Prepulse inhibition (PPI) of the startle response is an important measure of information processing deficits and inhibitory failure in schizophrenia patients. PPI is especially useful because it occurs in the same lawful manner in all mammals, from humans to rodents, making it an ideal candidate for cross-species translational research. PPI deficits occur across the "schizophrenia spectrum" from schizophrenia patients to their clinically unaffected relatives. Parallel animal model and human brain imaging studies have demonstrated that PPI is modulated by cortico-striato-pallido-thalamic (and pontine) circuitry. This circuitry is also implicated in schizophrenia neuropathology and neurophysiology. The finding of PPI deficits in schizophrenia patients has been replicated by many groups, and these deficits correlate with measures of thought disorder and appear to be "normalized" by second generation antipsychotic (SGA) medications. Consistent pharmacological effects on PPI have been demonstrated; among these, dopamine agonists induce PPI deficits and (in animal models) these are reversed by first and SGA medications. PPI is also significantly heritable in humans and animals and can be used as a powerful endophenotype in studies of families of schizophrenia patients. Genomic regions, including the NRGL-ERBB4 complex with its glutamatergic influences, are strongly implicated in PPI deficits in schizophrenia. PPI continues to hold promise as an exciting translational cross-species measure that can be used to understand the pathophysiology and treatment of the schizophrenias via pharmacological, anatomic, and genetic studies.

It is almost exactly 10 years since the first report of a PET ligand that specifically bound to a pathological protein in the brain was published (Klunk et al. , 2004). This tracer, 11C-Pittsburgh compound B (PIB), detected fibrillar aggregated forms of amyloid-β, the major constituent of the Alzheimer’s disease plaque and, according to many, the initiating event in Alzheimer’s disease pathogenesis. This report was soon followed by several amyloid imaging tracers that were radiolabelled with the longer half-life positron-emitting nuclide 18F, opening the door to commercial manufacture and clinical application. Several of these tracers are now approved by worldwide regulatory agencies including the US Food and Drug Administration and the European Medicines Agency, although reimbursement for clinical use remains problematic. In this issue of Brain , Okamura and colleagues report the first human PET studies with a new tracer for tau, 18F-THK5105, and reveal that retention of this tracer correlates with dementia severity and brain atrophy in Alzheimer’s disease (Okamura et al. , 2014). Because it is difficult to develop, test and validate a new PET imaging agent, it may have seemed overly optimistic 10 years ago to conclude that a new era in human brain imaging had begun, but it had. The latest developments are a series of PET tracers that bind to the microtubule-associated protein tau that is aggregated as neurofibrillary tangles in Alzheimer’s disease. Tau-related diseases also include the group of tauopathies often referred to as frontotemporal lobar degenerations, and the highly publicized chronic traumatic encephalopathy. Tau would seem to be a difficult target for PET imaging: it may be intracellular thus requiring tracer passage across both the blood–brain barrier and cell membranes; it is found in the brain at lower concentrations than amyloid-β; and it is characterized by different …

For optimal image quality in susceptibility-weighted imaging and accurate quantification of susceptibility, it is necessary to isolate the local field generated by local magnetic sources (such as iron) from the background field that arises from imperfect shimming and variations in magnetic susceptibility of surrounding tissues (including air). Previous background removal techniques have limited effectiveness depending on the accuracy of model assumptions or information input. In this article, we report an observation that the magnetic field for a dipole outside a given region of interest (ROI) is approximately orthogonal to the magnetic field of a dipole inside the ROI. Accordingly, we propose a nonparametric background field removal technique based on projection onto dipole fields (PDF). In this PDF technique, the background field inside an ROI is decomposed into a field originating from dipoles outside the ROI using the projection theorem in Hilbert space. This novel PDF background removal technique was validated on a numerical simulation and a phantom experiment and was applied in human brain imaging, demonstrating substantial improvement in background field removal compared with the commonly used high-pass filtering method.

A new class of composite RF pulses that perform well in the presence of specific ranges of B0 and B1+ inhomogeneities has been designed for volume (non-selective) excitation in MRI. The pulses consist of numerous (approximately 100) short (approximately 10 micros) block-shaped sub-pulses each with different phases and amplitudes derived from numerical optimization. Optimized pulses are designed to be effective over a specific range of frequency offsets and transmit field variations and are thus implementable regardless of field strength, transmit coil configuration, or the subject-specific spatial distribution of the static and RF fields. In the context of 7 T human brain imaging, both simulations and phantom experiments indicate that optimized pulses result in similar on-resonance flip-angle uniformity as BIR-4 pulses but with the advantages of superior off-resonance stability and significantly reduced average power. The pulse design techniques presented here are thus well-suited for practical application in ultra-high field human MRI.

Microsaccades are very small, involuntary flicks in eye position that occur on average once or twice per second during attempted visual fixation. Microsaccades give rise to EMG eye muscle spikes that can distort the spectrum of the scalp EEG and mimic increases in gamma band power. Here we demonstrate that microsaccades are also accompanied by genuine and sizeable cortical activity, manifested in the EEG. In three experiments, high-resolution eye movements were corecorded with the EEG: during sustained fixation of checkerboard and face stimuli and in a standard visual oddball task that required the counting of target stimuli. Results show that microsaccades as small as 0.15° generate a field potential over occipital cortex and midcentral scalp sites 100-140 ms after movement onset, which resembles the visual lambda response evoked by larger voluntary saccades. This challenges the standard assumption of human brain imaging studies that saccade-related brain activity is precluded by fixation, even when fully complied with. Instead, additional cortical potentials from microsaccades were present in 86% of the oddball task trials and of similar amplitude as the visual response to stimulus onset. Furthermore, microsaccade probability varied systematically according to the proportion of target stimuli in the oddball task, causing modulations of late stimulus-locked event-related potential (ERP) components. Microsaccades present an unrecognized source of visual brain signal that is of interest for vision research and may have influenced the data of many ERP and neuroimaging studies. Udgivelsesdato: 2008-Apr-15 With the appropriate radiolabeled tracers, positron emission tomography (PET) enables in vivo human brain imaging of markers for neurotransmission, including neurotransmitter synthesis, receptors, and transporters. Whereas structural imaging studies have provided compelling evidence that the human brain anatomy is largely genetically determined, it is currently unknown to what degree neuromodulatory markers are subjected to genetic and environmental influence. Changes in serotonin 2A (5-HT(2A)) receptors have been reported to occur in various neuropsychiatric disorders and an association between 5-HT(2A) receptor gene variants and neuropsychiatric illness susceptibility also exists. In a classical twin design involving 24 healthy male subjects (6 monozygotic twin pairs and 6 dizygotic twin pairs), we examined the relative contribution of genetic and environmental factors to interindividual variability in cortical 5-HT(2A) receptor binding as measured with [(18)F]altanserin PET imaging. The intraclass correlation coefficients were 0.67 for dizygotic and 0.87 for monozygotic twin pairs. For comparison, the intraclass correlation coefficient was 0.93 in a group of six male healthy subjects examined twice within two weeks with an identical experimental setup. Multivariate analysis was used to separate the phenotypic variance of individuals into additive genetic (heritability) effect (A), shared (family) environment (C), and non-shared (individual-specific) environment (E). Irrespective of whether a full ACE model or a reduced AE model was used to fit the data, the variance due to non-shared environment was below 10% indicating that the contribution of individual specific environmental factors to 5-HT(2A) receptor binding is limited.

We review recent findings in human brain imaging, for example, which brain areas are used during perception of colors, moving objects, human faces, facial expressions, sadness and happiness etc. One study used fluorine-18-labeled deoxyglucose positron emission tomography (PET) in patients with unipolar depression and bipolar depression, and found hypometabolism in the left anterolateral prefrontal cortex. Another study reported increased regional cerebral blood flow in the amygdala in familial pure depressive disease. Using 11C-glucose PET, we reported that the glutamic acid pool was reduced in cortical areas of the brain in patients with major depression. We also found that the thalamic and cingulate areas were hyperactive in drug-naive (never medicated) acute schizophrenics, while the associative frontal, parietal, temporal gyri were hypoactive in drug-naive chronic schizophrenics. Brain biochemical disturbances of schizophrenic patients involved glutamic acid, N-acetyl aspartic acid, phosphatidylcholine and sphingomyelin which are important chemical substances in the working brain. The areas of the thalamus and the cingulate which become hyperactive in acute schizophrenic patients are important brain areas for perception and communication. The association areas of the cortex which become disturbed in chronic schizophrenia are essential brain areas in human creativity (language, concepts, formation of cultures and societies) and exist only in human beings.

One of the challenges in functional brain imaging is integration of complementary imaging modalities, such as magnetoencephalography (MEG) and functional magnetic resonance imaging (fMRI). MEG, which uses highly sensitive superconducting quantum interference devices (SQUIDs) to directly measure magnetic fields of neuronal currents, cannot be combined with conventional high-field MRI in a single instrument. Indirect matching of MEG and MRI data leads to significant co-registration errors. A recently proposed imaging method - SQUID-based microtesla MRI - can be naturally combined with MEG in the same system to directly provide structural maps for MEG-localized sources. It enables easy and accurate integration of MEG and MRI/fMRI, because microtesla MR images can be precisely matched to structural images provided by high-field MRI and other techniques. Here we report the first images of the human brain by microtesla MRI, together with auditory MEG (functional) data, recorded using the same seven-channel SQUID system during the same imaging session. The images were acquired at 46 microtesla measurement field with pre-polarization at 30 mT. We also estimated transverse relaxation times for different tissues at microtesla fields. Our results demonstrate feasibility and potential of human brain imaging by microtesla MRI. They also show that two new types of imaging equipment - low-cost systems for anatomical MRI of the human brain at microtesla fields, and more advanced instruments for combined functional (MEG) and structural (microtesla MRI) brain imaging - are practical., 8 pages, 5 figures - accepted by JMR

[ 18 F]MK-9470 is a selective, high-affinity, inverse agonist (human IC 50 , 0.7 nM) for the cannabinoid CB1 receptor (CB1R) that has been developed for use in human brain imaging. Autoradiographic studies in rhesus monkey brain showed that [ 18 F]MK-9470 binding is aligned with the reported distribution of CB1 receptors with high specific binding in the cerebral cortex, cerebellum, caudate/putamen, globus pallidus, substantia nigra, and hippocampus. Positron emission tomography (PET) imaging studies in rhesus monkeys showed high brain uptake and a distribution pattern generally consistent with that seen in the autoradiographic studies. Uptake was blocked by pretreatment with a potent CB1 inverse agonist, MK-0364. The ratio of total to nonspecific binding in putamen was 4–5:1, indicative of a strong specific signal that was confirmed to be reversible via displacement studies with MK-0364. Baseline PET imaging studies in human research subject demonstrated behavior of [ 18 F]MK-9470 very similar to that seen in monkeys, with very good test–retest variability (7%). Proof of concept studies in healthy young male human subjects showed that MK-0364, given orally, produced a dose-related reduction in [ 18 F]MK-9470 binding reflecting CB1R receptor occupancy by the drug. Thus, [ 18 F]MK-9470 has the potential to be a valuable, noninvasive research tool for the in vivo study of CB1R biology and pharmacology in a variety of neuropsychiatric disorders in humans. In addition, it allows demonstration of target engagement and noninvasive dose-occupancy studies to aid in dose selection for clinical trials of CB1R inverse agonists.

Deep graph-theoretic ideas in the context with the graph of the World Wide Web led to the definition of Google's PageRank and the subsequent rise of the most popular search engine to date. Brain graphs, or connectomes, are being widely explored today. We believe that non-trivial graph theoretic concepts, similarly as it happened in the case of the World Wide Web, will lead to discoveries enlightening the structural and also the functional details of the animal and human brains. When scientists examine large networks of tens or hundreds of millions of vertices, only fast algorithms can be applied because of the size constraints. In the case of diffusion MRI-based structural human brain imaging, the effective vertex number of the connectomes, or brain graphs derived from the data is on the scale of several hundred today. That size facilitates applying strict mathematical graph algorithms even for some hard-to-compute (or NP-hard) quantities like vertex cover or balanced minimum cut. In the present work we have examined brain graphs, computed from the data of the Human Connectome Project, recorded from male and female subjects between ages 22 and 35. Significant differences were found between the male and female structural brain graphs: we show that the average female connectome has more edges, is a better expander graph, has larger minimal bisection width, and has more spanning trees than the average male connectome. Since the average female brain weighs less than the brain of males, these properties show that the female brain has better graph theoretical properties, in a sense, than the brain of males. It is known that the female brain has a smaller gray matter/white matter ratio than males, that is, a larger white matter/gray matter ratio than the brain of males; this observation is in line with our findings concerning the number of edges, since the white matter consists of myelinated axons, which, in turn, roughly correspond to the connections in the brain graph. We have also found that the minimum bisection width, normalized with the edge number, is also significantly larger in the right and the left hemispheres in females: therefore, the differing bisection widths are independent from the difference in the number of edges.

Transgenic mice are used increasingly to model brain amyloidosis, mimicking the pathogenic processes involved in Alzheimer's disease (AD). In this chapter, a strategy is described that has been successfully used to map amyloid deposits in transgenic mouse models of AD with magnetic resonance imaging (MRI), utilizing molecular targeting vectors labeled with MRI contrast agents to enhance selectively the signal from amyloid plaques. To obtain sufficient spatial resolution for effective and sensitive mouse brain imaging, magnetic fields of 7-Tesla (T) or more are required. These are higher than the 1.5-T field strength routinely used for human brain imaging. The higher magnetic fields affect contrast agent efficiency, and determine the choice of pulse sequence parameters for in vivo MRI, all addressed in this chapter. Ex vivo imaging is also described as an important step to test and optimize protocols prior to in vivo studies. The experimental setup required for mouse brain imaging is explained in detail, including anesthesia, immobilization of the mouse head to reduce motion artifacts, and anatomical landmarks to use for the slice alignment procedure to improve image co-registration during longitudinal studies, and for subsequent matching of MRI with histology.

International audience; Wavelets provide an orthonormal basis for multiresolution analysis and decorrelation or 'whitening' of nonstationary time series and spatial processes. Wavelets are particularly well suited to analysis of biological signals and images, such as human brain imaging data, which often have fractal or scale-invariant properties. We briefly define some key properties of the discrete wavelet transform (DWT) and review its applications to statistical analysis of functional magnetic resonance imaging (fMRI) data. We focus on time series resampling by 'wavestrapping' of wavelet coefficients, methods for efficient linear model estimation in the wavelet domain, and wavelet-based methods for multiple hypothesis testing, all of which are somewhat simplified by the decorrelating property of the DWT.

Functional organization of the human brain involved in attention has been determined in many human brain imaging studies. Most of these studies focused on brain mechanism related to selective attention using different sensory modality or sub-modality stimuli. Recently, we have shown other aspects of human attentional system, that is spatial attention, using event-related functional MRI. In this study, using delayed sequential saccade paradigm, subjects were forced to pay their attention towards right or left visual field. We could not find significant difference in brain activation patterns between right and left visual field attention tasks. The results are inconsistent to the findings of the previous lesion studies of humans showing spatial neglect. Further collaborative studies of neurological and brain mapping researches should be necessary to understand human higher cognitive functions.

Most high-field MRI systems do not have the actively detuned body coils that are integral to clinical systems operating at 1.5T and lower field strengths. Therefore, many clinical applications requiring homogeneous volume excitation in combination with local surface coil reception are not easily implemented at high fields. To solve this problem for neuroimaging applications, actively detunable transverse electromagnetic (TEM) head coils were developed to be used with receive-only surface coils for signal-to-noise ratio (SNR) gains and improved spatial coverage from homogeneously excited regions. These SNR and field of view (FOV) gains were achieved by application of a detunable TEM volume coil to human brain imaging at 4T.

▪ Abstract Depression is a disorder of the representation and regulation of mood and emotion. The circuitry underlying the representation and regulation of normal emotion and mood is reviewed, including studies at the animal level, human lesion studies, and human brain imaging studies. This corpus of data is used to construct a model of the ways in which affect can become disordered in depression. Research on the prefrontal cortex, anterior cingulate, hippocampus, and amygdala is reviewed and abnormalities in the structure and function of these different regions in depression is considered. The review concludes with proposals for the specific types of processing abnormalities that result from dysfunctions in different parts of this circuitry and offers suggestions for the major themes upon which future research in this area should be focused.

Technological advances in imaging have made it possible to noninvasively extract information from the human brain regarding a drug's mechanism and site of action. Until now, our understanding of human brain pharmacology has depended primarily on indirect assessments or models derived from animal studies. However, the advent of multiple techniques for human brain imaging allows researchers to focus directly on human pharmacology and brain function. In this review article, our PET studies on histaminergic neuron system were presented as an example. We have developed and used the PET techniques for 10 years in order to examine the functions of histaminergic neurons in the living human brain. This review outlines available PET techniques and examines how these various methods have already been applied to the drug development process and neuropharmacology in the living human brain.

A review of the behavior of different diffusion anisotropy indices (DAIs) to changes in diffusion anisotropy was undertaken and the limitations of several previously proposed DAIs are discussed. A mathematical representation of anisotropy sensitivity was used to enable image contrast and sensitivity to noise to be predicted. This technique was utilized to devise a new DAI, the gamma-variate anisotropy, which has a low sensitivity to noise and an improved contrast range for human brain imaging. This index can be easily modified for other applications, in that the range of anisotropy values to which it is highly sensitive can be altered. Magn Reson Med 44:117-121, 2000.

Facial identity recognition has been studied mainly with explicit discrimination requirement and faces of social figures in previous human brain imaging studies. We performed a PET activation study with normal volunteers in facial identity recognition tasks using the subject's own face as visual stimulus. Three tasks were designed so that the activation of the visual representation of the face and the effect of sustained attention to the representation could be separately examined: a control-face recognition task (C), a passive own-face recognition task (no explicit discrimination was required) (P), and an active own-face recognition task (explicit discrimination was required) (A). Increased skin conductance responses during recognition of own face were seen in both task P and task A, suggesting the occurrence of psychophysiological changes during recognition of one's own face. The left fusiform gyrus, the right supramarginal gyrus, the left putamen, and the right hypothalamus were activated in tasks P and A compared with task C. The left fusiform gyrus and the right supramarginal gyrus are considered to be involved in the representation of one's own face. The activation in the right supramarginal gyrus may be associated with the representation of one's own face as a part of one's own body. The prefrontal cortices, the right anterior cingulate, the right presupplementary motor area, and the left insula were specifically activated during task A compared with tasks C and P, indicating that these regions may be involved in the sustained attention to the representation of one's own face.

A novel MRI method, DIFRAD-FSE (diffusion-weighted radial acquisition of data with fast spin-echo), is demonstrated that enables rapid, high-resolution multi-shot diffusion-weighted MRI without significant artifacts due to motion. Following a diffusion-weighting spin-echo preparation period, multiple radial lines of Fourier data are acquired using spin-echo refocusing. Images can be reconstructed from the radial data set using a magnitude-only filtered back-projection reconstruction algorithm that removes phase errors due to motion. Results from human brain imaging demonstrate the ability of DIFRAD-FSE to acquire multiple radial lines of Fourier data each TR period without significant artifacts due to relaxation and to produce high-resolution diffusion-weighted MRI images without significant artifacts from motion.

Proton magnetization transfer contrast (MTC) imaging, using continuous wave off-resonance irradiation, was performed on the rat brain in vivo at 4.7 Tesla. The observed MTC was studied in three different brain regions: the corpus callosum, the basal ganglia, and the temporal lobe. By systematically varying the offset frequency and the amplitude of the RF irradiation, the observed signal intensities for each region of interest were modeled using a system including free water and a pool of protons with restricted motions (R. M. Henkelman, X. Huang, Q. Xiang, G. J. Stanisz, SD Swanson, M. J. Bronskill, Magn. Res. Med. 29, 759 (1993)). Most of the relaxation parameters of both proton pools remained fairly constant for the three regions of interest, with a T2 value of about 9 micros for the immobilized protons, whereas the rate of exchange increased significantly from the temporal lobe to the corpus callosum. The optimal acquisition parameters for the improved MTC under steady-state saturation were found to be 2-10 kHz offset frequency and 500-800 Hz RF irradiation amplitude. Conversely, an irradiation amplitude of 3 kHz at an offset frequency of 12 kHz is required to minimize the direct effect of off-resonance irradiation. Such an approach could be extended to human brain imaging with the aim of characterizing tissue-specific disease.

Since the invention of x-ray computed tomography 20 years ago, 1 a progressive array of constantly improving structural and functional imaging techniques has been developed to study the human brain in vivo. The modern structural imaging methods include x-ray computed tomography, magnetic resonance imaging, and magnetic resonance angiography. As discussed in several articles in this issue, 2-7 these techniques are of increasing value for investigating the morphometric structure of the human brain when it is healthy and diseased. Functional human brain imaging methods include single photon emission computed tomography, positron emission tomography, functional magnetic resonance imaging, magnetic resonance spectroscopy, optical intrinsic signal imaging, electroencephalography, and magnetoencephalography. The brain has revealed many of its secrets during the decade dedicated to its honor. Many of the exquisite techniques used in quite ingenious ways to uncover the workings of the brain involve microanalysis of the chemistry and biophysics of the brain’s complex and varied cellular structure. But, most revelations that can be readily appreciated by a general audience come from the giant leaps made in brain imaging. In this field we can see the human brain in action. We can see the brain registering a sound, a sight, or a touch. We can capture the neural origins of the movement of a finger or toe or the complex movement involved in uttering a word. We can image a thought process. Skilled investigators are dissecting the inner workings of the mind, seeing behavior in the functioning brain in startling and compelling ways. Herein are described the tools that have revealed much. Investigators such as Marcus Raichle, Richard Frackowiak, and John Mazziotta have pioneered work that is inherently interesting and has a solid application in clinical settings for diagnosis and treatment. The new advances that allow imaging of the brain are driven by the biology of the brain and its control of its own blood flow, complex physical principles, and computer technology. These several advances of modern technology have enabled us to understand the structure and function of the mind. With the newest technologies we can actually see a thought formulate in the mind. Among these are magnetic encephalography, functional magnetic resonance imaging, and positron emission tomography. Most of these approaches have become rather well known to the world-at-large. Human brain imaging began in the early seventies with X-ray computed tomography (CT or CAT), which shows only coarse structure. The real underlying advance was the ability to generate, using computers, maps of the brain taken from signals being generated and recorded.

Research on the nicotinic cholinergic system function in the brain was previously mainly derived from animal studies, yet, research in humans is growing. Up to date, findings allow significant advances on the understanding of nicotinic cholinergic effects on human cognition, emotion and behavior using a range of functional brain imaging approaches such as pharmacological functional magnetic resonance imaging or positron emission tomography. Studies provided insights across various mechanistic psychological domains using different tasks as well as at rest in both healthy individuals and patient populations, with so far partly mixed results reporting both enhancements and decrements of neural activity related to the nicotinic cholinergic system. Moreover, studies on the relation between brain structure and the nicotinic cholinergic system add important information in this context. The present review summarizes the current status of human brain imaging studies and presents the findings within a theoretical and clinical perspective as they may be useful not only for an advancement of the understanding of basic nicotinic cholinergic-related mechanisms, but also for the development and integration of psychological and pharmacological treatment approaches. Patterns of functional neuroanatomy and neural circuitry across various cognitive and emotional domains may be used as neuropsychological markers of mental disorders such as addiction, Alzheimer's disease, Parkinson disease or schizophrenia, where nicotinic cholinergic system changes are characteristic. The principal aim of this thesis was to investigate human brain energy metabolism during physiological activation using proton (1H) magnetic resonance spectroscopy (MRS) at 7 Tesla (T). High magnetic fields (≥ 7 T) provide several advantages in human brain imaging and spectroscopy. In particular, signal-to-noise ratio (SNR) and chemical shift dispersion increase at higher magnetic fields, allowing for improved accuracy and precision of metabolite quantification. To take profit of the benefits of MRS at high field, one needs to provide an advanced methodology including hardware, pulse sequences experimental setup and data analysis. Each of these aspects was thoroughly explored in this thesis. In the field of functional studies, functional MRS (fMRS) has recently been used as a research tool to investigate the neuroenergetic and metabolic basis of physiologic brain activation. It provides direct insight into brain metabolism by non-invasively determining concentrations of metabolites. This is critical for the basic understanding of overall brain function, and potentially of the pathogenesis of many neurodegenerative diseases. In general, an optimized MR system and accurate acquisition methodology are required for high sensitivity and reliable metabolite quantification. The metabolites of interest for fMRS studies (e.g. lactate, glucose) are present in low concentration and metabolite changes are small under activation. Hence, studies of metabolite changes, in particular lactate, have led to inconsistent reports in the literature over the last decades. To address these challenges, it was essential to develop a robust MR protocol for the quantitative measurement of metabolite changes. A fMRS study was performed to investigate metabolite changes during visual stimulation using the enhanced sensitivity of the SPin ECho full Intensity Acquired Localized (SPECIAL) sequence. Small but significant increases of lactate (19 ± 4 %, P < 0.05) and glutamate (4 ± 1 %, P < 0.001) were observed using a small number of subjects (n = 6). With the exception of glucose (12 ± 5 %, P < 0.001), no other significant metabolite concentration changes beyond experimental error were observed. Based on this successful fMRS study, we further investigated brain energy metabolism. A subsequent fMRS study was performed to determine metabolite changes occurring during motor activation in the human brain. This second study demonstrated that increases in lactate (17 ± 5 %, P < 0.001) and glutamate (2 ± 1 %, P < 0.005) during motor stimulation were small, but similar to those observed during visual stimulation. These metabolite changes were further analyzed, and they supported the hypothesis of an increase in the change of cerebral metabolic rate of oxygen, ∆CMRO2, that is transiently lower than that of glucose, ∆CMRGlc, during the first one to two minutes of stimulation. Finally, we hypothesized that the observed glutamate and lactate increases were a general manifestation of the blood-oxygenation level dependent effect. The accuracy and the reliability of metabolite quantification is particularly challenging at short echo times due to the presence of broad underlying resonances overlapping with those of metabolite. Such resonances arise from macromolecules, which are characterized by short T1 and T2 relaxation times. Two studies were performed at 3 T and 7 T to investigate the characteristics of the macromolecule signal. An important finding was that the mathematical approximation (spline baseline) was a sufficient estimation of the macromolecule contribution to 1H spectra at 3 T compared to the experimentally measured macromolecule signal for healthy subjects. As a caveat, small, but significant, differences in the quantification may need to be taken into account when comparing metabolite concentrations obtained with these two different approaches, such as for glutamate and for γ-amino-butyric, which were more reliably quantified when using the measured macromolecule baseline. In addition, a study on the tissue-specificity of the macromolecule signal in the healthy human occipital lobe at 7 T reported that a general average macromolecule signal ensured reliable metabolite quantification. In general, these studies helped to set up a robust and reliable methodology in order to investigate human brain metabolism with improved accuracy. Despite their great potential for MRS, high main magnetic fields also compose challenges. In particular, the wavelength of the proton spin resonance becomes comparable to the size of a human head (at 7 T, λ = 11 cm). The phase variation across the brain produces signal addition (constructive interference, central bright spot) and signal cancellation (destructive interference, signal drop). Transceive arrays have been developed to address this issue. In this study, we designed, tested, and built an eight-channel transceive array, which was used to acquire images of the human head at 7 T (297 MHz). The design aimed for a low mutual coupling between the microstrip elements (> -15 dB), a relatively large penetration depth in the loading sample, reduced radiation losses and load-invariant tuning and matching. Electromagnetic simulations were run using Microwave Studio (CST, Darmstadt, Germany) for design optimization. The model was further demonstrated with the construction of the transceive array. Finally, the resulting array coil was shown to be handling, stable and also suited for techniques in which the transmission signals from all channels with different amplitudes and phases are combined. To conclude, this thesis provided complementary and new metabolic information about cerebral energy metabolism during physiological activation. Furthermore, the macromolecule studies and the building of the transceive array allowed to improve the accuracy and sensitivity of NMR measurements.

The Brain Images of Normal Subjects (BRAINS) Imagebank (http://www.brainsimagebank.ac.uk) is an integrated repository project hosted by the University of Edinburgh and sponsored by the Scottish Imaging Network: A Platform for Scientific Excellence (SINAPSE) collaborators. BRAINS provide sharing and archiving of detailed normal human brain imaging and relevant phenotypic data already collected in studies of healthy volunteers across the life-course. It particularly focusses on the extremes of age (currently older age, and in future perinatal) where variability is largest, and which are under-represented in existing databanks.BRAINS is a living imagebank where new data will be added when available. Currently BRAINS contains data from 808 healthy volunteers, from 15 to 81 years of age, from 7 projects in 3 centres. Additional completed and ongoing studies of normal individuals from 1st to 10th decades are in preparation and will be included as they become available.BRAINS holds several MRI structural sequences, including T1, T2, T2* and fluid attenuated inversion recovery (FLAIR), available in DICOM (http://dicom.nema.org/); in future Diffusion Tensor Imaging (DTI) will be added where available. Images are linked to a wide range of ‘textual data’, such as age, medical history, physiological measures (e.g. blood pressure), medication use, cognitive ability, and perinatal information for pre/post-natal subjects. The imagebank can be searched to include or exclude ranges of these variables to create better estimates of ‘what is normal’ at different ages.
