Perinatal mortality includes fetal mortality and early neonatal mortality (0 to 6 days of life). The study described perinatal deaths in Brazil in 2018 according to the modified Wigglesworth classification. The data sources were the Brazilian Mortality Information System and the Brazilian Information System on Live Births. Fetal mortality and perinatal mortality rates were calculated per 1,000 total births (live births plus stillbirths) and the early neonatal mortality rate per 1,000 live births, compared using their respective 95% confidence intervals (95%CI). Perinatal deaths were classified in groups of antepartum causes, congenital anomalies, prematurity, asphyxia, and specific causes. For each group of causes, the study calculated the number of deaths by weight group, in addition to mortality rates and respective 95%CI, besides the spatial distribution of mortality rates by state of Brazil. A total of 35,857 infant deaths were recorded, of which 18,866 (52.6%) were early neonatal deaths, while stillbirths totaled 27,009. Perinatal deaths totaled 45,875, for a mortality rate of 15.5‰ births. The highest mortality rate (7.6‰; 7.5‰-7.7‰) was observed in the antepartum group, followed by prematurity (3.6‰; 3.6‰-3.7‰). In the antepartum group, 14 of the 27 states (eight of which in the Northeast and four in the North) presented perinatal mortality rates above the national rate. Perinatal mortality in Brazil was high, and most deaths could have been prevented with investment in prenatal and childbirth care.A mortalidade perinatal engloba a mortalidade fetal e a neonatal precoce (0 a 6 dias). Este estudo descreveu os óbitos perinatais ocorridos no Brasil em 2018, segundo a classificação de Wigglesworth modificada. As fontes de dados foram os Sistemas de Informações sobre Mortalidade e sobre Nascidos Vivos. Foram calculadas as taxas de mortalidade fetal e perinatal por mil nascimentos totais (nascidos vivos mais natimortos) e a taxa de mortalidade neonatal precoce por mil nascidos vivos, e comparadas usando seus respectivos intervalos de 95% de confiança (IC95%). Os óbitos perinatais foram classificados nos grupos de causas anteparto, anomalias congênitas, prematuridade, asfixia e causas específicas. Foi calculado, para cada grupo de causas, o número de óbitos por faixa de peso, além das taxas de mortalidade e os respectivos IC95%, e feita a distribuição espacial das taxas de mortalidade por Unidade da Federação (UF). Foram registrados 35.857 óbitos infantis, sendo 18.866 (52,6%) neonatais precoces; os natimortos somaram 27.009. Os óbitos perinatais totalizaram 45.875, perfazendo uma taxa de mortalidade de 15,5‰ nascimentos. A maior taxa de mortalidade (7,6‰; 7,5‰-7,7‰) foi observada no grupo anteparto, seguido da prematuridade (3,6‰; 3,6‰-3,7‰). No grupo anteparto, 14 das 27 UFs (sendo oito na Região Nordeste e quatro na Região Norte) apresentaram as taxas de mortalidade perinatal acima da nacional. A taxa de mortalidade perinatal no Brasil mostrou-se elevada, e a maioria dos óbitos poderia ser prevenida com investimento em cuidados pré-natais e ao nascimento.La mortalidad perinatal engloba la mortalidad fetal y neonatal precoz (0 a 6 días). Los óbitos perinatales se clasificaron en los grupos de causas: anteparto, anomalías congénitas, prematuridad, asfixia y causas específicas. Se calculó, para cada grupo de causas, el número de óbitos por franja de peso, además de las tasas de mortalidad y los respectivos IC95%, y se realizó la distribución espacial de las tasas de mortalidad por Unidad de la Federación (UF). Se registraron 35.857 óbitos infantiles, siendo 18.866 (52,6%) neonatales precoces; los mortinatos sumaron 27.009. Los óbitos perinatales totalizaron 45.875, ascendiendo a una tasa de mortalidad de un 15,5‰ nacimientos. La mayor tasa de mortalidad (7,6‰; 7,5‰-7,7‰) se observó en el grupo anteparto, seguido de la prematuridad (3,6‰; 3,6‰-3,7‰). En el grupo anteparto, 14 de las 27 UFs (estando ocho en la región Nordeste y cuatro en la región Norte) presentaron tasas de mortalidad perinatal por encima de la nacional. La tasa de mortalidad perinatal en Brasil se mostró elevada y la mayoría de los óbitos podría ser prevenido con inversión en cuidados prenatales y en el nacimiento.

OBJECTIVE: To describe the victimization and bullying practice in Brazilian school children, according to data from the National Adolescent School-based Health Survey and to compare the surveys from 2009 and 2012. METHODS: This is a cross-sectional study with univariate and multivariate analyzes of the following variables: to have been treated badly by colleagues, to have been bullied and to have bullied other children. The following independent variables were analyzed: age, sex, race/color, type of school, maternal education. Prevalence rates were compared between the editions of 2009 and 2012 of the survey. RESULTS: Of all the adolescents analyzed, 27.5% have not been treated well by peers at school, with greater frequency among boys (OR = 1.50), at the age of 15 years (OR = 1.29) and 16 (OR = 1.41), public school students (OR = 2.08), black (OR = 1.18) and whose mothers had less education; 7.2% reported having been bullied, with a greater chance in younger students (13 years old), male (OR = 1.26), black (OR = 1.15) and indigenous (OR = 1.16) and whose mothers had less education; 20.8% reported to have bullied other children, with a greater chance for older students, at the age of 14 (OR = 1.08) and 15 years (OR = 1.18), male (OR = 1.87), black (OR = 1.14) and yellow (OR = 1.15), children of mothers with higher education, private school students. There was an increase of bullying in the Brazilian capitals, from 5.4 to 6.8%, between 2009 and 2012. DISCUSSION: The occurrence of bullying reveals that the Brazilian school context is also becoming a space of reproduction of violence, in which it is crucial to act intersectorally and to articulate social protection networks, aiming to face this issue. OBJETIVO: Descrever a vitimização e a prática de bullying em escolares brasileiros, segundo dados da Pesquisa Nacional de Saúde do Escolar (PeNSE), e comparar a evolução entre as pesquisas de 2009 e 2012. MÉTODOS: Trata-se de estudo transversal com análises univariadas e multivariadas das variáveis: não ser bem tratado, sofrer bullying e praticar bullying. Como variáveis independentes, foram analisadas: idade, sexo, raça/cor, tipo de escola, escolaridade materna. Foram comparadas as prevalências entre as edições da PeNSE de 2009 e 2012. RESULTADOS: Não foram bem tratados pelos colegas na escola 27,5% dos adolescentes, tendo sido maior a frequência entre meninos (OR = 1,50) mais velhos, 15 anos (OR = 1,29) e 16 anos (OR = 1,41), alunos de escolas públicas (OR = 2,08), de raça/cor preta (OR = 1,18) e cujas mães tinham menor escolaridade. Relatam ter sofrido bullying 7,2%, tendo sido a maior chance em alunos mais jovens (13 anos) do sexo masculino (OR = 1,26), da raça/cor preta (OR = 1,15) e indígena (OR = 1,16) e cujas mães apresentaram menor escolaridade. A prática de bullying foi relatada por 20,8% e mostrou maior chance em alunos mais velhos, 14 anos (OR = 1,08) e 15 anos (OR = 1,18), do sexo masculino (OR = 1,87), raça/cor preta (OR = 1,14) e amarela (OR = 1,15), filhos de mães com maior escolaridade e alunos de escola privada. Ocorreu aumento de bullying nas capitais, passando de 5,4 para 6,8% entre 2009 e 2012. DISCUSSÃO: A ocorrência de bullying revela que o contexto escolar brasileiro também tem se tornado um espaço de reprodução da violência, sendo fundamental atuar de forma intersetorial e articulando redes de proteção social, visando seu enfrentamento.

to describe the prevalence coefficients and characterize cases of microcephaly at birth in Brazil from 2000-2015.this is a descriptive study with data from the Information System on Live Births (Sinasc). The coefficients were calculated by region and characteristics of mothers and live births (LB).the annual average number of microcephaly cases was 164 for the period 2000-2014, whilst in 2015, 1,608 cases were registered (54.6 cases per 100 thousand LB). Higher coefficients were observed among preterm babies (81.7; 95%CI 72.3;92.2), born from black-skinned (70.9; 95%CI 58.5;85.9) or to brown-skinned (71.5; 95%CI67.4;75.8) women, to women aged ≤19 (70.3; 95%CI 63.5;77.8) or ≥40 (62.1; 95%CI 46.6;82.6), with ≤3 years of study (73.4; 95%CI 58.2;92.4) and residents in the Northeast region (138.7; 95%CI 130.9;147.0).the high number of microcephaly cases in 2015 reinforces the importance of Sinasc and the need to improve the surveillance of congenital anomalies.

The aim of this study is to identify and describe the occurrence of bullying among students in the 9th year (8th grade) from public and private schools from 26 Brazilian state capitals and the Federal District. It is a cross-sectional study involving 60,973 students and 1,453 public and private schools. Data analysis indicates that 5.4% (IC95%: 5.1%-5.7%) of students reported having suffered bullying almost always or always in the last 30 days, 25.4% (IC95%: 24.8%-26.0%) were rarely or sometimes the victim of bullying and 69.2% (IC95%: 68.5%-69.8%) of students felt no humiliation or provocation at school. The capital with higher frequency of bullying was Belo Horizonte (6.9%; IC95%: 5,9%-7,9%), Minas Gerais, and the lowest was Palmas (3.5%; IC95%: 2.6%-4.5%), Tocantins. Boys reported more bullying (6,0%; IC95%: 5.5%-6.5%) compared with girls (4,8%; IC95%: 4.4%-5.3%). There was no difference between public schools 5.5% (IC95%: 5.1%-5.8%) and private (5.2%) (IC95%: 4.6%-5.8%), except in Aracaju, Sergipe, that show more bullying in private schools. The findings indicate an urgent need for intersectoral action from educational policies and practices that enforce the reduction and prevention of the occurrence of bullying in schools in Brazil. O objetivo deste estudo é identificar e descrever a ocorrência do bullying, episódios de humilhação ou provocação perpetrados pelos colegas da escola, entre estudantes do 9º ano do ensino fundamental de escolas públicas e privadas das 26 capitais dos estados brasileiros e do Distrito Federal. Trata-se de estudo transversal feito com 60.973 escolares de 1.453 escolas públicas e privadas. A análise dos dados aponta que 5,4% (IC95%: 5,1%-5,7%) dos estudantes relataram ter sofrido bullying quase sempre ou sempre nos últimos 30 dias; 25,4% (IC95%: 24,8%-26,0%) foram raramente ou às vezes vítimas de bullying e 69,2% (IC95%: 68,5%-69,8%) não sentiram nenhuma humilhação ou provocação (bullying). A capital com maior frequência de foi Belo Horizonte-MG (6,9%; IC95%: 5,9%-7,9) e a menor foi Palmas-TO (3,5%; IC95%: 2,6%-4,5%). Meninos relatam mais bullying (6,0%; IC95%: 5,5%-6,5%) do que meninas (4,8%; IC95%: 4,4%-5,3%). Não houve diferença entre escolas públicas (5,5%; IC95%: 5,1%-5,8%) e privadas (5,2%; IC95%: 4,6%-5,8%), exceto em Aracaju-SE, onde foi registrada maior ocorrência de bullying em escolas privadas. Os dados mostram urgente necessidade de ações intersetoriais a partir de políticas e práticas educativas que efetivem redução e prevenção da ocorrência do bullying nas escolas.

OBJECTIVE To describe the prevalence of alcohol consumption among adolescent school students and identify its individual and contextual associated factors. METHODS The present research used data from the 2009 National School Health Survey (PeNSE), which included a sample of 59,699 9th grade students in Brazilian capitals and the Federal District. The association between regular alcohol consumption and independent explanatory variables was measured by means of the Pearson’s Chi-square test, with a 0.05 significance level. The explanatory variables were divided into four groups based on affinity (sociodemographic; school and family context; risk factors; and protection factors). A multivariate analysis was carried out for each group, always adjusting for age and sex. Variables with p < 0.10 were used in the final multivariate analysis model. RESULTS The highest alcohol consumption in the preceding 30 days was independently associated with pupils aged 15 years (OR = 1.46) and over, female (OR = 1.72), white, children of mothers with higher education, studying in private school, students who had tried smoking (OR = 1.72) and drug use (OR = 1.81), with regular tobacco consumption (OR = 2.16) and those who have had sexual intercourse (OR = 2.37). The factors related to family were skipping school without parental knowledge (OR = 1.49), parents not knowing what children do in their free time (OR = 1.34), having fewer meals with their parents (OR = 1.22), reporting that parents do not care (OR = 3.05), or care little (OR = 3.39) if they go home drunk, and having suffered domestic violence (OR = 1.36). CONCLUSIONS The results reinforce the importance of viewing alcohol consumption among adolescents as a complex, multifactorial and socially determined phenomenon.

This study evaluates the relation between the use of tobacco, alcoholic beverages and illicit drugs and family protective factors. Data from the National School Health Survey (PeNSE) were analyzed in a sample of 60,973 students at the freshman year of high school, from public and private schools of Brazilian state capitals. Most adolescents lived with both their parents and about a third lived in households only with their mothers. Half the parents or responsible parties are aware of what adolescents do in their free time. Living with both parents is a protective factor for smoking, drinking, and drug use. Family supervision is also important for the prevention of such behavior. Sharing a meal with parents or responsible parties most days of the week and the fact that the parents know what the adolescents have done in their free time in the past 30 days are also protective factors. Students that miss classes without telling their parents have higher chances of using tobacco, alcohol, and drugs. The family plays an essential role to prevent tobacco, alcohol, and drug use, and to promote health among teenagers.

To describe the prevalence of alcohol and other drugs consumption, among adolescent students.A cross-sectional study with conglomerate samples of 60,973 students at freshman year high school in public and private schools in capitals and the Federal District in Brazil, in 2009. The 95% confidence interval and the prevalence of alcohol and drug consumption were analyzed.For the set of surveyed students, the following were identified: experimenting alcoholic beverages (71.4%; 95%CI 70.8-72.0); regular alcoholic beverage consumption (27.3%; 95%CI 26.7-28.0); drunkenness in lifetime (22.1%; 95%CI 21.6-22.7); family is worried when the student gets home drunk (93.8%; 95%CI 93.3-94.2); problems with alcohol use (9.0%; 95%CI 8.6-9.4); consumption of other drugs (8.7%; 95%CI 8.3-9.1).This study shows the extension of the alcohol and drugs problem among Brazilian adolescents, with special emphasis on the easy access of students to alcoholic beverages at parties, bars, stores, and at home.

The objective of this study was to describe situations related to sexual health of adolescents, according to the National Survey of School Health (PeNSE). It is a cross-sectional study performed by the Brazilian Institute of Geography and Statistics (IBGE), in partnership with the Ministry of Health that involved 60,973 students and 1,453 public and private schools. Data analysis points out that 30.5% (95%CI 29.9-31.2) of the adolescents had already had sexual relations sometime in their lives, being more frequent for males (43.7%; 95%CI 42.7-44.7) than females (18.7%; 95%CI 18.0-19.4), especially those who go to public schools (33.1%; 95%CI 32.4-33.9), aged 15 years or older (47.3%; 95%CI 45.7-48.9) and 16 years (63.5%; 95%CI 61,5-65.4). The sexual initiation age was early and 40.1% (95%CI 38.8-41.4) reported having had only one partner in life. The use of condoms in the last sexual relation was high both for protective (75.9%; CI95% 74.8-76.9) and contraceptive methods (74.7%; 95%CI 73.6-75.7). It is necessary to emphasize actions for promoting sexual health towards adolescents in order to minimize vulnerabilities.

Road traffic injuries are the cause of an expressive number of deaths and hospitalizations among young people in the world. In Brazil, it is responsible for 17.1% of all deaths among adolescents aged 10 to 14 years. This article presents the results of the National Adolescent School-based Health Survey (PeNSE), using a probabilistic sample of students in the 9th grade of high schools (public and private) of the Brazilian capitals in 2009. The prevalence (and 95%CI) of risk factors for road traffic injury among adolescents were estimated. Main results from the 60,973 interviews were: in the last 30 days, 26.3% (25.5% to 27.0% 95%CI) reported no use of seat-belts while riding in a moving vehicle; 18.5% (18.0% to 19.1% 95%CI) of youths younger than 18 years reported driving a motor vehicle (or= once); 18.7% (18.1% a 19.2% 95%CI) reported a history of being driven by a driver who had been drinking (or= once); and 35.0% (33.8% to 36.2% 95%CI) reported no use of helmets while riding a motorcycle (or= once). The results are in accordance with the high rates of morbidity and mortality from traffic injury among youths, reinforcing the need of integrated intersectoral actions, specific legal measures and strict control.

This article presents the main results of the National Adolescent School-based Health Survey (PeNSE). A questionnaire was applied to a sample of 60,973 students of the 9th year of Junior high school in public and private schools of the Brazilian state capitals and the Federal District, between March and June 2009. The prevalence and confidence interval of 95% (CI 95%) of the violence situations involving adolescents were analyzed. The following situations were identified: lack of safety on the way home-school (6.4%; CI95%: 6.1%-6.8%) and at school (5.5%; CI95%: 5.2%-5.8%); involving fights with physical aggression (12.9%; CI95%: 12.4%-13.4%), with knife (6.1%; CI95%: 5.7%-6.4%) or fire arm (4.0%; CI95%: 3.7%-4.3%); physical aggression by family member (9.5%; CI95%: 9.1%-9.9%). Violence situations were more prevalent among male students. There were great variations among the cities studied. Adolescents are exposed to different violence manifestations in the institutions that supposedly must assure their protection and healthy development: school and the home. These results aim to support health promotion measures and prevention of these risk factors.

Background In Brazil, 72% of all deaths in 2007 were attributable to non-communicable diseases (NCD). We used a risk and related factor based index to prioritize NCD prevention programs in the combined 26 capital cities and the federal district (i.e., Brasilia) of Brazil. Methods We used 2006–2011 data (adults) from census and Brazil's surveillance of 12 NCD risk factors and 74 disease group mortality. The risk and related factors were: smoking, physical inactivity, overweight-obesity, low fruits and vegetables intake, binge drinking, insufficient Pap smear screening (women aged 25 to 59 years), insufficient mammography screening (women aged 50 to 69 years), insufficient blood pressure screening, insufficient blood glucose screening, diagnosis of hypercholesterolemia, diagnosis of hypertension and diagnosis of diabetes. We generated six indicators: intervention reduction of the risk factor prevalence, intervention cost per person, prevalence of risk factor, deaths attributable to risk factor, risk factor prevalence trend and ratio of risk factor prevalence between people with and without a high school education. We transformed risk and related factor indicators into priority scores to compute a priority health index (PHI). We implemented sensitivity analysis of PHI by computing it with slightly altered formulas and altering values of indicators under the assumption of bias in their estimation. We ranked risk factors based on PHI values. Results We found one intermediate (i.e., overweight-obesity) and six top risk and related factors priorities for NCD prevention in Brazil's large urban areas: diagnosed hypertension, physical inactivity, blood pressure screening, diagnosed hypercholesterolemia, smoking and binge drinking. Conclusion Brazil has already prioritized the six top priorities (i.e., hypertension, physical inactivity, blood pressure screening, hypercholesterolemia, smoking and binge drinking) and one intermediate priority (i.e., overweight-obesity) for NCD prevention identified in this report. Because effective interventions to reduce disease burden associated with each of the six priority risk factors are available, strategies based on these interventions need to be sustained in order to reduce NCD burden in Brazil. PHI can be used to track NCD prevention and health promotion actions at the local and national level in Brazil and in countries with similar public health surveillance systems.
