasks. METHODS Thirty visually normal subjects (mean age, 24.4 ± 3.3 years; 20 females; mean spherical equivalent of -1.28 D) were enrolled. Subjects wore five different spectacles during testing, all corrected for distance refraction, in random order: three single-vision spectacles with clear central apertures of 10, 12.5, and 15° radii with the periphery defocused using Fresnel "press-on" lenses (+3.5 D sphere), progressive addition lens (PAL) spectacles with a +3.5 D addition, and single-vision lens (SVL) spectacles with no peripheral defocus. Static and kinetic visual field sensitivities, reading rate and comprehension, head movements, global saccadic tracking, and saccadic visual search were evaluated. RESULTS Reading rate and comprehension did not differ across the five test conditions; however, increased head movement was found with the smallest aperture compared with the PAL condition with adjusted P < .05. Static visual field sensitivity was reduced for all three apertures in eccentric regions when compared with the SVL and PAL conditions with adjusted P < .05, whereas kinetic sensitivity did not differ for any lens condition. The 15° aperture was superior to the 10 and 12.5° apertures based on its similarity to the SVL and PAL spectacle conditions in head movement during reading, the Michigan Tracking Test, and the vertical results of the Developmental Eye Movement Test. CONCLUSIONS Visual performance is least affected adversely by a 15° aperture surrounded by a peripheral myopic defocus. This finding has important applications for spectacle and contact lens designs to optimize myopia treatment with minimal impact on visual performance.

Rapid eye movement (REM) behavior disorder (RBD) is characterized by loss of skeletal muscle atonia that can lead to dream enactment. This condition can cause harm to patients and their bed partners if appropriate safety measures are not ensured. This condition is often the initial presenting symptom in a group of complex neurodegenerative processes. Definitive diagnosis requires a thorough history and an in-laboratory polysomnogram to look for evidence of REM sleep without atonia. Treatment options are limited but consist of sleep safety measures and pharmacotherapy. Patients diagnosed with idiopathic RBD associated with alpha-synucleinopathy are likely to have progression of disease.
