The aim of the study was to evaluate air-displacement plethysmography (ADP) thoracic gas volume (TGV) prediction equations in children representing a wide range of body mass index (BMI).254 children (5 to 17 years) were recruited from two centres (England and the United States). Subjects were stratified into three BMI categories according to the International Obesity TaskForce (IOTF) criteria: lean (48 male, 42 female), overweight (15 male, 29 female), and obese (52 male, 68 female). As part of the normal ADP procedure, TGV was measured (TGV(Meas)), predicted using child specific equations developed by Fields (TGV(Fields)) and adult derived equations by Crapo (TGV(Crapo)) with percentage body fat (PF) estimates subsequently calculated (PF(Meas), PF(Fields), PF(Crapo)).Compared with the mean TGV(Meas), the TGV(Fields) estimates were within+/-0.2 L in all groups, except obese males (+0.5 L), while the mean TGV(Crapo) estimates were greater than+/-0.3 L in all groups except lean males (+0.1 L). When converted to PF, the mean PF(Fields) estimates were within+/-1% of the measured value in all groups, except obese males (+1.1%), while the mean PF(Crapo) estimates were greater than+/-1% in all groups, except lean males (+0.5%). Using either prediction equation, Bland-Altman analysis revealed that the greatest PF+/-95% limits of agreement were in the lean and overweight groups and lowest in the obese groups.The Fields child-specific TGV prediction equations provide accurate mean PF estimates and appear better than using the Crapo equations if a measured TGV cannot be achieved in children of a wide range of BMI. However, individual predictions may result in large PF errors especially in lean children.

Infant formulas provide more protein than breast milk. High protein intakes may place infants at risk of later obesity. The present study tested whether a formula with protein content below the regulatory level supports normal growth from age 3 months.Randomized double-blind trial enrolled healthy infants less than age 3 months. At 3 months, formula-fed infants were assigned to experimental (EXPL, 1.61 g protein/100 kcal; modified bovine whey proteins with caseinoglycomacropeptide removed) or control (CTRL 2.15 g protein/100 kcal; unmodified bovine milk protein with a whey/casein ratio of 60/40) formula; breast-fed (BF) infants were enrolled in a reference group. Complementary foods were allowed in small amounts from 4 to 6 months and unrestricted after 6 months.Weight gain (g/day) from 3 to 6 months was similar in the EXPL and CTRL groups (EXPL-CTRL -0.84 g/day; 95% confidence interval -2.25 to 0.57) and faster in the EXPL and CTRL groups than in the BF group. Weight analyzed longitudinally from 4 to 12 months was lower in the EXPL group than in the CTRL group (P = 0.031) but higher than in the BF group (P 0.0001). Longitudinal analysis of odds ratios from 4 to 12 months indicated fewer infants with weight85th percentile in the EXPL group than in the CTRL group (P = 0.015). Length z scores were lower than, and body mass index z scores were similar to, World Health Organization Standards in all of the groups. Serum biochemical parameters in the EXPL group reflected lower protein intake and were closer to parameters in the BF infants than in the CTRL group.A formula with 1.61 g of protein/100 kcal supports normal growth of infants after age 3 months. This protein content is adequate if provided from a high-quality source.

BACKGROUND: Offspring of women with diabetes mellitus (DM) during pregnancy have a risk of developing metabolic disease in adulthood greater than that conferred by genetics alone. The mechanisms responsible are unknown, but likely involve fetal exposure to the in utero milieu, including glucose and circulating adipokines. The purpose of this study was to assess the impact of maternal DM on fetal adipokines and anthropometry in infants of Hispanic and Native American women.METHODS: We conducted a prospective study of offspring of mothers with normoglycemia (Con-O; n = 79) or type 2 or gestational DM (DM-O; n = 45) pregnancies. Infant anthropometrics were measured at birth and 1-month of age. Cord leptin, high-molecular-weight adiponectin (HMWA), pigment epithelium-derived factor (PEDF) and C-peptide were measured by ELISA. Differences between groups were assessed using the Generalized Linear Model framework. Correlations were calculated as standardized regression coefficients and adjusted for significant covariates.RESULTS: DM-O were heavier at birth than Con-O (3.7 ± 0.6 vs. 3.4 ± 0.4 kg, p = 0.024), but sum of skinfolds (SSF) were not different. At 1-month, there was no difference in weight, SSF or % body fat or postnatal growth between groups. Leptin was higher in DM-O (20.1 ± 14.9 vs. 9.5 ± 9.9 ng/ml in Con-O, p CONCLUSIONS: Maternal DM results in fetal hyperleptinemia independent of adiposity. HMWA appears to influence postnatal growth. Thus, in utero exposure to DM imparts hormonal differences on infants even without aberrant growth.

Advances in body composition measurement, evaluation, and analysis have contributed vastly to our knowledge of human biology. These advances can be organized into three distinct, interconnected areas: body composition paradigms, body composition methodology, and body composition response to external influences. The first research area describes the architecture of human body composition identifying proportions of various compartments and their steady-state associations among the atomic, molecular, cellular, tissue system, and whole-body levels; the second research area investigates the merits of different body composition measurement methods for in vivo; and the third investigates the response of body composition to factors such as growth and aging. The five-level model proposed by Z. Wang et al. in 1992 was an important advancement towards building an appropriate structure for body composition research by organizing components into five distinct levels of increasing complexity: atomic, molecular, cellular, tissue system, and whole-body. The eight articles that appear in this special issue are categorized with some overlap into these three main research areas. The review paper proposed by A. M. Silva et al. provides the relationship between body composition architecture and methodology research. The authors discuss the assumptions of hydrometric and densitometric two-component models, namely, 73.2% and 1.1 g/cc for the fat-free mass (FFM) hydration and density, correspondingly. These averages do not include interindividual variability in FFM, particularly in the pediatric population. The review highlights the need for multicomponent models for more accurate body composition assessment in children. To meet this need, the authors reviewed skinfolds and bioelectrical impedance-based predictive equations developed using multicomponent models for use as a reference method. In general, development of methodology, methodology evaluation, and methodology validation was the main focus of one review and three original papers. These articles develop mathematical formulae derived from statistical analysis of experimental observations. At the whole-body level of analysis, A. F. Casey reviewed assessment validity and reliability in individuals with intellectual disability. D. Machado et al. proposed new anthropometric-based equations to assess bone mineral, lean soft tissue, and fat mass for a male pediatric population. E. Forsum et al. analyzed the relationship between body mass index and body fat assessed by air displacement plethysmography in 4-year-old children. At the tissue level of analysis, W. Shen et al. compared bone marrow fat measurements among T1-weighed magnetic resonance imaging (MRI) and magnetic resonance spectroscopy. E. L. Rolfe et al. analyzed the validity of ultrasound visceral and subcutaneous abdominal depth as a proxy for MRI measured internal abdominal and subcutaneous fat in infants. Finally, in the last research category, two original papers investigated body composition response to aging and ethnicity. M. F. Almeida et al. examined longitudinal anthropometric changes in Brazilian older adults from 2000 to 2006. C. L. Carpenter et al. evaluated the efficacy of the body mass index (BMI) as a proxy for adiposity across different ethnic groups by comparing to measured percent body fat. The research field investigating body composition responses also includes responses to growth, development, nutrition, exercise, hormonal changes, and medication. In this regard, the study of the dynamic relations between body components and its associate's functions has been recently explored. A large body of research evaluating body composition responses to these factors at both the experimental and theoretical levels is now available. In fact, the concept of functional body composition has been proposed, providing more sophisticated view of nutritional status, metabolism, endocrinology, and diseases. More than just the assessment of the compartments in any of these five levels, functional body composition addresses the quantitative and biological interactions of these compartments with energy balance status, metabolic features, healthy and unhealthy biomarkers, sports performance markers, and many other measurable physiological expressions of a living organism. In conclusion, the original research and reviews contained in this special issue identify gaps and provide potential alternatives to more accurate routine body composition assessment in basic science and clinical settings. From a clinician's perspective, these papers mostly tell us about body composition models, techniques, and assessment in health across the age span, with the underlying assumption of steady state. However, these papers do not address how robust these techniques are in a nonsteady state (i.e. clinical illness). This is important in an obesodiabetogenic environment that appears to hit all corners of the world. This aspect is also intimately linked to the functional aspects of body composition where a key question is the nature and the extent of pathophysiological influences on body composition changes, on the one hand, and the effect of those body composition changes on pathophysiology on the other hand. What these papers do is to help in setting a modern scene for interpreting clinical findings, though more research is needed to capture the longitudinal body composition response and related energy regulation during clinical illness, growth, and aging, as well as the effect of diet and exercise interventions. Differences in body composition are associated with increased disease risk in various stages of life. Despite numerous available methods in assessing body composition (air displacement plethysmography, dual-energy X-ray absorptiometry, bioelectrical impedance, hydrometry, and magnetic resonance imaging), due to innate technical limitations, the ability for one singular method to track body composition over the life span (ie, infancy to adulthood) is challenging and imperfect. The primary goal of this review is to determine if there are body composition methods that can accurately track body composition from infancy into adulthood. After careful consideration and taking into account the best available scientific evidence, we feel air displacement plethysmography is the best instrument at this time for tracking body composition, starting in infancy and forward into adulthood, partly because it is the only "practical" clinical tool currently available for use during infancy.

To determine whether exposure to diabetes in utero affects resting energy expenditure (REE) and fuel oxidation in infants.At 35 ± 5 days after birth, body composition and REE were measured in full-term offspring of Native American and Hispanic women with either well-controlled diabetes (13 girls, 11 boys) or normal healthy pregnancies (18 girls, 17 boys).Control of dysglycemia during gestation in the women with diabetes mellitus met current clinical standards, shown by average glycated hemoglobin (5.9 ± 0.2%; 40.6 ± 2.3 mmol/mol). Infant body mass (offspring of women with diabetes: 4.78 ± 0.13, control offspring: 4.56 ± 0.08 kg) and body fatness (offspring of women with diabetes: 25.2 ± 0.6, control offspring: 24.2 ± 0.5 %) did not differ between groups. REE, adjusted for lean body mass, was 14% lower in offspring of women with diabetes (41.7 ± 2.3 kJ/h) than control offspring (48.6 ± 2.0, P = .025). Fat oxidation was 26% lower in offspring of women with diabetes (0.54 ± 0.05 g/h) than control offspring (0.76 ± 0.04, P.01) but carbohydrate oxidation did not differ. Thus, fat oxidation accounted for a lower fraction of REE in the offspring of women with diabetes (49 ± 4%) than control offspring (60 ± 3%, P = .022). Mothers with diabetes were older and had higher prepregnancy body mass index than control mothers.Well-controlled maternal diabetes did not significantly affect body mass or composition of offspring at 1-month old. However, infants with mothers with diabetes had reduced REE and fat oxidation, which could contribute to adiposity and future disease risk. Further studies are needed to assess the impact differences in age and higher prepregnancy body mass index.

Objective: The objective of this study was to investigate changes in body weight, BMI, body composition, and fat distribution among freshman women during their 1st year of college. Research Methods and Procedures: Freshman women during the 2004 to 2005 academic year were recruited to participate. The initial baseline visit occurred within the first 6 weeks of the fall 2004 semester, with the follow-up visit occurring during the last 6 weeks of the spring 2005 semester. At each visit, height, weight, BMI, waist and hip circumferences, and body composition (by DXA) were obtained. Results: One hundred thirty-seven participants completed both the fall and spring visits. Significant (p < 0.0001) increases between the fall and spring visits were observed for body weight (58.6 vs. 59.6 kg), BMI (21.9 vs. 22.3), percentage body fat (28.9 vs. 29.7), total fat mass (16.9 vs. 17.7 kg), fat-free mass (38.1 vs. 38.4 kg), waist circumference (69.4 vs. 70.3 cm), and hip circumference (97.4 vs. 98.6 cm), with no significant difference observed in the waist-to-hip ratio (0.71 vs. 0.71; p = 0.78). Discussion: Although statistically significant, changes in body weight, body composition, and fat mass were modest for women during their freshman year of college. These results do not support the purported freshman 15 weight gain publicized in the popular media.

Objective: To determine whether activity counts obtained with the Actiwatch monitor are associated with total expenditure and body composition in young children. Research Methods and Procedures: Actiwatch activity monitors were tested in 29 children 4 to 6 years old under field conditions over eight days. Total energy expenditure (TEE) was assessed with the doubly labeled water (DLW) technique. Correlation analyses were used to identify variables related to energy expenditure and percentage body fat. Multiple linear regression analyses were used to examine the variance in TEE and percentage body fat explained by activity counts after adjusting for relevant covariates. Results: Both average total daily activity counts (658, 816 ± 201, 657) and the pattern of activity were highly variable among subjects. TEE was significantly related to lean body mass (r = 0.45) and age (r = 0.48; p < 0.05 for both). Activity counts alone were not associated with TEE. In multiple linear regression analyses, TEE was independently associated with only lean body mass. Percentage fat mass was independently associated with body weight, being a girl, and being white, but not with average total activity counts. Discussion: Activity counts obtained with the Actiwatch under free-living conditions do not reflect TEE in 4- to 6-year-old children and are not correlated with percentage fat mass. Therefore, average total activity counts obtained with the Actiwatch may be of limited value in identifying children at risk for becoming obese.

Human breast milk (HBM) contains numerous bioactive components, recently shown to be associated with growth and body composition in breastfed offspring. Reciprocity in adipogenic and osteogenic pathways suggests bone mass may also be influenced by these components. The association between bioactive components found in HBM and bone mineral content (BMC), to our knowledge, is unknown. The purpose of this proof-of-principle study was to evaluate the association between specific bioactive components in HBM in exclusively breastfed infants and skeletal health in the first 6 months of life and examine potential gender differences in these associations. Thirty-five mother-infant dyads were followed from 1 to 6 months. The contents of a single breast expression were used for analyses of bioactive components (insulin, glucose, leptin, interleukin-6 and tumor necrosis factor-α (TNFα), whereas BMC was evaluated by dual-energy X-ray absorptiometry. In the total sample, there was a positive association between TNFα and BMC at 1 (P=0.004) and 6 months (P=0.007). When stratified by sex, females exhibited a positive association between BMC and glucose and an inverse relationship between BMC and TNF-α at 1 month with TNF-α strengthening (P=0.006) at 6 months. In males, at 6 months a positive relationship between BMC and HBM glucose and an inverse relationship with HBM leptin were observed with no associations observed at 1 month. Although preliminary, the associations between bioactive components in HBM highlight the importance HBM has on bone accretion. It is critically important to identify factors in HBM that contribute to optimal bone health.

Summary Purpose Bioelectrical impedance analysis (BIA) equations can predict total body water (TBW) and extracellular water (ECW) in non-athletic healthy populations. This study aimed: a) to develop BIA-based models for TBW and ECW prediction based on dilution methods in a sample of national level athletes; and b) to validate the new models with a cross-validation approach in a separate cohort using dilution methods as criterion. Methods Two hundred and eight highly trained athletes (21.3 ± 5.0 years) were evaluated during their respective competitive seasons. Athletes were randomly split into development (n = 139) and validation groups (n = 69). The criterion method for TBW was deuterium dilution and for ECW was bromide dilution, where ICW was the respective difference between both. Resistance (R) and reactance (Xc) were obtained with a phase-sensitive 50 kHz BIA device and used for the estimation of TBW and ECW. Results Athletic BIA-based models were developed for TBW and ECW [TBW = 0.286 + 0.195*S 2 /R + 0.385*Wt + 5.086*Sex; ECW = 1.579 + 0.055*S 2 /R + 0.127*Wt + 0.006*S 2 /Xc + 0.932*Sex, where sex is 0 if female or 1 if male, Wt is weight (kg), S is stature (cm), and R and Xc are in ohm (Ω)]. Cross validation revealed R 2 of 0.91 for TBW and R 2 0.70 for ECW and no mean bias. Conclusions The new equations can be considered valid, with no observed bias, thus affording practical means to quantify TBW and ECW in national level athletes.

Objective: The objective of this study was to determine the effect of body hair (scalp and facial) on air displacement plethysmography (BOD POD) estimates of percentage of body fat. Research Methods and Procedures: A total of 25 men (31.4 ± 8.0 years, 83.4 ± 12.2 kg, 181.8 ± 6.9 cm) agreed to grow a beard for 3 weeks to participate in the study. Total body density (g/cm3) and percentage of body fat were evaluated by BOD POD. To observe the effect of trapped isothermal air in body hair, BOD POD measures were performed in four conditions: criterion method (the beard was shaven and a swimcap was worn), facial hair and swimcap, facial hair and no swimcap, and no facial hair and no swimcap. Results: The presence of only a beard (facial hair and swimcap) resulted in a significant underestimation of percentage of body fat (16.2%, 1.0618 g/cm3) vs. the criterion method (17.1%, 1.0597 g/cm3, p < 0.001). The effect of scalp hair (no swim cap worn) resulted in a significant underestimation in percentage of body fat relative to the criterion method, either with facial hair (facial hair and no swimcap; 14.8%, 1.0649 g/cm3) or without facial hair (no facial hair and no swimcap; 14.8%, 1.0650 g/cm3, p < 0.001 for both). Discussion: A significant underestimation of percentage of body fat was observed with the presence of facial hair (∼1%) and scalp hair (∼2.3%). This underestimation in percentage of body fat may be caused by the effect of trapped isothermal air in body hair on body-volume estimates. Thus, excess facial hair should be kept to a minimum and a swimcap should be worn at all times to ensure accurate estimates of body fat when using the BOD POD.

Large for gestational age infants are at increased risk of childhood obesity and maternal nutrition impacts birthweight. The aim of this study was to assess the impact of a maternal low glycaemic index (GI) diet on neonatal anthropometry.The ROLO (Randomised Control Trial of Low Glycaemic Index Diet in Pregnancy) study was a randomized controlled trial to assess the impact of a low GI diet versus usual diet on birthweight. It took place in a tertiary maternity hospital in Ireland and women were randomized to receive either a low GI diet or no dietary intervention from early pregnancy to term. Two hundred sixty-five neonates had anthropometric measurements on day 1-2 of life, 126 in the intervention group and 139 in the control group. Measurements included birthweight, length and circumferences of the head, chest, abdominal, thigh and mid-upper arm. A subgroup of 219 (82%) neonates also had skin-fold measurements.Neonates whose mothers had a low GI diet in pregnancy had lower thigh circumference (15.9 ± 1.7 cm vs. 16.6 ± 1.5 cm, P = 0.04). There was no difference between the intervention and control groups in head, chest abdominal or mid-upper arm circumferences. No difference was noted between the two groups for any skin-fold measurements (subscapular, thigh, biceps and triceps).Neonatal thigh circumference was altered by maternal low GI diet in pregnancy. These findings suggest that maternal low GI diet is safe in pregnancy and may positively impact infant adiposity.

This study compared the growth of healthy infants fed a hypoallergenic 100% whey-based extensively hydrolyzed formula (EHF) with Bifidobacterium lactis (test) with that of infants fed an extensively hydrolyzed casein formula (control). Formula-fed infants (14 ± 3 days) were randomized to test or control groups until 112 days of age. Anthropometrics were assessed at 14, 28, 56, 84, and 112 days, and daily records were kept for 2 days prior to study visits. Serum albumin and plasma amino acids at 84 days were assessed in a subset. A total of 282 infants were randomized (124 test, 158 control). Significantly more infants dropped out of the control (56%) as compared with the test (41%) group. Mean daily weight gain was significantly higher in the test group compared with the control group (27.95 ± 5.91 vs 25.93 ± 6.12 g/d; P = .027) with the test group reporting significantly fewer stools (2.2 vs 3.6 stools/d; P < .0001). The control group reported significantly more days with >3 loose stools/d and a higher incidence of vomiting as compared with the test group. There were no differences in gas, mood, sleep, or serum albumin. Plasma arginine and valine were significantly lower in the test group, whereas leucine and lysine were higher; all values were within normal limits. Significantly more adverse events attributed to the study formula were reported in the control group. The 100% whey-based hypoallergenic EHF containing Bifidobacterium lactis and medium chain triglycerides supported growth of healthy infants. Future studies on the application of this formula in clinically indicated populations are warranted.

Body composition assessment during infancy is important because it is a critical period for obesity risk development, thus valid tools are needed to accurately, precisely, and quickly determine both fat and fat-free mass. The purpose of this study was to compare body composition estimates using dual-energy x-ray absorptiometry (DXA) and air displacement plethysmography (ADP) at 6 months old. We assessed the agreement between whole body composition using DXA and ADP in 84 full-term average-for-gestational-age boys and girls using DXA (Lunar iDXA v11-30.062; Infant whole body analysis enCore 2007 software, GE, Fairfield, CT) and ADP (Infant Body Composition System v3.1.0, COSMED USA, Concord, CA). Although the correlations between DXA and ADP for %fat (r = 0.925), absolute fat mass (r = 0.969), and absolute fat-free mass (r = 0.945) were all significant, body composition estimates by DXA were greater for both %fat (31.1 ± 3.6% vs. 26.7 ± 4.7%; P < 0.001) and absolute fat mass (2,284 ± 449 vs. 1,921 ± 492 g; P < 0.001), and lower for fat-free mass (5,022 ± 532 vs. 5,188 ± 508 g; P < 0.001) vs. ADP. Inter-method differences in %fat decreased with increasing adiposity and differences in fat-free mass decreased with increasing infant age. Estimates of body composition determined by DXA and ADP at 6 months of age were highly correlated, but did differ significantly. Additional work is required to identify the technical basis for these rather large inter-method differences in infant body composition.

Among judo athletes, strong grip strength is crucial for performing offensive and defensive maneuvers that rely predominantly on forearm maximal strength (FMS). The study aims were to evaluate changes in total-body water (TBW) and its compartments (extracellular water [ECW] and intracellular water [ICW]) and their relationship with loss of FMS in elite judo athletes. At baseline (weight stability), 27 male elite athletes were evaluated (age: 23.2 ± 2.8 years) and again evaluated 1-3 days before competition. Athletes were free to gain or lose weight based upon their specific competition needs. Using dilution techniques (deuterium and bromide), TBW and ECW were estimated, and ICW was calculated (ICW = TBW - ECW). Fat, fat-free mass, and appendicular lean soft tissue (LST) were assessed by dual-energy x-ray absorptiometry. Handgrip was used to assess FMS. Using a reduction of 2% as a representative outcome for decreased FMS, 10 athletes were identified as having lost FMS, whereas 17 changed2% or gained. Comparison of means and logistic regression analysis were performed. Results from baseline to before competition indicated that those who lost ≥2% of FMS significantly decreased TBW and ICW by -2.7 ± 3.0 and -4.4 ± 4.2%, respectively. The groups differed in ICW changes (-4.4 ± 4.2 vs. 1.9 ± 6.1%), respectively, for those who lost FMS by ≥2%. The ICW changes, but not in TBW or ECW, significantly predicted the risk of losing FMS (β = 0.206; p = 0.027), even adjusting for weight and arm LST changes. These findings indicated that reductions in ICW increased the risk of losing grip strength in elite judo athletes.

We determined whether arterial compliance measured by pulse wave analysis is impaired in obese pubertal children compared to normal weight controls, and assessed whether arterial compliance is associated with ambulatory activity. Body fat percentage was significantly different between the normal (n = 33) and obese (n = 34) participants ( P < .001). Large ( P = .012) and small ( P < .001) arterial compliance were lower in the normal-weight group. After adjusting for height, systolic and diastolic blood pressure, race, sex, and Tanner stage, large arterial compliance was no longer different between groups ( P = .066), whereas small arterial compliance remained higher in the obese group ( P < .001). Obese pubertal children have paradoxically increased small arterial compliance compared to that of normal weight children, even after adjusting for height, blood pressure, race, sex, and Tanner stage. Thus, obesity in adolescence is not associated with impairments in small arterial compliance.

Reference %fat and total fat-free mass data is necessary for evaluating growth in infants. We aimed to develop longitudinal %fat and total fat-free mass data in infants from birth to 6 months of age. An observational, multicenter, prospective cohort study was conducted with assessments at birth, 1 week, 2 weeks, 1, 2, 3, 4, 5, and 6 months of age. Subjects were exclusively breast-fed and were enrolled at three centers. Whole-body composition (i.e., % fat and total fat-free mass) were assessed using air-displacement plethysmography (ADP) (PEA POD; Life Measurement, Concord, CA). Maternal prepregnancy BMI, gestational weight gain, and infant anthropometric data were collected. A total of 160 infants (boys = 84) were assessed from birth to 4 months of age. Mean birth weight was 3.46 ± 0.39 kg % fat and fat-free mass significantly increased from birth to 4 months of age (P < 0.0001). Gender-specific %fat and total fat-free mass curves for infants from birth to 4 months of age were created. This study will be beneficial to health-care professionals in evaluating normal growth and nutritional patterns in the first months of life.

High performance terahertz (THz) radiation sources hold great promise for a variety of military and space applications. With micro-electro-mechanical systems (MEMS) fabrication techniques, it is possible to attain the smaller, more precisely machined resonant structures required by Vacuum Electronic Devices (VEDs) to function in these frequencies. The research presented here proposes a design and fabrication process for a micro-klystron with a targeted operating frequency of 200 GHz; being developed jointly by Duke University, the University of Strathclyde, UK, and Logos Technologies. It also analyzes the use of a pseudospark (PS) discharge as a novel electron beam source to drive the klystron. Dimensional tolerances are investigated using both analytic and numeric techniques. The incorporation of alignment structures into the fabrication process that utilize kinematic and elastic averaging effects, along with clever stacking techniques, allows submicron alignment tolerances yielding an expected power output of approximately 5W per klystron with an overall efficiency of 20%. The device proposed here, with a volume on the order of 0.01 cc, should be capable of output power densities of up to 1kW/cc. A fabrication run recently completed at MIT's Microsystems Technology Laboratories yielded promising results and 32 silicon die were successfully bonded into a stack 1.4cm tall. Difficulties remain, however, in controlling surface roughness and integrating a klystron with alignment features for parallel processing. Several alternative fabrication schemes have been proposed and another fabrication run based on these modifications is currently underway.

Nutrition in early life, growth, and subsequent health over a lifetime are significantly interrelated. The aim of this study was to assess body composition changes in exclusively breast-fed infants from birth up to 6 mo of age. An observational, prospective, cohort study was conducted. Fifty-nine full-term, healthy, exclusively breast-fed infants underwent assessment of growth and body composition, using air-displacement plethysmograph (i.e. PEA POD) by Life Measurement, Inc. (Concord, CA). Body composition was assessed at birth, 2 wk, and 1, 2, 3, 4, 5, and 6 mo of age. Mean birth weight (g) and gestational age (wk) of the infants were 3170 ± 420 and 39.21 ± 1.29, respectively. Percentage of fat mass increased significantly over the first 4 mo (p < 0.001), both in boys and girls, with no differences detected between boys and girls at any time point. This article provides preliminary in-depth data on whole-body composition, in exclusively breast-fed infants during the first months of life. Further studies assessing larger sample sizes are desirable to develop reference body composition data.

Objective The purpose of this study was to compare bodyweight and composition (percent fat, fat mass, and fat-free mass) in neonates born to mothers with a normal pregravid body mass index (BMI; 2 ) vs neonates born to mothers with an overweight/obese pregravid BMI (≥ 25 kg/m 2 ). Study Design Seventy-two neonates (33 from normal mothers and 39 from overweight/obese mothers) of singleton pregnancies with normal glucose tolerance had their bodyweight and body composition assessed by air-displacement plethysmography. Results After controlling for neonate age at time of testing, significant differences were found between groups for percent fat (12.5 ± 4.2% vs 13.6 ± 4.3%; P ≤ .0001), fat mass (414.1 ± 264.2 vs 448.3 ± 262.2 g; P ≤ .05), and fat-free mass (3310.5 ± 344.6 vs 3162.2 ± 343.4 g; P ≤ .05), with no significant differences between birth length (50.7 ± 2.6 vs 49.6 ± 2.6 cm; P = .08) or birthweight (3433.0 ± 396.3 vs 3368.0 ± 399.6 g; P = .44). Conclusion Neonates born to mothers who have a normal BMI have significantly less total and relative fat and more fat-free mass than neonates born to overweight/obese mothers. Although preliminary, these data suggest that the antecedents of future disease risk (eg, cardiovascular disease, diabetes, and obesity) occur early in life.

Background: ACAT-related enzyme 2 required for viability 1 (ARV1) is a putative lipid transporter of the endoplasmic reticulum that is conserved across eukaryotic species. The ARV1 protein contains a conserved N-terminal cytosolic zinc ribbon motif known as the ARV1 homology domain, followed by multiple transmembrane regions anchoring it in the ER. Deletion of ARV1 in yeast results in defective sterol trafficking, aberrant lipid synthesis, ER stress, membrane disorganization and hypersensitivity to fatty acids (FAs). We sought to investigate the role of Arv1 in mammalian lipid metabolism. Methods: Homologous recombination was used to disrupt the Arv1 gene in mice. Animals were examined for alterations in lipid and lipoprotein levels, body weight, body composition, glucose tolerance and energy expenditure. Results: Global loss of Arv1 significantly decreased total cholesterol and high-density lipoprotein cholesterol levels in the plasma. Arv1 knockout mice exhibited a dramatic lean phenotype, with major reductions in white adipose tissue (WAT) mass and body weight on a chow diet. This loss of WAT is accompanied by improved glucose tolerance, higher adiponectin levels, increased energy expenditure and greater rates of whole-body FA oxidation. Conclusions: This work identifies Arv1 as an important player in mammalian lipid metabolism and whole-body energy homeostasis.

Background With the rapid increase in obesity rates, determining critical periods for weight gain and the effects of changes in fat mass is imperative. The purpose of this study was to examine changes in body weight and composition over the holiday season (Thanksgiving through New Year's) in male and female college students. Methods Subjects completed three visits: the first occurred within 2 weeks prior to Thanksgiving, the second occurred within 5 to 7 days following Thanksgiving, and the third occurred within 10 days following New Year's Day. A total of 82 healthy male and female college age subjects participated. Body composition by dual energy x-ray absorptiometry (DXA) was assessed at visits 1 and 3 while body weight was assessed at all three visits. Results Average body weight remained relatively unchanged from pre-Thanksgiving to post-New Year's (71.3 ± 14 kg vs. 71.2 ± 15 kg; P = 0.71) and, in fact, a subset of normal weight subjects lost a significant amount of body weight. However, percent body fat (25.9 ± 9 %fat vs. 27.0 ± 9 %fat; P < 0.01) and fat mass (18.3 ± 8 kg and 19.1 ± 8 kg; P < 0.01) significantly increased from pre-Thanksgiving to post-New Year's while fat-free mass (48.7 ± 12 kg and 48.3 ± 11 kg; P = 0.08) was not significantly different than the post-New Year's. A significant positive relationship (P < 0.001) between the change in BMI and percent fat, total fat mass, total fat free mass, and trunk fat mass for the pre-Thanksgiving and post-New Year's visits were found. The same significant positive relationships (P < 0.001) were also observed between the change in body weight and percent fat, total fat mass, total fat free mass, and trunk fat mass. Conclusion Despite the fact that body weight remained unchanged over the course of the holiday season, a significant increase in %body fat and fat mass was observed. With recent evidence showing marked morbidity and mortality to be associated with increased body fat (particularly abdominal adiposity), results from this study suggest body weight alone may underestimate the potentially deleterious effects of the holiday season.

Objective To compare the accuracy of air displacement plethysmography (ADP) and dual energy x-ray absorptionmetry (DXA) in tracking changes in body composition after a 16 month weight loss intervention in overweight and obese females. Methods 93 healthy female subjects (38.9 ± 5.7 yr, 159.8 ± 5.6 cm, 76.7 ± 9.9 kg, 30.0 ± 3.4 kg/m2) completed a 16 month weight loss intervention. Eligible subjects attended 15 treatment sessions occurring over the course of 4 months with educational content including topics relating to physical activity and exercise, diet and eating behavior, and behavior modification. In the remaining 12 months, subjects underwent a lifestyle program designed to increase physical activity and improve eating habits. Before and after the intervention, subjects had their percent body fat (%fat), fat mass (FM), and fat-free mass (FFM)) assessed by DXA and ADP. Results Significant differences (p ≤ 0.001) were found between DXA and ADP at baseline %fat (46.0 % fat vs. 42.0 % fat), FM (35.3 kg vs. 32.5 kg) and FFM (40.8 kg vs. 44.2 kg) as well as at post intervention for %fat (42.1% fat vs. 38.3 % fat), FM (30.9 kg vs. 28.4 kg) and FFM (41.7 kg vs. 44.7 kg). At each time point, ADP %fat and total FM was significantly lower (p ≤ 0.001) than DXA while FFM was significantly higher (p ≤ 0.001). However, both techniques tracked %fat changes similarly considering that there were no differences between the two means. Furthermore, a Bland-Altman analysis was performed and no significant bias was observed, thus demonstrating the ability of ADP to measure body fat across a wide range of fatness. Conclusion At baseline and post weight loss, a significant difference was found between ADP and DXA. However, the results indicate both methods are highly related and track changes in %fat similarly after a weight loss program in overweight and obese females. Additionally, the mean changes in %fat were similar between the two techniques, suggesting that ADP can be translated to its use in clinical practice and research studies as DXA currently is used.

Background To determine the validity of the recently developed child-specific thoracic gas volume (TGV) prediction equations for use in air-displacement plethysmography (ADP) in diverse pediatric populations. Methods Three distinct populations were studied: European American and African American children living in Birmingham, Alabama and European children living in Lisbon, Portugal. Each child completed a standard ADP testing protocol, including a measured TGV according to the manufactures software criteria. Measured TGV was compared to the predicted TGV from current adult-based ADP proprietary equations and to the recently developed child-specific TGV equations of Fields et al. Similarly, percent body fat, derived using the TGV prediction equations, was compared to percent body fat derived using measured TGV. Results Predicted TGV from adult-based equations was significantly different from measured TGV in girls from each of the three ethnic groups (P < 0.05), however child-specific TGV estimates did not significantly differ from measured TGV in any of the ethnic or gender groups. Percent body fat estimates using adult-derived and child-specific TGV estimates did not differ significantly from percent body fat measures using measured TGV in any of the groups. Conclusion The child-specific TGV equations developed by Fields et al. provided a modest improvement over the adult-based TGV equations in an ethnically diverse group of children.

Air-displacement plethysmography holds promise as an alternative to more traditional body composition techniques, although our understanding of air-displacement plethysmography is less than complete. Specifically, factors that influence its validity and application in certain populations, for example children, the obese, and athletes, must be better understood. This review will summarize recent findings on the validity and precision of air-displacement plethysmography and will focus primarily on papers published since 2004, with particular attention on its use in infants.The most significant recent findings in the air-displacement plethysmography literature are mechanistic in nature specifically dealing with measurement issues such as heat, moisture, clothing, and recently, inter-device variability.It is important to recognize that air-displacement plethysmography can be a practical instrument in the evaluation of body composition in a wide range of populations. Therefore, based on the body of literature that has emerged, air-displacement plethysmography appears to be a suitable and reliable instrument in the assessment of body composition. Of particular interest is its use in pediatric and obese individuals, areas requiring further study. Research is also needed to help us better understand sources of measurement error.

Background The purpose of this study was to examine the accuracy of total body density and percent body fat (% fat) using air displacement plethysmography (ADP) and hydrostatic weighing (HW) in children. Methods Sixty-six male and female subjects (40 males: 12.4 ± 1.3 yrs, 47.4 ± 14.8 kg, 155.4 ± 11.9 cm, 19.3 ± 4.1 kg/m2; 26 females: 12.0 ± 1.9 yrs, 41.4 ± 7.7 kg, 152.1 ± 8.9 cm, 17.7 ± 1.7 kg/m2) were tested using ADP and HW with ADP always preceding HW. Accuracy, precision, and bias were examined in ADP with HW serving as the criterion method. Lohman's equations that are child specific for age and gender were used to convert body density to % fat. Regression analysis determined the accuracy of ADP and potential bias between ADP and HW using Bland-Altman analysis. Results For the entire group (Y = 0.835x + 0.171, R2 = 0.84, SEE = 0.007 g/cm3) and for the males (Y = 0.837x + 0.174, R2 = 0.90, SEE = 0.006 g/cm3) the regression between total body density by HW and by ADP significantly deviated from the line of identity. However in females, the regression between total body density by HW and ADP did not significantly deviate from the line of identity (Y = 0.750x + 0.258, R2 = 0.55, SEE = 0.008 g/cm3). The regression between % fat by HW and ADP for the group (Y = 0.84x + 3.81, R2 = 0.83, SEE = 3.35 % fat) and for the males (Y = 0.84x + 3.25, R2 = 0.90, SEE = 3.00 % fat) significantly deviated from the line of identity. However, in females the regression between % fat by HW and ADP did not significantly deviate from the line of identity (Y = 0.81x + 5.17, R2 = 0.56, SEE = 3.80 % fat). Bland-Altman analysis revealed no bias between HW total body density and ADP total body density for the entire group (R = 0.-22; P = 0.08) or for females (R = 0.02; P = 0.92), however bias existed in males (R = -0.37; P ≤ 0.05). Bland-Altman analysis revealed no bias between HW and ADP % fat for the entire group (R = 0.21; P = 0.10) or in females (R = 0.10; P = 0.57), however bias was indicated for males by a significant correlation (R = 0.36; P ≤ 0.05), with ADP underestimating % fat at lower fat values and overestimating at the higher % fat values. Conclusion A significant difference in total body density and % fat was observed between ADP and HW in children 10–15 years old with a potential gender difference being detected. Upon further investigation it was revealed that the study was inadequately powered, thus we recommend that larger studies that are appropriately powered be conducted to better understand this potential gender difference.

Background Air-displacement plethysmography (ADP) is becoming a popular method to assess body composition. Several studies have shown certain types of clothing can affect measurements of body density, however no study has specifically investigated the effect of cotton gym shorts and spandex bicycle shorts on body density. Methods Thirty-seven males (23.0 ± 3.2 yr., 177.3 ± 5.4 cm., 74.8 ± 7.5 kg.) and thirty-eight females (23.7 ± 5.3 yr., 163.6 ± 8.4 cm., 57.1 ± 7.0 kg.) had their body density measured by ADP in three clothing schemes: 1) a tight fitting Speedo® swim suit (criterion measure), 2) cotton gym shorts, and 3) spandex bicycle shorts. The clothing was provided by the University of Oklahoma Body Composition Laboratory and the testing schemes were performed in random order. Results The regression of body density by the criterion measure against body density while wearing cotton gym shorts for the entire group (y = 0.001 + 0.991x, SEE = 0.003 g/cm3) and the females (y = 0.059 + 0.934x, SEE = 0.003 g/cm3) did not significantly deviate from the line of identity. However in males the regression significantly deviated from the line of identity (y = 0.052 + 0.944x, SEE = 0.002 g/cm3). Body density by the criterion measure and body density while wearing spandex bicycle shorts did not significantly differ from the line of identity for the entire group (y = -0.018 + 1.013x SEE = 0.003 g/cm3), in males (y = -0.002 + 1.001x, SEE = 0.003 g/cm3), or females (y = 0.073 + 0.925x, SEE = 0.003 g/cm3). Residual plot analysis revealed no group or gender bias in either the cotton gym shorts or in the spandex bicycle shorts. Conclusion It would appear bicycle spandex shorts are an acceptable alternative to a Speedo® like swim suit, however we advise that subjects adhere to the strict clothing protocol that is recommended by the manufacturer.

Objective: To develop child-specific thoracic gas volume (TGV) prediction equations for use in air-displacement plethysmography in 6- to 17-year-old children. Research Methods and Procedures: Study 1 developed TGV prediction equations using anthropometric variables after completing a measured TGV and air-displacement plethysmography test in 224 healthy boys and girls (11.2 ± 3.2 years, 45.3 ± 18.7 kg, 149.9 ± 18.5 cm). Study 2 cross-validated the prediction equations in a separate cohort of 62 healthy boys and girls (11.2 ± 3.4 years, 44.2 ± 15.3 kg, 149.4 ± 19.3 cm). Results: In Study 1 (development of TGV prediction equations), the quadratic relationship using height as the independent variable and the measured TGV as the dependent variable yielded the highest adjusted R2 and the lowest SE of estimate in both genders, thus producing the following prediction equations: TGV = 0.00056 × H2 − 0.12422 × H + 8.15194 (boys) and TGV = 0.00044 × H2 − 0.09220 × H + 6.00305 (girls). In Study 2 (cross-validation), no significant difference between the predicted and measured TGVs (−0.018 ± 0.377 liters) was observed. The regression between the measured TGV and the predicted TGV yielded a slope and intercept that did not significantly differ from the line of identity. Prediction accuracy was good as indicated by a high R2 (0.862) and low SE of estimate (0.369 liters). Discussion: The new child-specific TGV prediction equations accurately, precisely, and without bias estimated the actual TGV of 6- to 17-year-old children.

This review will focus chiefly on recently published studies utilizing air-displacement plethysmography (i.e. BOD POD) in geriatric populations. This innovative technology has been available commercially since 1995. As the test procedure is relatively easy to perform and quick, it may provide an improvement in body composition testing, especially in older individuals with poor ambulation and health.This review will explain in a concise and detailed manner the underlying principles of air-displacement plethysmography specifically related to the BOD POD. Second, it will review the studies using this new technology in comparison with more commonly used techniques (hydrostatic weighing, dual energy X-ray absorptiometry, deuterium oxide, multi-compartmental models) for body composition analysis in geriatric populations. Third, it will provide a direction for future studies.A review of the current body of literature in which air-displacement plethysmography was used is beginning to emerge with a clear picture. Although this technique is still new, it appears that air-displacement plethysmography is a valid and reliable alternative to more traditional body composition techniques, as indicated by small mean differences between techniques. This has special implications in a geriatric population because traditional techniques are difficult to perform in individuals with joint pain, ambulation issues, and overall poor health.

The purpose of this study was to determine what effects 26 wk of resistance training have on resting energy expenditure (REE), total free-living energy expenditure (TEE), activity-related energy expenditure (AEE), engagement in free-living physical activity as measured by the activity-related time equivalent (ARTE) index, and respiratory exchange ratio (RER) in 61- to 77-yr-old men ( n = 8) and women ( n = 7). Before and after training, body composition (four-compartment model), strength, REE, TEE (doubly labeled water), AEE (TEE − REE + thermic response to meals), and ARTE (AEE adjusted for energy cost of standard activities) were evaluated. Strength (36%) and fat-free mass (2 kg) significantly increased, but body weight did not change. REE increased 6.8%, whereas resting RER decreased from 0.86 to 0.83. TEE (12%) and ARTE (38%) increased significantly, and AEE (30%) approached significance ( P = 0.06). The TEE increase remained significant even after adjustment for the energy expenditure of the resistance training. In response to resistance training, TEE increased and RER decreased. The increase in TEE occurred as a result of increases in both REE and physical activity. These results suggest that resistance training may have value in increasing energy expenditure and lipid oxidation rates in older adults, thereby improving their metabolic profiles.

OBJECTIVE: The objective of this study was to examine the influence of body weight and body composition on aspects of aerobic fitness. Our hypothesis was that increased body weight, specifically increased fat mass (FM), would not limit VO2max relative to fat-free mass (FFM), but would reduce maximal and sub-maximal VO2max relative to body weight. DESIGN: We used data from two ongoing studies. In Study 1 a cross-sectional analysis of 129 children across a wide spectrum of body composition was performed. In Study 2 we examined data from 31 overweight women before and after weight loss. METHODS: VO2max was measured using a treadmill test. Sub-maximal aerobic capacity was evaluated with respiratory exchange ratio (RER), heart-rate (HR), and oxygen uptake relative to VO2max at a given workload (%VO2max). Body composition was assessed using dual energy X-ray absorptiometry (DXA) (Study 1) and a four-compartment model (Study 2). For 10 years, the Centers for Disease Control and Prevention (CDC) and the American College of Sports Medicine (ACSM) have called on U.S. adults to incorporate moderate intensity physical activity (MPA) into their daily routine. Given that one fourth of U.S. women participate in no physical activities, it is important to assess whether messages promoting MPA, such as those in the mass media, help women adequately understand the intensity of activity required to meet the recommendation or whether additional methods may be needed.

Background Years spent in college represents a critical time for obesity development though little information is known regarding how body weight and composition changes beyond the first year of college. The aim of this study was to investigate changes in body weight and composition and the factors influencing those changes among sophomore females. Methods Body composition by dual energy X-ray absorptiometry was obtained in participants beginning during their freshman year and continued through their sophomore year. Results No difference was observed between sophomore year fall and spring visits for body weight (60.4 versus 60.6 kg) or fat mass (19.3 versus 18.7 kg). However, a significant (P ≤ 0.05) decrease was observed for body fat (31.9 versus 30.9 %fat) and a significant increase was observed for fat-free mass (37.7 versus 38.4 kg). Participants living off campus significantly (P ≤ 0.05) declined in body fat (33.0 versus 31.0 %fat) and fat mass (19.4 versus 18.2 kg) and increased in fat-free mass (36.1 versus 37.2 kg) with no differences in those living on campus. Conclusion No change in body weight was observed in females during their sophomore year. However, an increase in fat-free mass accompanied with a decrease in fat mass resulted in a decrease in body fat. Participants living off campus had favorable changes in their body composition by means of decreasing %fat and fat mass while increasing fat-free mass. Participants living on campus did not demonstrate these favorable changes.

To investigate the effect of body temperature and moisture on body fat (%fat), volume and density by air-displacement plethysmography (BOD POD). %fat, body volume and density by the BOD POD before (BOD PODBH) and immediately following hydrostatic weighing (BOD PODFH) were performed in 32 healthy females (age (yr) 33 ± 11, weight (kg) 64 ± 14, height (cm) 167 ± 7). Body temperature and moisture were measured prior to BOD PODBH and prior to BOD PODFH with body moisture defined as the difference in body weight (kg) between the BOD PODBH and BOD PODFH measurements. BOD PODFH %fat (27.1%) and body volume (61.5 L) were significantly lower (P ≤ 0.001) and body density (1.0379 g/cm3) significantly higher (P ≤ 0.001) than BOD PODBH %fat (28.9%), body volume (61.7 L), and body density (1.0341 g/cm3). A significant increase in body temperature (~0.6°C; P ≤ 0.001) and body moisture (0.08 kg; P ≤ 0.01) were observed between BOD PODBH and BOD PODFH. Body surface area was positively associated with the difference in %fat independent of changes in body temperature and moisture, r = 0.30, P < 0.05. These data demonstrate for the first time that increases in body heat and moisture result in an underestimation of body fat when using the BOD POD, however, the precise mechanism remains unidentified.

Background More people than ever are considered obese and the resulting health problems are evident. These facts highlight the need for identification of critical time periods for weight gain. Therefore the purpose was to assess potential changes that occur in body weight during the Thanksgiving holiday break in college students. Methods 94 college students (23.0 ± 4.6 yrs, 72.1 ± 14.0 kg, 172.6 ± 9.3 cm, 24.0 ± 3.9 kg/m2) reported to the human body composition laboratory at the University of Oklahoma following a 6-hour fast with testing occurring prior to, and immediately following the Thanksgiving holiday break (13 ± 3 days). Body weight (BW) was assessed using a balance beam scale while participants were dressed in minimal clothing. Paired t-tests were used to assess changes in BW pre and post Thanksgiving holiday with additional analysis by gender, body mass index (BMI), and class standing (i.e. undergraduate vs. graduate). Results Overall, a significant (P < 0.05) increase in BW was found between pre (72.1 kg) and post (72.6 kg) Thanksgiving holiday. When stratified by gender and class standing a significant (P < 0.05) increase in body weight was observed between the pre and post Thanksgiving holiday in males (0.6 kg), females (0.4 kg) and graduate students (0.8 kg). When participants were classified by BMI as normal or as overweight/obese, a significant 1.0 kg BW gain was found (P < 0.05) in the overweight/obese (≥25 kg/m2) group compared to a non significant 0.2 kg gain in the normal group (2). Conclusion These data indicate that participants in our study gained a significant amount of BW (0.5 kg) during the Thanksgiving holiday. While an increase in BW of half a kilogram may not be cause for alarm, the increase could have potential long-term health consequences if participants retained this weight gain throughout the college year. Additionally, because the overweight/obese participants gained the greatest amount of BW, this group may be at increased risk for weight gain and further obesity development during the holiday season.
