We present the case of a 74-year-old man with a history of a squamous cell carcinoma in the left axilla. The patient underwent a multidisciplinary surgical resection through an extended forequarter amputation with thoracic wall resection and reconstruction. With regard to the complexity of the case, three-dimensional virtual reality–based patient-specific reconstructions were used as a supplemental tool to conventional computed tomography imaging to plan the procedure. With this report, we aim to stimulate further research to improve and automate the workflow and to bring virtual and augmented reality reconstructions into the surgical theater of the future.

A 68-year-old man with right cheek melanoma after resection underwent 18F-FDG PET/CT, which was unremarkable except for multiple FDG-avid subcentimeter but rounded lymph nodes in the left axilla. The patient had undergone a COVID-19 vaccination in the left arm 3 weeks prior. As under vaccinations have been documented to cause reactive FDG-avid lymph nodes, the nodes in our patient were considered benign, reactive to the COVID-19 vaccination. Although FDG-avid benign, reactive nodes have been an uncommon finding in the past, the upcoming surge in COVID-19 vaccinations makes this an important finding for the interpreting physician to consider and recognize.

During the dissection of a 79-year-old Caucasian female cadaver, a variant of Langer's axillary arch was found unilaterally in the left axilla. While Langer's axillary arches are not uncommon, this particular variant, attaching to the biceps brachii, is much rarer with a reported prevalence of only 0.25%. The case reported here, however, is only the third example of a Langer's axillary arch that has been found in the last 14 years in the Dissecting Room at St. George's, University of London, giving it an overall prevalence of approximately 1.0% amongst a population of around 280 Caucasian cadavers, much lower than the reported frequency of 7%. Langer's axillary arches can be completely asymptomatic in life, but may also cause a variety of issues both clinically and surgically.

BACKGROUND Excessive hair is undesirable and represents both a social and emotional burden, especially among females. Laser hair removal (LHR) has gained popularity in the past 2 decades. It is a common practice to instruct patients not to pluck or wax their hair before undergoing LHR; only shaving is allowed to improve the efficacy of the LHR. OBJECTIVE To compare the percentage of hair reduction in preshaved versus prewax epilated axillae after LHR in healthy adult Saudi females. METHODS AND MATERIALS This study is a quantitative experimental pilot clinical trial. Twenty female volunteers were enrolled into this study; all underwent 3 laser sessions using a long-pulsed alexandrite laser. Their left axilla was prewaxed and right axilla was preshaved immediately before laser treatment. Hair was counted manually before and 6 months after the third laser session. RESULTS The right and left axillae both showed significant hair reduction, between the first session and the hair count 6 months after the third laser session. However, there was no statistically significant difference between these 2 means (p = .44). CONCLUSION There is no statistical difference in hair reduction after LHR whether the area is preshaved or prewax epilated.

Giant condyloma acuminatum (GCA), also known as Buschke -Lowenstein tumor, is a huge, rapidly growing variant of condyloma found in the anogenital region. Extragenital GCA is rare and, in most cases, affecting the intertriginous area and treated by excision. We described a case of refractory extragenital GCA involving the large part of the left axilla area successfully treated with immunotherapy and podophyllotoxin. Considering the large size and the location, the lesions were not treated by wide excision because it might result in scar contracture with limitation of shoulder movement or axillary web syndrome. Our case illustrates that combination therapy of podophyllotoxin, imiquimod plus weekly cryotherapy may be an effective and less invasive option for treating GCA of the axilla with good cosmetic and functional outcomes.

Seroma formation after axillary dissection is a common problem in breast cancer surgery. We report the case of a 68-year-old female with breast cancer who underwent a wide local excision and axillary clearance due to stage III breast cancer. Patient received post-operative whole breast irradiation therapy and developed a painful, infected seroma one month after surgery. This was treated with antibiotic therapy after which the infection subsided. One year after surgery patient presented with a painful persisting seroma in the left axilla. We decided to surgically treat the seroma by removing the fibrous seroma capsula and closing of the dead space with a latissimus dorsi flap. Six weeks after surgery, patient was pain and seroma free and was happy with the surgical result. Latissimus dorsi flap harvesting is an ideal way to treat persisting fibrous encapsulated seroma pockets after axillary clearance in the treatment of breast cancer.

Pancreatic cancer is usually high-grade and metastatic when diagnosed. Skin metastasis is very rare and is usually found in the periumbilical area. Non-umbilical metastasis is extremely rare. Only 19 cases have been reported in the literature. Only one case in the buttock has been described: thus our case is the second one known. In this note, we present a cutaneous metastasis of a pancreatic cancer case that was admitted to the clinic with a mass each on the left buttock and in the left axilla, surgically treated for palliation. We include a review of the literature.

The objective of this study was to examine the effect of metrics-based vs. non-metrics-based feedback on novices learning predefined competencies for acquisition and interpretation of sonographic images relevant to performance of ultrasound-guided axillary brachial plexus block. Twelve anaesthetic trainees were randomly assigned to either metrics-based-feedback or non-metrics-based feedback groups. After a common learning phase, all participants attempted to perform a predefined task that involved scanning the left axilla of a single volunteer. Following completion of the task, all participants in each group received feedback from a different expert in regional blocks (consultant anaesthetist) and were allowed to practise the predefined task for up to 1 h. Those in the metrics-based feedback group received feedback based on previously validated metrics, and they practised each metric item until it was performed satisfactorily, as assessed by the supervising consultant. Subsequently, each participant attempted to perform ultrasonography of the left axilla on the same volunteer. Two trained consultant anaesthetists independently scored the video recording pre- and post-feedback scans using the validated metrics list. Both groups showed improvement from pre-feedback to post-feedback scores. Compared with participants in the non-metrics-based feedback group, those in the metrics-based feedback group completed more steps: median (IQR [range]) 18.8 (1.5 [17-20]) vs. 14.3 (4.5 [11-18.5]), p = 0.009, and made fewer errors 0.5 (1 [0-1.5]) vs. 1.5 (2 [1-6]), p = 0.041 postfeedback. In this study, novices' sonographic skills showed greater improvement when feedback was combined with validated metrics.

Pilomatrixoma, a rare benign tumour of sebaceous gland is commonly known as calcifying epithelioma of Malherbe or Tricholemmal cyst. We came across even the rarest form of this benign tumour of skin. Our case report was interestingly detected at a higher age group, unusual site of the body, multiple in nature following postoperative incision drainage of abscess in left axilla.

It is currently not fully established whether human individuals have a genetically determined, individual-specific body odour. Volatile carboxylic acids are a key class of known human body odorants. They are released from glutamine conjugates secreted in axillary skin by a specific N α -acyl-glutamine-aminoacylase present in skin bacteria. Here, we report a quantitative investigation of these odorant acids in 12 pairs of monozygotic twins. Axilla secretions were sampled twice and treated with the N α -acyl-glutamine-aminoacylase. The released acids were analysed as their methyl esters with comprehensive two-dimensional gas chromatography and time-of-flight mass spectrometry detection. The pattern of the analytes was compared with distance analysis. The distance was lowest between samples of the right and the left axilla taken on the same day from the same individual. It was clearly greater if the same subject was sampled on different days, but this intra-individual distance between samples was only slightly lower than the distance between samples taken from two monozygotic twins. A much greater distance was observed when comparing unrelated individuals. By applying cluster and principal component analyses, a clear clustering of samples taken from one pair of monozygotic twins was also confirmed. In conclusion, the specific pattern of precursors for volatile carboxylic acids is subject to a day-to-day variation, but there is a strong genetic contribution. Therefore, humans have a genetically determined body odour type that is at least partly composed of these odorant acids.

Acrochordons or fibroepithelial polyps, are very common skin lesions. However, giant acrochordons are extremely rare. We present a case of a morbidly obese diabetic man who was treated with primary excision of a giant 18 cm acrochordon of the left axilla.

Segmental neurofibromatosis type 5 (NF5) is a rare form of NF, in which the cutaneous and/or neural changes are confined to one region of the body. It is often underdiagnosed or undetected due to the absence of symptoms. Inheritance of NF is different from other types of NF. Intraosseous neurofibromas are rare tumors which develop from nerves supplying the periosteum or nerves following the intraosseous course. Plexiform neurofibroma with associated intraosseous neurofibroma is extremely rare. We report a case of plexiform neurofibroma in the left axilla of a 17-year-old female with associated intraosseous neurofibroma in adjoining left humerus.

A 32 year old female patient with a soft boggy mass in the left axilla was referred from the surgery clinic for fine needle aspiration with a provisional diagnosis of fat hypertrophy. Physical examination revealed a well defined mobile nodule within the mass. Cytological examination of the aspirated material and histopathological examination was consistent with fibroadenoma. Accessory breast tissue is rare accounting to less than 1% cases seen in females. It is usually bilateral. We report a case of 24-year-old woman with a lump in the left axilla in view of its rarity and made a differential diagnosis of fibroadenoma, which following the investigations and histopathological report was confirmed as revealed fibroadenoma in the axilla. It should also be considered as a differential diagnosis for all axillary swellings.

Background: Cellular angiofibroma is a benign and rare tumor. It usually arises in middle-aged women and involves the vulva. Complete local excision of the tumor is the best cure, and, usually, there is no recurrence after surgery. Case Report: We describe a 20-year-old woman with a painless, growing vulvar mass who presented about 3 years ago. Her past medical history was negative for oral contraceptives, tobacco, and alcohol, and there were no similar lesions in her family history. A physical examination revealed two masses on the right and the left labia majora and similar lesions on the left axilla and both breasts. An uncomplicated simple resection of the vulvar masses was done in the operating room. There was no evidence of recurrence 12 months after surgery. Histopathologic examination revealed that the findings were consistent with the diagnosis of the cellular angiofibroma. Based on our knowledge, this patient is the youngest case of vulvar angiofibroma. The lesions were large and symmetrical (on both labia majora), although they differed in size. Extravulvar (breast and axilla) lesions were also noteworthy in this patient.

We described a case of a 71-year-old woman with an epithelioid hemangioendothelioma (EHE) in her left axilla, a rare location which hasn’t been reported yet. The patient suffered from numbness, pain and decreased muscle strength of her left upper extremity. Sonography revealed a hypoechoic mass surrounded the axillary artery and brachial artery. No obvious capsule was demonstrated. CT showed a soft-tissue mass with some calcifications and peripheral ring-like enhancement. The MRI indicated a mass with mainly intermediate signal intensity on T1-weighted imagine and intermediate signal intensity on T2-weighted imagine. The diagnosis was confirmed by histopathologic examination after surgery. There are some correlations of these imaging features with its histopathologic characters.

SUMMARY: BACKGROUND: Ectopic breast tissue usually develops along the mammary ridges, and the incidence has been reported to be 2-6% of the general population. Occurrence of primary carcinoma in ectopic breast tissue is rare. CASE REPORT: We report the case of 59-year-old woman with accessory breast carcinoma in her left axilla. CONCLUSION: Because an accessory areola or nipple is often missing and awareness of physicians and patients about these unsuspicious masses is lacking, clinical diagnosis of accessory breast carcinoma is frequently delayed. Therefore, a mass along the 'milk line' should be examined carefully, and any suspicious lesions should be evaluated.

A case of extramammary triple Paget's disease of perianal area and bilateral axilla is repored and discussed.A 66 year-old man was referred with the diagnosis of Paget's disease of perianal region, in which wide hypopigmeted lesion was observed around the half side of the anus.After making the temporary colostomy. We perfomed wide tumor excision and the full thickness skin graft for the anal duct and split shickness skin graft for the perianal area. During the procedure, some hypopigmented spots were found on left axilla, also the Paget's cells were identified later.Under the diagnosis of triple Paget's disease, the additional wide tumor excision was performed on both axilla.The postoperative result is good and the function of anal sphincter is satisfied.

PubMedID: 15934442 We describe a 16-year old female with lupus erythematosus panniculitis with unusual manifestations. She had noted to have developed erythematous nodules and plaques in the right axilla and inguinal region at the age of one year. These lesions resolved gradually with scar formation. However, new lesions were noted at the same locations in the following years. Some of her lesions at the scalp and the left axillary regions developing within the last two years slowly enlarged showing an annular configuration and subsequently resulted in hair loss. The erythematous border of her lesion in the left axilla consisted of two parallel red lines. Histopathological and direct immunofluorescent findings were consistent with lupus erythematosus panniculitis. Primary apocrine sweat gland carcinomas (PASGCs) are rare tumors, commonly located in the axilla. Metastases are common and confer poor prognosis. Given the rarity of these tumors, there is limited knowledge re- garding its diagnosis and management. Here we show 18 F-FDG PET/CT images of a 61-year-old man with PASGCs of the left axilla. PET/CT confirmed the diagnosis as primary axillary malignancy with nodal, pulmonary, and skeletal metastases. Another interesting finding in this case was the presence of FDG- avid calcified metastatic lymph nodes during the initial evaluation. Follow-up PET/CT showed progression of the disease. FDG PET/CT seems to be a promising tool in the management of PASGCs.

Cutaneous metastases from pancreatic carcinoma are uncommon. Eight cases have been described in the literature. The metastases display some common features such as umbilical nodules and the histologic pattern of adenocarcinoma. Our patient had erythematous infiltrated plaques in the left axilla and on the upper portion of the chest. A skin biopsy specimen disclosed many poorly differentiated atypical cells throughout the dermis. The diagnosis was confirmed by positive immunohistochemical staining for carbohydrate antigen 19-9.

Basal cell carcinoma (BCC) rarely metastasizes. However, this unfortunate outcome can occur, usually in neglected tumors. We report a 52-year-old man with a BCC on the left chest that enlarged and then ulcerated over a 6-year period. Metastasis of the tumor to lymph nodes in the left axilla resulted, but the patient remains free of disease 24 months after wide excision, lymph node dissection, and local radiation therapy to the axilla.

Fibrous hamartoma of infancy (FHI) is a rare, benign lesion which is commonly seen under two years of age. Clinical and radiological features of FHI can mimic malignant soft tissue sarcomas, it is important to make differential diagnosis. Ultrasound (US) examination reveals heterogeneous echogenicity that can be also suggestive for other soft tissue tumors but newly defined "serpentine pattern" of intervening hypoechoic portions in the hyperechoic mass with poorly defined margins and with poor vascularity is special for FHI. Here we report a 15-month-old-boy with FHI with serpentine pattern on US. He initially presented with a painless mass in his left axilla existing for approximately seven months. The mass was successfully excised and he has been followed for three years without any evidence of recurrence. Fibrous hamartoma of infancy should always be considered in differential diagnosis in children under two years of age with a firm and solitary mass in the axilla especially when US reveals serpentine pattern with poorly defined margins and with poor vascularity. If these clinical and ultrasonographic findings are seen in a child under two years old, surgery can be performed without any additional imaging modalities. Awareness and careful assessment are important in order not to misdiagnose this benign mass for which surgical excision is curative.

We report a case of Paget's disease of the mamma and axilla which occurred simultaneously. An 80-year-old woman, who visited another hospital because of abnormal discharge from the right nipple and erosion on the right mamma and left axilla, was seen at the hospital because she was aware of a right breast tumor and no remission of the erosion was observed. A 3×4cm tumor was palpable in the CA region of the right breast. Needle aspiration cytology offered a diagnosis of invasive ductal carcinoma. Histological examinations of the erosion of the mamma and axilla showed numerous Paget's cell. Simultaneous occurrence of mammary and axillary Paget's disease like this case is very rare, and have not been reported previously, to our knowledge. When we inspect mammary lesions, careful observation including distant cutaneous change would be mandatory.

The case of a 31-year-old woman with accessory breast cancer in the left axilla is described. She had noticed a swelling in the left axilla during her three pregnancies. The preoperative diagnosis of accessory breast cancer was made on the basis of ultrasound-guided fine-needle aspiration biopsy (US-FNAB) and clinical history. She was treated by wide local resection and regional lymph node dissection. Although cancer originating from accessory breast tissue has been reported very rarely, knowledge of this disorder may facilitate the correct diagnosis of axillary tumors. US-FNAB is a useful and simple technique for the tissue diagnosis of axillary tumors.

The authors report a case of recurrent breast carcinoma in the right chest in a patient who earlier had a right breast mastectomy. Injection of Tc-99m sulfur colloid into the lesion site in the right midchest revealed drainage to a very faint node in the contralateral axilla on the left, an unexpected site, and none to the ipsilateral axilla, the expected site. Disease was found in the left axilla in the sentinel nodes. Lymphoscintigraphy added valuable information in the management of this patient.

Variations in the termination of the axillary artery (AA) were observed in 51 out of 356 axilla during routine dissections. The incidence of variation was higher on the right axilla in 17.42% cases (CI: 11.60-22.51) than the left axilla 11.24% cases (CI: 6.46-15.55) and was found to be 12.33% in male and 40.63% in female cadavers. The variations were divided into four groups. The first group showed that the terminal end of the AA divided into lateral and medial divisions in 6.16% (CI: 2.21-9.82) male and in 15.63% (CI: 2.76-25.25) female cadavers. The second group variation showed that the terminal end of the axillary artery divided into deep and superficial divisions in 2.74% (CI: 0.10-5.27) male and in 6.25% (CI: -1.87-13.13) female cadavers. The third group showed that the terminal end of the AA divided into three branches in 2.74% (CI: 0.10-5.27) male and in 6.25% (CI: -1.87-13.13) female cadavers. The fourth group showed the tapering of the terminal end of AA in 0.68% (CI: -0.63-1.99) male and 12.50% (CI: 0.97-21.46) female cadavers. The variable origin of the circumflex humeral, subscapular and profunda brachii arteries was also observed. These variations in the terminal end of the axillary artery are not only of significant clinical importance to orthopaedic and reconstructive surgeons but also have very significant medico-legal implications.

Introduction: Cysticercosis is commonly encountered in this part of the world. It can manifest in many different form. Objective: To report a case of cysticercosis which presented with a lump in axilla. Case: A 36 year old man from Dharan presented with the chief complaint of lump in the right axilla for 9 months and two lumps in left axilla for 3 months. The onset of lump was insidious and gradually progressive in nature. The examination revealed a single mass in the medial surface of right upper arm, approximately 3x3cm in size, not fixed with the skin, slightly movable and mildly tender and another two masses around 1x2 cm in left axilla with similar characteristics as the right one. The ultrasonography showed heterogenous lesion measuring 27mm x 18 mm in medial aspect of right upper arm and two similar lesions measuring 14.5 x11mm and 11 x 10 mm in left axilla suggesting "Cysticercosis". The lumps were surgically removed successfully. Conclusion: Cysticercosis is one of the causes of lump in axillay region. Castleman's disease is a rare benign lymphoproliferative disorder of uncertain origin which most commonly involves the mediastinum but rarely affects the axilla. We report a case of localized Castleman's disease involving the axillary lymph node. Mammography revealed a well-defined, homogeneously dense ovoid mass, 3 cm in size, in the left axilla, while gray-scale ultrasonography (US) demonstrated a well-defined, uniformly hypoechoic ovoid mass with good through transmission. Peripheral hypervascularity was observed at power Dopper US, and early rapid homogeneous enhancement at contrast-enhanced dynamic CT.

Rounded and velvety epidermal naevus, naevoid acanthosis nigricans, also called the acanthosis nigricans form of epidermal naevus is an extremely rare entity, with fewer than 15 cases described in the literature. We report the case of a 23-year-old woman with asymptomatic velvety verrucous plaques on her left axilla starting at the age of 14. As far as we know this is the first case of such a verrucous form with only axillar involvement of naevoid acanthosis nigricans. We report this rare case, and we make a brief review of the clinical pathophysiology and treatment of this uncommon tumour.

Fourteen healthy adult Japanese females were selected to participate in a comparative study of hair plucking (a temporary hair removal technique) and the blend method (a permanent hair removal technique). The effectiveness of permanent hair removal and the safety of the blend method were examined in this study. If hairs of the left axilla were removed by the blend method, then hairs of the right axilla were removed by plucking, and vice versa. Every hair was removed and the number of hairs was counted each time. Sessions were conducted every three weeks, and the experiment continued until no new hair growth was seen using the same experimental standard. Electrologists with more than 1000 hours of experience were selected to perform electrolysis. Examinations of the axilla, before and after each test, were conducted by the authors. The results showed that the number of hairs decreased in the axilla with each session using the blend method: permanent hair removal was achieved in an average of 26.8 weeks or 9.9 sessions. However, the number of hairs did not decrease in the axilla after plucking. Also, there were no reports of hair removal related side-effects in this study. It was thus demonstrated that the blend method is a safe and effective way of achieving permanent hair removal.

Static lymphoscintigraphy performed 24 hours after peritumoral injection of Tc-99m nanocolloid in a 62-year-old woman with invasive ductal carcinoma in the lower medial quadrant of the left breast showed drainage to 2 sentinel nodes in the left and 2 in the right axilla. Four sentinel nodes were excised from the left axilla, all without metastases. The right axilla was not explored. The authors regard the contralateral lymphatic drainage as an aberrant route from the primary tumor, most probably via the subdermal plexus. One previous report showed drainage to one sentinel node in each axilla, both containing metastases.

INSTANCES of the occurrence of two entirely different types of neoplasm in the same patient at the same time have come within the experience of all radiologists. However, each such occurrence has peculiar aspects that make its study of value. The following case presents a number of interesting features and will, therefore, be reported in some detail. REPORT OF A CASE A 9-month-old male infant was referred because of asymptomatic pigmented papules in the left axilla (Fig 1). The lesions were first noted at age three months with an increase in number during the next three months. Although the lesions were usually tan, they occasionally seemed red. The child was otherwise in good health and there was no family history of medical or dermatologic disease. Examination revealed several round and oval tan papules in the left axillary area. The remaining skin and mucosal surfaces were free of lesions. To the Editor:— Every patient who has received a tetanus toxoid series should have a small tattoo mark placed in an appropriate spot, such as in the left axilla, to indicate that the individual has been immunized. If the patient has damage to the left axilla, the mark could be placed in the opposite axilla. Since records are lost and memories are short, this would provide a fool-proof way of identifying an individual who has had his immunizations completed, as surely as, or more surely than we can identify the same by inquiring if he or she has served in the Armed Forces. We are reporting the case of a large axillary aneurysm of 13 years' duration, first, because of cardiac arrest which occurred at the first induction of anesthesia and did not occur at the time of surgical repair and, secondly, because a type of venous shunt was applied which in this arteriosclerotic syphilitic patient with cardiac irritability permitted a less traumatic and more rapid solution of the problem than would otherwise have been possible. Report of a Case:— A 25-year-old white man was seen on Dec 16, 1965, because of an eruption of one month's duration involving the left ring finger, right wrist, and the left axilla. The eruption in the axilla started after Right Guard deodorant had been applied to the area. The eruption on the finger was beneath the patient's ring and that on the wrist was beneath his watch. The patient had applied 0.25% methylprednisolone acetate with neomycin for one week without relief. Examination revealed a scaly, erythematous, lichenified eruption involving the left axilla and also the left ring finger and the right wrist.
