Abstract

The advance of digitalization has generated an ever-increasing number of options in the digital sphere. This phenomenon has transformed multiple sectors, including e-commerce and, in particular, the fashion industry. Artificial intelligence (AI) has emerged as a powerful tool that is redefining the online shopping experience. However, there is little research on how AI influences this purchasing process, limiting its full exploitation. This study provides new insight into how artificial intelligence influences online purchase intention in this sector by examining Generation Z consumers’ attitude and purchase intention using the Echo Look AI device. This study involved 210 university students aged between 18 and 25 years old who were surveyed in the cities of Madrid and Barcelona. The results indicate that perceived quality, attitude towards AI and perceived usefulness have a positive influence on purchase intention. Based on these results, theoretical and practical implications are discussed.

Abstract

AbstractConsumer interactions on social media have been analysed in the literature as sources of information about their ability to generate engagement and knowledge about how to interact with brands. The present study contributes to this line of research, focusing on empirically determining those elements that affect the interaction between a fast fashion brand and its users, providing central elements of brand strategies on Instagram. This study examines the engagement generated by Primark posts on Instagram based on three coding categories that analyse the content of the message, the communication strategy and the formal and product category aspects present in the posts. This study therefore identifies the variables with the greatest potential to generate engagement with this brand’s posts on Instagram, which in turn predicts previous unexplored outcomes such as brand image and consumer loyalty. This study also provides valuable academic insights and guides fast fashion brands in their social media strategies.

Abstract

Summary Background & aims COVID-19 patients present a high hospitalization rate with a high mortality risk for those requiring intensive care. When these patients have other comorbid conditions and older age, the risk for severe disease and poor outcomes after ICU admission are increased. The present work aims to describe the preliminary results of the ongoing NUTRICOVID study about the nutritional and functional status and the quality of life of adult COVID-19 survivors after ICU discharge, emphasizing the in-hospital and discharge situation of this population. Methods A multicenter, ambispective, observational cohort study was conducted in 16 public hospitals of the Community of Madrid with COVID-19 survivors who were admitted to the ICU during the first outbreak. Preliminary results of this study include data retrospectively collected. Malnutrition and sarcopenia were screened at discharge using MUST and SARC-F; the use of healthcare resources was measured as the length of hospital stay and requirement of respiratory support and tracheostomy during hospitalization; other study variables were the need for medical nutritional treatment; and patients’ functional status (Barthel index) and health-related quality of life (EQ-5D-5L). Results A total of 176 patients were included in this preliminary analysis. Most patients were male and older than 60 years, who suffered an average (SD) weight loss of 16.6% (8.3%) during the hospital stay, with a median length of stay of 53 (27–89.5) days and a median ICU stay of 24.5 (11–43.5) days. At discharge, 83.5% and 86.9% of the patients were at risk of malnutrition and sarcopenia, respectively, but only 38% were prescribed medical nutrition therapy. In addition, more than 70% of patients had significant impairment of their mobility and to conduct their usual activities at hospital discharge. Conclusions This preliminary analysis evidences the high nutritional and functional impairment of COVID-19 survivors at hospital discharge and highlights the need for guidelines and systematic protocols, together with appropriate rehabilitation programs, to optimize the nutritional management of these patients after discharge.

Abstract

There is a lack of data about the effects of remote ischemic postconditioning (RIPostC) on hypoxia-inducible factor-1α (HIF-1α) plasma levels after on-pump cardiac surgery (OPCS). This study aimed to measure the effects of RIPostC on postoperative HIF-1α plasma levels, cardiac markers and arterial oxygenation in patients undergoing OPCS. This single-centre randomized, double blind, controlled trial, enrolled 70 patients (35 control and 35 RIPostC). RIPostC was performed by 3 cycles (5 min of ischemia followed by 5 min of reperfusion) administered in upper arm immediately after the pump period. The primary outcome was to measure HIF-1α plasma levels: before surgery (T0), and 2 h (T1), 8 h (T2), 24 h (T3), 36 h (T4) and 48 h (T5) after RIPostC. As secondary endpoint, Troponin T, CK-MB, CPK plasma levels and PaO2/FiO2 ratio were measured. HIF-1α plasma levels were increased at T1–T3 compared to T0 in both groups (P

Abstract

Resumen Objetivos Conocer la prevalencia de enfermedad renal cronica (ERC) y determinar los factores asociados al deterioro de la funcion renal en poblacion asistida en atencion primaria. Pacientes y metodo Estudio transversal y multicentrico realizado en los pacientes basales del estudio IBERICAN (identificacion de la poblacion espanola de riesgo cardiovascular y renal). Se considero ERC a un filtrado glomerular estimado (FGe) Resultados Se incluyeron 7.895 pacientes (edad media 57,9 ± 14,8 anos; 54,5% mujeres). El 14,4% (IC 95%: 13,6-15,1) presentaba ERC; en hombres un 16,1% (IC 95%: 14,9-17,3) y en mujeres un 12,9% (IC 95%: 11,9-14,0). Se observo un aumento continuo de la prevalencia con la edad (24,8% ≥ 65 anos) y con la agregacion de FRCV. Las variables que mas se asociaron a la probabilidad de padecer ERC fueron hipertrofia ventricular izquierda (OR: 1,95; p Conclusiones Catorce de cada 100 pacientes incluidos en el estudio IBERICAN presenta ERC. La prevalencia de ERC afecta a la cuarta parte de pacientes ≥ 65 anos y aumenta exponencialmente con la agregacion de FRCV.

6. Life cycle assessment of charcoal production and electricity generation from eucalyptus in an industrial batch kiln

Abstract

Brazilian charcoal is produced from planted eucalyptus forest wood in traditional batch reactors. The ordinary technology applied in Brazil does not use pyrolysis waste gases, which leads to the loss of 30% of wood energy and decreases air quality. This study evaluated the synchronous use of industrial batch kilns, waste gas burning, and energy recovery to produce electricity. Three scenarios were analyzed: (S1) Eucalyptus charcoal production without gas burning (Base Scenario); (S2) with gas burning; and (S3) with gas burning and electricity generation. Since a eucalyptus forest can fix carbon into its biomass through photosynthesis and finally into charcoal, S1 was able to reduce 3402.5 kg of CO2-eq per Mg of charcoal produced, and S2 reduced 6453.1 kg of CO2-eq due to waste gas methane burning. Electricity production is environmentally positive for all evaluated environmental indicators thanks to gas pollutants destruction and renewable energy generation. For S1, a ratio difference of 6.3 was found between the output of renewable energy and fossil energy input during the charcoal life cycle. For a combined production of charcoal and electricity (S3), a ratio difference of 6.9 was found. Photochemical oxidation was the main impact which can be significantly reduced by adopting gas flaring.

Abstract

This report describes the case of a pregnant woman who arrived for preanesthetic assessment of External Cephalic Version (ECV) for fetus in breech presentation and cesarean section in case of ECV failure. Although the technique seems simple, attempts to rotate the fetus can result in elevated intracranial pressure, which might cause malformation bleeding. The most appropriate anesthetic technique in cases of arteriovenous malformations during C-sections has not been determined. Neuroaxial anesthesia is safe only in stable brain cavernomas, but the presence of spinal malformations contraindicates it. Anesthetic goals include stabilizing the blood pressure and reducing the risk of rupture.

Abstract

Background: Despite the frequent use of the Timed Up and Go (TUG) test in clinical trials, evaluation of longitudinal test-retest reliability is generally lacking and still inconclusive for patients with Parkinson’s disease (PD). Objective: We aimed to further investigate long-term reliability and sensitivity of the TUG test among this population. Furthermore, we explored alternative assessment strategies of the test aimed at elucidating whether the inclusion or combination of timed trials may have potential implications on outcome measure. Methods: Relative and absolute reliability of the TUG performance were obtained in forty-three subjects with PD over three timed trials in two different testing sessions separated by a two-months period. Results: Our results reported excellent intra-session and moderate inter-session reliability coefficients. The use of different assessment strategies of the TUG was found to have an important impact on outcome measure, highlighting the averaging of several timed trials in each testing session as a recommended alternative to minimize measurement error and increase reliability in longitudinal assessments. Nevertheless, beyond acceptable reliability, poor trial-to-trial stability of the measure appears to exist, since the ranges of expected variability upon retesting were wide and the incidence of spurious statistical effects was not negligible, especially in longitudinal repeated testing. Conclusion: Limitations may exist in the interpretation of the TUG outputs as part of longitudinal assessments aimed at evaluating treatment effectiveness in PD population. Researchers and practitioners should be aware of these concerns to prevent possible misrepresentations of functional ability in patients for a particular intervention.

Abstract

Trabajo presentado en la 18th European Conference on Thermoelectrics, celebrada en Barcelona (España), del 14 al 16 de septiembre de 2022, In the last few years, the exploration of new thermoelectric materials with low-toxicity, earth-abundance, and high-efficiency has become essential. Following this trend, sustainable, easily scalable, and cost-effective fabrication methods, such as electrochemical deposition, are also desirable. In this work, the Pourbaix diagram of silver¿selenium¿water was developed to find an adequate pH and reduction potential for the electrodeposition of stable silver selenide. Based on this diagram, a solution without the incorporation of additives was developed. Silver selenide films were electrodeposited at different reduction potentials, and after the deposition, the compositional, morphological, and structural characterizations of the silver selenide thin films were analysed. The thermoelectric properties of the electrodeposited silver selenide films were measured at room temperature. The maximum power factor was found for the films grown at 0.071 V with a value of 3421 ± 705 mW/·m-1·K-2 and a thermal conductivity of 0.56 ± 0.06 W/m-1·K-1. Even better, when it can be done by employing a technique that is easily scalable to an industrial level and allows large areas to be obtained, such as electrodeposition. Finally, films with similar properties were deposited on a flexible Kapton substrate. A unileg thermoelectric power generator was produced with maximum output powers of 14.7, 29.4, and 37 mW under temperature differences of 10, 15, and 19 K, respectively; and maximum power densities of 55.1, 110.1, and 138.6 mW·m-2 under temperature differences of 10, 15, and 19 K, respectively.

Abstract

Aims Iatrogenic varus malreduction after intramedullary nailing of intertrochanteric fractures (wedge effect) may be more common than previously reported. The aim of study is to determine its incidence and to define easy fluoroscopic landmarks to recognize it intraoperatively. Patients and methods We included surgical procedures with residual varus malreduction and sufficient intraoperative fluoroscopy images to assess an intraoperative anatomical reduction at the beginning of the procedure. Two intraoperative fluoroscopic landmarks were defined to assess varus in the anteroposterior view: the position of the greater trochanter (GT) related to the femoral shaft (medialized GT sign), and the position (superior, centre or inferior) of the guide wire in both the neck and the head of the femur (cross wire sign). We describe our current method of treatment to avoid this complication. Results A total of 369 extracapsular hip fractures were treated using PFNA and Gamma 3 systems. We found 55 (14.9%) fractures with a varus malreduction. Six were underreduced and fixed in varus. Thirty-five (9.48%) were well reduced by closed reduction but suffered a iatrogenic varus displacement when introducing the nail. 31 were female; mean age was 86.6 (range 70-97). Mean postoperative head-shaft angle was 119,2° range (113°-123°). Mean TAD was 24,37 range (14-36 mm). Conclusion Wedge effect (iatrogenic varus displacement of a well reduced intertrochanteric fracture when introducing the nail) could occur in at least 10% of intertrochanteric fractures. The medialized GT and the “crossing wire sign” seem to be good predictors of iatrogenic varus malalignement.

Abstract

Purpose To report healthcare resource use and associated costs in controlled versus uncontrolled carcinoid syndrome (CS) in patients with neuroendocrine tumours. Methods A cross-sectional, non-interventional multicentre study was conducted with retrospective data analysis. Resource use was compared between two patient groups: those with controlled CS (> 12 months with no uncontrolled CS episodes) and uncontrolled CS ( Results Twenty-six university hospitals in Spain participated, between July 2017 and April 2018. 137 patients were enrolled; 104 were analysed (2 groups of 52). Patients with uncontrolled CS had 10 times more emergency department (ED) visits (mean 1.0 vs 0.10 visits; P = 0.0167), were more likely to have a hospital admission (40.4% vs 19.2%; P = 0.0116) and had longer hospital stays (mean 7.87 vs 2.10 days; P = 0.0178) than those with controlled CS. This corresponded to higher annual hospitalisation costs (mean €5511.59 vs €1457.22; P = 0.028) and ED costs (€161.25 vs €14.85; P = 0.0236). The mean annual total healthcare costs were 60.0% higher in patients with uncontrolled than controlled CS (P = NS). Conclusion This study quantifies higher health resource use, and higher hospitalisation and ED costs in patients with uncontrolled CS. Better control of CS may result 3in lower medical costs.

12. La persistencia de síntomas de la COVID-19 y su diagnóstico en la primera ola de la pandemia en España

Desde el inicio de la pandemia por SARS-CoV-2 al menos un 10 % de los contagiados pueden presentar sintomas que persisten mas alla de las 12 semanas, lo que se denomina “COVID persistente” o Long COVID (LC). El reconocimiento esta lejos de alcanzarse debido, en gran parte, a dudas respecto al diagnostico de partida. Este estudio pretende describir la situacion de los afectados por LC al respecto de su acceso a la realizacion de pruebas diagnosticas, su resultado e interpretacion, asi como la repercusion en el diagnostico y en la tipologia de los afectados en la primera ola de la pandemia en Espana. Material y metodos. Se realizo una encuesta online, voluntaria, autocumplimentada, anonimizada, entre el 13 de julio y el 14 de octubre de 2020. Se incluyeron preguntas referentes a la edad, sexo, residencia, pruebas de diagnostico y seguimiento, asi como el tiempo de persistencia de sintomas. Resultados. De los 2.120 pacientes que respondieron, cumplieron cumplieron criterios de LC 1.834, 1.448 mujeres (79 %), edad media de 43,3 anos; el 50 % central se encontraba entre 36 y 50 anos de edad. La media de persistencia de sintomas fue de 185,8 dias. Manifiestan que se les realizaron pruebas diagnosticas 1.437 encuestados (78,4 %); en 1.207 de ellos (65,8 %) fue PCR. El resultado fue positivo en 880 personas, lo que supone un 72,9 % de las realizadas y un 48 % de la muestra total. Al 46,4 %, sin embargo, se les realizo una PCR de seguimiento. No se detectaron diferencias estadisticamente significativas ni clinicamente relevantes en funcion de la PCR, ni en relacion con los sintomas ni con el tiempo de su persistencia. Comentario. Exigir una PCR positiva en la orofaringe en algun momento del curso clinico de los pacientes con LC en la primera ola de la pandemia en Espana constituye una incoherencia con lo sucedido, y una penalizacion de los afectados a los que en un primer momento se nego la prueba y ahora se les niega un diagnostico por no aportar un resultado positivo de la misma. Since the beginning of the SARS-CoV-2 pandemic, at least 10% of those infected may present symptoms that persist beyond 12 weeks, which is called Persistent COVID or Long COVID (LC). Recognition is far from being achieved due, in large part, to doubts regarding the initial diagnosis. This study aims to describe the situation of those affected by CL with respect to their access to diagnostic tests, their result and interpretation, as well as the impact on the diagnosis and the typology of those affected in the first wave of the pandemic in Spain. Material and methods. An online, voluntary, self-completed, anonymized survey was carried out between July 13 and October 14, 2020. Questions regarding age, sex, residence, diagnostic and follow-up tests, as well as the time of persistence of symptoms were included. Results. Of the 2,120 patients, 1,834 met CL criteria, 1,448 women (79%), mean age 43.3 years, and the central 50% were between 36-50 years of age. The mean persistence of symptoms was 185.8 days. They state that 1437 respondents (78.4%) underwent diagnostic tests, in 1,207 (65.8%) it was PCR. The result was positive in 880 people out of the 1207 PCR carried out, which represents 72.9% of the PCR carried out and 48% of the total sample. 46.4%, however, underwent a follow-up PCR. No statistically significant or clinically relevant differences were detected as a function of CRP, neither in relation to symptoms nor the time of their persistence. Discussion. Requiring a positive PCR in the oropharynx at some point in the clinical course of LC patients in the first wave of the pandemic in Spain constitutes an inconsistency with what happened, and a penalty for those affected who were initially denied the test and now they are denied a diagnosis for not providing a positive test result.

Abstract

Background Surgical strategies are being adapted to face the COVID‐19 pandemic. Recommendations on the management of acute appendicitis have been based on expert opinion, but very little evidence is available. This study addressed that dearth with a snapshot of worldwide approaches to appendicitis. Methods The Association of Italian Surgeons in Europe designed an online survey to assess the current attitude of surgeons globally regarding the management of patients with acute appendicitis during the pandemic. Questions were divided into baseline information, hospital organization and screening, personal protective equipment, management and surgical approach, and patient presentation before versus during the pandemic. Results Of 744 answers, 709 (from 66 countries) were complete and were included in the analysis. Most hospitals were treating both patients with and those without COVID. There was variation in screening indications and modality used, with chest X‐ray plus molecular testing (PCR) being the commonest (19·8 per cent). Conservative management of complicated and uncomplicated appendicitis was used by 6·6 and 2·4 per cent respectively before, but 23·7 and 5·3 per cent, during the pandemic (both P, The COVID‐19 pandemic required reorganization of surgical services, affecting patients with common surgical diseases including acute appendicitis. No evidence is available on the topic. This study found global variation in screening policies, use of personal protective equipment and intraoperative directives. Abstract

Introduction Disease-related knowledge is a key component of shared decision making and a relevant outcome to measure the effectiveness of information provision interventions. However, no systematic psychometric reviews have been found that assess the measurement instruments aimed at evaluating the disease-related knowledge of people affected by multiple sclerosis. This review aims to systematically assess the quality of the measurement properties of all available disease-related knowledge measurement instruments of people affected by multiple sclerosis. Methods and analysis A systematic psychometric review will be carried out in accordance with the guidelines proposed by the international ‘COnsensus-based Standards for the selection of health Measurement INstruments (COSMIN)’ initiative. Studies that meet the following criteria will be selected: (1) those whose aim is to measure disease-related knowledge, (2) those whose study populations are affected by multiple sclerosis and (3) those whose aims are to develop measurement instruments or evaluate one or more of their measurement properties. The information sources will be MEDLINE (via PubMed), CINAHL, PsycINFO and OpenGrey. The methodological quality will be assessed using the ‘COSMIN Risk of Bias’ checklist. Available evidence will be synthesised and graded using a modified Grading of Recommendations Assessment, Development and Evaluation approach. Ethics and dissemination As this is a systematic review, no ethics approval is needed. Study findings will be shared with multiple sclerosis patient support groups and in reports to funders. Abstract

Resumen Objetivo Se aconsejan diferentes medidas para disminuir la pancreatitis poscolangiopancreatografia retrograda endoscopica (PPCPRE). Efectuamos un estudio en pacientes con CPRE tratados con diclofenaco rectal o Ringer Lactato o bien ambas intervenciones para valorar si existe una disminucion en el numero de PPCPRE. Material y metodos Estudio de cohortes mixto con 1.896 pacientes desde 2009 hasta 2018. Hasta junio de 2012 sin tratamiento (grupo i ). Posteriormente 100 mg de diclofenaco rectal (grupo ii ). Desde 2016 Ringer Lactato 200 ml/h durante el procedimiento y 4 h despues del mismo, ademas 500 ml en 30 min cuando se canulo el pancreas (grupo iii ). Desde 2017 Ringer Lactato mas diclofenaco (grupo iv ). Hubo 725 pacientes en el grupo i , 530, 227 y 414 pacientes en grupos ii , iii y iv . Se han recogido factores predisponentes a PPCPRE y los casos de PPCPRE que fue definida por criterios de consenso. Resultados Hubo 65 PPCPRE (3,4%); 2,9; 3,4; 3,1 y 4,3% en los grupos i , ii , iii y iv respectivamente (p = 0,640). En el grupo i hubo un 4,2% de PPCPRE en papilas naive y un 4; 4,9% y 6,3% en los grupos ii , iii y iv respectivamente (p = 0,585). La gravedad de PPCPRE y los efectos adversos fueron similares en los grupos. El 38,4% eran pacientes de alto riesgo. Tampoco hubo diferencias de PPCPRE en este grupo (p = 0,501). Conclusion En este trabajo no se ha obtenido beneficio con diclofenaco mas hidratacion en la disminucion del numero y gravedad de la PPCPRE. Tampoco con las otras medidas profilacticas.

Abstract

Objective Different measures are recommended to reduce pancreatitis after endoscopic retrograde cholangiopancreatography (ERCP). We conducted a study in patients with ERCP treated with rectal diclofenac or lactated Ringer’s solution, or both interventions, to assess whether there is a decrease in the number of cases of post-ERCP pancreatitis. Material and methods A mixed cohort study involving 1896 patients from 2009 to 2018. Up to June 2012 without treatment (Group I). Subsequently, 100 mg of rectal diclofenac (Group II). Since 2016, lactated Ringer’s solution 200 ml/h during the procedure and 4 h after it, in addition to 500 ml over 30 min when the pancreas was cannulated (Group III). Since 2017, lactated Ringer’s solution plus Diclofenac (Group IV). There were 725 patients in group I, and 530, 227 and 414 patients in groups II, III and IV, respectively. Factors predisposing to post-ERCP pancreatitis and post-ERCP pancreatitis cases that were defined by consensus criteria have been collected. Results There were 65 cases of post-ERCP pancreatitis (3.4%); 2.9%, 3.4%, 3.1% and 4.3% in groups I, II, III and IV, respectively (p = .640). In group I, there was 4.2% of post-ERCP pancreatitis in naive papillae and 4%, 4.9% and 6.3% in groups II, III and IV, respectively (p = .585). The severity of post-ERCP pancreatitis and adverse effects were similar in all groups. 38.4% were high-risk patients. There were also no differences in post-ERCP pancreatitis in this group (p = .501). Conclusion In this work, no benefit was obtained with diclofenac plus hydration in reducing the number and severity of cases of post-ERCP pancreatitis nor with the other prophylactic measures.

Abstract

Background: A vascular access team (VAT) was created in 2018 with the aim of improving vascular access and reducing complications associated with catheters. The impact of the introduction of a VAT in the insertion and maintenance of peripheral insertion central catheters (PICCs) was assessed. The cost-benefit associated with the use of a VAT was evaluated and the satisfaction of patients and professionals interacting with the VAT was measured. Methods: In a prospective study, 275 PICCs inserted by the VAT were assessed for their impact on complications. PICCs were implanted with maximum barrier measures using an ultrasound with IC-ECG. Also, patient and professional satisfaction have been analysed thought a questionnaire over the phone or online, and hospital financial data was used to assess the cost impact of the insertion methodology followed by the VAT versus Anaesthesia Service. Results: The thrombosis rate was 2.5% (7) and the bacteraemia rate was 1.1% (3). The use of the IC-ECG was correlated with a lower complication than the RX + IC-ECG (OR = 3.28, p = 0.021). In addition, there was a high level of perceived satisfaction for the patients surveyed and for the healthcare professionals involved in the care and management of these devices. The calculated saving for the implementation of the VAT was 61.81% compared with PICCs implanted in Anaesthesia Service. Conclusion: Low complication rates and high overall satisfaction scores in patients and professionals were observed, showing that a specialist VAT can have a positive impact in the insertion of PICCs and which also has a clear economic benefit.

Abstract

OBJECTIVES: Low-molecular-weight heparins are widely used in clinical practice for the treatment or prophylaxis of venous thromboembolism (VTE). As these drugs are eliminated mainly by renal means, any renal function impairment may lead to higher plasma concentrations and increase the risk of bleeding. This study aims to evaluate whether in clinical practice there is an increase in the occurrence of bleeding in patients with renal insufficiency (RI) during treatment or prophylaxis with dalteparin, and to analyse the risk factors potentially influencing the appearance of such bleeding events. METHODS: Patients were sampled from the Universitary Severo Ochoa Hospital, Leganés, Spain. This was a retrospective cohort study with a 1 year inclusion period, conducted at a Spanish university hospital with 400 beds, on patients undergoing treatment or prophylaxis for VTE with dalteparin for a minimum of 3 days. The main outcome measure was the number of patients who had bleeding events, independently of their severity, during dalteparin administration in patients with RI. RESULTS: 367 patients were included in the study. Bleeding occurred in 17.9% of patients in the group with RI and in 7.3% of patients with normal renal function (NRF). Most haemorrhages in both cohorts were grade 2 on the WHO scale (64.7% in the RI group and 69.2% in the NRF group). Abstract

Objective: To estimate the prevalence of uveitis and to describe its etiologic and anatomical patterns based on a population study carried out in a Spanish region. Material and methods: A cross-sectional, descriptive, population-based multicenter study was conducted. The selection criteria consisted of having a diagnosis of uveitis. All data were collected from existing information in medical records.Clinical information was collected in all cases that had a diagnosis of uveitis, regardless of its etiology, in participating centers from the date of the study to the end of the following year. All patients underwent a complete ophthalmological examination, which included assessment of their visual acuity, biomicroscopy, applanation tonometry, and indirect ophthalmoscopy. Results: During the study, 389 cases of uveitis were registered. The prevalence was 58.7 (95% confidence interval [CI] 53.0-64.9). The mean age was 47.0 ± 20.6 years and 57.8% were women. The most prevalent anatomical pattern was anterior uveitis (54.2; 95% CI 48.1-60.8). For adults, the idiopathic group constituted the highest prevalence (31.7; 95% CI: 27.1-36.9), while autoimmune etiology was most frequent for children (10.6; 95% CI: 5.8-17.7). Conclusion: The results of this population-based study offer a representative estimate of the magnitude of uveitis in this area of Spain. Abstract

Educational inclusion represents a commitment to attend to diversity, based on accessible curricular structures able to respond to the needs of an increasingly diverse student body. In the Campaign Education to transform lives, promoted by Unesco, in addition to approving the 2030 Agenda, the 17 Sustainable Development Goals were specified, claiming the need for education systems to move towards inclusive education capable of eradicating discriminatory situations of distress and social injustice. Within the university, this challenge involves the formation of student-focused learning spaces, as shown by the results of several investigations, which recognize the commitment with the educational inclusion that the current university must assume. The purpose of this study has been to identify and understand the perceptions of education professionals at the University of Malaga in the implementation of the education committed to inclusion. Following a qualitative methodology, teachers from the Primary Education and Social Education Grades have participated, collecting information from semi-structured ad hoc interviews. The main contributions of this research in its field of study show that teachers have some difficulties in addressing educational inclusion, as they lack specific training. In addition to this, the professional sector considers itself responsible for offering a response to diversity, opting for innovative methodologies that help to overcome the barriers that prevent the increase of knowledge and that commit to promote shared learning experiences.

Abstract

This study explores the online financing forms and practices of contemporary Chinese animation cinema. The research focuses on the use of crowdfunding by this industry, and analyses three recent cases, One Hundred Thousand Bad Jokes (2014), Monkey King: Hero Is Back (2015), and Big Fish & Begonia (2016), selected based on the perspective of the high economic revenue earned and the artistic and creative singularity of these films, as well as the consideration of the undeniable influence of these productions on the imagination of new generations. Using an exploratory and descriptive research methodology, the authors uncover the main features of the production of animated films based on crowdfunding. The results show that obtaining funds is a secondary objective for these movies that mainly use crowdfunding as a promotional strategy for creating an active fan base and channelling their affective involvement towards the communicative interests of each project.

Abstract

Though most studies have been conducted in the luxury electronic commerce sector, few have focused on the influence of Instagram. This study improves research by examining new dimensions of users’ experiences on this social network. This study aims to provide the core elements of brand engagement on Instagram, focusing on an empirical comparison of the relationships established between two fashion retailers and their followers on Instagram. The study is based on the coding of 728 posts collected over 1 year and carries out a comparative analysis using bivariate and multivariate modelling of the interactions between brands and their followers. The results of this work show that fashion retailers still generate little interaction with Instagram users, although there is some engagement between the two parties. The empirical results are evidence of the importance of a communication strategy in generating involvement and engagement with the brand. The study thereby provides industry professionals with the patterns in those posts that can encourage greater engagement with and participation from users online.

[ES] Objetivo Explorar las opiniones de los usuarios y de los facultativos sobre el avance de la e-salud en atención primaria. Diseño Grupos focales realizados entre el 1 de mayo y el 25 de septiembre de 2018. Emplazamiento Nivel de atención primaria en los servicios autonómicos de salud. Participantes Un total de 29 usuarios y 33 facultativos vinculados a atención primaria contactados, respectivamente, a través de asociaciones de usuarios y por medio del servicio autonómico de salud. Método Análisis inductivo de las transcripciones de grupos focales en torno a los siguientes temas: accesibilidad y seguridad; eficiencia; problemas de información y técnicos; potencialidades. Resultados Los servicios de salud en línea son positivamente valorados por usuarios y facultativos de atención primaria en términos de ahorro de costes. No obstante, la interacción presencial sigue considerándose clave en la calidad de servicio. Se comparte la preocupación por la seguridad de los datos y por las deficiencias formativas de usuarios y de facultativos en el acceso y la provisión de servicios de e-salud. El diseño adecuado de las aplicaciones constituye un condicionante para la aceptación y la difusión de las tecnologías que sostienen la salud en línea. Conclusiones La administración sanitaria debería intensificar la utilización de las nuevas tecnologías con un enfoque facilitador de la labor del facultativo, simplificando tareas burocráticas, aportando seguridad diagnóstica, prescriptiva y de protección de datos. [EN] Objective To find out the opinions of users and primary care doctors on the progress of e-health in primary care. Design Focus groups set up between 1st May and 25th September 2018. Location Primary care level in regional online health services. Participants A total of 29 users and 33 primary care doctors linked to primary care that were contacted, through user associations and regional public health services, respectively. Method Inductive analysis of transcripts from focus groups about the following issues: accessibility and security; efficiency; information and technical problems; potentialities. Results Online health services are positively valued in terms of cost savings both by users and doctors of primary care. Nevertheless, face-to-face interaction is still considered a key factor in the quality of service. When it comes to the access and provision of e-health services, there is shared concern for the security of the data and for the training deficiencies of users and doctors. An adequate design of the applications becomes a condition for the acceptance and diffusion of technologies that support online health. Conclusions Health authorities should intensify the use of new technologies oriented towards easing the work of doctors, simplifying bureaucratic tasks, providing diagnostic and prescription security, and protecting data. As long as systems generate failures and mistrust, new technologies will remain to be perceived as a complement, but not as an alternative to the traditional care model. espanolIntroduccion y objetivos: Las lesiones coronarias calcificadas pueden impedir una correcta expansion del stent que en ocasiones conduce a eventos adversos. La litotricia intracoronaria es una nueva herramienta de modificacion de la placa, cuyas seguridad y viabilidad en pacientes con enfermedad coronaria estable han sido evaluadas en el ensayo Disrupt CAD II. Aunque su uso ha aumentado rapidamente, hasta el momento solo se han comunicado casos aislados en escenarios concretos. Se presentan los resultados en pacientes clinicamente complejos de la vida real con calcificacion coronaria grave. Metodos: Entre octubre de 2018 y marzo de 2019 se trato a 25 pacientes (37 lesiones) en 2 centros espanoles, lo que representa el 2,7% de los pacientes tratados con intervencion coronaria percutanea. Resultados: Las tasas de exito clinico y del dispositivo fueron del 84 y el 95%, y no se observaron complicaciones relacionadas con el procedimiento. En todos los casos se consiguio cruzar la lesion con el balon de litotricia intracoronaria, si bien en el 8% de los casos se rompio el balon durante el inflado. Se trataron con exito lesiones complejas, como oclusiones coronarias totales y estenosis del tronco comun. En comparacion con el estudio Disrupt CAD II, nuestros pacientes eran mas jovenes, pero tenian peor escenario clinico, con mayor prevalencia de diabetes (68%) e insuficiencia renal (22%), y hasta el 76% se presento como sindrome coronario agudo. En 3 de 4 pacientes con infraexpansion de stent tratados con litotricia intracoronaria no se consiguio una expansion adecuada tras el procedimiento. No hubo complicaciones ni mortalidad hospitalaria. Un paciente fallecio por causa no cardiaca a los 30 dias de seguimiento. Conclusiones: La litotricia intracoronaria se ha demostrado efectiva y segura en una cohorte de pacientes complejos de la vida real con lesiones calcificadas. One patient died of non-cardiac causes at the 30-day follow-up. Conclusions: The ICL-assisted percutaneous coronary intervention was performed safely and effectively in a real-life cohort of patients with calcified and highly complex lesions.

Abstract

Objective the main purpose of this study is to determine the agreement between two radiological tests, ultrasonography and magnetic resonance, in the diagnosis of distal brachial biceps tendon injury. Material and method A retrospective study was made of patients over 3 years who underwent distal brachial biceps tendon reattachment open surgery, and whose preoperative diagnosis was complemented either by ultrasonography, by magnetic resonance or both. The agreement was calculated by Cohen’s kappa coefficient (k). Results information of 79 patients was analyzed. The concordance was excellent between resonance and surgical findings (k = .950), and a good Cohen´s kappa coefficient was also achieved between both ultrasonography and surgery (k = .706) and between information of sonography and magnetic resonance (k = 0.667). Conclusions Resonance and ultrasonography could be considered reliable diagnostic tests to determine the acute rupture of the distal brachial biceps tendon in patients with consistent aetiology and physical examination.

Introducción. El propósito de este artículo es analizar las imágenes publicadas, por los principales candidatos, durante la campaña electoral a la presidencia del Gobierno de España en junio de 2016. Metodología. Esta investigación realiza un análisis cualitativo a través del empleo del análisis de contenido se estudia la tipología de cada imagen que en total suponen un corpus de estudio de 1325 imágenes. Resultados. Los datos revelan la construcción de la comunicación política que cada candidato trasmite al elector en la red social Twitter. Conclusiones. En este trabajo se alcanzan dos conclusiones esenciales: En primer lugar, la imagen se asienta como la herramienta de comunicación en Twitter, finalmente, un uso excesivo de Twitter en tiempo electoral no implica un mayor éxito entre la audiencia.

Abstract

Frailty; Psychometrics; Feasibility Fragilitat; Psicometria; Viabilitat Fragilidad; Psicometría; Viabilidad The study aimed to assess the reliability of the scores, evidence of validity, and feasibility of the Frail-VIG index. A validation study mixing hospitalized and community-dwelling older people was designed. Intraclass correlation coefficient (ICC) was used to assess the inter-rater agreement and the reliability. The construct validity of the Frail-VIG index with respect to the Frailty Phenotype (FP) was evaluated by calculating the area under the receiver operating characteristic curve (AUC-ROC). Convergent validity with the Clinical Frailty Scale (CFS) was assessed using Pearson’s correlation coefficients. The feasibility was evaluated by calculating the average time required to administer the Frail-VIG index and the percentage of unanswered responses. A sample of 527 older people (mean age of 81.61, 56.2% female) was included. The inter-rater agreement and test–retest reliability were very strong: 0.941 (95% CI, 0.890 to 0.969) and 0.976 (95% CI, 0.958 to 0.986), respectively. Results indicated adequate convergent validity of the Frail-VIG index with respect to the FP, AUC-ROC 0.704 (95% CI, 0.622 to 0.786), and a moderate to strong positive correlation between the Frail-VIG index and CFS (r = 0.635, 95% CI, 0.54 to 0.71). The Frail-VIG index administration required an average of 5.01 min, with only 0.34% of unanswered responses. The Frail-VIG index is a reliable, feasible, and valid instrument to assess the degree of frailty in hospitalized and community-dwelling older people.

Abstract

BACKGROUND: In patients receiving total parenteral nutrition (TPN), the frequency of hyponatraemia is high. However, the causes of hyponatraemia in TPN have not been elucidated, although diagnosis is required for appropriate therapy. The aim of this study is to describe the aetiology of hyponatraemia in non-critical hospitalised patients receiving TPN. METHODS: Prospective multicentre study in 19 Spanish hospitals. Non-critically hyponatraemic patients receiving TPN and presenting hyponatraemia over a 9-month period were studied. Data collected included sex, age, previous comorbidities, and serum sodium levels (SNa) before and following TPN initiation. Parameters for study of hyponatraemia were also included: clinical volaemia, the presence of pain, nausea, gastrointestinal losses, diuretic use, oedema, renal function, plasma and urine osmolality, urinary electrolytes, cortisolaemia, and thyroid stimulating hormone. RESULTS: 162 patients were included, 53.7% males, age 66.4 (SD13.8) years. Volume status was evaluated in 142 (88%): 21 (14.8%) were hypovolaemic, 96 (67.6%) euvolaemic and 25 (17.6%) hypervolaemic. In 111/142 patients the analytical assessment of hyponatraemia was completed. Hypovolaemic hyponatraemia was secondary to GI losses in 10/111 (9%), and to diuretics in 3/111 (2.7%). Euvolaemic hyponatraemia was due to Syndrome of Inappropriate Antidiuretic Hormone secretion (SIADH) in 47/111 (42.4%), and to physiological stimuli of Arginine Vasopressin (AVP) secretion in 28/111 (25.2%). Hypervolaemic hyponatraemia was induced by heart failure in 19/111 (17.1%), cirrhosis of the liver in 4/111 (3.6%). CONCLUSIONS: SIADH was the most frequent cause of hyponatraemia in patients receiving TPN. The second most frequent cause was physiological stimuli of AVP secretion induced by pain/nausea.

Abstract

When organized into functional categories, the set of texts found in the Ugaritic archive in the “Maison du prêtre-magicien” manifests a clear raison d’être as a collection. Of the twelve main categories of texts from this deposit, the most significant serve to justify various forms of magical praxis and detail how to carry them out. Almost the entire collection of Ugaritic offering texts is found here, since all include a purification rite of the king, the responsibility of the magician priest as mḥll, whose function is stated in KTU 1.119. This fact had been ignored up to now. In addition, the main texts setting out the royal ideology were entrusted to this magician priest, thus reflecting their magical-cultic nature. Additional texts belong to the realm of scribal training, another responsibility of this public official. The adjacent library with a wide variety of Akkadian texts was of fundamental value in this latter connection.

Abstract

Acute appendicitis remains a common reason for hospital admission. Reports have suggested a reduction in patients attending emergency departments during the acute phase of the SARSCoV- 2 pandemic. Moreover, a global surge in conservative management of acute appendicitis has recently been registered by the Appy Study of the Association of Italian Surgeons in Europe (ACIE)1. This is a treatment option that has been known for some years, although quite seldom used before the pandemic2–4. As most countries are experiencing new waves of the pandemic, the attitude of surgeons towards SARS-CoV-2 screening policies and personal protective equipment (PPE) used during the management of patients with acute appendicitis need to be established. According to a subanalysis of the ACIE Appy Study, half of surgeons globally were testing patients for SARS-CoV-2 only when symptomatic or there was suspicion of infection; approximately 12 per cent did not test patients at all (Fig. 1 and Table S1). There were regional differences. In Europe, respondents tested all patients (50.8 per cent) or those with suspected infection (43.9 per cent), with only 5.3 per cent not being tested at all. In the USA, the majority of participants only tested patients with a suspected infection (65.6 per cent). A similar picture of testing only those with a suspected infection was also reported from Latin America (57.2 per cent), Asia/Middle East (76.8 per cent), and Africa (41.7 per cent). Even more worrisome, 58.3 per cent of respondents from Africa and 27.6 per cent from Latin America were not testing patients at all before appendicectomy. Concerning the screening modality, most respondents used PCR alone or in combination with chest imaging. Serology was rarely used overall and never in Africa (Fig. 1 and Table S2 ). It is now accepted that chest imaging is not routinely required and that PCR is an accurate screening modality. Serology might, however, be useful to shed light on the disease course and previous exposure to the virus, but respondents from some countries still have restricted access to this test. In terms of PPE during appendicectomy, most African respondents did not use different PPE compared with the prepandemic period in patients who tested negative for COVID-19. More concerning is that 58.3 per cent did not use different PPE in untested patients. This differed from other regions where the rate of those not considering a change of PPE in untested patients did not exceed 22 per cent. One in 10 respondents from Latin America also reported that they were not using different PPE compared with the prepandemic phase in patients who tested positive for COVID-19. These data, and taking into account the high prevalence of acute appendicitis, leads to the conclusion that omission of routine patient screening may have contributed to local clusters among patients and threatened the safety of healthcare workers5. In this respect, it is likely that limited access to PPE explains the attitude of surgeons towards patients with unknown SARSCoV- 2 status or those infected, raising ethical concerns about the safety of surgical staff. It is of outmost importance that, even during challenging times and stress on economic stability, industrialized countries make efforts to sustain low-income countries and those with limited resources. This would ensure equal working conditions, safer treatment for patients with acute appendicitis, and better control of the pandemic

Abstract

Background The Frail-VIG frailty index has been developed recently. It is an instrument with a multidimensional approach and a pragmatic purpose that allows rapid and efficient assessment of the degree of frailty in the context of clinical practice. Our aim was to investigate the convergent and discriminative validity of the Frail-VIG frailty index with regard to EQ-5D-3L value. Methods We carried out a cross-sectional study in two Primary Health Care (PHC) centres of the Catalan Institute of Health (Institut Català de la Salut), Barcelona (Spain) from February 2017 to January 2019. Participants in the study were all people included under a home care programme during the study period. No exclusion criteria were applied. We used the EQ-5D-3L to measure Health-Related Quality of Life (HRQoL) and the Frail-VIG index to measure frailty. Trained PHC nurses administered both instruments during face-to-face assessments in a participant’s home during usual care. The relationships between both instruments were examined using Pearson’s correlation coefficient and multiple linear regression analyses. Results Four hundred and twelve participants were included in this study. Frail-VIG score and EQ-5D-3L value were negatively correlated (r = − 0.510; P Conclusions Frail-VIG index demonstrated a convergent validity with the EQ-5D-3L value. Its discriminative validity was optimal, as their scores showed an excellent capacity to differentiate between people with better and worse HRQoL. These findings provide additional pieces of evidence for construct validity of the Frail-VIG index.

Abstract

The start of the pandemic in the middle of the 2019/20 academic year meant a great change and adaptation for the entire educational community. The objective of this publication is to analyze and compare the evolution of the publications with the greatest interaction that Andalusian universities, public and private, have made on the Facebook social network from the 2018/19 academic year to the 2019/20 academic year. A mixed, quantitative and qualitative methodology is used. The objectives and themes of the publications, the target audience and the elements and formats of the post are analyzed. The results show an increasing trend in publications dealing with human support, research and development issues, the purpose of which is to inform and inspire the entire community.

Abstract

Tubular photobioreactors (TPBRs) are closed devices used for the mass culture of microalgae. TPBRs are supposed to be well-mixed, but the influence of their specific fluid dynamics in photosynthesis efficiency has never been studied in detail. Here, we use Computational Fluid Dynamics (CFD) coupled to a dynamic photosynthesis model to analyze the efficiency of the photosynthetic response in the loop of TPBRs of different sizes (14, 24, 44, 64, and 84 mm) and circulation velocities (0.4 to 1 m s−1). The results show that only the smallest diameters cause enough radial mixing for a photosynthesis-enhancing light regime (integration factor Γ = 0.199 for D = 14 mm and v = 1 m s−1) while high circulation velocities in larger diameters (up to 1 m s−1) increase operating costs but do not enhance photosynthetic productivity. It is also shown the relevance of the characteristic frequency of the strain (β), which is crucial for high productivity.

Abstract

Introducing methodologies that promote innovation and continuous improvement in organizations is no longer optional, therefore, organizations are increasingly using methodologies based on Lean principles. Among them, the 6S tool stands out from the rest, commonly used to establish and maintain a high-quality environment, which it has capacity for due to its status as a kaizen process. Thus, this research seeks to evaluate the relationship between Corporate Social Responsibility and the Lean 6S tool and, in the end, create synergies between them in order to enhance the 6S tool’s capabilities. To achieve this, a literature review and analysis of Lean 6S and CSR were performed, and a survey was also proposed to further the understanding of the relationship. With the analyzed sample, it can be confirmed that a relationship exists between the level of implementation of Lean tools and the level of development of CSR policies, therefore, companies that have implemented Lean tools such as 6S are organizations concerned with sustainability, and the hypothesis that organizations that apply Lean also have a high implementation of CSR is validated. Future work should further develop this relationship so that sustainability is no longer considered as implicit in the application of Lean tools but rather as part of them. This research proposes to develop the 6S tool toward the 7S tool to facilitate the inclusion of a CSR policy in a procedural and simple way.

Abstract

OBJECTIVE The aim of the current study was to determine the clinical characteristics of migraine with aura (MA) as well as the frequency and patterns of perfusion-computed tomography (PCT) alterations, in a series of patients with MA mimicking acute ischemic stroke. BACKGROUND MA is one of the most frequent stroke mimics, following seizures and psychiatric disorders. Previous case reports and short series have reported abnormal PCT patterns in patients with MA. METHODS We conducted a retrospective cohort study including all consecutive patients presenting with focal neurological symptoms during complete multimodal CT including baseline CT, angio-CT, and PCT with a final diagnosis of MA. We collected demographic data and clinical information about MA variables using the hospital electronic database. RESULTS We found 25 patients with a final diagnosis of MA among 1761 patients who attended our stroke center with complete multimodal CT (1.4% [95% CI: 0.9-2.1]). Among them, 14/25 (56%) were women, average age 38.7 years (SD 12.5), and 16/25 (64%) had a previous history of migraine. The most frequent type of aura was sensory. The median time elapsed between the onset of symptoms and CT was 171 min (IQR: 119-244). PCT alteration was found in 3/25 (12%) consisting of a hypoperfusion pattern not restricted to a vascular territory. The three patients had aphasia as the presenting symptom. CONCLUSION This is, to the best of our knowledge, the largest series of patients with MA managed as presumed stroke with clinical characteristics and PCT. In our study, most patients were young and had a prior history of migraine. PCT was normal in 88% of cases, with patients being still symptomatic by the time they were scanned. Further research will clarify the presence and type of PCT alterations in this entity.

ObjectiveTo determine whether the Perx app improves medication adherence and clinical outcomes over 12 months compared with standard care in patients requiring polypharmacy.DesignRandomised controlled trial with 12-month follow-up.SettingOutpatient clinics in three tertiary hospitals in Sydney, Australia.ParticipantsEligible participants were aged 18–75 years, with at least one chronic condition, taking ≥3 different medications (oral medications or injections), with smartphone accessibility. Participants were randomised in a 1:1 ratio.InterventionsThe intervention group used the Perx app that contained customised reminders and gamified interactions to reward verified medication adherence.Main outcome measuresThe primary outcome was medication adherence over 12 months measured using pill counts. Secondary outcomes included clinical outcomes (haemoglobin A1c (HbA1c), cholesterol, blood glucose, triglycerides, creatinine, thyroid function, blood pressure and weight).ResultsOf 1412 participants screened for eligibility, 124 participants were randomised; 45 in the Perx arm and 40 in the control arm completed the study. The average age was 59.5, 58.9% were women, chronic conditions were cardiovascular disease (78%), type 2 diabetes (75%), obesity (65%) or other endocrine disorders (18%). On average, participants were taking six medications daily. The Perx group had greater improvements in adherence at month 2 (Coef. 8%; 95% CI 0.01 to 0.15), month 3 (Coef. 7%; 95% CI 0.00 to 0.14) and month 12 (Coef. 7%; 95% CI 0.00 to 0.13). The probability of HbA1c ≤6.5% was greater in the Perx group at months 9 and 12 and cholesterol (total and low-density lipoprotein cholesterol) was lower in the Perx group at month 3. The intervention was particularly effective for those with obesity, taking medications for diabetes and taking ≤4 medications.ConclusionsThis study provides evidence that app-based behavioural change interventions can increase medication adherence and produce longer-term improvements in some clinical outcomes in adults managing multimorbidity. More trials are needed to build the evidence base.Trial registration numberACTRN12617001285347.

Abstract

Induction machines have been key components in the industrial sector for decades, owing to different characteristics such as their simplicity, robustness, high energy efficiency and reliability. However, due to the stress and harsh working conditions they are subjected to in many applications, they are prone to suffering different breakdowns. Among the most common failure modes, bearing failures and stator winding failures can be found. To a lesser extent, High Resistance Connections (HRC) have also been investigated. Motor power connection failure mechanisms may be due to human errors while assembling the different parts of the system. Moreover, they are not only limited to HRC, there may also be cases of opposite wiring connections or open-phase faults in motor power terminals. Because of that, companies in industry are interested in diagnosing these failure modes in order to overcome human errors. This article presents a machine learning (ML) based fault diagnosis strategy to help maintenance assistants on identifying faults in the power connections of induction machines. Specifically, a strategy for failure modes such as high resistance connections, single phasing faults and opposite wiring connections has been designed. In this case, as field data under the aforementioned faulty events are scarce in industry, a simulation-driven ML-based fault diagnosis strategy has been implemented. Hence, training data for the ML algorithm has been generated via Software-in-the-Loop simulations, to train the machine learning models.

Abstract

Background: The relationship between the quality of the diet and the adherence to the Mediterranean diet with the presence of persistent or recurrent depressive symptoms have been described. The objective of this study is to analyze the relationship between adherence to the Mediterranean diet and the intake of specific foods in primary care patients aged 45 to 75, having subclinical or major depression. The study also specifically analyzes this relationship in individuals suffering from chronic diseases. Methods: A cross-sectional descriptive study was conducted. 3062 subjects met the inclusion criteria from the EIRA study. Sociodemographic variables, clinical morbidity, depression symptomatology (PHQ-9) and adherence to Mediterranean diet (MEDAS) were collected. Results: Being female, younger, with a higher BMI, consuming more than 1 serving of red meat a day and drinking more than one carbonated or sugary drink daily, not consuming 3 servings of nuts a week and not eating 2 vegetables cooked in olive oil a week are predictors of having higher depressive symptomatology. Conclusions: Assessing the type of diet of patients presenting depressive symptoms and promoting adherence to a healthy diet is important, especially in patients with chronic diseases. However, depression is a very complex issue and the relationship between nutrition and depression must be further examined.

Abstract

Intermittent water supplies (IWS) are routinely experienced by drinking water distribution systems around the world, either due to ongoing operational practices or due to one off interruptions. During IWS events changing conditions may impact the endemic biofilms leading to hydraulic mobilisation of organic and inorganic materials attached to pipes walls with a resulting degradation in water quality. To study the impact of IWS on the microbiological and physico-chemical characteristics of drinking water, an experimental full-scale chlorinated pipe facility was operated over 60 days under realistic hydraulic conditions to allow for biofilm growth and to investigate flow resumption behaviour post-IWS events of 6, 48 and 144 hours.\ud \ud \ud \ud Turbidity and metal concentrations showed significant responses to flow restarting, indicating biofilm changes, with events greater than 6 hours generating more turbidity responses and hence discolouration risk. The increase in pressure when the system was restarted showed a substantial increase in total cell counts, while the subsequent increases in flow led to elevated turbidity and metals concentrations. SUVA254 monitoring indicated that shorter times of non-water supply increased the risk of aromatic organic compounds and hence risk of disinfection-by-products formation. DNA sequencing indicated that increasing IWS times resulted in increased relative abundance of potential pathogenic microorganisms, such as Mycobacterium, Sphingomonas, and the fungi Penicillium and Cladosporium.\ud \ud \ud \ud Overall findings indicate that shorter IWS result in a higher proportion of aromatic organic compounds, which can potentially react with chlorine and increase risk of disinfection-by-products formation. However, by minimising IWS times, biofilm-associated impacts can be reduced, yet these are complex ecosystems and much remains to be understood about how microbial interactions can be managed to best ensure continued water safe supply.

Abstract

Currently, international organizations involved in education assign to universities the social responsibility of designing interdisciplinary approaches to the study of major global challenges. These global challenges, including sustainability, educational inclusion and development cooperation, present an opportunity for seeking synergies that will make the study of these challenges a reality in the classrooms of higher education centers. The objective of this work is to investigate the opportunities and barriers universities face for undertaking such missions. The methodology consists, in the first place, of constructing a theoretical framework that allows for establishing relationships within education for studying sustainability, inclusion, poverty and mobility. The second part deals with the technique of service learning and environmentalization of the university curriculum. We address the latter at a conceptual level as well as from perceptions of university teachers based on a qualitative study done by the Faculty of Education of the University of Malaga. The conclusions reveal the relationship between an education that ensures social and environmental justice, in addition to the difficulties of working towards sustainability due to the lack of training of university teaching staff.

Abstract

Intermittent and excessive ethanol consumption over very short periods of time, known as binge drinking, is common in the adolescence, considered a vulnerable period to the effects of alcohol in terms of cognitive performance. One of the brain functions most drastically affected by ethanol in adolescent individuals seems to be spatial learning and memory dependent on the hippocampus. In the current study we have focused on the long-lasting effects on spatial learning and memory of intermittent and excessive alcohol consumption compared to chronic and moderate alcohol exposure during adolescence. Five-week old male Wistar rats consumed ethanol for 24 days following two different self-administration protocols that differed in the intake pattern. Spatial learning and memory were evaluated in the radial arm maze. Hippocampal synaptic plasticity was assessed by measuring field excitatory postsynaptic potentials. Hippocampal expression of AMPA and NMDA receptor subunits as well as levels of phosphorylated Ser9-GSK3β (the inactive form of GSK3β) were also quantified. Our results show that both patterns of ethanol intake during adolescence impair spatial learning, memory and cognitive flexibility in the adulthood in a dose-dependent way. Nevertheless, changes in synaptic plasticity, gene expression and levels of inactive GSK3β depended on the pattern of ethanol intake.

Abstract

Agricultural biogas plants are becoming attractive in several countries, mainly due to the possibility of obtaining additional incomes by selling energy to electrical grid. However, specific conditions under which biogas plants would be economically viable are not yet well known. This paper presents results of analysis made to find optimal sizes of biogas plants in terms of electrical capacity. They were evaluated based on economic models of centralized and decentralized technological schemes and their respective mono-digestion and co-digestion systems of cattle manure, maize silage and grass silage as feedstock, for electricity generation capacities between 100 and 1000 kWe. Results show that biogas plants using mono-substrates such as cattle manure present economic viability for electrical power higher than 740 kWe. Co-digestion system presents economic viability for electrical power higher 1000 kWe. Finally, public policies related to development of these technologies, mainly in the form of subsidies, such as those existing in developed countries, could help to make the co-digestion agricultural biogas plants economically viable for the Brazilian scenario. The novelty of the paper consists in the determination of the optimum size of mono- and co-digestion patterns for Brazilian conditions showing as results the necessary level of subsidies and the paths to economic viability.

Abstract

Background The use of the treadmill as a gait rehabilitation tool has provided novel options for treatment of gait impairments in Parkinson's Disease (PD). However, the neural mechanisms underlying these therapeutic effects in PD remain unknown and whether any therapeutic effects from treadmill training can be reproduced on overground walking. Objective To examine the immediate short-term effects of a single session of treadmill and overground walking on gait, spinal and corticospinal parameters in PD.METHODSPD participants (N = 15) were evaluated in two separate sessions under two walking conditions: walking over a treadmill and walking overground. Overground walking performance, the Soleus H-reflex, Reciprocal Ia-Inhibition, Intracortical Facilitation (ICF) and Short Intracortical Inhibition (SICI), were evaluated before and after each condition. Results Gait speed and stride length improved in post-treadmill compared with pre-treadmill. No significant changes in these gait parameters were found for the pre vs. post-overground condition. ICF values and Hmax/Mmax ratio decreased after, compared with before, the two walking conditions. Conclusions Treadmill walking, but not overground walking, lead to an improvement in the stride length and gait speed in the PD patients without evidence of different modulation on spinal and corticospinal parameters.

Abstract

Resumen Objetivo Evaluar el nivel de implementacion de los protocolos asociados a la prevencion de la debilidad muscular adquirida en la unidad de cuidados intensivos (UCI), asi como la presencia del fisioterapeuta en distintas UCI de Espana. Metodo Estudio descriptivo, transversal realizado en 86 UCI de adultos de Espana entre los meses de marzo a junio de 2017. Se excluyeron UCI neuroquirurgicas y de grandes quemados. Se utilizo encuesta multirrespuesta que incluia preguntas sobre los protocolos de: control de glucemia, sedacion, valoracion del dolor, prevencion del delirium, manejo del delirium y movilizacion precoz. La encuesta fue rellenada a traves de un aplicativo protegido por usuario y contrasena. Analisis inferencial con t de Student o U de Mann-Whitney y de correlacion con Pearson o Rho de Spearman. Resultados El 89,5% de las UCI tenian protocolo de control de glucemia, con rango predominante de 110-140 mg/dl. El 74,4% evaluaban el nivel de sedacion, si bien solo tenian protocolos de sedacion el 36% de ellas. Con relacion a la valoracion del dolor se realizaba en el 73,7% de los pacientes comunicativos, mientras que en los no comunicativos solo era del 47,5%. Solo el 37,2% realizaban screening diario para detectar el delirium, y disponian de protocolos de prevencion del delirium el 31,4% de las UCI, del manejo del delirium el 26,7% y de movilizacion precoz el 14% de las UCI. En el 34,9% de los casos se solicita interconsulta al servicio de rehabilitacion. Conclusiones La implementacion de los diferentes protocolos asociados a la prevencion de la debilidad muscular adquirida ha sido elevada en relacion con los protocolos de control de glucemia, valoracion del nivel de sedacion y del dolor de pacientes comunicativos, mientras que baja en los de movilizacion precoz y screening y prevencion del delirio. Asimismo, es poco frecuente la presencia del fisioterapeuta en la UCI.

Abstract

Purpose In recent years, a process of reform and innovation in higher education has been witnessed. A change in the evaluation of student learning in universities is necessary for new teaching-learning proposals to be developed. The authors propose implementing a learning assessment process based on the idea of participatory evaluation, verifying the benefits of this method in the acquisition of cross-disciplinary skills. Design/methodology/approach The method implemented follows the principles of action research. Findings The Kahoot! app has an effect on the teaching-learning process and on the training skills and academic performance measured through the student’s grades. Originality/value This paper presents an innovation proposal that aims to observe how students acquire more competences.
