Objectives Varied work-related musculoskeletal disorders (WMSDs) claim rates were found between companies even when they were in the same sectors with similar sizes. This study aimed to (i) identify common risk factors for back, shoulder, hand/wrist, and knee WMSDs among manufacturing jobs, and (ii) characterize the biomechanical exposures in jobs and work organizational practices between high and low WMSD claim rate companies so that more focused, industry-specific intervention strategies may be developed. Methods Using historical workers' compensation data, manufacturing companies were divided into two paired groups (low and high in the lower 25%ile and higher 75%ile, respectively). On-site job evaluations were conducted in 16 companies to determine job biomechanical risk levels. Management and workers' representatives in 32 paired companies were interviewed to identify possible differences between management strategies and management/worker relationships. A total of 39 injured workers were also interviewed to gather information of self-reported injury causes and suggested preventive measures. Results Analyses of 432 job evaluations showed that more jobs had higher risk levels of prolonged standing and heavy lifting in the high back WMSD claim rate companies than the low claim rate ones. No high biomechanical risk factors were found to be associated with jobs in high shoulder claim rate companies. High repetition, pinch force, and Strain Index were associated with high hand/wrist WMSD claim rate companies. High work pace and job stress were common among high knee WMSD claim rate companies. There were no statistically significant differences for the organizational factors between high and low WMSD claim rate companies. Heavy lifting, fast work pace, high hand/wrist repetition, high hand force, and awkward shoulder postures were identified as major contributing factors by the injured workers. Conclusions High WMSD claim rate companies appeared to have more high biomechanical exposure jobs than low WMSD claim rate companies. Available job evaluation methods for the low back and hand/wrists are satisfactory in quantifying job risk levels in the manufacturing industry. Objective: A method for automatically classifying lifting postures from simple features in video recordings was developed and tested. We explored if an "elastic" rectangular bounding box, drawn tightly around the subject, can be used for classifying standing, stooping, and squatting at the lift origin and destination.Background: Current marker-less video tracking methods depend on a priori skeletal human models, which are prone to error from poor illumination, obstructions, and difficulty placing cameras in the field. Robust computer vision algorithms based on spatiotemporal features were previously applied for evaluating repetitive motion tasks, exertion frequency, and duty cycle.Methods: Mannequin poses were systematically generated using the Michigan 3DSSPP software for a wide range of hand locations and lifting postures. The stature-normalized height and width of a bounding box were measured in the sagittal plane and when rotated horizontally by 30°. After randomly ordering the data, a classification and regression tree algorithm was trained to classify the lifting postures.Results: The resulting tree had four levels and four splits, misclassifying 0.36% training-set cases. The algorithm was tested using 30 video clips of industrial lifting tasks, misclassifying 3.33% test-set cases. The sensitivity and specificity, respectively, were 100.0% and 100.0% for squatting, 90.0% and 100.0% for stooping, and 100.0% and 95.0% for standing.Conclusions: The tree classification algorithm is capable of classifying lifting postures based only on dimensions of bounding boxes.Applications: It is anticipated that this practical algorithm can be implemented on handheld devices such as a smartphone, making it readily accessible to practitioners. Optimization combined with a musculoskeletal shoulder model has been used to estimate mechanical loading of musculoskeletal elements around the shoulder. Traditionally, the objective function is to minimize the summation of the total activities of the muscles with forces, moments, and stability constraints. Such an objective function, however, tends to neglect the antagonist muscle co-contraction. In this study, an objective function including an entropy term is proposed to address muscle co-contractions. A musculoskeletal shoulder model is developed to apply the proposed objective function. To find the optimal weight for the entropy term, an experiment was conducted. In the experiment, participants generated various 3-D shoulder moments in six shoulder postures. The surface EMG of 8 shoulder muscles was measured and compared with the predicted muscle activities based on the proposed objective function using Bhattacharyya distance and concordance ratio under different weight of the entropy term. The results show that a small weight of the entropy term can improve the predictability of the model in terms of muscle activities. Such a result suggests that the concept of entropy could be helpful for further understanding the mechanism of muscle co-contractions as well as developing a shoulder biomechanical model with greater validity. Abstract: The existence of multiple local coordinate systems (LCSs) for the scapula makes it difficult to compare the kinematics of the scapula across various studies and reports. This study aimed to build transformation matrices between different LCSs for the scapula and to provide the coordinates of previously measured muscles and ligaments around the scapula with respect to the International Society of Biomechanics (ISB) recommended LCS. The bony landmarks necessary for building various local coordinate systems were digitized on 13 CT scanned scapulae. The LCSs were built based on the digitized bony landmarks and then used for calculating the transformation equations. The approximate coordinates of 28 muscles and ligaments of the scapula were expressed with respect to the ISB-recommended LCS using the derived transformation equations. The results of this study may be used for the comparison of scapula kinematics data with respect to various LCSs and for building a scapula biomechanical model with respect to ISB-recommended LCS. Abstract: Holzbaur et al. (2005) proposed a comprehensive 3-D biomechanical upper extremity model. Since then, this model has been adopted by many other studies for kinetic and kinematic analysis of the shoulder joint. Because of the 3-D anatomical structure, three angles are necessary to define or describe shoulder kinematics. In the Holzbaur et al. model, the three angles are shoulder elevation, elevation angle, and shoulder rotation. The computational implementation of the elevation angle degree of freedom is considered in a different way than described in the recommendation of the International Society of Biomechanics (ISB). This paper presents an analysis of the transformation between the coordinates of the shoulder kinematic defined in the Holzbaur et al. upper extremity model and those defined by the ISB. The results of this study could be used for comparing the coordinates between the different descriptions of the shoulder kinematics. [Copyright &y& Elsevier]

Abstract: Hand–handle interface is seldom considered in contemporary upper limb biomechanical analyses of pushing and pulling strength. A laboratory study was designed to examine if handle rotation in the frontal plane (0°-horizontal, 45°, and 90°-vertical), anterior tilt (0°-parallel to the frontal plane, and 15°), and distance between two handles (31 and 48.6cm) affect pushing strength and subjective rating of handle preference. A special testing station was constructed to elicit upper limb push exertions that involved minimal contribution of the torso and legs. Within the station, four load cells were used to measure the horizontal (forward pushing) and vertical components of the pushing forces. Thirty-one participants performed seated bi-manual pushing strength tests. Comparing to the reference handle configuration (horizontal, straight, and a 31-cm between-handle distance), the 45°-rotated and tilted handles with a 31-cm between-handle distance allowed 6.7% more pushing output, while the horizontal and tilted handles with a 31-cm between-handle distance resulted in 2.8% less. Subjective preference was correlated with normalized pushing strength (r =0.89). Tilted handles, at 45°-rotated and vertical positions received highest subjective ratings of preference among all handle configurations. Men exerted greater pushing strength with the 48.6-cm handle distance while women’s capacity was greatest with the 31-cm distance. The results demonstrated that handle rotation and tilt angles affected pushing strength and should be taken into consideration when evaluating or designing pushing tasks. [Copyright &y& Elsevier]

Repetitive use of hand-held power tools is associated with work-related upper extremity musculoskeletal disorders. Using a pneumatic nutrunner, 21 men completed twelve 360 repetitive fastener-driving sessions on three joints (hard, soft and control) at slow and fast pace, and two different work:rest patterns. Handgrip force and perceived exertions were collected throughout each session. For the control joint, the mean grip force exerted was 39.6% of maximum voluntary exertion (MVE) whereas during hard and soft joint sessions it was 48.9% MVE and 56.9% MVE, respectively. Throughout each session, the grip force decreased, more while operating soft and hard joints as compared with the control joint (regression slope: −0.022 and −0.023, compared with −0.007 N/drive, respectively), suggesting considerable upper extremity muscular effort by participants during torque buildup. Fast work pace resulted in higher average grip forces by participants but a greater decrease in the force as the session progressed. Providing rest breaks reduced perceived exertions. The findings gain additional knowledge for assembly task design to possibly reduce the hand/arm injury risks for the operator. A hand-handle interface force and torque measurement system is introduced to fill the void acknowledged in the international standard ISO 6544, which governs pneumatic, assembly tool reaction torque and force measurement. This system consists of an instrumented handle with a sensor capable of measuring grip force and reaction hand moment when threaded, fastener-driving tools are used by operators. The handle is rigidly affixed to the tool in parallel to the original tool handle allowing normal fastener-driving operations with minimal interference. Demonstration of this proposed system was made with tools of three different shapes: pistol grip, right angle, and in-line. During tool torque buildup, the proposed system measured operators exerting greater grip force on the soft joint than on the hard joint. The system also demonstrated that the soft joint demanded greater hand moment impulse than the hard joint. The results demonstrate that the measurement system can provide supplemental data useful in exposure assessment with power hand tools as proposed in ISO 6544. [ABSTRACT FROM AUTHOR]

The human operator is modelled as a single-degree-of-freedom dynamic mechanical system for predicting the response to impulsive torque reaction forces produced by rotating spindle power hand tools such as nutrunners or screwdrivers. The model uses mass, spring and damping elements to represent the standing operator supporting the tool in the hand. It was hypothesized that these mechanical elements are affected by work location and vary among individuals. These elements were ascertained by measuring the resulting frequency and amplitude of a freely oscillating defined mechanical system when externally loaded using maximal effort to oppose its motion. Twenty-five subjects (13 female, 12 male) participated in the full factorial experiment that measured the effects of gender, vertical and horizontal work location for various tool shapes (in-line, pistol, right angle), and orientations (horizontal and vertical). The mean operator stiffness decreased from 1721 to 1195 N/m when the horizontal work location increased from 30 to 90 cm in front of the ankles for a pistol-grip handle used on a vertical surface. Males had greater mass moment of inertia of (0.0099 kg m2) than females (0.0072 kg m2) for an in-line handle used on a horizontal surface. Internal validation by independently measuring apparatus torque found that the model satisfactorily explained the measured operator dynamics with an average error of 2.86%. Group variance reflects the range of operator capacities to react against power hand tool generated forces for the sample group and therefore it may also be useful for understanding the range of capacities among a group of operators performing similar tasks. The study considers the dynamic nature of the human power handtool operator as a single degree-of-freedom mechanical torsional system. The hand and arm are, therefore, represented as a single mass, spring and damper. The values of these mechanical elements are dependent on the posture used and operator. The apparatus used to quantify these elements measured the free vibration frequency and amplitude decay of a known system due to the external loading of the hand and arm. Twenty-five subjects participated in the investigation. A full factorial experiment tested the effects on the three passive elements in the model when operators exerted maximum effort for gender, horizontal distance (30, 60, 90 cm), and vertical distance (55, 93, 142 190 cm) from the ankles to the handle. The results show that the spring element stiffness and mass moment of inertia changed by 20.6 and 44.5% respectively with vertical location (p [sup <] 0.01), and 23.6 and 41.2% respectively with horizontal location (p [sup <] 0.01). Mass moment of inertia and viscous damping for males were 31.1 and 38.5% respectively greater than for females (p [sup <] 0.01). Tool handle displacement and hand force during torque buildup can, therefore, be predicted based on this model for different tool and workplace parameters. The biomechanical model was validated by recalling five subjects and having them operate a power handtool for varying horizontal distances (30, 60, 90 cm), vertical distances (55, 93, 142 cm), and two torque build-up times (70, 200 ms). Tool reaction displacement was measured using a 3D-motion analysis system. The predictions were closely correlated with these measurements (R = 0.88), although the model underpredicted the response by 27%. The movements of the humerus, the clavicle, and the scapula are not completely independent. The coupled pattern of movement of these bones is called the shoulder rhythm. To date, multiple studies have focused on providing regression-based 3-D shoulder rhythms, in which the orientations of the clavicle and the scapula are estimated by the orientation of the humerus. In this study, six existing regression-based shoulder rhythms were evaluated by an independent dataset in terms of their predictability. The datasets include the measured orientations of the humerus, the clavicle, and the scapula of 14 participants over 118 different upper arm postures. The predicted orientations of the clavicle and the scapula were derived from applying those regression-based shoulder rhythms to the humerus orientation. The results indicated that none of those regression-based shoulder rhythms provides consistently more accurate results than the others. For all the joint angles and all the shoulder rhythms, the RMSE are all greater than 5°. Among those shoulder rhythms, the scapula lateral/medial rotation has the strongest correlation between the predicted and the measured angles, while the other thoracoclavicular and thoracoscapular bone orientation angles only showed a weak to moderate correlation. Since the regression-based shoulder rhythm has been adopted for shoulder biomechanical models to estimate shoulder muscle activities and structure loads, there needs to be further investigation on how the predicted error from the shoulder rhythm affects the output of the biomechanical model.
