Aim: To systematically review the literature for observational studies evaluating periodontal outcomes in pre- and post-liver transplant (LT) individuals compared to a control group. Methods: Specific strategies were designed to search five databases and the grey literature. Hand-searches were also performed. The following periodontal outcomes were analyzed in pre-LT, post-LT and control groups: plaque index (PI), gingival index (GI), gingival growth (GO), alveolar bone loss (ABL) probing depth (PD) and clinical attachment level (CAL). Meta-analyses and subgroup analyses were conducted. Results: Among the 257 studies retrieved, 11 studies were included. Four studies were incorporated into the meta-analysis and showed that PI (mean difference=0.15, 95%CI: 0.12-0.18) and GI (mean difference=0.31, 95%CI: 0.06-0.56) were significantly higher in post-LT individuals compared to the control group. There was no significant mean difference of PD (mean difference=0.49, 95%CI: -0.26-1.25) but CAL (mean differ- ence=1.47, 95%CI: 0.19-2.75) were significantly higher in pre-LT individuals compared to the control group. Conclusion: The higher means of PI and GI in post-LT and CAL in pre-LT compared to healthy individuals suggest a relationship between the periodontal condition and the transplantation status.

Aim: Although bulk fill composites have been widely used as restorative material, there is no consensus regarding the best clinical protocol in terms of composite technique and adhesive system. Therefore, this clinical trial evaluated the clinical performance of bulk fill composites for class I restorations under different protocols. Methods: A randomized clinical trial including 155 class I restorations was conducted using different adhesive systems: conventional technique (phosphoric acid + conventional three-step adhesive system) (Group 1, 2 and 3); or self-etching adhesive system (Groups 4, 5 and 6). Control groups 1 and 4 were restored with conventional composite; groups 2 and 5 with low viscosity bulk fill and conventional composite as occlusal coverage; groups 3 and 6 with high viscosity bulk fill. The FDI criteria was used for clinical evaluation at baseline and after 6 months. Results: All groups showed good clinical performance. At baseline, the adhesive system did not affect postoperative hypersensitivity. OBJECTIVE The aim of this systematic review and meta-analysis was to evaluate the experience of dental caries in individuals pre- or postliver transplant. MATERIALS AND METHODS Searches in PubMed, Scopus, Web of Science, Ovid, and Lilacs from databases' inception date up to April 2020 were undertaken. Gray literature and manual searches were also conducted. Observational studies were eligible. The retrieved references were evaluated by two independent reviewers. Meta-analysis and risk of bias assessment using the University of Adelaide tool were conducted. The strength of the evidence was assessed with GRADE. RESULTS The search retrieved 1990 references. Twenty-four cross-sectional studies were included. One subgroup analysis demonstrated no significant difference in the number of teeth with dental caries between pre-liver transplant and healthy individuals (mean difference = 1.65, confidence interval = -0.87 to 4.17). The prevalence of dental caries among pre-liver transplant individuals was 73.82% and in the post-transplant individuals was 72.83%. In the included studies, the main concern regarding risk of bias was the absence of control for confounding variables. The strength of the evidence was very low. CONCLUSIONS Dental caries may be a relevant issue in pre- and postliver transplant individuals. Oral health counseling should be included in the care of pre- and/or postliver transplant individuals.

Introduction This study radiographically analyzed the prevalence of root canal treatment (RCT) and apical periodontitis (AP) in type 1 diabetes mellitus (T1DM) and nondiabetic individuals and its association with the history/current status of T1DM. Methods In a cross-sectional paired study, the radiographic records of 50 individuals with T1DM and 100 age- and sex-matched nondiabetic subjects were examined. The presence of RCT and AP was evaluated. Information regarding the history and current status of T1DM was collected from the medical records of each patient. Results One or more RCTs were found in 76% and 44% of diabetic and nondiabetic subjects, respectively (P = .000). AP in 1 or more teeth was found in most T1DM patients (58%) and in 15% of the control subjects (P = .000). One or more RCTs associated with AP were found in 52% and 8% of T1DM and nondiabetic subjects, respectively (P = .000). Bivariate logistic regression analyses suggested that RCT (odds ratio [OR] = 10.435, P = .000), AP (OR = 3.508, P = .011), and RCT + AP (OR = 7.220, P = .000) were significantly associated with the presence of T1DM. Multivariate logistic regression showed that among T1DM individuals, there is an association between 11–15 years of diagnosis time and at least 1 RCT (OR = 46.316, P = .038) and an association between T1DM control and at least 1 tooth with AP (OR = 15.611, P = .016). Conclusions RCT, AP, and RCT with AP were more prevalent in individuals with T1DM than in nondiabetic individuals. RCT and AP were associated with the presence of T1DM, specifically RCT with diagnostic time and AP with glycemic control.

CAPES - Coordenação de Aperfeiçoamento de Pessoal de Nível Superior Maxillofacial prosthetic rehabilitation is almost always indicated after surgical treatment of affected individuals with oral cavity and pharynx cancers. Few articles have described in detail the technical sequence of maxillofacial prosthesis fabrication. The first case is a 56-year-old woman who was rehabilitated with pharyngeal obturator prosthesis after a partial maxillectomy, including soft palate, tonsil, oropharynx, and retromolar space regions. The second case is an 83-year-old man who was rehabilitated with hard palatal obturator prosthesis after a maxillectomy, including hard palate and nasal floor. In both cases, the patients complained of oronasal regurgitation and difficulty in chewing, swallowing, and speaking. A multi-professional approach including oral rehabilitation should be part of the treatment plan for individuals with oral and pharyngeal cancer after tumor resection.

Dental implants are a common and successful option for the rehabilitation of edentulous areas. However, there are some limitations to their use. The present clinical report describes a 5-year follow-up of maxillary rehabilitation using an attachment-retained removable partial denture (RPD) associated with fixed partial denture (FPD) units as an alternative to orthodontic treatment associated with orthognathic surgery and dental implant placement. Rehabilitation with fixed prosthodontics associated with a removable partial prosthesis was proposed. For greater precision and stability, a gold attachment was prepared. The patient was very satisfied with the rehabilitation and has been clinically followed for 5 years. In cases where the use of dental implants and/or conventional FPDs is limited or not indicate the association between an FPD and an RPD by means of attachments remains an important alternative to conventional clasp-retained RPDs.

This study goal is to present a case report of tooth bleaching treatment with the combination of the custom tray and in-office techniques, and to discuss about the considerations for result maintenance. A treatment approach was planned involving exogenous bleaching with custom trays with 7.5% hydrogen peroxide, and in-office bleaching with 35% hydrogen peroxide. The in-office bleaching agent was applied in two 15-minute applications, without led light. The patient was instructed to use the custom trays only a day after, with one drop of the bleaching agent for each tooth space. After the application of the bleaching agent, the patient used the trays for 1 hour per day, continuously for 30 days, 15 days for each arch. Considering the executed treatment approach, we may conclude that when correctly indicated, tooth bleaching with the combination of the in-office and custom trays techniques can be an acceptable choice of treatment, capable of enhancing the smile esthetics with a simple technique that preserves the sound dental structure. The patient should be instructed for the maximum color preservation and esthetic result maintenance, and should be advised that retreatments may become necessary depending on the oral hygiene and frequence of coloured food and drinks intake. RESUMO O objetivo deste estudo é apresentar um relato de caso em que foi realizado o clareamento dentário com a combinação das técnicas de consultório e de moldeiras e discutir sobre considerações para a manutenção do resultado. Um plano de tratamento foi proposto envolvendo clareamento exógeno com moldeiras com peróxido de hidrogênio 7,5% e o clareamento em consultório com peróxido de hidrogênio 35%. O gel clareador de clareamento em consultório foi aplicado 2 vezes, por 15 minutos cada aplicação, sem luz do tipo Led. A paciente foi orientada a usar as moldeiras customizadas para o clareamento caseiro apenas a partir do dia seguinte, sendo uma gota de gel clareador no espaço equivalente a cada dente. Após a dosagem do gel, ela usou as moldeiras por 1 hora por dia, ininterruptamente por 30 dias, sendo 15 dias para cada arco. Diante do planejamento e terapia executados no caso clínico apresentado, pode-se concluir que a técnica clareadora utilizando a combinação de clareamento em consultório e clareamento com moldeiras customizadas, quando corretamente indicada, pode ser considerada uma alternativa de tratamento bastante satisfatória por ser uma técnica simples capaz de melhorar a estética do sorriso preservando a estrutura dental hígida. O paciente deve ser orientado para a máxima preservação da cor e manutenção estética alcançada, e deve ser ressaltado que retratamentos podem ser necessários dependendo da higiene oral e da frequência de ingestão de alimentos e bebidas corantes.

Background The aim of the present study was to investigate pain perception and anxiety within the context of surgery for the placement of mandibular block bone and to evaluate the causality effect between theses variables. Material and methods A total of 13 patients were recruited for the study and were submitted to mandibular autogenous block bone surgery. Demographic data were collected and the anxiety level was determined using the State-Trait Anxiety Inventory (STAI). The STAI was administered on the day of surgery and on the 14th postoperative day. Pain was determined using the visual analogue scale (VAS) and limitation of daily activities and postoperative symptoms were also reported. Data were analyzed using parametric tests (α=0.05) and cross-lagged analysis was performed to verify a causality effect. Results Few patients reported interference with daily activities or the presence of postoperative symptoms. A significant association of bad breath/taste with STAI-State was detected on the 14th postoperative day. No evidence of causality between STAI and VAS was detected. Conclusions The patient's self-evaluation indicates that the pain and anxiety level felt during treatment was not directly associated with the clinical aspects of the surgical procedure or with postoperative activities/symptoms limitations. Objective To evaluate studies assessing the prevalence of dental caries and developmental defects of enamel (DDE) in individuals with chronic kidney disease (CKD) in comparison with individuals without CKD. Materials and methods Electronic searches were performed in PubMed, Web of Science, Scopus, Medline via Ovid, and ProQuest databases from their inception date until February 2018. Two review authors independently selected the studies, extracted data, and assessed the methodological quality. Meta-analysis was performed. Results Twenty-seven studies were included in this study. For permanent teeth, 14 studies found that individuals without CKD had higher dental caries scores than those with CKD. However, only five studies presented results with a statistically significant difference between groups. Among the studies evaluating primary teeth, five showed that individuals without CKD had higher dental caries scores than those with CKD. Five studies showed that individuals with CKD had a significantly higher prevalence of DDE than individuals without CKD. The meta-analyses showed that individuals without CKD had significantly higher scores of dental caries teeth and surfaces than individuals with CKD. For DDE, no statistical difference between groups was observed. Conclusions Individuals with CKD present lower dental caries scores and a higher prevalence of DDE in comparison with individuals without CKD.

SUMMARYIt was possible to restore the shape and function of a severely ankylosed tooth by fabricating a ceramic crown for placement on a resin reconstruction supported by mini fiberglass posts. By increasing the retention of the morphological reconstruction for the future support of indirect restorations, cementable intradentinal fiberglass posts enhance the longevity of these restorations. Ultimately, all proposals that seek to improve the long-term prognosis of restorations on ankylosed teeth, especially severely impacted ones, are of extreme clinical relevance.

The study evaluated the longevity, effectiveness, safety, and impact on the oral health-related quality of life of in-office dental bleaching using low-concentration hydrogen peroxides. Randomized, parallel, and double-blinded clinical trial was performed with 54 participants using 6% or 15% hydrogen peroxide (HP) in-office bleaching activated via hybrid LED/laser light. The aim of this study was to evaluate the clinical performance of silorane-based composite restorations applied after different surface treatments. This controlled and randomized clinical trial included 26 patients with class I restoration indications. The teeth were randomly assigned to the following treatments: control 1/G1, silorane specific self-etching adhesive (P90 self-etch primer and bond) + silorane-based composite resin (Filtek™ P90 low shrink posterior restorative); G2, 37% phosphoric acid + silorane specific self-etching adhesive + silorane-based composite resin; G3, blasting with aluminum oxide + silorane specific self-etching adhesive + silorane-based composite resin; and control 2/G4, self-etching adhesive (Adper™ SE Plus self-etch adhesive) + dimethacrylate-based composite resin (Filtek™ P60 posterior restorative). The clinical performance was evaluated at baseline and after 1 year. A total of 141 restorations were made and evaluated. This clinical report describes an 8-year follow-up evaluation using different thicknesses of porcelain laminate veneers of diastema and malformed anterior teeth. Minimally invasive treatment with no preparation or minimal reduction options could be considered and chosen based on the characteristics of each case. Laminate veneers with or without reduction can offer an excellent result regarding esthetics and function. Despite its failure without reduction, this treatment is indicated because it can be applied regardless of the structure of the teeth. At first, no tooth preparation with rotatory instruments was performed, and porcelain laminates of minimum thickness were made for the four maxillary anterior teeth. After 6 years, the adhesive interfaces were stained, and the patient was unsatisfied with the esthetics of her smile. Therefore, at that time, the professional decided to prepare the anterior teeth for porcelain laminate veneers. Diastema closure and/or correction of malformed anterior teeth using porcelain laminate veneers is a viable option for the clinician because it restores esthetic harmony. The patient was very pleased with the new laminate veneers.

This study aimed to investigate anxiety in patients attended at a university dental clinic. The present cross-sectional study included 49 patients recruited at the undergraduate dental clinics, Federal University of Juiz de Fora, Governador Valadares campus, Minas Gerais, Brazil, from September 2016 to August 2017. Information regarding gender, age, family income, educational level, frequency of dental consultations and dental procedure causing discomfort were collected using a questionnaire. The level of anxiety was evaluated by the Dental Anxiety Scale. A mild level of anxiety (median=6) was observed, with a minimum level of 4 (not anxious) and a maximum level of 19 (extremely anxious). Women had higher anxiety than men (p=0.047). Individuals with 9-11 years of schooling presented a lower level of dental anxiety than those with 0-8 years of schooling (p=0.025). Participants who attributed a greater discomfort to surgeries and the use of high-speed burs had a greater anxiety (p=0.002). Light anxiety is present in patients undergoing dental treatment in university clinics, and women are more anxious than men. Patients with higher education were less anxious than lower education level. The report of a greater discomfort with surgeries and procedures with high-speed burs were associated with a greater anxiety. Este estudo objetivou investigar a ansiedade em pacientes atendidos em clínicas odontológicas universitárias. O presente estudo transversal incluiu 49 pacientes recrutados nas clínicas odontológicas da Universidade Federal de Juiz de Fora, campus Governador Valadares, Minas Gerais, Brasil, de setembro de 2016 a agosto de 2017. Informações sobre sexo, idade, renda familiar, escolaridade, frequência de consultas odontológicas e procedimentos odontológicos causadores de desconforto foram coletadas por meio de questionário. O nível de ansiedade foi avaliado pela Dental Anxiety Scale. Observou-se um nível de ansiedade leve (mediana = 6), com mínimo de 4 (não ansioso) e máximo de 19 (extremamente ansioso). As mulheres apresentaram maior ansiedade que os homens (p = 0,047). Indivíduos com 9 a 11 anos de escolaridade apresentaram menor nível de ansiedade odontológica do que aqueles com 0 a 8 anos de estudo (p = 0,025). Os participantes que atribuíram maior desconforto às cirurgias e ao uso de alta rotação apresentaram maior ansiedade (p = 0,002). Conclui-se que a ansiedade leve está presente em pacientes submetidos a tratamento odontológico em clínicas universitárias, sendo as mulheres mais ansiosas do que os homens. Background The increasing demand of tooth bleaching has also increased the need of researches focusing on the durability of the resultant color. The aim of this study was to evaluate the effect of a system based on calcium silicate, sodium phosphate and sodium monofluorophosphate (Regenerate™) on color maintenance of bleached enamel considering two waiting times for the contact with a cola drink. Material and Methods This in vitrostudy was performed on bovine enamel specimens (n = 100), bleached with 35% H2O2 and treated with: G1 Distilled water; G2 Artificial saliva; G3 RegenerateTM Serum and Toothpaste; G4 RegenerateTM Toothpaste; G5 RegenerateTM Serum. The groups G3, G4 and G5 received one application of the respective products for 3 min, in 3 consecutive days. Statement of problem Decementation is one of the most frequent causes of failure in the post-cement system. The bond strength in different parts of the canal may be influenced by technical and anatomic factors. Purpose The purpose of this in vitro study was to assess the effects of anatomic root levels to reach the canal and different resin cements on the bond strength of fiberglass posts along the canal. Material and methods One hundred thirty-five roots of bovine teeth were endodontically treated, prepared with 15-mm-long post spaces and divided according to the anatomic root level: coronal (with 15-mm post space), middle (with 10-mm post space), and apical (with 5-mm post space). Fiberglass posts were luted with the cements (n=15): conventional resin cement with 3-step etch-and-rinse adhesive system (RelyX ARC/SBMP); self-adhesive resin cement (RelyX U200) and autopolymerizing resin cement with etch-and-rinse adhesive system (C&B/All-Bond 2). After 24 hours, specimens were sectioned and subjected to the push-out test. The maximum extrusion load was recorded (0.5 mm/minute, 200 N). Data were analyzed with analysis of variance (ANOVA) and the Tukey test (α=.05). Failure mode was analyzed by using multinomial logistic regression. Results There were no significant effects of resin cements ( P >.05). The bond strength at apical third was higher when the canal was reached at the apical level than at the coronal level ( P =.022). When each root level was directly reached, the coronal bond strength was lower than apical ( P =.001) and middle ( P =.021) for all cements. Conclusions When the canal was reached at the coronal, medium, and apical levels, the bond strength of conventional, self-adhesive and autopolymerizing resin cements was lower in the coronal root third.

Objective. This study evaluated the effect of immediate or delayed finishing/polishing using different systems on the surface roughness, hardness, and microleakage of a silorane-based composite.Material and Methods. Specimens were made with silorane-based composite (Filtek P90, 3M ESPE) and assigned to the treatments: control (light-cured); aluminum oxide discs (Sof-Lex, 3M ESPE); diamond-impregnated silicone tips (Astropol, Ivoclar Vivadent); aluminum oxide-impregnated silicone tips (Enhance, Dentsply). Half of the specimens were finished/polished immediately and the rest after 7 days. Surface roughness (Ra,μm;n=20) and Vickers microhardness (50 g; 45 s;n=10) were measured. Cavities were prepared in bovine incisors and filled with Filtek P90. The fillings received immediate or delayed finishing/polishing (n=10) and were subjected to dye penetration test (0.5% basic fuchsin, 24 h). Data were analyzed by ANOVA and Scheffe, Kruskal-Wallis, and Mann-Whitney tests (p<0.05).Results. The finishing/polishing system significantly influenced roughness and microhardness (p<0.0001). For enamel, microleakage was not affected by the finishing/polishing system (p=0.309). For dentin, Sof-Lex discs and Astropol points promoted greater microleakage than Enhance points (p=0.033).Conclusion. Considering roughness, microhardness, and microleakage together, immediate finishing/polishing of a silorane-based composite using aluminum oxide discs may be recommended.

Objective.To study the prevalence, risk factors, and mortality of invasive fungal infections (IFI) in patients with childhood-onset systemic lupus erythematosus (cSLE).Methods.A retrospective multicenter cohort study was performed in 852 patients with cSLE from 10 pediatric rheumatology services. An investigator meeting was held and all participants received database training. IFI were diagnosed according to the European Organization for Research and Treatment of Cancer/Invasive Fungal Infections Cooperative Group and the National Institute of Allergy and Infectious Diseases Mycoses Study Group Consensus Group criteria (proven, probable, and possible). Also evaluated were demographic, clinical, and laboratory data, and disease activity [SLE Disease Activity Index 2000 (SLEDAI-2K)], cumulative damage (Systemic Lupus International Collaborating Clinics/American College of Rheumatology Damage Index), treatment, and outcomes.Results.IFI were observed in 33/852 patients (3.9%) with cSLE. Proven IFI was diagnosed in 22 patients with cSLE, probable IFI in 5, and possible IFI in 6. Types of IFI were candidiasis (20), aspergillosis (9), cryptococcosis (2), and 1 each disseminated histoplasmosis and paracoccidioidomycosis. The median of disease duration was lower (1.0 vs 4.7 yrs, p < 0.0001) with a higher current SLEDAI-2K [19.5 (0–44) vs 2 (0–45), p < 0.0001] and current prednisone (PRED) dose [50 (10–60) vs 10 (2–90) mg/day, p < 0.0001] in patients with IFI compared with those without IFI. The frequency of death was higher in the former group (51% vs 6%, p < 0.0001). Logistic regression analysis revealed that SLEDAI-2K (OR 1.108, 95% CI 1.057–1.163, p < 0.0001), current PRED dose (OR 1.046, 95% CI 1.021–1.071, p < 0.0001), and disease duration (OR 0.984, 95% CI 0.969–0.998, p = 0.030) were independent risk factors for IFI (R2 Nagelkerke 0.425).Conclusion.To our knowledge, this is the first study to characterize IFI in patients with cSLE. We identified that disease activity and current glucocorticoid use were the main risk factors for these life-threatening infections, mainly in the first years of disease course, with a high rate of fatal outcome.

SUMMARY Objective The aim of this study was to investigate the clinical performance and to validate the Lesion Activity Assessment (LAA) in conjunction with the International Caries Detection and Assessment System (ICDAS) for occlusal caries diagnosis in permanent teeth. Methods Patients with erupted or partially erupted third molars were recruited from the surgery clinic of the School of Dentistry of the Universidade Federal de Minas Gerais, Brazil. A calibrated examiner evaluated 49 teeth using the ICDAS-LAA criteria. The histologic criterion proposed by Ekstrand and others was used to validate severity at the thresholds D1 (outer half of the enamel), D2 (inner half of the enamel and outer third of the dentin), and D3 (inner or middle third of the dentin). Lesion activity was validated using 0.1% methyl red solution. Results The method demonstrated good reliability (weighted kappa for severity=0.60; unweighted kappa for activity=0.61). The ICDAS presented a higher performance for lesion detection (area under the receiver operating characteristic curve [Az]=0.79) using the threshold D3. At the thresholds D1 and D2, the results for Az were 0.57 and 0.74, respectively. Regarding the ICDAS-LAA, Az = 0.59. Conclusions Clinical protocols can use ICDAS for the severity diagnosis of occlusal caries, but the LAA performance was poor.

The aim of this study was to compare the areas of stress concentration in a three-dimensional (3D) premolar tooth model with anisotropic or isotropic enamel using the finite element method. A computed tomography was imported to an image processing program to create the tooth model which was exported to a 3D modeling program. The mechanical properties and loading conditions were prescribed in Abaqus. In order to evaluate stresses, axial and oblique loads were applied simulating realistic conditions. Compression stress was observed on the side of load application, and tensile stress was observed on the opposite side. Tensile stress was concentrated mainly in the cervical region and in the alveolar insertion bone. Although stress concentration analyses of the isotropic 3D models produced similar stress distribution results when compared to the anisotropic models, tensile stress values shown by anisotropic models were smaller than the isotropic models. Oblique loads resulted in higher values of tensile stresses, which concentrate mainly in the cervical area of the tooth and in the alveolar bone insertion. Anisotropic properties must be utilized in enamel stress evaluation in non-carious cervical lesions.

Purpose Different periodontitis case definitions have been used in clinical research and epidemiology. The aim of this study was to determine more accurate criterion for the definition of mild and moderate periodontitis case to be applied to head and neck cancer individuals before radiotherapy. Methods The frequency of periodontitis in a sample of 84 individuals was determined according to different diagnostic criteria: (1) Lopez et al. (2002);(2) Hujoel et al. (2006); (3) Beck et al. (1990); (4) Machtei et al. (1992); (5) Tonetti and Claffey (2005); (6) and Page and Eke (2007). All diagnosis were based on the clinical parameters obtained by a single calibrated examiner (Kw=0.71). The individuals were evaluated before radiotherapy. They received oral hygiene instructions, and the cases diagnosed with periodontitis (Page and Eke 2007) were treated. The gold standard was the definition 6, and the others were compared by means of agreement, sensitivity (SS), specificity (SP), and the area under ROC curve. The kappa test evaluated the agreement between definitions. Results The frequency of periodontitis at baseline was 53.6 % (definition 1), 81.0 % (definition 2), 40.5 % (definition 3), 26.2 % (definition 4), 13.1 % (definition 5), and 70.2 % (definition 6). The kappa test showed a moderate agreement between definitions 6 and 2 (59.0 %) and definitions 6 and 1 (56.0 %). The criterion with higher SS (0.92) and SP (0.73) was definition 1. Conclusion Definition 1 was the most accurate criterion to case periodontitis definition to be applied to head and neck cancer individuals.

Aim: Little is known about the biomechanical behavior of disocclusion patterns in implant-supported prostheses. Thus, the aim of the present study was to analyze the stresses generated at bone-implant interface during different patterns of disocclusion in an implant-supported lower complete denture without free distal ends using the three-dimensional (3-D) finite element method. Study Design: Finite element method. Methods: A 3-D model of a complete denture supported by five inter-foraminal implants and two distal was developed (CAE software Abaqus). A canine guide disocclusion (CG) was simulated applying a nodal load of 15 N at an angle of 45° on the canine tooth prosthesis, while to a bilateral balanced occlusion (BBO) a similar load pattern was applied at four distinct points, bilaterally. Linear elastic static analysis was used to compare the magnitude of maximum and minimum principal stresses at bone-implant interface for each simulation. Results: The disocclusion pattern generated during CG exhibited a greater stress concentration at the bone-implant interface of the distal implant on the working side. BBO showed a more homogeneous stress distribution pattern at the bone-implant interface of the two distal implants. Anterior implants showed lower stress concentration. Conclusions: Bilateral balanced occlusion (BBO) resulted in a more favorable stress distribution in this complete implant-supported prosthesis model.

Background Bleached dental enamel can be more susceptible to staining than the enamel that has never been bleached, especially right after tooth bleaching. The aim of this study is to evaluate the effect of surface treatments and waiting time prior to contact with dye on bleached enamel staining susceptibility. Material and Methods One hundred teeth were bleached with 35% hydrogen peroxide (Whiteness HP, FGM) and randomly assigned to G1 artificial saliva, G2 2% sodium fluoride (Flugel, Nova DFL), G3 casein phosphopeptide-amorphous calcium phosphate fluoride paste (CPP-ACPF, MI Paste Plus, GC America), G4 rinse for color maintenance after bleaching (Keep White Rinse, DMC) and G5 polishing with aluminum oxide-impregnated disks (Super Buff Disk, Shofu). Fifty specimens were immersed in red wine for 15 minutes, immediately after treatment, and the others one hour after. Color difference (∆E) was evaluated with a spectrophotometer (Vita EasyShade). Surface treatments and waiting time effects were analyzed with Kruskal-Wallis and Mann Whitney tests (p0.05) and waiting time (p>0.05) were not significant to decrease bleached enamel susceptibility to red wine staining. Conclusions Surface treatments were similar to artificial saliva for bleached enamel susceptibility to red wine staining. Immediate or one-hour-postponed contact with red wine did not affect bleached enamel color. Key words:Tooth bleaching, color, dental enamel, hydrogen peroxide, pigmentation.

Statement of problem The mechanical stability of the prosthetic components in the implant-prosthesis complex is essential to the long-term success of the restorations. However, little is known about the differences in the biomechanical behavior of screw- and cement-retained prostheses. Purpose The purpose of this study was to compare the preload maintenance, stresses, and displacements of prosthetic components of screw- and cement-retained implant-supported prostheses by using the finite element method in a nonlinear analysis. Material and methods Two 3-dimensional models were constructed: implant-supported fixed partial prostheses with 3 elements retained either by screws (SFP) or cement (CFP). After the simulation of screw tightening, the preload was calculated for both prostheses. Then vertical and oblique loads (100 N) were applied on the models. The preload was identified, the maximum von Mises equivalent stresses (SEQV) were obtained on the screws, and the displacement among the abutment, the implant, and screw was identified by observing the penetration and gap in the contact interfaces. Results Under vertical load, there was a higher decrease in the preload and in the SEQV on the screw in the SFP. Under oblique load, the SEQV was 24% higher on the screw of the SFP. In the displacement analysis under vertical load, penetration was concentrated in the threads of the screw in the SFP and between the abutment and implant in the CFP. The gap was 118% greater for the SFP and was concentrated on the abutment extension. Under oblique load, the displacement pattern was similar for both prostheses, but with values 66% higher for penetration and 96% higher for gap for the SFP. Conclusions The SFP showed a higher biomechanical risk of failure than the CFP.

PURPOSE The aim of the study was to present a methodology of development of a virtual 3-dimensional dental implant model for analyses via the finite element method. MATERIALS AND METHODS A set, consisting of a dental implant and abutment, was embedded in acrylic resin for subsequent metallographic grinding and polishing. After the evidentiation of the internal geometry of the implant, the specimen was treated in a sputter for observation using scanning electron microscopy (SEM). The SEM image was transported to computer-aided design software by which all details of the implant were measured. With the measures obtained, the geometry was reproduced with 3-dimensional modeling software. Finally, the model was imported into finite element method analysis software with which it was discretized, generating a mesh. RESULTS A model with the accurate geometry of the implant was developed. A mesh of 297,600 elements and 490,045 nodes was generated. An aleatory acceleration simulation was performed to test the mesh, and no errors were identified. CONCLUSION The developed methodology generated a precise dental implant model, which can be applied in different finite element method simulations.

This study aimed to systematically review the scientific evidence for the association between noncarious cervical lesions (NCCL) and occlusal risk factors (ORF) [occlusal interferences in excursive movements; occlusal force; premature contacts; type of guidance; skid of centric occlusion to maximum intercuspidation] in adults.Pubmed, Web of Science, Cochrane, Lilacs, Clinical Trials, National Research Register and National Institute for Health were searched.From 1082 potentially eligible studies, 106 were selected for full text analysis. Two independent reviewers (Kappa=0.8; p0.001) selected the studies, abstracted information and assessed quality based on standardised scales. Six cross-sectional, two case-controls and one clinical trial were included. Several occlusal variables were analysed among the studies, but there was no standardisation of the units used in the analysis of occlusal factors. The majority of studies did not find significant associations between NCCL and ORF. Three studies found associations between NCCL and some variables (occlusal contact area, right canine guidance, premature contacts in centric relation and working side) (p0.05). The methodological quality varied across studies, and there was high heterogeneity among them.Current scientific evidence does not support an association between ORF and NCCL. Further prospective studies with standardised methods are vital to strengthen the evidence.Understanding the risk factors for NCCL is important to control the causes and to help the dentist choose the best approach for the patient. The evidence does not support intervention to alter some occlusal factors for the prevention or control of the progression of NCCL.

The aim of this in vivo study was to validate the DIAGNOdent laser fluorescence method and the International Caries Detection and Assessment System (ICDAS) for caries classification against the gold standard, histological examination, for detecting occlusal caries on permanent molars; the thresholds used were the outer enamel (D1), the inner enamel and outer third of the dentine (D2), and the inner two-thirds of the dentine (D3). Patients with non-impacted third molars (n = 43) were recruited from a university clinic. A trained examiner performed the examinations. After the teeth were extracted, the histological criterion was used to determine the severity of the lesions. Intra-examiner agreement (weighted kappa) for ICDAS was 0.60 and reliability (intraclass correlation coefficient) for DIAGNOdent was 0.968. The correlations with the histological reference were weak for DIAGNOdent (rs = 0.369) and moderate for ICDAS (rs = 0.515). The areas under the receiver-operating characteristics (ROC) curve at D1, D2, and D3 were 0.60, 0.69, and 0.91, respectively, for ICDAS and 0.55, 0.65, and 0.92, respectively, for DIAGNOdent. ICDAS and DIAGNOdent proved to be reproducible methods with similar performance in the detection of occlusal carious lesions in dentine. The ability of DIAGNOdent to detect initial enamel lesions was higher than that of ICDAS, but with low specificity. The usefulness of DIAGNOdent as an adjunct method for assessment of initial occlusal caries in permanent molars is questionable.

AimThe aim of this randomized clinical trial was to compare the proximal contact of a silorane-based resin composite with a conventional methacrylate-based resin composite in class II restorations after a 6 months follow-up period.Materials and methodsAfter obtaining informed consent, 33 patients were randomly allocated into a test group (Filtek P90/Adhesive System-3M ESPE) or control group (Filtek P60/ Adper SE Plus-3M ESPE), and 100 direct resin composite restorations (n = 50) were placed. A single operator performed the cavities and restorations. After rubber dam placement, a metal matrix and wooden wedge were placed. The restorative systems were applied according to the manufacturer's instructions. After 1 week, the restorations were finished and polished. The proximal contacts were assessed blindly and independently by two calibrated examiners (kW = 0.8) at the baseline and after 6 months according to a three-step grading criteria. Data were analyzed with the Mann-Whitney U-test and Wilcoxon signed Rank tests (α = 0.05). Results: After 6 months, 96% of the restoration contacts were present for evaluation. The frequencies of restorations classified as Bravo in control and test groups were 6 and 8% at the baseline, and 6.25 and 12.75% after 6 months. The cementoblastoma is a very rare odontogenic neoplasm characterised by the formation of cementum-like tissue in connection with the root of a tooth. An 18-year-old boy was referred to the Clinic of Oral Pathology of the School of Dentistry, Universidade Federal de Minas Gerais, Brazil, complaining of an irradiating, itching pain from the region of the lower right first molar. He reported that the symptoms began three months prior, with a gradual increase in intensity during this period. The pain episodes were of short duration, with characteristics of toothache originating from the pulp. Upon intra-oral clinical examination, an open bite and slight increase in volume at the buccal alveolar region of the lower right first molar was observed. He also reported palpation and percussion pain symptoms in this region. Electrical, heat and cold pulp vitality tests were performed, and the tooth was vital. Periapical and panoramic radiographs showed radiolucent images with diffuse, poorly defined borders at the mesial root periapex extending to the interradicular region suggesting an inflammatory periapical lesion. In this early presentation, the three-dimensional image was fundamental to the final diagnosis of cementoblastoma.

Materials and methods: An official list of all dentists registered in the city of Belo Horizonte, Brazil, was obtained from the Regional Dentistry Council. A questionnaire was delivered to 276 randomly selected dentists; the survey contained questions designed to characterise the sample and identify dentists’ conduct towards elderly patients. Secondary caries is still the main cause of restoration replacement, especially on the root surface OBJECTIVE: This in vitro study evaluated the cariostatic effects of fluoride-containing restorative materials associated with fluoride gels, on root dentin. MATERIALS AND METHODS: A randomized complete block design was used to test the effects of the restorative systems, fluoride regimes and the interactions among them at different distances from restoration margins. Standardized cavities were prepared on 240 bovine root specimens and randomly assigned to 15 groups of treatments (n=16). Cavities were filled with the following restorative materials: Ketac-Fil (3M-ESPE); Vitremer (3M-ESPE); Dyract/Prime & Bond NT (Dentsply); Charisma/Gluma One Bond (Heraeus Kulzer) and the control, Z250/Single Bond (3M-ESPE). The specimens were subjected to a pH-cycling model designed to simulate high-caries activity. During the cycles, 1.23% acidulated phosphate fluoride, 2.0% neutral sodium fluoride or deionized/distilled water (control) was applied to the specimens for 4 min. The surface Knoop microhardness test was performed before (KHNi) and after (KHNf) the pH cycles at 100, 200 and 300 mm from the margins. Dentin microhardness loss was represented by the difference in initial and final values (KHNi - KHNf). Data were analyzed by Friedman's and Wilcoxon's tests, ANOVA and Tukey's test (α=5%). RESULTS: The interaction of restorative systems and topical treatments was not significant (p=0.102). Dentin microhardness loss was lowest closer to the restoration. Ketac-fil presented the highest cariostatic effect. Vitremer presented a moderate effect, while Dyract and Charisma did not differ from the control, Z250. The effects of neutral and acidulated fluoride gels were similar to each other and higher than the control. CONCLUSION: Conventional and resin-modified glass ionomer cements as well as neutral and acidulated fluoride gels inhibit the progression of artificial caries adjacent to restorations. The associated effect of fluoride-containing restorative materials and gels could not be demonstrated.

This study determined the prevalence of oral mucosal alterations and associated factors among the institutionalized elderly in Brazil. Data were collected through a structured questionnaire, a review of their medical records and an intra-oral examination. A sample of 335 individuals over 60 years of age was randomly selected. In total, 646 alterations were diagnosed; 59.1% were variations of the normal oral mucosa. The most frequent variation consisted of sublingual varicosities (51.6%). Denture stomatitis (15.2%) and denture hyperplasia (12.8%) were the most frequent lesions. Elderly patients who wore dentures had a significantly higher prevalence of oral mucosal lesions (p = 0.00) than those that didn't wear dentures. There was a high prevalence of variations of the normal oral mucosa and of mucosal lesions, especially denture-induced lesions, among the institutionalized elderly.

Periodontitis has been associated with adverse pregnancy outcomes. Results from intervention studies are few and controversial. The present study assessed the effects of non-surgical periodontal treatment in the occurrence of adverse pregnancy outcomes. Two hundred forty-six eligible women were randomly divided into two groups: periodontitis intervention (n = 122; undergoing non-surgical treatment during gestation) and periodontitis control (n = 124; not treated during gestation). Univariate analysis was performed and estimates of relative risk were reported. Data from 225 women were analyzed. No differences for preterm birth (p = 0.721), low birth weight (p = 0.198), and preterm low birth weight (p = 0.732) rates were observed. Relative risk estimates for preterm birth, low birth weight, and preterm low birth weight in the periodontitis intervention group were 0.915 (95% CI 0.561-1.493), 0.735 (95% CI 0.459-1.179), and 0.927 (0.601-1.431), respectively. Non-surgical periodontal treatment during the second semester of gestation did not reduce the risk for preterm birth, low birth weight, and preterm low birth weight.

Purpose: This study evaluated microleakage on amalgam restorations repaired by amalgam and bonded amalgam. Methods: Thirty extracted human pre-molars were restored by amalgam. A simulated defect was prepared and assigned to two treatment groups (n=15): G1 - repaired by amalgam (Permite C-SDI); G2 - repaired by bonded amalgam (Caulk 34% Tooth Conditioner Gel – Dentsply + Prime & Bond 2.1 – Dentsply + Permite C- SDI). The teeth were immersed in a 50% silver nitrate solution, thermocycled and then, sectioned longitudinally through the restoration center and examined by 3 examiners using a stereomicroscope. Microleakage was evaluated in a 0-4 scale for dye penetration. Microleakage data were analyzed by Kruskal Wallis and Dunn test. Results: The bonded amalgam technique was significantly the most effective in repair/tooth interfaces sealing (score 0=53.3%, P=0.0012). Aim The aim of this article is to review the clinical and biological features underlying the development and progression of radiation caries. Background Although radiotherapy (RT) plays an important role in the management of patients with head and neck cancer (HNC), it is also associated with several undesired side effects such as radiation caries which is a common, yet serious, complication. To review the condition, the Pubmed database was searched using the keywords “radiotherapy,” “radiation,” “caries,” “hyposalivation,” “prevention” and “management”. Only studies published in the English language were selected. Cross-referencing identified additionally relevant studies. Review Results RT leads to alterations in the dentition, saliva, oral microflora, and diet of patients. Consequently, irradiated patients are at increased risk for the development of a rapid, rampant carious process known as radiation caries. Motivation of patients, adequate plaque control, stimulation of salivary flow, fluoride use, and nutritional orientation are essential to reduce the incidence of radiation caries and ultimately improve the quality of life for HNC patients. Conclusion Radiation caries is an aggressive side effect of RT. Dentists play an important role in the prevention of the condition via comprehensive oral healthcare before, during, and after the active cancer therapy. Clinical Significance Dentists should understand the clinical and biological aspects underlying radiation caries to prevent the development of lesions and provide optimal treatment when needed. To notify a disease is to communicate its occurrence to the sanitary authorities. It can be done by health professionals or by the common citizen, and it triggers the information-decision-action process. Notification is the principal mechanism through which the Ministry of Health becomes aware of the epidemiological data needed for the adoption of the applicable intervention measures. In Brazil, the cases of diseases that might entail isolation or quarantine measures are of compulsory notification, as are those cited on a list of diseases issued by the Ministry of Health and adapted for each state of the country, which is updated periodically. In spite of its importance and compulsory character, notificaton has been precarious, and under-notification has posed a problem for the national health system. The literature on this issue is scarce, being almost exclusively restricted to laws and official directives issued by the Ministry of Health. Thus, a broader publication of the subject is needed in journals of wide circulation, among health professionals, and to the population in general, since common people also play an important role in reporting such diseases. The purpose of this article is to inform the diseases of compulsory notification to health professional, and to reflect on the importance of making future dental professionals aware of their role in a system of epidemiological surveillance. Notificação é a comunicação da ocorrência de determinada doença ou agravo à saúde, feita à autoridade sanitária por profissionais de saúde ou qualquer cidadão, desencadeando o processo de informação-decisão-ação. A Notificação é o principal mecanismo através do qual o Ministério da Saúde recebe os dados epidemiológicos necessários para a adoção de medidas de intervenção cabíveis. No Brasil, são de notificação compulsória os casos de doenças que podem implicar em medidas de isolamento ou quarentenas, e aquelas que constam de uma lista elaborada pelo Ministério da Saúde, adaptada para cada Unidade da Federação e atualizada periodicamente. Apesar da importância e da obrigatoriedade, a notificação tem sido precária, sendo a subnotificação um problema para o sistema de saúde. Existe pouca literatura sobre o assunto, resumindo-se a publicações oficiais como leis, portarias etc. Faz-se necessária uma maior divulgação do assunto em revistas de grande circulação, entre os profissionais de saúde e os futuros profissionais, e até para a população em geral, que é parte importante da informação sobre ocorrência de doenças. Este artigo tem como objetivo divulgar as doenças de notificação compulsória aos profissionais de saúde, e refletir sobre a importância da conscientização do futuro profissional de Odontologia sobre seu papel na manutenção do Sistema de Vigilância Epidemiológica.

The aims of this in vitro study were both to determine the time necessary for removal of carious dentin (efficiency) and the Knoop Hardness Number (KHN) of the remaining dentin (effectiveness), using a chemomechanical method (Carisolv) or hand excavation. Thirty human molars were bisected through occlusal carious lesions into two equal halves. Each half was randomly excavated by hand in circular movements with a spoon excavator or using Carisolv gel according to the manufacturer's instructions. The duration of carious dentin removal was recorded. Tooth sections were resin-embedded, ground flat and polished. Dentin KHN was determined at distances of 100, 200, 300, 400 and 500 mum from the cavity floor. Data were analyzed by Wilcoxon's test (alpha=0.01), ANOVA and Student's t test (alpha= 0.05). The median of the time necessary for chemomechanical excavation was significantly greater than for hand excavation. KHN means (± SD) at 100, 200, 300, 400, 500 µm for chemomechanical method were, respectively: 15.6 (±4.96), 18.0 (±6.22), 21.3 (±9.30), 24.3 (±9.25), 28.5 (±11.80); and for hand excavation were: 21.2 (±10.26), 23.4 (±9.49), 28.2 (±11.62), 31.0 (±12.17), 34.3 (±11.95). It may be concluded that hand excavation presented higher efficiency and effectiveness than chemomechanical excavation. O objetivo deste estudo in vitro foi determinar o tempo gasto para a remoção de dentina cariada (eficiência) e a microdureza Knoop (KHN) da dentina remanescente (eficiência), usando o método químico-mecânico (Carisolv) ou escavação manual. Trinta molares humanos foram seccionados através da lesão de cárie oclusal, produzindo duas metades iguais. Cada metade foi escavada, aleatoriamente, utilizando um escavador manual ou o kit Carisolv gel, seguindo as instruções do fabricante. O tempo gasto para a remoção da dentina cariada foi registrado. As hemi-secções foram incluídas em resina, planificadas e polidas. KHN da dentina remanescente foi determinada nas distâncias de 100, 200, 300, 400 e 500 mim, a partir do assoalho cavitário. Os dados foram analisados pelos testes de Wilcoxon (alfa=0,01), ANOVA e t de Student (alfa=0,05). O tempo gasto na escavação químico-mecânica foi significativamente maior que na escavação manual. As médias e os desvios-padrão de KHN a 100, 200, 300, 400, 500 mim foram, respectivamente, para o método químico-mecânico: 15,6 (±4,96), 18,0 (±6,22), 21,3 (±9,30), 24,3 (±9,25), 28,5 (±11,80); e para a escavação manual: 21,2 (±10,26); 23,4 (±9,49); 28,2 (±11,62); 31,0 (±12,17); 34,3 (±11,95). Concluiu-se que a escavação manual apresentou maior efetividade e eficiência que a escavação assistida pelo Carisolv.

Objective . The aim of this in vitro study was to evaluate the interaction between two sources of fluoride (restorative systems and dentifrices) in inhibiting artificial root caries development. Methods . One hundred and eighty tooth segments were embedded in polyester resin, and sanded flat. Cylindrical cavities 1.0 mm-deep and 1.5 mm-diameter were prepared in root dentin and randomly restored by fluoride-containing restorative systems: Ketac-fil/Espe (Ke), Fuji II LC/GC Corp (Fj), F2000/3M (F2), Surefil/Dentsply (Su) or a control: Filtek Z250/3M (Z2). Ten experimental groups were made to test the association among the five restorative systems and two dentifrices: with F − (Sensodyne Baking Soda) or without F − (Sensodyne Original) ( n =18). After surface polishing, a 1 mm-wide margin around the restorations was demarcated and initial dentin surface Knoop microhardness values (KHN i ) were obtained. The specimens were submitted to a pH-cycling model, and to applications of slurries of dentifrice. Afterwards the final dentin surface Knoop microhardness values (KHN f ) were measured. Results . The differences between KHN i and KHN f , and the covariate KHN i were considered by the ANCOVA and Tukey's test ( α =0.05). The interaction between restorative system and dentifrice was statistically significant ( p =0.0026). All restorative systems provided some protection against artificial caries challenge when associated with the fluoride-containing dentifrice treatment. The means (standard deviation) of reductions in Knoop hardness values for systems associated with the fluoride-containing dentifrice were: Ke: 40.0(1.02) a , Fj: 41.9(1.02) b , F2: 43.3(1.04) c , Su: 43.5(1.00) c , Z2: 44.0(1.02) c ; and with the non-fluoride-containing dentifrice were: Ke: 42.9(1.02) a , Fj: 44.7(1.01) b , F2: 45.2(1.09) bc , Su: 46.0(0.99) c , Z2: 46.6(0.99) c (statistical differences were expressed by different letters). Conclusion . The cariostatic effect shown by the fluoride-containing dentifrice could enhance that shown by Ketac-fil and Fuji II LC, and could mask that shown by F2000.

Introduction: Fixed orthodontic appliances patients suffer limitations on the effective control of biofilm by mechanical methods, bringing the need of a coadjutant in the control of inflammation and oral health improvement.Objective: The aim of this prospective split-mouth blind study was to analyze the effect of a 40% chlorhexidine (CHX) varnish on gingival growth of patients with orthodontic fixed appliances. Methods: Healthy teenage patients with fixed orthodontic appliances and increased gingival volume were recruited (n = 30). Each individual was his own control, having in the maxilla one control side and one treatment side. An application of varnishes occurred on the vestibular area of the upper premolars and first molar crowns, on the control side (placebo varnish) and on the experimental side (EC40(r) Biodentic CHX varnish). The varnishes and sides were randomly chosen and its identification and group was kept by a third party observer and it was not revealed to the researchers and participants until the end of study. In order to establish a baseline registration, digital photographs were taken by a trained photographer before varnish application at baseline (T0), as well as 14 days (T14) and 56 days (T56) after the application. The gingival volume was calculated indirectly using the vestibular areas (mm2) of the upper second premolars' clinical crowns by RapidSketch(r) software, at all study times. The data were analyzed using ANOVA and the Turkey-Krammer test.Results:It was observed, in the final sample of 30 individuals, that at T0, the control and treatment groups were similar. At T14 and T56, a progressive reduction of the clinical crown area was seen in the control group, and an increase in the average area was detected in the experimental group (p < 0,05).Conclusions: The use of 40% CHX varnish decreases the gingival overgrowth in patients undergoing orthodontic treatment. Further studies are necessary to set the action time and frequency of application. Introdução: pacientes com aparelhos ortodônticos fixos sofrem limitações no controle efetivo de biofilme por métodos mecânicos, trazendo a necessidade de um coadjuvante no controle na inflamação e melhora na saúde bucal.Objetivo:esse estudo cruzado prospectivo randomizado teve como objetivo analisar o efeito do verniz de clorexidina (CHX) a 40% no crescimento gengival de pacientes com aparelhos ortodônticos fixos.Métodos:indivíduos adolescentes com aparelhos ortodônticos fixos e aumento de volume gengival foram recrutados para a pesquisa (n = 30). Cada participante atuou como seu próprio controle, tendo, na maxila, um lado controle e um tratamento. No lado controle, aplicou-se verniz placebo e no lado experimental, o verniz EC40(r) Biodentic CHX, ambos na face vestibular das coroas dos pré-molares e primeiro molar superiores. Os vernizes e lados foram escolhidos de forma aleatória e a identificação deles e a que grupo pertenciam foi mantida por um terceiro observador, não sendo revelada aos pesquisadores nem aos participantes até o final do estudo. Fotografias digitais foram tiradas por um fotógrafo treinado, antes da aplicação do verniz no tempo inicial (T0), bem como 14 dias (T14) e 56 dias (T56) após a aplicação. O volume gengival foi calculado indiretamente, por meio das áreas vestibulares (mm2) das coroas dos segundos pré-molares superiores, com o softwareRapidSketch(r), em todos os tempos de estudo. Os dados foram analisados usando ANOVA e teste de Turkey-Krammer.Resultados:na amostra final de 30 indivíduos, observou-se que, em T0, os grupos controle e tratamento foram semelhantes. Já em T14 e T56, foi observada uma progressiva redução na área da coroa clínica no grupo controle, e um aumento na área média do grupo experimental (p< 0,05).Conclusão:o uso do verniz de CHX a 40% diminui o excessivo crescimento gengival em pacientes sob tratamento ortodôntico. Estudos futuros são necessários para se determinar o tempo de ação e a frequência de aplicação.

OBJECTIVE: The aim of this study was to assess the clinical success rate of implant-supported complete dental prostheses, which are being used as a feasible alternative in planning of oral rehabilitation. Over the years, the two-stage surgical protocol has increasingly been replaced by immediate loading procedures, especially in the mandibular anterior region in cases of rehabilitation of completely edentulous patients. This procedure has become feasible due to the high success rate of the technique with very satisfactory results. METHODS: By means of a literature review focused on published articles with clinical follow-up between 8 and 15 years. RESULTS: Implants placed in the mandible had higher success rates than those inserted in the maxilla. Cases with longer follow-up periods, 15 years, showed higher success rates in comparison with follow-up periods of 8 and 10 years. CONCLUSION: Success rates of implant-supported complete dental prostheses are very high. OBJETIVO: Avaliar o índice de sucesso clínico dos implantes pilares de próteses totais implantossuportadas. As próteses implantossuportadas vêm sendo utilizadas como uma alternativa viável ao planejamento de reabilitação oral. O protocolo de dois estágios cirúrgicos, ao passar dos anos, vem sendo cada vez mais substituídos pelos procedimentos de carga imediata, principalmente na região anterior inferior, nos casos de reabilitação de pacientes totalmente desdentados. Tal procedimento torna-se viável devido ao alto índice de sucesso da técnica com resultados bastante satisfatórios. MÉTODOS: Por meio de uma revisão de literatura focada em trabalhos publicados com acompanhamento clínico entre 8 e 15 anos. RESULTADOS: Os implantes instalados em mandíbula apresentaram índices de sucesso maiores que os instalados em maxila. Os casos com acompanhamento mais longos, com 15 anos, apresentaram índices de sucesso maiores do que quando comparados aos acompanhamentos de 8 e 10 anos. CONCLUSÃO: O índice de sucesso dos implantes pilares de prótese total é bastante alto.

Background: Endemic dental fluorosis has already been described in some regions of the world. The aim of this study was to evaluate the functional and psychosocial impact of direct aesthetic restorative treatments in endemic fluorosis patients in the northern state of Minas Gerais, Brazil. Was a quasi-experimental intervention study. Methods: The reference population consisted of individuals between 9 and 27 years of age that were served by a project intended to recover the smiles of patients with severe fluorosis. The questionnaires were administered on two occasions, 24 months apart (before and after dental treatment). Initially, descriptive analyses were conducted. Prevalence and severity, as well as the extent of the functional and psychosocial impact of oral disorders were estimated based on the Oral Health Impact Profile instrument (OHIP-14). Comparisons between baseline and follow-up and between treatment techniques were carried out using the McNemar, Wilcoxon, and Mann–Whitney tests. Results: The study involved 53 individuals, with a mean age of 15.9 years, treated with microabrasion, dental composite, or a combination of both techniques. The treatments performed proved to be competent for reducing the functional and psychosocial impact of oral disorders as measured by the OHIP-14, pointing to the possibility of establishing protocols to be used in programs aimed at restoring the aesthetics and functionality of the anterior teeth in large populations. Conclusions: After performing the direct aesthetic restorative treatments in patients with endemic fluorosis, a significant improvement was observed in the prevalence and severity, as well as the extent of the functional and psychosocial impact of oral disorders.

A hipótese testada neste estudo foi a de que o uso de resina composta/sistema adesivo tem efeito cariostático semelhante a um material adesivo que libera íons flúor - cimento de ionômero de vidro - sobre a superfície radicular adjacente às restaurações. Foram utilizadas 20 raízes de terceiros molares humanos extraídos, embutidas em resina de poliestireno e planificadas. Cavidades padronizadas foram preparadas e restauradas aleatoriamente com (a) Chelon-Fil (Espe) ou (b) Z100/SingleBond (3M). Valores iniciais (KHNi) de microdureza superficial Knoop da dentina foram obtidos a 100, 200 e 300 mim da margem oclusal das restaurações. Uma área de 2,0 mm ao redor da restauração foi delimitada e submetida à indução de cárie artificial. Obtiveram-se, então, os valores finais (KHNf) de microdureza, nas mesmas condições e localizações da leitura inicial. As diferenças entre KHNi e KHNf foram consideradas para a análise estatística. As medianas de KHNi - KHNf nas distâncias de 100, 200 e 300 mim foram para (a): -3,8; -0,3; -1,0; e para (b): 3,3; 2,5; 1,7. O teste de Kruskal-Wallis não evidenciou diferença significativa entre as distâncias dentro de cada grupo. Às distâncias de 200 e 300 mim, não houve diferença significativa entre os materiais avaliados. À distância de 100 mim, (a) diferiu significativamente de (b) (p < 0,05). Sob as condições deste estudo, o cimento de ionômero de vidro apresentou maior potencial cariostático que a resina composta com sistema adesivo dentinário. Dental materials that release fluoride have been shown to be effective in caries inhibition around restorations. Adhesive materials would also be effective in caries inhibition by sealing and protecting cavity margins from acidic demineralization. This in vitro study tested the hypothesis that composite restorations with a dentin adhesive system have a caries preventive effect similar to that of an adhesive material with fluoride - glass-ionomer cement - on root surfaces. Twenty roots from extracted sound third molars were embedded in polystyrene resin and ground flat. Standardized cavities were prepared in leveled root surfaces and randomly restored with (a) Chelon-Fil (Espe) or (b) Z100/SingleBond (3M). Baseline indentations were measured at 100, 200 and 300 mum from the occlusal margins of each restoration and the surface microhardness values were obtained using a Knoop diamond indenter. A 2.0 mm wide margin around the restorations was submitted to a pH-cycling model, at 37ºC. After that, surface microhardness was measured again, as it was before. The differences between baseline and final surface microhardness were considered for statistical analysis. The median values of differences were (a): -3.8; -0.3; -1.0; and (b): 3.3; 2.5; 1.7, for the distances of 100, 200 and 300 mum, respectively. The Kruskal-Wallis test did not show statistically significant difference between 100, 200 and 300 mum distances in each tested group. Objective To compare the caries experience of adolescents and young adults with cleft lip and/or palate (CL/P) with a noncleft control group. Design Thirty CL/P subjects and 30 controls were clinically examined to obtain the decayed, missing and filled teeth (DMFT) and the decayed, missing and filled surfaces (DMFS) indices, gingival bleeding index, plaque index, and active caries lesions. Data concerning oral hygiene, access to fluoridated water, mother's education level, and family income were also collected. Setting Pro-Smile Center, a reference center for the treatment of facial deformities, Alfenas, Minas Gerais, Brazil. Subjects Subjects aged 12 to 21 years with CL/P and without associated syndromes were matched to noncleft controls by sex, age, living habits, and use of orthodontic devices. Null Hypothesis Formulated Prior to Data Collection Caries experience in CL/P adolescents and young adults is similar to that observed in noncleft controls. Statistical Analysis Data were analyzed using SPSS 17.0 software for Windows Data Editor. The CL/P and control groups were compared using the McNemar test, paired t test and Wilcoxon test. A significance level of 5% was adopted for all tests. Results There were no significant differences between the groups for oral hygiene and contact with fluoride. Significant differences were found in per capita income, presence of active caries, decayed surfaces, plaque index, and gingival bleeding. Conclusions The caries experience of CL/P subjects was higher than that of the noncleft individuals.

Este estudo avaliou a atuação de cirurgiões-dentistas (CD) nas instituições de longa permanência de Belo Horizonte (MG) e as práticas de higiene bucal adotadas. Questionários semiestruturados foram entregues aos coordenadores das 37 instituições filantrópicas e trinta privadas e coletados após uma semana. Os resultados foram comparados pelos testes Qui-quadrado e Exato de Fisher (p0,05). Nas instituições filantrópicas com consultório, 67% possuíam CD, e nenhum CD trabalhava onde não havia consultório. Mesmo sem consultório, 13% das instituições privadas possuíam CD. 69,6% das filantrópicas encaminhavam o idoso em caso de necessidade para centros de saúde e, nas privadas, 58,1% direcionavam ao familiar (p=0,00). Maior percentual de instituições privadas adotava medidas sistematizadas de higiene bucal (p=0,01), com grande variabilidade nas condutas relatadas. Há necessidade da incorporação do CD na equipe de saúde das instituições e da sistematização das práticas de higiene bucal. This study evaluated the activities of dentists, dental care and oral hygiene practices in the long-term care institutions of Belo Horizonte (Minas Gerais, Brazil). A semi-structured questionnaire was handed out to the coordinators of 37 philanthropic and 30 private institutions. The data was compared by the chi-square and Fisher's Exact Tests. 81% of the questionnaires were answered. The majority of the private (74.2%) and philanthropic institutions (87%) do not have a dentist (p=0.21). The location, period of existence, type institution kind and number of residents weren't factors regarding the presence of a dentist (p>0.05). 67% of the philanthropic institutions with equipped consultation rooms had dentists, though there were none when there was no consultation room. Even without consultation rooms, 13% of the private institutions had dentists. When necessary, 69.6% of the philanthropic institutions refer the elderly to public health centers, while 58.1% of the private institutions refer them to their family dentists. A higher percentage of the private institutions adopted systematic oral hygiene procedures (p=0.01), with a considerable divergence of treatment reported. There is a need to include a dentist on the health staff in the institutions and for systematization of oral hygiene practices.

The aim of the present study was to validate the dental aesthetic index (DAI) and index of complexity, outcome and need (ICON) based on the opinions of a panel of Brazilian orthodontists. A comparison of these two orthodontic treatment need indices was carried out based on the consensus of a panel of 20 experienced orthodontists. A set of 108 study casts representing the full spectrum of malocclusions was selected. A calibrated examiner scored the casts for both indices. The orthodontists individually rated the casts regarding the degree of orthodontic treatment need. The panel's mean rating of the need for treatment was used as the gold standard for evaluating the validity of the indices. The accuracy of the indices, as reflected in the area under receiver-operating characteristic curves, was high: DAI = 81.83% (95%CI: 71.21-92.44); ICON = 88.75% (95%CI: 78.57-98.92). Although the accuracy of the ICON was higher than that of the DAI, both indices are recommended for determining orthodontic treatment need in Brazil. O objetivo do presente estudo foi validar os índices dental aesthetic index (DAI) e index of complexity, outcome and need (ICON) a partir da opinião de um painel de ortodontistas brasileiros. A comparação desses dois índices de necessidade de tratamento ortodôntico foi feita baseada no consenso de um painel de 20 experientes ortodontistas. Um conjunto de 108 modelos de estudo representando uma grande variedade de tipos de maloclusão foi selecionado. Um examinador calibrado mensurou os modelos para ambos os índices. Os ortodontistas, de forma individual, avaliaram a necessidade de tratamento ortodôntico de cada modelo. O valor médio do painel de ortodontistas em relação à necessidade de tratamento foi utilizado como padrão-ouro para validação dos índices. Os valores de acurácia dos índices, medidos pela curva de característica de operação do receptor, foram altos: DAI = 81,83% (IC95%: 71,21-92,44); ICON = 88,75% (IC95%: 78,57-98,92). Embora a acurácia do ICON tenha sido mais alta do que a do DAI, ambos os índices são recomendados para determinar a necessidade de tratamento ortodôntico no Brasil.

The aim of this study was to evaluate the standard quality of 1,347 root fillings performed by postgraduate students in Endodontics according to 3 radiographic quality parameters. The analyzed quality parameters included apical extension (AE), taper (TA) and homogeneity (HO), which received scores S2 (ideal standard), S1 (slight deviation) or S0 (accentuated deviation). A perfect filling (PF) received S2 for all parameters. In the absence of one or two S2 score, the fillings were deemed as satisfactory (SF) or deficient (DF), respectively. The results showed 51.7%, 41.5% and 6.8% of PF, SF, and DF, respectively. AE, TA, and HO presented equivalent quality parameters in root-filled canals of mandibular incisors and mandibular premolars (p>0.05). Conversely, in maxillary incisors, canines and distal root of mandibular molars, significant differences (p0,05). Active implant periapical lesion (IPL) is a rare lesion which has been reported as one of the causes of dental implant failures. Usually, an affected implant shows radiolucency in the apical area, while remaining clinically stable. IPL is often accompanied by symptoms of pain, swelling, tenderness, and fistulation. In this paper, we describe two cases of IPL with very unusual findings which led to implant failure. A large IPL associated with an inflammatory cyst in the anterior maxilla, and a mandibular IPL resulting in an extra-oral fistula are presented. The etiology and treatment approaches for IPL are discussed.

Esse estudo qualitativo analisou as repercussões da perda dentária total de usuários do serviço público. Aplicou-se o Oral Health Impact Profile simplificado(OHIP14) a cinquenta voluntários e, após a apuração dos resultados, foram selecionados treze pacientes para entrevistas individuais baseadas em um roteiro com questões abertas. A seleção das pessoas foi de acordo com o grau de percepção do impacto da condição bucal na qualidade de vida, medida pela pontuação OHIP, de forma a incluir a maior variedade de perfis da amostra inicial. Da análise do conteúdo dos depoimentos, verificou-se que a perda dentária total tem forte impacto na vida das pessoas e implica consequências negativas como vergonha, dificuldade em se alimentar, prejuízo ao relacionamento social e sentimento de incompletude. A possibilidade de reposição protética dos dentes gera ansiedade nas pessoas e, embora elas saibam que a utilização das próteses possa representar sacrifícios, a expectativa de retorno ao padrão social geralmente aceito e de recuperação de sua própria imagem rejuvenescida faz valer a pena quaisquer sacrifícios. This qualitative research analyses the repercussions of total dental loss on people's quality of life. The Oral Health Impact Profile short-form (OHIP14) was applied to 50 volunteers, and after analyzing the results, 13 out of those 50, were selected to be individually interviewed in an opened questions script. The criteria to select these 13 people were: perception of the impact of mouth condition on quality of life measured by the score reached on OHIP. We tried to comprise the sample with a good variety, according to the profile of the initial sample. After analyzing people's statements, it was possible to verify that dental loss had strong negative consequences on people's life, like shame, difficulty to eat, impact on social relationship and feeling of not being complete. The possibility of having their teeth replaced generates some anxiety. Although being aware that wearing total prosthesis represents some sacrifices, people considered it worthwhile because the possibility of regaining the social pattern and the self-image is renewed.
