OBJECTIVE Analyze the performance of the oriented oral health care network from its implementation, in 2004 to 2020, according to publications on the subject. METHODS This is a research with a methodological description of metasynthesis. RESULTS The searches resulted in 600 complete publications (586 in the first search and another 14 in the second search), according to the established criteria. 539 articles were excluded: 151 after duplication analysis, 236 after reading the title, 45 by type of publication and 107 after reading the abstract, as they did not fit the research theme. Thus, 61 original publications and another 29 publications in snowball sampling were selected and analyzed, totaling 90 publications. From this selection, we chose to use the model proposed by the Projeto de Avaliações de Desempenho de Sistemas de Saúde (PROADESS — Health Systems Performance Assessment Project). The study will use its guiding principles on the dimension “Health Services Performance”. CONCLUSION The analyzed set allowed us to identify that the way the Brazilian State organizes and finances oral health care made it possible to expand access and the number of procedures performed, but not the creation of an effective comprehensive care network, after more than a decade of implementation of Brasil Sorridente (Smiling Brazil).

The option to use emergency dental services is dependent upon the social, epidemiological and organizational conditions of the health services. In order to evaluate the specific emergency indicators in the care network for understanding access and performance, a time series study of emergency dental care according to the codes of care by health facilities in Brazil from 2008 to 2015 was carried out. Health services were grouped into primary and secondary care points and total services. Secondary services were subdivided into specialized and hospital services. The primary care group accounted for 72.75% of the emergency care and the mean of the standardized rate of total emergencies was 0.04 visits per inhabitant. The trend for total services was stable, though there was growth in primary health care services (5.58%/year). The results highlight the participation of primary health care services in the care of dental emergencies, followed by specialized and hospital services, which is in line with the assumption of the inverse relationship between the capacity of emergency dental care service and its technological density.A utilização dos serviços de atenção às urgências odontológicas é moldada pelas condições sociais, epidemiológicas e de organização dos serviços de saúde. Visando avaliar os indicadores específicos de urgência na rede de atenção para compreensão do acesso realizado e desempenho, foi realizado um estudo de série temporal dos atendimentos de urgência odontológica segundo os códigos de atendimento por estabelecimentos de saúde no Brasil no período de 2008 a 2015. Os estabelecimentos foram agrupados em seu total e segundo os pontos de atenção primária e secundária, este subdividido em atenção secundária especializada e hospitalar. O grupo da atenção primária respondeu por 72,75% dos atendimentos de urgência e a média da taxa padronizada do total de urgências foi de 0,04 atendimentos por habitante. Foi encontrado padrão de estabilidade no total de atendimentos por urgência odontológica para o total de estabelecimentos e de crescimento na atenção primária (5,58%/ano). Os resultados ressaltam a participação dos serviços de atenção primária na atenção às urgências odontológicas, seguida dos grupos atenção secundária especializada e hospitalar, alinhando-se ao pressuposto da relação inversa entre capacidade de atendimento de urgência odontológica e a densidade tecnológica.

Part of the oral health care in the care network encompasses users in cases of emergency. This study proposed to map the determinants of the use of dental care services within the health care network to address dental emergencies of the Brazilian Unified Health System (UHS) and to verify the main gaps in the research in this area. This is a scoping review that took place in 2018 using Andersen’s behavioral model as a reference. A total of 16 studies, out of 3786 original articles identified, were included and reviewed. Two reviewers independently conducted the selection process and the decision was consensually taken. The mapping of the determinants revealed a greater number of enabling factors and a larger gap in results. Greater use of the emergency service was registered by people in pain, women, adults, from an urban area, with a lower income, and less education. In future studies, primary surveys are recommended, which include all ages, analyzing different groups of needs and users that take into account the country’s northern region and the different subjects pointed out in this review.

To describe and evaluate the factors associated with actions for the control of tuberculosis (TB) in primary care (PC) in the five Brazilian macroregions.This cross-sectional study was carried out with data from the second cycle of the National Program for Improving Access to and Quality of Primary Care. Theoutcome of the study was constructed based on a set of items that were considered essential for the treatment and control of tuberculosis in Primary Care Units (PCUs). Data were analyzed using the χ2 test and Poisson regression with robust variance.The national prevalence of the set of items to control tuberculosis was 17.22%. TheNortheast (11.18%) and North (12.15%) had the worst performance. The main results indicate association with this outcome for PCUs performing educational actions for TB (PR = 1.53; 95%CI 1.45 - 1.62), those performing HIV serology (PR = 1.68; 95%CI 1.11 - 2.54), those that have a reception room (PR = 1.61; 95%CI 1.46 - 1.79) and those performing continuing education activities (PR = 1.73; 95%CI 1.54 - 1.95).The results show a weakness in the structures and in the work process of PC in relation to the control of tuberculosis in all Brazilian regions.Descrever e avaliar os fatores associados ao conjunto de ações para o controle da tuberculose (TB) na atenção básica (AB) nas cinco macrorregiões brasileiras.Trata-se de um estudo transversal de base em serviço com dados obtidos a partir do segundo ciclo do Programa Nacional de Melhoria do Acesso e da Qualidade da Atenção Básica (PMAQ-AB). O desfecho foi construído a partir de um conjunto de itens que caracterizam a realização de ações para o cuidado no controle e tratamento da TB nas unidades básicas de saúde (UBSs). Os dados foram analisados por meio do teste do χ2 e da regressão de Poisson com variância robusta.A prevalência nacional do conjunto de ações para controle da TB foi de 17,22%, sendo que as macrorregiões Nordeste (11,18%) e Norte (12,15%) tiveram o pior desempenho. Os resultados principais apontam que houve associação da presença do conjunto de ações para o controle da TB com as UBSs que realizam ações educativas para TB [razão de prevalência - RP = 1,53 (intervalo de confiança de 95% - IC95% 1,45 - 1,62)], sorologia para HIV [RP = 1,68 (IC95% 1,11 - 2,54)], possuem sala de acolhimento [RP=1,61(IC95% 1,46 - 1,79)] e atividades de educação permanente [RP = 1,73 (IC95% 1,54- 1,95)].Os resultados demonstram fragilidade nas estruturas e no processo de trabalho da AB em relação ao controle da TB em todas as regiões brasileiras.
