Abstract

ABSTRACT. The article covers the history of over a 60-year experience of dermatome skin grafting in burn patients. The study emphasizes a great contribution of Academician B.A. Petrov in the development of dermatome skin grafting methodology, its principles remaining unchanged so far. Based on his own observations, the author states that anemia complicating the severe burn course and the lack of anemia prevention caused by a number of factors make a negative impact on the outcome in operative burn treatment, leading to a gradual loss of dermatome skin grafting principles. The author makes a conclusion on the necessity of adequate blood transfusions in patients with severe burns in order to keep the red blood parameters normal.

2. Combined dermal autograft in the surgical treatment of full thickness burns

Abstract

The authors developed the method of skin grafting using a combined (epidermis-dermis) autograft that allows increasing the skin graft area two-fold or even more. We have assessed the survival of the dermal component of the graft in 10 patients who sustained full thickness burns of 5 - 15 %of the total body surface area (TBSA). We have demonstrated that the most complete survival of the dermal coinponent of the skin graft was observed in the cases of postoperative using the dressings containing epidermal growth factor (Hebermin ointment) and moisture- retaining film bandages (native-manufactured Breiemenderm film bandage). The period till the final epithelialization of the dermal component and donor wound made 2.5 - 3 weeks after grafting procedure. We hope that this period may be considerably shortened thanks to applying cellular technology and tissue engineering achievements in postoperative period. Improved results obtained in the survival of the dermal component of the combined skin graft allow using it to solve the problem of deficient donor skin resources in the patients with severe extensive burns.

Abstract

Two methods of preparing the pig-skin at a meat-packing plant and its application as a biological bandage in the clinic of thermal injuries are described. The first method is the sterile preparing. The second one--xenotransplants are prepared without an observation of aseptics rules. The material is subject to lyophilic drying and sterilization by gamma rays. The application of xenotransplants gave good results in the treatment of 107 patients with burns.

Abstract

The authors carried out serological typing of 98 Pseudomonas aeruginosa strains, isolated from patients of burn department of the Sklifosovsky First Aid Institute in January-July, 1974, and of 215 strains obtained from other sources; their sensitivity to 13 antibiotics was determined. Pseudomonas aeruginosa cultures isolated from the patients were typed with O-sera of 10 serological types. The presence of several hospital strains of Pseudomonas aeruginosa was found by means of serological typing; along with these there were revealed cultures of this causative agent sporadically appearing in the department. Sensitivity to some antibiotics could serve as an additional criterion for differentiation of Pseudomonas aeruginosa strains of the same serological type.
