Objective: There is currently no evidence-based method for defining competency in pediatric flexible bronchoscopy (FB). Based on expert opinion, guidelines using numbers of procedures have been published in defining competency for pediatric FB. The purpose of this study was to formally survey the opinion of USA pediatric pulmonology training directors about the assessment of competency and training experiences in pediatric FB in their programs., Methods: An Internet-based Survey Monkey™ of the Pediatric Pulmonary Training Directors Association (PEPTDA) was administered 10/12/10 through 11/1/10 with a supplemental survey 6/1/11-6/30/11., Results: This survey of US pediatric pulmonology training directors about competency and training in pediatric FB showed that a majority (86%) felt there was a minimum threshold of procedures for developing competency that could be defined, with a median of 50 and an average of 56.4 (SD = 33.0). The actual number of FBs performed by fellows during their 3-year fellowship averaged 89.4 (SD = 45.3) with a range of 10-200. The survey also revealed a variety of teaching techniques used for FB, including simulation technology. Finally many differences were reported in skill assessment, locations for performance of FB, and the range of underlying indications and patient populations. The apprenticeship model is the predominant method of learning FB in the surveyed programs., Conclusions: A majority of US pediatric pulmonology training directors felt that a minimum number of procedures could be defined for developing competency in pediatric FB. There was variability in the numbers of procedures performed, training techniques and assessment, and application of FB. Great advances had been made in rigid instrumentation, with the introduction of the glass rod telescope. With relatively few exceptions, however, bronchoscopy was still performed primarily for therapeutic indications, such as the removal of foreign bodies from the airway. It would remain for the introduction of the flexible bronchoscope to stimulate the widespread development of diagnostic bronchoscopy in pediatric practice.

Endoscopic techniques for evaluation and management of airway and pulmonary problems in pediatric patients have matured greatly in the past decade, especially with the introduction of flexible instruments suitable for use in the smallest infants. Careful selection of the most appropriate instrument and technique by well-trained medical or surgical endoscopists will result in safe and effective diagnosis and treatment of a wide variety of conditions affecting the upper and lower respiratory tract.

Technological advances in flexible bronchoscopy have expanded the clinician's ability to diagnose and treat pulmonary disease in children. During the neonatal period, flexible bronchoscopy has contributed to the understanding of the incidence and factors responsible for acquired airway lesions. The ability to selectively collect lower airway secretions has contributed to the care of immunocompromised patients with new pulmonary infiltrates. New therapies may use the flexible bronchoscope to specifically target lower airway tissues of interest. Because of the breadth of both current and future applications, most pediatricians will require a working familiarity with the benefits of flexible bronchoscopy in their patients.

Bronchopleural fistulae that occur following pulmonary resection are usually managed by direct, operative closure. In complex cases, in which the risk of repeat thoracotomy is great, other means may be preferable. We report two patients, one with cystic fibrosis and one with extensive radiation fibrosis post-Askin's tumor, in whom the risk of thoracotomy was considered to be prohibitive. Both had a large fistula between the pleural cavity and a segmental bronchus from the right upper lobe. The cystic fibrosis patient had recurrent massive bleeding from the pleural space. A Teflon catheter was passed through a flexible bronchoscope and Super Glue (butyl or methyl methacrylate) was deposited into the fistula. In both cases, the fistula resolved promptly. One patient developed a large, recurrent granuloma at the site of the fistula requiring endoscopic resection. We believe that tissue adhesive may be a reasonable approach to the management of large bronchopleural fistulas when the risk of operative closure is great.

Considerable practice is necessary to develop the skills essential to successful and safe performance of flexible bronchoscopy, especially in pediatric patients. We review a number of model systems, both mechanical and living, for learning and maturing bronchoscopic skills. For each of the animal models (canine, feline, leporine, and simian), the relevant anatomy and anesthetic techniques as well as the relative advantages and disadvantages of each model are discussed.

Flexible bronchoscopy is an important diagnostic technique for study of pediatric patients with pulmonary problems. Many pitfalls await the unwary, but with experience and care, most can be overcome or circumvented.

We believe that many of our readers will appreciate having some familiarity with the procedures described in this manuscript. For those who have had difficulty in deciding whether the need for diagnosis justifies the risk of a procedure, this perspective will shed new light on the problem. It is appropriate to warn our readers that complete unanimity on several points is not yet evident. For example, the indications for flexible bronchoscopy, by whom the procedure should be done, and even more specific technical aspects are questions not completely resolved. However, this article is not an effort to instruct anyone in how to do the procedure. Rather, it is an effort to acquaint our readers with progress in the field. With that in mind, we are confident that the remaining questions will be answered on the basis of the training, experience, and, ultimately, the good judgment of all the physicians involved.

The author discusses his experience with the flexible bronchoscope, covering current instrumentation, techniques, and indications and contraindications, showing that, when used carefully by a well-trained physician, the flexible bronchoscope is a safe, relatively simple, and effective tool for exploration of the pediatric airway.

Over a 5-year period 1,000 pulmonary endoscopic procedures (172 laryngoscopies, 828 bronchoscopies) were performed in children less than 10 years of age using flexible fiberoptic bronchoscopes, with sedation and topical anesthesia. An endoscopic diagnosis of direct relevance to the primary indication for the procedure was established in 76% of the cases; in an additional 15%, abnormalities relevant to a secondary indication were found. Findings were normal in 9% of the cases. The bronchoscope was most useful in the evaluation of patients who had stridor, atelectasis, persistent wheezing, or a suspected foreign body for which there was insufficient evidence to warrant open-tube bronchoscopy, and for patients who had tracheostomies. The high diagnostic yield and low complication rate strongly support the use of the flexible bronchoscope in the diagnostic evaluation of infants and children who have a variety of pulmonary problems.
