Anemia and intestinal helminth infections are overlapping health problems in developing countries. This study examined the determinants of intestinal helminth infection and anemia in a human population in Harbu Town, northeastern Ethiopia. A total of 484 individuals provided stool and blood samples as well as information about their sociodemographic characteristics and living practices in a community-based cross-sectional survey conducted between May and June, 2013. Stool specimens were examined for intestinal helminth infections using the Kato–Katz method. While a HemoCue machine was used to measure blood hemoglobin levels, a CareStartTM malaria Pf/Pv combo test was used to test the blood specimens for Plasmodium infection. Out of 484 individuals examined, 15.5% were anemic and 32.0% were infected with intestinal helminths. Plasmodium infection was not detected in any of the study participants. Schistosoma mansoni infection was most common (26.7%) followed by Hymenolepis nana (4.1%). The prevalence of S. mansoni and H. nana infection was greater among school-age children than in pre-school-age children and adults. The prevalence of helminth infection decreased with an increase in monthly income (P = 0.048) and varied among different occupations (P = 0.023). The odds of anemia increased with an increase in the age of individuals (adjusted odds ratio = 1.03, 95% CI = 1.01, 1.06). Hookworm infection was associated with anemia (P = 0.029). In conclusion, intestinal helminth infections and anemia were public health problems among the community of Harbu Town. Increasing age and hookworm infection may increase susceptibility to anemia. Controlling helminth infection may help to reduce the burden of anemia in Harbu Town, Ethiopia.

Plasmodium knowlesi occurs throughout Southeast Asia, and is the most common cause of human malaria in Malaysia. Severe disease in humans is characterised by high parasite biomass, reduced red blood cell deformability, endothelial activation and microvascular dysfunction. However, the roles of intravascular haemolysis and nitric oxide (NO)-dependent endothelial dysfunction, important features of severe falciparum malaria, have not been evaluated, nor their role in acute kidney injury (AKI). In hospitalised Malaysian adults with severe (n = 48) and non-severe (n = 154) knowlesi malaria, and in healthy controls (n = 50), we measured cell-free haemoglobin (CFHb) and assessed associations with the endothelial Weibel–Palade body (WPB) constituents, angiopoietin-2 and osteoprotegerin, endothelial and microvascular function, and other markers of disease severity. CFHb was increased in knowlesi malaria in proportion to disease severity, and to a greater extent than previously reported in severe falciparum malaria patients from the same study cohort. In knowlesi malaria, CFHb was associated with parasitaemia, and independently associated with angiopoietin-2 and osteoprotegerin. As with angiopoietin-2, osteoprotegerin was increased in proportion to disease severity, and independently associated with severity markers including creatinine, lactate, interleukin-6, endothelial cell adhesion molecules ICAM-1 and E-selectin, and impaired microvascular reactivity. Osteoprotegerin was also independently associated with NO-dependent endothelial dysfunction. AKI was found in 88% of those with severe knowlesi malaria. Angiopoietin-2 and osteoprotegerin were both independent risk factors for acute kidney injury. Our findings suggest that haemolysis-mediated endothelial activation and release of WPB constituents is likely a key contributor to end-organ dysfunction, including AKI, in severe knowlesi malaria.

Background Soil-transmitted helminth (STH) infections are still prevalent in Indonesia, with roughly one-third of infected population being preschool-age children (PSC), which are generally at higher risk of morbidity such as malnutrition and anemia. This study aimed to investigate the association of STH infections with nutritional status and anemia among PSC in Nangapanda subdistrict, Ende, East Nusa Tenggara. Methods A cross-sectional survey involving PSC ranging from 12 to 59 months old from Nangapanda subdistrict, Ende district, East Nusa Tenggara was performed. Socio-demographic, breastfeeding, and complementary feeding information was obtained from structured questionnaires, while nutritional and anemia status was determined from anthropometry and hemoglobin measurements, respectively. Anthropometric z-scores were calculated based on the World Health Organization 2006 standards and stool samples were examined using Kato-Katz method. Results A total of 393 PSC randomly selected from 22 villages were examined. The prevalence of underweight, stunting, wasting, and anemia were 33.1%, 40.2%, 17.1%, and 60.3%, respectively. STH infection, predominated by Ascaris lumbricoides, was found in 160 (58.8%) PSC. Single STH infection, but not multiple infection, was independently associated with a lower risk of anemia (odds ratio [OR] 0.320, 95% confidence interval [CI]: 0.126–0.809, p = 0.016). Similar association with anemia was also found on mild STH infection (OR 0.318 [95% CI: 0.114–0.887], p = 0.029). On the other hand, younger children were found to have a higher risk of anemia and stunting. None of the examined variables were independently associated with underweight and wasting. Conclusion STH infection as well as anemia and malnutrition were prevalent in this region. However in this study, current STH infections seemed to have minimal negative impact on children’s nutritional status., Author summary Soil-transmitted helminth (STH) infection, which frequently coexist with anemia and malnutrition, remains a major disease burden to preschool-age children (PSC) as they may bring negative impacts on children’s growth and development. In this cross-sectional study, we investigated the prevalence of STH infection in Nangapanda subdistrict in Indonesia and explored the association between STH infection with poor nutritional status and anemia among PSC. Poor nutritional status was classified into underweight, stunting, and wasting, while anemia was defined as hemoglobin levels below 11 gr/dL. We revealed that many PSC in this area suffered from STH infection (primarily by Ascaris lumbricoides), malnutrition, and anemia. We also discovered that younger PSC were more vulnerable to stunting and anemia, whereas current single and mild STH infection was independently associated with a lower risk of anemia. Although no evidence for negative impact of STH infection was found in this study, the high prevalence of STH infection, malnutrition, and anemia in the study population warrants urgent intervention in order to improve the children’s health and wellbeing. Further research is required to assess the quality and quantity of food intake, as well as to investigate socioeconomic and environmental factors contributing to the poor nutritional status.

Anemia is a worldwide public health concern especially in preschool children in developing countries and iron deficiency (ID) is generally assumed to cause at least 50% of the cases. However, data on this contribution are scarce. To close this gap, we determined in 2013 the contribution of ID in the etiology of anemia and measured others factors associated to noniron deficiency anemia (NIDA) in 900 preschool children randomly selected during a two-stage cluster nutritional survey in the Miti-Murhesa health zone, in eastern Democratic Republic of the Congo. In these children, we collected sociodemographic, clinical, and biological parameters and determined the nutritional status according to the World Health Organization 2006 standards. Anemia was defined as altitude-adjusted hemoglobin < 110 g/L and ID was defined as serum ferritin < 12 μg/L or < 30 μg/L in the absence or presence of inflammation, respectively. Median (interquartile range) age was 29.4 (12–45) months. The prevalence of anemia was 46.6% (391/838) among whom only 16.5% (62/377) had ID. Among children without signs of inflammation, only 4.4% (11/251) met the ferritin-based (unadjusted) definition of ID. Logistic regression analysis identified ID, history of fever during the last 2 weeks and mid-upper arm circumference < 125 mm as the only independent factors associated to anemia. In conclusion, anemia is a severe public health problem in the Miti-Murhesa health zone, but NIDA is mostly predominant and needs to be further studied. Control of infections and prevention of acute undernutrition (wasting) are some of appropriate interventions to reduce the burden anemia in this region.

Background Because of the different genetic backgrounds, living environments and economic conditions, the epidemiologic, clinical characteristics and risk factors for coronary artery abnormalities (CAAs) in the Chinese population may differ among different parts of China. Methods We did a retrospective study to explore the clinical characteristics and risk factors for CAAs in east China between 2006 and 2014. Results There were 1016 patients during the study period. Of the 1004 patients who completed echocardiographic studies, 23.9% had CAAs. Sex, serum albumin, erythrocyte sedimentation rate (ESR),Mycoplasma pneumoniae(MP) infection, intravenous immunoglobulin (IVIG) started after the 10th day of illness and IVIG non-responders were independent risk factors associated with CAA. Conclusions East China has a lower incidence of CAAs compared with southwest and northeast China, while similar to north China. Male gender, serum albumin, ESR, MP infection, IVIG started after the 10th day of illness and IVIG non-responders were predictive of CAA.

Studies of the prevalence and incidence of malaria were conducted in children < 10 years old living in 10 rural villages on the island of Espiritu Santo, Vanuatu, south-west Pacific. Malaria prevalence remained stable at 30% throughout the year but the relative contributions of the 2 major species were highly dependent on season. Plasmodium falciparum predominated in the long wet season (November-May) and P. vivax in the dry season (June-October). Case definitions for malaria, derived using a multiple logistic regression method, showed that parasite densities associated with clinical disease were low; case definitions for P. falciparum (> 1000 parasites/microL in children > 1 year old and > 500 microL in infants) and P. vivax (> 500 parasites/microL at all ages) were both associated with a specificity and sensitivity of > 90%. Like prevalence data, malaria morbidity was highly seasonal; 80% of clinical P. falciparum infections occurred in the wet season and 66% of clinical P. vivax in the dry season. Mixed infections were rare. Malaria was important cause of morbidity with children < 5 years old experiencing 1.3-3.0 episodes of clinical malaria per year and 23% of fevers being attributable to malaria in this age group. Children aged 5-9 years continued to suffer one episode of clinical malaria per year. The peak incidence of P. vivax malaria occurred earlier in life than the peak incidence of P. falciparum malaria. The possible interactions between these 2 parasite species are discussed.

Parvovirus B19 (B19) is a common childhood infection that has recently been found to be associated with severe anemia in Papua New Guinean children. Population surveys were performed in 15 villages in Maprik district, East Sepik Province, Papua New Guinea in 2005. Plasma samples collected from children less than 10 years of age were tested for IgM and IgG antibodies to B19 by enzyme immunoassay. The prevalence of IgG antibody to B19 was 53.8% and ranged from 20% in those less than one year of age to 85.5% in those nine years of age. Considerable variation in IgG prevalence was observed between study areas, indicating complex patterns of transmission. Prevalence of IgM antibody to B19 was 1.5%. This study confirms that B19 infection is common among children in this tropical area. With 19.5% of children one year of age showing evidence of previous infection, any preventive measures should be targeted at the very young.

Objectives: To study the incidence and risk factors for failure of treatment with antiretroviral therapy among adults in the national treatment program in India, and to estimate the possible number of persons living with human immunodeficiency virus (HIV) who will need a second-line treatment regimen in the next 3 and 3.5 years. Design and setting: Data of a cohort of HIV-positive adult patients, who were enrol led in the government-sponsored antiretroviral therapy program, were obtained from the electronic medical record system of the largest HIV care center in India and subjected to analysis. Main outcomes: Treatment failure defined by the World Health Organization criteria, assessed immunologically on the basis of CD4 T cell count, with a minimum period of 12 months of follow-up and with a minimum of two CD4 T cell follow-up measures. Results: The cumulative incidence of treatment failure in the 1370 adult patients included in the study was 3.9% (95% confidence interval [Cl] 2.9 to 4.9). Men had a 3.5 (1.6 to 7.4) times significantly greater risk of treatment failure. Patients who had negative changes in absolute lymphocyte count, hemoglobin concentration and body weight had 3.1 (1.6 to 6.2), 3.2 (1.6 to 6.2), and 3.5 (1.9 to 6.4) times significantly greater risk of treatment failure. In India, after 2007, by 2, 3, and 3.5 years, respectively, an estimated 16000, 35000, and 51 000 patients receiving antiretroviral therapy are likely to require second-line treatment. Conclusion: Monitoring of hemoglobin concentration, absolute lymphocyte count, and body weight during follow-up emerged as inexpensive predictors of treatment failure in a resource-poor setting. A significant number of patients will need second-line therapy as a result of failure of their first-line antiretroviral therapy regimen in 3 and 3.5 years in India, and therefore the development of an appropriate policy for second-line drugs is urgently needed.

Background Assessments of the epidemiology of malaria over time are needed to understand changes in transmission and guide control and elimination strategies. Methods A longitudinal population study was established in 1985 in Nyamisati village in the Rufiji River Delta, Tanzania. A physician and research team lived in the village 1984–2000. Parasite prevalence by microscopy and two PCR methods, spleen rates and haemoglobin levels were measured in repeated cross-sectional surveys between 1985 and 2010. Passive surveillance of malaria cases was maintained until end 1999. Bed nets were distributed after the surveys 1993, 1999 and 2010. Results In 1985, overall parasite prevalence by microscopy was 70% (90% in children ages two to nine years). The prevalence decreased gradually by microscopy (38.9% 1994, 26.7% 1999) and msp2-PCR (58.7% 1994, 44.8% 1999), whereas real-time PCR prevalence remained higher throughout the 1990s (69.4% 1994, 64.8% 1999). In 2010, parasite prevalence was 17.8% by real-time PCR and 16.3% by msp2-PCR, and estimated to 4.8% by microscopy. Spleen rates in children ages two to nine years decreased earlier than parasite prevalence, from >75 to 42% in the 1980s, to nil during the 1990s. The prevalence of severe and moderate anaemia decreased from 41.1 to 13.1%. No deaths at the time of acute malaria were recorded when the research team lived in the village. Conclusions A marked decline in malaria transmission was observed over 25 years. The decrease was detected after the arrival of the research team and continued gradually both before and after distribution of bed nets. Spleen rates and microscopy identified early changes when transmission was still intense, whereas real-time PCR was a more sensitive metric when transmission was reduced. The study provides historical data on malaria within a closely monitored rural village and contributes to the understanding of changing epidemiology in sub-Saharan Africa. Background The epidemiology of malaria and anaemia is characterized by small-scale spatial and temporal heterogeneity, which might be influenced by human activities, such as mining and related disturbance of the environment. Private sector involvement holds promise to foster public health, including the prevention and control of malaria and anaemia. Here, results from a cross-sectional epidemiological survey, conducted in communities that might potentially be affected by the Bonikro Gold Mine (BGM) in Côte d’Ivoire, are reported. Methods In December 2012, a cross-sectional survey was carried out in seven communities situated within a 20-km radius of the BGM in central Côte d’Ivoire. Capillary blood samples were obtained from children aged six to 59 months. Samples were subjected to a rapid diagnostic test (RDT) for Plasmodium falciparum detection, whilst haemoglobin (Hb) was measured to determine anaemia. Additionally, mothers were interviewed with a malaria-related knowledge, attitudes and practices questionnaire. Results A total of 339 children and 235 mothers participated in the surveys. This study investigated the epidemiology of uncomplicated falciparum malaria in an area of unstable and seasonal transmission in eastern Sudan. About 90% of malaria morbidity in this region occurs in the months of September to November, and very few malaria cases occur during the intensely arid Sudanese dry season and during years of drought. The malaria situation in the study site, the village of Daraweesh, was analysed during 3 consecutive malaria seasons in 1993–1995 during which the 457 inhabitants suffered a total of 436 episodes of falciparum malaria. Using an Andersen-Gill proportional hazard model for recurrent events stratified by family, we have calculated the relative hazard for clinical malaria episodes by age, sex, haemoglobin genotype, blood type and infection in the previous season. The malaria risk was significantly lower in individuals aged 20–88 years than in the 5–19 years age-group. The relative protection due to adulthood varied between seasons (relative risk, RR, 0·34 to 0·67). Serological data were not consistent with the hypothesis that the age difference in incidence was due to differences in exposure. During the 1993 season the malaria incidence in males was lower than in females (RR = 0·75), during the 1994 season the incidences were comparable, whereas males had an increased risk of malaria in 1995 (RR = 1·87). The relative risk in individuals carrying the haemoglobin AS genotype compared to homozygous AA individuals was 0·57.

Intestinal nematode infections are recognized as a major public health problem, and helminth control is currently being directed towards school-aged children who are known to harbour the heaviest infections and are most likely to suffer from associated morbidity. However, few data are available for the epidemiology of intestinal nematodes in pre-school children in Africa, and the contribution of hookworm infection to the aetiology and severity of anaemia among pre-school children remains poorly understood. This paper investigates the epidemiology of parasitic infections in 460 pre-school children who were part of a larger case-control study of severe malaria in Kilifi on the Kenyan coast. Almost one-third (28.7%) were infected with hookworm, 20.2% with Ascaris lumbricoides and 15.0% with Trichuris trichiura. Infection prevalence of each species rose with age, and the prevalence of heavy infection with hookworm and mean intensity of hookworm were markedly age-dependent. One-third (34.3%) of children had malaria. Overall, 76.3% of children were anaemic (haemoglobin < 110 g/L), with the prevalence decreasing with age. Anaemia was significantly worst in children with heavy hookworm infection (> 200 eggs per gram). This relationship held for all ages, both sexes, and was independent of socioeconomic factors. The application of attributable morbidity methods confirmed the contribution of hookworm infection to anaemia.

Background Intermittent iron-folic acid supplementation and regular de-worming are effective initiatives to reduce anemia, iron deficiency, iron deficiency anemia, and soil transmitted helminth infections in women of reproductive age. However, few studies have assessed the long-term effectiveness of population-based interventions delivered in resource-constrained settings. Methodology/Principal Findings The objectives were to evaluate the impact of weekly iron-folic acid supplementation and de-worming on mean hemoglobin and the prevalence of anaemia, iron deficiency, and soil transmitted helminth infection in a rural population of women in northern Vietnam and to identify predictive factors for hematological outcomes. A prospective cohort design was used to evaluate a population-based supplementation and deworming program over 54 months. The 389 participants were enrolled just prior to commencement of the intervention. After 54 months 76% (95% CI [68%, 84%]) were taking the iron-folic acid supplement and 95% (95% CI [93%, 98%]) had taken the most recently distributed deworming treatment. Mean hemoglobin rose from 122 g/L (95% CI [120, 124]) to 131 g/L (95% CI [128, 134]) and anemia prevalence fell from 38% (95% CI [31%, 45%]) to 18% (95% CI [12%, 23%]); however, results differed significantly between ethnic groups. Iron deficiency fell from 23% (95% CI [17%, 29%]) to 8% (95% CI [4%, 12%]), while the prevalence of iron deficiency anemia was reduced to 4% (95% CI [1%, 7%]). The prevalence of hookworm infection was reduced from 76% (95% CI [68%, 83%]) to 11% (95% CI [5%, 18%]). The level of moderate or heavy infestation of any soil-transmitted helminth was reduced to less than 1%. Conclusions/Significance Population-based interventions can efficiently and effectively reduce anemia and practically eliminate iron deficiency anemia and moderate to heavy soil transmitted helminth infections, maintaining them below the level of public health concern., Author Summary Nutritional deficiencies are common in women in rural and disadvantaged regions, and, in particular, iron and folate deficiencies may impact their health and that of their offspring. Weekly iron-folic acid supplementation and regular deworming help to prevent these deficiencies, yet many women in developing countries do not have access to these interventions. Using the local health system, we implemented a weekly iron-folic acid supplementation program with regular deworming for approximately 250,000 women in a rural mountainous province of Viet Nam. We present the results of a survey of women's iron and worm infection status after the program has been running for 54 months. Overall, anemia prevalence fell from 38% to 18%, iron deficiency fell from 23% to 8% and the prevalence of iron deficiency anemia was reduced to 4%. The level of moderate or heavy infestation of any soil-transmitted worm infection was reduced to less than 1%. These results show that a health system-based program can reduce and maintain the prevalence of these diseases in the community at a level where they do not constitute a public health risk.

Summary Background Short-term semi-immunity to malaria in sub-Saharan African migrants who have recently arrived in non-endemic countries results in less severe imported malaria. Our aim was to investigate the factors associated with imported malaria that would favour the hypothesis of a ‘long-term’ semi-immunity to malaria in adult travellers of sub-Saharan origin living in France and visiting family or relatives in their country of origin (VFR group). Method The epidemiological, clinical and biological characteristics of imported Plasmodium falciparum malaria in VFR were compared with those of travellers of European origin (TEO). Newly arrived African migrants and European expatriates were excluded. Results This retrospective study included 106 adult VFR (30%) and 240 adult TEO (70%) with imported P. falciparum malaria treated at the University Hospital Center of Bordeaux between 2000 and 2007. The main regions visited were West Africa (58%) and Central Africa (34%). P. falciparum was associated with severe malaria in 8% of patients (VFR 3% vs. TEO 11%), of which two TEO died. In univariate analysis, the factors associated with P. falciparum malaria in VFR vs. TEO were: female sex, younger age, less frequent use of mosquito nets, poor compliance with chemoprophylaxis, less severe malaria without death, less severe thrombocytopenia and a tendency towards a lower level of parasitaemia and higher haemoglobinaemia. In multivariate analysis, the only factor to be independently associated with P. falciparum malaria in VFR compared to TEO was less frequent severe malaria. Conclusions Our results give support to the hypothesis of ‘long-term’ semi-immunity to malaria in VFR living in France.

Background Accurate information about the burden of malaria infection at the district or provincial level is required both to plan and assess local malaria control efforts. Although many studies of malaria epidemiology, immunology, and drug resistance have been conducted at many sites in Indonesia, there is little published literature describing malaria prevalence at the district, provincial, or national level. Methods Two stage cluster sampling malaria prevalence surveys were conducted in the wet season and dry season across West Sumba, Nusa Tenggara Province, Indonesia. Results Eight thousand eight hundred seventy samples were collected from 45 sub-villages in the surveys. The overall prevalence of malaria infection in the West Sumba District was 6.83% (95% CI, 4.40, 9.26) in the wet season and 4.95% (95% CI, 3.01, 6.90) in the dry. In the wet season Plasmodium falciparum accounted for 70% of infections; in the dry season P. falciparum and Plasmodium vivax were present in equal proportion. Malaria prevalence varied substantially across the district; prevalences in individual sub-villages ranged from 0–34%. The greatest malaria prevalence was in children and teenagers; the geometric mean parasitaemia in infected individuals decreased with age. Malaria infection was clearly associated with decreased haemoglobin concentration in children under 10 years of age, but it is not clear whether this association is causal. Conclusion Malaria is hypoendemic to mesoendemic in West Sumba, Indonesia. The age distribution of parasitaemia suggests that transmission has been stable enough to induce some clinical immunity. These prevalence data will aid the design of future malaria control efforts and will serve as a baseline against which the results of current and future control efforts can be assessed.

Background Site preparation is a pre-requesite in conducting malaria vaccines trials. This study was conducted in 12 villages to determine malariometric indices and associated risk factors, during long and short rainy seasons, in an area with varying malaria transmission intensities in Korogwe district, Tanzania. Four villages had passive case detection (PCD) of fever system using village health workers. Methods Four malariometric cross-sectional surveys were conducted between November 2005 and May 2007 among individuals aged 0–19 years, living in lowland urban, lowland rural and highland strata. A total of 10,766 blood samples were collected for malaria parasite diagnosis and anaemia estimation. Blood smears were stained with Giemsa while haemoglobin level was measured by HaemoCue. Socio-economic data were collected between Jan-Apr 2006. Results Adjusting for the effect of age, the risk of Plasmodium falciparum parasitaemia was significantly lower in both lowland urban, (OR = 0.26; 95%CI: 0.23–0.29, p < 0.001) and highlands, (OR = 0.21; 95%CI: 0.17–0.25, p < 0.001) compared to lowland rural. Individuals aged 6–9 years in the lowland rural and 4–19 years in both lowland urban and highlands had the highest parasite prevalence, whilst children below five years in all strata had the highest parasite density. Prevalence of splenomegaly and gametocyte were also lower in both lowland urban and highlands than in lowland rural. Anaemia (Hb Conclusion Higher SES and use of bed nets in the lowland urban could be the important factors for low malaria infections in this stratum. Results obtained here were used together with those from PCD and DSS in selecting a village for Phase 1b MSP3 vaccine trial, which was conducted in the study area in year 2008.

SUMMARYWe conducted a 4-year retrospective cohort study to better define the risk of haemolytic anaemia and haemolytic uraemic syndrome (HUS) in children following sporadic gastrointestinal infection with the O 157.H7 serotype ofEscherichia coli. Of the 72 children infected with this organism, 9 (12·5%) developed haemolytic anaemia, 6 of whom had HUS. No child in a cohort of 72 age-matched controls withCampylobacter jejunigastroenteritis developed haemolytic anaemia (P= 0·003). Females had a significantly greater risk of developing haemolytic anaemia afterE. coliO 157.H7 infection than did males (8/29 femalesv.1/43 males;P= 0·003). In a logistic regression model, female gender emerged as the only statistically significant risk factor for haemolytic anaemia (odds ratio 3·85; 95% confidence interval 1·24–12). These results are consistent with recent reports of a moderate increase in the risk of HUS for females.

Background Across tropical Africa the bulk of malaria-related morbidity and mortality is particularly high during childhood. Classical malariometric surveys have relied on assessing malaria infection prevalence. The last comprehensive evaluation of the malaria situation in Mozambique was carried out during the 1950s. This study aims to characterize the malaria transmission intensities and to estimate the disease burden that may help guide control programme. Methods Between February 2002 and April 2003, a house-to-house survey, was carried out in 24 districts randomly selected. A total of 8,816 children aged below 10 years old were enrolled. Finger prick and blood collection were performed to prepare thick and thin films for malaria parasite species identification, density and haemoglobin concentration. Axillary temperature was also measured. Prevalence of infection, parasite density and anaemia were estimated for age groups category in each region/stratum. Comparisons between proportions were made using Chi-square test or Fisher exact. Relationship between age groups, region/stratum and parasite prevalence, density was determined using linear regression. All survey mean estimations were adjusted for sampling weights, clustering and stratification. Results Malaria parasite prevalence was 58.9% (5.190/8.816), the majority of blood smears 52.4% (4,616/8,816) were due to Plasmodium falciparum and geometric mean parasite density was 1,211 parasites/μl (95% CI, 1,141 – 1.286). Gametocytes prevalence, only for P. falciparum was 5.6% (518/8,816). The burden was highest in the northern regions and in the coastal stratum. Parasite infection and geometric mean parasite density peaked during the second year of life and thereafter decreased with increasing age. Mean haemoglobin concentrations was 9.9 g/dl (95% CI 9.5 – 10.2). Anaemia prevalence was 69.8% (6.257/8.816) and among anaemic children 11.5% (743/6.257) were severely anaemic. Anaemia rose dramatically during the first year of life to peak among children in the 12 – 23 months age group. Highest levels of anaemia were recorded in both northern and central-northern regions 77.9% and 79.4% respectively. Conclusion This survey confirms that malaria especially that caused by P. falciparum, remains endemic throughout the country. The burden of malaria disease and anaemia-related malaria during childhood constitute a major public health problem and warrant integrated and collaborative interventions towards its control.

It has been recently reported that the standard threefold conversion from haematocrit to haemoglobin underestimates the prevalence of anaemia and low levels of haemoglobin in children living in areas endemic for Plasmodium falciparum malaria. The data presented herein describes the experience in a malaria-endemic zone in northeastern Venezuela (state of Sucre), where a similar bias between haematocrit and haemoglobin in patients with Plasmodium vivax infection was found. In summary, the relationship between haematocrit and haemoglobin needs to be specifically evaluated according to each particular region or epidemiological setting.

In two complementary epidemiological surveys of villages on the Ugandan shoreline of Lake Victoria, the putative occurrence of intestinal schistosomiasis in the local infants (children aged < 3 years) was investigated. When, during the first survey, 136 mother-and-infant pairs from a total of 12 villages were studied, only 7% of the infants but 45% of the mothers were found to be egg-patent for Schistosoma mansoni infection. The use of dipstick tests for urine-circulating cathodic antigen indicated, however, a much higher prevalence, of approximately 40%, among the infants. In the second survey, urine samples and multiple, not single, stool samples were collected from another 19 mother-and-infant pairs in two of the 12 study villages (Bugoto and Bwondha), and a standardized questionnaire was implemented. The prevalence of egg-patent infection was then found to be markedly higher in the study infants from Bugoto (86%) than in those from Bwondha (25%). A greater level of mother-and-infant water contact, a higher abundance of (infected) Biomphalaria choanomphala, and an unusual lakeshore topology may explain why S. mansoni infection was so much more common in the Bugoto subjects than in the Bwondha. We compared malaria indicators among sympatric groups to study human heterogeneities in the response to Plasmodium falciparum malaria infection. Four cross-sectional surveys and two longitudinal surveys in two sympatric ethnic groups (Dogon and Fulani) in Mali were carried out from 1998 to 2000. Spleen and parasite rates were evaluated during the cross-sectional surveys and disease incidence was assessed during longitudinal surveys. In spite of similar sociocultural factors and entomologic inoculation rates between ethnic groups, the Fulani had a significantly higher spleen enlargement rate, lower parasite rate, and were less affected by the disease than the Dogon group, whose frequency of hemoglobin C was higher than that recorded among the Fulani group. The Fulani group had significantly higher levels of IgG and IgE against crude malaria antigen than the Dogon group, suggesting a role of anti-malaria antibodies in the immune protection seen in this group.

Objective To describe temporal patterns in total lymphocyte count (TLC) and hemoglobin (Hgb) concentration during HIV infection and relate these patterns to changes in other markers and to clinical disease. Design Prospective cohort study. Methods Longitudinal trajectories of total lymphocyte count and hemoglobin from men in the Multicenter AIDS Cohort Study were studied by applying, to each individual, a segmented regression model to capture changes in marker trajectories at an inflection point. The estimated slope of these markers before and after the inflection point were examined to determine whether those with AIDS onset could be distinguished from those remaining free of AIDS through the use of receiver operating characteristic (ROC) curves. Results Prior to the estimated inflection points, TLC and Hgb marker slopes were distributed around zero in all men; afterwards, those who developed AIDS showed rapid declines in both markers. A TLC decline greater than 10% and Hgb decline greater than 2.2% was present in over 77% of HIV-positive men who developed AIDS but only 23% of HIV-positive men who did not. Among those who developed AIDS, rapid declines in TLC and Hgb began as CD4+ cell counts fell from 350 to 200 x 106 cells/l and preceded clinical AIDS by a median of about 1.6 years. Conclusions Both TLC and Hgb showed a period of stability, which was followed by a rapid decline beginning shortly before the onset of AIDS. These results suggest that TLC and Hgb might be useful for monitoring disease progression in resource-limited settings.

The health of 227 children enrolled at primary school was compared with that of 214 non enrolled children living in rural Tanga, Tanzania. No consistent difference was observed with respect to prevalence and intensity of parasitic infection (hookworm, T. trichiura, A. lumbricoides, S. haematobium and P. falciparum). Since enrolled children were as commonly and as heavily infected as non enrolled children, treatment of enrolled children would be effective in reducing transmission throughout the total population. Non enrolled children were more stunted (P=0.0001) and wasted (P=0.0001) than enrolled children and also tended to be more anaemic (P=0.080) showing that poor nutrition is not only associated with delayed enrolment but continues to be associated with non enrolment throughout the school age years. Given that treatment has the greatest impact on the most malnourished children, additional measures to extend treatment to non enrolled children would be justified.

The domestic environment was investigated for the presence of viruses and body fluids that may contain viruses. A range of surfaces in 39 homes (17 visited on 2 occasions) were sampled by swabbing and analysed using cell culture, reverse transcription polymerase chain reaction for enteroviral RNA, haemoglobin as a marker for blood, amylase as an indicator of urine, saliva and sweat, and protein as an indicator of general hygiene. Haemoglobin was found on 1·9% of surfaces sampled and of the positive samples 30% were from articles frequently handled. Amylase (>5 U/l) was found in 29·3% of samples tested. Protein was found in 97·8% of samples tested. Enteroviral RNA, indicating the presence of virus, was detected in 3 out of 448 samples tested; they were from a tap handle, telephone handpiece and a toilet bowl. No viruses were isolated in cell culture, however significant problems were encountered with bacterial and fungal contamination. This work demonstrates that only testing environmental samples for bacteria and ATP may not give a total view of the microbiological problem in the home. A range of test methods is useful to gain a broad view of the problems of hygiene in the home and to allow comparative studies of specific areas such as the kitchen and bathroom.

Six hundred eighty-nine #Plasmodium falciparum$ malaria attacks were observed during a three-year period among 226 habitants of the village of Dielmo, Senegal, an area of high malaria transmission. Malaria attacks were defined as clinical episodes with fever (body temperature is greater than or equal to 38.0° C) or reporting of fever or headache or vomiting, associated with a parasite:leukocyte ratio above an age-dependent pyrogenic threshold identified in this population. The symptom frequencies were tested against age, gender, and parasite density using a random-effect logistic regression model and the study of distinguishable clinical presentations was carried out by multi-correspondence analysis. There was little difference between the severity of symptoms during the initial course of attacks in young children and adults, and this severity was not correlated with the duration of the pathologic episode. It was not possible to distinguish objectively different malaria attack types according to the severity of clinical manifestations. In contrast, the duration of fever, symptoms, and parasite clearance were significantly longer among the youngest children than among the oldest children and adults. These findings suggest that of the two components of protective immunity, anti-parasite immunity and anti-toxic immunity, only the first would play a major role as age increases. They suggest also that the initial clinical presentation of malaria attacks is not predictive of the level of protective immunity. (Résumé d'auteur)

Background Soil-transmitted helminth (STH) infections are endemic in Honduras and efforts are underway to decrease their transmission. However, current evidence is lacking in regards to their prevalence, intensity and their impact on children's health. Objectives To evaluate the prevalence and intensity of STH infections and their association with nutritional status in a sample of Honduran children. Methodology A cross-sectional study was done among school-age children residing in rural communities in Honduras, in 2011. Demographic data was obtained, hemoglobin and protein concentrations were determined in blood samples and STH infections investigated in single-stool samples by Kato-Katz. Anthropometric measurements were taken to calculate height-for-age (HAZ), BMI-for-age (BAZ) and weight-for-age (WAZ) to determine stunting, thinness and underweight, respectively. Results Among 320 children studied (48% girls, aged 7–14 years, mean 9.76±1.4) an overall STH prevalence of 72.5% was found. Children >10 years of age were generally more infected than 7–10 year-olds (p = 0.015). Prevalence was 30%, 67% and 16% for Ascaris, Trichuris and hookworms, respectively. Moderate-to-heavy infections as well as polyparasitism were common among the infected children (36% and 44%, respectively). Sustainable efforts to control STH infections in Honduras are required. Future research providing more insight on the nutritional impact of polyparasitic STH infections in childhood is necessary.

Objective: Iron deficiency (ID) and iron deficiency anaemia (IDA) are global major public health problems, particularly in developing countries. Whilst an association between H. pylori infection and ID/IDA has been proposed in the literature, currently there is no consensus. We studied the effects of H. pylori infection on ID/IDA in a cohort of children undergoing upper gastrointestinal endoscopy for upper abdominal pain in two developing and one developed country. Methods: In total 311 children (mean age 10.763.2 years) from Latin America - Belo Horizonte/Brazil (n = 125), Santiago/ Chile (n = 105) - and London/UK (n = 81), were studied. Gastric and duodenal biopsies were obtained for evaluation of histology and H. pylori status and blood samples for parameters of ID/IDA. Results: The prevalence of H. pylori infection was 27.7% being significantly higher (p,0.001) in Latin America (35%) than in UK (7%). Multiple linear regression models revealed H. pylori infection as a significant predictor of low ferritin and haemoglobin concentrations in children from Latin-America. A negative correlation was observed between MCV (r = 20.26; p = 0.01) and MCH (r = 20.27; p = 0.01) values and the degree of antral chronic inflammation, and between MCH and the degree of corpus chronic (r = 20.29, p = 0.008) and active (r = 20.27, p = 0.002) inflammation. Conclusions: This study demonstrates that H. pylori infection in children influences the serum ferritin and haemoglobin concentrations, markers of early depletion of iron stores and anaemia respectively.

Background The intensity categories, or thresholds, currently used for Trichuris trichiura (ie. epg intensities of 1–999 (light); 1,000–9,999 epg (moderate), and ≥10,000 epg (heavy)) were developed in the 1980s, when there were little epidemiological data available on dose-response relationships. This study was undertaken to determine a threshold for T. trichiura-associated anemia in pregnant women and to describe the implications of this threshold in terms of the need for primary prevention and chemotherapeutic interventions. Methodology/Principal Findings In Iquitos, Peru, 935 pregnant women were tested for T. trichiura infection in their second trimester of pregnancy; were given daily iron supplements throughout their pregnancy; and had their blood hemoglobin levels measured in their third trimester of pregnancy. Women in the highest two T. trichiura intensity quintiles (601–1632 epg and ≥1633 epg) had significantly lower mean hemoglobin concentrations than the lowest quintile (0–24 epg). They also had a statistically significantly higher risk of anemia, with adjusted odds ratios of 1.67 (95% CI: 1.02, 2.62) and 1.73 (95% CI: 1.09, 2.74), respectively. Conclusions/Significance This analysis provides support for categorizing a T. trichiura infection ≥1,000 epg as ‘moderate’, as currently defined by the World Health Organization. Because this ‘moderate’ level of T. trichiura infection was found to be a significant risk factor for anemia in pregnant women, the intensity of Trichuris infection deemed to cause or aggravate anemia should no longer be restricted to the ‘heavy’ intensity category. It should now include both ‘heavy’ and ‘moderate’ intensities of Trichuris infection. Evidence-based deworming strategies targeting pregnant women or populations where anemia is of concern should be updated accordingly., Author Summary Infection by the soil-transmitted helminth Trichuris trichiura is defined as ‘light’, ‘moderate’ and ‘heavy’ depending on its intensity. However, these intensity categories were developed in the 1980s, before any epidemiological data were available on the association between specific T. trichiura infection intensities and adverse health outcomes. Here, we re-analyzed data from a study of T. trichiura infection and anemia in pregnant women to determine the threshold (i.e. the lowest infection intensity) associated with an increased risk of anemia. Women with T. trichiura infections of intensities ranging from 601 to 1632 eggs per gram of feces (epg) (ie. a ‘moderate’ level of intensity) had a significantly higher prevalence of anemia and a significantly lower hemoglobin level than the reference group (i.e. women with T. trichiura infections of intensities ranging between 0 and 24 epg). This finding contrasts with the common belief that only ‘heavy’ T. trichiura infection (10,000 epg and above) can cause anemia.

A malaria prevalence survey was carried out in young children in northern Ghana between October 1990 and September 1991, in an area with continuous mortality and morbidity surveillance. There was marked seasonal variation in malaria deaths, reported fevers, parasite rates and mean parasite densities, with parasite rates reaching 85-94% in the wet season. The monthly numbers of malaria deaths were highly correlated with rainfall in the previous 2 months (r = 0.90, P0.001). Parasite rates were highest in the oldest children (5-7 years), but parasite densities and rates of febrile illness were highest in those 6-11 months old. Haemoglobin levels were also at their lowest in this age group. The predominant species, Plasmodium falciparum, was present in 71% of all blood films. Febrile illness was well recognized by mothers, but it was not possible to construct a simple clinical diagnostic algorithm which would identify even 50% of children with high levels of malaria parasitaemia (or = 4000 parasites/microL). Malariometric indicators appear to have changed little in this area since a previous survey in 1955.

Stool specimens from a sample of schoolchildren at six schools in Kweneng District were examined for hookworm infection, using the brine flotation method. Necator americanus was the only hookworm identified. The western part of the District forms part of the Kalahari Desert, and in four villages here 90%, 88%, 88% and 86% of the children were infected. In two villages in the eastern non-desert part, only 13% and 9% were infected. Most infections were light. There was no significant correlation between severity of infection and anaemia. In one school (228 pupils; 86% infected with hookworm), half the children were treated with tetrachloroethylene (0.1 ml/kg, maximum 5 ml) and the other half with placebo. Two weeks after treatment the prevalence of infection were 28% and 75% respectively (p less than 0.001), and five months after treatment 51% and 69% (p less than 0.05). Measured over the five-month period there were no significant changes in haemoglobin and nutritional status (weight/height). Based on the results of the survey, a hookworm mass treatment programme was not recommended.

SUMMARYCaptive, healthy, adult badgers have blood containing haemoglobin at 13·3 g/dl, and 8·4×1012/l red cells with an MCV of 46·2 f1 and an MCH of 15·6 pg. They have 5·1×109 white cells/1 of which 3·29×109 are polymorphs, 1·49×109 are lymphocytes, 0·26×109 are monocytes, 0·07×109 are eosinophils and 0·01×109 are basophils. These values are somewhat less in adult animals just trapped from the wild, and are lower still in wild cubs.Changes associated with tuberculosis are a rise, and then a fall in red blood count and white blood count, an increase in the proportion of polymorphs and momocytes and a fall in lymphocytes late in the disease. This picture is similar to that seen in widespread, disseminated, tuberculin negative, tuberculosis in humans, a type of disease similar to that occurring in many badgers.BCG vaccination of badgers did not produce any measurable change in the blood picture.

SUMMARYRelationships between ascariasis and lactose digestion and between ascariasis and food transit time from mouth to caecum were investigated in young children from Chiriqui Province, Republic of Panama. The breath hydrogen method was used in both studies.Ascaris-infected children showed a significantly poorer degree of lactose digestion following a test oral load than uninfected children. Recovery of the capacity of the children to digest lactose was still not fully complete for at least 3 weeks following anthelmintic treatment. On average, the mouth-to-caecum transit time was similar in infected and uninfected children, but among theAscaris-infected children the transit time tended to be shorter in relation to the intensity of infection. Evidence from a cross-sectional survey indicated that ascariasis was significantly associated with reduced plasma vitamin A and carotenoid concentrations. This relationship remained after controlling for a range of socio-economic variables.Ascaris-infected children were frequently found to have lower haematocrits and blood haemoglobin concentrations than uninfected children, but these relationships could not be attributed to ascariasis alone.

The epidemiology and clinical features of skin rashes, pyoderma, and pyoderma with nephritis in children and adults in Zaria is described. Scabies is endemic in Zaria especially among young children, and boys at the Koranic schools. Pyoderma follows secondary infection of scabies and is more prevalent towards the end of the humid rainy season in August and September. Acute glomerulo-nephritis is common at this time also. Of 20 patients with pyoderma and nephritis 16 had scabies which had become infected. Group A streptococci were isolated from the skin lesions of 11 patients, but only 2 strains—one each of types M49 and M55—recognized to be associated with nephritis, were recovered. 3 patients had an uncommon type, M26 which had not previously been suspected of causing nephritis. Serum complement was low in all 10 patients with nephritis who were tested. Anti-deoxyribonuclease B was significantly more useful as an index of streptococcal infection than the antistreptolysin 0 titre.

The authors present a survey of gastrointestinal helminths of sheep on permanent pastures in the extreme north region of Tunisia (Mediterranean climate). Dynamic infestation of animals has been monitored by using batches of three tracer lambs introduced each two months during 2004 and 2005. These lambs were kept in the pens of veterinary school of Sidi Thabet (Tunisia) during three months and then necropsied. Faecal and blood samples were took from tracer lambs each two months during the whole period, and from animal flock only during 2004. The main helminth genera encountered were Trichostrongylus spp., Teladorsagia spp., Strongyloides papillosus and Anoplocephalidea; occasionaly were found Nematodirus , Oesophagostomum , Chabertia , Cooperia , Trichuris and Paramphistomum . The egg count of the ewes and lambs in the flock showed two peaks. For both ewes and lambs there is a gradual increase from January with a peak in May-June. This first peak is considered to be due to acquisition of infective larvae during the rainy and cold season, as evident from the worm burdens of tracer lambs. The second peak was exclusively observed in ewes during late autumn-early winter (November-December); it has two origins: infestation by third larvae stage and the periparturient rise. The worm burdens of tracer lambs showed that there was a gradual accumulation of nematodes from September- October, reaching a peak in March-April; a very low or naught infection is reported during the dry period (July-August). Infection by Anoplocephalidea was higher during the dry season. This study is primordial for a comprehensive control programme implementation against gastrointestinal helminths.
