Background and study aims Achalasia can be classified as either primary (idiopathic) achalasia or secondary achalasia, which is a consequence of another systemic disease. Peroral endoscopic myotomy (POEM) is an effective and safe treatment for achalasia. We evaluated the efficacy and safety of POEM in patients with Chagasic achalasia compared to idiopathic achalasia. Patients and methods We evaluated POEM procedures performed at a single institution from November 2016 to January 2018. Demographic data, Eckardt score, lower esophageal sphincter (LES) pressure, body mass index, post-operative erosive esophagitis, adverse events, length of hospital stay, and procedure-related parameters were analyzed. Results Fifty-one patients underwent POEM as a treatment for achalasia in this period (20 patients with Chagasic and 31 with Idiopathic etiology). The overall clinical success rate was 92.1 %, with no statistical difference between groups (90 % in the Chagasic group vs. 93.5 % in the Idiopathic group, P = 0.640). Both groups had significant reduction in Eckardt score and in LES pressure, and increase in bodey mass index (BMI) at 1-year follow-up. There was no statistical difference between groups regarding Eckardt score (P = 0.439), LES pressure (p = 0.507), BMI (P = 0.254), post erosive esophagitis (35 % vs. 38.7 %, P = 0.789), adverse events (30 % vs. 12 %, P = 0.163,) length of hospital stay (3.75 days vs. 3.58 days, P = 0.622), and operative time (101.3 min vs. 99.1 min, P = 0.840). Conclusion POEM is an effective and safe treatment for patients with achalasia. There is no difference in POEM outcomes for those patients with Chagasic or Idiopathic achalasia.

Lung diseases have a strong relationship with gastroesophageal reflux disease (GERD). It has been previously demonstrated that conditions such as tracheal stenosis, asthma and even lung transplantation may worsen with reflux and these patients have few symptoms of GERD. With the COVID-19 pandemic, the number of people who needed mechanical ventilation and tracheostomy increased. Our objective was to demonstrate the prevalence of gastro-oesophageal reflux in patients with tracheostomy and describe its characteristics. Methods Esophageal manometry and 24 h pH-metry was performed in 137 consecutive patients with a tracheostomy already in a chronic phase, independent of symptoms. Inquire on respiratory and digestive symptoms was also carried out at the time of the examination. Prevalence of gastroesophageal reflux was identified in this population and description of the groups with reflux and without it, as well as comparison between them. Results Of the 137 patients, 49 were male, the average age was 40.94 ± 17.3 and the body mass index was 26.3 ± 4.85. The prevalence of gastroesophageal reflux was 45.2%. Baseline characteristics were similar between the groups with and without reflux. In the reflux group, the mean DeMeester score was 36.5 ± 20.8 and the presence of lower sphincter hypotonia was found in only 31% of the patients and was not correlated with reflux between the groups (p = 0.285). Regarding the symptoms, 48% had heartburn symptoms and only 30% had a combination of typical symptoms (heartburn + regurgitation). Conclusion The presence of tracheostomy is related to an increased prevalence of reflux, even without typical symptoms most of the time. The mechanism for this is still unknown, perhaps the altered respiratory dynamics has a role. These patients should be investigated with functional exams if they develop any condition that may be affected by reflux.

Background: The catheter of the esophageal pH monitoring is associated with nasal and throat discomfort, and different behave in patients. The capsule of the wireless pH monitoring may cause chest pain and complications. Aim: To compare the wireless and conventional pH monitoring concerning the degree of discomfort and limitations in daily activities, complications, ability to diagnose pathological reflux, and costs. Methods: Twenty-five patients with symptoms of gastroesophageal reflux were prospectively submitted, in a simultaneous initial period, to 24-hour catheter esophageal pH monitoring and 48-hour wireless system. After removing each system, patients underwent a specific clinical questionnaire. Results: Fifteen patients (60%) pointed a higher discomfort in the introduction of the capsule (p=0.327). Systemic sclerosis (scleroderma) is frequently associated with both gastroesophageal reflux disease (GERD) and simultaneous esophageal dysmotility (ED). Antireflux surgery have to considerate the individual esophageal dysmotility degree of each patient. They need GERD treatment as a priority to help control worsening lung function, in patients developing pulmonary fibrosis or in lung transplantation patients. Another issue is the possibility that fundoplications in this patients can cause esophageal stasis, an event that can also disrupt lung function. Methods 113 patients were referred for surgical treatment of GERD, related with pulmonary impairment. SS patients are the most critical, once the ED may be an obstacle to fundoplication. We divided this patients in three groups regarding the esophageal function. A- Partial fundoplication—without important affection of ED, checked by manometry, confirmed with a complete emptying of esophagus in barium swallow test. B- Laparoscopic gastric bypass for treatment of GERD (RYGBP)—aperistalsis or atonic esophageal body, with normal esophageal empty at barium test. C- Esophagectomy—Atonic esophageal body with important delayed emptying of esophagus in barium swallow test. Results We could follow up 4 patients in group A, 3 patients in group B and 2 patients in group C. Median follow-up was 75, 30 and 18 months for Groups A,B and C respectively. All 3 patients in the RYGB group resolved all the symptoms including heartburn and regurgitation, with a sensitive improve in lung function. Only 1 patients in the fundoplication group reported symptom improvement or resolution. The Patients at Group C have no complain of Dysphagia but referred regurgitation and difficulty feedingtill first year. Conclusion Laparoscopic RYGB as an anti-reflux procedure is safe and may provide an alternative to fundoplication in the treatment of GERD for systemic sclerosis patients with esophageal dysmotility. Both reflux control and dysphagia rates were improved in the RYGBP group compared with fundoplication and esophagectomy.. This finding suggests that RYGBP may be an option for the primary manage- ment of scleroderma-associated gastroesophageal reflux in patients without importnat impairing of esophageal emptying.

BACKGROUND: The high-resolution manometry has been a significant advance in esophageal diagnostics. There are different types of catheter and systems devices to capture esophageal pressures that generate variable data related to Chicago Classification (CC) and consequently influence normal values results. There are not normative data for the 24-channel water-perfused high-resolution manometry system most used in Brazil with healthy volunteers in supine posture. OBJECTIVE: To determine manometric esophageal normative values for a 24-channel water-perfused high-resolution manometry catheter in supine posture using healthy volunteers according to CC 3.0 parameters. METHODS: A total of 92 volunteers with no gastrointestinal symptoms or medications affecting GI motility underwent esophageal high-resolution manometry by standard protocol. Age, gender and manometry parameters analyzed using Alacer software were collected. The median, range, and 5th and 95th percentiles (where applicable) were obtained for all high-resolution manometry metrics. Normal value percentiles were defined as 95th integrated relaxation pressure, 5th-100th distal contractile integral, and 5th distal latency. RESULTS: The mean age was 40.5±13.2 years. Our normative metrics were integrated relaxation pressure 4 cm). For EGJ-CI the range 5th-95th was 21.7-86.9 mmHg.cm.s. CONCLUSION: This is the first report of normative data for the 24-channel water-perfused system in supine posture. It revealed higher integrated relaxation pressure and distal latency duration which suggest the need to change CC 3.0 cutoffs for this system. Objective: to evaluate esophageal dysmotility (ED) and the extent of Barrett’s esophagus (BE) before and after laparoscopic Nissen fundoplication (LNF) in patients previously diagnosed with BE and ED. Methods: twenty-two patients with BE diagnosed by upper gastrointestinal (GI) endoscopy with biopsies and ED diagnosed by conventional esophageal manometry (CEM) were submitted to a LNF, and followed up with clinical evaluations, upper GI endoscopy with biopsies and CEM, for a minimum of 12 months after the surgical procedure. Results : sixteen patients were male (72.7%) and six were females (27.3%). The mean age was 55.14 (± 15.52) years old. and the mean postoperative follow-up was 26.2 months. The upper GI endoscopy showed that the mean length of BE was 4.09 cm preoperatively and 3.91cm postoperatively (p=0.042). The evaluation of esophageal dysmotility through conventional manometry showed that: the preoperative median of the lower esophageal sphincter resting pressure (LESRP) was 9.15 mmHg and 13.2 mmHg postoperatively (p=0.006). The preoperative median of the esophageal contraction amplitude was 47.85 mmHg, and 57.50 mmHg postoperatively (p=0.408). Preoperative evaluation of esophageal peristalsis showed that 13.6% of the sample presented diffuse esophageal spasm and 9.1% ineffective esophageal motility. In the postoperative, 4.5% of patients had diffuse esophageal spasm, 13.6% of aperistalsis and 22.7% of ineffective motor activity (p=0.133). Conclusion: LNF decreased the BE extension, increased the LES resting pressure, and increased the amplitude of the distal esophageal contraction; however, it was unable to improve ED.

Background Abnormal gastroesophageal acid reflux (GEaR) is often found in laryngeal diseases and tracheal stenosis. The study focuses on the prevalence of GEaR and the impact of anti-reflux surgery in the outcome of post-intubation tracheal stenosis Methods Prospective non-randomized study of patients with post-intubation tracheal stenosis. Inclusion criteria: adult with subglottic or recurrent tracheal stenosis. Exclusion criteria: previous gastroesophageal surgery. Patients were submitted to esophageal manometry and pH study. Patients were divided in 3 groups: one with pH study negative, one with pH study positive receiving treatment with proton pump inhibitor and one with pH study positive submitted to Nissen fundoplication. Follow-up was 24 months. Outcome was considered satisfactory when airway stenosis could be successfully managed by resection, no need for dilatation or definitive decannulation. In order to match the 3 groups, it was used a propensity score match (PSM) and them they were compared. Statistical significance was assumed for P Results Prevalence of GEaR was assessed in 175 patients. Abnormal pH study was found in 74 (42%) with a predominance of supine reflux. Typical GEaR symptoms were found in only 21%. Abnormal manometry was found in 37%, and hypotonic lower sphincter was the most frequent finding. Of the 124 patients who met the inclusion criteria, pH study was normal in 72 and abnormal in 52 (32 underwent clinical treatment and 20 nissen fundoplication). There were no peri-operative complications. After PSM, patients with abnormal pH study submitted to fundoplication had a higher chance of a satisfactory outcome of the stenosis compared to those treated with Omeprazole (OR 5.31; CI 2.41–11.07; P = 0.03) which was similar to those without GEaR (OR 1; CI 0.47–2.11; P = 0.99). Patients without GeAR had a higher chance of resolution of the tracheal stenosis compared to the patients treated with Omeprazole (OR 3.54; CI 2.02–6.19; P = 0.02). Conclusion Asymptomatic GEaR is a frequent finding in tracheal stenosis. Nissen fundoplication was safe and effective in selected patients with tracheal stenosis and GEaR. Control of the airway stenosis was superior after fundoplication if compared to medical treatment of GEaR. Background and study aims Us of proton pump inhibitors (PPIs) has made endoscopic treatment of gastroesophageal reflux disease (GERD) more efficient, with reduction in morbidity and complications. However, some patients persist with symptoms despite medical treatment and some are not compliant with it or cannot afford it for financial reasons, and thus they require non-pharmacological therapeutic options such as surgical fundoplication. Surgery may be effective in the short term, but there is related morbidity and concern about its long-term efficacy. The possibility of minimally invasive endoluminal surgeries has resulted in interest in and development of newly endoscopic devices. Good short-term results with surgical fundoplication lack of studies of is with long follow-up justify our interest in this study. The aim of this study was to investigate the efficacy of endoscopic polymer injection and endoluminal full-thickness plication in the long-term control of GERD. Patients and methods Forty-seven patients with GERD who underwent an endoscopic procedure were followed up for 60 months and evaluated for total response (RT), partial response (RP) and no response (SR) to endoscopic treatment with reintroduction of PPIs. Results Twenty-one patients received polymer injection (G0) and 26 endoluminal plication (G1). The number of patients with no response to endoscopic treatment with reintroduction of PPIs increased in time for both techniques (G0 P = 0.006; G1 P Conclusion Endoscopic therapies were ineffective in controlling GERD in the long term.

Objective This study focuses on the impact of antireflux surgery in the outcome of tracheal stenosis. Methods We performed a retrospective study including patients with benign tracheal stenosis who underwent esophageal manometry and dual-probe 24-hour ambulatory esophageal pH study. Patients with an abnormal pH study were managed with laparoscopic modified Nissen fundoplication or medically (omeprazole 80 mg/d, orally). Patients with normal pH study results were observed. After a 24-month follow-up, the outcome was considered satisfactory if tracheal stenosis could be managed by resection and there was no need for further dilatation or definitive decannulation. The management groups were compared using propensity score matching. Results A total of 175 patients were included. Abnormal pH study results were found in 74 patients (42.3%), and 12.6% of patients had typical gastroesophageal reflux symptoms. Follow-up was completed in 124 patients (20 had fundoplication, 32 received omeprazole, and 72 were observed). After propensity score matching, the outcome of tracheal stenosis in the fundoplication group was similar to that of the observation group (odds ratio, 1; P = .99) and better than that of the omeprazole group (odds ratio, 5.31; P = .03). The observation (no gastroesophageal reflux) group had a better outcome of stenosis than those treated with omeprazole (odds ratio, 3.54; P = .02). Conclusions The outcome of the airway stenosis was superior after laparoscopic fundoplication compared with medical treatment with omeprazole and was similar to the outcome of patients without gastroesophageal reflux. A prospective randomized trial is warranted.

BACKGROUND: Prolonged monitoring increased our knowledge on gastroesophageal reflux (GER), and the disease became known as gastroesophageal reflux disease (GERD). Prolonged reflux monitoring permits the diagnosis of GERD when endoscopic findings are not enough to characterize it. OBJECTIVE: The objective of this paper is to review the current knowledge on impedance-pH monitoring, taking into account the published literature and the authors experience with 1,200 exams. METHODS: The different types of prolonged reflux monitoring, namely: conventional pHmetry, catheter-free pHmetry and impedance-pHmetry will be briefly described. The new possibilities of evaluation with impedance-pHmetry are emphasized, namely: the study of symptomatic patients in use of proton pump inhibitors (PPIs); evaluation of patients with symptoms suggestive of GERD although with normal endoscopy and normal pHmetry, diagnostic elucidation of patients with atypical symptoms or supra-esophageal symptoms, mainly chronic cough, study of patients complaining of belch, differentiating gastric and supra-gastric belching, and the proper work-up before anti-reflux surgery. RESULTS: When impedance was associated to pH monitoring, an impressive technological evolution became apparent, when compared to pH monitoring alone. The main advantages of impedance-pHmetry are: the ability to detect all types of reflux: acid, non-acid, liquid, gaseous. In addition, other important measurements can be made: the ability of the esophagus in transporting the bolus, the measurement of basal mucosal impedance and the evaluation of primary peristalsis post reflux. CONCLUSION: Impedance-pHmetry is a promising method, with great advantages over conventional pHmetry. The choice between these two types of monitoring should be very judicious. The authors suggest the importance of careful evaluation of each reflux episode by the physician responsible for the examination, necessary for the correct interpretation of the tracings. RESUMO CONTEXTO: A monitorização prolongada ampliou o conhecimento sobre o refluxo gastroesofágico; a afecção decorrente do mesmo, passou a ser designada por doença do refluxo gastroesofágico (DRGE). O estudo prolongado do refluxo viabiliza o diagnóstico da DRGE nos casos sem alterações endoscópicas suficientes para caracterização da afecção. OBJETIVO: O objetivo do presente trabalho é apresentar uma revisão sobre a monitorização do refluxo por impedâncio-pHmetria, ponderando-se os dados de literatura com a experiência dos autores com o método em 1.200 exames realizados. MÉTODOS: São apresentados detalhes dos diferentes tipos de monitorização prolongada do refluxo; ou seja, a pHmetria convencional, a pHmetria sem cateter e a impedâncio-pHmetria. Salientam-se as novas possibilidades de avaliação que a impedâncio-pHmetria propicia e suas principais indicações: estudo de pacientes que permanecem sintomáticos durante o tratamento do refluxo com antissecretores; análise de pacientes com sintomas sugestivos de DRGE que não apresentem esofagite ao exame endoscópico e que tenham pHmetria normal; esclarecimento diagnóstico de pacientes com sintomas atípicos e supraesofágicos - em especial na tosse crônica; estudo da eructação diferenciando-as em dois grupos: gástricas e supragástricas e no auxílio na indicação do tratamento cirúrgico do refluxo. RESULTADOS: A monitorização associando duas modalidades de avaliação: a impedancio-pHmetria representa evolução tecnológica expressiva em relação à modalidade baseada apenas na análise do pH (pHmetria). As principais vantagens da impedâncio-pHmetria são: possibilidade de avaliação de todas modalidades de refluxo; ou seja, ácido, não-ácido, líquido e gasoso e também a possibilidade de estudo de outras variáveis importantes; ou seja: capacidade de transporte do bolus, impedância basal do esôfago e peristalse pós refluxo. CONCLUSÃO: A impedancio-pHmetria é um método promissor, com grandes vantagens sobre a pHmetria convencional. A escolha do tipo de monitorização a ser utilizada, deve ser criteriosa. Os autores destacam a importância da análise cuidadosa de cada episódio de refluxo, pelo médico responsável pela execução do exame, para correta interpretação e valorização dos dados obtidos.

Achalasia is a relatively rare primary motor esophageal disorder, characterized by absence of relaxations of the lower esophageal sphincter and of peristalsis along the esophageal body. As a result, patients typically present with dysphagia, regurgitation and occasionally chest pain, pulmonary complication and malnutrition. New diagnostic methodologies and therapeutic techniques have been recently added to the armamentarium for treating achalasia. With the aim to offer clinicians and patients an up-to-date framework for making informed decisions on the management of this disease, the International Society for Diseases of the Esophagus Guidelines proposed and endorsed the Esophageal Achalasia Guidelines (I-GOAL). The guidelines were prepared according the Appraisal of Guidelines for Research and Evaluation (AGREE-REX) tool, accredited for guideline production by NICE UK. A systematic literature search was performed and the quality of evidence and the strength of recommendations were graded according to the Grading of Recommendations Assessment, Development and Evaluation (GRADE). Given the relative rarity of this disease and the paucity of high-level evidence in the literature, this process was integrated with a three-step process of anonymous voting on each statement (DELPHI). Only statements with an approval rate >80% were accepted in the guidelines. Fifty-one experts from 11 countries and 3 representatives from patient support associations participated to the preparations of the guidelines. These guidelines deal specifically with the following achalasia issues: Diagnostic workup, Definition of the disease, Severity of presentation, Medical treatment, Botulinum Toxin injection, Pneumatic dilatation, POEM, Other endoscopic treatments, Laparoscopic myotomy, Definition of recurrence, Follow up and risk of cancer, Management of end stage achalasia, Treatment options for failure, Achalasia in children, Achalasia secondary to Chagas' disease.

Dysplasia and esophageal adenocarcinoma may arise in patients with Barrett's esophagus after fundoplication esophageal pH monitoring showing no acid in esophagus. This suggests the need to develop methodology to evaluate the occurrence of ultra-distal reflux (1cm above the LES). The objective of the study was to compare acid exposition in three different levels: 5cm above the upper border of the LES, 1cm above the LES and in the intrasphincteric region. Eleven patients with Barrett's esophagus after Nissen fundoplication with no clinical, endoscopic and radiologic evidence of reflux were selected. Four-channel pH monitoring took place: channel A, 5cm above the upper border of the LES; channel B, 1cm above the LES; channel C, intrasphincteric; channel D, intragastric. Achalasia surgical treatment alters the esophagogastric junction anatomy (cardiomyotomy plus fundoplication or esophagectomy and gastric pull-up), thus favoring a certain degree of gastroesophageal reflux. Gastric secretory and hormonal functioning is not completely known in chagasic patients. The aim of this study was to evaluate the gastric secretory and hormonal response in patients with end-stage chagasic achalasia compared with normal subjects. Gastric secretion and hormonal response were assessed by estimation of gastric acid secretion (GAS) in basal condition and after pentagastrin stimulation, basal serum gastrin, and serum pepsinogen (SP) in basal condition and after betazole hydrochloride (Histalog; Eli Lilly and Company, Indianapolis, IN, USA) stimulation in 27 patients with chagasic achalasia. The results were then compared with those of 24 normal subjects. In the chagasic group, the mean basal and stimulated GAS were significantly lower than in the control group (basal: 1.277 vs. 3.13, P = 0.002; stimulated: 15.9 vs. 35.8, P = 0.0001). Chagasic patients' SG levels showed a significantly higher basal value than the control group (83.3 vs. 36.8, P = 0.0001). There was a significant increase of SP after stimulation compared with the basal levels in both chagasic and control groups. Although the chagasic patients' SP values were higher than the controls, this difference was not statistically significant, either in basal and stimulated conditions (basal: 122.0 vs. 108.9, stimulated 120 min: 177.1 vs. 158.9). In patients with chronic Chagas' disease (ChD), although autonomic denervation does not suppress the strength of the gastric mucosal cells' secretory response to stimulation, it reduces GAS (parietal cell) without, however, affecting SP production (chief cells). On the other hand, the gastrin-producing cells have continuously been stimulated by low GAS.

RACIONAL: As anomalias do processo da deglutição têm etiologia multifatorial e complexa. A videofluoroscopia da deglutição tem sido apontada como o exame de maior utilidade na investigação diagnóstica desses casos. Tal método, quando precedido de anamnese clínica adequada, consegue caracterizar convenientemente o grau de disfunção e, freqüentemente, identificar a causa da anomalia com grande precisão. OBJETIVOS: Estudar as queixas clínicas e os achados da videofluoroscopia em pacientes com distúrbios da deglutição, sem evidências clínicas de afecções neurológicas associadas, distribuídos em duas faixas etárias - adultos e idosos - e analisar as manifestações sintomáticas, o tipo de disfunção (orofaríngea ou esofágica) e a capacidade de elucidação da queixa clínica pelo método de imagem. MATERIAL E MÉTODOS: Foram analisados 70 pacientes com queixa de alteração na capacidade de deglutição, sem evidências clínicas de afecções neurológicas associadas, distribuídos em dois grupos: adultos (GI) - idade inferior ou igual a 65 anos (n = 36) e idosos (GII) - idade superior a 65 anos (n = 34). Todos foram submetidos a anamnese para obtenção de informações sobre queixas em relação à deglutição; as queixas foram caracterizadas como altas ou baixas, de acordo com o local predominante de manifestação. Todos os pacientes foram submetidos a videofluoroscopia da deglutição; as alterações observadas com esse exame foram caracterizadas como orofaríngeas ou esofágicas. Avaliou-se a capacidade de elucidação da queixa clínica pela videofluoroscopia nos dois grupos. RESULTADOS: Dentre as queixas analisadas, a única cuja análise estatística demonstrou diferença significante entre os grupos, foi a de pirose, que ocorreu com maior freqüência no grupo de adultos (oito pacientes (22,2%) do GI e um (2,9%) do GII referiam tal queixa). Ao estudo videofluoroscópico, observou-se maior ocorrência de disfunção orofaríngea no grupo de pacientes idosos (41,2% GII x 13,9% GI) e ocorrência semelhante, nos dois grupos, de disfunção esofágica (35,3% GII x 33,3% GI). Dezenove (52,8%) pacientes do GI e 23 (67,6%) do GII tiveram suas queixas clínicas elucidadas pela videofluoroscopia. CONCLUSÕES: 1. As queixas clínicas associadas ao quadro de dificuldade de deglutição ocorrem com freqüência semelhante em adultos e idosos, com exceção da queixa de pirose que predomina no grupo com idade menos avançada; 2. pacientes idosos apresentam maior ocorrência de alterações altas de deglutição (orofaríngeas); 3. a videofluoroscopia da deglutição representa método de grande importância diagnóstica, pois permite a identificação de alterações morfofuncionais relevantes que não são passíveis de identificação adequada à anamnese, e 4. a capacidade de elucidação diagnóstica da videofluoroscopia da deglutição é maior no grupo com idade mais avançada. BACKGROUND: The abnormalities of swallowing process have multifactor and complex etiologies. The videofluoroscopy has been pointed as the exam of greater utility in diagnostic investigation for these cases. This method, when preceded of an adequate anamnesis, can characterize conveniently the level of the dysfunction and usually identify the cause of abnormality with great precision. AIMS: To study the clinical complaints and findings of the videofluoroscopy examination in patients with deglutition disorders and no clinical evidence associated with neurological disorder, classified into two age groups: adults and older people, and to analyze: symptomatic manifestations, kinds of disorders (oropharyngeal or esophageal) and the capacity of clarifying the clinical complaints through the method of images. MATERIALS AND METHODS: Seventy patients with complaint of the capacity of deglutition were analyzed. They had no clinical evidence of associated neurological syndromes or disorders and were classified into two age groups: adults (GI) - £65 years (n = 36) and older (GII) >65 years (n = 34). All patients were submitted to anamnesis to obtain the information about their complaints concerning deglutition. The complaints were characterized as high or low, according to their predominant location of manifestation. All the patients were submitted to videofluoroscopy of the deglutition; these alterations were characterized as oropharyngeal or esophageal. The capacity of clarifying the clinical complaints by videofluoroscopy was evaluated in both groups. RESULTS: Among the complaints analyzed, the only one in which the statistical analysis presented a significant difference between the groups was the complaint of heartburn, which occurred more often in the group GI - eight patients (22.2%) and GII - one patient (2.9%). In the study of videofluoroscopy, it was observed a higher incidence in the oropharyngeal disorder in group GII - 41.2% while in group GI - 13.9%. As for the esophageal disorder, the incidence was similar in both groups GI - 35.3% and GII - 33.3%. Nineteen patients (52.8%) in GI and 23 (67.6%) in GII had their complaints clarified through the videofluoroscopy. CONCLUSIONS: Under the conditions of this study, it can be concluded that: 1.The clinical complaints associated with the difficulty of deglutition occur at a similar frequency in adults and older people, with the exception of heartburn that occurs in larger number among adults; 2. Older people present a higher incidence of oropharyngeal deglutition problems; 3. The videofluoroscopy of the deglutition represents a method of great importance for the diagnosis, because it allows the identification of morphofunctional disorders that cannot be adequately identified by anamnesis; 4. The capacity of clarifying diagnosis of the videofluoroscopy of the deglutition is higher in the older people group.

Due to the introduction of computer technology into manometry laboratories, three-dimensional manometric images of the lower esophageal sphincter can be constructed based on radially oriented pressures, a method termed 'computerized axial manometry.' Calculation of the sphincter pressure vector volume using this method is superior to standard manometric techniques in assessing lower esophageal sphincter function in patients with gastroesophageal reflux disease and idiopathic achalasia. Despite similarities between idiopathic achalasia and chagasic esophagopathy found using clinical, radiological, and manometric studies, controversy around lower esophageal sphincter pressure persists. The goal of this study was to analyze esophageal motor disorders in Chagas' megaesophagus using computerized axial manometry. Twenty patients with chagasic megaesophagus (5 men, 15 women, and average age 50.1 years, range 17-64) were prospectively studied. For three-dimensional imaging construction of the lower esophageal sphincter, a low-complacency perfusion system and an eight-channel manometry probe with four radial channels placed in the same level were used. For probe traction, the continuous pull-through technique was used. Results showed that the lower esophageal sphincter of patients with chagasic megaesophagus have significantly elevated pressure, length, asymmetry, and vector volumes compared to those of normal volunteers (P < 0.05). Aperistalsis of the esophageal body waves was observed in all patients and contraction amplitude was lower than that in normal patients. We conclude that patients with chagasic megaesophagus have hypertonic lower esophageal sphincter and aperistalsis of the esophageal body.

Background : Gastroesophageal reflux disease is a worldwide prevalent condition that exhibits a large variety of signs and symptoms of esophageal or extra-esophageal nature and can be related to the esophagic adenocarcinoma. In the last few years, greater importance has been given to the influence of physical exercises on it. Some recent investigations, though showing conflicting results, point to an exacerbation of gastroesophageal reflux during physical exercises. Aim : To evaluate the influence of physical activities in patients presenting with erosive and non erosive disease by ergometric stress testing and influence of the lower esophageal sphincter tonus and body mass index during this situation. Methods : Twenty-nine patients with erosive disease (group I) and 10 patients with non-erosive disease (group II) were prospectively evaluated. All the patients were submitted to clinical evaluation, followed by upper digestive endoscopy, manometry and 24 h esophageal pH monitoring. An ergometric testing was performed 1 h before removing the esophageal pH probe. During the ergometric stress testing, the following variables were analyzed: test efficacy, maximum oxygen uptake, acid reflux duration, gastroesophageal reflux symptoms, influence of the lower esophageal sphincter tonus and influence of body mass index in the occurrence of gastroesophageal reflux during these physical stress. Results : Maximum oxigen consumption or VO 2 max, showed significant correlation when it was 70% or higher only in the erosive disease group, evaluating the patients with or without acid reflux during the ergometric testing (p=0,032). The other considered variables didn't show significant correlations between gastroesophageal reflux and physical activity (p>0,05). Conclusions : 1) Highly intensive physical activity can predispose the occurrence of gastroesophageal reflux episodes in gastroesophageal reflux disease patients with erosive disease; 2) light or short sessions of physical activity have no influence on reflux, regardless of body mass index; 3) the lower esophageal sphincter tonus does not influence the occurrence of reflux disease episodes during exercise testing. Racional : A doença do refluxo gastroesofágico é afecção com elevada prevalência em todo o mundo, que apresenta grande variedade de sinais e sintomas esofagianos ou extra-esofágico, podendo ter entre suas complicações o adenocarcinoma esofágico. Nos últimos anos, maior importância tem sido dada à influência dos exercícios físicos na sua patogênese. Algumas investigações recentes, embora com resultados conflitantes, apontam para agravamento do refluxo gastroesofágico durante eles. Objetivos : Avaliar a influência da atividade física em pacientes com doença erosiva e não erosiva através do teste ergométrico de esforço, e ainda, a relevância do tônus do esfíncter esofagiano inferior e do índice de massa corpórea durante esta situação. Métodos : Vinte e nove pacientes com doença do refluxo erosiva (grupo I) e 10 com não-erosiva (grupo II) foram avaliados prospectivamente. Todos foram submetidos à avaliação clínica, seguida pela endoscopia digestiva alta, manometria e pH-metria esofágica de 24 horas. Um teste ergométrico foi realizado uma hora antes de retirar a sonda de pH-metria. Durante ele as seguintes variáveis foram analisadas: eficácia do teste, o consumo máximo de oxigênio ou VO2 max, tempo de refluxo ácido, sintomas de refluxo gastroesofágico, influência do tônus do esfíncter esofágico e do índice de massa corporal na ocorrência de refluxo gastroesofágico durante esta situação. Resultados : VO2 max apresentou correlação significativa, quando foi maior ou igual a 70%, apenas no grupo doença erosiva, avaliando os pacientes com ou sem refluxo ácido durante o teste ergométrico (p=0,032). As demais variáveis consideradas não demostraram correlação significativa entre a ocorrência de refluxo gastroesofágico e a atividade física (p>0,05). Conclusões : 1) Atividade física de alta intensidade pode predispor à ocorrência de episódios de refluxo gastroesofágico em pacientes em portadores da forma erosiva; 2) atividade física de baixa intensidade ou de curta duração não exercem influência, independentemente do índice de massa corpórea; 3) o tônus do esfíncter esofagiano não influencia na ocorrência de episódios de refluxo gastroesofágico durante atividade física.

Racional — Os portadores da doença do refluxo gastroesofágico podem apresentar ou não esofagite por refluxo. Imagina-se que os portadores de esofagite são mais sintomáticos que os pacientes sem esofagite. Contudo, poucos estudos comparam adequadamente esses grupos de pacientes entre si. Objetivos - Comparar as formas com e sem esofagite da doença do refluxo gastroesofágico em relação à idade, sexo, presença e intensidade de sintomas. Material e Métodos - Foram estudados, por meio de entrevista clínica, 122 portadores da doença — 90 com e 32 sem esofagite — identificados por estudo endoscópico do esôfago e por pHmetria esofágica prolongada. Foram considerados idade, sexo e a presença das seguintes queixas: pirose, disfagia, dor torácica não-cardíaca e sintomas respiratórios. A queixa de pirose foi analisada com mais detalhes, levando-se em conta além da presença do sintoma, a intensidade, a freqüência e a duração da queixa. Em função dessas variáveis, a pirose foi caracterizada, quando presente, como sendo de grau leve/moderado ou intenso/muito intenso. Resultados - Não houve diferença significativa entre os grupos em relação à idade, ao sexo e à presença dos sintomas. Observou-se, contudo, em relação à intensidade da queixa de pirose, maior ocorrência de pirose caracterizada como intensa/muito intensa no grupo com esofagite. Conclusões - 1. As formas com e sem esofagite da doença do refluxo gastroesofágico são muito semelhantes em relação à idade, ao sexo e à presença dos sintomas analisados nos pacientes acometidos; 2. Apesar dos sintomas analisados estarem presentes em níveis semelhantes nos dois grupos estudados, observou-se que a ocorrência de pirose caracterizada como de maior intensidade foi mais comum no grupo de pacientes com esofagite; 3. Dentre os portadores da doença do refluxo analisados, observou-se contingente significativo de pacientes (26,2%) que não apresentavam esofagite endoscópica; a pHmetria esofágica prolongada foi de fundamental importância para a identificação desse grupo; 4. Existe a necessidade de serem adotados conceitos mais homogêneos em relação à doença do refluxo gastroesofágico, à esofagite por refluxo e à caracterização do refluxo patológico, para obter-se maior precisão diagnóstica e para que se possa comparar, adequadamente, resultados de diferentes estudos sobre o tema. Background — Patients with gastroesophageal reflux disease may or may not have endoscopic esophagitis; there are few studies comparing these groups among themselves. Objectives - This study was designed in order to evaluate differences between patients with gastroesophageal reflux disease with and without esophagitis. Patients/Methods - A hundred and twenty-two patients with gastroesophageal reflux disease characterized by esophageal endoscopy and pHmetry were included, 90 with and 32 without esophagitis. Assessment involved an anamnesis, including the following data: age, sex, heartburn, dysphasia, non-cardiac chest pain and respiratory symptoms. Heartburn was analyzed in more detail, its duration, intensity and periodicity being determined. Results - No statistical significant difference was observed between the groups, regarding age, sex or presence of symptoms. However, in the group with esophagitis, heartburn classified as severe or very severe was more frequent. Conclusions - 1. The groups of patients with or without esophagitis analyzed were very similar concerning age, gender and presence of symptoms. However, regarding the heartburn's intensity, it was more intense in the group with esophagitis. 2. Among patients with gastroesophageal reflux disease, there is a large number of cases without esophagitis (26.2%) and that prolonged pH-monitoring is fundamental in its identification; 3. A better definition of reflux disease, esophagitis and pathological reflux is needed, in order to allow better diagnostic accuracy and comparisons in different studies on this subject.

Context Impairment of esophageal motility is a common finding in patients with gastroesophageal reflux disease (GERD) as reduced lower esophageal sphincter (LES) basal pressure. A very low LES pressure might facilitate the occurrence of more gastroesophageal reflux whereas abnormal esophageal peristalsis may contribute to impaired esophageal clearance after reflux. Objective Evaluate the esophageal motor function of the lower esophageal sphincter and esophageal body in the various forms of gastroesophageal reflux disease. Methods The manometrics records of 268 patients, who had evaluation of the esophageal motility as part of the diagnostic gastroesophageal reflux disease were split into four groups, as follows: 33 patients who had no esophagitis; 92 patients who had erosive esophagitis; 101 patients who had short Barrett's esophagus and 42 patients who had long Barrett's esophagus. Results The group who had long Barrett's esophagus showed smaller mean LES pressure and higher percentage of marked LES hypotonia; in the distal segment of the esophageal body the this group showed higher percentage of marked hypocontractility of the distal segment (

CONTEXT: Esophageal pH monitoring is considered to be the gold standard for the diagnosis of gastroesophageal acid reflux. However, this method is very troublesome and considerably limits the patient's routine activities. Wireless pH monitoring was developed to avoid these restrictions. OBJECTIVE: To compare the first 24 hours of the conventional and wireless pH monitoring, positioned 3 cm above the lower esophageal sphincter, in relation to: the occurrence of relevant technical failures, the ability to detect reflux and the ability to correlate the clinical symptoms to reflux. METHODS: Twenty-five patients referred for esophageal pH monitoring and with typical symptoms of gastroesophageal reflux disease were studied prospectively, underwent clinical interview, endoscopy, esophageal manometry and were submitted, with a simultaneous initial period, to 24-hour catheter pH monitoring and 48-hour wireless pH monitoring. RESULTS: Early capsule detachment occurred in one (4%) case and there were no technical failures with the catheter pH monitoring (P = 0.463). Percentages of reflux time (total, upright and supine) were higher with the wireless pH monitoring (P < 0.05). Pathological gastroesophageal reflux occurred in 16 (64%) patients submitted to catheter and in 19 (76%) to the capsule (P = 0.355). The symptom index was positive in 12 (48%) patients with catheter pH monitoring and in 13 (52%) with wireless pH monitoring (P = 0.777). CONCLUSIONS: 1) No significant differences were reported between the two methods of pH monitoring (capsule vs catheter), in regard to relevant technical failures; 2) Wireless pH monitoring detected higher percentages of reflux time than the conventional pH-metry; 3) The two methods of pH monitoring were comparable in diagnosis of pathological gastroesophageal reflux and comparable in correlating the clinical symptoms with the gastroesophageal reflux. Contudo, é bastante incômoda e restringe consideravelmente as atividades cotidianas do paciente. A pHmetria sem cateter foi desenvolvida para contornar tais limitações. OBJETIVO: Comparar as primeiras 24 horas das pHmetrias convencional e sem cateter, posicionadas a 3 cm acima do esfíncter inferior do esôfago, em relação à: ocorrência de falhas técnicas relevantes, capacidade de detecção do refluxo e capacidade de relacionar as queixas clínicas com o refluxo. MÉTODOS: Foram estudados, de modo prospectivo, 25 pacientes encaminhados para pHmetria esofágica, com sintomas típicos da doença do refluxo gastroesofágico, submetidos a entrevista clínica, endoscopia digestiva, manometria esofágica e realização, com período inicial simultâneo, de pHmetrias com cateter por 24 horas e com cápsula por 48 horas. RESULTADOS: Houve queda precoce da cápsula em um paciente (4%) e nenhuma falha técnica na pHmetria com cateter (P = 0,463). RACIONAL: Por padronização aceita internacionalmente, posiciona-se o sensor distal de pHmetria esofágica a 5 cm acima da borda superior do esfíncter inferior do esôfago, localizado por manometria esofágica. Porém, vários autores sugerem técnicas alternativas de posicionamento que prescindem da manometria. Dentre essas, destaca-se a da viragem do pH, tema este controverso pela sua duvidosa confiabilidade. OBJETIVO: Avaliar a adequação do posicionamento do sensor distal de pHmetria pela técnica de viragem do pH, considerando-se a presença, o tipo e o grau de erro de posicionamento que tal técnica proporciona, e também estudar a influência da posição adotada pelo paciente durante a técnica da viragem. MÉTODOS: Foram estudados de modo prospectivo, durante o período de 1 ano, 1.031 pacientes. Durante entrevista clínica, foram registrados os dados demográficos e as queixas clínicas apresentadas. Todos foram submetidos a manometria esofágica para localização do esfíncter inferior do esôfago e a técnica da viragem do pH. A identificação do ponto de viragem foi realizada de dois modos distintos, caracterizando dois grupos de estudo: com o paciente sentado (grupo I - 450 pacientes) e com o paciente em decúbito dorsal horizontal (grupo II - 581 pacientes). Após a identificação do ponto de viragem, o sensor distal de pHmetria era posicionado na posição padronizada, baseada na localização manométrica do esfíncter. Registrava-se onde seria posicionado o sensor de pH se fosse adotada a técnica da viragem. Para avaliação da adequação do posicionamento, considerou-se que o erro é representado pela diferença (em centímetros) entre a localização padronizada (manométrica) e a localização que seria adotada caso fosse empregada a técnica da viragem. Considerou-se que o erro seria grosseiro se fosse maior que 2 cm. Analisou-se também o tipo de erro mais freqüente (se acima ou abaixo da posição padronizada). Foram incluídos todos pacientes que aceitaram participar da pesquisa e excluídos os casos nos quais não se identificou acidificação intragástrica. RESULTADOS: Se fosse adotada a técnica da viragem, haveria erro no posicionamento do sensor em 945 pacientes (91,6%), portanto, o sensor seria posicionado na posição padronizada em apenas 86 (8,4%) casos. Em relação à caracterização do grau de erro, haveria erro considerado grosseiro em 597 (63,2%) pacientes. Em relação ao tipo de erro, o sensor seria posicionado abaixo do local padronizado em 857 (90,7%) casos. Não houve diferença significante entre os dois grupos de estudo em relação a nenhum dos parâmetros analisados, indicando que a posição adotada pelo paciente durante a manobra da viragem não interfere no erro inerente à técnica. CONCLUSÕES: 1. O posicionamento do sensor distal de pHmetria pela técnica da viragem do pH não é confiável. 2. A técnica da viragem proporciona margem de erro expressiva. 3. O tipo de erro mais comum que tal modalidade técnica proporciona é o posicionamento mais distal do sensor, que pode superestimar a ocorrência de refluxo. 4. Não há influência da posição adotada pelo paciente durante a realização da técnica da viragem do pH na eficiência do método. BACKGROUND: By internationally accepted standardization, the esophageal pH-meter distal sensor is positioned 5 cm above the superior border of the lower esophageal sphincter, identified by esophageal manometry. However, several authors suggest alternative positioning techniques that leave out the manometry; among such techniques, the pH step-up is the one to be pointed out. This subject is controversial; some publications state that the step-up technique is not reliable while some others consider it reliable. AIMS: Considering the existent controversy and the small number of prospective works with suitable sample and methodology, we have idealized the present study, that aims the evaluation of the suitability of the pH-meter distal sensor positioning based on the step-up technique, by analyzing the presence, the type and the degree of the error of positioning that such technique provides and the influence of the position adopted by the patient during the procedure. METHODS: One thousand and thirty one patients conducted to the esophageal pH-meter procedure were studied in a prospective way. During the clinical interview, the demographic data and the presented clinical complaints were registered. All the patients were submitted to both esophageal manometry in order to localize the lower esophageal sphincter and the pH step-up technique, that consists of the introduction of the pH-meter sensor in the gastric chamber and in the sensor's gradual traction until the pH steps up to levels over 4. The step-up point was identified by two distinct ways, characterizing two study groups: with the sitting patient (group I - 450 patients) and with the patient in supine position (group II - 581 patients). After the step-up point identification, the pH-meter distal sensor was placed in the standard position (based on the sphincter manometric placement). It was registered where the pH sensor would be positioned if the step-up technique were adopted. To evaluate the positioning suitability, the error was considered to be represented by the difference (in centimeters) between the suitable placement (manometry) and the one that would be adopted in case the step-up technique were adopted. The positioning error was considered rough if it were larger than 2 cm. The most frequent type of error was also analyzed (if above or below the standard position). RESULTS: It was observed that if the step-up technique were adopted, there would be error in the sensor positioning in 945 patients (91.6%). In terms of error degree, there would be a rough error in 597 (63.2%) cases. Concerning the type of error, the sensor would be positioned below the standard place in 857 (90.7%) patients. As to the interference of the position adopted by the patient during the step-up technique, it was observed that there was no significant difference among the groups of study in any of the analyzed parameters. CONCLUSIONS: 1. The positioning of the pH-meter distal sensor by the step-up technique is not reliable; 2. the step-up technique provides expressive error margin; 3. the most common type of error that such technique mode provides is the placement of the sensor below the standard positioning, which may overestimate the reflux occurrence; 4. there is no influence in the position adopted by the patient during the pH step-up technique procedure, in terms of method efficiency.

RACIONAL: A doença do refluxo gastroesofágico, considerada uma das afecções digestivas de maior prevalência nos países ocidentais, tem sido bastante estudada. Merece destaque o grande número de publicações existentes na literatura médica sobre o tema. Contudo, há controvérsia em relação a vários aspectos relevantes da afecção. OBJETIVOS: Salientar tópicos importantes da doença e apresentar revisão de literatura do tema. Foram revistos os seguintes tópicos: definição, prevalência, manifestações clínicas, diagnóstico, indicações e limitações dos principais métodos diagnósticos e tratamento clínico, cirúrgico e endoscópico MATERIAL E MÉTODOS: Pesquisou-se, via Internet: artigos de revisão, consensos, "guidelines" e revisões sistemáticas de literatura com meta-análise, publicados nos últimos 5 anos (2000 a 2005), nas seguintes bases de dados: Central de Registro de Estudos Controlados da Cochrane Library e na PubMed. Dentre muitos trabalhos encontrados, foram selecionados nove revisões sistemáticas de literatura com meta-análise, cinco revisões de literatura com critérios de inclusão bem definidos, cinco "guidelines" e quatro publicações de consensos, que abordassem os tópicos propostos para a revisão. Vale salientar que, como se observa nas referências bibliográficas, não foram consideradas apenas as publicações encontradas na referida pesquisa; outras publicações relevantes (algumas anteriores ao período de tempo analisado) foram consideradas na redação do presente texto CONCLUSÕES: Dentre as 12 conclusões apresentadas, destacam-se duas: 1. a impedanciometria esofágica tem contribuído substancialmente para o melhor conhecimento da doença do refluxo gastroesofágico e se desponta, quando associada à pHmetria (impedanciopHmetria esofágica), como o novo padrão-ouro para o diagnóstico do refluxo gastroesofágico, e 2. dentre os sintomas da afecção há os que podem ser considerados ácido-dependentes que costumam responder bem ao tratamento com inibidores de bomba de prótons. Contudo, há também sintomas que são mais dependentes da presença física do refluxo do que da sua acidez. Esses, que não dependem tanto do teor ácido do refluxo, não são bem controlados pelo uso de anti-secretores. BACKGROUND: Gastroesophageal reflux disease, considered one of the most common digestive diseases in western countries, has been very much studied. The great number of publications in medical literature based upon this issue must be pointed out. However, some of its aspects remain controversial. AIMS: To emphasize important topics of the disease and to present a review on the theme. The following topics were reviewed: defi nition, prevalence, clinical complaints, indications and limitations of the major diagnostic methods and medical, surgical and endoscopic treatments. MATERIAL AND METHODS: We searched, by internet: selected review articles, consensus, guidelines and systematic reviews with meta-analysis, published in the last 5 years (from 2000 to 2005), in the following data bases: Cochrane Central Register of Controlled Trials (The Cochrane Library) and PubMed. Among lots of papers which were found, we selected nine systematic reviews with meta-analysis, fi ve review articles, fi ve guidelines and four consensus, that dealt with the topics we had decided to review. It must be pointed out that, as it has been observed in our bibliographical references, not only the publications found were taken into account; other relevant ones (some of them published previously to the analyzed time period) were considered in the composition of this present paper. CONCLUSIONS: Among the 12 conclusions presented, two have to be emphasized: 1. the esophageal multichannel intraluminal impedance has largely contributed to the better knowledge of the gastroesophageal reflux disease and it suggests, when associated with pHmetry (esophageal impedance-pHmetry), as a new gold standard to the gastroesophageal reflux diagnosis and 2: among the gastroesophageal reflux disease symptoms some of them may be considered acid dependents and can usually get good responses to the proton pump inhibitors. Nevertheless, there are also some symptoms which are more dependent on the physical presence of the reflux rather than its acidity. Such symptoms, which do not depend so much on the acidity of the reflux, are not well controlled by the proton pump inhibitors.

The aim of this study was to evaluate the effects of Botox injections in the clinical response and esophageal function of patients with dysphagia due to chagasic achalasia. Background - Mechanical lifting of the abdominal wall, a method based on traction and consequent elevation of the abdominal wall, is an alternative procedure to create enough intra-abdominal space necessary for videolaparoscopic surgery, dispensing the need for intraperitoneal gas insufflation. Objective - This study aims to evaluate the technical feasibilility of this procedure to carry out a videolaparoscopic cholecystectomy, while analyzing the clinical and functional aspects of this technique. Patients and Methods - In the Digestive Tract Surgery Discipline of the Medical School at the University of São Paulo, São Paulo, SP, Brazil, was created the equipment to perform videolaparoscopic surgery using this method. The equipment has two sections: an external part which consisted of a frame attached to the operating table, inside which there is a sliding steel cable, moved by a ratched which is located at the lower end of one of the frame rods; the internal rod, the support, has an "L" shape, and its horizontal branch is made up of three turning rods and which is connected to the steel cable after insertion into the abdominal cavity. Ten patients underwent videolaparoscopic cholecystectomy using this equipment. The time taken to install the equipment, the operating area characteristics, the interference from the lifting equipment on surgical movements and on the intra-operative cholangiography, the measurements made of the force used during traction and extension of the abdominal wall elevation, and the medication required for post-operative analgesia were all evaluated. Results - There were no intra-operative complications, and in none of the cases was it found necessary to convert to open surgery. We considered the insertion a safe and uncomplicated procedure, and the traction system efficient. Apart from the elevation of the abdominal wall, the distribution of the viscera inside the abdominal cavity is fundamental for the operating area. Depending on the position of the epigastric trocar, the lifting equipment can interfere with the surgical instruments mobility. It may be necessary to reposition the support to perform the intra-operative cholangiography. The tensional force applied to the peritoneal surface by the lifting rods is small, and no additional post-operative pain was observed using this procedure. Conclusion - These results show that using the equipment described in this study, mechanical lifting of the abdominal wall is a feasible alternative for undertaking videolaparoscopic cholecystectomy. Racional - A suspensão mecânica da parede abdominal, método baseado no mecanismo de tração e conseqüente elevação da parede abdominal, é procedimento alternativo para a criação de adequado espaço intra-abdominal necessário à cirurgia videolaparoscópica, prescindindo-se da insuflação gasosa intra-peritonial. Objetivo - Avaliar a viabilidade técnica desse procedimento para realização da colecistectomia videolaparoscópica, com a análise de aspectos clínicos e funcionais da suspensão mecânica da parede abdominal. Pacientes e Métodos - Na Disciplina de Cirurgia do Aparelho Digestivo da Faculdade de Medicina da Universidade de São Paulo foi criado equipamento para realização de cirurgia videolaparoscópica por esse método, constituído de duas partes: a externa consiste em um pórtico acoplado à mesa operatória, no interior do qual desliza um cabo de aço que é tracionado por um sistema de catraca presente na extremidade inferior de uma das hastes do pórtico; a interna, o dispositivo suspensor, tem o formato em "L", cujo ramo horizontal é constituído de três hastes giratórias, o qual é conectado ao cabo de aço após introdução na cavidade abdominal. Com esse equipamento 10 pacientes foram submetidos a colecistectomia videolaparoscópica; em um deles foi associada hiatoplastia e fundoplicatura. Foram avaliados o tempo de instalação do equipamento, as características do campo operatório, a interferência provocada pela presença do equipamento de suspensão nos movimentos operatórios e nas imagens da colangiografia intra-operatória; foram medidas a força aplicada na tração e a extensão da elevação da parede abdominal e, por fim, a necessidade de medicação para analgesia pós-operatória. Resultados - Não houve qualquer complicação intra-operatória, não sendo necessária conversão para cirurgia aberta em nenhum caso. Observou-se facilidade e segurança na introdução do suspensor, bem como eficácia do sistema de tração. No campo operatório, além da elevação da parede abdominal, desempenha papel fundamental o nível de distribuição das vísceras no interior da cavidade. Dependendo da posição do trocarte epigástrico, a presença do equipamento de suspensão pode interferir na movimentação dos instrumentos. Para realização da colangiografia intra-operatória pode ser necessário o reposicionamento do suspensor. É pequena a força tensional aplicada na superfície peritoneal pelas hastes do suspensor, não se observando qualquer indicativo de maior dor pós-operatória com esse procedimento. Conclusão - Em face desses resultados, a suspensão mecânica da parede abdominal, realizada com o equipamento descrito nesse trabalho, é alternativa viável para realização da colecistectomia videolaparoscópica.

There is controversial evidence that gastroesophageal reflux disease (GERD) is an etiologic factor for idiopathic laryngotracheal stenosis. We present the case of a 44-year-old woman with symptomatic tracheal stenosis managed as idiopathic stenosis. She underwent six endoscopic dilations during 1 year, and before surgery she underwent 24-hour esophageal pH monitoring that documented GERD. Anti-GERD treatment was started, which was confirmed as effective with 24-hour esophageal pH monitoring 3 months later. At 2-year follow-up the patient remained free of symptoms and no additional airway procedure was necessary. A close relationship between anti-GERD therapy and clinical outcome was noted.

The objective of surgical management of cancer of the esophagus is twofold: To perform radical resection of the tumor and to restore effective gastric continuity. The purpose of this paper is to analyze the short- and long-term progress of patients with squamous cell carcinoma of the esophagus submitted to this procedure, regarding the importance of lymphadenectomy on long-term survival.

The management of achalasia should depend on the degree of esophageal involvement assessed individually in order to indicate precisely the therapeutic measures for each case. Staging based on morphological and functional variables is used to distinguish the different degrees of esophageal involvement more accurately.

Obstructive diseases, especially neoplasms of the esophagus, frequently cause serious nutritional disturbances, besides the associated pulmonary changes due to aspiration of stagnant esophageal content. In addition, neoplasms occur in an older age group with a higher incidence of chronic obstructive pulmonary disease. Therefore, in these patients, there is a higher risk of postoperative pulmonary complications when performing esophagectomy via thoracotomy. Pain from thoracotomy also contributes, resulting in decreased ventilation and accumulation of secretions and pleural effusions; also the patient’s lateral position during the operation causes hypoventilation of the contralateral lung and passage of secretions from the compressed lung to the other. In order to eliminate these drawbacks, we remove the esophagus via a cervicoabdominal approach without thoracotomy [2]. This approach also allows the surgical staging of the neoplasm, with visualization of the lymph nodes of the lesser curvature, celiac trunk, and the hepatic parenchyma, and it provides better assessment of resectability without the need for thoracotomy.

Racional: Para a monitorização do refluxo ácido gastroesofágico, por pHmetria, convencionalmente, posiciona-se o sensor de pH 5 cm acima da borda superior do esfíncter inferior do esôfago. Porém, esta parece não ser a localização ideal para estabelecer-se o diagnóstico da doença do refluxo, visto que as alterações da mucosa esofágica decorrentes do refluxo ocorrem, principalmente, no segmento mais distal do órgão e que a exposição ácida diminui progressivamente do segmento mais distal do esôfago em direção ao proximal. Objetivos: Comparar dois níveis de mensuração do refluxo: o convencional com um mais distal (3 cm acima da borda superior do esfíncter inferior do esôfago) quanto à: ocorrência de alterações no traçado sugestivas de detecção de pH intragástrico, quantificação do refluxo e relação do mesmo com as queixas clínicas. Objetiva-se ainda, comparar duas modalidades de estudo da relação entre queixa clínica e refluxo: o Índice de Sintomas e a Probabilidade de Associação de Sintomas. Material e métodos: Foram estudados, prospectivamente, pacientes com sintomas típicos da doença do refluxo, submetidos à: entrevista clínica, endoscopia digestiva alta, manometria esofágica e pHmetria com dois sensores de registro: um proximal posicionado na localização convencional e um mais distal (3 cm acima da borda superior do esfíncter inferior do esôfago). Resultados: Foram incluídos 105 pacientes; 47 (44,8%) com esofagite erosiva, 34 (32,4%) com esofagite não-erosiva e 24 (22,9%) sem esofagite ao estudo endoscópico. Não houve alterações no traçado sugestivas de detecção de pH intragástrico em nenhum paciente. A quantificação do refluxo foi significantemente maior no segmento distal do esôfago, em relação ao proximal, em todos os parâmetros analisados: percentagem de tempo total de refluxo, percentagem do tempo de refluxo em posição ortostática, percentagem de refluxo em posição supina, número de episódios de refluxos, número de refluxos com duração maior que 5 minutos, duração do episódio mais longo de refluxo e pontuação de DeMeester. O Índice de Sintoma foi positivo em 46 (67,7%) pacientes no local convencional e em 49 (72,1%) no local mais distal; a Probabilidade de Associação de Sintomas foi positiva em 10 (14,4%) pacientes no local convencional e em nove (13,8%) no mais distal. Não se observou diferença significante entre os dois locais de monitorização, quanto à positividade dos índices utilizados. O Índice de Sintomas foi mais eficiente que a Probabilidade de Associação de Sintomas, em relação à capacidade de relacionar as queixas clínicas com o refluxo. Conclusões: 1. Em pacientes selecionados, não há evidências sugestivas de migração do sensor distal de pH para a câmara gástrica; 2. A quantificação do refluxo ácido foi significantemente maior na monitorização mais distal em relação à convencional; 3. A monitorização mais distal não promoveu incremento significante na capacidade de relacionar a queixa clínica com o refluxo, em relação à monitorização convencional e 4. O Índice de Sintomas foi mais eficiente que a Probabilidade de Associação de Sintomas, quanto à capacidade de relacionar a queixa clínica com o refluxo Background: For conventional gastroesophageal acid reflux monitoring, by pH-metry, a pH sensor is positioned 5cm above the upper edge of the lower esophageal sphincter. However, this appears to not be the ideal position to diagnose reflux disease, seeing as the alterations of the esophageal mucosa, arising due to reflux, mainly occur in the more distal section of the organ and that acid exposure reduces progressively from the distal section toward the proximal esophagus. Aims: to compare two levels of measurement of reflux: the conventional one with a more distal one (3cm above the upper edge of the lower esophageal sphincter) regarding: occurrence of alterations in the tract suggestive of intragastric pH detection, quantification of reflux and relation with the clinical complaints. It further aims to compare two methods of study of the relation between clinical complaints and reflux: Symptom Index and the Symptom-Association Probability. Material and Methods: Patients were studied prospectively, with typical symptoms of reflux disease, submitted to: clinical interview, upper gastrointestinal endoscopy, esophageal manometry and pH-metry with two sensors: a proximal, conventionally positioned, one and a more distal one (3cm above the upper edge of the lower esophageal sphincter). Results: 105 patients were included; 47 (44.8%) with erosive esophagitis, 34 (32.4%) with non-erosive esophagitis and 24 (22.9%) without esophagitis at endoscopic study. There were no alterations in the measurements which would suggest the detection of intragastric pH in any patient. The quantification of reflux was significantly higher in the distal section of the esophagus compared to the proximal, in all of the analyzed parameters: percentage of total time of reflux, percentage of time of reflux in orthostatic position, percentage of reflux in supine position, number of reflux episodes, number of refluxes with duration longer than 5 minutes, duration of the longest reflux episode and DeMeester Score. The Symptom Index was positive in 46 (67.7%) patients in the conventional position and in 49 (72.1%) in the more distal position; the Symptom- Association Probability was positive in 10 (14.4%) patients in the conventional position and in nine (13.8%) in the more distal one. No significant difference was observed between the two monitoring positions, or the positivity of the indices used. The Symptom Index was more efficient than the Symptom-Association Probability, in relation to its capacity to link the clinical complaints with reflux. Conclusions: 1. in selected patients, there was not suggestive evidence of migration of the distal pH sensor to the gastric chamber; 2. The quantification of acid reflux was significantly higher in the more distal compared to the conventional monitoring; 3. The most distal measurement did not promote significant increase in the ability to relate the clinical complaint with reflux, compared to conventional measurement and 4. The Symptom Index was more efficient than the Symptom-Association Probability in the capacity of relating the clinical complaints with reflux
