BACKGROUND AND OBJECTIVES: Priapism is one of the complications of sickle cell disease characterized by a persistent and painful erection, which can lead to erectile dysfunction and sexual impotence. The objective of this study was to understand how men with sickle cell disease and priapism access emergency care. METHODS: A qualitative study conducted in a reference healthcare unit to people with sickle cell disease in the second largest city in Bahia. Seven adult men with sickle cell disease who had experienced priapism participated in the study. The data were collected by semi-structured interview and thematic story designs and submitted to content analysis. RESULTS: Priapism is seen as a lack of genital health. Participants use strategies to manage it at home to avoid embarrassment, which ends up in cocooning. Access to emergency services is motivated by persistent and relentless pain; and limited by the fear of priapism being mistaken for sexual deviance, lack of knowledge about the complication as a urologic emergency and financial shortfall, which confers a worse prognosis about erectile function. Men are embarrassed and discriminated by healthcare and support professionals, which discourages them from accessing these services in the future. CONCLUSION: This study emphasizes the importance of early diagnosis of sickle cell disease, the orientation of family members and the need for healthcare professionals to educate young boys and men with sickle cell disease and their caregivers about priapism in advance to allow adequate self-care and prevent complications. RESUMO JUSTIFICATIVA E OBJETIVOS: O priapismo é uma das complicações da doença falciforme caracterizada por ereção persistente e dolorosa, podendo levar à disfunção erétil e impotência sexual. O objetivo deste estudo foi compreender como os homens com doença falciforme e priapismo acessam os cuidados nos serviços de emergência. MÉTODOS: Estudo qualitativo realizado em unidade de saúde referência para pessoas com doença falciforme no segundo maior município baiano. Participaram do estudo 7 homens adultos com doença falciforme que já vivenciaram priapismo. Utilizou-se entrevista semiestruturada e desenhos-história com o tema, analisados por análise de conteúdo. RESULTADOS: O priapismo é visto como uma falta de saúde genital. Os participantes usam estratégias para seu manuseio em domicílio para evitar constrangimentos, o que acaba isolando-os socialmente. O acesso aos serviços de emergência é motivado pela dor persistente e irredutível; e limitado pelo temor do priapismo ser confundido como resultado de desvio sexual, desconhecimento da complicação como emergência urológica e carência financeira, o que confere pior prognóstico sobre a função erétil. Os homens sofrem constrangimento e discriminação pelos profissionais de saúde e de apoio das unidades, o que os desmotiva a acessar esses serviços no futuro. CONCLUSÃO: Este estudo ressalta a importância do diagnóstico precoce da doença falciforme, da orientação de familiares e da necessidade de os profissionais de saúde educarem os meninos/homens jovens com doença falciforme e seus cuidadores sobre o priapismo de forma prévia, para permitir o adequado autocuidado futuro e prevenção de complicações.

BACKGROUND AND OBJECTIVES: Perineal pain in the puerperium of women with perineal traumas can affect the quality of life and interfere with normal activities and physiological needs. In addition, many obstetric practitioners neglect this symptom and an incipient scientific production about the characterization of this pain is observed. Therefore, this study aimed to compare the characteristics of perineal pain in women with perineal traumas due to episiotomy and laceration, according to the Brazilian Version of the McGill Pain Questionnaire, in a public maternity hospital in the interior of Bahia. METHODS: A cross-sectional study was carried out with 499 postpartum women who had a vaginal delivery and who presented with perineal pain associated with local traumas. RESULTS: The characterization of perineal pain was the same for both traumas, being described as “that jerk”, “boring” and “uncomfortable”. CONCLUSION: Perineal pain has considerable intensity and causes discomfort in women. Therefore, it is important that the episiotomy is performed in a restricted way and that the professionals seek to use techniques of perineal protection, as this will reduce the frequency of perineal pain and provide greater comfort to the woman in the immediate puerperium. RESUMO JUSTIFICATIVA E OBJETIVOS: A dor perineal no puerpério de mulheres com traumas perineais pode afetar a qualidade de vida e interferir nas atividades habituais e necessidades fisiológicas. Além disso, muitos profissionais da prática obstétrica negligenciam esse sintoma e observa-se uma incipiente produção científica acerca da caracterização dessa dor. Sendo assim, este estudo teve o objetivo de comparar as características da dor perineal em mulheres com traumas perineais decorrentes de episiotomia e laceração, conforme dimensões da Versão Brasileira do Questionário de dor de McGill, em uma maternidade pública do interior da Bahia. MÉTODOS: Estudo transversal realizado com 499 puérperas que tiveram parto vaginal e que apresentaram dor perineal associada a traumas locais. RESULTADOS: A caracterização da dor perineal foi igual para ambos os traumas, sendo descrita como “que repuxa”, “chata” e “incômoda”. CONCLUSÃO: A dor perineal possui intensidade considerável e causa desconforto nas mulheres. Sendo assim, é importante que a episiotomia seja realizada de forma restrita e que os profissionais procurem utilizar técnicas de proteção perineal, pois, dessa forma, reduzirá a frequência de dor perineal e proporcionará maior conforto à mulher no puerpério imediato.

Aim: To analyze the perception of women suffering from sickle cell anemia regarding pregnancy. Method: This is a qualitative, descriptive and exploratory study, involving 25 women suffering from sickle cell anemia who lived in Bahia. Data were obtained through open interviews and analyzed by the Collective Subject Discourse. Results: Three central ideas emerged from the discourses: fear and anxiety marked the pregnancy of women suffering from sickle cell anemia; dilemmas permeated the decision to abort, and; the desire to be a mother was frustrated by miscarriage. Discussion: Due to the lack of equipment and lack of social support for exercising motherhood, women experience sadness, depression and think of interrupting the pregnancy. Conclusion: The choice to have children, although risky, should be ensured by public policy with an integral assistance to women suffering from sickle cell anemia, involving the totality of their reproductive trajectory.
