Abstract: Supply chain collaborations may generate substantial cost savings. Many such initiatives cease to exist, however, due to not reaching an agreement on how to share the gains. We describe an exercise to understand the challenges in collaborative gain sharing. Abstract: Purpose To review how outcomes of clinical utility are operationalized in current amyloid-PET validation studies, to prepare for formal assessment of clinical utility of amyloid-PET-based diagnosis. Methods Systematic review of amyloid-PET research studies published up to April 2020 that included outcomes of clinical utility. We extracted and analyzed (a) outcome categories, (b) their definition, and (c) their methods of assessment. Results Thirty-two studies were eligible. (a) Outcome categories were clinician-centered (found in 25/32 studies, 78%), patient-/caregiver-centered (in 9/32 studies, 28%), and health economics-centered (5/32, 16%). (b) Definition: Outcomes were mainly defined by clinical researchers; only the ABIDE study expressly included stakeholders in group discussions. Clinician-centered outcomes mainly consisted of incremental diagnostic value (25/32, 78%) and change in patient management (17/32, 53%); patient-/caregiver-centered outcomes considered distress after amyloid-pet-based diagnosis disclosure (8/32, 25%), including quantified burden of procedure for patients’ outcomes (n = 8) (1/8, 12.5%), impact of disclosure of results (6/8, 75%), and psychological implications of biomarker-based diagnosis (75%); and health economics outcomes focused on costs to achieve a high-confidence etiological diagnosis (5/32, 16%) and impact on quality of life (1/32, 3%). (c) Assessment: all outcome categories were operationalized inconsistently across studies, employing 26 different tools without formal rationale for selection. Conclusion Current studies validating amyloid-PET already assessed outcomes for clinical utility, although non-clinician-based outcomes were inconsistent. A wider participation of stakeholders may help produce a more thorough and systematic definition and assessment of outcomes of clinical utility and help collect evidence informing decisions on reimbursement of amyloid-PET.

The telescopes will detect extraterrestrial and atmospheric neutrinos by means of the incident photons induced by the passage of relativistic charged particles through the seawater as a consequence of a neutrino interaction. The telescopes are configured in a three-dimensional grid of digital optical modules, each hosting 31 photomultipliers. The photomultiplier signals produced by the incident Cherenkov photons are converted into digital information consisting of the integrated pulse duration and the time at which it surpasses a chosen threshold. The digitization is done by means of time to digital converters (TDCs) embedded in the field programmable gate array of the central logic board. Subsequently, a state machine formats the acquired data for its transmission to shore. Abstract: Pressure load measurements in operative conditions represent a fundamental parameter for improving the knowledge of the in-flight forcing generation and transmission. Furthermore, it is a fundamental step for assessing the validity and confidence of numerical methods able to predict such pressure loads, usually generated by propulsion and turbulent boundary layer. The experimental in-flight measurements and analysis and the correlation with the numerical methods are activities required as a starting point for studying the transmission mechanism through the fuselage which is responsible for the vibration and noise level which can be measured inside and for identifying potential solutions for their mitigation. The in-flight measurements have been carried out on a Leonardo Helicopter tiltrotor with the main goals to perform the validation of an experimental setup in a fully new environment and the assessment of a data analysis procedure according to the objectives of the Clean Sky 2 Fast Rotorcraft Project T-WING (CS2, Clean Sky 2 joint undertaking third amended bi-annual work plan and budget, 2018–2019).

Abstract: The KM3NeT Collaboration runs a multi-site neutrino observatory in the Mediterranean Sea. Water Cherenkov particle detectors, deep in the sea and far off the coasts of France and Italy, are already taking data while incremental construction progresses. Data Acquisition Control software is operating off-shore detectors as well as testing and qualification stations for their components. The software, named Control Unit, is highly modular. It can undergo upgrades and reconfiguration with the acquisition running. Interplay with the central database of the Collaboration is obtained in a way that allows for data taking even if Internet links fail. Abstract: Lung cancer (LC) is the most common type of cancer and the second cause of death worldwide in men and women after cardiovascular diseases. Non-small-cell lung cancer (NSCLC) is the most frequent type of LC occurring in 85% of cases. Developing new methods for early detection of NSCLC could substantially increase the chances of survival and, therefore, is an urgent task for current research. Nowadays, explosion in nanotechnology offers unprecedented opportunities for therapeutics and diagnosis applications. In this context, exploiting the bio-nano-interactions between nanoparticles (NPs) and biological fluids is an emerging field of research. Upon contact with biofluids, NPs are covered by a biomolecular coating referred to as “biomolecular corona” (BC). In this study, we exploitedBC for discriminating between NSCLC patients and healthy volunteers. Blood samples from 10 NSCLC patients and 5 subjects without malignancy were allowed to interact with negatively charged lipid NPs, leading to the formation of a BC at the NP surface. After isolation, BCs were characterized by sodium dodecyl sulfate polyacrylamide gel electrophoresis (SDS-PAGE). We found that the BCs of NSCLC patients was significantly different from that of healthy individuals. Statistical analysis of SDS-PAGE results allowed discriminating between NSCLC cancer patients and healthy subjects with 80% specificity, 80% sensitivity and a total discriminate correctness rate of 80%. While the results of the present investigation cannot be conclusive due to the small size of the data set, we have shown that exploitation of the BC is a promising approach for the early diagnosis of NSCLC.

7. Evaluating an incobotulinumtoxinA and Cohesive Polydensified Matrix ® hyaluronic acid filler combination to treat moderate‐to‐severe periorbital and perioral rhytids

Abstract: Aim To evaluate the effectiveness of treating mimetic facial lines with an incobotulinumtoxinA (INCO) and Cohesive Polydensified Matrix® hyaluronic acid (CPM-HA) combination. Methods Subjects with moderate-to-severe lateral periorbital lines (LPL) and perioral lines at rest according to Merz Aesthetics Scales™ received 10 U of INCO/eye and up to 10 U to the perioral area. This was followed by intradermal injection of 1-3 cc CPM-HA to the same lines. Primary outcomes were physician- and subject-rated improvement at 1 month using the Global Aesthetic Improvement Scale (GAIS) and changes in line severity using the Merz Aesthetics Scales™ . Results The study included 10 women aged 42-74 years with moderate-to-severe LPL and perioral lines at baseline. At 1 month, treating physician GAIS scores indicated 45.0% of subjects were very much improved and 52.5% much improved; 75% remained at least improved at 6 months. Subject GAIS results indicated 7 women rated themselves as very much improved and 3 as much improved. For LPL, improvements of ≥1-point on the Merz Aesthetics Scales™ compared with baseline were seen at 1, 3, and 6 months in 86.8%, 74.3%, and 65.8% of subjects, respectively. For perioral lines, improvements of ≥1-point were observed for 89.2%, 86.5%, and 73.0% at 1, 3, and 6 months, respectively. No treatment-related adverse events were observed. Conclusions Combined INCO/CPM-HA filler in individuals with moderate-to-severe LPL and perioral lines was effective at improving overall appearance and reducing line severity in two highly mobile facial areas that are the first to show lines of aging.

Abstract: Background In dogs with congestive heart failure (CHF), the efficacy of torasemide, a loop diuretic, has been demonstrated. However, unlike in dogs and humans little has been described about the use of torasemide in the cat with spontaneous CHF. The objectives of this retrospective study were therefore to describe the therapeutic use of oral torasemide in cats with spontaneous CHF, document its potential adverse effects while reporting the clinical course of this feline population following torasemide administration in addition to standard medical therapy. Results Medical records of 21 client-owned cats with CHF (median age = 10.6 years [interquartile range (IQR) = 6.5–11.2]) receiving torasemide were reviewed. Data collected included torasemide dosages, other concurrent medications, physical examination features, echocardiographic data, and potential adverse effects during follow-up. A survival analysis was performed to estimate the time from diagnosis to cardiac death. Dyspnea related to CHF was identified in all cats (pleural effusion [8/21], pulmonary edema [5/21] or both [8/21]), associated with ascites in 4/21 cats. The CHF cause was determined by echocardiography in all cats: hypertrophic (n = 10), restrictive (n = 6), arrhythmogenic right ventricular (n = 3), dilated (n = 1) cardiomyopathies, and aortic valve abnormality (n = 1). At initiation, median torasemide dosage was 0.21 mg/kg [IQR = 0.17–0.23] q24h. Clinical signs declined in most cats (20/21) during the first 2 weeks with no remarkable adverse events. Median survival time after torasemide prescription was 182 days [IQR = 46–330]. A contemporary control group including 54 cats with CHF, receiving furosemide as sole loop diuretic treatment was compared with the study group. Median (IQR) survival time of cats in the control group was not significatively different (p = 0.962) from that of the torasemide group, i.e., 148 days (9–364), although the torasemide group included significantly more cats with recurrent episodes of CHF (52%) that the control group (19%). Conclusions This case series demonstrates that torasemide can be used in cats with spontaneous CHF. This therapeutic interest needs to be confirmed by prospective clinical trials.

9. Combination Approach to Tear Trough Deformities and Under Eye Appearance Using Microfocused Ultrasound With Visualization and Hyaluronic Acid Filler

Abstract: Filler for tear trough deformities and improved under eye appearance has become a well-accepted procedure to treat fine lines, wrinkles, shadowing, darkness, and hollowness. Microfocused ultrasound with visualization–induced skin tightening can be a powerful modality to treat periocular laxity on its own. There is also considerable rationale for combining these 2 modalities for synergistic rejuvenation of the entire periocular area. This case series recruited participants with mild-to-moderate skin laxity in the periocular area and tear trough deformities. Participants received microfocused ultrasound with visualization therapy followed 3 months later by injection with Cohesive Polydensified Matrix hyaluronic acid gel. Photographs were taken at baseline, 3 months after microfocused ultrasound with visualization therapy, and 1 month after Cohesive Polydensified Matrix hyaluronic acid injection for rating by physicians and participants using the Global Aesthetic Improvement Scale. All participants also completed a satisfaction questionnaire. This pilot study enrolled 10 participants ranging in age from 35 to 55 years. After microfocused ultrasound with visualization alone, physicians rated 4 participants “much improved,” 5 “improved,” and 1 with “no change.” After participants had received both treatment modalities, physicians rated 9 participants as “very much improved” and 1 as “much improved.” Participant satisfaction was high with microfocused ultrasound with visualization alone and even higher with the combined treatment approach with all 10 participants reporting their periocular appearance “very much improved” at the end of the study. No treatment complications were observed. In conclusion, microfocused ultrasound with visualization in combination with Cohesive Polydensified Matrix hyaluronic acid gel was effective at improving appearance of the eye area in participants with periocular skin laxity and tear trough deformities.

Abstract: To investigate the present attitude of the Italian Radiation Oncologists in the management of breast cancer (BC) concerning hypofractionated radiotherapy (hRT), partial-breast irradiation (PBI), re-irradiation (rRT) and radiotherapy after neoadjuvant chemotherapy (post-NAC RT). A nationwide, 21-point questionnaire was distributed online via SurveyMonkey. Seventy-four Italian Radiotherapy Centers answered to the survey. In most cases, the responding centers treated more than 100 BC patients/year between January 2016 and December 2017. Almost half of responding centers (49%) treated patients with hRT, out of these, 95% as routine practice for early-stage BC. Dose prescriptions ranged between 39 and 45 Gy indicating a high use of moderate hRT. The chest wall and regional lymph nodes were irradiated with hRT by 13% and 15% of the responding centers, respectively. PBI was used by 60% of responders, with different techniques. Only 0.6% of participants perform rRT after BC recurrence. Finally, only 11% of the interviewed centers responded to their attitude toward post-NAC RT, which, however, was indicated in 97% of patients after breast-conserving surgery. This survey shows a fairly good use of hRT and a moderate practice of PBI in Italy. Some practices like hRT to the chest wall and regional lymph nodes as well as rRT need further verification. Likewise, the management of post-NAC RT is very heterogeneous. Future national clinical collaborative studies are advocated in order to investigate these controversial topics about breast cancer radiotherapy.

Abstract: Background/Aim: Despite the large amount of clinical data available of Coronavirus-19 (COVID-19), not many studies have been conducted about the psychological toll on Health Care Workers (HCWs). Patients and Methods: In this multicentric descriptive study, surveys were distributed among 4 different Breast Cancer Centers (BCC). BCCs were distinguished according to COVID-19 tertiary care hospital (COVID/No-COVID) and district prevalence (DP) (High vs. Low). DASS-21 score, PSS score and demographic data (age, sex, work) were evaluated. Results: A total of 51 HCWs were analyzed in the study. Age, work and sex did not demonstrate statistically significant values. Statistically significant distribution was found between DASS-21-stress score and COVID/No-COVID (p=0.043). No difference was found in the remaining DASS-21 and PSS scores, dividing the HCWs according to COVID-19-hospital and DP. Conclusion: Working in a COVID-19-hospital represents a factor that negatively affects psychosocial well-being. However, DP seems not to affect the psychosocial well-being of BCC HCWs. During the outbreak, psychological support for low risk HCWs should be provided regardless DP.

Abstract: Background/Aim: In 2016, in the United States, more than 50% of breast cancer (BC) cases were diagnosed in patients older than 60 years of age. Our study aimed to estimate the risk of locoregional recurrence (LR) in patients who underwent breast-conservative treatment (BCT), according to age. Patients and Methods: This retrospective monocentric study analyzed 613 cases of patients who underwent BCT between 2003 and 2014. Patients were divided into groups according to age: Under70 (under 70 years old) and Over70 (above 70 years old). Margins width, histology results, prognostic and predictive factors were compared. Abstract: This report summarizes the contributions presented at the IAEA technical meeting on plasma disruptions and their mitigation, held virtually, 20–23 July 2020. The meeting brought together more than 120 experts from nuclear fusion research sites worldwide to discuss experimental, theoretical and modelling work in the field of plasma disruptions with special emphasis on developing a solid basis for possible disruption mitigation strategies in ITER and next generation fusion devices. The main topics of the meeting were: (i) disruption consequences, including electromagnetic loads, heat loads, and runaway electrons; (ii) disruption prediction and avoidance, including machine learning and physics-based approaches, and control aspects; and (iii) disruption mitigation, including shattered pellet injection, alternative techniques and general aspects of disruption mitigation.

Abstract: The Italian research project PROSIB (PROpulsione e Sistemi IBridi per velivoli ad ala fissa e rotante), is a 30-month initiative funded by the Italian Ministry of University and Scientific Research (MIUR) and coordinated by the Leonardo company. The project is aimed to investigate configurations for regional aircraft and rotary wing platforms and architectures for propulsion systems, and is dedicated to the identification of the best strategy for their use, given different on-board energy sources. The reduced environmental impact is the key for the success of the new hybrid/electric aircraft configurations. This not only considers the chemical pollution introduced in the atmosphere, but also the noise produced on the surrounding area of airports. The present paper describes the acoustic impact assessment resulting from the inclusion of new propulsion technologies and new configurations of regional aircraft (ATR42 pax) in a reference airport area.

Abstract: Simple Summary In the tumor microenvironment, interaction among tumor cells, immune cells, stromal cells, and the extracellular matrix is vital to support pro-tumor mechanisms such as drug resistance and metastases. Malignant pleural mesothelioma has a unique and complex tumor microenvironment. Several reports underlined the key role of immune and stromal cells in tumorigenesis and progression of mesothelioma. These non-cancer cells, via a reciprocal informational exchange with tumor cells, established a chronic inflammatory microenvironment that support the malignancy and the chemoresistant phenotype of the tumor. The knowledge of the cellular and molecular mechanisms underlying tumor microenvironment interconnection was recently considered a crucial point for the design of more effective therapeutic strategies. In this review, we summarize the molecular mechanisms by which stroma and immune cells support the malignancy of mesothelioma and their potential therapeutic targeting. Abstract Several studies have reported that cellular and soluble components of the tumor microenvironment (TME) play a key role in cancer-initiation and progression. Considering the relevance and the complexity of TME in cancer biology, recent research has focused on the investigation of the TME content, in terms of players and informational exchange. Understanding the crosstalk between tumor and non-tumor cells is crucial to design more beneficial anti-cancer therapeutic strategies. Malignant pleural mesothelioma (MPM) is a complex and heterogenous tumor mainly caused by asbestos exposure with few treatment options and low life expectancy after standard therapy. MPM leukocyte infiltration is rich in macrophages. Given the failure of macrophages to eliminate asbestos fibers, these immune cells accumulate in pleural cavity leading to the establishment of a unique inflammatory environment and to the malignant transformation of mesothelial cells. In this inflammatory landscape, stromal and immune cells play a driven role to support tumor development and progression via a bidirectional communication with tumor cells. Characterization of the MPM microenvironment (MPM-ME) may be useful to understand the complexity of mesothelioma biology, such as to identify new molecular druggable targets, with the aim to improve the outcome of the disease. In this review, we summarize the known evidence about the MPM-ME network, including its prognostic and therapeutic relevance.

Abstract: [EN] The KM3NeT research infrastructure being built at the bottom of the Mediterranean Sea will host water-Cherenkov telescopes for the detection of cosmic neutrinos. The neutrino telescopes will consist of large volume three-dimensional grids of optical modules to detect the Cherenkov light from charged particles produced by neutrino-induced interactions. Each optical module houses 31 3-in. photomultiplier tubes, instrumentation for calibration of the photomultiplier signal and positioning of the optical module, and all associated electronics boards. By design, the total electrical power consumption of an optical module has been capped at seven Watts. Abstract: Supplemental Digital Content is available in the text., Background: Current paradigms suggest that nitric oxide (NO) produced by endothelial cells (ECs) through endothelial nitric oxide synthase (eNOS) in the vessel wall is the primary regulator of blood flow and blood pressure. However, red blood cells (RBCs) also carry a catalytically active eNOS, but its role is controversial and remains undefined. This study aimed to elucidate the functional significance of RBC eNOS compared with EC eNOS for vascular hemodynamics and nitric oxide metabolism. Methods: We generated tissue-specific loss- and gain-of-function models for eNOS by using cell-specific Cre-induced gene inactivation or reactivation. We created 2 founder lines carrying a floxed eNOS (eNOSflox/flox) for Cre-inducible knockout (KO), and gene construct with an inactivated floxed/inverted exon (eNOSinv/inv) for a Cre-inducible knock-in (KI), which respectively allow targeted deletion or reactivation of eNOS in erythroid cells (RBC eNOS KO or RBC eNOS KI mice) or in ECs (EC eNOS KO or EC eNOS KI mice). Vascular function, hemodynamics, and nitric oxide metabolism were compared ex vivo and in vivo. Results: The EC eNOS KOs exhibited significantly impaired aortic dilatory responses to acetylcholine, loss of flow-mediated dilation, and increased systolic and diastolic blood pressure. RBC eNOS KO mice showed no alterations in acetylcholine-mediated dilation or flow-mediated dilation but were hypertensive. Treatment with the nitric oxide synthase inhibitor Nγ-nitro-l-arginine methyl ester further increased blood pressure in RBC eNOS KOs, demonstrating that eNOS in both ECs and RBCs contributes to blood pressure regulation. Although both EC eNOS KOs and RBC eNOS KOs had lower plasma nitrite and nitrate concentrations, the levels of bound NO in RBCs were lower in RBC eNOS KOs than in EC eNOS KOs. Reactivation of eNOS in ECs or RBCs rescues the hypertensive phenotype of the eNOSinv/inv mice, whereas the levels of bound NO were restored only in RBC eNOS KI mice. Conclusions: These data reveal that eNOS in ECs and RBCs contribute independently to blood pressure homeostasis.

Abstract: This paper concerns a high fidelity computational approach for the prediction of the acoustic impact of a VTOL vehicle operating in a urban area. The aerodynamic problem is solved with a free-wake vortex lattice boundary element method whereas the challenging acoustic scattering problem by a large urban area is carried out by coupling a Ffowcs Williams and Hawkings approach (FW-H) with a Boundary Element Method (BEM), accelerated with an Adaptive black-box Fast Multipole Method (AbbFMM). This work also demonstrates the set-up of a comprehensive numerical chain for assessing the noise of vertical take-off and landing (VTOL) vehicle during their whole operations meanwhile producing highly accurate noise shielding predictions when they are approaching in proximity of the urban areas.

Abstract: SummaryObjectives Malignant pleural mesothelioma (MPM) is an occupational disease mainly due to asbestos exposure. Effective therapies for MPM are lacking, making this tumour type a fatal disease. Materials and Methods In order to meet this need and in view of a future “drug repositioning” approach, here we screened five MPM (Mero-14, Mero-25, IST-Mes2, NCI-H28 and MSTO-211H) and one SV40-immortalized mesothelial cell line (MeT-5A) as a non-malignant model, with a library of 1170 FDA-approved drugs. Results Among several potential compounds, we found that fludarabine (F-araA) and, to a lesser extent, risedronic acid (RIS) were cytotoxic in MPM cells, in comparison to the non-malignant Met-5A cells. In particular, F-araA reduced the proliferation and the colony formation ability of the MPM malignant cells, in comparison to the non-malignant control cells, as demonstrated by proliferation and colony formation assays, in addition to measurement of the phospho-ERK/total-ERK ratio. We have shown that the response to F-araA was not dependent upon the expression of DCK and NT5E enzymes, nor upon their functional polymorphisms (rs11544786 and rs2295890, respectively). Conclusion This drug repositioning screening approach has identified that F-araA could be therapeutically active against MPM cells, in addition to other tumour types, by inhibiting STAT1 expression and nucleic acids synthesis. Further experiments are required to fully investigate this.

Abstract: We propose a Near Infra Red photon emitting 4H-SiC compact vertical pn diode. The “as fabricated” diode exhibits a strong visible emission at 500 nm associated with the intrinsic and/or processing induced D1 centers in the semiconductor. Thanks to opportune He+ ion irradiation, we are able to obtain color centers in the junction region responsible for photon emission around 900 nm while simultaneously quenching unwanted visible emission. Electro-optical investigation was conducted by Electro-Luminescence measurements in the 300 nm −1100 nm range and by Emission Microscopy, both with pumping a current between of 0.1 and 2 mA. The proportionality of the emission intensity to the pumping current opens the interesting perspective of engineering the concentration of defects with the aim of obtaining single defects and thus single photon source devices. This is of considerable interest in low-power proximity sensors for consumer, industrial, automotive and telecommunications applications.

23. Tonal and broadband noise calculations for aeroacoustic optimization of a pusher propeller

Abstract: A multidisciplinary analysis and optimization is carried out for a propeller in a real pusher aircraft configuration with the goal of reducing the radiated noise power levels, while preserving the aerodynamic efficiency. The optimization process involves the shape of the blade and the position of the engine exhaust ducts. A coupling of the unsteady aerodynamic and structural-dynamic blade models provides the aeroelastic propeller model that drives a tonal and broadband aeroaeonstic prediction. The tonal noise results from the periodic flow unsteadiness due to the nonaxial flight and to the impingement of the engine exhausts on the propeller disk. The broadband noise is mainly due to the interaction between the blade leading edge and the exhaust turbulence. It is shown that the tonal noise overwhelms the broadband noise and that the optimization affects the shape of the blade at the tip and in the spanwise segment hit by the exhausts. An overall sound pressure level reduction of 3.5 dB is achieved at the takeoff condition, while preserving the design propeller thrust and resulting in a small penalty on the propeller efficiency in cruise. 24. Hybrid empirical/computational aeroacoustics methodology for rocket noise modeling

Abstract: Sound pressure level contours and spectral sound distributions around the Vega launch system are computed by means of a standard empirical model developed by NASA and a novel hybrid empirical/computational aeroacoustics approach. The empirical prediction is validated against available noise spectra measured on a scaled mock-up, showing a fairly good agreement when standard values for the model constants are used. The jet sources employed in the empirical prediction are then used to convolute a database of tailored Green's functions computed by means of a frequency-domain computational aeroacoustics code. This second approach allows the consideration of the real launch pad geometry and, in the limit of the grid resolution drawn by the acoustic wavelength, the mean flow refraction effects in the jet stream. The mean flow is supplied by a FLUENT simulation and projected onto the computational aeroacoustics grid. This is a Cartesian grid generated automatically and used in connection with an immersed-boundary approach. It is shown that the hybrid empirical/computational aeroacoustics prediction is able to explain some discrepancies observed between the experimental data and the empirical predictions. In addition, it generates a basis of elementary fields to be used in a source localization method under development in the framework of the Innovative Aerothermodynamic Configurations for Space Transport Systems research project, funded by the Italian Space Agency. Abstract: The NEMO collaboration proposes to build an underwater neutrino telescope located South-East off the Sicily coast. This paper describes the concepts underlying the communication link design going over the whole data acquisition and transport from the front-end electronics to the module sending data on-shore through a fiber optic link which relies on Dense Wavelength Division Multiplexing. An on-shore board, plugged into a PC, extracts and distributes data both to first-level trigger and control systems. Underwater apparatus monitoring and controls are guaranteed by oceanographic instruments and dedicated sensors, whose data are packed and sent back to shore using the same optical link. The communication is fully bidirectional, allowing transmission of timing and control commands. The architecture described here provides a complete real-time data transport layer between the onshore laboratory and the underwater detector. During winter 2006 a first prototype of the apparatus has been deployed: calibration results from the currently working system are here reported. 26. Data Digitalization and Smart Workflows Provides a Powerful Asset Management Optimization Tool in Margarita Field, Bolivia

Abstract: This paper explores the results of Repsol Bolivia's successful implementation of a workflow-based solution for real-time production surveillance as part of a digital oilfield initiative on Margarita Field. We will examine the fundamental aspects of data integration across multiple disciplines in the asset and the impact generated when having all the needed information available to rule-based processes in the same environment. This project successfully incorporated the main aspects of the operations. The implementation complements Repsol's Digital Oilfield initiative, started in 2013, with hardware installations for automation of wells, plants and other operational equipment. This paper will discuss the value of an integrated platform (data-processes-people) and the impact on the decision-making process. Margarita Field produces from ten high-volume gas wells that yield 30% of the gas production in Bolivia. This project targeted real-time well integrity, and operational monitoring to ensure stable production levels while automating a broad suite of best practices to improve efficiency for engineers, avoid production deferments, and enhance safety. This paper details how to leverage data in real-time and fully benefit from it by applying smart engineering workflows designed to integrate modeling tools, data, and engineering best practices. Prior to this implementation, it was demonstrated that a significant portion of the asset engineers’ time was spent simply gathering data from multiple sources. The previously installed, field data automation and real-time feeds, generated significant analytical benefits. However, the increase in useful data was underutilized and limited by the existing spreadsheets. These activities were manual and error-prone, and routinely led to: data duplications, data inaccuracies, and, most importantly, to delayed or suboptimal decisions based on incomplete or inaccurate information. The new solution addresses these challenges by providing an integrated environment where engineers have all the information and workflows readily available and automatically executed. To ensure the workflow automation did not mask problems or simply automate error-prone outcomes, validation gates were established to help ensure the engineers that the results were robust and sufficiently checked for quality. Automated, system-based reporting, helped to eliminate weekly manual reporting efforts, support the QC, and eliminate man time previously required to support operational meetings and decisions. The workflows are designed to maintain evergreen models to support the asset surveillance process. "Live links" to all sources ensures data duplication is no longer an issue and helps to establish "one source of the truth". The workflow-based automation has brought the various, pre-existing, well, production network, and reservoir modeling tools "Online" so they can be incorporated into daily operational surveillance activities and decision-making.

27. Multidisciplinary Day Surgery Unit: a Health Technology Assessment

Abstract: Issue The aim of this study was to improve effectiveness, efficiency and quality of healthcare provided in Day Surgery (DS) setting at “Villa Malta” Hospital in Sarno (Italy). Health Technology Assessment (HTA) was used to compare two organisational models: the first one, in force at the beginning of the study, was characterized by a decentralisation of care activities at the individual Surgery Units; the other one provided for a centralisation of care activities in one unit, named the Multidisciplinary Day Surgery Unit (MDSU). Description of the Problem We evaluated economical (cost-minimisation analysis), performance (effectiveness, efficiency, appropriateness, quality and safety) and ethical-social aspects. The cost-minimisation analysis took into account costs incurred and revenues received (DRG-based reimbursement). The perceived quality and the ethical and social impacts were assessed by reference to the voluntary interruption of pregnancy (VIP) and the increasing offer of the Otorhinolaryngology services. Results MDSU has resulted in a net gain of €1,261,097 per year. This is a result of the increase in hospitalisation production of €1,235,869 and the savings on staff costs of €30,476. The new model also shown an improvement in efficiency through standardisation of the number of DS admissions and in perceived quality concerning VIP, ensuring greater privacy and humanisation for the patients. Finally, the introduction of the Otorhinolaryngology services in DS has increased the quality of healthcare on offer, without the need to transfer the surgical candidates to other hospitals. Lessons The centralised model has shown definite advantages over the decentralised one. MDSU was adopted to provide DS services at “Villa Malta” Hospital and this organisational model could be chosen by other hospitals to improve quality of healthcare. Key messages MDSU has advantages especially in economic, ethical and social terms. MDSU increases hospitalisation production, improves humanisation of patients and increases quality of healthcare on offer.

Abstract: Background Inadvertent intraarterial injection of soft tissue fillers during facial aesthetic procedures can result in serious adverse events including visual impairment and blindness. Once the retinal artery has been occluded, only a short window of opportunity exists before blindness becomes irreversible. All physicians should be prepared for the eventuality of intraarterial injection, despite its rarity. Objective The aim of this document is to provide a simple and evidence-based protocol using the easy to remember acronym EYE-CODE: EYE (I call retinal referral center), C (check vision), O (optic nerve function), D (decrease intraocular pressure), and E (erase filler). Methods The EYE-CODE acronym incorporates 2 key components: (1) a systematic office-based protocol to first determine whether vision loss is present and to what extent and (2) a treatment strategy that can be started in the acute office setting and continued by an emergency ophthalmologist. Results The protocol incorporates a crash kit of treatments readily available to an aesthetic physician combining measures to rapidly reduce intraocular pressure to allow the emboli to dislodge downstream with measures to improve retinal perfusion. Conclusion EYE-CODE provides an up-to-date, one-stop reference for appropriate management of retinal artery occlusion induced by injection of soft tissue fillers.

Abstract: RationalePremature infants exposed to oxygen are at risk for bronchopulmonary dysplasia (BPD), which is characterised by lung growth arrest. Inflammation is important, but the mechanisms remain elusive. Here, we investigated inflammatory pathways and therapeutic targets in severe clinical and experimental BPD.Methods and resultsFirst, transcriptomic analysis with in silico cellular deconvolution identified a lung-intrinsic M1-like-driven cytokine pattern in newborn mice after hyperoxia. These findings were confirmed by gene expression of macrophage-regulating chemokines (Ccl2, Ccl7, Cxcl5) and markers (Il6, Il17A, Mmp12). Secondly, hyperoxia-activated interleukin 6 (IL-6)/signal transducer and activator of transcription 3 (STAT3) signalling was measured in vivo and related to loss of alveolar epithelial type II cells (ATII) as well as increased mesenchymal marker. Il6 null mice exhibited preserved ATII survival, reduced myofibroblasts and improved elastic fibre assembly, thus enabling lung growth and protecting lung function. Pharmacological inhibition of global IL-6 signalling and IL-6 trans-signalling promoted alveolarisation and ATII survival after hyperoxia. Third, hyperoxia triggered M1-like polarisation, possibly via Krüppel-like factor 4; hyperoxia-conditioned medium of macrophages and IL-6-impaired ATII proliferation. Finally, clinical data demonstrated elevated macrophage-related plasma cytokines as potential biomarkers that identify infants receiving oxygen at increased risk of developing BPD. Moreover, macrophage-derived IL6 and active STAT3 were related to loss of epithelial cells in BPD lungs.ConclusionWe present a novel IL-6-mediated mechanism by which hyperoxia activates macrophages in immature lungs, impairs ATII homeostasis and disrupts elastic fibre formation, thereby inhibiting lung growth. The data provide evidence that IL-6 trans-signalling could offer an innovative pharmacological target to enable lung growth in severe neonatal chronic lung disease.

Abstract: BACKGROUND Hollowing of the temple area is an early sign of aging that is often overlooked, but when corrected, can lead to significant subject and physician satisfaction. AIMS To assess physician and subject awareness of the impact of temporal hollowing on appearance and to evaluate treatment satisfaction following injection of calcium hydroxylapatite with integral lidocaine (CaHA(+)). PATIENTS/METHODS Subjects with temporal depression grade ≥2 on a validated Temple Hollowing Scale were recruited. CaHA(+) was injected supraperiosteally using a bolus technique. Photographs were taken prior to treatment and 30 days, and graded by three evaluating physicians using the Global Aesthetic Improvement Scale (GAIS) and Temple Hollowing Scale. The same physicians estimated subjects' age before and after treatment. Photographs were graded by patients as part of a satisfaction questionnaire. RESULTS The study enrolled 10 subjects aged 32-68 years old. Physician GAIS scores of subject appearance at 1 month were rated as "very much improved" in 80% and "much improved" in 20% of subjects. Temple hollows were graded as 1 after treatment in 90% of subjects. Following blinded review of patients' true ages, estimates for subjects' perceived age were on average 4 years younger than their actual age. Most subjects had not been aware of temple hollowing prior to treatment, but nine out of 10 were very to extremely satisfied with results, and all were willing to repeat treatment. No treatment complications were observed. CONCLUSION Treatment of the temple area can have a dramatic aesthetic effect and should not be neglected when addressing facial rejuvenation. CaHA(+) is an effective and well-tolerated product for use in the temple area and is associated with a high degree of subject and physician satisfaction.

Abstract: Proton pump inhibitors (PPIs) are widely used for acid suppression in the treatment and prevention of many conditions, including gastroesophageal reflux disease, gastric and duodenal ulcers, erosive esophagitis, Helicobacter pylori infection, and pathological hypersecretory conditions. Most PPIs are metabolized primarily by cytochrome P450 2C19 (CYP2C19) into inactive metabolites, and CYP2C19 genotype has been linked to PPI exposure, efficacy, and adverse effects. We summarize the evidence from the literature and provide therapeutic recommendations for PPI prescribing based on CYP2C19 genotype (updates at www.cpicpgx.org). The potential benefits of using CYP2C19 genotype data to guide PPI therapy include (i) identifying patients with genotypes predictive of lower plasma exposure and prescribing them a higher dose that will increase the likelihood of efficacy, and (ii) identifying patients on chronic therapy with genotypes predictive of higher plasma exposure and prescribing them a decreased dose to minimize the risk of toxicity that is associated with long-term PPI use, particularly at higher plasma concentrations.

Abstract: International audience; Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), which originated in Wuhan, China, in 2019, is responsible for the COVID-19 pandemic. It is now accepted that the wild fauna, probably bats, constitute the initial reservoir of the virus, but little is known about the role pets can play in the spread of the disease in human communities, knowing the ability of SARS-CoV-2 to infect some domestic animals. In this cross-sectional study, we tested the antibody response in a cluster of 21 domestic pets (9 cats and 12 dogs) living in close contact with their owners (belonging to a veterinary community of 20 students) in which two students tested positive for COVID-19 and several others (n = 11/18) consecutively showed clinical signs (fever, cough, anosmia, etc.) compatible with COVID-19 infection. Although a few pets presented many clinical signs indicative for a coronavirus infection, no antibodies against SARS-CoV-2 were detectable in their blood one month after the index case was reported, using an immunoprecipitation assay. These original data can serve a better evaluation of the host range of SARS-CoV-2 in natural environment exposure conditions.

Abstract: Background Major brain injury and uncontrolled blood loss remain the primary causes of early trauma-related mortality. One-quarter to one-third of trauma patients exhibit trauma-induced coagulopathy (TIC). Thromboelastometry (ROTEM) and thrombelastography (TEG) are valuable alternatives to standard coagulation testing, providing a more comprehensive overview of the coagulation process. Purpose Evaluating thromboelastographic profile, the incidence of fibrinolysis (defined as Ly30 > 3%) in severe trauma patients, and factors influencing pathological coagulation pattern. Methods Prospective observational 2 years cohort study on severe trauma patients assisted by Helicopter Emergency Medical System (HEMS) and Level 1 Trauma Center, in a tertiary referral University Hospital. Results Eighty three patients were enrolled, mean NISS (new injury severity score) 36 (± 13). Mean R value decreased from 7.25 (± 2.6) to 6.19 (± 2.5) min (p R from T0 to T1. In NISS 25–40 and NISS > 40 groups, changes in R value increased their significance (p = 0.04 and p T0 and 74 (92.5%) at T1. Hypercoagulation was present in 57 (71.3%) patients at T0, and in 66(82.5%) at T1. 9 (11.3%) patients had hyperfibrinolysis at T0, 7 (8.8%) patients at T1. Prevalence of StO2 T0 was greater in patients whose TEG worsened (7 patients, 46.7%) against whose TEG remained stable or improved (8 patients, 17.4%) from T0 to T1 (p = 0.02). 48 (57.8%) patients received T1 (p T0, nor other TEG pattern. Conclusion Our population is representative of a non-hemorrhagic severe injury subgroup. Almost all of our trauma population had coagulation abnormalities immediately after the trauma; pro-coagulant changes were the most represented regardless of the severity of injury. NISS appears to affect only R parameter on TEG. Hyperfibrinolysis has been found in a low percentage of patients. Hypoperfusion parameters do not help to identify patients with ongoing coagulation impairment. Small volume resuscitation and mild hypotermia does not affect coagulation, at least in the early post-traumatic phase.

Abstract: Elderly breast cancer patients are frequently affected by significant comorbidities that make sophisticated radiotherapy treatments particularly challenging. We dosimetrically analyzed two different simple free-breathing external beam radiotherapy (EBRT) techniques for the hypofractionated treatment of the left breast in elderly patients with a low compliance, to compare target coverage, and heart and left anterior descending coronary artery (LADCA) sparing. We developed radiation plans for 24 elderly patients using 3D conformal (3DCRT) field-in-field tangential technique and intensity-modulated (IMRT) tangential beam technique. Dose-Volume-Histograms (DVHs) were used to provide a quantitative comparison between plans. The median breast volume was 645 cm3. IMRT and 3DCRT plans comparison demonstrated no significant differences in terms of organ sparing for the heart. Regarding LADCA, mean dose (10.3 ± 9.5 Gy vs 11.9 ± 9.6 Gy, p = 0.0003), maximum dose (26.1 ± 16.1 Gy vs 29.1 ± 16.1 Gy, p = 0.004) and V17 Gy (21.5% ± 26.9% vs 25.0% ± 27.2%, p = 0.002) significantly decreased using IMRT compared with 3DCRT. IMRT plans showed a better target coverage compared with 3DCRT (0.91 ± 0.05 vs 0.93 ± 0.04, p = 0.05). Comparing the two different EBRT techniques, we demonstrated few, although substantial, dosimetric differences in terms of doses to the organs at risk characterized by a statistically significant dose reduction of LADCA in the IMRT plans. Elderly patients with a low compliance to treatment might benefit from 3DCRT with field-in-field tangential arrangement or from a simple IMRT approach. IMRT should be preferred.

Abstract: PAMELA is a satellite-borne experiment that will study the antiproton and positron fluxes in cosmic rays in a wide range of energy (from 80 MeV up to 190 GeV for antiprotons and from 50 MeV up to 270 GeV for positrons) and with high statistics, and that will measure the antihelium/helium ratio with a sensitivity of the order of [10.sup.-8]. The detector will fly on-board a polar orbiting Resurs DK1 satellite, which will be launched into space by a Soyuz rocket in 2004 from Balkonur eosmodrome in Kazakhstan, for a 3-year-long mission. Particle identification and energy measurements are performed in the PAMELA apparatus using the following subdetectors: a magnetic spectrometer made up of a permanent magnet equipped with double-sided microstrip silicon detectors, an electromagnetic imaging calorimeter composed of layers of tungsten absorber and silicon detectors planes, a transition radiation detector made of straw tubes interleaved with carbon fiber radiators, a plastic scintillator time-of-flight and trigger system, a set of anticounter plastic scintillator detectors, and a neutron detector. Abstract: The VSiPMT (Vacuum Silicon PhotoMultiplier Tube) is an innovative design for a hybrid photodetector. The idea, born with the purpose to use a SiPM for large detection volumes, consists in replacing the classical dynode chain with a special SiPM. In this configuration, we match the large sensitive area of a photocathode with the performances of the SiPM technology, which therefore acts like an electron detector and so like a current amplifier. The excellent photon counting capability, fast response, low power consumption and the stability are among the most attractive features of the VSiPMT.We now present the progress on the realization of a 1-in. prototype and the preliminary tests we are performing on it. © 2017 Elsevier B.V.

Abstract: Background: Results from the BEVA2007 trial, suggest that the metronomic chemotherapy regimen with dose-fractioned cisplatin and oral etoposide (mPE) +/− bevacizumab, a monoclonal antibody to the vascular endothelial growth factor (VEGF), shows anti-angiogenic and immunological effects and is a safe and active treatment for metastatic non-small cell lung cancer (mNSCLC) patients. We carried out a retrospective analysis aimed to evaluate the antitumor effects of this treatment in a subset of patients with squamous histology. Methods: Retrospective analysis was carried out in a subset of 31 patients with squamous histology enrolled in the study between September 2007 and September 2015. All of the patients received chemotherapy with cisplatin (30 mg/sqm, days 1–3q21) and oral etoposide (50 mg, days 1–15q21) (mPE) and 14 of them also received bevacizumab 5 mg/kg on the day 3q21 (mPEBev regimen). Results: This treatment showed a disease control rate of 71% with a mean progression free survival (PFS) and overall survival (OS) of 13.6 and 17 months respectively. After 4 treatment courses, 6 patients showing a remarkable tumor shrinkage, underwent to radical surgery, attaining a significant advantage in term of survival (P=0.04). Kaplan-Meier and log-rank test identified the longest survival in patients presenting low baseline levels in neutrophil-to-lymphocyte ratio (NLR) (P=0.05), interleukin (IL) 17A (P=0.036), regulatory-T-cells (T reg s) (P=0.020), and activated CD83+ dendritic cells (DCs) (P=0.03). Conclusions: These results suggest that the mPE +/− bevacizumab regimen is feasible and should be tested in comparative trials in advanced squamous-NSCLC (sqNSCLC). Moreover, its immune-biological effects strongly suggest the investigation in sequential combinations with immune check-point inhibitors.

Abstract: Photon detection is a key factor to study many physical processes in several areas of fundamental physics research. Focusing the attention on photodetectors for particle astrophysics, we understand that we are very close to new discoveries and new results. In order to push the progress in the study of very high-energy or extremely rare phenomena (e.g. dark matter, proton decay, neutrinos from astrophysical sources) the current and future experiments require additional improvements in linearity, gain, quantum efficiency and single photon counting capability. To meet the requirements of these classes of experiments, we propose a new design for a modern hybrid photodetector: the VSiPMT (Vacuum Silicon PhotoMultiplier Tube). The idea is to replace the classical dynode chain of a PMT with a SiPM, which therefore acts as a single stage Geiger electron detector and amplifier, without statistical fluctuations. The aim is to match the large sensitive area of a photocathode with the performances of the SiPM technology. The previous VSiPMT prototypes already showed many attractive features such as low power consumption, very large dynamic range, excellent photon counting capability and low voltage driven gain. We now present the results of the full characterization of the latest and largest version achieved up to now, a 2-inches VSiPMT manufactured by Hamamatsu.

Abstract: The VSiPMT (Vacuum Silicon PhotoMultiplier Tube) is an original design for an innovative light detector we proposed with the aim to create new scientific instrumentation for future missions of exploration and observation of the universe. The idea behind this device is to replace the classical dynode chain of a photomultiplier tube with a silicon photomultiplier, the latter acting as an electron detector and amplifier. In this way we obtain a large area photodetector with an excellent photon counting, proper of the SiPMs, but with the dark noise of only one SiPM (1-inch is equivalent to ∼ 54 SiPM 3 × 3 mm2). From this point of view, the VSiPMT offers very attractive features and unprecedented performance in large area detection, such as: negligible power consumption, excellent SPE resolution, easy low-voltage-based stabilization and very good time performance. Hamamatsu realized for our group a 1-inch prototype. The results of the full characterization of the device are presented in this work.

Abstract: We have newly developed a Hybrid Photo-Detector (HPD) with 2-inch diameter. It consists of a bialkali photocathode and a 3 mm x 3 mm Multi-Pixel-Photon Counter (MPPC) sealed in a glass tube. The electric field inside the tube is designed so that photoelectrons emitted from the photocathode are collected in the MPPC, where photoelectrons are amplified. For conventional HPDs with an avalanche diode , extremely high voltage such as -8kV has to be provided to achieve gain sufficient for single photon detection. However, our HPD with MPPC has single photon sensitivity only with lower voltage like PMTs due to high gain of MPPC. In addition, this device can cover larger area with only one MPPC. These features enable us to easily handle high performance HPD in various applications, such as high energy physics, biomedical field etc. This test sample was produced in 2016, and we have checked its basic characteristics. The peak photocathode quantum efficiency was found to be very high of 37% at the wavelength of 340 nm. We have confirmed that HPD with MPPC has capability of single photon detection in low operation voltage less than -2 kV. This report will present detailed performance studies of this sample.

Abstract: [EN] The Hamamatsu R12199-02 3-inch photomultiplier tube is the photodetector chosen for the first phase of the KM3NeT neutrino telescope. About 7000 photomultipliers have been characterised for dark count rate, timing spread and spurious pulses. Abstract: Empirical models such as the Eldred standard model are commonly used for rocket noise prediction. Such models directly provide a definition of the Sound Pressure Level through the quadratic pressure term by uncorrelated sources. In this paper, an improvement of the Eldred Standard model has been formulated. This new formulation contains an explicit expression for the acoustic pressure of each noise source, in terms of amplitude and phase, in order to investigate the sources correlation effects and to propagate them through a wave equation. In particular, the correlation effects between adjacent and not-adjacent sources have been modeled and analyzed. The noise prediction obtained with the revised Eldred-based model has then been used for formulating an empirical/BEM (Boundary Element Method) hybrid approach that allows an evaluation of the scattering effects. In the framework of the European Space Agency funded program VECEP (VEga Consolidation and Evolution Programme), these models have been applied for the prediction of the aeroacoustics loads of the VEGA (Vettore Europeo di Generazione Avanzata – Advanced Generation European Carrier Rocket) launch vehicle at lift-off and the results have been compared with experimental data.

Abstract: Ocean acidification and increased ocean temperature from elevated atmospheric carbon dioxide can significantly influence the physiology, growth and survival of marine organisms. Despite increasing research efforts, there are still many gaps in our knowledge of how these stressors interact to affect economically and ecologically important species. This project is the first to explore the physiological effects of high pCO2 and temperature on the acclimation potential of the purple-hinge rock scallop (Crassadoma gigantea), a widely distributed marine bivalve, important reef builder, and potential aquaculture product. Scallops were exposed to two pCO2 (365 and 1050 μatm) and temperature (14 and 21.5 °C) conditions in a two-factor experimental design. Simultaneous exposure to high temperature and high pCO2 reduced shell strength, decreased outer shell density and increased total lipid content. Despite identical diets, scallops exposed to high pCO2 had higher content of saturated fatty acids, and lower content of polyunsaturated fatty acids suggesting reorganization of fatty acid chains to sustain basic metabolic functions under high pCO2. Metagenomic sequencing of prokaryotes in scallop tissue revealed treatment differences in community composition between treatments and in the presence of genes associated with microbial cell regulation, signaling, and pigmentation. Results from this research highlight the complexity of physiological responses for calcifying species under global change related stress and provide the first insights for understanding the response of a bivalve's microbiome under multiple stressors.

Abstract: Commercial waterborne polyurethane (PU) dispersions, different in chemistry and selected on the basis of eco-friendly components, have been applied to a common polypropylene (PP)-based woven fabric. Impregnation has been chosen as a textile treatment for improving the features of basic technical textiles in light of potential applicability in luggage and bag production. The effect of drying method, performed under conditions achieved by varying the process temperature and pressure, on the features of the treated textiles, has been verified. The prepared specimens were characterized in terms of mechanical behavior (tensile, tear and abrasion resistance) and water resistance (surface wettability and hydrostatic pressure throughout the treated textiles). The experimental results suggest an incremental improvement of the tensile features for all the investigated specimens. For tear strength, no augmentation compared to that of the neat textile, could be verified as a consequence of polyurethane treatment. Remarkable improvements of abrasion resistance were displayed for all the impregnated PP textiles. Benefits in water resistance could be attributed to the presence of hydrophobic PU in the textile weaving of the PP samples. The ultimate improvement in water resistance was dependent on drying conditions.

Abstract: [EN] The KM3NeT research infrastructure, currently under construction in the Mediterranean Sea, will host neutrino telescopes for the identification of neutrino sources in the Universe and for studies of the neutrino mass hierarchy. These telescopes will house hundreds of thousands of photomultiplier tubes that will have to be operated in a stable and reliable fashion. In this context, the stability of the dark counts has been investigated for photomultiplier tubes with negative high voltage on the photocathode and held in insulating support structures made of 3D printed nylon material. Small gaps between the rigid support structure and the photomultiplier tubes in the presence of electric fields can lead to discharges that produce dark count rates that are highly variable. A solution was found by applying the same insulating varnish as used for the high voltage bases directly to the outside of the photomultiplier tubes. Abstract: Photon detection is a key factor to study many physical processes in several areas of fundamental physics research. Focusing the attention on photodetectors for particle astrophysics, the future experiments aimed at the study of very high-energy or extremely rare phenomena (e.g. dark matter, proton decay, neutrinos from astrophysical sources) will require additional improvements in linearity, gain, quantum efficiency and single photon counting capability. To meet the requirements of these class of experiments, we propose a new design for a modern hybrid photodetector: the VSiPMT (Vacuum Silicon PhotoMultiplier Tube). The idea is to replace the classical dynode chain of a PMT with a SiPM, which therefore acts as an electron detector and amplifier. The aim is to match the large sensitive area of a photocathode with the performances of the SiPM technology.
