The wearable healthcare equipment is primarily designed to alert patients of any specific health conditions or to act as a useful tool for treatment or follow-up. With the growth of technologies and connectivity, the security of these devices has become a growing concern. The lack of security awareness amongst novice users and the risk of several intermediary attacks for accessing health information severely endangers the use of IoT-enabled healthcare systems. In this paper, a blockchain-based secure data storage system is proposed along with a user authentication and health status prediction system. Firstly, this work utilizes reversed public-private keys combined Rivest-Shamir-Adleman (RP 2 -RSA) algorithm for providing security. Secondly, feature selection is completed by employing the correlation factor-induced salp swarm optimization algorithm (CF-SSOA). Finally, health status classification is performed using advanced weight initialization adapted SignReLU activation function-based artificial neural network (ASR-ANN) which classifies the status as normal and abnormal. Meanwhile, the abnormal measures are stored in the corresponding patient blockchain. Here, blockchain technology is used to store medical data securely for further analysis. The proposed model has achieved an accuracy of 95.893% and is validated by comparing it with other baseline techniques. On the security front, the proposed RP 2 -RSA attains a 96.123% security level.

Detection of unknown malware and its variants remains both an operational and a research challenge in the Internet of Things (IoT). The Internet of Medical Things (IoMT) is a particular type of IoT network which deals with communication through smart healthcare (medical) devices. One of the prevailing problems currently facing IoMT solutions is security and privacy vulnerability. Previous malware detection methods have failed to provide security and privacy. In order to overcome this issue, the current study introduces a novel technique called biserial correlative Miyaguchi-Preneel blockchain-based Ruzicka-index deep multilayer perceptive learning (BCMPB-RIDMPL). The present research aims to improve the accuracy of malware detection and minimizes time consumption. The current study combines the advantages of machine-learning techniques and blockchain technology. The BCMPB-RIDMPL technique consists of one input layer, three hidden layers, and one output layer to detect the malware. The input layer receives the number of applications and malware features as input. After that, the malware features are sent to the hidden layer 1, in which feature selection is carried out using point biserial correlation, which reduces the time required to detect the malware. Then, the selected features and applications are sent to the hidden layer 2. In that layer, Miyaguchi-Preneel cryptographic hash-based blockchain is applied to generate the hash value for each selected feature. The generated hash values are stored in the blockchain, after which the classification is performed in the third hidden layer. The BCMPB-RIDMPL technique uses the Ruzicka index to verify the hash values of the training and testing malware features. If the hash is valid, then the application is classified as malware, otherwise it is classified as benign. This method improves the accuracy of malware detection. Experiments have been carried out on factors such as malware detection accuracy, Matthews's correlation coefficient, and malware detection time with respect to a number of applications. The observed quantitative results show that our proposed BCMPB-RIDMPL method provides superior performance compared with state-of-the-art methods.

The integration of artificial intelligence (AI) has caused information and communication technology (ICT) to undergo a number of recent rapid fluctuations. These changes have primarily affected the areas of management, end-to-end device interconnectivity, resource organization, communication, networking, and application-related aspects of ICT. Owing to the complex structure of applicational connectedness, evaluating each of the aforementioned opportunities concurrently reflects the idea of heterogeneity. The association of multiple end devices, particularly in interoperable space, integrity, privacy protection, security, provenance, and the massive volume of everyday media data generated in the modern healthcare setting could also provide significant issues. To address these issues, decentralized, secure, economical resource optimization, and intelligent network activities and organization are necessary. Blockchain technology plays a crucial role in providing distributed storage data organization, sharing, and exchange for automated decision-making, privacy, and security in AI-enabled machine learning (ML) models. However, machine learning models—support vector machine, in particular—have a significant impact on the growth of distributed consortium networks and the exchange of information among connected nodes, resolving issues with resource management, scalability, and data processing. By resolving the three main problems of seamless data integrity, peer-to-peer communication between nodes, and infrastructure security, we provide a novel interoperable technique in this proposed architecture. The approach is unique, as demonstrated by the simulation-based results, which display huge differences of 1.37%, 1.56%, and 1.87%, respectively. The background for the evaluation consists of the following three areas: (i) infrastructure security to protect automated decision-making; (ii) integrity between smooth data sharing and exchange; and (iii) network resource optimization to enable smooth communication across heterogeneous devices. This paper explores the transformative impact of the Internet of Medical Things (IoMT) on healthcare. By integrating medical equipment and sensors with the internet, IoMT enables real-time monitoring of patient health, remote patient care, and individualized treatment plans. IoMT significantly improves several healthcare domains, including managing chronic diseases, patient safety, and drug adherence, resulting in better patient outcomes and reduced expenses. Technologies like blockchain, Artificial Intelligence (AI), and cloud computing further boost IoMT’s capabilities in healthcare. Blockchain enhances data security and interoperability, AI analyzes massive volumes of health data to find patterns and make predictions, and cloud computing offers scalable and cost-effective data processing and storage. Therefore, this paper provides a comprehensive review of the Internet of Things (IoT) and IoMT-based edge-intelligent smart healthcare, focusing on publications published between 2018 and 2024. The review addresses numerous studies on IoT, IoMT, AI, edge and cloud computing, security, Deep Learning, and blockchain. The obstacles facing IoMT are also covered in this paper, including interoperability issues, regulatory compliance, and privacy and data security concerns. Summary: The rapid progression of the Internet of Things and its increasing use in healthcare has generated considerable concerns over the safeguarding and privacy of vital medical data. In response to these issues, blockchain has surfaced as a possible remedy, offering transparent, immutable, and decentralized storage. Nevertheless, conventional blockchain‐based systems still encounter constraints in maintaining anonymity, confidentiality, and privacy. Hence, this article suggests a framework based on a secure consortium blockchain that prioritizes data privacy and employs time‐based authentication to streamline patient data monitoring. First, we employ time‐based authentication to verify the identities of authorized users. This process utilizes the NIK‐512 hashing algorithm in conjunction with passwords and registered timestamps, which strengthens the confidentiality of data. Patient information undergoes encryption before transmission within the network. Further, our framework introduces a sensor registration service that the trusted node employs to assign a distinct identity to each sensor connected to a patient. The implementation of data processing and filtering techniques at the edge layer serves the purpose of mitigating disturbances that may occur during the collection of sensor‐based data. Finally, a comprehensive evaluation of performance and security has been carried out with various metrics. This study provides a revolutionary strategy that makes use of blockchain technology, information fusion, and federated illness prediction and deep extreme machine learning to meet the difficulties with regard to healthcare privacy. The suggested framework integrates several innovative technologies to make healthcare systems safe and privacy-preserving. The framework leverages the blockchain system, a distributed and unchangeable ledger, to secure the integrity, traceability and openness of private medical information. Patient privacy is better protected as a result, and there is less chance of data breaches or unauthorized access. The system makes use of the Linear Discriminant Analysis (LDA), Decision Tree, Extra Tree Classifier, AdaBoost, and Federated Deep Extreme Machine Learning algorithms to increase the accuracy and efficacy of illness prediction. This method allows for collaborative learning across many healthcare organizations without disclosing raw data, protecting privacy. A healthcare-focused version of the Internet of Things (IoT), the Internet of Medical Things (IoMT) enables real-time monitoring and remote medical support via integrated medical devices, programs, and support solutions. However, patients' safety and anonymity are in jeopardy by the open access networks employed in IoMT, which makes the systems susceptible to several threats and security lapses. By harnessing the synergies of blockchain, cloud computing, and digital twins, this study presents a comprehensive architecture and a secure lightweight authentication mechanism (which integrates the benefits offered by Yu and Park, Yu et al., and Amintoshi et al.) that addresses these concerns. The suggested method entails using session keys for secure communication while authenticating medical professionals and patients through a gateway. Cloud computing offers a flexible and robust framework for managing and storing medical data. Additionally, it simulates digital twins to enable data-driven decision-making and predictive analysis, and the incorporation of blockchain offers a decentralized and immutable ledger for recording and validating patient data and transaction logs enhancing data integrity, transparency, and traceability. Healthcare systems may confidently embrace the potential of IoMT by implementing this framework since it offers promising solutions to enhance the security and confidentiality of patient data in IoMT while supporting the provision of the best healthcare services, especially in emergency scenarios like the COVID-19 pandemic. The Internet of Medical Things (IoMT) has significantly advanced healthcare, but it has also brought about critical security challenges. Traditional security solutions struggle to keep pace with the dynamic and interconnected nature of IoMT systems. Machine learning (ML)-based Intrusion Detection Systems (IDS) have been increasingly adopted to counter cyberattacks, but centralized ML approaches pose privacy risks due to the single points of failure (SPoFs). Federated Learning (FL) emerges as a promising solution, enabling model updates directly on end devices without sharing private data with a central server. This study introduces the BFLIDS, a Blockchain-empowered Federated Learning-based IDS designed to enhance security and intrusion detection in IoMT networks. Our approach leverages blockchain to secure transaction records, FL to maintain data privacy by training models locally, IPFS for decentralized storage, and MongoDB for efficient data management. Ethereum smart contracts (SCs) oversee and secure all interactions and transactions within the system. We modified the FedAvg algorithm with the Kullback–Leibler divergence estimation and adaptive weight calculation to boost model accuracy and robustness against adversarial attacks. For classification, we implemented an Adaptive Max Pooling-based Convolutional Neural Network (CNN) and a modified Bidirectional Long Short-Term Memory (BiLSTM) with attention and residual connections on Edge-IIoTSet and TON-IoT datasets. We achieved accuracies of 97.43% (for CNNs and Edge-IIoTSet), 96.02% (for BiLSTM and Edge-IIoTSet), 98.21% (for CNNs and TON-IoT), and 97.42% (for BiLSTM and TON-IoT) in FL scenarios, which are competitive with centralized methods. The Internet of Medical Things (IoMT) is an extended genre of the Internet of Things (IoT) where the Things collaborate to provide remote patient health monitoring, also known as the Internet of Health (IoH). Smartphones and IoMTs are expected to maintain secure and trusted confidential patient record exchange while managing the patient remotely. Healthcare organizations deploy Healthcare Smartphone Networks (HSN) for personal patient data collection and sharing among smartphone users and IoMT nodes. However, attackers gain access to confidential patient data via infected IoMT nodes on the HSN. Additionally, attackers can compromise the entire network via malicious nodes. This article proposes a Hyperledger blockchain-based technique to identify compromised IoMT nodes and safeguard sensitive patient records. Furthermore, the paper presents a Clustered Hierarchical Trust Management System (CHTMS) to block malicious nodes. In addition, the proposal employs Elliptic Curve Cryptography (ECC) to protect sensitive health records and is resilient against Denial-Of-Service (DOS) attacks. Finally, the evaluation results show that integrating blockchains into the HSN system improved detection performance compared to the existing state of the art. E-health is the use of information and communication technology (ICT) for the healthcare-related services. It uses various types of digital technologies and telecommunications, such as computers, sensing devices, Internet, and mobile devices to deliver medical services. Internet of Medical Things (IoMT) is a communication environment optimized for low-power devices (for example, health sensors and actuators) and operation on, in, or around the human body (i.e., a patient). It can be used in various applications that are related to healthcare, such as "body automation," "healthcare," "medical monitoring," "body interaction," and "medical implants (i.e., pacemaker)." Most of the communications happen in IoMT-based e-healthcare system are wireless in nature. This may cause severe threats to the security of the system. Various information securityrelated attacks, i.e., replay, man-in-the-middle attack (MiTM), impersonation, privileged insider, unauthorised session key computation, credentials leakage, stolen verifier, malware injection are possible in IoMT-based ehealthcare system. These threats and attacks can create serious problems in the social life of an individual, as this may reveal their confidential healthcare information to other unauthorised parties. Therefore, it is essential to propose an access control and key management scheme to secure the communication of a IoMT-based e-healthcare system. Moreover, the security of such kind of scheme can also be enhanced through the deployment of a blockchain mechanism. Therefore, in this article, we propose a blockchain-enabled access control and key management protocol for IoMT-based e-healthcare system that is named as "BACKM-EHA" in short. The security analysis of proposed BACKM-EHA is also provided through the standard, i.e., "Real-Or-Random model." The various conducted security analyses prove the security of BACKM-EHA against the different types of potential attacks. The healthcare sector is a very crucial and important sector of any society, and with the evolution of the various deployed technologies, like the Internet of Things (IoT), machine learning and blockchain it has numerous advantages. However, in this section, the data is much more vulnerable than others, because the data is strictly private and confidential, and it requires a highly secured framework for the transmission of data between entities. In this article, we aim to design a blockchain-envisioned authentication and key management mechanism for the IoMT-based smart healthcare applications (in short, we call it SBAKM-HS). We compare the various attributes of the proposed SBAKM-HS and other existing schemes to demonstrate that SBAKM-HS outperforms other existing schemes. The conducted security analysis and formal security verification via Scyther automated validation tool prove the security of the proposed SBAKM-HS against various possible potential attacks. Next, a real-tested implementation of SBAKM-HS is provided to observe its impact on the performance of the system.

Secure storage and sharing of Personal Health Records (PHRs) in Internet of Medical Things (IoMT) is one of the significant challenges in the healthcare ecosystem. Due to the high value of personal health information, PHRs are one of the favourite targets of cyber attackers worldwide. Over the years, many solutions have been proposed; however, most solutions are inefficient for practical applications. For instance, several existing schemes rely on the bilinear pairings, which incur high computational costs. To mitigate these issues, we propose a novel PHR-sharing scheme that is dynamic, efficient, and practical. Specifically, we combine searchable symmetric encryption, blockchain technology and a decentralized storage system, known as Inter-Planetary File System (IPFS) to guarantee confidentiality of PHRs, verifiability of search results, and forward security. Moreover, we provide formal security proofs for the proposed scheme. Finally, we have conducted extensive test-bed experiments and the results demonstrate that the proposed scheme can be used in practical scenarios related to IoMT environment.

The Internet of Medical Things is developing rapidly in recent years. However, the timeliness and security of healthcare applications challenge its adoption. In this article, we propose a blockchain-assisted edge computing platform that timely and securely processes time-sensitive healthcare applications. We propose a blockchain-assisted framework that leverages distributed edge servers to achieve fast data processing. We design smart contracts to verify the identity and data credibility of network entities. We formulate the problem as a directed acyclic graph organized scheduling model and develop online orchestrating algorithms to meet the timeliness requirement. We implement the blockchain prototype and evaluate the performance of the proposed algorithm under extensive configurations. Results show that fast blockchain-assisted edge computing platform for healthcare achieves a significant timeliness guarantee, and at the same time outperforms conventional schemes from the latency perspective. Recently, blockchain-based Internet of Medical Things (IoMT) has started to receive more attention in the healthcare domain as it not only improves the care quality using real-time and continuous monitoring but also minimizes the cost of care. However, there is a clear trend to include many entities in IoMT systems, such as IoMT sensor nodes, IoT wearable medical devices, patients, healthcare centers, and insurance companies. This makes it challenging to design a blockchain framework for these systems where scalability is a most critical factor in blockchain technology. Motivated by this observation, in this survey we review the state-of-the-art in blockchain-IoMT systems. Comparison and analysis of such systems prove that there is a substantial gap, which is the negligence of scalability. In this survey, we discuss several approaches proposed in the literature to improve the scalability of blockchain technology, and thus overcoming the above mentioned research gap. These approaches include on-chain and off-chain techniques, based on which we give recommendations and directions to facilitate designing a scalable blockchain-based IoMT system. We also recommended that a designer considers the well-known trilemma along with the various dimensions of a scalable blockchain system to prevent sacrificing security and decentralization as well. Moreover, we raise several research questions regarding benchmarking; addressing these questions could help designers determining the existing bottlenecks, leading to a scalable blockchain.

The Internet of Medical Things (IoMT) is a kind of connected infrastructure of smart medical devices along with software applications, health systems and services. These medical devices and applications are connected to healthcare systems through the Internet. The Wi-Fi enabled devices facilitate machine-to-machine communication and link to the cloud platforms for data storage. IoMT has the ability to make accurate diagnoses, with fewer mistakes and lower costs of care. IoMT with smartphone applications permits the patients to exchange their health related confidential and private information to the healthcare experts (i.e., doctors) for the better control of diseases, and also for tracking and preventing chronic illnesses. Due to insecure communication among the entities involved in IoMT, an attacker can tamper with the confidential and private health related information for example an attacker can not only intercept the messages, but can also modify, delete or insert malicious messages during communication. To deal this sensitive issue, we design a novel blockchain enabled authentication key agreement protocol for IoMT environment, called BAKMP-IoMT. BAKMP-IoMT provides secure key management between implantable medical devices and personal servers and between personal servers and cloud servers. The legitimate users can also access the healthcare data from the cloud servers in a secure way. The entire healthcare data is stored in a blockchain maintained by the cloud servers. A detailed formal security including the security verification of BAKMP-IoMT using the widely-accepted Automated Validation of Internet Security Protocols and Applications (AVISPA) tool is performed to demonstrate its resilience against the different types of possible attack. The comparison of BAKMP-IoMT with relevant existing schemes is conducted which identifies that the proposed system furnishes better security and functionality, and also needs low communication and computational costs as compared to other schemes. Finally, the simulation of BAKMP-IoMT is conducted to demonstrate its impact on the performance parameters.

Recent years have witnessed a paradigm shift in the storage of Electronic Health Records (EHRs) on mobile cloud environments, where mobile devices are integrated with cloud computing to facilitate medical data exchanges among patients and healthcare providers. This advanced model enables healthcare services with low operational cost, high flexibility, and EHRs availability. However, this new paradigm also raises concerns about data privacy and network security for e-health systems. How to reliably share EHRs among mobile users while guaranteeing high-security levels in the mobile cloud is a challenging issue. In this paper, we propose a novel EHRs sharing framework that combines blockchain and the decentralized interplanetary file system (IPFS) on a mobile cloud platform. Particularly, we design a trustworthy access control mechanism using smart contracts to achieve secure EHRs sharing among different patients and medical providers. We present a prototype implementation using Ethereum blockchain in a real data sharing scenario on a mobile app with Amazon cloud computing. The empirical results show that our proposal provides an effective solution for reliable data exchanges on mobile clouds while preserving sensitive health information against potential threats. The system evaluation and security analysis also demonstrate the performance improvements in lightweight access control design, minimum network latency with high security and data privacy levels, compared to the existing data sharing models.

Our work illustrates an empirical case study on the analysis of real COVID-19 patients and shows the importance of the proposed intelligent framework to limit the current outbreaks for COVID-19. New types of patient health records aim to help physicians shift from a medical practice, often based on their personal experience, towards one of evidence based medicine, thus improving the communication among patients and care providers and increasing the availability of personal medical information. These new records, allowing patients and care providers to share medical data and clinical information, and access them whenever they need, can be considered enabling Ambient Assisted Living technologies. Furthermore, new personal disease monitoring tools support specialists in their tasks, as an example allowing acquisition, transmission and analysis of medical images. The growing interest around these new technologies poses serious questions regarding data integrity and transaction security. The huge amount of sensitive data stored in these new records surely attracts the interest of malicious hackers, therefore it is necessary to guarantee the integrity and the maximum security of servers and transactions.
