Purpose: This in vitro study evaluated the effect of home dental bleaching on oxygen release, shear bond strength to enamel, and microleakage of direct composite restorations. Methods: Forty bovine incisors were used for oxygen dissolved titration by means of iodometric methods modified by azide iodine and 26 for the shear bond strength and microleakage tests. For titration the teeth were divided into four groups: T1 - bleached and titrated after 24 h; T2 - bleached and titrated after 48 h; T3 - unbleached and titrated after 24 h; T4 - unbleached and titrated after 48 h. T5 group served as control for the initial oxygen release rate. To analyze shear bond strength and microleakage, the teeth were randomly divided into two groups: CM1 - bleached; CM2 - unbleached. The bleaching agent Opalescence 15% (Ultradent, South Jordan, UT, USA) was applied for 8 h per day for three weeks. Z 250 (3M-ESPE, St. Paul, MN, USA) composite and Single Bond (3M-ESPE, St. Paul, MN, USA) adhesive were used for the composite restoration procedures. Results: There was an increase in oxygen release during the first 24 h post bleaching (ANOVA, P=0.0001). The bleached teeth showed a decrease in shear bond strength (Student’s t test, P=0.0001). The microleakage scores increased with bleaching independently from the examiner (Wilcoxon’s test: 1. P=0.0003; 2 P=0.0023; 3. P=0.0029). Conclusion: Considerable variation was noticed in the results of oxygen release, as well as in the results of shear bond strength and microleakage on bleached teeth. Objetivo: Avaliaram-se os efeitos do clareamento dental caseiro na liberação de oxigênio a partir de dentes clareados, na resistência de união ao cisalhamento e microinfiltração de restaurações com resina composta. Metodologia: Foram selecionados 40 incisivos bovinos para titulação dos níveis de oxigênio pelo método iodométrico e 26 dentes para resistência de união e microinfiltração. Para titulação os dentes foram divididos em 4 grupos: T1- clareado e titulado após 24 h; T2- clareado e titulado após 48 h; T3- não clareado e titulado após 24 h; T4- não clareado e titulado após 48 h. No grupo T5 não se usaram dentes, servindo como controle inicial do teor de oxigênio. Para cisalhamento associado a microinfiltração dividiram-se aleatoriamente os dentes em 2 grupos: CM1- clareado; CM2- não clareado. O regime de tratamento clareador (Opalescense a 15%, Ultradent, South Jordan, UT, USA) foi de 8 h por dia durante 3 semanas. Para as restaurações utilizou-se a resina Z250 (3M-ESPE, St. Paul, MN, USA) com adesivo Single Bond (3M-ESPE, St. Paul, MN, USA). Resultados: Houve aumento significativo na liberação de oxigênio durante nas primeiras 24 h pós clareamento (ANOVA, P=0,0001). O teste t de Student confirmou diminuição da resistência de união para o grupo clareado (P=0,0001). Houve significante aumento na microinfiltração no grupo clareado, independentemente do examinador (teste de Wilcoxon: 1. P=0,0003; 2 P=0,0023; 3. P=0,0029). Conclusão: Há considerável variação nos resultados de liberação de oxigênio, resistência de união e microinfiltração em dentes submetidos a clareamento dental caseiro.

Purpose: In addition to the chemical damage due to bleaching gels penetration into the pulp during pulp vitality dental bleaching, another possible aggressive factor could be the heat generated by the exothermal oxidation reaction of the bleaching gel, which may also be aggravated by the use of light activation. This study assessed the temperature variation in the pulp chamber in human teeth, using three different bleaching gels with or without LED light activation. Methods: Thirty human pre-molars were cut longitudinally to obtain buccal and lingual halves. The 60 specimens were divided into 3 groups, and the bleaching gel used varied as follows: 35% hydrogen peroxide (WHP); 37% carbamide peroxide (W) and 38% hydrogen peroxide (OX). Half of the specimens were submitted to bleaching with light activation and, the other half, without light activation. The light source used was the light emitting diode appliance (LED, 3-Light, Clean Line), and the intrapulpal temperatures were measured by using a digital thermometer. Data were analyzed by ANOVA and Tukey’s tests (alpha=5%). Results: The intrapulpal temperatures (in oC) were as follows: without light activation (WHP= 0.68b; W= 0.40b; OX= 0.48b); with light activation (WHP= 2.35a; W= 1.60a; OX= 1.80a ). Conclusion: LED light activation of bleaching gels increased the temperature in the pulp chamber, but did not achieve the critical temperature value of 5.5 oC. Objetivo: Além da agressão química devido à penetração dos géis clareadores na polpa durante o clareamento de dentes com vitalidade pulpar, outro possível fator agressor pode ser o calor gerado pela reação de oxidação exotérmica do gel clareador, que pode também ser agravada pela fotoativação. Este estudo avaliou a variação da temperatura na câmara pulpar de dentes humanos, utilizando-se três diferentes géis clareadores, com ou sem fotoativação. Metodologia: Trinta pré-molares humanos foram cortados longitudinalmente para obtenção de duas metades: vestibular e lingual. Os 60 espécimes foram divididos em 3 grupos e os agentes clareadores utilizados variaram como segue: peróxido de hidrogênio 35% (WHP), peróxido de carbamida 37% (W) e peróxido de hidrogênio 38% (OX). Metade dos espécimes foi submetida ao clareamento com fotoativação e, a outra metade, sem fotoativação. A fonte de luz utilizada foi o aparelho à base de diodo emissor de luz (LED, 3-Light, Clean Line) e as temperaturas foram medidas por um termômetro digital. Os dados foram analisados por análise de variância e teste de Tukey (alfa=5%). Resultados: Os resultados de temperatura foram: sem fotoativação (WHP= 0.68b; W= 0.40b; OX= 0.48b); com fotoativação (WHP= 2.35a; W= 1.60a; OX= 1.80a). Conclusão: A fotoativação dos géis clareadores com LED contribuiu para um maior aumento de temperatura na câmara pulpar, mas não se atingiu a temperatura crítica de 5,5oC.

Tradicionalmente, esses dentes eram tratados de maneira invasiva com confecção de facetas ou coroas dentárias. O clareamento externo é uma alternativa de tratamento que possibilita uma solução estética, conservadora e biológica, especialmente nos casos em que há uma dificuldade de acesso ao sistema de canais radiculares. O presente caso clínico mostra uma paciente que apresentava alteração de cor no incisivo central superior esquerdo, decorrente de calcificação distrófica total da câmara pulpar e do canal radicular, em função de um trauma. O tratamento englobou trinta dias de clareamento caseiro supervisionado, a partir do uso de moldeira com o gel de peróxido de carbamida a 16% e mais duas sessões de clareamento de consultório com peróxido de hidrogênio a 35% para resolver a intensa saturação no colo do dente. Introduction: The number of patients who seek orthodontic treatment that may have a history of tooth bleaching is increasing over the time. Bleaching may influence the decrease of the bond strength of orthodontic brackets. Objective: To determine and prove the effect of mangosteen peel (MP) extract to reverse the reduced shear bond strength (SBS) of orthodontic brackets after bleaching. Methods: A total of 150 maxillary first premolar teeth were randomly divided into 6 experimental groups as follow (n=25): negative-control (N: no bleaching), positive-control (P: bleaching + no treatment), and the treatment groups (bleaching + 10% sodium ascorbate (SA), 10% (MP-10), 20% (MP-20) and 40% (MP-40) MP extract gel). After treatment, the brackets were bonded with the resin-modified glass ionomer cement, SBS testing was performed using universal testing machine, and the adhesive remnant index (ARI) was examined using stereoscopic microscope after debonding. The SBS data were analyzed by analysis of variance (Anova) and the Tukey test. For the ARI, the Kruskal-Wallis test was performed. Result: There was significant SBS difference (p< 0.001) between various groups. The group without bleaching showed significantly higher SBS (8.19 ± 2.26 MPa) compared to others, while SBS in the group treated with 40% MP gel was significantly higher (7.93 ± 1.92 MPa) than other groups treated with antioxidants. The failure of orthodontic brackets bonded after bleaching and treatment using MP extract occurred at the enamel-adhesive interface. Conclusion: The application of MP extract as an antioxidant after bleaching was effective in reversing the reduced shear bond strength of orthodontic brackets after bleaching.
