Abstract

Background: In Germany, current school-based first aid training recommendations do not include children under 13 years. Our aim was therefore to evaluate a primary school-based first aid course for children aged 6–13 years. Methods: Our 2-day course (4 hours per day) for primary schools in the German rural state Brandenburg included basic elements of scene safety, compression bandages, recovery position, and resuscitation. The evaluation of specific knowledge and skills took place 1-day post-course. Results: Among the 1,268 included children (median age 9y, 49% girls) from six primary schools, 25% had previously attended first aid courses. Most children knew emergency phone number(s) pre-course. Afterward, this knowledge improved by 15% in the 6–7-year age group (reaching 80%) whereas it remained at 90% in children aged 8–13 years. After the course, about 80% of the 8–13-year-old children (40% in 6–7-year age group) answered at least one resuscitation question correctly. In the practical skill assessments, about 80% of the 8–13-year-old children (60% in the 6–7-year age group) chose a correct or almost-correct chest compression rate during resuscitation. The correct depth of compression was achieved by about 80% (10–13 years), 60% (8–9 years), and 40% (6–7 years). Three-quarters successfully put another child in the recovery position; more than two-thirds applied a compression bandage appropriately. Conclusions: A 2-day course in a primary school setting showed considerable improvement in first aid and resuscitation skills of children aged 6–13 years. Future studies should examine these effects’ sustainability and the appropriate timing for refresher courses. Zusammenfassung Hintergrund: In Deutschland umfassen die aktuellen schulbasierten Erste-Hilfe-Schulungsempfehlungen keine Kinder unter 13 Jahren. Unser Ziel war es daher, einen Erste-Hilfe-Kurs für Grundschulen zu evaluieren. Material und Methode: Unser zweitägiger Kurs (4 Stunden pro Tag) für Grundschulen im Bundesland Brandenburg umfasste Basiselemente der eigenen Sicherheit am Unfallort, Kompressionsverbände, stabile Seitenlage und Wiederbelebung. Nach der Basiserhebung kurz vor Schulungsbeginn fand die Evaluation spezifischer Kenntnisse (mit Fragebogen) und Fertigkeiten einen Tag nach dem Kurs statt. Ergebnisse Unter den 1.268 eingeschlossenen Kindern (Altersmedian 9 Jahre, 49% Mädchen) von sechs Grundschulen hatten 25% bereits früher an einem Erste-Hilfe-Kurs teilgenommen. Die meisten Kinder kannten vor dem Kurs bereits die Notrufnummer(n). Danach verbesserte sich dieses Wissen um 15% und erreichte 80% in der Altersgruppe der 6-7-Jährigen, während es bei älteren Kindern (8-13 Jahre) bei 90% blieb. Nach dem Kurs beantworteten 40% der 6-7-Jährigen und etwa 80% der 8-13-jährigen Kinder mindestens eine Frage zur Reanimation richtig. Bei den Bewertungen der praktischen Fertigkeiten wählten 60% der 6-7-Jährigen und etwa 80% der 8-13-jährigen Kinder während der Wiederbelebung die korrekte oder nahezu korrekte Rate der Kompression des Brustkorbs aus. Die richtige Kompressionstiefe wurde von 40% (6-7 Jahre), 60% (8-9 Jahre) und etwa 80% (10-13 Jahre) erreicht. Drei Viertel der Teilnehmenden setzten erfolgreich ein anderes Kind in die Stabile Seitenlage; mehr als zwei Drittel legten einen Kompressionsverband angemessen an. Schlussfolgerungen Ein zweitägiger Kurs in der Grundschule führte in allen Altersgruppen von 6-13 Jahren zu einer deutlichen Verbesserung ihrer Kenntnisse in Erster-Hilfe und Fähigkeiten bei der Wiederbelebung. Zukünftige Studien sollten die Nachhaltigkeit dieser Effekte und den geeigneten Zeitpunkt für Auffrischungskurse untersuchen. Naszym celem było zatem ocenienie kursu pierwszej pomocy dla dzieci w wieku od 6 do 13 lat prowadzonego w szkole podstawowej. Metody Nasz dwudniowy kurs (4 godziny dziennie) dla szkół podstawowych na obszarach wiejskich w niemieckim kraju związkowym Brandenburgia obejmował podstawowe elementy bezpieczeństwa w miejscu zdarzenia, bandażowanie uciskowe, pozycję bezpieczną i resuscytację. Ocena konkretnych umiejętności i wiedzy odbyła się dzień po zakończeniu kursu. Wyniki Wśród 1268 uwzględnionych dzieci (średni wiek 9 lat, 49% dziewczyn) z sześciu szkół podstawowych, 25% uczestniczyło wcześniej w kursach pierwszej pomocy. Większość dzieci przed kursem znała już numer(y) alarmowy. Abstract

This bachelor's thesis deals with the issue of maternal awareness in the field of lay resuscitation of newborns and infants and is concveived as a theoretical-practical work. The theoretical part of the bachelor thesis is divided into seven chapters. The first chapter describes the neonatal and infant period. The next chapters present the recommended practices in neonatal and infant resuscitation according to the current 2021 Guidelines, and define the chain of survival, emergency pre-hospital care, basic and advanced life support. The fifth chapter deals with education in basic life support. The final chapters discuss sudden life-threatening episodes - apparent life-threatening event (ALTE), brief resolved unexplained event (BRUE), sudden unexpected postnatal collapse (SUPC), and last but not least, sudden infant death syndrome (SIDS), with emphasis on its risk factors and preventive measures. For the practical part of my bachelor's thesis, a quantitative questionnaire survey was used, which took place between 12/2022 and 2/2023 at the Gynaecology and Obstetrics Clinic of the 1st Faculty of Medicine and General University Hospital in Prague. 4. The lay resuscitation – An analysis to the efficiency increase employing virtual reality

Abstract

Die frühe Erkennung eines plötzlichen Herz-Kreislauf-Stillstandes und das sofortige Einleiten der Rettungskette sind für die betroffene Person die wichtigsten Grundlagen um zu Überleben. In solch einer Situation ist eine Laienreanimation unabdingbar, weswegen die zu setzenden Handlungen in den meisten Fällen von unmittelbaren Beobachtern übernommen werden. Jedoch wird dies leider zu selten durchgeführt, da bestimmte Umstände die anwesenden Personen davon abhalten. Das Ziel der vorliegenden Arbeit ist es zu beantworten, aufgrund welcher Gegebenheiten sich Laien und Laiinnen in einer Notfallsituation nicht in der Lage fühlen Reanimationsmaßnahmen setzen zu können. Darüber hinaus soll herausgefunden werden, ob dem mit Virtueller Realität, in Form von gezielten Aus- und Weiterbildungen, entgegengewirkt und damit die Laienreanimation gestärkt werden kann. Bei dieser Arbeit handelt es sich um eine empirische Studie mit einer Datenerhebung in Form einer Kombination aus qualitativer und quantitativer Forschung, sprich ein Mixed-Methods-Ansatz. Mittels Online-Umfrage konnten N=234 Personen zu Ihren Reanimationskenntnissen befragt werden. Zur Auswertung der gewonnen Daten wurde die deskriptive Statistik verwendet. Um diese Ergebnisse optimal darstellen zu können, wurden Grafiken erstellt. Die Antworten der Teilnehmer*innen zeigen, dass es oft an Grundlagen und Wissen über die zu setzenden Maßnahmen in Bezug auf die Wiederbelebung einer Person fehlt. Der mitunter stärkste aller Hemmer unter den befragten Personen, ist die nachträgliche Alleinschuld. Die Bereitstellung von Reanimationswissen mittels ausgewählter Virtual-Reality-Anwendungen wird von mehr als dreiviertel aller Teilnehmer*innen als nützlich angesehen. Weitere Studien auf diesem Themengebiet könnten vielmehr die Effizienz solcher Anwendung untersuchen. Die Ergebnisse dieser Bachelorarbeit könnten als Literaturerweiterung dafür herangezogen werden. The early detection of a sudden cardiac arrest and the immediate start with the rescue chain are the most important items for an affected person to survive. In such situations, resuscitation is highly essential. These actions are taken on by bystanders in most cases. Unfortunately, this is carried out too seldom, as certain circumstances prevent the people around from doing so. The aim of this work is to answer the circumstances, which make people feel unable to carry out resuscitation measures during an emergency. In addition, it has to be found out if lay resuscitation can be strengthened accordingly employing virtual reality in the form of specific training. This work is an empirical study with data collection in the form of a combination of qualitative and quantitative research, i.e. a mixed-methods approach. Using an online survey, N=234 people could be asked about their resuscitation knowledge. Descriptive statistics were used to evaluate the data obtained. To present these results accordingly, graphics were created. The participants' answers show that there often is a lack of fundamental knowledge and knowledge about the resuscitation measures. Amongst all interviewed people, one of the strongest inhibitors is the feeling of guilt afterwards. More than three quarters of the participants considered using selected virtual reality applications for the provision of resuscitation knowledge as useful. Further specialized studies could examine the efficiency of such applications. The results of this bachelor thesis could be used as beneficial literature.

5. Schoolchildren as lifesavers in Europe – Training in cardiopulmonary resuscitation for children.

Abstract

Sudden cardiac arrest is a major contributor to avoidable deaths in Europe. Immediate initiation of basic life support (BLS) by lay bystanders is among the most successful strategies in its treatment. Despite the fact that more than half of all cardiac arrests are witnessed in a number of European countries, layperson resuscitation is initiated in only one-fifth of all cases. One strategy to promote bystander BLS is to establish cardiac resuscitation training in schools. BLS instructions for schoolchildren – including the use of automatic external defibrillators (AEDs) – have been shown to be feasible independently of the children's age or physical ability. Nonetheless, it appears reasonable to implement age-adjusted curricula. The earlier in the course of life-long learning BLS instruction begins, the more sustainable training may be. Abstract

Background Contemporary resuscitation guidelines for basic life support recommend an immediate onset of cardiac compressions in case of cardiac arrest followed by rescue breaths. Effective ventilation is often omitted due to fear of doing harm and fear of infectious diseases. In order to improve ventilation a pre-stage of an automatic respirator was developed for use by laypersons. Methods Fifty-two healthy volunteers were ventilated by means of a prototype respirator via a full-face mask in a pilot study. The pre-stage public access ventilator (PAV) consisted of a low-cost self-designed turbine, with sensors for differential pressure, flow, FO2, FCO2 and 3-axis acceleration measurement. Sensor outputs were used to control the respirator and to recognize conditions relevant for efficiency of ventilation and patients’ safety. Different respiratory manoeuvres were applied: a) pressure controlled ventilation (PCV), b) PCV with controlled leakage and c) PCV with simulated airway occlusion. Sensor signals were analysed to detect leakage and airway occlusion. Detection based upon sensor signals was compared with evaluation based on clinical observation and additional parameters such as exhaled CO2. Results Pressure controlled ventilation could be realized in all volunteers. Leakage was recognized with 93.5% sensitivity and 93.5% specificity. Simulated airway occlusion was detected with 91.8% sensitivity and 91.7% specificity. Conclusion The pre-stage PAV was able to detect potential complications relevant for patients’ safety such as leakage and airway occlusion in a proof of principle study. Prospectively, this device provides a respectable basis for the development of an automatic emergency respirator and may help to improve bystander resuscitation. Abstract

Aims: This prospective longitudinal study over 6 years compared schoolteachers and emergency physicians as resuscitation trainers for schoolchildren. It also investigated whether pupils who were trained annually for 3 years retain their resuscitation skills after the end of this study. Methods: A total of 261 pupils (fifth grade) at two German grammar schools received resuscitation training by trained teachers or by emergency physicians. The annual training events stopped after 3 years in one group and continued for 6 years in a second group. We measured knowledge about resuscitation (questionnaire), chest compression rate (min-1), chest compression depth (mm), ventilation rate (min-1), ventilation volume (mL), self-efficacy (questionnaire). Their performance was evaluated after 1, 3 and 6 years. Results: The training events increased the pupils' knowledge and practical skills. When trained by teachers, the pupils achieved better results for knowledge (92.86% +/- 8.38 vs. 90.10% +/- 8.63, P = 0.04) and ventilation rate (4.84/min +/- 4.05 vs. 3.76/min +/- 2.37, P = 0.04) than when they were trained by emergency physicians. There were no differences with regard to chest compression rate, depth, ventilation volume, or self-efficacy at the end of the study. Knowledge and skills after 6 years were equivalent in the group with 6 years training compared with 3 years training. Conclusions: Trained teachers can provide adequate resuscitation training in schools. Health-care professionals are not mandatory for CPR training (easier for schools to implement resuscitation training). The final evaluation after 6 years showed that resuscitation skills are retained even when training is interrupted for 3 years. (C) 2016 The Authors. Published by Elsevier Ireland Ltd.
