Abstract: It is education - the system of formation of the nation's intellectual capital and as one of the main areas of production innovation - creating the basic conditions for intensive growth of the markets on the basis of rapid updating of technologies and products. Education acts as the first link "education - research - innovation development of mass" of the innovation cycle. This educational sphere acts not only as a necessary element of reproduction of intellectual capital, but also as a dominant element of economic growth, which determines the stability of the external and internal competitive advantages of national economic systems. From the power of the national economy play an individual and a public intellectual capital, which implements the level of economic thinking of the nation, it is largely determined by economic strength, well-being, and the choice of its strategy and the subsequent trajectory of development in a global world order. In this connection, the Russian education there are urgent tasks related to the need to comply with the transformation of the education sector changes.

Abstract: In this work we discuss a case of Bullosus pemfigoid (B.P.) developed after pneumonia caused by new coronavirus disease COVID‑19 in a patient with psoriasis. Provided description is supported with the data of histological and serologic analysis, the lgG of epithelial basement membrane was detected to antigen ВР230-CF and to antigen ВР180-NC16A.

3. Expression of CD80 and HLA-DR molecules on blood monocytes in patients with pulmonary tuberculosis

Abstract: We examined expression pattern of CD80 and HLA-DR pro-inflammatory molecules on the monocytes in patients with pulmonary tuberculosis (TB), depending on the clinical form of the disease and susceptibility of the pathogen to anti-tuberculosis drugs. The study involved forty-five patients with newly diagnosed pulmonary TB (25 men and 20 women aged 18 to 55 years, average age — 44.0±12.4 years). The control group included 15 healthy donors with similar socio-demographic characteristics as in TB patients. Venous blood was used as biomaterial for assays. Studies of the monocyte immunophenotype were carried out by flow cytometry of whole blood cells using Cytoflex flow cytometer (Beckman Coulter, USA) with specific monoclonal antibodies (eBioscience, USA). We determined the content of cells expressing surface markers of monocytes, i.e., CD14, CD45, CD80, and HLA-DR. The results of this study were evaluated using SPSS Statistics 17.0 standard software package and Microsoft Excel. In the course of the study, we have suggested a working hypothesis that the monocytes in TB patients, still being in circulation, can express activation markers during their migration to inflammation focus, especially CD80 and HLA-DR molecules. Analysis of the total CD14+ monocyte number showed its decrease in all forms and variants of clinical course of pulmonary tuberculosis compared with the control group. Assessment of pro-inflammatory markers expressed on CD14 positive monocytes, i.e., HLA-DR activation marker and CD80 co-stimulatory molecule, showed that the number of monocytes with HLA-DR expression in all TB patients was higher than in healthy donors. HLA- DR expression on CD14+ monocytes in the group of patients with infiltrative TB proved to be 15% higher than in patients with disseminated TB. The expression of CD80 on CD14+ monocytes in TB patients showed no differences between the groups and varied within the normal range. Hence, an imbalance within monocyte population in patients with pulmonary tuberculosis, regardless of its clinical form and drug sensitivity of the pathogen is developed, due to decrease in total number of CD14+ cells, along with increased relative number of monocytes expressing HLA-DR activation marker (pro-inflammatory phenotype). Meanwhile, expression of the CD80 co-stimulatory molecule on monocytes was within normal values.

Abstract: High blood pressure is a risk factor for cardiovascular morbidity and mortality, as well as cognitive decline and loss of autonomy in the elderly and old age. Randomized clinical trials (RCTs) in populations of older patients living at home with low comorbidity and preserved autonomy indicate the benefit of lowering elevated blood pressure in patients over 80 years of age. Older patients with senile asthenia, loss of autonomy and other geriatric problems were excluded from RCTs, and observational studies in these groups of patients indicate an increase in morbidity and mortality with lower blood pressure and antihypertensive therapy. Obviously, in very elderly patients, a universal strategy for the treatment of arterial hypertension cannot be applied due to the significant heterogeneity of their functional status. The geriatric approach to the management of arterial hypertension in older patients involves an assessment of the functional status, the presence of senile asthenia, and the degree of autonomy for the choice of antihypertensive therapy tactics.

5. Fetal growth and development disorders in smoking pregnant women

Abstract: BACKGROUND: Due to the increased frequency of smoking in pregnant women, an interest in the study of the mechanisms of the fetoplacental unit in women with tobacco addiction has also been increased all over the world. The effect of low degrees of tobacco addiction of a pregnant woman on the fetus has not been studied in the available literature. AIM: The aim of this study was to identify the growth and developmental abnormalities of the fetus at 30-34 weeks of gestation in smoking pregnant women at the third-trimester ultrasound screening. MATERIALS AND METHODS: Pregnant women, who were observed in the Northern Medical Clinical Center named after N.A. Semashko, Arkhangelsk, Russia were examined during the ultrasound screening. A continuous examination of pregnant women with three ultrasound screenings was carried out, with the third screening performed in 1048 individuals. RESULTS: The survey cohort included 120 pregnant women using the inclusion criteria. Two groups were formed depending on the presence or absence of smoking during pregnancy. The first group contained non-smoking pregnant women ( n = 40); the second group comprised smokers during pregnancy ( n = 80). Comparison of fetal development parameters in the group of pregnant smokers was carried out in two subgroups: the second “a” subgroup only consisted of smokers in the first trimester (embryonic period) and the second “b” subgroup contained smokers throughout pregnancy. All pregnant women who took part in the study signed a Patient Informed Consent form. The study design was observational, cross-sectional (one-step). The main manifestations of fetal growth and development disorders at 30-34 weeks of gestation in pregnant smokers were low estimated fetal weight, low tubular bone length and low head circumference by the gestational age. Low (below the 10th percentile) estimated fetal weight by the gestational age was recorded only in the group of pregnant women who smoke ( p = 0.001) and in 90.0% of cases even with a weak degree of tobacco addiction. It was accompanied by low bone sizes and was detected in 10.0% of cases among women who stopped smoking in the first trimester and in 15.0% of cases among those who continued to smoke throughout pregnancy. This result confirmed early symmetrical intrauterine growth restriction of the fetus. Pregnant smokers at 30-34 weeks of gestation had significantly more often low (below the 5 th percentile) fetometric parameters characterizing bone growth: femur length ( p = 0.01), shinbone length ( p = 0.035), shoulder bone length ( p = 0.004), biparietal head size ( p = 0.006), and head circumference ( p = 0.002). Low values of the fetal head circumference were found in 50.0% of cases among pregnant smokers. In the absence of signs of fetal bone growth restriction and the estimated fetal weight in P10-95 values in the group of smoking pregnant women, significantly more often ( p = 0.027) than in non-smokers, low (below the 5 th percentile) head circumference for gestational age was recorded in 29.8% of cases. In addition, in this group of fetuses of pregnant smokers, elevated ratios of abdominal circumference to head circumference were found, which indicated fetal head growth restriction. The fetometry data obtained were confirmed by anthropometric measurements in the newborns during term delivery, the length of full-term newborns in pregnant smokers being significantly lower ( p = 0.040). CONCLUSIONS: Fetuses of pregnant smokers were more likely to have low fetometric parameters by gestational age. Low estimated weights of the fetuses were found in 90.0% of cases with a weak degree of tobacco addiction.

Abstract: In most countries worldwide, hypertension (HTN) occupies a leading position in the structure of modiﬁable risk factors for cardiovascular diseases and their complications. One of the main reasons for this trend is the increasing prevalence of obesity, which is becoming an important risk factor for high blood pressure (BP). The results of numerous epidemiological studies showed an association between obesity (assessed by body mass index (BMI)) and BP levels, as well as a prognostic role of BMI in relation to the development of HTN. However, there is currently no consensus on the obesity criterion that most accurately predicts the risk of HTN and associated complications. Data from prospective observational studies showed a greater prognostic value of indirect and direct indicators of abdominal and ectopic visceral adipose tissue.These facts stimulated research aimed at revealing the pathophysiological mechanisms of HTN in patients with overweight and obesity, which are reviewed in this article.

Abstract: Mantle cell lymphoma (MCL) is a rare and clinically aggressive lymphoma subtype. Current approaches have greatly improved patients outcomes, but relapse is inevitable. In phase IIIII clinical trials, ibrutinib has shown significant activity in patients with relapsed or refractory (R/R) MCL.To assess efficacy and toxicity of ibrutinib monotherapy in patients with R/R MCL in routine practice outside of clinical trials.The study enrolled patients with confirmed R/R MCL who had received at least one line of previous chemotherapy. ECOG 24, cytopenia, infectious complications, hemorrhagic syndrome were not exclusion criteria. Patients received daily oral ibrutinib 560 mg until progression or unacceptable toxicity.From May 2015 to September 2020 ibrutinib therapy was started in 106 patients with R/R MCL in 16 regions of Russia. The median age was 66 years; ECOG2 18%, blastoid variant (or Ki6740% or WBC50109/l) 43%. The median number of previous treatment lines was 2 (111). The ORR was 78.4% (CRR 27.4%). The median PFS was 13.6 months and OS 23.2 months. In the blastoid group the median PFS was 4.4 months vs 36.5 months in the alternative group (p0.001), the median OS 9.0 vs 41.0 (p=0.001). The median OS of patients after progression on ibrutinib was 3.2 months. The common complications are hemorrhages (63%), diarrhea (62%), myalgia and muscle cramps (60%), infections (31%), skin and nail toxicity 15%, arrhythmia 8%. None of recipients had to completely discontinue ibrutinib therapy due to complications.Ibrutinib is effective and well tolerated in routine practice of R/R MCL treatment and our results are consistent with international clinical trials. The favorable toxicity profile and the high response rate made it possible to prescribe ibrutinib in severe somatic status, cytopenia, and even in the presence of infectious complications.Обоснование. Лимфома из клеток мантийной зоны (ЛКМЗ) редкая и агрессивная В-клеточная лимфома. Интенсификация химиотерапии значительно улучшила результаты лечения, но развитие рецидива неизбежно. В исследованиях IIIII фазы ибрутиниб продемонстрировал высокую эффективность у пациентов с рецидивом и резистентным (р/р) течением ЛКМЗ. Цель. Оценить эффективность и токсичность монотерапии ибрутинибом у пациентов с р/р ЛКМЗ в повседневной практике. Материалы и методы. В исследование включены пациенты с подтвержденным диагнозом р/р ЛКМЗ, получившие не менее одной линии химиотерапии. Тяжелый соматический статус, цитопения, инфекционные осложнения и геморрагический синдром не служили противопоказаниями для начала терапии. Ибрутиниб назначался в дозе 560 мг ежедневно до прогрессирования или развития неприемлемой токсичности. Результаты. С мая 2015 по сентябрь 2020 г. включены 106 пациентов в 16 регионах России. Медиана возраста 66 лет; ECOG2 18%; бластоидный вариант (или Ki6740%, или WBC50109/л) 43%. Медиана предшествующих линий терапии 2 (111). Общий ответ достигнут у 78,4% (полная ремиссия 27,4%). Медиана беспрогрессивной выживаемости (БПВ) 13,6 мес, общей выживаемости (ОВ) 23,2 мес. При бластоидном варианте медиана БПВ составила 4,4 мес против 36,5 мес в альтернативной группе (p0,001), медиана ОВ 9,0 мес против 41,0 мес (р=0,001). Медиана ОВ при развитии резистентности к ибрутинибу 3,2 мес. Наиболее частые осложнения: геморрагии (63%), диарея (62%), миалгии и мышечные судороги (60%), инфекции (31%), кожная токсичность 15%, аритмия 8%. Ни одному из пациентов полностью терапию ибрутинибом из-за токсичности не остановили. Заключение. Ибрутиниб эффективен и хорошо переносится в рутинной практике терапии р/р ЛКМЗ, а результаты согласуются с данными международных исследований. Благоприятный профиль токсичности и высокая частота ответов позволили назначать ибрутиниб при тяжелом соматическом статусе, цитопении или наличии инфекционных осложнений.

8. Laparoscopic dissection in children with celiac artery compression syndrome

Abstract: BACKGROUND: A cause of abdominal pain in children may be compression stenosis of the celiac trunk (Dunbars syndrome). This disease occurs when the median arcuate ligament of the diaphragm compresses the celiac trunk, thereby creating compression stenosis, causing the arterial hemodynamics in the artery to suffer and provide adequate blood circulation to the abdominal cavity organs. Medical statistics indicate that 10% to 15% of children and adolescents suffering from chronic abdominal pain have compression stenosis of the celiac trunk. AIM: This study aims to improve the diagnostic results and identify the indications for the surgical treatment of children with compression stenosis of the celiac trunk. MATERIALS AND METHODS: From 2015 to 2020 at the N.F. Filatov Childrens Hospital, 64 patients, aged 4 to 17 years underwent surgical treatment for compression stenosis of the celiac trunk. There are 42 boys (66%) and 22 girls (34%). The leading clinical manifestation in all patients was abdominal pain. Associated surgical pathology was observed in 34 of them. The diagnosis was based on anamnesis, examination, ultrasound examination with Doppler and celiac trunk blood flow measurements, multispiral computed tomography, and angiography. RESULTS: After completing the examination, 61 patients underwent laparoscopic decompression of the celiac trunk, and three children were operated on using the laparotomic approach. In all cases, the leading cause of compression stenosis of the celiac trunk was the median arcuate ligament of the diaphragm combined with the neurofibrous tissue of the celiac plexus. The average duration of the operation was 50 minutes. Intraoperative blood loss did not exceed 530 ml. One conversion completed. There were no postoperative complications in the early postoperative period. The patients were discharged in satisfactory condition. The control examination was conducted in the period from six months to three years. In 97% of patients, clinical symptoms of abdominal ischemia were not detected. CONCLUSION: Our experience indicates the possibility of diagnosing compression stenosis of the celiac trunk in children at early disease stages and laparoscopic treatment success of patients with this disease.

Abstract: Background: The article discusses the criteria, forensic and radiological life tests, difficulties in assessing the results of live births and stillbirths. It also dives into the history of the development of X-ray, including pre-sectional examination of newborn corpses. Two cases of newborn corpses that were subjected to pre-sectional computed tomography (CT) are presented. Case presentation: Case no. 1: Examination of a newborn corpse found in a cardboard box on the unheated terrace of a private house after a secret self-birth. Case no. 2: Examination of the corpse of an infant with massive injuries and the division of the body into two fragments, found on a sorting tape in the premises of a waste sorting shop. Conclusion: Postmortem CT of newborn corpses was an evidence-based and visual addition to the traditional forensic medical study. This helped determine the maturity of fetuses even at the pre-dissection stage to identify injuries and anatomical variants of the structure, to refute the presence of congenital deformities, and to establish and record evidence-based CT signs of live birth and stillbirth.

Abstract: Over the last decade, obesity rates have continued to rise in the United States as well as worldwide and are showing no signs of slowing down. This rise is in parallel with the increasing rates of type 2 diabetes mellitus (T2DM). Given the association between obesity and T2DM and their strong correlation with increased morbidity and mortality in addition to healthcare expenditure, it is important to recognize the most effective ways to combat them. Thus, we performed a review of literature that focused on assessing the outcomes of T2DM following bariatric surgery. Available evidence suggests that bariatric surgery provides better T2DM resolution in obese patients when compared to best medical management alone. Additionally, Biliopancreatic diversion with duodenal switch as well as Roux-en-Y gastric bypass have demonstrated higher rates of T2DM resolution when compared with other bariatric procedures.

11. Resource Change Representation in the Russian Population after the First Three Months of the COVID-19 Pandemic Outbreak

Abstract: The 2019 Coronavirus disease outbreak leads to negative psychological outcomes not only for healthcare workers and patients, but also for the general public. S. Hobfoll’s Conservation of Resources theory is one of the most applicable models for conceptualizing and evaluating natural and social catastrophes and their impact. A web-based screening has been conducted at the end of May, 2020. A representation of resource loss, threat of loss, gain, and value data was collected three months after first patient and restrictions. After meeting an inclusion criterion, data of 1100 respondents have been selected for the analysis. Basing on literature review, four comparison groups were formed: COVID-19 patients (N=65); single male respondents aged from 18 to 34 with low financial income (N=42); female respondents aged from 18 to 34 with low financial income who share living space with more than two persons (N=32); a “resourceful” group of male participants aged from 35 to 54 with high income and ongoing relationships (N=54). Descriptive statistics and Cohen’s d criteria are applied. Measures: social-demographic questionnaire and modified COR-E questionnaire. Resource change due to the pandemic circumstances in general population appears to be modest. Threat of resource loss is higher than real resource loss. The COVID-19 situation mostly impacted following resources: Financial Stability, Feeling of Safety, Sense of Control in Life, Sense of Independence, Intimates’ Health, and Work Stability. Comparison groups show significant differences in resource change constellations consistent with similar data from other countries.

Abstract: Over the last decade, sleeve gastrectomy (SG) has steadily gained popularity and has now become the most commonly performed bariatric procedure in the United States. It is technically less complex than Roux-en-Y gastric bypass (RYGB) or biliopancreatic diversion with duodenal switch and has relatively good weight loss results in addition to comorbidity resolution. The long-term complication profile is appealing to patients and surgeons alike, save one issue, gastroesophageal reflux disease (GERD). GERD is frequently seen in bariatric surgery patients; thus, proper patient selection for SG is paramount. However, SG effects on GERD remain controversial in the literature. Thus, patients are intensively cautioned of the possibility of de novo GERD or worsening of already existing GERD following SG. Therefore, it is imperative that the discussion also occurs regarding potential treatment options if GERD does occur. GERD management following SG consists of a multitude of options. While lifestyle modifications and proton pump inhibitors remain the initial treatment of choice, some will have persistent symptoms needing additional interventions, which may range from a variety of endoscopic techniques (lower esophageal sphincter [LES], radiofrequency ablation, magnetic LES augmentation, and antireflux mucosectomy) to ReSleeve and ultimately conversion to RYGB.

Abstract: The work represents the prospective cohort analysis of patients with psoriasis treated with certolizumab pegol (CZP) at the Tula Regional Clinical Dermatovenerologic Dispensary in years 2017–2019, who achieved remission and discontinued CZP and real-time observation of the patients. The patients remained in sustained remission after discontinuation of certolizumab pegol (mean drug-free remission was 42 weeks). Patients who had not responded to systemic therapy prior to CZP treatment demonstrated good response to methotrexate and cyclosporin A, which suggests the modulation of immune response by certolizumab pegol. The obtained results demonstrate that intermittent treatment with certolizumab pegol effiiently controls psoriasis, reduces drug burden.

Abstract: To configure the control systems of the ship movement along the trajectory, it is necessary to be concerned with the parameters of its controllability. There has been proposed building a matrix model based on measurements of the state vector of a controlled object. The construction of the model is considered on the example of the problem of ship course control. An algorithm for determining the matrix coefficients of the selected model is proposed. The operation of the considered algorithm has been checked for square matrices by finding their inverse matrices, as well as for rectangular matrices for which the pseudo inverse matrix was found. The illustration of the proposed approach is carried out using the example of a simple 1-order linear Nomoto model. The considered approach is quite universal and can be applied to higher order models, including nonlinear ones.

Abstract: Mechanical asphyxia is one of the leading causes of violent death, which determines the improvement of the evidence base for this type of death. At forensic examination, the morphological picture of asphyxia doesn’t differ from sudden death, as well as with other types and mechanisms sudden (acute) death, which makes it difficult to determine the immediate cause of death. The article discusses to use case of computed tomography (CT) for visualize formation, which found on the corpse (injury of bones, stab wounds, determination of the level of location of a foreign object in the oropharynx child) with the establishment of the immediate cause of death. Posthumous CT examination made it possible to assess the state of "hard to reach" for the classic study of corpse zones, confirm the direct cause of death, identify the presence of gas in the abdominal cavity and small pelvis that can’t be established during sectional examination of a corpse

Abstract: Background. The article presents a case study of the local forensic Department GBUZ MO “Bureau of forensic medical examination” in which the investigation of the corpse of a girl who died from drowning utilized the method of pre-sectional computed tomography (virtual autopsy), with subsequent radiological-anatomical comparison of the results. Aim. Identification of possibilities of computed tomography for establishing diagnostically significant signs of drowning; pre-sectional identification/exclusion of injuries. Material and methods. Computed tomography was performed in the Radiology Department of the district hospital at MSCT Optima of General Electric (64-slice tomograph, with 1.5-mm- and 2-mm-thick sections). Results. When studying the Russian medical literature, there was no description of the presence of fluid in the maxillary and frontal sinuses of corpses extracted from water. Conclusions. The presence of fluid in the maxillary and frontal sinuses revealed during CT examination allowed us to supplement the number of objects for the algological study. The discovery of plankton and quartz-containing particles in the fluid from the paranasal sinuses expanded the evidence base for death by drowning.

Abstract: The aim of the study was to identify general patterns and features of changes in the content of hypoxia-inducible factors-1 and -2 in association with an imbalance of cytokines (IL-10, IL-13, galectin-2 and -9, IFN-gamma) in the blood in diseases associated with hypoxia. Materials and methods. We examined 25 patients with coronary heart disease (CHD) with heart failure II-III according to NYHA, 16 patients with chronic obstructive pulmonary disease (COPD) without exacerbation, 16 patients with infiltrative pulmonary tuberculosis (TB) before anti-TB therapy, and 18 relatively healthy donors. Plasma concentrations of HIF-1alpha, HIF-2alpha, IL-10, IL-13, galectins-2 and -9, and IFN-gamma were determined by enzyme-linked immunosorbent assay (ELISA). Results. Positive outcomes of quantity determination of HIF-2alpha in the blood (24.00 ± 8.54 %, 75.00 ± 10.83%, 43.75 ± 12.40% of patients, respectively, against «zero» values in healthy donors) and also signs of immunosuppression at normal plasma concentrations of HIF-1alpha were determined in diseases associated with chronic hypoxia (in patients with CHD, COPD, TB). Immunological insufficiency in CHD and TB is caused by a deficiency of IFN-gamma and galectin-2 in association with an excess of galectin-9 (in patients with CHD 1.10 [0.52; 2.60] pg/ml p = 0.038) or IL-13 (in patients with TB 0.81 [0.79; 1.40] pg/ml, p = 0.043), and in patients with COPD it is caused by a surplus of galectin-9 and IL-13 (8.50 [3.96; 15.00] pg/ml, p = 0.001 and 2.62 [1.20; 7.58] pg/ml, p = 0.002, respectively) at normal concentrations of IFN-gamma and galectin-2. The content of IL-10 in the blood tends to increase in CHD and COPD. Conclusion. In patients with CHD, COPD and TB, chronic hypoxia is associated with immunosuppression mediated by an imbalance of IL-10, IL-13, IFN-gamma, galectins (2 and 9) in the blood and the secretion of HIF-2alpha, which has the property to stimulate the differentiation of M2-macrophages synthesizing anti-inflammatory cytokines.

Abstract: BACKGROUND. Determining the pathogenetic mechanisms of small-caliber renal artery remodeling in chronic glomerulonephritis (GN) is an urgent task of nephrology, the implementation of which will allow establishing new diagnostic and therapeutic approaches to patients management. THE AIM: To evaluate the influence of hemodynamic, tubulointerstitial, and endothe- liotropic risk factors on the probability of remodeling of interlobular arteries in an experimental model of glomerulonephritis. MATERIALS AND METHODS. The experiment included 45 individuals of white mongrel rats, 3 of which were used to prepare an antigen suspension (AS), and 42 rats were divided into 4 groups: 9 individuals in the main groups (with the introduction of only AS, with the introduction of an incomplete Freund adjuvant, with AS and sodium chloride, with AS and perindopril) and 6 ratsin the control group. Glomerulonephritis was formed in the main groups of the experiment. Systolic blood pressure (SAD), the protein level in the urine, and the presence of edematous syndrome were monitored initially, on the 15th, 30th, and 60th day of the experiment. The size of the interlobular artery (MA) was determined by the morphological study, and the expression of VEGF and TGFp in the kidneys. RESULTS. Morphological signs of glomerulonephritis were obtained in all the main groups of the experiment as early as day 15. The greatest increase in SAD, protein in the urine, the presence of edematous syndrome in groups with the introduction of AS and AS with sodium chloride was found. The highest expression of VEGF and TGFp was found in these groups of rats. In the group with AS with perindopril, normotension was formed, the protein level was lower than in rats with AS with or without the use of sodium chloride, and there was no edematous syndrome. The expression of VEGF and TGFp was minimal. Interlobular artery remodeling in established groups of AS an AC with sodium chloride. In the remaining groups of rats, the size of the interlobular arteries was comparable to the control group. CONCLUSION. The leading role of systemic blood pressure in the remodeling of small-diameter kidney arteries in glomerulonephritis has been established. Despite the presence of active glomerulonephritis in rats, the structure of small arteries does not change during the formation of normotension.

Abstract: Studies in the area of chemistry teaching discovered that problem-based learning strategies (PBL) effectively improve students’ conceptual understanding. However, there is minimal information about the effectiveness of PBL if it is applied with multiple representations (MR) for the students with different learning preferences. This study aimed to prove the effects of problem-based learning strategies (PBL) using multiple representations (MR) and students’ learning styles on the conceptual understanding in chemistry. There were two whole classes in the Mechanical Engineering Department of Bali State Polytechnic that were assigned to be the experimental group (n = 59) and control group (n = 58). Felder-Soloman's Index of Learning Style (ILS) was applied to differentiate the students’ learning preferences, while their conceptual understanding in chemistry was assessed using a test after 6 weeks of treatment. The analysis of covariance (ANCOVA) shows that the conceptual understanding of students taught using PBL strategies with MR was significantly better than those taught using PBL strategies only. Besides, based on the variation of learning styles, students with a visual style had a better conceptual understanding than those with verbal style, but the difference was not statistically significant. The absence of interaction effect between PBL strategies with MR and learning style on students’ conceptual understanding suggests that the support of multiple representations in PBL strategies can effectively enhance students' understanding of concepts in chemistry regardless of their learning style.

Abstract: Postmortem computer tomography (CT) came into practice of forensic medicine in the 1990s and has later been complemented with magnetic resonance imaging (MRI). A pioneer virtual autopsy was conducted in Germany in 1983. In the Czech Republic, this examination was first performed in 1993.A typical examination requires about 30 min, with the most resource-demanding stage being the image data rendering. CT was shown to better capture skeletal structures, while MRI contrasting is superior in terms of visualising soft tissues. In the Czech Republic, CT-based virtopsy is legislated mandatory to document deaths inflicted by gunshots, road traffic and aviation accidents, high falls, occupational and explosive-related injuries, thermal and mechanical traumas, strangulation, drowning as well as to examine unidentified or decomposing bodies, deceased children and adolescents aged under 18.CT scanning prior to conventional autopsy provides a forensic expert with guidance to reveal pathologies non-invasively in particular regions that are difficult to dissect or access. The advantage of virtopsy is the objective acquisition of data that can be re-examined, reinterpreted or juxtaposed with the results of conventional autopsy and easily recovered for possible further expertise.

Abstract: The Ministry of Health of the Russian Federation jointly with professional association and experts in the field of pediatrics, infectious diseases and resuscitation has developed guidelines “Clinical Features and Management of the Disease Caused by New Coronaviral Infection (COVID-19) in Children” in order to provide the child population with effective medical care during the pandemic of the new coronaviral infection. The practical experience of specialists from various countries was considered during the development of this document. Special attention should be given to the evidence base of the presented data, as well as to the efficiency and safety issues of medications used in treatment of coronaviral infection and its complications. The authors highlight the problems of prevention, diagnostics and management of pathological conditions caused by COVID-19 in the article according to the presented guidelines. Patient’s management is presented depending on the age and severity of the disease itself. The therapy is considered with regard to etiological, pathogenetic and symptom focus.

Abstract: The multi-modular transformer watercrafts based on modules with vortex engines might represent a new class of watercrafts significantly expanding the range of achievable parameters in production processes carried out in the World Ocean. Composed of modules of the same type and capable of functioning autonomously with vortex propulsion devices, they can, firstly, be assembled to sizes and shapes that are not accessible to modern vessels of traditional design, and secondly, if necessary, they will not have limitations in terms of independent operation, so their properties are similar to artificial islands in any given water area of the Global Ocean, while possessing mobility.The objective of this paper is to assess the prospects for the use of multi-modular transformer watercrafts as carriers of technological systems for scientific and production processes carried out in the World Ocean.Models of application of multi-modular transformer watercrafts with vortex propulsion are suggested for their autonomous year-round continuous operation in given non-freezing waters. To assess the effectiveness of the use of the proposed transformer watercrafts, a simulation of their operation was carried out, as well as a direct comparison of their specifications with traditional vessels that support operation of scientific and production complexes in the World Ocean. For comparison, the currently known marine complexes are selected that require long-term continuous operation (fish canning floating base; a floating complex for extraction and production of liquefied natural gas).The paper shows that, in relation to all the considered processes, the proposed multi-modular transformer watercrafts have at least two advantages. Abstract: The article discusses the application of pre-autopsy computed tomography (Virtopsy) for studying the sudden death of an adolescent with undiagnosed Marfan syndrome. Aim . To identify the capabilities of the pre-autopsy computed tomography (CT) in determining the cause of the sudden death of an adolescent, as well as the extent of injuries, if present. Material and methods . A CT scan was obtained at the Department of Diagnostic Radiology of a district hospital using a modern multifunctional CT scanner Siemens SOMATOM Perspective (64-slice configuration, slice width of 1.5 mm). Results . The forensic medical examination revealed the following information about the corpse: asthenic body type, a height of 178 cm, long extremities, thin and long spider-like fingers, pectus excavatum, little fat beneath the skin and poor muscle development; blood in the pericardium, rupture of the dissecting aneurysm of the ascending aorta. The histological examination of the aorta revealed its dramatic thinning, fragmentation of the elastic fibres along with the alteration of the elastic framework, fragmentation and poor development of muscle fibres in the media. A CT scan showed the rupture of the dissecting aneurysm of the ascending thoracic aorta with para-aortic haematoma and cardiac tamponade. Conclusion . The analysis of obtained data allowed us to diagnose a multisystemic disorder of connective tissue (Marfan syndrome) undiagnosed intra vitam. The sudden death was caused by the rupture of the dissecting aneurysm of the ascending aorta which is a complication of the congenital genetic pathology. Intravitam CT or MRI scans of children being at risk of the multisystemic disorder of connective tissue will help detect pathological changes in the aortic wall and perform cardiac surgery promptly which would significantly increase the length of life and improve its quality for such patients.

Abstract: The research deals with styles of business communication, which appears to be a topical issue nowadays. Its topicality correlates with the current needs and trends in the sphere of professional activity: in personnel selection the focus is on the ability to solve specific problems, not just on the profession; orientation is not towards an individual person but towards joint activity in a unique environment; there is an emphasis on complex assessment and employment of the resources, not of separate qualities of a person, etc. The purpose of the present study is a comparative analysis of business communication styles in the samples of penal colony employees, hospital nurses and managers. The subject of research is business communication styles of persons engaged in socionomic professions. Hypotheses of the study are as follows: 1) Styles of business communication are determined by the conditions of the environment; 2) In each particular environment, there are assumed to be some style niches, within the boundaries of which a person chooses certain patterns of behavior. Research methods: expert survey, psychodiagnostics (test questionnaires «Coping behavior in stressful situations» and «Questionnaire for ways of coping» by T. Kryukova, test questionnaires 16–PF by R.B. Cattell, questionnaire «Level of subjective control»); research technique — questionnaire «Styles of business communication» by V.A. Tolochek. In this article, we consider the results of t-comparison and correlation analysis obtained with the help of the research questionnaire. The results confirm the working hypotheses.

Abstract: Recently the role of herpes viruses in an aggravation of inflammatory diseases of the upper respiratory tract, in particular, herpes simplex virus and Epstein − Barr virus, has become increasingly evident in otorhynolaryngology practice. To determine the extent of infection with Epstein − Barr virus and to study the immunogram of the first level for the children with secretory otitis media, 48 patients aged 3−9 years were examined for the purpose of an adequate treatment. Infection was revealed by serological diagnosis (enzyme immunoassay) with the determination of IgM to capsid complex (VCA) and IgG to early antigen (EA). Level 1 immunograms were also determined by immune enzyme analysis. Children with secretive middle otitis (22.9 %) were infected with Epstein − Barr virus, corresponding to an acute phase of the disease, as well as they had a reduce cell immunity. All children received comprehensive treatment for secretory middle otitis. It was concluded about the need for children with middle otitis to be screened for an infection with the Epstein−Barr virus and treated conservatively by an immunologist. Abstract: The article is dedicated to a rare case of spontaneous rupture of the pathologically altered left common iliac vein which was a late complication of the cava filter placement. To date this complication has not been described in the national medical literature. In foreign literature only 21 cases of spontaneous rupture of the iliac vein with unclear cause were registered. The article presents a case observed in the practice of the Bureau of forensic medical examination of the Moscow region. It was found that on he background of a number of conditions (occlusion of the inferior vena cava; pathological changes in the venous wall, taking into account its anatomical location; immobilized state of the patient; lack of adequate anticoagulant therapy; predisposing factors in the form of muscle tension) spontaneous rupture of the left common iliac vein should be considered as one of the late cava filter placement complications.Conclusion. Thus, the authors of the article have established the causes of spontaneous rupture of the iliac vein, as well as proposed the inclusion of this pathology in the official list of the late cava filter placement complications.

Abstract: Background: Gastric and colon tumors are often associated with eosinophilic infiltration of tumor tissue, the significance of which is still not entirely clear. The recruitment of eosinophils into the tissues can be in part regulated by galectins ― galactose-binding proteins which are expressed by a variety of tissues and are capable of exerting a broad range of effects. Aims: To evaluate the expression of galectin-1 and galectin-3 in tumor tissue, and gal-3 gene mRNA expression in blood eosinophils in patients with gastric and colon cancer with or without tissue eosinophilia. Materials and methods: The study included a total of 107 patients (84 males and 23 females, average age 60,9 6,8) with verified gastric cancer (52 persons) and colon cancer (55 persons), who underwent treatment or were registered at the dispensary at the regional medical institution Tomsk Regional Oncology Center (Tomsk, Russia). The control group consisted of 15 men and 11 women of comparable age. The materials of the research included samples of gastric and colon tumors obtained during surgery, and eosinophilic granulocytes isolated from whole blood by immunomagnetic separation. Galectin-1 and galectin-3 expression in tumor tissue was evaluated by immunohistochemistry. The expression of gal-3 gene mRNA in eosinophils was determined by real-time reverse transcription polymerase chain reaction. Statistical analysis of the results was carried out using the non-parametric Mann-Whitney U test for independent samples with Benjamini-Hochberg procedure for multiple comparisons, and the Chi-square Pearson criterion with Yates correction. Results: In patients with gastric cancer and colon cancer, regardless of the presence of tissue eosinophilia, low expression of galectin-3 in the tumor tissue and high expression of gal-3 gene mRNA in peripheral blood eosinophils were found. Gastric and colon cancer patients with eosinophilic infiltration of tumor tissue were characterized by low expression of galectin-1 within tumor cells (in 64.0% cases, 2 = 4.890, р = 0.029; and in 73.9% cases, 2 = 5.981, p = 0.031 respectively). There was a statistically significant connection between the level of galectin-1 expression by tumor cells and the presence of tissue eosinophilia both in gastric ( = 0.307) and colon cancer ( = 0.330). Conclusion: Low expression of galectin 1 and 3 by tumor cells in gastric and colon cancer with tissue eosinophilia indicates the lack of a significant effect of these proteins on the process of recruiting eosinophilic granulocytes into tumor tissue. Increased expression of galectin-3 in blood eosinophils in gastric and colon cancer is not associated with the presence of eosinophilic infiltration of tumor tissue.

Aim: The SAMe-TT2R2 score helps identify patients with atrial fibrillation (AF) likely to have poor anticoagulation control during anticoagulation with vitamin K antagonists (VKA) and those with scores >2 might be better managed with a target-specific oral anticoagulant (NOAC). We hypothesized that in clinical practice, VKAs may be prescribed less frequently to patients with AF and SAMe-TT2R2 scores >2 than to patients with lower scores. Methods and results: We analyzed the Phase III dataset of the Global Registry on Long-Term Oral Antithrombotic Treatment in Patients with Atrial Fibrillation (GLORIA-AF), a large, global, prospective global registry of patients with newly diagnosed AF and ≥1 stroke risk factor. We compared baseline clinical characteristics and antithrombotic prescriptions to determine the probability of the VKA prescription among anticoagulated patients with the baseline SAMe-TT2R2 score >2 and ≤ 2. Among 17,465 anticoagulated patients with AF, 4,828 (27.6%) patients were prescribed VKA and 12,637 (72.4%) patients an NOAC: 11,884 (68.0%) patients had SAMe-TT2R2 scores 0-2 and 5,581 (32.0%) patients had scores >2. The proportion of patients prescribed VKA was 28.0% among patients with SAMe-TT2R2 scores >2 and 27.5% in those with scores ≤2. Conclusions: The lack of a clear association between the SAMe-TT2R2 score and anticoagulant selection may be attributed to the relative efficacy and safety profiles between NOACs and VKAs as well as to the absence of trial evidence that an SAMe-TT2R2-guided strategy for the selection of the type of anticoagulation in NVAF patients has an impact on clinical outcomes of efficacy and safety. The latter hypothesis is currently being tested in a randomized controlled trial. 40. Features of changes in the level of fibrosis markers in lipotoxic myocardial damage in patients with epicardial obesity

Abstract: In visceral obesity, lipotoxic myocardial damage develops, which leads to the formation of myocardial fibrosis with impaired diastolic function and the development of heart failure (HF). Currently, the search for markers of early diagnosis of left ventricular (LV) diastolic dysfunction (DD) is relevant. Objective To study the features of changes in the level of fibrosis markers in lipotoxic myocardial damage in patients with epicardial obesity (EO). Materials and methods The study included 125 men with general obesity (BMI average 33.24±3.65 kg / sq. m.). All patients underwent transthoracic echocardiography (ECG) to assess the thickness of epicardial adipose tissue (tEAT) as the equivalent of visceral obesity, as well as to diagnose DD LV. According to the results of echocardiography, the patients were divided into 2 groups: EO(+) with epicardial fat thickness (tEAT) ≥7 mm (n=78); EO(−) with tEAT Results When studying the features of changes in the level of serum markers of myocardial fibrosis, a statistically significant increase in the level of all the studied markers was revealed in the EO(+) group compared to the EO(−) group, and a statistically significant increase in the level of FFA in the EO(+) group compared to the EO(−) group (0.84±0.02 mmol/l and 0.35±0.01 mmol/L, respectively, p=0.02). A statistically significant relationship was found between the levels of FFA and collagen III (r=0.32, p=0.03) and TGF – β (r=0.30, p=0.04). According to the results of speckle-tracking ECG in the EO(+) group, an increase in the rate of LV untwist to −125.56 (−141.0; −117.0) deg/s (p=0.003) was determined. When studying the effect of FFA as a marker of lipotoxic effect on the myocardium, a weak statistically significant relationship was revealed between the level of FFA and the rate of LV untwist (r=0.27; p=0.03) in the EO(+) group, while no such effect was found in the EO(−) group. Conclusion In EO, as a result of neurohumoral disorders, lipotoxic myocardial damage develops and progresses, leading to LV DD, the detection of which is probably possible with the help of the LV unwinding rate. Abstract: Currently, there is no single non-invasive marker that can directly assess left ventricular (LV) diastolic function. Untwist of LV contributes to diastolic suction and early filling. Speckle-tracking imaging can be used to diagnose diastolic dysfunction at the preclinical stage. Objective To study the features of changes in the parameters of LV mechanics and their relationship with the levels of serum markers of myocardial fibrosis in patients with epicardial obesity (EO). Materials and methods The study included 125 men with general obesity. All patients underwent transthoracic echocardiography (ECG) to assess the thickness of epicardial adipose tissue (tEAT) as equivalent to visceral obesity, as well as to diagnose diastolic dysfunction (DD) of left ventricle (LV). According to the results of ECG, the patients were divided into 2 groups: EO(+) with epicardial fat thickness (tEAT) ≥7 mm (n=78); EO(−) with (tEAT) Profibrotic markers in blood serum (MMP-3, collagen I, collagen III, TGF-β, VEGFA, PICP) were determined in all patients using enzyme immunoassay. Using speckle-tracking ECG, the mechanics of LV were studied (twist LV, peak twist ratio LV, time to peak twist of LV, peak untwist ratio LV, time to peak untwist of LV). The exclusion criteria were the presence of coronary pathology, arterial hypertension, and type 2 diabetes mellitus. Results In the group of patients with EO(+) statistically significant increase the level of all studied profibrotic markers was revealed. According to the results of speckle-tracking ECG in the EO(+) group an increase peak untwist ratio LV to −125.56 (−141.0; −117.0)deg/s (p=0.003) and an increase time to peak untwist of LV of 469.44 (509.0; 401.0) msec was determined in comparison with the EO(−) group (p=0.03). A weak statistically significant effect of tEAT on the peak untwist ratio LV in EO(+) group was revealed (r=0.28; p=0.03). In addition, there was a significant relationship between peak untwist ratio LV and markers of myocardial fibrosis MMP-3 (r=0.23; p=0.03) and collagen type III (r=0.25; p=0.04). Conclusion Thus, LV unwinding may be a new non-invasive marker of LV DD at the preclinical stage, since this parameter reflects the mechanical aspect of global diastolic function, especially the early phase of diastole. Abstract: Aims The aim of this study was to determine the frequency of heterozygous truncating ALPK3 variants (ALPK3tv) in patients with hypertrophic cardiomyopathy (HCM) and confirm their pathogenicity using burden testing in independent cohorts and family co-segregation studies. Methods and results In a discovery cohort of 770 index patients with HCM, 12 (1.56%) were heterozygous for ALPK3tv [odds ratio(OR) 16.11, 95% confidence interval (CI) 7.94–30.02, P = 8.05e−11] compared to the Genome Aggregation Database (gnomAD) population. Abstract: Digital, Background Guidelines recommend nonstatin lipid-lowering agents in patients at very high risk for major adverse cardiovascular events (MACE) if low-density lipoprotein cholesterol (LDL-C) remains ≥70 mg/dL on maximum tolerated statin treatment. It is uncertain if this approach benefits patients with LDL-C near 70 mg/dL. Lipoprotein(a) levels may influence residual risk. Objectives In a post hoc analysis of the ODYSSEY Outcomes (Evaluation of Cardiovascular Outcomes After an Acute Coronary Syndrome During Treatment With Alirocumab) trial, the authors evaluated the benefit of adding the proprotein subtilisin/kexin type 9 inhibitor alirocumab to optimized statin treatment in patients with LDL-C levels near 70 mg/dL. Effects were evaluated according to concurrent lipoprotein(a) levels. Methods ODYSSEY Outcomes compared alirocumab with placebo in 18,924 patients with recent acute coronary syndromes receiving optimized statin treatment. In 4,351 patients (23.0%), screening or randomization LDL-C was 13.7 mg/dL or ≤13.7 mg/dL; corresponding adjusted treatment hazard ratios were 0.82 (95% CI: 0.72-0.92) and 0.89 (95% CI: 0.75-1.06), with Pinteraction = 0.43. Conclusions In patients with recent acute coronary syndromes and LDL-C near 70 mg/dL on optimized statin therapy, proprotein subtilisin/kexin type 9 inhibition provides incremental clinical benefit only when lipoprotein(a) concentration is at least mildly elevated. Abstract: Summary Background Etrolizumab is a gut-targeted anti-β7 integrin monoclonal antibody. In an earlier phase 2 induction study, etrolizumab significantly improved clinical remission relative to placebo in patients with moderately to severely active ulcerative colitis. The HIBISCUS studies aimed to compare the efficacy and safety of etrolizumab to adalimumab and placebo for induction of remission in patients with moderately to severely active ulcerative colitis. Methods HIBISCUS I and HIBISCUS II were identically designed, multicentre, phase 3, randomised, double-blind, placebo-controlled and active-controlled studies of etrolizumab, adalimumab, and placebo in adult (18–80 years) patients with moderately to severely active ulcerative colitis (Mayo Clinic total score [MCS] of 6–12 with an endoscopic subscore of ≥2, a rectal bleeding subscore of ≥1, and a stool frequency subscore of ≥1) who were naive to tumour necrosis factor inhibitors. All patients had an established diagnosis of ulcerative colitis for at least 3 months, corroborated by both clinical and endoscopic evidence, and evidence of disease extending at least 20 cm from the anal verge. In both studies, patients were randomly assigned (2:2:1) to receive subcutaneous etrolizumab 105 mg once every 4 weeks; subcutaneous adalimumab 160 mg on day 1, 80 mg at week 2, and 40 mg at weeks 4, 6, and 8; or placebo. Randomisation was stratified by baseline concomitant treatment with corticosteroids, concomitant treatment with immunosuppressants, and baseline disease activity. All patients and study site personnel were masked to treatment assignment. The primary endpoint was induction of remission at week 10 (defined as MCS of 2 or lower, with individual subscores of 1 or lower, and rectal bleeding subscore of 0) with etrolizumab compared with placebo. Pooled analyses of both studies comparing etrolizumab and adalimumab were examined for several clinical and endoscopic endpoints. Efficacy was analysed using a modified intent-to-treat population, defined as all randomly assigned patients who received at least one dose of study drug. These trials are registered with ClinicalTrials.gov , NCT02163759 (HIBISCUS I), NCT02171429 (HIBISCUS II). Findings Between Nov 4, 2014, and May 25, 2020, each study screened 652 patients (HIBISCUS I) and 613 patients (HIBISCUS II). Each study enrolled and randomly assigned 358 patients (HIBISCUS I etrolizumab n=144, adalimumab n=142, placebo n=72; HIBISCUS II etrolizumab n=143; adalimumab n=143; placebo n=72). In HIBISCUS I, 28 (19·4%) of 144 patients in the etrolizumab group and five (6·9%) of 72 patients in the placebo group were in remission at week 10, with an adjusted treatment difference of 12·3% (95% CI 1·6 to 20·6; p=0·017) in favour of etrolizumab. In HIBISCUS II, 26 (18·2%) of 143 patients in the etrolizumab group and eight (11·1%) of 72 patients in the placebo group were in remission at week 10, with an adjusted treatment difference of 7·2% (95% CI –3·8 to 16·1; p=0·17). In the pooled analysis, etrolizumab was not superior to adalimumab for induction of remission, endoscopic improvement, clinical response, histological remission, or endoscopic remission; however, similar numerical results were observed in both groups. In HIBISCUS I, 50 (35%) of 144 patients in the etrolizumab group reported any adverse event, compared with 61 (43%) of 142 in the adalimumab group and 26 (36%) of 72 in the placebo group. In HIBISCUS II, 63 (44%) of 143 patients in the etrolizumab group reported any adverse event, as did 62 (43%) of 143 in the adalimumab group and 33 (46%) in the placebo group. The most common adverse event in all groups was ulcerative colitis flare. The incidence of serious adverse events in the pooled patient population was similar for etrolizumab (15 [5%] of 287) and placebo (seven [5%] of 144) and lower for adalimumab (six [2%] of 285). Two patients in the etrolizumab group died; neither death was deemed to be treatment related. Interpretation Etrolizumab was superior to placebo for induction of remission in HIBISCUS I, but not in HIBISCUS II. Etrolizumab was well tolerated in both studies. Funding F Hoffmann-La Roche.

Abstract: Myocardial fibrosis is considered a key pathological process in the development of cardiovascular diseases. In epicardial obesity (EO), the main cause of fibrosis development is lipotoxic myocardial damage. It is important to detect myocardial fibrosis at an early stage, using non-invasive diagnostic methods. According to the results of echocardiography (ECG), 110 men with general obesity were divided into the following two groups: Group I with epicardial fat thickness (tEAT) ≥ 7 mm (n = 70) and Group II with tEAT < 7 mm (n = 40) without diastolic dysfunction. The levels of metabolic factors, pro-inflammatory cytokines, adipokines, and free fatty acids (FFA), profibrotic markers were determined in both groups. In Group I, the level of interleukin (IL)-6, C-reactive protein, and tumor necrosis factor (TNF)-α increased and that of leptin and adiponectin decreased compared with those in Group II. There was an increase in the level of all studied profibrotic factors in Group I. The level of TNF-α and IL-6 showed a positive correlation with the level of leptin and FFA and a negative correlation with the level of adiponectin. We also observed a relationship between the level of collagen, transforming growth factor (TGF)-β, and metalloproteinase (MMP)-3 and EO. Our results showed that confirmed EO correlates with not only disadipocytosis and increased levels of pro-inflammatory cytokines, but also increased levels of profibrotic factors. This suggests that the studied markers of fibrosis may be used to determine preclinical cardiac fibrosis with lipotoxic myocardial damage in patients with EO.

Abstract: The article represents the results of a prospective analysis of 28 patients with severe plaque psoriasis and advanced psoriatic arthritis with insufficient response to previous systemic therapy, treated with certolizumab pegol at the Tula Regional Clinical Dermatovenerologic Dispensary in years 2017–2019. Certolizumab pegol demonstrated high and sustained efficacy in improving skin disease and manifestations of psoriatic arthritis. Safety profile of certolizumab pegol was consistent with the therapeutic class.

Abstract: The article presents the cases observed in the practice of the Office of medico-legal examinations of the Moscow Region when computed tomography (CT) was used for the first time in Russia for pre-autopsy examination (virtual autopsy) of three corpses with gunshot wounds followed by imaging-anatomical comparison of the results.Objectives. Determination of the injury volume, visualization of the wound tracts, localization of the bullets.Material and methods. Computed tomography was performed in radiology department using CT Scanner Hitachi Eclos‑16 (16 slices per rotation, slice thickness 1.5 and 2.0 mm) followed by multi-planar reconstruction of the images.Results. 3D reconstruction of the CT scans of the corpses has visualized the whole volume of the trauma. Firearm perforating skull fractures, crushing injuries of the brain, injuries of the thoracic and abdominal organs along the wound tracks, bullets at the ends of the blind wound tracks were revealed on CT-scans.Conclusion. Pre-autopsy CT with 3D-reconstruction has allowed to determine localization of the bullets in the bodies accurately. This allowed to choose the optimal examination tactics in each particular case and provided invaluable assistance in the search for the bullets. Virtopsy in the cases of firearm injury has great diagnostic opportunities in visualization and 3D presentation of the wound tracks and their direction. 3D CT has recorded the original position and characteristics of the skull fractures under the undamaged tissues. The authors made the assumption that a virtual autopsy could be a reliable alternative to traditional autopsy in the foreseeable future in cases of firearm injuries.

Abstract: This article presents a clinical case of a 52-year-old man suffering from oligobacillary border-tuberculoid leprosy in the acute phase associated with an established diagnosis of pulmonary tuberculosis and hepatitis C.Aim: to present a clinical case of leprosy with atypical and complicated manifestations.Materials and methods. A 52-year-old patient had been receiving treatment for allergic dermatitis for a long time.Results. The patient was diagnosed with: A 30.2. Borderline tuberculoid leprosy (Paucibacillary Leprosy, Borderline tuberculoid form. Acute phase). The patient demonstrated atypical complications of the leprosy process in the form of ulcerative necrotic rashes.

Abstract: The clinical recommendations on management of children with Gitelman syndrome developed by the experts of the Union of pediatricians of Russia are presented in this article. Gitelman syndrome is a dysfunction of distal renal tubules with further development of hypomagnesemia, hypocalciuria and secondary aldosteronism which determine hypokalemia and metabolic alkalosis. Features of epidemiology, etiology and pathogenesis, disease progression, differential diagnostics and evidence-based treatment are presented.
