Templating is essential in hip arthroplasty preparation, facilitating implant size prediction and surgical rehearsal. It ensures the selection of suitable implants according to patient anatomy and disease, aiming to minimize post-operative complications. Various templating methods exist, including traditional acetate templating on both analog and digital images, alongside digital templating on digital images, which is categorized into 2D and 3D approaches. Despite the popularity of acetate templating on digital images, challenges such as the requirement for physical templates and result preservation persist. To address these limitations, digital templating with software like OrthoSize and Orthoview has been suggested, although not universally accessible. This technical note advocates for Microsoft PowerPoint as an effective alternative for 2D digital templating, highlighting its user-friendly features for image manipulation without needing specialized software. The described method involves scanning acetate templates, adjusting the images in PowerPoint 365 for size, position, and calibration on patient radiographs, and demonstrating reliability through preliminary assessments, with intraclass correlation coefficient (ICC) values indicating a high level of agreement for cup and stem size (ICC = 0.860, 0.841, respectively) but moderate for neck length (ICC = 0.592). We have introduced a method for performing 2D digital templating in the clinical field without the need for specialized software dedicated to digital templating. We believe this method significantly improves the accessibility to 2D digital templating, which was previously limited by the need for digital templating software. Background: Preoperative planning is important for successful total hip arthroplasty (THA) and has been historically performed using acetate templates. Digital software templating has been adopted for evaluating implant size, position, and alignment. Commercial software can be expensive, but free programs exist. Detroit Bone Setter (detroitbonesetter.com, Detroit, MI) is a freely available templating program, but hasn’t been validated. Our study reports this program’s accuracy for templating THA. Methods: Sixty-five patients undergoing THA between 2017 and 2022 at 2 hospitals were included. All cases were templated by the senior author or orthopaedic trauma fellow prospectively or retrospectively in a blinded fashion. Direct anterior or posterior approaches were used based on attending surgeon’s preference. A student's t-test was used to compare means of templated vs actual implant sizes of femoral and acetabular components. Results: There was no significant difference between implanted (mean [M] = 6.4, standard deviation [SD] = 2.0) and templated femoral component sizes (M = 5.7, SD = 2.1). There was a significant difference between implanted (M = 57.0, SD = 3.9) and templated acetabular component sizes (M = 53.4, SD = 3.0). Bland-Altman testing demonstrated femoral components with positive measurement bias of 0.62, indicating slight overestimation of implant size. Acetabular component size was overestimated with positive measurement bias of 3.6 mm. Conclusions: Detroit Bone Setter is advantageous as it is freely available and supports most major company implants. It accurately templated femoral component size but consistently overestimated acetabular component size by 3.6 mm. Further studies are needed prior to recommending its routine use for templating THA when other validated methods exist. It could be used with caution when no other methods are available.

Result After excluding ineligible patients, this study included 391 patients, 193 (49.4%) males and 198 (50.6%) females with a mean age of 43.3 ± 14.9. The average cup sizes predicted before and after surgery were 52.12 ± 14.28 and 52.21 ± 15.05 respectively, and the mean delta cup size (before and after surgery) was 2.79 ± 2.94. The delta stem size before and after surgery has a mean value of 1.53 ± 1.49. The acetabular cup components, measured within ± 0, ± 1, and ± 2 sizes, were 28.9%, 63.9%, 83.1% accurate, respectively. The femoral stem design component measured within ± 0, ± 1, and ± 2 sizes were 27.2%, 61.0%, 78.6% accurate, respectively. Wagner Cone® stem brand, DDH patients, and females showed significantly higher accuracy of stem size templating. Revision THA has the lowest accuracy in terms of cup size templating. The compression of accuracy rate between resident and fellow revealed no significant differences. Also, no significant difference was detected between the accuracy of templating performed in the first months with the second months of the arthroplasty course period. Conclusion Our study showed that under mentioned condition, templating using mediCAD® has acceptable accuracy in predicting the sizes of femoral and acetabular components in THA patients. Digital software like mediCAD® remains favorable because of the short learning curve, user-friendly features, and low-cost maintenance, leading to level-up patient care and THA efficacy. Developmental dysplasia of the hip (DDH) is accompanied by morphological alterations on both the acetabular and the femoral side. Total hip arthroplasty (THA) provides effective treatment in cases of neglected DDH but requires elaborate preoperative planning. To determine the morphological changes resulting from the dysplasia, the anatomic acetabular position, the height of the femur head dislocation, the height of the femur head dislocation, and the combined anteversion must all be established. In addition, a vital and complicated process of strategizing leg length balance must be conducted in cases of severe DDH. Each type of leg length discrepancy (LLD), including bony and functional and anatomical LLD, should be evaluated in the context of the presence or absence of a fixed pelvic tilt. Moreover, with severe unilateral dislocated hips, a more inferior change in the original rotational center of the hip must be accounted for. Due to these multiple morphological changes, the accurate size of the prosthesis and the cup position are difficult to predict. In comparison with other methods, CT scan‐based 3‐dimensional templating provides the best accuracy. Despite the presence of anatomic alterations, various types of acetabular and femoral prostheses have been developed to treat hip dysplasia. Both cemented and cementless cups are used in DDH cases. In DDH accompanied by insufficient acetabular bone stock, a cemented cup combined with bone graft provides a reliable treatment. Monoblock stems can be used when the combined anteversion is less than 55°, and a modular stem system when this parameter is greater than 55°. Customized stems can be designed for DDH coupled with severe proximal femoral distortion. A ceramic‐on‐ceramic bearing is considered optimal for young DDH patients.

Introduction: Pre-operative templating for total hip arthroplasty (THA) remains inaccurate due to improper magnification and alignment. We aimed to describe an improved templating strategy using computed tomography (CT) to predict component sizes and offsets with greater accuracy.Materials and Methods: We analysed 184 CT images acquired for pre-operative templating of primary THA. We aimed to restore native (pre-arthritic) femoral offset and limb length, by raising the head center to the level of the templated cup center cranio-caudally, but maintaining the pathologic (pre-operative) head center medio-laterally (except in medialized hips). Acetabular offset (AO) and femoral offset (FO) were measured on pre-operative CT scans, during acetate templating, and on post-operative true antero-posterior radiographs.Results: The post-operative offsets were within ± 5 mm from templated estimates in 174 hips (91%) for AO, in 116 hips (61%) for FO, in 111 hips (58%) for GO, and in 134 hips (70%) for neck cut level. The post-operative hip architecture reproduced the templated hip architecture within ±5 mm in 77 hips (40%). The agreement between planned and post-operative parameters was moderate for stem size (0.57), cup size (0.62), AO (0.50), but fair for FO (0.45). The AO decreased in most arthritic types, notably in lateralized hips (6.6 mm), but remained unchanged in medialized hips. The FO increased in most arthritic types (1.8-3.1 mm) but remained unchanged in medialized and lateralized hips.Conclusions: We described a strategy for pre-operative templating in THA. Despite the accuracy of CT, the authors found significant variations between planned and post-operative reconstructions, which suggest that pre-operative templating should only be used as an approximate guide. Introduction: Few studies evaluated clinical benefits of pre-operative templating in total hip arthroplasty (THA). We investigated whether mismatch between planned and real implant sizes and medio-lateral offsets compromises THA outcomes.Materials and Methods: We reviewed records of 184 primary THAs with pre-operative CT scans used for templating. Acetabular offset (AO), femoral offset (FO) and global offset (GO) were measured on pre-operative CT scans, during acetate templating, and post-operative antero-posterior radiographs. Multivariable analyses were performed to determine if Forgotten Joint Score (FJS) and Oxford Hip Score (OHS) at > 2 years were associated with differences between post-operative and planned parameters.Results: The FJS and OHS were not influenced by mismatch of component sizes nor of FO and GO. The FJS was better when the post-operative AO was greater than planned (p = 0.050). The FJS differed among arthritic types (p = 0.015). Multivariable analyses confirmed that older patients had better OHS (beta - 0.16; p = 0.033) and FJS (beta 0.74; p = 0.002), medialized hips had worse FJS (beta - 20.1; p = 0.041) and hips with greater AO than planned had better FJS (beta 1.71; p = 0.024) CONCLUSIONS: Implanting a component of different size than planned did not compromise THA outcomes, but medialized hips had worse scores, and conservative acetabular reaming improved scores. Introduction: While numerous classifications of hip arthritis have been proposed, none considered the magnitude and direction of femoral head translation relative to the native acetabulum. A more precise classification of architectural hip deformities is necessary to improve preoperative templating and anticipate surgical challenges of total hip arthroplasty (THA). The purpose of the present study was to introduce a classification system to distinguish different types of architectural hip deformities, based on femoral head translation patterns, and to evaluate its repeatability using plain radiographs (qualitative) and Computed Tomography (CT) measurements (quantitative).Materials and Methods: We studied pre-operative frontal and lateral hip radiographs and CT scans of 191 hips (184 patients) that received primary THA. The distance between the femoral head center (FC) and the acetabular center (AC) was measured, as well as femoral offset, acetabular offset, head center height, acetabular floor distance and femoral neck angle. The hips were classified qualitatively using frontal plain radiographs, and then quantitatively using CT scans (with an arbitrary threshold of 3 mm as Centered, Medialized, Lateralized, Proximalized or Proximo-lateralized. The agreement between qualitative and quantitative classification methods was compared for applying the same classification.Results: Qualitative classification identified 120 centered (63%), 8 medialized (4%), 49 lateralized (26%), 3 proximalized (2%), and 11 proximo-lateralized (6%) hips, while quantitative classification identified 116 centered (61%), 8 medialized (4%), 51 lateralized (27%), 5 proximalized (3%), and 11 proximo-lateralized (6%) hips. The agreement between the two methods was excellent (0.94; CI 0.90-0.98). Medialization reached 9.7 mm, while lateralization reached 10.9 mm, and proximalization reached 8.5 mm. Proximalized and proximo-lateralized hips had more valgus necks, while medialized hips had more varus necks (p = 0.003).Conclusions: The classification system enabled repeatable distinction of 5 types of architectural hip deformities. Purpose: Patient-specific instrumentation (PSI) for unicompartmental knee arthroplasty (UKA) has been available for a few years. However, limited literature is available on this subject. Hence, the aim of this cohort study is to evaluate the 2 years' results of our first experiences with the use of PSI in UKA. It is hypothesised that there is no advantage in rate of adverse events and in radiological and functional outcomes in comparison to literature on the conventional method.Methods: This cohort included 129 knees of 122 patients, operated by one surgeon. Outcome measures were the rate of adverse events (AEs); implant position as determined on radiographs; the accuracy of the default and approved planning of the implant sizes and the patient-reported outcome measures (PROMs) preoperatively, and at 3, 12 and 24 months, postoperatively.Results: A total of 6 (4.9%) AEs were observed in this study, with 4 (3.3%) tibial fractures being the main complication. The mean postoperative biomechanical axis was 176.4° and in the majority of cases, the radiographic criteria, as determined by the manufacturer, were met. The tibial component showed 20 (16.4%) outliers in the sagittal and 3 (2.5%) outliers in the frontal plane. There were no outliers of the femoral component. For the femoral and tibial components, respectively, in 125 (96.9%) and 79 (61.7%) cases, there was an agreement between approved planning and implanted component size. All PROMs improved significantly after surgery.Conclusion: Tibial fracture was the most common AE, probably related to the transition from cemented to uncemented UKA. Perioperative modifications to the surgical technique were made in order to prevent this AE. Improvements should be made to the operation technique of the uncemented tibial plateau to obtain an adequate placement and at the same time reduce the risk for tibial fracture. The PSI technique was a reliable tool for the placement of the femoral component. Functional outcome was in line with literature on the conventional method. Preoperative digital templating in total knee arthroplastiy (TKA) is useful in predicting implant size, the level of bone resections and the need for special implants. This study should evaluate the templating of a rotating hinge prosthesis and the realization of the preoperative plan. Two observers with different experience levels templated 40 cases which received TKA and R-TKA using digital planning on standard preoperative x-rays. The examiners templated all cases independently and were blinded to the component sizes used intraoperatively. The kappa coefficient and Pearson coefficient were determined. The accuracy in predicting the correct implant size in revision TKA varied from 67,9% to 82,1% depending on the training level of the observer. The two observers show moderate and substantial correlation. The coefficient indicates a substantial agreement in between the two observers in templating revision TKA. The accuracy depends on the experience of the observer. In the cases were the templating was incorrect, the prosthesis was implanted smaller than the preoperative plan. Objectives: The purpose of this study was to evaluate the accuracy and reliability of pre-operative templating in predicting the size of femoral and tibial components and the effect of coronal deformity on templating accuracy.Methods: This was a retrospective study of 39 pre-operative templates prepared by three different surgeons with different levels of training. The accuracy and reliability measures were evaluated by alpha and kappa coefficients of agreement. The analysis of the effect of coronal deformity on the accuracy of the template was measured by the Spearman's correlation test.Results: Templating was accurate for the femoral component in 28.21% of anterposterior (AP) radiographs and 35.90% of lateral radiographs. Kappa coefficients were respectively 0.111 (95% confidence interval [95%CI]: -0.19 to 0.241) and 0.200 (95%CI: -0.010 to 0.401), indicating poor agreement. Templating accuracy for the tibial component were, respectively, 37.61% and 47.01% for AP and lateral views. Kappa coefficients were 0.186 (95%CI: -0.070 to 0.379) for the AP view and 0.315 (95%CI: -0.199 to 0.431) for the lateral view, showing poor and slight agreement respectively. Considering a margin of error of ±1 sizes, the agreement level improved for all components, particularly for tibia, where agreement levels become very good. The inter-observer agreement was fair for all components, except for the lateral view of the femoral component, whose agreement was good. The Spearman correlation test showed no correlation between accuracy of templating and coronal deformity.Conclusion: Pre-operative templating is an unreliable and inaccurate tool. Purpose: This study examines the accuracy of digital templating in uncemented total hip arthroplasty (THA), i.e., whether the templated components where actually inserted during surgery. The surgical outcome was evaluated on the basis of limb length equality. Methods: We retrospectively examined digital x-rays of 41 patients scheduled for uncemented THA. These were templated using templating software. The template was compared to the surgical choice of implant registered in the patients' journal. Postoperative x-rays were evaluated for limb length equality. The data underwent statistical analysis to assess accuracy. Results: The acetabular component was templated accurately in 7.3%, while 41% was within +/- 1 component size difference, and 73% was within +/- 2 size differences. The femoral stem was templated accurately in 34%, while 76% was within +/- 1 component size difference, and 90% was within +/-2 size differences. The neck length was templated accurately in 29%, while 88% was within +/-1 component size difference, and 100% was within +/- 2 size differences. Fifty four percent of patients experienced radiologic equalization within +/- 5 mm, and 85% within 10 mm. Fifteen percent had leg length discrepancy of more than 10 mm postoperatively. There was no systematic tendency to overestimate or underestimate leg length peroperatively. Conclusions: We find that the accuracy of digital templating in uncemented THA is acceptable for the femoral stem, but somewhat inferior for the acetabular shell and poor for neck length. Purpose: The purpose of this study was to evaluate the use of pre-operative digital templating to minimize complications including limb length discrepancy (LLD), intraoperative fractures and early dislocations in patients with intracapsular femoral neck fractures.Methods: We retrospectively compared 23 patients undergoing total hip arthroplasty (THA) for intracapsular femoral fractures with pre-operative digital templating and 48 patients without templating.Results: The mean post-operative LLD was significantly lower in patients who had pre-operative templating than in the control group (6.7 vs. 11.5 mm, p = 0.023). Only three patients (13 %) with templating had LLD greater than 1.5 cm, compared to the 15 patients (31 %) without templating (p = 0.17). In eight cases the final femoral stem size matched the templated size, while 19 patients were within two size increments. Complications included one dislocation and one intra-operative fracture in the control group.Conclusion: The present study demonstrated that careful pre-operative planning may reduce LLD in patients undergoing THA due to intracapsular hip fractures. Many orthopaedic surgeons use a 'standard' stem offset length, typically 37.5 mm and 44 mm for females and males respectively, in total hip arthroplasty. With increasingly personalized surgery, 'standard' one-size-fits-all stem lengths may be outdated. This study aims to test whether pre-operative templating affects stem length choice and whether 'standard' stem sizes are therefore outdated. We performed a retrospective chart review of all total hip arthroplasty patients using Stryker's Exeter cemented femoral hip system in our centre between 2016 and 2020. Demographic and surgical data were collected. Data from surgeons who templated pre-operatively were compared to data from those who did not. 780 patients were included (309 male, 471 female), average age 71.4 years (range 23–96). We found a significant difference between male and female offset lengths; more males had an offset length of 44 mm and more females had an offset length of 37.5 mm (p = 0.004). Among surgeons who did not template pre-operatively, 20.6% of female patients and 10.3% of male patients had other 'non-standard' offset lengths. Among surgeons who did template pre-operatively, the proportion of both female and male patients who had other 'non-standard' offset lengths was significantly higher (43.1% and 23.4%, respectively p < 0.05) The difference between templating and non-templating surgeons' stem choice revealed significant individual variability between males and females. 'Standard' offset lengths for males and females were still used in the majority of our cohort. Purpose: The aim of this study was to determine how well pre-operative size selection for total knee and hip arthroplasties based on the digital imaging with and without additional referencing correlated with the size actually implanted. Methods: Size selection planning of total knee arthroplasty by digital templating was documented in 46 cases with reference ball (group A) and in 48 cases without ball (group B). In addition, prospective analysis of pre-operative planning was conducted for 52 acetabular components with reference ball (group C) and 69 without ball (group D) as well as stem planning in 38 cases with ball (group E) and 54 cases without ball (group F). The data were analysed and compared with the size of the final component selected during surgery. Results: The correlation between planned and implanted size for total knee arthroplasty in group A resulted in femoral anteroposterior (AP) r = 0.8622 and lateral r = 0.8333 and in group B AP r = 0.4552 and lateral r = 0.6950. Tibial in group A was AP r = 0.9030 and lateral r = 0.9074 and in group B AP r = 0.7000 and lateral r = 0.6376. For the acetabular components, the results in group C were r = 0.5998 and group D r = 0.6923. For stems, group E was r = 0.5306 and group F r = 0.5786. No correlation between BMI and the difference between planned and implanted size was found in any of the groups. Conclusion: In the case of total hip arthroplasty there was a relatively low correlation between planned and implanted sizes with or without reference ball. For total knee arthroplasties the already high precision of size planning was further improved by the additional referencing with a reference ball. [ABSTRACT FROM AUTHOR]

Objectives: To evaluate the validity, interobserver reliability, and intraobserver reproducibility of a digital templating system, the Mdesk™ in preoperative templating in cemented and reverse hybrid total hip arthroplasty (THA). Materials and methods: Validity was evaluated by comparing the planned cup size, stem size, CCD angles, and neck length with the components used in 129 patients operated with cemented and reverse hybrid THA. The reliability was measured by comparing the templating results of two surgeons with each other (interobserver) and the results of two templatings carried out by first surgeon (intraobserver). The leg length discrepancy was measured before and after the operation to assess the templating ability to correct it. Results: The Mdesk™ system showed good validity (kappa value ranged from 0.64 to 0.96), especially when one size over and under the planned size were included. No difference between cemented and cementless stems was found. The interobserver reliability ranged from fair (kappa 0.23) to substantial (kappa 0.61) while the intraobserver reproducibility ranged from substantial (kappa 0.70) to excellent (kappa 0.82). Templating and intraoperative measures succeeded to restore the leg length. Conclusions: The Mdesk™ system has comparable validity and reliability with other templating systems used in clinical practice. We recommend that the same surgeon who does the preoperative radiographic templating to also perform the operation. Abstract: This study aims to assess the accuracy of digital templating at our institution, by comparing the templated component sizes with those implanted, and to determine whether templating the preoperative films had any measurable difference on the radiographic outcome, and if, where there was a mismatch between the implanted and templated sizes, the templated size would have been preferable. While a number of studies have evaluated the accuracy of both acetate and digital templating, none has to our knowledge looked back at post-operative radiographs and reviewed these in light of the templated and implanted sizes. Data was collected from 90 PFC Sigma (DePuy, UK) total knee replacements done sequentially, 45 of whom were templated digitally using a calibrating ball and Agfa Orthopaedic Tools software. Postoperative radiographs were graded independently for correct sizing. All templates were within one size of the implanted prosthesis. The femoral component appeared to be more often oversized on the postoperative radiographs in the non-templated group. In addition, most tibial trays that were found be too small had been templated to a larger size. There was a trend towards tibial trays templated too large to have been templated to a smaller size. We conclude that digital templating with a calibrating device is a useful part of preoperative planning for total knee arthroplasty. [Copyright &y& Elsevier]

We have investigated the accuracy of automatic calibration of digital pelvic radiographs using the single and double marker techniques. Both markers were applied by the radiographer at the time of routine, postoperative radiographs. Each radiograph was loaded into the TraumaCadT™ software package which calculated the magnification and scaled the radiograph automatically. The median error of magnification in the double marker group was 1.14% when calculated automatically. The median error of the single marker was 5.98%. The relationship between true and predicted magnification was strongest in the double marker group with excellent correlation (r=0.91) and agreement (ICC<0.91). We believe that the double marker method with automatic calibration is the most reliable method of scaling a pelvic radiograph in clinical practice currently available. [ABSTRACT FROM AUTHOR]

Abstract: We reviewed 75 primary total hip arthroplasty preoperative and postoperative radiographs and recorded limb length discrepancy, change in femoral offset, acetabular position, neck cut, and femoral component positioning. Interobturator line, as a technique to measure preoperative limb length discrepancy, had the least amount of variance when compared with interteardrop and intertuberosity lines (Levene test, P = .0527). The most common error in execution of preoperative templating was excessive limb lengthening (mean, 3.52 mm), primarily due to inferior acetabular cup positioning (Pearson correlation coefficient, P = .036). Incomplete medialization of the acetabular component contributed the most to offset discrepancy. The most common errors in the execution of preoperative templating resulted in excessive limb lengthening and increased offset. Identifying these errors can lead to more accurate templating techniques and improved intraoperative execution. [Copyright &y& Elsevier]

Background: The purpose of this paper is to examine the intra-observer and inter-observer reliability of preoperative templating to help determine its usefulness for approximating proximal humeral stem and head size in shoulder arthroplasty. Methods: Thirty-one nonconsecutive patients underwent uncemented total shoulder arthroplasty using 1 of 2 commonly used prostheses. Templating was performed by 4 independent observers (2 experienced shoulder surgeons and 2 PGY-3 residents). Two readings were performed with a minimum 2-week interval between templating sessions. Templating was performed in a blinded manner using appropriately magnified templates provided by the prosthesis manufacturer. Postoperative radiographs were analyzed in the same fashion and each observer graded qualitative stem and head size radiographically. Pre-operative templated sizes were compared to actual implant sizes obtained from dictated operative reports. Results: There was substantial (kappa > 0.7) intraobserver reliability for stem and head size selection for both prostheses. Interobserver reliability was moderate for stem size (kappa = 0.53) and substantial (kappa = 0.66) for head size. Preoperative templating accurately predicted stem (84-95%) and head (44-66%) size within 1 size variation (stem width, head thickness, or diameter) for shoulder replacement surgery. There was no significant difference in accuracy between attendings and residents when templating for stem or head size. Conclusion: Preoperative templating is a reliable and reasonably accurate step when planning shoulder replacement surgery. Templating for stem size is more accurate than for head size. Abstract: Improvement of alignment and position of the components in TKAs using Computer Assisted Orthopaedic Surgery (CAOS) has been described. However, much less is known about the accuracy of CAOS in determining the size of the components. The purpose of this study was to evaluate the size of the femoral and tibial component using the CAOS system from Brainlab. The component sizes were compared to pre-operative templating and post-operative scoring the adequateness of size. Forty TKAs (NexGen) were evaluated: 20 using CAOS and 20 conventional. Statistical analysis of the templated and implanted size indicated a fair agreement for the femur (kappa 0.38) and the tibia (kappa 0.35). In the CAOS group significantly more oversizing occurred for the femoral component (p =0.020). No significant difference was found for the tibial component. We conclude there is a risk of oversizing the femoral component of the NexGen system when using CAOS. [Copyright &y& Elsevier]

Templating of preoperative X-rays is routinely performed before a total knee replacement. We performed this retrospective study on preoperative templating in primary total knee replacement to assess its accuracy and reproducibility. Preoperative radiographs of 47 patients were templated twice by two observers separately after a gap of 3 months. The size of actual implant used was compared to the size predicted by templating. Data were analysed to assess inter and intra-observer variation. Templating was accurate for both tibial and femoral components in only 53.2% of observations. This study also revealed that templating is a highly-subjective and observer-dependent technique. Inter-observer and intra-observer mismatch was present in 46.8% and 43.6% of readings respectively.
