Otosclerosis is a progressive middle-ear disease that affects conductive transmission through the middle ear. Ear-canal acoustic tests may be useful in the diagnosis of conductive disorders. This study addressed the degree to which results from a battery of ear-canal tests, which include wideband reflectance, acoustic stapedius muscle reflex threshold (ASRT), and transient evoked otoacoustic emissions (TEOAEs), were effective in quantifying a risk of otosclerosis and in evaluating middle-ear function in ears after surgical intervention for otosclerosis.To evaluate the ability of the test battery to classify ears as normal or otosclerotic, measure the accuracy of reflectance in classifying ears as normal or otosclerotic, and evaluate the similarity of responses in normal ears compared with ears after surgical intervention for otosclerosis.A quasi-experimental cross-sectional study incorporating case control was used. Three groups were studied: one diagnosed with otosclerosis before corrective surgery, a group that received corrective surgery for otosclerosis, and a control group.The test groups included 23 ears (13 right and 10 left) with normal hearing from 16 participants (4 male and 12 female), 12 ears (7 right and 5 left) diagnosed with otosclerosis from 9 participants (3 male and 6 female), and 13 ears (4 right and 9 left) after surgical intervention from 10 participants (2 male and 8 female).Participants received audiometric evaluations and clinical immittance testing. Experimental tests performed included ASRT tests with wideband reference signal (0.25–8 kHz), reflectance tests (0.25–8 kHz), which were parameterized by absorbance and group delay at ambient pressure and at swept tympanometric pressures, and TEOAE tests using chirp stimuli (1–8 kHz). ASRTs were measured in ipsilateral and contralateral conditions using tonal and broadband noise activators. Experimental ASRT tests were based on the difference in wideband-absorbed sound power before and after presenting the activator. Diagnostic accuracy to classify ears as otosclerotic or normal was quantified by the area under the receiver operating characteristic curve (AUC) for univariate and multivariate reflectance tests. The multivariate predictor used a small number of input reflectance variables, each having a large AUC, in a principal components analysis to create independent variables and followed by a logistic regression procedure to classify the test ears.Relative to the results in normal ears, diagnosed otosclerosis ears more frequently showed absent TEOAEs and ASRTs, reduced ambient absorbance at 4 kHz, and a different pattern of tympanometric absorbance and group delay (absorbance increased at 2.8 kHz at the positive-pressure tail and decreased at 0.7–1 kHz at the peak pressure, whereas group delay decreased at positive and negative-pressure tails from 0.35–0.7 kHz, and at 2.8–4 kHz at positive-pressure tail). Using a multivariate predictor with three reflectance variables, tympanometric reflectance (AUC = 0.95) was more accurate than ambient reflectance (AUC = 0.88) in classifying ears as normal or otosclerotic.Reflectance provides a middle-ear test that is sensitive to classifying ears as otosclerotic or normal, which may be useful in clinical applications.

The brainstem-mediated ASR reveals the integrity of the neuronal components of the auditory reflex arc, including the cochlea receptors, eight cranial nerves, and brainstem neural projections to the cochlear nuclei, bilateral superior olivary nuclei, facial nerve nuclei, and facial nerve and its stapedius branch. Introduction The stapedius nerve is one of the branches of the facial nerve in the temporal bone. It supplies the stapedius muscle, which is responsible for the attenuation reflex that protects the inner ear from loud noises. The stapedius (acoustic) reflex (SR) test is useful in identifying the site of facial nerve injury. The return of the SR (acoustic) to normal after an injury is a good prognostic factor in the treatment of facial nerve palsy. Objective The aim of this study was to evaluate the effect of FNP on the SR (acoustic) response and determine the acoustic reflex threshold (ART) levels on the affected side. Material and method In this study, 70 patients, 3–7 years old, were screened. The study population consisted of 26 boys (37%) and 44 girls (63%). Follow-up tests were performed 3–18 months after the initial tests. Results Most patients in the study population had a negative SR (acoustic) response on the affected side. In other patients, mean ART values were statistically higher on the affected side. There was no statistically significant relationship between a reflex response and the time from the onset of facial nerve palsy. Discussion In the available literature, the SR (acoustic) testing is limited in determining whether or not the reflex is present without stimulus frequency or ART measurements. It is estimated that the reflex response is negative with ipsilateral stimulation on the affected side in 35–80% patients. Conclusions The SR (acoustic) is absent in most patients on the affected side. The ART value was statistically higher on the affected side. The SR (acoustic) response was statistically time independent.

Objectives The upper loudness limit of electrical stimulation in cochlear implant patients is sometimes set using electrically elicited stapedius reflex thresholds (eSRTs), especially in children for whom reporting skills may be limited. In unilateral cochlear implant patients, eSRT levels are measured typically in the contralateral unimplanted ear because the ability to measure eSRTs in the implanted ear is likely to be limited due to the cochlear implant surgery and consequential changes in middle ear dynamics. This practice is particularly limiting in the case of fitting bilaterally implanted pediatric cases because there is no unimplanted ear option to choose for eSRT measurement. The goal of this study was to identify an improved measurement protocol to increase the success of eSRT measurement in ipsilateral or contralateral or both implanted ears of pediatric cochlear implant recipients. This work hypothesizes that use of a higher probe frequency (e.g., 1000 Hz compared with the 226 Hz standard), which is closer to the mechanical middle ear resonant frequency, may be more effective in measuring middle ear muscle contraction in either ear. Design In the present study, eSRTs were measured using multiple probe frequencies (226, 678, and 1000 Hz) in the ipsilateral and contralateral ears of 19 children with unilateral Advanced Bionics (AB) cochlear implants (mean age = 8.6 years, SD = 2.29). An integrated middle ear analyzer designed by AB was used to elicit and detect stapedius reflexes and assign eSRT levels. In the integrated middle ear analyzer system, an Interacoustics Titan middle ear analyzer was used to perform middle ear measurements in synchrony with research software running on an AB Neptune speech processor, which controlled the delivery of electrical pulse trains at varying levels to the test subject. Changes in middle ear acoustic admittance following an electrical pulse train stimulus indicated the occurrence of an electrically elicited stapedius reflex. Results Of the 19 ears tested, ipsilateral eSRTs were successfully measured in 3 (16%), 4 (21%), and 7 (37%) ears using probe tones of 226, 678, and 1000 Hz, respectively. Contralateral eSRT levels were measured in 11 (58%), 13 (68%), and 13 (68%) ears using the three different probe frequencies, respectively. A significant difference was found in the incidence of successful eSRT measurement as a function of probe frequency in the ipsilateral ears with the greatest pair-wise difference between the 226 and 1000 Hz probe. A significant increase in contralateral eSRT measurement success as a function of probe frequency was not found. These findings are consistent with the idea that changes in middle ear mechanics, secondary to cochlear implant surgery, may interfere with the detection of stapedius muscle contraction in the ipsilateral middle ear. The best logistic, mixed-effects model of the occurrence of successful eSRT measures included ear of measurement and probe frequency as significant fixed effects. No significant differences in average eSRT levels were observed across ipsilateral and contralateral measurements or as a function of probe frequency. Conclusion Typically, measurement of stapedius reflexes is less successful in the implanted ears of cochlear implant recipients compared with measurements in the contralateral, unimplanted ear. The ability to measure eSRT levels ipsilaterally can be improved by using a higher probe frequency.

An electrical model of the human middle ear periphery is described in this work. It is used to observe more closely the potential effect of the most common pathologies affecting the middle ear on tinnitus generation. Each pathology is related to a change in the physical characteristics of the middle ear. The studied analog network is modified to take this change into consideration. Thus, the corresponding numerical values of its elements are borrowed from anatomical data, the nonlinearity due to the contraction of stapedius muscle and the stapes clipping displacement have also been taken into consideration. The simulated pathologies demonstrate, through the obtained frequency responses, the effect sound transmission. The proposed analysis allowed us to distinguish the potential alterations in the middle ear that can eventually causes tinnitus. This method is based on the frequency response analysis tools to discover and identify easily the middle ear behavior and subsequently an eventual tinnitus cause.

Facial nerve is the seventh paired cranial nerve. The main trunk of CN VII consists of motor fibers and the intermediate nerve which is a part of CN VII has parasympathetic and sensory fibers. One of the branches of CN VII is the chorda tympani which may play a significant role in responses related to normal Eustachian tube function. Other branches of CN VII, such as the greater petrosal nerve and tympanic branch, have motor fibers that open the Eustachian tube, which connects the tympanic cavity with the nasopharynx. It is located at the base of the cranium and consists of bone and cartilage. To evaluate the Eustachian tube and middle ear function, impedance audiometry (IA) is used. It is specifically used in hearing organ evaluation in children. The impedance audiometry consists of tympanometry, registration of reflex from the stapedius muscle and evaluation of Eustachian tube function. Tympanometry is an examination, evaluating response of the tympanic membrane and related structures of the middle ear on artificially induced pressure changes in the external auditory meatus. The main aim of the article is to evaluate middle ear function in children and adolescents with peripheral facial nerve palsy using results obtained in tympanometry and comparing the values for pressure, amplitude, and tympanometric curve gradient between the side with CV VII palsy (affected side) and the opposite side (healthy side).

The N1 and P2 event-related potentials (ERPs) are attenuated when the eliciting sounds coincide with our own actions. Although this ERP attenuation could be caused by central processes, it may also reflect a peripheral mechanism: the coactivation of the stapedius muscle with the task-relevant effector, which reduces signal transmission efficiency in the middle ear, reducing the effective intensity of concurrently presented tones, which, in turn, elicit lower amplitude auditory ERPs. Because stapedius muscle contraction attenuates frequencies below 2 kHz, no attenuation should occur at frequencies above 2 kHz. A self-induced tone paradigm was administered with 0.5, 2.0, and 8.0 kHz pure tones. Self-induced tones elicited attenuated N1 and P2 ERPs, but the magnitude of attenuation was not affected by tone frequency. This result does not support the hypothesis that ERP attenuation to self-induced tones are caused by stapedius muscle contractions.

Power-based procedures are described to measure acoustic stapedius-muscle reflex threshold and supra-threshold responses in human adult and infant ears at frequencies from 0.2 to 8 kHz. The stimulus set included five clicks in which four pulsed activators were placed between each pair of clicks, with each stimulus set separated from the next by 0.79 s to allow for reflex decay. Each click response was used to detect the presence of reflex effects across frequency that were elicited by a pulsed broadband-noise or tonal activator in the ipsilateral or contralateral test ear. Acoustic reflex shifts were quantified in terms of the difference in absorbed sound power between the initial baseline click and the later four clicks in each set. Acoustic reflex shifts were measured over a 40-dB range of pulsed activators, and the acoustic reflex threshold was objectively calculated using a maximum 10 likelihood procedure. To illustrate the principles underlying these new reflex tests, reflex shifts in absorbed sound power and absorbance are presented for data acquired in an adult ear with normal hearing and in two infant ears in the initial and follow-up newborn hearing screening exams, one with normal hearing and the other with a conductive hearing loss. The use of absorbed sound power was helpful in classifying an acoustic reflex shift as present or absent. The resulting reflex tests are in use in a large study of wideband clinical diagnosis and monitoring of middle-ear and cochlear function in infant and adult ears.

The results suggested mercury (Hg)-induced anomalies in the brainstem-mediated acoustic stapedius muscle reflex in children.Hg exposure has been associated with hearing impairment and brainstem anomalies. Acoustic stapedius reflex (ASR) thresholds, growth functions, decay/adaptation times, and behavioral auditory thresholds were used to screen Andean children and adults for Hg-induced auditory brainstem and facial nerve impairment.Fifty-one participants, which included 22 children (aged 6-17 years) and 29 adults (aged 19-83 years) living in gold mining areas of Ecuador where Hg is widely used in amalgamation, were screened using ASR immittance procedures.Mean blood mercury (HgB) level in the children was 15.6 μg/L (SD, 21.3; median, 7 μg/L; range, 2.0-89 μg/L), and in the adults 8.5 μg/L (SD, 7.1; median, 6 μg/L; range, 2.0-32 μg/L). Mean contralateral ASR thresholds (ASRT) for the screening frequency of 2000 Hz in the children (39 ears) was 92.9 dB HL (SD, 6.1; range, 80-105 dB HL), and in the adults (53 ears) 90.0 dB HL (SD, 6.4; range, 65-105 dB HL). The ASRT in the children increased significantly with HgB level (rho = 0.433; p = 0.008).

The purpose of this study was to investigate the neurological integrity and physiological status of the auditory brainstem tracts and nuclei in children with chronic lead (Pb) exposure using non-invasive acoustic stapedius reflex (ASR) measurements of afferent and efferent-neuromuscular auditory function. Following audiological examinations, uncrossed (ipsilateral) and crossed (contralateral) brainstem ASR responses were evoked by pure tone (500, 1000, and 2000 Hz), and broadband noise (bandwidth: 125-4000 Hz) stimulus activators. The ASR threshold (ASRT), amplitude growth, and decay/fatigue were measured by conventional clinical middle ear immittance methods in a group of Andean children (age range: 2-18 years) with a history of chronic environmental Pb exposure from occupational Pb glazing. Blood lead (PbB) levels of the study group (n = 117) ranged from 4.0 to 83.7 μg/dL with a mean PbB level of 33.5 μg/dL (SD: 23.6; median: 33.0: CDC III Classification). The PbB distribution data indicated that 77.8% (n = 91) of the children had PbB levels greater than the CDC action line of 10 μg/dL. Repeatable, normal ASRTs were elicited for ipsilateral (mean: < 90 dB HL) and contralateral (mean: < 97 dB HL) stimulation for each acoustic activator. Spearman Rho correlation analysis indicated no significant association between PbB level and ipsilateral or contralateral ASRT for any of the stimulus activators. The ASR amplitude growth results showed typical growth functions with no Pb-associated aberrations. No statistical association was found between ASR decay/adaptation (ASRD) and PbB level for any of the stimulus activators. The results of stapedius muscle reflex testing using several stimulus activators showed no significant relationship between PbB level and the physiological integrity of the auditory brainstem mediated ASR responses in children with chronic Pb exposure and elevated PbB levels.

The stapedius muscle constitutes one of the efferent feedback pathways to the auditory periphery, which can be elicited by sound in either ear. Contraction of the stapedius muscle causes attenuation of the low frequency component of the input signal by increasing the stiffness of the ossicular chain. The effects of the acoustic reflex have been investigated for many years and a number of hypotheses have been proposed. Two of these hypotheses are: (1) that this reflex helps to protect the ear from acoustic injuries and (2) that it aids in the control of masking. However, there is still significant controversy as to its functional significance. In the present paper, possible functional roles of the stapedius muscle are discussed based on the physiological properties of the acoustic reflex of this muscle.

Hydranencephaly is a rare condition in which the cerebral hemispheres are absent at birth and are replaced by membranous sacs in a cerebrospinal fluid-filled cranium. Surviving hydranencephalic patients have a functional brainstem and possible remnants of the cerebral cortex. This case report examines hearing function and the integrity of the brainstem mediated stapedius muscle reflex (SMR) in an adult with hydranencephaly, using middle ear impedance change measures. The brainstem mediated ipsilateral and cross-brainstem contralateral SMRs were elicited in the right ear at normal threshold levels for noise bands of 0.25-1.0, 1.0-4.0, and 0.25-4.0 kHz (broadband), and at the sinusoidal frequencies of 0.5, 1.0, and 2.0 kHz. The ipsilateral and contralateral SMR decay rates were normal. The growth in the SMR amplitude in response to noise and pure tone stimuli from threshold to saturation over a 15-20 dB range was normal and showed essentially sigmoidal curves. The normal ipsilateral and crossed brainstem contralateral electrophysiological SMR in this hydranencephalic patient demonstrated the preservation of peripheral hearing reception and functional brainstem auditory afferent and efferent tracts and nuclei in the absence of corticofugal influence.

Summary Moebius Sequence has been described as a pathology which involves the VI and VII cranial nerves, causing facial palsy. Acoustic reflexes are elicited by a high intensity stimulation of the stapedius and the tensor tympani muscles. The VII cranial nerve is responsible for innervating the stapedius muscle. No acoustic reflexes are expected for individuals with this Sequence. Aim: To describe immittance findings in a series of individuals with Moebius Sequence. Materials and Methods: We had 17 individuals with Moebius Sequence of both gender, with age raging from 3 to 13 years, who were submitted to otoscopy and immittance measures. Results: The results of this study indicated a Type A tympanometry in the majority of the analyzed ears, demonstrating normal function of the stapedius muscle. For the contralateral acoustic reflexes we observed it present in 50% of the ears. The ipsilateral acoustic reflexes were absent in the majority of the ears. Conclusion: The results of the acoustic reflexes suggested that this measure could help in the prognosis of VII cranial nerve lesions, since half of the individual presented those reflexes.

Ipsilateral and contralateral stapedius muscle contractions were studied as functions of the sound pressure level (SPL) and duration of 2-kHz tone bursts. The study complemented a preceding study in which temporal summation of stapedius muscle contractions produced by pairs of short tone bursts was determined and analyzed. The muscle contractions were determined indirectly by measuring changes in the acoustic impedance they produced at the tympanic membrane. The data for the stapedius muscle contraction as a function of tone-burst duration were derived from another study and analyzed in part with the help of the SPL functions obtained in the present study. According to the experimental results, the stapedius muscle contraction produced by contralateral stimulation follows a compressive power function paralleling both the cochlear output and loudness functions. The ipsilateral contraction follows an expansive power function. Mathematical analysis showed that the muscle tension due to contralateral stimulation increases with tone duration approximately according to the characteristic of a linear integrator having an exponentially decaying memory with a time constant that increases with SPL from ≈200 to 370 msec. The simple relationship appears to be possible because of mutual cancellation of neural-processing characteristics preceding and following the temporal integration.

Air conduction (AC) versus bone conduction (BC) loudness balance testing was conducted at frequencies of 0.25, 0.5, 0.75, 1, 2, and 4 kHz for two groups: 23 normal hearing subjects and eight subjects with a mild to moderate pure sensorineural hearing loss. Narrow-band noise was presented interchangeably between earphones and a bone transducer fitted to the subjects. Loudness matching was carried out at each frequency and at the levels 30-80 dB hearing level (HL) (10 dB steps) in the following manner: the sound pressure from the earphones was fixed and the subject adjusted the output level of the bone transducer for equal loudness by bracketing the standard. The results revealed somewhat different loudness functions for AC and BC sound with a 6-10 dB difference in the AC and BC loudness functions for the normal hearing group over the dynamic range 30-80 dB HL at the frequencies 250-750 Hz. At the higher frequencies, 1-4 kHz, the difference was only 4-5 dB over the same dynamic range. Similar results were obtained for the sensorineural hearing-impaired group. The difference between the AC and the BC loudness functions may originate from changes with level of the AC sound path, e.g. contraction of the stapedius muscle, but also distortion from the bone transducer and tactile stimulation could have contributed to the results seen.

Previous studies have demonstrated a correlation between cochlear implant recipients' comfort levels (C level, upper limit of dynamic range of stimulation) and the contralateral electrical stapedius reflex (ESR) threshold, detected by acoustic impedance change. However, the utility of the approach is limited because many recipients have no detectable impedance change. The goals of this study were to investigate the utility of the stapedial electromyogram (EMG) for estimating onset and strength of the ESR. Ketamine-anesthetized guinea pigs were implanted with Nucleus electrode arrays and stimulated with biphasic current pulse trains (250 pps) via a Cochlear Corporation CI24M stimulator. Typical EMG recordings (obtained with bipolar microwire electrodes) contained easily detectable unit potentials up to 300 microV in amplitude. Growth response curves (obtained from threshold-crossing counts or rms of the EMG signal) were typically monotonic with dynamic ranges spanning 700 microA or 8 dB. Based on adaptation and temporal properties, the stimulus protocol (500 ms duration with 4-5 s interstimulus intervals) was adequate for producing independent responses. The data presented are consistent with ESR characteristics (acoustic impedance technique) of cochlear implant recipients and with EMG properties of acoustically stimulated guinea pigs. Use of the EMG for characterizing the ESR may eventually be applied to human cochlear implant recipients as a guide in setting the upper limit of the dynamic range.

Previous studies of the relationship between the intraoperatively obtained electrically evoked stapedius reflex threshold (ESRT) and the maximum comfortable loudness (MCL) values used to program the speech processor of cochlear implant (CI) patients have indicated that ESRTs provide an inexact estimate of MCL. In order to determine whether this estimate might be more exact for electrodes in different parts of the electrode array, we studied the electrode-specific relationships between ESRT and MCL in 29 Med-El Combi 40+ and 25 Nucleus Cl 24 M patients after first fitting of the speech processor and 2 and 6 months later. The MCL values were mostly less than the ESRT values, but increased progressively over the first 6 months, reaching 83% and 72% of the ESRT values, on average, across all electrodes for the Med-El and Nucleus systems respectively. The contraction of the stapedius muscle during the effort of vocalization was examined by measurement of acoustic compliance in subjects who had undergone laryngectomy or tracheostomy. No significant level of persistent compliance change was recorded in any of the subjects, indicating the absence of effective contraction of the stapedius during the vocalization effort. In two subjects who use an electrolarynx, although no significant compliance change resulted from the simple vocalization effort without the electrolarynx (no actual vocalized sound), a remarkable level of persistent compliance change was observed during the vocalization using the electrolarynx. These results seem to indicate that a simple vocalization effort without actual voice cannot elicit effective contraction of the middle ear muscles, and that sound generation during vocalization is essential for effective contraction of the middle ear muscles during vocalization, at least in some human subjects.

We have developed a procedure to measure the averaged stapedius muscle reflex in response to electrical stimulation (AESR) with a cochlear implant. The AESR, as activated by ipsilateral stimulation with a cochlear implant, was investigated intra-operatively in a series of 12 children. After the cochlear implant was placed into the cochlea and secured by the surgeon, an electromyographic (EMG) needle electrode was gently placed into the stapedius muscle. During stimulation of the cochlear implant, the stapedius reflex was monitored visually (VESR) and as an averaged EMG response (AESR). Consistent AESRs were obtained in 7 of the 12 children. These measures provide a method to obtain stapedius muscle responses that may be helpful in the programming oi young children with cochlear implants. Comparative post-operative measures were also obtained, including behavioral threshold levels, behavioral comfort levels, and the contralateral stapedius reflex threshold (ESR) on selected electrodes.

During surgery for otosclerosis, it is common for the surgeon to cut the stapedius tendon. Even without reconstruction of the tendon, the results of this kind of surgery are particularly satisfactory. The stapedial reflex is more important for improving intelligibility of speech in the presence of background noise than for protection against hazardous levels of noise. An intact stapedial reflex improves ones ability to follow speech in the presence of background noise. This report will present a technique to reconstruct the stapedius tendon, along with the results obtained when adding this procedure to stapedotomy surgery.

The effect of the acoustic middle ear reflex (MER) was quantified using electrodes chronically implanted in the middle ears of rats. Cochlear microphonics (CM) and middle ear muscle EMG were measured under light Ketamin anesthesia after stimulation with tone pulses of 5-20 kHz ranging between 75 and 120 dB SPL. With increasing intensity, the CM measured before the onset of the MER increased to a maximum amplitude and then decreased with higher SPLs. At 10 kHz this maximum was reached at 95 dB SPL, for other stimulus frequencies at higher SPLs. After a latency of 10-20 ms, CM to 10 kHz stimuli of 80-95 dB SPL were decreased by the attenuating action of the MER. The lowest threshold of the MER was also measured at 10 kHz (77 dB SPL in the mean). To stimuli greater than 100 dB SPL after a latency of 6-10 ms, the CM amplitude was increased. That this CM increase to intense stimuli is caused by the action of the MER was confirmed by control experiments such as cutting the tendons of the middle ear muscles. The CM decrease to stimuli below 100 dB SPL, as well as the increase to very intense stimuli, can be explained by sound attenuation caused by the MER, together with the nonlinear dependence of CM amplitude on stimulus level. The observed shift of the maxima of the CM input-output function by the MER to higher stimulus levels probably indicates an increase of the dynamic range of the ear.

The metabolic disorder Pompe disease mainly affects the skeletal muscle in adults. The hearing impairment due to stapedius muscle involvement in adult patients is not known.The frequency, severity, and type of hearing impairment was characterized prospectively using pure-tone audiometry, tympanometry, stapedial reflexes, otoacoustic emissions, and brainstem-evoked response audiometry in adult patients on enzyme replacement therapy for genetically confirmed Pompe disease.11 adult patients (median age: 47 years, range: 22-71).Four patients complained about subjective hearing disturbances. Using World Health Organization definition of hearing impairment, abnormal hearing thresholds resulting in mild hearing loss were found in 36% of patients. Compared to normative data (ISO 7029), the hearing threshold was below the median in all but three ears. Stapedial reflexes could not be elicited ipsilateral in 18% and contralateral in 36%. Auditory brainstem responses showed no retrocochlear pathology.The prevalence of hearing loss slightly exceeded the normative data of the general population. Consistent with previous studies the hearing impairment was usually mild. The percentage of pathological stapedial reflexes exceeded that of matched control subjects and suggests a selective involvement of the stapedius muscle, potentially as a sequela of Pompe disease.

In 1975, the threshold hearing levels (HLs) of 83 Swedish and British pop/rock musicians were examined with pure tone audiometry. At that time we found 13% of the musicians with a hearing loss > 20 dB HL at a high frequency pure tone average (3, 4, 6, and 8 kHz). The aim of the present investigation was to conduct an audiometric follow-up study of these musicians 16 years after the first examination. Fifty-three of the 83 pop/rock musicians were retested in 1991 to 1992. They also filled in a questionnaire concerning occupational noise activities, leisure noise activities, general health problems and subjective symptoms related to loud music exposure. The median pure tone audiogram was within 20 dB HL at all test frequencies. On an individual basis, 63% had normal high high frequency pure tone average scores and another 15% had a very limited high frequency hearing loss (< or = 25 dB HL). It seems surprising that pop/rock musicians after performing for 26 years have such well-preserved hearing. There might be a protective effect by the generally positive attitude from the musicians toward their performance and their audience. Regarding the loud and more or less continuous sound levels over 85 dBA there might also be a protective effect from a continuous contraction of the stapedius muscle.

Myoclonus of the middle ear is a very rare condition. We present the case of a 20-year-old soldier who was exposed to intense artillery noise during a fierce battle and immediately afterward complained of an incapacitating tinnitus in both ears. Microscopic examination of the ears demonstrated rapid rhythmic movements of the tympanic membrane coinciding with the tinnitus, which was also easily heard by the examiner. There was no evidence of palatal myoclonus, and thus the diagnosis of middle ear myoclonus was made. Exploratory tympanotomy confirmed the diagnosis of stapedial muscle myoclonus. Bilateral sectioning of the stapedial tendons brought immediate relief of the tinnitus to the patient. We review the literature of middle ear myoclonus and discuss the symptoms, evaluation, and management of this rare condition.

Animal studies have shown that toluene can cause hearing loss and can exacerbate the effects of noise by inhibiting the middle ear acoustic reflex. In this investigation, carried out in Long-Evans rats, the tensor tympani tendon was cutoff and the stapedius muscle was electrocoagulated in one or both middle ears. Rat hearing was evaluated by measuring cubic distortion otoacoustic emissions (2f1-f2; f1 5 8000 Hz; f2 5 9600 Hz; f1/f2 5 1.2) prior to, during, and after activation of the olivocochlear (OC) reflex. A band noise centered at 4 kHz was used as suppressor noise. It was delivered contralaterally to decrease 2f1-f2 amplitude. The strength of the inner ear acoustic reflex was tested by increasing contralateral noise intensity, and toluene injected into the carotid artery was used to study physiological efficacy. Results showed that the protective effect of the OC reflex is intensity dependent. In addition, the OC reflex was found to be less sensitive to toluene than the middle ear acoustic reflex. This may be because the efferent neurons involved in inner ear and middle ear reflexes are located differently. In conclusion, the synergistic effects on hearing of co-exposure to noise and aromatic solvents are because of solvents depressing the central nuclei, which mainly drive the middle ear acoustic reflex.

The acoustic stapedius reflex (ASR) is the sound-evoked contraction of the stapedius muscle. The ASR is mediated by a neural network, receiving its afferent input from the VIIIth cranial (auditory) nerve and sending its efferent output to the VIIth cranial (facial) nerve. Several centers in the brain stem comprise the central portion of the reflex pathway. The stapedius muscle in each ear responds to ipsilateral, contralateral, and binaural stimulation. The normal ASR threshold is 90 to 95 dB SPL for tones and 70 to 75 dB SPL for wideband noise. In ears with sensorineural hearing loss, ASR thresholds are elevated more for wideband noise than for tones. These bandwidth-related changes form the basis for detection of hearing loss from ASR data. A number of methods have been proposed which differ in the nature of the outcome. Some try to quantitatively estimate hearing loss, whereas others categorize outcomes as pass or fail. In general, methods that attempt quantitative estimation of hearing loss do so at the expense of higher error rates. With adequate recording techniques, the ASR has been shown to effectively detect hearing losses exceeding 30 dB in adult subjects. Although ASR recording in newborns requires a higher probe frequency than the commonly used 226 Hz, ASR thresholds in newborns appear to be similar to those in adults. Limited information exists on the use of the ASR for detection of hearing loss in the infant population. With adequate attention to methodological requirements of testing infants, the ASR may offer a cost-effective method for the early detection of hearing loss.

1. Outer hair cells (OHCs) have active micromechanical properties that are thought to be the origin of evoked otoacoustic emissions (EOAEs). In the present study, click-evoked otoacoustic emissions were recorded in humans with or without various contralateral acoustic stimulations. A previous study, concentrating on contralateral stimulation with broadband noise, had shown a decrease of the EOAE amplitude in humans. Results support a role for the efferent system in cochlear mechanics; indeed, medial efferent neurons of the olivocochlear bundle terminate on the OHCs. To obtain a better understanding of the medial efferent system functioning in humans, the present study looked at the contralateral suppressive effect as a function of stimulus parameters. 2. The study of the input-output function of the EOAE amplitude with and without a 50-dB SPL contralateral broadband noise showed that the suppressive effect was equivalent to a mean reduction of 3.77 dB. 3. For the EOAEs to tone pips, the contralateral suppressive effect was strongest when the contralateral ear stimuli were narrow bands that were centered around the central EOAE frequency. This frequency specificity disappeared for contralateral narrow band noise levels greater than 50 dB SPL. 4. The contralateral suppressive effect was also observed with transient contralateral sounds (nonfiltered clicks). Significant reductions of the EOAE amplitude were seen with contralateral click levels as low as 17.5 dB SL. Above this level, the EOAE amplitude decreased as the contralateral stimulus level increased. This effect was still present in subjects without any stapedial reflex, but absent in total unilateral hearing-loss subjects. Therefore this suppressive effect is unlikely to be due to alteration of the middle ear function or to transcranially conducted sound. 5. When the contralateral interclick interval exceeded 14.2 ms. the suppressive effect was smaller. With contralateral stimulus level maintained subjectively constant, the effect was found to disappear when the interclick interval was greater than 49.9 ms. A saturation of the contralateral suppressive effect was observed for click rates greater than 70/s (interclick interval less than 14.2 ms). 6. Our study confirms and specifies the contralateral sound suppression effect on cochlear mechanisms in humans, assessing the equivalent reduction, showing a frequency specificity and extending these findings to contralateral transient sounds. Any influence of the acoustic crosstalk was eliminated. A role played by middle ear muscles cannot be absolutely ruled out but is not necessary to produce such a contralateral suppressive effect (the effect being found in subjects after surgical removal of the stapedius muscle) and could not explain the frequency specificity.(ABSTRACT TRUNCATED AT 400 WORDS)

Threshold and suprathreshold temporal integration (TI) effects were studied in the crossed and uncrossed human acoustic stapedius reflex. Changes in reflex threshold were compared at four stimulus durations and for six threshold response criteria; suprathreshold effects were similarly compared at four stimulus durations and at five stimulus sound-pressure levels. Our results showed that reflex thresholds were significantly lower for the uncrossed condition and for longer duration stimuli. Both effects were dependent on threshold response criteria. Threshold TI measurements in both crossed and uncrossed conditions were equivalent at low criterion levels (0%-4%, p greater than 0.05) but were significantly larger in the crossed condition at higher criterion levels (5%-6%, p less than 0.05). Prominent suprathreshold effects also occurred. The main effect was characterized by significantly larger reflex magnitudes in the uncrossed condition. Duration dependent slope differences in magnitude intensity functions (MIFs) were also observed and were characterized by steeper slopes for longer duration stimuli. Saturation effects and/or intensity-dependent slope decelerations in MIFs were predominant in the uncrossed condition and for stimulus durations exceeding 20 ms. These data provide further quantitative evidence for asymmetric threshold and suprathreshold response properties of the crossed and uncrossed human acoustic stapedius reflex and demonstrate the dependence of these effects on stimuli of different durations.

There are few physiological data available on the origin and nature of tinnitus. It is not even known whether tinnitus associated with cochlear pathology is a manifestation of increased or decreased activity in the cochlear nerve. In previous investigations of cochlear pathology, the spontaneous neural activity has generally been found to be depressed. In the present experiments, an animal model has been established by the administration of sodium salicylate in doses producing blood concentrations that evoke tinnitus in humans. Under these conditions, changes occur in cochlear nerve-fibre thresholds and tuning, similar to those obtained in other types of cochlear pathology. However, under salicylate, the distribution of spontaneous discharge shifts significantly to higher rates than normal. These changes are accompanied in some, but not all, fibres by changes in the temporal patterns of discharge suggestive of excitation. In the second animal model studied, a normal guinea-pig that had a naturally occurring continuous tonal emission, analogous to that recently recorded in human "physiological" tinnitus, was investigated in detail. The emitted signal was recorded in the ear-canal acoustic pressure and in the round-window potential. Several lines of evidence point to the signal as being cochlear in origin, including: its resistance to muscular paralysis and section of the stapedius muscle; the effects of changes in middle-ear pressure; its reversible elimination by hypoxia; and its suppression by tones of higher frequency.

The electrical stapedius reflex (ESR) threshold, detected by eardrum acoustic impedance change, is strongly correlated with cochlear implant recipients' behavioral comfort levels. However reports suggest acoustic impedance changes are not detectable in 30-40% of patients. The goals of this study were to develop an animal model and investigate the characteristics of the stapedius muscle electromyogram (SEMG) elicited by a cochlear implant, as an alternative measure of ESR activation. Bipolar tungsten micro wire electrodes recorded the SEMG signal from the stapedius muscle of 6 rats. The cochlea was implanted with a multichannel intracochlear electrode that delivered biphasic electrical pulses. Maximum SEMG potentials were 20-500 /spl mu/V (mean: 174 /spl mu/V) or 8-42 dB SNR (mean: 24 dB). The dynamic range of the responses that reached saturation were approximately 10 dB, with threshold inversely dependent on pulse-width and electrode separation. The electrical brainstem response (EABR) threshold was 5.6 dB lower than the ESR threshold on average, but the standard deviation was relatively high (2.4 dB), suggesting that these two signals could provide independent information for objective cochlear implant fitting. Post-operative SEMGs were recorded in several animals; including one animal for up to 63 days. The results suggest the overall feasibility of the approach for objective cochlear implant fitting.

A Seqüência de Moebiüs tem sido descrita como uma patologia envolvendo paralisia do VI e VII pares cranianos. O reflexo acústico é resposta à estimulação acústica de forte intensidade, através da contração dos músculos estapédio e tensor do tímpano. O VII par craniano é responsável pela inervação do músculo estapédio. Espera-se ausência de reflexos acústicos em alguns pacientes com a Seqüência. OBJETIVO: Descrever uma série de casos do resultado imitanciométrico dos portadores da Seqüência de Moebiüs. MATERIAL E MÉTODO: Participaram desta pesquisa 17 indivíduos identificados com Seqüência de Moebiüs, de ambos os sexos com idade variando entre 3 e 13 anos, todos submetidos à meatoscopia e imitanciometria. RESULTADOS: Os resultados indicaram timpanogramas do Tipo A na maioria das orelhas, demonstrando resultados compatíveis com funcionamento normal do músculo estapédio. Para o reflexo acústico contralateral, observou-se que metade das orelhas analisadas apresentou ausência de respostas. A outra metade apresentou reflexos acústicos contralaterais presentes. Os resultados do reflexo acústico ipsilateral se encontraram ausentes na maioria dos casos. CONCLUSÃO: Os resultados dos reflexos acústicos sugerem que esta medição pode demonstrar auxílio no prognóstico do comprometimento da lesão do VII par craniano, uma vez que metade dos participantes apresentou reflexos acústicos presentes. Moebius Sequence has been described as a pathology which involves the VI and VII cranial nerves, causing facial palsy. Acoustic reflexes are elicited by a high intensity stimulation of the stapedius and the tensor tympani muscles. The VII cranial nerve is responsible for innervating the stapedius muscle. No acoustic reflexes are expected for individuals with this Sequence. AIM: To describe immittance findings in a series of individuals with Moebius Sequence. MATERIALS AND METHODS: We had 17 individuals with Moebius Sequence of both gender, with age raging from 3 to 13 years, who were submitted to otoscopy and immittance measures. RESULTS: The results of this study indicated a Type A tympanometry in the majority of the analyzed ears, demonstrating normal function of the stapedius muscle. For the contralateral acoustic reflexes we observed it present in 50% of the ears. The ipsilateral acoustic reflexes were absent in the majority of the ears. CONCLUSION: The results of the acoustic reflexes suggested that this measure could help in the prognosis of VII cranial nerve lesions, since half of the individual presented those reflexes.

It is known that stapedius reflex (SR) shows bilateral interaction and that its center is located in the brainstem. In the present study, the pattern of SR was compared in two different conditions, either with binaural acoustic stimuli or with monaural stimuli using the impedance measurement method. When the sound source was placed in front of the head model, the amplitude of impedance change was higher in the binaural hearing condition than in the monaural condition, suggesting bilateral superposition. When the sound source was placed laterally to the head model, the amplitude of impedance change was higher in the ipsilateral ear than in the contralateral ear in the binaural condition. In the monaural condition, the amplitude of impedance change in the ipsilateral ear was as high as that obtained in the ipsilateral ear in the binaural condition, whereas it was markedly low in the contralateral ear. It is assumed that sense of sound direction cannot be well maintained without binaural interaction of SR.

A 55 year old female with idiopathic Tullio phenomenon is presented. Binocular, scleral search eye coil recordings demonstrated a predominantly torsional left-beating and vertical down-beating nystagmus in response to sound intensities over 100 dB HL to the left ear, increasing in amplitude and slow phase velocity with sound intensity and removal of visual fixation. The vertical ocular movement was conjugate, i.e. without skew deviation. Neuro-imaging, all other neuro-otological features, including ipsilateral-contralateral stapedius muscle reflexes, and surgical exploration of the middle ear, were normal. Click-evoked vestibulo-collic potentials were normal from the right ear but showed low threshold (70 dB) and increased amplitude from the left. There was no evidence that the Tullio phenomenon in this patient arises from stapes footplate hypermobility. The findings suggest that some cases of the Tullio phenomenon may be due to a hyperexcitability of the normal vestibular response to sound.

Objective - The reversed ipsilateral acoustic reflex (RIAR) has been the subject of few studies in the past literature and the nature of this "reflex" response is not yet completely understood. It can be observed in different diseases, all of which result from inactivity of the stapedial reflex (SR). We aimed to investigate the occurrence of the RIAR in different diseases. Material and methods - An extensive clinical investigation of the RIAR was carried out. Results - The RIAR was found to be present in 100% of ears lacking an effective SR arc. Its amplitude is modulated by the air pressure level in the external ear canal whilst its sign is constantly positive and never reverses. Conclusion - Kinetic analysis of the RIAR suggests the existence of a single underlying phenomenon while the interaction of the RIAR with the SR may explain the complex morphology frequently observed in the normal ipsilateral acoustic reflex.

Increased intracranial pressure (ICP) is known to affect the phases and levels of lower-frequency distortion-product otoacoustic emissions (DPOAE) in a characteristic: manner suggestive of an increase in the stiffness of the stapes system. likely in relation to an attendant increased intracochlear pressure (ICoP). DPOAEs may thus provide an easy non-invasive means of gaining access to the otherwise elusive ICoP. However, the mechanisms by which DPOAEs actually relate to ICoP are unclear and may involve changes in the stiffness of the annular ligament, stapedius muscle and even some indirect contributions of other parts of the middle ear such as the tensor tympani. A systematic study of the role of each middle-ear element on ICoP-to-DPOAE relationships as a function of frequency was undertaken in gerbils under direct control of ICP via an intracranial catheter (from 0 to 500 daPa). After the bulla was widely opened, the tendons of the stapedius and tensor tympani muscles were severed in turn. A standard electroacoustic analog model of the middle ear was used for predicting the forward and reverse middle-ear transfer-functions changes under different experimental manipulations and their consequences on DPOAEs. The observed DPOAE changes chiefly consisted in a phase-lead peaking around 2.15 kHz in closed-bulla, and 1.2 kHz in open-bulla conditions. It was proportional to ICP increase provided ICP exceeded a threshold of about 50 daPa. The profiles of DPOAE shifts matched those derived from the premise that ICoP mainly induced a change in the stiffness of the stapes system. The possible involvement of the stapedius muscle was ruled out by the absence of any effect of cutting its tendon so that the intrinsically non-linear stiffness of the annular ligament must have been the main factor. A relatively minor contribution from the tensor tympani was observed, possibly in relation to the detection of ICoP-induced displacement of the ossicular chain by neuromuscular spindles. The intra-operative electrical auditory brainstem response (EABR), electrical stapedius reflex threshold (ESRT) and the early post-operative behavioural threshold level (T-level) were recorded in five children undergoing cochlear re-implantation. The aim of the study was to assess objectively the effect of re-implantation on intra-operative objective measures and to investigate neuronal function. The children were aged between 2.06 years and 4.5 years at first implantation. Following failure of the first device, re-implantation was carried out 1.42-5.52 years later. Characteristics of the EABR and ESRT across the electrode array were typical of the expected pattern of responses on both occasions. In particular, the slopes of the amplitude input/output (I/O) functions for wave eV of the intraoperative EABR were similar for both the first and second implants even though absolute thresholds were generally elevated after re-implantation. This elevation in intra-operative threshold was more pronounced than the change in early post-operative behavioural threshold level for electrical stimulation (T-level). Our findings confirm a high level of neuronal survival after re-implantation. Threshold of the intra-operative EABR at the time of re-implantation greatly underestimates the sensitivity of the subsequent early post-operative T-levels.

Evoked otoacoustic emissions (EOAEs) are generated within the cochlea in response to external sounds, and they can be acoustically detected in the external auditory meatus after backward propagation through the middle ear. In addition to being used to probe the cochlear mechanisms, they are expected to be sensitive to minute changes in middle ear impedance. Systematic measurements of the changes in the vectorial components of EOAEs were carried out after various manipulations of the human middle ear in order to characterize the influence of stiffness and inertia of the stapes and tympanic-membrane systems. For this purpose, stapedius muscle contractions were elicited by high-level contralateral sound, controlled changes in middle ear pressure (range +/- 100 daPa) were produced and the tympanic membrane was loaded with water droplets. A computer model of the middle ear network was implemented using a standard lumped-element electric: analog of the middle ear (Zwislocki's model). Forward and backward transmission changes were simulated and model predictions were compared to experimental data. Apart from the case of positive middle ear pressures. a close qualitative correspondence was found between model and real-ear results. Each of the effects was characterized by a unique pattern of phase and magnitude changes as a function of frequency, in relation to the mechanical characteristics of the involved subsystem (i.e. stapes stiffness, tympanic-membrane stiffness or mass) and its resonance properties. Owing to their high sensitivity; EOAEs could be helpful for an accurate-individual multifrequency analysis of middle ear impedance by comparisons under rest and lest conditions. The medial olivocochlear efferent bundle is the key element of a bilateral efferent reflex activated by sound in either ear and acting directly on cochlear outer hair cells (OHC) via numerous cholinegic synapses. It probably contributes to regulating the mechanical activity of the cochlea Otoacoustic emissions, being sounds emitted by the cochlea as a reflection of its activity and suppressed by efferent activation, are increasingly considered to be the privileged tool for a noninvasive assessment of the efferent reflex. However, confounding effects on otoacoustic emissions can occur. A primary influence is middle-ear muscle reflex activation, which shares common features with the effects of cochlear efferent activation. We report a systematic comparison of the responses of human otoacoustic emissions to efferent activation by low-level noise in the contralateral ear to various middle-ear manipulations (reflex contractions of the stapedius muscle induced by high-level contralateral noise, moderate middle-ear pressure changes). The profiles of level and phase changes of otoacoustic emissions as a function of frequency were highly specific to the origin of the effects. The changes induced by middle-ear manipulations matched the predictions computed from a standard lumped-element middle-ear model, with one or two peaks around the resonance frequency(ies) of the involved subsystem, stapes or tympanic membrane. In contrast, the efferent effect was completely different, exhibiting a broadband-level suppression associated with a small phase lead. We propose that a careful vector analysis of otoacoustic emission modifications enables the identification of the contribution of the efferent reflex without ambiguity even when it is mixed with middle-ear effects. Thereby, otoacoustic emissions can be used more reliably as noninvasive probes of efferent olivocochlear function. (C) 2000 Published by Elsevier Science B.V. All rights reserved.

Objectives: To review our intraoperative and post-operative testing protocol for cochlear implant patients. This study describes the methodologies and applications of a new technique called neural response telemetry (NRT) for the Nucleus CI24M cochlear implant system. NRT uses radiofrequency telemetry technology to measure the action potentials of the auditory nerve. Study Design: We have developed a specific protocol for intraoperative testing of the implant device before, during, and after implantation. This includes device integrity tests, visual detection of electrical stapedius muscle reflexes (VESR), and NRT. Methods: Our methodologies use the commercial software (Windows-based Diagnostic and Programming System [WIN-DPS] and NRT) for the Nucleus CI24M. We describe the details of our protocol used on all of the patients (14 adults and 14 children) who received CI24M implants at Mayo Clinic (Rochester, MN). Our protocol correlates the NRT threshold with the behavioral responses for each patient on at least four electrodes. Results: From August 1, 1998, to December 31,1998, we completed electrode integrity tests, NRT, and VESR testing intraoperatively on 12 patients with the Nucleus CI24M. We have measured normal implant function on all 28 of our CI24M patients with one exception. One of our children had a device failure after approximately 4 months as a result of head trauma. We have also obtained NRT results from an additional 10 patients postoperatively. Conclusions: The measurement of device and electrode array function is quite simple with the CI24M software. These measurements can be obtained intraoperatively as well as postoperatively. We conclude that VESR and NRT measurements can be very helpful in programming for patients with cochlear implants, especially children, because they provides us with target settings for the speech processor.

Familial conductive deafness is rare. This report confirms the existence of a lineage with congenital conductive hearing loss in 3 generations. The results of otologic evaluations, including pure-tone audiometry, tympanometry, acoustic reflex test, and liquid test, in 14 patients in this family were consistent with the findings of ossicular fixation in 10 patients. Tympanotomies performed in both ears of 4 patients indicated stapes ankylosis caused by ossification of the stapedius tendon. The patients gained normal hearing levels using a simple surgical procedure.

It is known that the threshold of an acoustically induced middle-ear-muscle (MEM) reflex can be lowered by the simultaneous presentation of a second tone (facilitator), which is presented to the ipsilateral or contralateral ear at a level below the acoustic reflex threshold (ART) of the facilitator itself ( Sesterhenn and Breuninger, 1976 ; Blood and Greenberg, 1981 ). In the present study, a primary elicitor and a facilitator were presented to the ear contralateral to that used for measurement of the acoustic reflex (AR), and the effects of changing frequencies and sound levels of the facilitator were investigated in human subjects with normal ears. The sound levels of facilitators, which caused a significant reduction of ART for the primary elicitors (facilitation thresholds), showed an asymmetrical pattern as a function of frequency of the facilitators. The facilitation thresholds tended to be lower when a facilitator with a frequency lower than the frequency of the elicitor (1 kHz) was used. In addition, effects of the elicitor on the masked thresholds of the facilitator were examined to observe the possible interaction between elicitor and facilitator from the viewpoint of ‘spread of excitation.’ The underlying mechanism of summation effects of two tones are discussed based on the possible input mechanism involved in the acoustically induced MEM reflex arc.

A new method for detecting the acoustic reflex that utilizes standard otoacoustic emissions recording techniques is introduced and discussed. Two successive identical tone bursts of 100 ms duration and 10 ms interstimulus interval are presented in the occluded ear canal at a repetition rate of one per second. If the acoustic reflex is elicited, the contraction of the stapedius muscle is delayed with respect to the onset of the first stimulus. Hence, the acoustic compliance in the ear canal decreases primarily during the second stimulus. The difference of the microphone signals produced by the two stimuli is computed and averaged across a certain number of repetitions of the sequence. The presentation level is increased until this difference is larger than -40 dB (with respect to the stimulus level) and if its signal-to-noise ratio exceeds 20 dB. For normal-hearing subjects, the acoustic reflex threshold measured with this method is on average 8 dB lower than in a standard clinical setup. In 5 out of the 10 tested hearing-impaired subjects, the new method could detect an acoustic reflex at one or more frequencies where no reflex was detected in the clinical setup.

The contraction of the stapedius muscle results in a frequency dependant attenuation of sound through the middle ear. Idiopathic facial nerve paralysis (Bell's palsy) usually results in a paralysis of the stapedius muscle. This prospective study included 119 patients presenting with Bell's palsy over an 18-month period. After applying exclusion criteria, 80 patients with normal pure-tone audiograms underwent speech audiometry. This was performed on presentation and after recovery of the facial nerve palsy. Fifty-six patients (70%) with an absent stapedius reflex showed marked 'roll-over' from means of 98-49% on their speech audiogram. This resolved completely with recovery of the facial nerve palsy and return of the stapedial reflex. To determine whether this effect was due primarily to the paralysis of the stapedius muscle or to an associated polyneuropathy, a second study on six normal patients was done. These patients had both ears (12 in total) tested with speech filtered to simulate a paralysed stapedius muscle. This resulted in a mean 'roll-over' from 90.9 to 59.9%. The magnitude of this roll-over (31%) was only two-thirds of that seen in Bell's palsy patients (49%) with the difference between these means statistically significant (P < 0.05). This suggests that Bell's palsy, usually considered a mononeuropathy, involves certain of the auditory fibres of the eighth nerve and is a polyneuropathy. Stapedius function is important in speech discrimination at higher levels of sound intensity such as speech in noise and severing the stapedius tendon in stapes surgery may affect speech discrimination after successful surgery.

Tympanograms were recorded during compression of cervical veins in 19 normal ears to investigate whether pressure elevation in the middle and inner ears affects acoustic impedance. Upon cervical compression, the peak pressure of the tympanogram shifted to the positive pressure side. The peak amplitude of the tympanogram also changed in a manner similar to that after stapedius muscle contraction. These results suggest that the impedance changes caused by cervical compression reflected changes in both perilymphatic pressure and middle ear pressure.

The role of middle ear muscles (MEMs) in the development of increased resistance to noise induced hearing loss (NIHL) was studied using monaural chinchillas. Animals with severed MEMs as well as those with intact MEMs were exposed to an octave band noise (OBN) centered at 0.5 kHz at 95 dB for six hours/day for ten consecutive days. Results indicated that animals with severed MEMs showed greater initial threshold shifts (TS) than the animals with intact MEMs. Both the groups showed a decrease in TS over the ten days of exposure. The subjects were given five days of recovery and then re-exposed to the same noise at 106 dB for 48 h. Permanent threshold shifts (PTS) in each group was compared against those in a control group exposed to the noise only at the higher level. Interestingly, both the ‘conditioned’ groups incurred substantially less PTS than the control group exposed only to the higher level.

Purpose: patients with insulin-dependent diabetes mellitus (IDDM) are especially susceptible to microangiopathic complications such as nephropathy, retinopathy, and neuropathy. Microangiopathic changes are also the most important findings in histopathologic studies of the inner ear and central nervous systems in diabetic subjects. No previous studies have measured acoustic-reflex latencies (ARL) or amplitudes (ARA) in patients with IDDM. ARL and ARA reflect the function of the acoustic-reflex arch. Furthermore, possible changes in the tympanic membrane, ossicular chain, and stapedius muscle may affect the shape of acoustic-reflex. Subjects and Methods: Acoustic-reflex thresholds, latencies, and amplitudes were studied in 53 patients with IDDM and 42 randomly selected nondiabetic control subjects, aged between 20 and 40 years, using the Madsen Model ZO 73 Impedance Bridge (Madsen Electronics, Copenhagen, Denmark). Subjects with an abnormal tympanic membrane, conductive hearing loss, and known cause for hearing impairment eg, noise damage, were excluded from the study. Results: There were no differences between control and diabetic subjects in the contralateral acoustic-reflex thresholds. In contrast, patients with IDDM had longer ARLs and decreased ARAs compared with those of control subjects. ARA amplitude had linear correlation with the amplitude of tympanogram, whereas ARL had no linear correlation with auditory brainstem latencies in the same study subjects. Acoustic-reflex responses in insulin-dependent diabetic patients were not associated with the duration of diabetes, metabolic control, microangiopathy, or neuropathy. Concluslons: Prolonged ARLs and decreased ARAs in patients with insulin-dependent diabetes are probably caused more by the stiff middle ear system than disturbances in the brainstem.
