1. Impact of specific modes of circumferential resection margin involvement in rectal cancer local recurrence: A retrospective study

Abstract

Purpose To compare the local recurrence rate and overall survival between patients with circumferential resection margin (CRM) involvement by direct tumor spread and by a tumor within a lymph node. Methods A total of 776 patients diagnosed with rectal cancer underwent rectal resection. Patients with CRM involvement by direct tumor spread were named group A. CRM involvement by tumor within a lymph node formed group B. Patient data, including sex, age, pT, pN, stage I-III versus IV, neoadjuvant radiotherapy, adjuvant chemotherapy, carcinoembryonic antigen, primary tumor location, lymph node retrieval, and need for abdominoperineal resection, were compared between both groups. Results In total, 10.5% of the patients had CRM involvement. Of these, in 57 cases (7.3%), it was by direct tumor spread and in 19 cases (2.4%) by tumor within a lymph node. Other types of CRM involvement were found in six patients. With a mean follow-up of 32.9 months, 18 patients from group A (31.5%) and one patient from group B (5.2%) suffered a local recurrence. Local recurrence-free survival was significantly higher in patients from group B (P = 0.049). Patients in stage I-III (P = 0.037) and from group B ( P = 0.049) had better overall survival. Conclusion Patients with CRM involvement by tumor within a lymph node have a low risk of local recurrence and better overall survival than patients with CRM involvement by direct tumor spread.

Abstract

There is growing evidence that the frequency and magnitude of extreme weather events may be increasing in conjunction with climate change. This means that many communities will encounter phenomena, such as extreme storm surge events, never before experienced by local residents. The tragic effects of Typhoon Haiyan on the city of Tacloban, Philippines, in November 2013 were attributed, in part, to the inability of routine technical bulletins to communicate the unprecedented nature of the predicted storm surge. In response, the authors construct a relational model of risk communication that suggests that narrative messages that simulate direct face-to-face communication may be more effective in spurring action. Conducting a postevent target audience study in the city of Tacloban, the authors tested the relative effectiveness of narrative-based versus technical message designs on residents who chose not to evacuate during the typhoon. Results show increased effectiveness of the narrative design vis-à-vis intent to evacuate, self-relevance and vividness of the message, and perceived authority of the message source. The study also explored factors behind noncompliance with evacuation advisories. The research supports the relational model, which captures insights from recent research on evacuation and emergency preparedness for extreme hazard events. It supports a broader effort to democratize risk communication and, in so doing, increase people’s sense of agency in preparing for these events.

Abstract

We report on a measurement of the attenuation length for the scintillation light in the tonne size liquid argon target of the ArDM dark matter experiment. The data was recorded in the first underground operation of the experiment in single-phase operational mode. The results were achieved by comparing the light yield spectra from 39Ar and 83mKr to a description of the ArDM setup with a model of full light ray tracing. A relatively low value close to 0.5 m was found for the attenuation length of the liquid argon bulk to its own scintillation light. We interpret this result as a presence of optically active impurities in the liquid argon which are not filtered by the installed purification systems. We also present analyses of the argon gas employed for the filling and discuss cross sections in the vacuum ultraviolet of various molecules in respect to purity requirements in the context of large liquid argon installations.

Abstract

Large size detectors based on Cs2LiYCl6:Ce (CLYC) are capable of performing a combined $\gamma$-ray and neutron spectrometry and constitute a promising technology for a wide range of applications in nuclear and high energy physics. Due to their novelty, the comprehensive characterization of the performance of individual CLYC detectors is of great importance for determining their range of applicability. In this work we report on a wide series measurements performed with a commercial 2"x2" CLYC crystal. Good energy and timing resolution values of 4.7% (@ 662 keV) and 1340 ps (FWHM), respectively, were achieved, and a neutron/$\gamma$ separation figure of merit value of 4.2 was obtained. A dedicated measurement for investigating the intrinsic background of the detector was performed at the Laboratorio Subterr\'aneo de Canfranc (Spain). It evidenced a sizeable contamination in the detector materials which poses limits in the use of CLYC in low background experiments. In addition, detailed Monte Carlo simulations with the GEANT4 toolkit were performed for modeling the response function of the CLYC detector to gamma-rays. Abstract

Introduction: During climacteric, women experience physical and psychological changes due to declining ovarian estrogen. The cardiovascular system is one of those affected. However, there are not sufficient studies of the effects of the climacteric on heart function. The fluctuations of the heart rate are the most common way to analyze the changes in heart function. Materials and Methods: In this work, ten minutes of ECG signal at rest were recorded for a group of 57 healthy climacteric females. In this study, two methodologies were used to analyze the ECG signal: Heart Rate Variability (HRV) and Detrended Fluctuation Analysis (DFA). Results and Discussion: Heart rate variability and detrended fluctuation analysis can be used to monitor changes of cardiovascular system during the climacteric stages (pre-, peri-, and postmenopausal). Both methodologies are sensitive almost at the same level to cardiovascular changes observed during the peri- menopausal stage.

Introduction: Cardiovascular response to acute stress is a widely addressed topic because there is a clinical association of psychological and physical stress with high risk for stroke and cardiovascular events and also with the increase of morbid-mortality of the population. The Heart Rate Variability (HRV) is one of the most common methods of evaluation although some other alternatives have been emerged to complement the information about this response. One of these alternatives is the Detrended Fluctuation Analysis (DFA). Materials and Methods: Here we present a comparison between the results from HRV via the area under low frequency spectrum versus those from DFA via the scaling parameter alpha. Fifty seven healthy women, aging between 40 and 60 years old, were evaluated according to the following paradigm: 1) ECG in basal state (ten min); 2) ECG during psychological stress test (Stroop test, 3 min); and 3) ECG in basal state (three min). Results and Discussion: Heart rate variability, and detrended fluctuation analysis can be used, in short term recordings, to evaluate psychological stress response to Stroop test. HRV and DFA are sensitive to different stages of the paradigm used in this study, being the HRV which gives a fast reaction to the psychological stress, but DFA gives information about a more delayed response, at the last step of the paradigm (resting state).

Abstract

Formation sand production and stimulation proppant flowback are large and costly production problems in maturing assets. Recently, a new coiled-tubing-conveyed proppant treatment (CTCPT) was applied for the operator in the Carr-Lundell field in Webb County, Texas. This technology effectively eliminated the production of formation solids and stimulation proppant. The result yielded increased efficiency in associated production time and decline rates, which have optimized return on investment. The new treatment has also reduced residual operational expenditures by eliminating costly workovers, equipment repair, and lost production caused by this escalating industry problem. Introduction Currently, the oil and gas industry is being stretched and pressured to generate and provide energy for an ever-expanding world market. This increasing need for energy from current and developing countries has put stress on industry companies to complete wells faster and to recover supplementary reserves from pre-existing wells. This situation has influenced operators to look at overall asset production and to increase volume. Production for this new demand will not only come from vast new explorations, but also from examination of maturing fields and assets to streamline maintenance efforts and maximize recovery. (Soliman, East, Gorrell 1999) Today, maturing assets provide more than 70% of production. Yet the production from these assets has already peaked, stabilized, or declined. Maintaining steady production levels involves continuous management and maintenance of these ever changing wells. Mature assets compete with new fields for capital and personnel. These wells must be able to deliver a comparable cost to lift hydrocarbon equivalent with a new asset in early development in a company’s portfolio to receive adequate monetary support for the management and maintenance mentioned. A mature asset faces many challenges that require constant management. These challenges can include depleted pressure regimes, depleted hydrocarbon layers, inefficiently swept zones, and undesired production of water, proppant, formation fines, gas, hydrogen sulfide, and carbon dioxide. These problems not only require additional maintenance procedures but also advanced health, safety, and environmental compliance protocol because of the conditions noted above. These problems associated with maturing wells directly hinder efforts to find more recoverable hydrocarbons, increase production, and improve recovery. Increased operating expenditures and capital expenditures are common with these persistent problems. One of the most assiduous industry problems is solids flowback, which costs operators millions of dollars every year through the loss of production, remedial procedures, and expensive equipment damage (Halliburton 2006). Recent research has shown that a major cause of conductivity loss in a proppant pack is diagenesis, that is, the chemical reaction between the proppant and the formation face that results in scale-like deposits (Top Technology Solutions 2006). These deposits form the basis from which proppant flowback can initiate. The production of formation sand and fines particulates is an expensive area to address with solids production. It is often preceded by water intrusion, where the cementation between formation sand grains can deteriorate, allowing the formation sand and fines to migrate, or produce, with the production fluids. The fines migration often causes formation damage as pore channels or flow paths become plugged with fine particles (Ingram et al. 2006). There are situations where concurrent production of both solids occurs, which just worsens the situation. Normally, fracture-stimulated wells encountering these problems can experience a wellbore filling effect, in which the wellbore essentially becomes filled with proppant and/or formation sand. Abstract

This paper describes the techniques used to create a fully ionized plasma by the multiphoton ionization of gases at pressures of a few Torr to give an initial electron density precision of better than a few percent, necessary for beat‐wave experiments. A gas containment vessel has been constructed using high vacuum techniques with a temperature control system to maintain the fill gas pressure constant. Novel alignment techniques are used that do not require an alignment target placed in the vessel. This involves the use of a rotating pencil beam to determine the focusing of the optics.

9. High resolution beam monitoring with optical transition radiation at 3 MeV electron energy

Abstract

In the framework of the plasma beat wave accelerator experiment at Ecole Polytechnique, high precision position and focussing monitoring of a 3 MeV electron beam is needed. A device is proposed that uses backward optical transition radiation (OTR) from a tilted metal foil held into the beam. For an electron energy of 3 MeV, OTR is emitted within a large solid angle (typical apex angle about 400) around the direction of specular reflection. The design requirements are a high resolution of the imaging optics (/spl ap/10 /spl mu/m), a high sensitivity (/spl ap/ 10 /spl mu/A beam current, not focussed), robustness, and low cost. A prototype has been constructed and successfully tested. A similar device will be used for adjusting a laser focal spot on an electron focal spot, and for monitoring the beam on entry and exit of a gas vessel. >

Abstract

The absolute bioavailability of fenspiride has been studied in twelve healthy volunteers. It was administered IV and orally in single doses of 80 mg fenspiride hydrochloride according to a randomised crossover pattern. Following IV administration, the plasma clearance of fenspiride was about 184 ml.min-1, and its apparent volume of distribution was moderately large (215 l). When given orally as a tablet, fenspiride exhibited fairly slow ab- sorption; the maximum plasma concentration (206 ng.ml-1) was achieved 6 h after administration. The absolute bioavailability was almost complete (90%). The tablet had slow release characteristics. The elimination half-life obtained from the plasma data was 14 to 16 h independent of the route of administration.

Abstract

Human parvovirus B19 has been recognized as the causative agent of a number of cutaneous eruptions, including classic erythema infectiosum or fifth disease; some less well-defined, often purpuric, acute exanthematous eruptions; vasculitis; and the recently described petechial glove-and-sock syndrome. 1,2 On the other hand, acute infection with parvovirus B19 may trigger a number of visceral manifestations. We report a new cutaneous manifestation of parvovirus B19 as a primary infection with a striking generalized livedo reticularis, occurring simultaneously with an impressive myasthenialike muscular weakness. Report of a Case. A 29-year-old woman was first referred to our institution in December 1993 for the evaluation of a recent, generalized livedo reticularis with fever and general alteration in her health. Her medical history was unremarkable, and she denied taking any medication. The disease began about 4 days before her admission to the hospital; it had an acute onset, consisting of a temperature as high as 40°C and chills, rapidly followed by generalized and increasing muscular weakness, and, eventually, by an extensive livedo reticularis. Abstract

An electromagnetic calorimeter has been constructed with a sensitive area of 60 cm × 30 cm and a sensitive thickness of 8.75 radiation-lengths. The detecting-medium is either solid neon or solid argon, contained in a multilayered sandwich of leads plates and printed-circuit-boards. Copper strips on the printed-circuit-boards form anodes which collect the charge released in the solid by the passage of charged particles. These are 40 readout channels with ADC's in CAMAC. The calorimeter, mounted in its own cryostat, has been tested in a mixed particle beam at CERN. It is concluded that the technique is suitable for building a large calorimeter with good resolution on position and energy, together with sampling in depth for electron-pion separation. Such a calorimeter has been proposed for use inside a cryogenic bubble chamber in neutrino experiments.

Abstract

International audience; Objectives: Our aim was to assess if switching from a protease inhibitors (PI)-based regimen to a PI-free one is associated with an increased risk of Kaposi Sarcoma (KS) relapse among patients living with HIV (PLHIV) with history of KS and controlled HIV replication.Methods: In a retrospective analysis of the prospectively collected Dat'AIDS database we selected patients who both had a past KS history and a HIV-1 viral load below 200 copies/mL while being PI-treated. We searched for KS relapses while persistent virological success was maintained for at least 6 months, whether patients kept taking the PI, or switched to PI-free regimen.Results: Among the 216 patients with past KS event and a history of HIV-1 infection efficiently treated by a PI-based regimen, 148 patients (68.5%) later switched to a PI-sparing regimen. Their baseline characteristics were not different from non-switching patients. We described 7 cases of relapse (3.2% of the 216 patients). Five cases of relapse occurred in switching patients (3.4%). The remaining two relapses occurred in PI-treated patients (2.9%). At KS relapse, CD4 cell count was 459 cells/μL (range 225-560) for switching patients, compared with 362 and 136 cells/μL for the other two patients.Conclusions: In this large cohort of PLHIV with a history of KS and ART-controlled HIV replication, KS relapses were described in 3.2% of the patients, and were not more frequent when a PI-containing ART regimen has been switched to a PI-free regimen. Our results do not support a specific effect of PI on KS.

Abstract

Background The arrival of highly effective, well-tolerated, direct-acting antiviral agents (DAA) led to a dramatic decrease in hepatitis C virus (HCV) prevalence. Human immunodeficiency virus (HIV)-HCV–coinfected patients are deemed a priority population for HCV elimination, while a rise in recently acquired HCV infections in men who have sex with men (MSM) has been described. We describe the variations in HIV-HCV epidemiology in the French Dat’AIDS cohort. Methods This was a retrospective analysis of a prospective cohort of persons living with HIV (PLWH) from 2012 to 2018. We determined HCV prevalence, HCV incidence, proportion of viremic patients, treatment uptake, and mortality rate in the full cohort and by HIV risk factors. Results From 2012 to 2018, 50 861 PLWH with a known HCV status were followed up. During the period, HCV prevalence decreased from 15.4% to 13.5%. HCV prevalence among new HIV cases increased from 1.9% to 3.5% in MSM but remained stable in other groups. Recently acquired HCV incidence increased from 0.36/100 person-years to 1.25/100 person-years in MSM. The proportion of viremic patients decreased from 67.0% to 8.9%. MSM became the first group of viremic patients in 2018 (37.9%). Recently acquired hepatitis represented 59.2% of viremic MSM in 2018. DAA treatment uptake increased from 11.4% to 61.5%. More treatments were initiated in MSM in 2018 (41.2%) than in intravenous drug users (35.6%). In MSM, treatment at the acute phase represented 30.0% of treatments in 2018. Conclusions A major shift in HCV epidemiology was observed in PLWH in France from 2012 to 2018, leading to a unique situation in which the major group of HCV transmission in 2018 was MSM. Abstract

International audience; ObjectivesPost-exposure prophylaxis (PEP) care remains a challenge for individuals with potential sexual exposure to HIV in terms of PEP completion and ongoing risk behaviours.MethodsA retrospective analysis was carried out on data from the French Dat’AIDS prevention cohort (NCT03795376) for individuals evaluated for PEP between 2004 and 2017. A multivariable analysis was performed of predictors of both PEP completion and condom use [odds ratios (ORs)] and their associated probabilities (P, with P > 95% being clinically relevant).ResultsOverall, 29 060 sexual exposures to HIV were evaluated for PEP [36% in men who have sex with men (MSM) and 64% in heterosexuals]. Overall, 12 different PEP regimens were offered in 19 240 cases (46%). Tenofovir disoproxil fumarate (TDF)/emtricitabine (FTC) was the preferred backbone (n = 14 304; 74%). We observed a shift from boosted protease inhibitor-based regimens to nonnucleoside reverse transcriptase inhibitor- or integrase inhibitor-based regimens in recent years. Overall, 20% of PEP prescriptions were prematurely discontinued. Older age, MSM, intercourse with a sex worker, rape and intercourse with a known HIV-infected source patient were factors associated with increased rates of PEP completion (OR > 1; P > 98%). None of the 12 PEP regimens was associated with premature discontinuation. We also found 12 774 cases of unprotected sexual intercourse (48%). Condom use decreased (OR < 1; P > 99%) with the year of exposure, and was lower in MSM and rape victims. Condom use increased (OR > 1, P > 99%) with age, and was higher in those who had intercourse with a sex worker or with a female partner and in those with knowledge of the partner’s HIV status.ConclusionsWe provide new insights into how rates of condom use and PEP completion might be improved in those receiving PEP by targeting certain groups of individuals for interventions. In particular, youth and MSM at risk should be linked in a prevention-to-care continuum.

Abstract

International audience; Background - Although antiretroviral therapy (ART) has reduced the risk of Kaposi sarcoma (KS), KS cases still occur in HIV-infected people. Objective - To describe all KS cases observed between 2010 and 2015 in a country with high ART coverage. Methods - Retrospective study using longitudinal data from 44 642 patients in the French Dat'AIDS multicenter cohort. Patients' characteristics were described at KS diagnosis according to ART exposure and to HIV-plasma viral load (HIV-pVL) (≤50 or >50) copies/mL. Results - Among the 209 KS cases diagnosed during the study period, 33.2% occurred in ART naïve patients, 17.3% in ART-experienced patients and 49.5% in patients on ART, of whom 23% for more than 6 months. Among these patients, 24 (11.5%) had HIV-pVL ≤50 cp/mL, and 16 (66%) were treated with a boosted-PI-based regimen. The distribution of KS localization did not differ by ART status nor by year of diagnosis. Limitations - Data on human herpesvirus 8, treatment modalities for KS and response rate were not collected. Conclusion - Half of KS cases observed in the study period occurred in patients not on ART, reflecting the persistence of late HIV diagnosis. Factors associated with KS in patients on ART with HIV-pVL ≤50 cp/mL remain to be explored.
