Background Although the prevalence of sarcopenic obesity is increasing, nowadays a universally accepted definition still does not exist. Because, this clinical entity is defined as the combination of obesity and sarcopenia, the diagnosis appears to be strictly linked to criteria used for sarcopenia and the available prevalence data are not uniform. To investigate the prevalence of sarcopenic obesity in older persons according to EWGSOP2 and FNIH criteria. Second, to evaluate the prevalence of diabetes in patients with sarcopenia diagnosed by the two definitions. Methods Observational multicenter study performed in 2014 on older patients admitted to 12 Italian hospitals (GLISTEN Study). Data were collected through standardized questionnaires, which assessed: socio-demographic data, cognitive status, functional abilities, pharmacological therapy, comorbidities, and blood tests. Moreover, muscle mass and strength and physical performance were evaluated. Results Six hundred and ten were included in the analyses. Among sarcopenic patients, the prevalence of sarcopenic obesity was 30.8% with FNIH and 0% with EWGSOP2 criteria. According to EWGSOP2 criteria, 23.7% of sarcopenic and 30.8% of non-sarcopenic patients were affected by diabetes (p = 0.101); otherwise, using FNIH criteria, 36.3% of sarcopenic and 26.9% of non-sarcopenic patients were diabetic (p = 0.030). After adjustment for potential confounders, diabetic patients had a 73% higher probability of being sarcopenic according to FNIH criteria (OR 1.73; 95% CI 1.13–2.64). Conclusions The EWGSOP2 and FNIH sarcopenia criteria are differently related to the prevalence of obesity and diabetes. The EWGSOP2 criteria seem to be not suitable to identify people with sarcopenic obesity.

Patient involvement in the health care process has been documented as a critical component of successful disease management. However, inadequate functional health literacy among patients is a well-known barrier to effective doctor-patient communication, which has an adverse effect on health outcomes. This study aimed at investigating the association of functional health literacy and doctor-patient communication among HIV/AIDS patients in Homa Bay County, Kenya. It was a cross-sectional hospital-based survey conducted among 362 HIV/AIDS patients receiving care at the eight sub-county hospitals of Homa Bay county. Data was collected using a self-administered structured questionnaire. Cronbach’s alpha and confirmatory factor analysis tests were used to ascertain the reliability and validity of study instruments, while Logistic regression logistic analysis was used to measure the association between functional health literacy and doctor-patient communication. 51% of the respondents were females, and the majority (30.9%) of respondents fell in the age group of 45 years and above. The respondents’ functional health literacy levels were inadequate at weighted means scores of 3.23 [SD 1.31]. The study further established that functional health literacy significantly influenced doctor-patient communication [Nagelkerke R square = 0.318]. There is, therefore, a need to document the determinants of functional health literacy to improve it and make doctor-patient interaction an enjoyable and meaningful experience.

Aim To describe the methods and feasibility of the nutritional intervention carried out within the SPRINTT Randomized cotrolled trial. We also illustrate how nutrition interventionists identified participants at risk of malnutrition and the lessons learnt from the nutrition intervention. Findings SPRINTT nutrition intervention was well-received by the majority of the participants. It was mainly carried out using tailored nutrition counselling, but also other means of delivering the intervention were successfully used. Compared with a standard nutrition prescription, an individualized protocol to diagnose malnutrition and follow-up by tailored nutrition counselling helped achieve nutritional targets more effectively in spite of diversity of population in nutritional habits and in some cases reluctance to accept changes. Message The SPRINTT nutrition intervention was feasible and allowed flexibility to the varying needs and cultural differences of this heterogeneous population of frail, older Europeans. It may serve as a model to educate and improve nutrition among community-dwelling older people at risk of mobility limitations. Supplementary Information The online version contains supplementary material available at 10.1007/s41999-020-00438-4., Background The “Sarcopenia and Physical Frailty in Older People: Multicomponent Treatment Strategies” (SPRINTT) project sponsored a multi-center randomized controlled trial (RCT) with the objective to determine the effect of physical activity and nutrition intervention for prevention of mobility disability in community-dwelling frail older Europeans. We describe here the design and feasibility of the SPRINTT nutrition intervention, including techniques used by nutrition interventionists to identify those at risk of malnutrition and to carry out the nutrition intervention. Methods SPRINTT RCT recruited older adults (≥ 70 years) from 11 European countries. Eligible participants (n = 1517) had functional limitations measured with Short Physical Performance Battery (SPPB score 3–9) and low muscle mass as determined by DXA scans, but were able to walk 400 m without assistance within 15 min. Participants were followed up for up to 3 years. The nutrition intervention was carried out mainly by individual nutrition counseling. Nutrition goals included achieving a daily protein intake of 1.0–1.2 g/kg body weight, energy intake of 25–30 kcal/kg of body weight/day, and serum vitamin D concentration ≥ 75 mmol/L. Survey on the method strategies and feasibility of the nutrition intervention was sent to all nutrition interventionists of the 16 SPRINTT study sites. Results Nutrition interventionists from all study sites responded to the survey. All responders found that the SPRINTT nutrition intervention was feasible for the target population, and it was well received by the majority. The identification of participants at nutritional risk was accomplished by combining information from interviews, questionnaires, clinical and laboratory data. Although the nutrition intervention was mainly carried out using individual nutritional counselling, other assisting methods were used as appropriate. Conclusion The SPRINTT nutrition intervention was feasible and able to adapt flexibly to varying needs of this heterogeneous population. The procedures adopted to identify older adults at risk of malnutrition and to design the appropriate intervention may serve as a model to deliver nutrition intervention for community-dwelling older people with mobility limitations. Objective Children with single ventricle cardiac disease (SVCD) have poor growth in early life. Tube-assisted feeding (TF) is used to improve weight gain, but its impact on long-term growth remains unknown. We sought to compare the longitudinal growth of SVCD patients receiving TF after initial cardiac surgery with those fed entirely by mouth. Design We conducted a retrospective cohort study of SVCD patients who underwent initial surgical palliation between 1999 and 2009. We defined TF as the use of nasogastric, gastrostomy, or jejunostomy TF. We compared maximal attained growth z-scores for each year of life between TF and non-TF patients. A secondary analysis compared surgical and clinical factors between groups. Results A total of 134 patients were included; 64% were male and 68% underwent the Norwood operation. One third of patients (44) received TF. Adjusting for age, TF patients had an average of 0.56 lower weight-for-age z-score (WAZ) than non-TF patients (P = 0.007) through the age of 6 years. Longitudinal height was not affected by TF status (P = 0.15). In a subanalysis of Norwood patients, TF patients had lower WAZ at initial hospital discharge despite longer LOS. TF patients had diminished WAZ after adjusting for complications, interstage hospitalizations, and timing of subsequent operations. Conclusions In this single-center study, patients with SVCD requiring TF at discharge from initial surgical palliation had diminished WAZ at discharge and on long-term follow-up, despite controlling for other identifiable risk factors. Further investigation is needed to understand the mechanisms underlying this phenomenon and to risk stratify infants who go home on TF.

Recurrent pancreatitis (RP) occurs in children between 15- 35 % of the cases. To determine the etiology and outcome of RP in children a descriptive, cross-sectional cohort study was conducted in children under 18 years of age with RP seen at Hospital Garrahan between 2008-2016. Of 10 patients with RP, 90 % were girls. Median age of the diagnoses of RP was 11. Median number of episodes was 9. Initial abdominal ultrasonography and/or magnetic resonance cholangiopancreatography were abnormal in four patients. Of six patients with normal studies at onset, three developed signs of chronic pancreatitis (CP) and one stenosis of the duct of Wirsung suggestive of autoimmune pancreatitis. In 40 % of our patients, the etiology was determined. On follow-up, three patients with idiopathic pancreatitis developed CP. Genetic factors may play a role in patients considered to have idiopathic pancreatitis.

There is wide variability in the timing of heart transplant (HTx) after pediatric VAD implant. While some centers wait months before listing for HTx, others accept donor heart offers within days of VAD surgery. We sought to determine if HTx within 30 days versus ≥ 30 after VAD impacts post-HTx outcomes.Children on VAD pre-HTx were extracted from the Pediatric Heart Transplant Study database. The primary endpoints were post-HTx length of hospital stay (LOS) and one-year survival. Confounding was addressed by propensity score weighting using inverse probability of treatment. Propensity scores were calculated based on age, blood type, primary cardiac diagnosis, decade, VAD type, and allosensitization status.A total of 1064 children underwent VAD prior to HTx between 2000 to 2018. Most underwent HTx ≥ 30 days post-VAD (70%). Infants made up 22% of both groups. Patients ≥ 12 years old were 42% of the30 days group and children 1 to 11 years comprised 47% of the ≥ 30 days group (p0.001). There was no difference in the prevalence of congenital heart disease vs. cardiomyopathy (p = 0.8) or high allosensitization status (p = 0.9) between groups. Post-HTx LOS was similar between groups (p = 0.11). One-year survival was lower in the30 days group (adjusted mortality HR 1.76, 95% CI 1.11-2.78, p = 0.016).A longer duration of VAD support prior to HTx is associated with a one-year survival benefit in children, although questions of patient complexity, post-VAD complications and the impact on causality remain. Additional studies using linked databases to understand these factors will be needed to fully assess the optimal timing for post-VAD HTx.

The goals of the procedure are to eliminate pain, improve function, restore stability, and realign the hind foot. The procedure has high patient satisfaction with low complications while preserving motion in adjacent tarsal joints. Sinus venosus defects (SVD) of the inferior vena cava (IVC) type, or inferior SVDs, are an uncommon form of atrial communication located outside the confines of the fossa ovalis and involve override of the IVC. Despite numerous studies describing the anatomical and echocardiographic features of the inferior SVD, distinguishing this defect from a large secundum atrial septal defect (ASD) by echocardiography is often challenging. Accurate diagnosis of an inferior SVD and correct differentiation from a secundum ASD is essential for appropriate presurgical planning. Absence of the posterior rim in the parasternal short-axis views may serve as a useful clue in diagnosing inferior SVDs. We sought to determine the utility of using the presence or absence of a posterior atrial rim in the parasternal short-axis view to help distinguish an inferior SVD from a secundum ASD. This sign may help clinch the diagnosis when subcostal imaging is suboptimal.We retrospectively reviewed transthoracic echocardiograms from 15 patients with a known surgical diagnosis of an inferior SVD between 2004 and 2015. The presence or absence of a posterior rim in the parasternal short-axis view was determined by two primary investigators. The posterior rim was also evaluated in 14 patients with a secundum ASD repair as controls. Echocardiograms were then reviewed blindly by attending-level echocardiographers and cardiology fellows in training. Diagnostic accuracy was assessed both with and without the use of the posterior rim criterion. Statistical analysis was used to determine the effect of using the rim criterion on inferior SVD diagnosis. We also reviewed all surgically diagnosed secundum ASDs that were incorrectly diagnosed as inferior SVD by preoperative imaging and determined whether use of the posterior rim criterion would have resulted in the correct diagnosis.The posterior rim was absent in all 15 patients with a surgical diagnosis of inferior SVD and present in all 14 patients with a secundum ASD. For all observers, there was a statistically significant increase in diagnostic accuracy of inferior SVDs with the use of the rim criterion (P .0001). We noted that secundum ASDs with inferior extension also have persistent posterior rims. The rim criterion correctly classified all large secundum ASDs with inferior extension that were previously misdiagnosed by echocardiogram preoperatively.Absence of the posterior rim ("bald" posterior wall) is a consistent finding in patients with an inferior SVD and distinguishes an inferior SVD from a large secundum ASD with inferior extension. Parasternal short-axis evaluation of the posterior atrial rim is a helpful tool for all levels of physician training in improving diagnostic accuracy for detecting inferior SVDs and in distinguishing them from secundum ASDs.

Durable ventricular assist devices (VADs) are increasingly used to treat children with heart failure. Studies demonstrate worse outcomes for those in cardiogenic shock at the time of VAD, but limited data exist on less acutely ill children. We describe the association between illness severity and outcomes in this population.Data were analyzed from 373 children (aged19 years) receiving durable VADs from 46 centers in the Pediatric Interagency Registry for Mechanical Circulatory Support. Outcomes were compared by Interagency Registry for Mechanical Circulatory Support (INTERMACS) Patient Profile (PP) and pre-implant characteristics using competing risks methodology.Analyses identified 97 patients in cardiogenic shock (PP 1), 222 with progressive decline (PP 2), and 42 stable on inotropes (PP 3). There were 39 infants, 124 were aged 1 to 9 years and 210 were aged 10 to 19 years. A majority had cardiomyopathy and 66 had congenital heart disease (CHD). There were 224 (62%) continuous-flow VADs. Before implant, 40% received mechanical ventilation (MV). Within 6 months post-implant, 57% underwent transplant and 14% died. PP 1 mortality was highest (25% vs 10% for PP 2, hazard ratio [HR]: 2.5, 95% CI: 1.4-4.4, p = 0.02). In PP 1, CHD was an independent mortality risk factor (HR: 2.9, 95% CI: 1.1-7.8, p = 0.03). In PP 2, pulsatile VADs were associated with death (HR: 3.9, 95% CI: 1.6-9.5, p = 0.003). Patients on MV had high mortality (20%-30%) across PP 1 to PP 3 (HR: 3.0 vs no MV, p0.001).Children in shock at the time of VAD implant have poor outcomes. MV is associated with increased mortality even in lower acuity INTERMACS profiles. Further study is needed to identify modifiable risk factors in this population.

Digital deformities and hammertoe may present in a variety of forms and may be one of the most challenging surgical procedures for the foot and ankle surgeon to obtain reliable, predictable, and reproducible results. Typically the underlying pathology is associated with an imbalance of the tendons that insert into the toes resulting in subsequent secondary changes in digital deformities and hammertoes. The result of this tendon imbalance often will develop soft tissue contractures leading to a variety of deformities in the sagittal, transverse, and frontal planes. When evaluating digital deformities and hammertoes, rarely present is an osseous pathology identified at the proximal, middle, or distal phalanx. Typically it is a soft tissue contracture which causes the hammertoe or digital deformities in the sagittal, transverse, and frontal plane; therefore, tendon transfers can be utilized in the correction of flexible, semirigid, and rigid digital deformities. Flexor substitution, flexor stabilization, and extensor substitution/recruitment deformities are well-known mechanisms in development of digital deformities and can be corrected through tendon transfers. The modification of the Hibbs procedure as well as a modification of the Taylor-Girdlestone procedure is indicated for surgical correction of digital deformities and hammertoes. Unlike arthroplasty and arthrodesis procedures, this method corrects the underlying pathology and does not require any fusing, shortening, or resection of the bone from the proximal, middle, or distal phalanx or of the given joint or joints, therefore maintaining the cubic volume of the bone and maintaining continued stability within the digits. These procedures address the underlying pathology and balance the toes, therefore decreasing the likelihood of reoccurrence. Additionally, it is more patient and surgeon friendly. The incision sites are better placed for improved cosmesis. The postoperative edema is markedly reduced when compared to the osseous digital procedures as the surgery does not invade the bone. There is no shortening or instability of the toes as the medial and lateral collateral ligaments are left intact limiting the chance of rotation or transverse plane complications. The modification to the Hibbs (extensor tendons) and Taylor-Girdlestone (flexor tendons) procedure produces reproducible, long-term correction with minimal complications.

Heart transplantation is an established therapy in children with end-stage heart failure. Ninety percent of pediatric heart transplant recipients have congenital heart disease or cardiomyopathy as the underlying disease. Exercise performance in the early posttransplant period is affected by factors that precede transplant, such as malnutrition and deconditioning from heart failure, as well as associated medical conditions (e.g., neuromuscular disease). Improvement in exercise performance in the months following transplant may be aided by referral to a formal rehabilitation program. Performance on long-term follow-up remains subnormal due to transplant-specific factors such as cardiac autonomic denervation with resulting chronotropic incompetence and persistent left ventricular diastolic dysfunction. Exercise testing with stress echocardiography has potential utility in noninvasive surveillance for posttransplant coronary artery disease.

BACKGROUND The authors examine trends in spending on cancer from 1998 through 2012, including cancer care costs, prevalence, and cases by payer, and discuss the results within the context of a prior analysis and recent health policy and programmatic changes. METHODS Condition-specific distribution of expenditures from the Medical Expenditure Panel Survey, supplemented with results from the National Nursing Home Survey and other data sources, was used as the basis for allocating the Personal Health Care components of the National Health Expenditure Accounts among conditions. RESULTS Cancer care expenditures grew at an annualized rate of 2.9% from 1998 to 2012. The share of expenditures for hospital-based care declined to a low of 48% during 2007 through 2009. Professional and clinical services' shares declined substantially between 2007 to 2009 and 2010 to 2012 when the hospital share increased. Treated prevalence decreased for all payers between the first and last study periods with the exception of private payers (11.2% increase). Out-of-pocket expenditures declined to 4.7%, whereas Medicare's share increased slightly. Medication expenditures increased, notably within retail and mail order settings. CONCLUSIONS The previous rapid growth of cancer prevalence and expenditures has now slowed, most remarkably since the 2007 recession. Out-of-pocket expenses for cancer treatment continue to decline, most recently reaching the lowest point in 25 years. In addition, the early effects of Affordable Care Act expansion can be observed in the decline of treated prevalence in the Medicaid population as the demographics of Medicaid enrollees change. Despite high sexual activity among adolescent girls in Kenya, contraceptive uptake is very low with only about 26 percent sexually active adolescent girls currently using a contraceptive method. This exposes them to HIV infections and unplanned pregnancies which consequently lead to school dropouts, unsafe abortions, and lack of employment opportunities. This cross-sectional study aimed at assessing the utilization of contraceptives among secondary school adolescent girls in Karuri Town Council, Kiambu County. Overall, 421 girls aged between 13.0 to 19.0 years took part in the study. Findings showed that despite majority (77.5%) of the adolescent girls having had sexual debut by the age of 15 years, contraceptive utilization was very low at 43%. The results revealed that age of the adolescent, knowledge of contraceptives options, perception and accessibility of the contraceptives had positive significant effect on contraceptive utilization. Adolescents aged 18 years and above were more likely to utilize contraceptives as compared to those of a lesser age (p ≤ 0.001; OR: 9.870 (95% CI: 3.781 - 25.763)). Those with knowledge on contraceptives were OR 3.2 times more likely to use contraceptives (p = 0.025), similarly, accessibility was significantly associated with increased contraceptive utilization (p = 0.34, 95% CI: 1.054 - 4.187). Adolescents who perceived use of contraceptives as wise were more likely to use a contraceptive than those of a divergent opinion (OR: 2.053 (95% CI: 1.024 - 4.115), p = 0.041). This revealed that high level of knowledge on contraceptives did not always amount to practice. There is therefore a need to develop age specific reproductive health messages to guide school education curriculum as well as parent or guardian-child communication.

Serum troponin (Tn) is often elevated in viral myocarditis; however, its prognostic significance is unknown. We tested the hypothesis that abnormal serum Tn is associated with mortality in children hospitalized with myocarditis. We retrospectively studied data from six large children’s hospitals participating in the Pediatric Health Information System Plus (PHIS+) database. Analysis was performed on patients hospitalized with viral myocarditis between 2007 and 2013, in whom at least one Tn was recorded within 72 h of admission. Abnormal baseline Tn was defined as any value outside the upper limit of normal within the first 72 h. Primary outcome was mortality. Secondary outcomes included mechanical support, defined as use of extracorporeal membrane oxygenation (ECMO) or a ventricular assist device (VAD), cardiac transplantation, intravenous immunoglobulin (IVIg), mechanical ventilation, and inotrope use. A total of 149 patients with myocarditis (61% male, 48% adolescents) across all PHIS+ centers had TnI (n = 113) or TnT (n = 36) recorded. At least one abnormal Tn was present in 81% of cases. The Contraceptive Prevalence Rate for West Pokot County is estimated to be at a low of 23% compared to the national prevalence which stands at 43%. This analytic cross sectional study was aimed at establishing the male factors associated with family planning in West Pokot County, Kenya. A total of 266 married men were involved and household was the unit of analysis. The findings of this study demonstrated that 48% of the respondents were not involved at all in family planning and only 6% of men were using a family planning method. The age of respondents, educational level, number of children, and type of marriage, knowledge and ease of access to family planning services were all significantly associated with male involvement. Having no education made a man 89% less likely to be highly involved in family planning (OD 0.117; 95% CI: 0.03-0.454). The study concludes that the level of male involvement in family planning in the county is still low despite interventions both by the government and other nongovernmental organizations. Therefore, more efforts should be made to improve education standards especially for men and to reorient family planning services to make them more accessible to men.

Methods: Study Design: Random, Retrospective Chart Review of a single surgeon. 186 met the inclusion criteria with 172 complete charts available for review Procedure: Osseous foot and ankle surgery including osteotomies, fusions, and fracture repair. (1st MPJ fusions, Lapidus, Ankle, TTC) Inclusion Criteria: Undergo osseous surgery including osteotomies, fusions, or fracture repair Received Toradol in the perioperative period Complete chart with radiographs Outcomes: Radiographic and chart analysis for non-union between the initial post-operative visit and the final visit Results: A total of 186 patients were identified in having undergone a boney procedure and having received Toradol in the post - operative period. Out of the 186 patients, 172 charts were available for review. 165 of the 172 patients underwent unremarkable post operative healing. Seven patients (4.1%) were identified to have a clinical, radiographic or advanced imaging confirmed non- union. This was found to be not statistically significant with a p-value of < 0.005 Conclusion: Since the mainstream data available has been unable to draw a clear conclusion regarding NSAID use in bone surgery cases, our study aimed to present a large patient group that had no ill effects from its usage post operatively in osseous foot & ankle surgery. As the data demonstrates, there was no correlation that post operative use of potent NSAIDs increased the risk of non-union in the patient group.

Total ankle replacement can be a challenging surgery especially when pre-operative deformity exists. Most authors advocate the indication for total ankle replacement should be narrowed to patients with less than 10 to 15 degrees of pre-operative varus or valgus. When greater than 10-15 degrees of coronal plane malalignment is found, ancillary procedures must be performed. We report a case of a 56 year old male with 20 degrees of pre-operative ankle valgus after a pronation-external rotation injury that was malreduced at an outside institution. In addition to the valgus, the patient presented with medial ankle instability, distal lateral tibial osteonecrosis and a shortened, posteriorly rotated fibula. Staged procedures were employed to successfully realign the patient’s ankle joint. The patient was first brought to the operating room and stressed under c-arm fluoroscopy. He was found to have instability of the deltoid complex. He subsequently underwent an ankle arthrotomy, synevectomy and deltoid imbrication to re-establish medial ankle stability. Second, the patient underwent a fibular lengthening and derotation, syndesmotic fusion and medial opening wedge tibial osteotomy. Once consolidation was confirmed by CAT scan, the patient had a gastrocnemius lengthening total ankle arthroplasty with a Zimmer trabecular metal implant (Zimmer Inc®, Warsaw, IN) and debridement and grafting of the distal lateral tibial osteonecrosis. The patient is now greater than 24 months post operatively and able to ambulate pain free without assistive devices.

Category: Bunion Introduction/Purpose: A 6-8 week non-weightbearing period post modified lapidus arthrodesis has traditionally been universally accepted. A review of the literature reports nonunion rates between 5-33% after modified lapidus with 6-8 weeks of non- weightbearing. There are only a few reports in the literature that discuss immediate weightbearing after the modified lapidus procedure. In this retrospective review, we examine the nonunion rate among patients undergoing the modified lapidus arthrodesis who were immediately weightbearing in a controlled ankle motion (CAM) boot. It is our hypothesis that the non- union rate will be comparable to the traditional post-operative protocol nonunion rate. Methods: In this retrospective study, we evaluated chart data on patients who underwent a modified Lapidus arthrodesis with immediate weight- bearing in a CAM boot. The goal of the study was to determine if a modified Lapidus surgical technique in conjunction with rigid internal fixation produces the necessary stability post-operatively in order to allow bony consolidation while being stressed with immediate weight bearing forces. Patients who underwent a modified Lapidus arthrodesis using either a 3 screw construct or a medially placed locking plate with an interfragmentary screw were included in the study. Patients with other adjunctive procedures that would prevent early weight bearing were not included in the investigation. Results: A total of 376 patients were identified with 74 patients meeting inclusion criteria. 4 patients had bilateral procedures performed at separate times for a total of 78 Lapidus procedures. 31 patients had a 3 screw construct while 43 patients had a medial locking plate with an interfragmentary screw. 15 patients admitted to using nicotine. Additionally, 7 patients had type II Diabetes Mellitus. Autogenous shear- strain relief bone graft was used in 75 of the 79 procedures.Three patients (3.8%) experienced a post-operative non-union. Interestingly, none of the patients with non-unions were smokers and only one patient was diabetic. Conclusion: The results of this study found a 3.8% nonunion rate. Based on these results we conclude that a modified Lapidus procedure using a long, solid interfragmentary screw with either additional screw support or a locking plate, we may permit patients to ambulate safely in a CAM boot immediately post- operative period without affecting surgical outcome.

Category: Ankle, Ankle Arthritis Introduction/Purpose: The purpose of this review is to present a case of post-traumatic ankle valgus and distal lateral tibial osteonecrosis successfully treated with staged deltoid repair, opening wedge tibial osteotomy, fibular lengthening, syndesmotic fusion and total ankle arthroplasty. Methods: Initial surgery consisted of ankle joint arthrotomy and deltoid imbrication. The second surgery consisted of a tibial opening wedge osteotomy with autogenous cortical fibular bone graft superior to the area of osteonecrosis to correct the 20 degree ankle valgus. Fibular lengthening osteotomy and fusion of the distal syndesmosis were also performed. CT scan confirmed bony consolidation at the distal tibiofibular syndesmosis as well as union of the allograft opening wedge. The final surgery was total ankle joint replacement with bone grafting of the area of osteonecrosis. Results: After 5 years of follow up the patient has progressed out of his AFO to full weightbearing. He reports no ankle pain, improved function and range of motion and is ambulating independently with no assistive devices. Conclusion: We successfully treated a case of distal lateral tibial osteonecrosis, and a 20 degree ankle valgus with staged deformity correction and ankle replacement. Radiographs demonstrate a well seated and positioned implant. We believe that with proper alignment that total ankle arthroplasty is a safe treatment option in the face of bone infarction.

The effect of low sample frame rate on interpretability is often confused with the impact it has on encoding processes. In this study, the confusion was avoided by ensuring that none of the low-frame rate clips had coding artifacts. Under these conditions, the lowered frame rate was not associated with a statistically significant change in interpretability. Airborne, high definition 720P, 60 FPS video clips were used as source material to produce test clips with varying sample frame rates, playback rates, and degrees of target motion. Frame rates ranged from 7.5 FPS to 60 FPS. Playback rates ranged up to 8X normal speed. Target motion ranged from near zero MPH up to 300 MPH.

Intrathecal liposomal cytarabine (ITLC) has shown prolonged time to neurological deterioration in some patients with leptomeningeal spread of breast cancer. We describe a patient with fulminant chemical ventriculomeningitis leading to cerebral edema and death following ITLC administration. A 56-year-old Caucasian female developed progressive headache, depressed level of consciousness and adventitious movements immediately following ITLC. Neurological examination was normal prior to injection. This progressed to loss of all brainstem function and a head CT scan demonstrated diffuse cerebral edema. Due to the absence of neurological function on examination, the family opted to withdraw care. To our knowledge we report the first patient with fulminant chemical ventriculomeningitis leading to cerebral edema and death following ITLC administration. Adjunctive glucocorticoids are recommended. Treatment options are limited and complicated by the liposomal formulation of intrathecal cytarabine.

The following material is given to address the effect of low slant angle on video interpretability: 1) an equation for the minimum slant angle as a function of field-of-view to prevent no more than a s change in GSD across the scene; 2) evidence for reduced situational awareness due to errors in perceived depth at low slant angle converting to position errors; 3) an equation for optimum slant angle and target orientation with respect to maximizing exposed target area; 4) the impact of the increased probability of occlusion as a function of slant angle; 5) a derivation for the loss of resolution due to atmospheric turbulence and scattering. In addition, modifications to Video-NIIRS for low slant angle are suggested. The recommended modifications for low-slant angle Video-NIIRS are: 1) to rate at or near the center of the scene; and 2) include target orientations in the Video-NIIRS criteria.

The effect of video compression is examined using the task-based performance metrics of the new Video National Intelligence Interpretability Rating Scale (Video NIIRS). Video NIIRS is a subjective task criteria scale similar to the well-known Visible NIIRS used for still image quality measurement. However, each task in the Video NIIRS includes a dynamic component that requires video of sufficient spatial and temporal resolution. The loss of Video NIIRS due to compression is experimentally measured for select cases. The results show that an increase in the compression and an associated increase in artifacts reduces task based interpretability and lowers the Video-NIIRS rating of the video clips. The extent of the effect has implications for system design.

The Video National Imagery Interpretability Rating Standard (V-NIIRS) consists of a ranked set of subjective criteria to assist analysts in assigning an interpretability quality level to a motion imagery clip. The V-NIIRS rating standard is needed to support the tasking, retrieval, and exploitation of motion imagery. A criteria survey was conducted to yield individual pair-wise criteria rankings and scores. Statistical analysis shows good agreement with expectations across the 9-levels of interpretability, for each of the 7 content domains.

A perceptual evaluation compared tracking performance when using color versus panchromatic synthetic imagery at low frame rates. Frame rate was found to have an effect on tracking performance for the panchromatic motion imagery. Color was found to be associated with improved tracking performance at 2 frames per second (FPS), but not at 6 FPS or greater. A self estimate of task confidence given by the respondents was found to be correlated to the measured tracking performance, which supports the use of task confidence as a proxy for task performance in the future development and validation of a motion imagery rating scale.

We report a case of idiopathic inflammatory myopathy accompanied with the presence of anti-Jo1 antibodies and complicated by diffuse parenchymal lung disease (antisynthetase syndrome). Efficacy of different therapeutic agents, including corticosteroids, cyclophosphamide and cyclosporin A, is described. We present beneficial effect of cyclosporin A and corticosteroids regimen, and confirm no benefits of plasmapheresis. An interesting association of the disease onset with toxoplasmosis infection is discussed and difficulties regarding diagnosis of cardiac involvement are addressed. We also review other case reports of this rare disorder of poor prognosis.

Cardiogenic shock (CS) is a dreadful complication of acute myocardial infarction (AMI) associated with a poor prognosis. Percutaneous coronary intervention (PCI) is widely recommended by current treatment guidelines.To evaluate the in-hospital and 30-day mortality rate and to determine independent predictors of mortality in a cohort of unselected consecutive patients with CS.Rabin Medical Center cardiac catheterization laboratory database was analyzed between 1/2000 and 8/2003. Fifty of the 472 patients (10.6%) treated using emergent PCI for AMI had cardiogenic shock on presentation. Patients with cardiogenic shock were older, more likely to be female and with higher frequency of co-morbidities. The time from symptom onset until seeking medical treatment was longer in cardiogenic shock patients. In-hospital mortality rate was 48.0% in the cardiogenic shock group as compared to 3.3% in the non-cardiogenic shock group (P0.0001). In patients with shock, total mortality after 30 days was 52% (26/50). Most of these patients (25/26) died within 48 hours following admission because of refractory cardiogenic shock. A multivariate analysis adjusted for baseline differences showed that age 75 years (odds ratio [OR]: 11; 95% confidence interval [CI]: 1.0-1.24, P=0.05), and the use of GP 2b/3a antagonist (OR: 0.97; 95% CI: 0.95-1.0, P=0.05), were independent predictors of all cause mortality at 30 days.Cardiogenic shock remains an important cause of mortality in AMI. Younger age and the use of GP 2b/3a antagonists during primary PCI for cardiogenic shock patients seems to be associated with better clinical outcomes.

This registry describes our preliminary experience with a novel introducing-catheter allowing direct percutaneous introduction of the 6F guiding catheter (G-C), minimizing the puncture size, preventing vessel scraping, and improving the pushability and torque response of the G-C. In 1995, 203 patients had sheathless PTCA, using this device. Eighty-five percent were male. Mean age was 65 ± 10 years. Thirty-nine percent had stable angina, 35% unstable angina, 7% evolving infarction, and 19% recent infarction. Two hundred fifty-six lesions were treated (1.26/patient). One hundred eight patients (52%) received one (85%) or more than one (15%) stent. The procedural success rate was 98%. Mean coronary stenosis was 82 ± 10% and decreased to 20 ± 15% after PTCA. No major complication occurred. The guiding catheter was immediately removed in 95% of patients, despite heparinization. No patient required surgery or blood transfusion for vascular complications, and only 7 had minor local complications (3.5%). Sheathless angioplasty provides no technical difficulties and has the same safety and quality as conventional angioplasty using a sheath. Immediate removal of the guiding catheter, without keeping vascular access, has no deleterious effect, allows early mobilization, and may limit the risk of vascular complications. Cathet. Cardiovasc. The authors performed a retrospective study on a data base of 525 patients with periph eral arterial disease, to analyze the pathophysiologic meaning of resting transcutaneous pressure of carbon dioxide (PCO2) and of CO2 production during three minutes of local ischemia. The resting and postischemic PCO2 and its maximum increase related to rest (PCO2 production) were measured with Kontron 7640 equipment. The results show a significant increase of PCO2 production in the Fontaine stage 2A (183 patients, 4.61 mmHg, P < 0.0001), in stage 2B (194 patients, 5.22 mmHg, P < 0.0001), in the third stage (83 patients, 6.10 mmHg, P < 0.0001), and in the fourth stage (53 patients, 8.66 mmHg, P < 0.0001). Only the patients at the first stage showed an insignificant increase, perhaps because of the small number (12) in this group. The authors feel that the measurement of tcPCO2 production during local ischemic stress can be a very important parameter for evaluating peripheral arterial disease as an expression of metabolic tissue performance and, overall, of the tissue resistance to ischemia.

Forty patients with a mean age of 62.6 ± 6 years, 36 men and 4 women, with peripheral arterial occlusive disease (PAOD) at Leriche-Fontaine IIb class, were randomly allocated to one of two treatment groups, receiving either 12,500 IU/ day of subcutaneous (sc) calcium-heparin (CAE) or 250 mg/day of oral ticlopi dine, each given for ninety days. The following parameters were evaluated be fore the start of the active treatment period and after thirty and ninety days of treatment: pain-free walking distance (PWD), maximum walking distance (WDmax), systolic and diastolic blood pressure (BP), posterior tibial arterial pressure and Winsor index at rest and after exercise (treadmill), transcutaneous oxygen and carbon dioxide pressures at rest (TcPO2 and TcPCO2 respectively), and time to 50% TcPo2 recovery after three-minute ischemia. Both treatments induced an improvement in PWD/WDmax, which, at the end of the study, were increased by 50.7/58.7% and 31.7/36.2%, respectively, for CAE and ticlopidine treatments, respectively. Statistical analysis did not show any difference between treatments, al though a statistically significant difference was found in comparison with basal measurements for both CAE and ticlopidine treatments (p < 0.01). Neverthe less, the effects determined by ticlopidine were, on the whole, less impressive than those of CAE. In fact, interestingly, although not in a statistically significant way, the CAE group walked longer in comparison with the ticlopidine group irrespective of the more critical baseline clinical conditions. Moreover, TcPo2 was favorably modified by CAE (p < 0.01) but not by ticlopidine treatment while the other parameters remained unchanged. In conclusion, CAE, given at the daily dose of 12,500 IU in patients with PAOD at the Leriche-Fontaine IIb class, showed beneficial clinical effects, which, in turn, could be due both to the removal of the thrombophilic im balance and to the restoration of the fibrinolytic-coagulatory hemostasis in the involved microvascular circulation.

A unique case of aortic atresia and tricuspid atresia associated with transposition of the great vessels is described in a newborn infant. The clinical, echocardiographic, and heart catheterization data are presented. The embryological and surgical implications are discussed.

The planning of the Italian Network Supervision Center (CSN) that the Operating Company is currently developing, has given rise to considerations about purposes and technical studies of traffic management strategies.

Five cases are described in which noninflammatory pulmonary arterial aneurysmal disease existed. In each case, severe pulmonary hypertension had been present. In one (woman, age 50 years), the pulmonary hypertension was of the primary type, while in each of the others, congenital heart disease was an underlying condition (atrial septal defect in three cases; each woman, age range 35 to 56 years) and complete transposition in the remaining case (boy, age ten). In one case with atrial septal defect, saccular aneurysm involving each major pulmonary arterial branch was considered to have resulted from atherosclerotic disease. In each of the remaining cases, cystic medial necrosis was present, and the aneurysmal disease was the result of laceration of a major pulmonary arterial segment. In one of the cases with atrial septal defect, the laceration was recent and extended through the full thickness of the pulmonary trunk in a circumferential manner. In the remaining three cases, lacerations were usually old and had not extended through the full thickness of the involved arterial segments. Saccular aneurysm formation (usually multiple) had resulted from weakness of the wall as a result of laceration and retraction of tissue.

A major drawback of currently used exchange guide wires is insufficient stiffness, causing them to buckle or dislodge when used during catheter exchanges. A new heavy-duty exchange guide wire is described that facilitates catheter exchanges around sharp corners, such as those encountered during various cardiac interventional procedures. The wire was successfully used in experimental valvuloplasty and experimental dilation of the ductus arteriosus.

Physiologic correction of complete transposition of the great vessels (DTGV) by intraatrial baffle, as performed by Senning and Mustard and subsequently modified by Shumacker, has dramatically extended the survival of patients born with this lethal cyanotic heart defect. Rhythm disturbances, including bradycardia and tachycardia, are common and serious problems following surgery [1–3]. The cause of the rhythm disturbances has not been completely established, but in some patients, rhythm disturbances have been documented prior to atrial baffle [4].

Fifty-four specimens of heart with persistent truncus arteriosus (PTA) were reviewed anatomically. According to the Collett-Edwards classification there were 28 examples of type I and 26 type II. The sex distribution was equal. The number of the truncal cusps ranged from one to four (42% tricuspid, 30% bicuspid, 24% quadricuspid, and 4% unicommissural). A unicommissural truncal valve has not been previously reported. In 72% of cases, the truncal valve leaflets were thickened or dysplastic. Two valves were stenotic. The truncus arteriosus originated from both ventricles equally in 42% of the cases, predominantly from the right ventricle in 42%, and predominantly from the left ventricle in 16% of the cases. In unoperated cases of PTA originating predominantly from the right ventricle, it appeared to us that usual operative correction might result in left ventricular outflow obstruction. Variations in coronary arterial origins and patterns were present in nearly half of the cases. A single coronary artery was observed in ten cases (18.5%). Stenosis of the ostium of one coronary artery was seen in each of four cases (7%). High posterior origin of the left coronary artery was observed in ten cases (18.5%). Among the associated cardiovascular anomalies, the most common were right aortic arch (36%) and interruption of the aortic arch (11%). Three cases with the latter condition exhibited crossed pulmonary arteries. Isolated cases with tricuspid atresia, vascular sling (left pulmonary artery arising from right pulmonary artery), and persistent common atrioventricular canal were encountered.

An introducer-sheath dilator is described, which was modified to protect the leading lip of the sheath during insertion, thus reducing arterial damage. The new sheath and a standard sheath were inserted into the femoral arteries of six dogs. Subsequent histologic examination of the arteries proved the new sheath to be both safe and effective.

Transesophageal study was used for diagnosis and treatment of 51 episodes of tachycardia in 13 patients with complete transposition of the great arteries who had undergone atrial baffle procedure. At the time of atrial baffle procedure, patients were 6 to 36 months old (mean 23). Tachycardia (1 to 17 episodes per patient) first occurred 1 to 23 days (4 patients) or 1.8 to 12 years (9 patients) after atrial baffle. Transesophageal study was performed using a bipolar silicone rubber-coated catheter. Tachycardia conversion was accomplished with stimulation bursts using 4 to 10 stimuli 9.9 ms in duration at 20 to 28 mA and an interstimulus interval of 50 to 100 ms less than the atrial cycle length. All tachycardia episodes had regular atrial cycle lengths ranging from 200 to 350 ms. In 12 patients, second-degree atrioventricular (AV) block was observed during tachycardia, suggesting primary atrial tachycardia. However, in 1 patient, occurrence of AV block always resulted in tachycardia termination, suggesting the presence of AV reentrant tachycardia. Transesophageal stimulation converted 48 of 51 tachycardia episodes to sinus/junctional rhythm. Ten tachycardia episodes in 6 patients were transiently converted to atrial fibrillation lasting 3 seconds to 28 minutes before spontaneous conversion to sinus junctional rhythm. Conversion attempts were unsuccessful on 3 occasions. Acceleration of ventricular rate after stimulation necessitated DC cardioversion on 1 occasion. Conversion was not achieved in 2 tachycardia episodes using stimuli less than 10 mA. Transesophageal study is a safe and effective minimally invasive technique for diagnosis and treatment of tachycardia in infants and children who have had atrial baffle for transposition of the great arteries.

Occult pulmonary artery is an uncommon cardiovascular defect. Associated symptoms include recurrent pulmonary infections and congestive heart failure. The authors describe a one-year-old boy initially diagnosed as having broncho- pulmonary dysplasia who developed severe failure to thrive, recurrent pneumo nias, and pulmonary hypertension. The presence of an occult right pulmonary artery was suspected by lung perfusion scan and diagnosed by cardiac catheteri zation and angiography. After surgical repair, his clinical course improved and his growth and development were normal. This case demonstrates the impor tance of including occult pulmonary artery in the differential diagnosis of in fants with failure to thrive associated with recurrent pulmonary infection.
