Novel word learning ability has been associated with language treatment outcomes in people with aphasia (PWA), and its assessment could inform prognosis and rehabilitation. We used a brief experimental task to examine novel word learning in PWA, determine the value of phonological cueing in assessing learning outcomes, and identify factors that modulate learning ability. Twelve PWA and nineteen healthy controls completed the task, and recall and recognition tests of learning ability. Most PWA showed comparable learning outcomes to those of the healthy controls. Learning assessed via expressive recall was more clearly evidenced with phonological cues. Better single word processing abilities and phonological short-term memory and higher integrity of the left inferior frontal gyrus were related to better learning performance. Verbal short-term memory (STM) deficits are associated with language processing impairments in people with aphasia. Importantly, the integrity of STM can predict word learning ability and anomia therapy gains in aphasia. While the recruitment of perilesional and contralesional homologous brain regions has been proposed as a possible mechanism for aphasia recovery, little is known about the white-matter pathways that support verbal STM in post-stroke aphasia. Here, we investigated the relationships between the language-related white matter tracts and verbal STM ability in aphasia. Nineteen participants with post-stroke chronic aphasia completed a subset of verbal STM subtests of the TALSA battery including nonword repetition (phonological STM), pointing span (lexical-semantic STM without language output) and repetition span tasks (lexical-semantic STM with language output). Using a manual deterministic tractography approach, we investigated the micro- and macrostructural properties of the structural language network. Next, we assessed the relationships between individually extracted tract values and verbal STM scores. We found significant correlations between volume measures of the right Uncinate Fasciculus and all three verbal STM scores, with the association between the right UF volume and nonword repetition being the strongest one. These findings suggest that the integrity of the right UF is associated with phonological and lexical-semantic verbal STM ability in aphasia and highlight the potential compensatory role of right-sided ventral white matter language tracts in supporting verbal STM after aphasia-inducing left hemisphere insult., (© 2023. People with aphasia (PWA) present with language deficits including word retrieval difficulties after brain damage. Language learning is an essential life-long human capacity that may support treatment-induced language recovery after brain insult. This prospect has motivated a growing interest in the study of language learning in PWA during the last few decades. Here, we critically review the current literature on language learning ability in aphasia. The existing studies in this area indicate that (i) language learning can remain functional in some PWA, (ii) inter-individual variability in learning performance is large in PWA, (iii) language processing, short-term memory and lesion site are associated with learning ability, (iv) preliminary evidence suggests a relationship between learning ability and treatment outcomes in this population. Based on the reviewed evidence, we propose a potential account for the interplay between language and memory/learning systems to explain spared/impaired language learning and its relationship to language therapy in PWA. Sung melody provides a mnemonic cue that can enhance the acquisition of novel verbal material in healthy subjects. Recent evidence suggests that also stroke patients, especially those with mild aphasia, can learn and recall novel narrative stories better when they are presented in sung than spoken format. Extending this finding, the present study explored the cognitive mechanisms underlying this effect by determining whether learning and recall of novel sung vs. spoken stories show a differential pattern of serial position effects (SPEs) and chunking effects in non-aphasic and aphasic stroke patients (N = 31) studied 6 months post-stroke. The structural neural correlates of these effects were also explored using voxel-based morphometry (VBM) and deterministic tractography (DT) analyses of structural MRI data. Non-aphasic patients showed more stable recall with reduced SPEs in the sung than spoken task, which was coupled with greater volume and integrity (indicated by fractional anisotropy, FA) of the left arcuate fasciculus. In contrast, compared to non-aphasic patients, the aphasic patients showed a larger recency effect (better recall of the last vs. middle part of the story) and enhanced chunking (larger units of correctly recalled consecutive items) in the sung than spoken task. In aphasics, the enhanced chunking and better recall on the middle verse in the sung vs. spoken task correlated also with better ability to perceive emotional prosody in speech. Neurally, the sung > spoken recency effect in aphasic patients was coupled with greater grey matter volume in a bilateral network of temporal, frontal, and parietal regions and also greater volume of the right inferior fronto-occipital fasciculus (IFOF). Human learners can resolve referential ambiguity and discover the relationships between words and meanings through a cross-situational learning (CSL) strategy. Some people with aphasia (PWA) can learn word-referent pairings under referential uncertainty supported by online feedback. However, it remains unknown whether PWA can learn new words cross-situationally and if such learning ability is supported by statistical learning (SL) mechanisms. The present study examined whether PWA can learn novel word-referent mappings in a CSL task without feedback. We also studied whether CSL is related to SL in PWA and neurologically healthy individuals. We further examined whether aphasia severity, phonological processing and verbal short-term memory (STM) predict CSL in aphasia, and also whether individual differences in verbal STM modulate CSL in healthy older adults. Sixteen people with chronic aphasia underwent a CSL task that involved exposure to a series of individually ambiguous learning trials and a SL task that taps speech segmentation. Their learning ability was compared to 18 older controls and 39 young adults recruited for task validation. CSL in the aphasia group was below the older controls and young adults and took place at a slower rate. Importantly, we found a strong association between SL and CSL performance in all three groups. CSL was modulated by aphasia severity in the aphasia group, and by verbal STM capacity in the older controls. Our findings indicate that some PWA can preserve the ability to learn new word-referent associations cross-situationally. We suggest that both PWA and neurologically intact individuals may rely on SL mechanisms to achieve CSL and that verbal STM also influences CSL. Recent research suggests that some people with aphasia preserve some ability to learn novel words and to retain them in the long-term. However, this novel word learning ability has been studied only in the context of single word-picture pairings. We examined the ability of people with chronic aphasia to learn novel words using a paradigm that presents new word forms together with a limited set of different possible visual referents and requires the identification of the correct word-object associations on the basis of online feedback. We also studied the relationship between word learning ability and aphasia severity, word processing abilities, and verbal short-term memory (STM). We further examined the influence of gross lesion location on new word learning. The word learning task was first validated with a group of forty-five young adults. Fourteen participants with chronic aphasia were administered the task and underwent tests of immediate and long-term recognition memory at 1 week. Their performance was compared to that of a group of fourteen matched controls using growth curve analysis. The learning curve and recognition performance of the aphasia group was significantly below the matched control group, although above-chance recognition performance and case-by-case analyses indicated that some participants with aphasia had learned the correct word-referent mappings. Verbal STM but not word processing abilities predicted word learning ability after controlling for aphasia severity. Importantly, participants with lesions in the left frontal cortex performed significantly worse than participants with lesions that spared the left frontal region both during word learning and on the recognition tests. Our findings indicate that some people with aphasia can preserve the ability to learn a small novel lexicon in an ambiguous word-referent context. The ability to learn to use new words is thought to depend on the integrity of the left dorsal temporo-frontal speech processing pathway. We tested this assumption in a chronic aphasic individual (AA) with an extensive left temporal lesion using a new-word learning paradigm. She exhibited severe phonological problems and Magnetic Resonance Imaging (MRI) suggested a complete disconnection of this left-sided white-matter pathway comprising the arcuate fasciculus (AF). Diffusion imaging tractography confirmed the disconnection of the direct segment and the posterior indirect segment of her left AF, essential components of the left dorsal speech processing pathway. Despite her left-hemispheric damage and moderate aphasia, AA learned to name and maintain the novel words in her active vocabulary on par with healthy controls up to 6 months after learning. This exceeds previous demonstrations of word learning ability in aphasia. Interestingly, AA's preserved word learning ability was modality-specific as it was observed exclusively for written words. Functional magnetic resonance imaging (fMRI) revealed that in contrast to normals, AA showed a significantly right-lateralized activation pattern in the temporal and parietal regions when engaged in reading. Moreover, learning of visually presented novel word-picture pairs also activated the right temporal lobe in AA. Both AA and the controls showed increased activation during learning of novel versus familiar word-picture pairs in the hippocampus, an area critical for associative learning. AA's structural and functional imaging results suggest that in a literate person, a right-hemispheric network can provide an effective alternative route for learning of novel active vocabulary. We describe a study where a specific treatment method for word-finding difficulty (so-called contextual priming technique, which combines massive repetition priming with semantic priming) was applied with three chronic left hemisphere-damaged aphasics. Both before and after treatment, which focused on naming of a series of pictures, naming-related brain activity was measured by magnetoencephalography (MEG). Due to its excellent temporal resolution and good spatial resolution, we were able to track treatment-induced changes in cortical activity. All three subjects showed improved naming of the trained items. In all subjects, a single source area, located in the left inferior parietal lobe, close to the lesioned area, displayed statistically significant training-induced changes. This effect was of long latency as it started 300-600 msec after picture presentation. The change in activation was specific to training, as it could not be accounted for by variation of cortical dynamics associated with increased proportion of correct answers. Our interpretation is that the training effect reflects more effective phonological encoding and storage of the trained items through the engagement of a left hemispheric word-learning system. This is in line with recent functional imaging studies, which have linked left inferior parietal lobe activity to the phonological storage component of the verbal working memory, as well as with theoretical arguments stating that the primary role of the phonological loop is to acquire new words. Finally, the MEG results showed no evidence of increased right hemisphere participation following training, supporting the view that restoration of language-related networks in the damaged left hemisphere is crucial for anomia recovery.

Objective: Of current interest in aphasia research is the relevance of what we can learn from studying word learning ability in aphasia. In a preliminary study, we addressed 2 issues related to the novel word learning ability of individuals with aphasia. First, as word learning engages large-scale cognitive-linguistic systems (language skills, verbal short-term memory [STM], other memory and executive functions), we probed whether novel word learning practice in 3 people with aphasia could stimulate these language-related systems. Second, as lesion correlates affecting word learning in aphasia remain unclear, we examined whether the structural integrity of the left arcuate fasciculus (AF) in the same 3 individuals is related to outcomes of novel word learning practice. Method: To stimulate word learning systems, our 3 participants practiced for 4 weeks with an explicit novel word-novel referent word learning task, adopted from the Ancient Farming Equipment learning paradigm (Laine & Salmelin, 2010). The participants' progress on receptive and expressive novel word learning was followed up, and their language and verbal STM abilities as well as single-session novel word learning (Learning to Name Aliens by Gupta, Martin, Abbs, Schwartz, & Lipinski, 2006) were tested before and after the practice period. To address the second question, we analyzed the participants' structural magnetic resonance images with respect to the integrity of the left AF and its overlap with the lesion areas. Results: All participants showed some receptive word learning in the trained task, as well as improvements in verbal STM span at posttesting. Two of the 3 participants also showed improved performance on some of the language outcome measures. One participant with a partially spared left AF, especially temporoparietal connections, exhibited better word learning performance than the other 2 who had larger damage and disconnection of the AF. Conclusions: Although the present results are preliminary, they open the possibility that novel word learning practice in aphasia may stimulate remaining word learning mechanisms in aphasia and thereby influence language and verbal STM abilities. These results also suggest that preservation of novel word learning ability in aphasia in part depends on the integrity of the left arcuate track.

Objective: Previous studies suggest that daily music listening can aid stroke recovery, but little is known about the stimulus‐dependent and neural mechanisms driving this effect. Building on neuroimaging evidence that vocal music engages extensive and bilateral networks in the brain, we sought to determine if it would be more effective for enhancing cognitive and language recovery and neuroplasticity than instrumental music or speech after stroke. Methods: Using data pooled from two single‐blind randomized controlled trials in stroke patients (N = 83), we compared the effects of daily listening to self‐selected vocal music, instrumental music, and audiobooks during the first 3 poststroke months. Outcome measures comprised neuropsychological tests of verbal memory (primary outcome), language, and attention and a mood questionnaire performed at acute, 3‐month, and 6‐month stages and structural and functional MRI at acute and 6‐month stages. Results: Listening to vocal music enhanced verbal memory recovery more than instrumental music or audiobooks and language recovery more than audiobooks, especially in aphasic patients. Voxel‐based morphometry and resting‐state and task‐based fMRI results showed that vocal music listening selectively increased gray matter volume in left temporal areas and functional connectivity in the default mode network. Interpretation: Vocal music listening is an effective and easily applicable tool to support cognitive recovery after stroke as well as to enhance early language recovery in aphasia. The rehabilitative effects of vocal music are driven by both structural and functional plasticity changes in temporoparietal networks crucial for emotional processing, language, and memory. Background: Speech segmentation is one of the initial and mandatory phases of language learning. Although some people with aphasia have shown a preserved ability to learn novel words, their speech segmentation abilities have not been explored. Aims: We examined the ability of individuals with chronic aphasia to segment words from running speech via statistical learning. We also explored the relationships between speech segmentation and aphasia severity, and short-term memory capacity. We further examined the role of lesion location in speech segmentation and short-term memory performance. Methods & Procedures: The experimental task was first validated with a group of young adults (n = 120). Participants with chronic aphasia (n = 14) were exposed to an artificial language and were evaluated in their ability to segment words using a speech segmentation test. Their performance was contrasted against chance level and compared to that of a group of elderly matched controls (n = 14) using group and case-by-case analyses. Outcomes & Results: As a group, participants with aphasia were significantly above chance level in their ability to segment words from the novel language and did not significantly differ from the group of elderly controls. Speech segmentation ability in the aphasic participants was not associated with aphasia severity although it significantly correlated with word pointing span, a measure of verbal short-term memory. Case-by-case analyses identified four individuals with aphasia who performed above chance level on the speech segmentation task, all with predominantly posterior lesions and mild fluent aphasia. Their short-term memory capacity was also better preserved than in the rest of the group. Conclusions: Our findings indicate that speech segmentation via statistical learning can remain functional in people with chronic aphasia and suggest that this initial language learning mechanism is associated with the functionality of the verbal short-term memory system and the integrity of the left inferior frontal region. Background: In recent years some critical voices have been raised in regard to the significance of cognitive neuropsychology (CNP) to the study of brain and mind. Given the central role of language disorders in CNP research, it is time to consider the relevance of this research approach in aphasiology. Aims: We analyse the main points of criticism raised against the CNP research approach, evaluate the significance of this approach to the study of acquired language disorders, and make some suggestions concerning further development of the field. Main Contribution: The main points of criticism against CNP (reliance on single-case studies; single-minded hunt for dissociations; emptiness of theorising) have been important long-term concerns but do not take into account the fact that, during its history of circa four decades, the CNP approach has diversified. There are thus CNP studies that rely on case series analyses, focus on error analyses rather than mere dissociations, or employ computational modelling rather than the “boxes-and-arrows” models of the mental architecture. The CNP approach to cognition and its disorders is thus applicable to different research questions and theoretical stances, providing experimental rigour to single-case patient studies. With regard to clinical applications in aphasia diagnostics and treatment, the CNP approach provides a richer view on the strengths and weaknesses of a patient's cognitive-linguistic abilities. Conclusions: We believe that CNP case studies continue to be an important source of information for generating hypotheses and providing converging evidence for research on the mind and on the brain. However, there is a need for further research development especially in computational modelling of language processes, their impairments, and recovery. Background: The study of novel word learning in aphasia can shed light on the functionality of patients' learning mechanisms and potentially help in treatment planning. Previous studies have indicated that persons with aphasia are able to learn some new vocabulary. However, these learning outcomes appear short-lived and evidence for the ability to use the newly learned words in the long term is lacking. Aims: Participants with aphasia and matched controls underwent short training where they were taught to name novel objects with novel names. We studied the participants' word learning and particularly their long-term maintenance. We also examined whether the language and verbal short-term memory impairments of the participants with aphasia related to their ability to acquire and maintain phonological and semantic information on novel words. Methods & Procedures: Two participants with nonfluent aphasia, LL and AR, and two matched controls took part in the experiment. They were taught to name 20 unfamiliar objects by repeating the names in the presence of the object picture. Half of the items carried a definition that was used to probe incidental semantic learning. There were four training sessions, a post-training test, and follow-up tests up to 6 months post-training. Learning measures included recognition of the trained objects, as well as spontaneous and cued recall in visual confrontation naming. Incidental semantic learning was measured by spontaneous recall of the definitions. Outcomes & Results: Combining spontaneous and phonologically cued responses, LL acquired 70% and AR 55% of the novel words. With phonological cueing, LL named 50% of the items correctly up to 6 months post-training (vs 95–100% for the controls) and AR 25% up to 8 weeks post-training. AR's lexical-semantic processing, pseudoword repetition and verbal short-term capacity were inferior to those of LL. In line with this, AR learned fewer words and showed more decline in recognition memory for the trained items, and weaker recall of the semantic definitions. Conclusions: Our results support previous findings that people with aphasia can learn to name novel items. More importantly, the results show for the first time that, with phonological cueing, an individual with aphasia can maintain some of this learning up to 6 months post-training. Background: Language performance in aphasia can vary depending on several variables such as stimulus characteristics and task demands. This study focuses on the degree of verbal working memory (WM) load inherent in the language task and how this variable affects language performance by individuals with aphasia. Aims: The first aim was to identify the effects of increased verbal WM load on the performance of judgements of semantic similarity (synonymy) and phonological similarity (rhyming). The second aim was to determine if any of the following abilities could modulate the verbal WM load effect: semantic or phonological access, semantic or phonological short-term memory (STM), and any of the following executive processing abilities: inhibition, verbal WM updating, and set shifting. Method & Procedures: A total of 31 individuals with aphasia and 11 controls participated in this study. They were administered a synonymy judgement task and a rhyming judgement task under high and low verbal WM load conditions that were compared to each other. In a second set of analyses multiple regression was used to identify which factors (as noted above) modulated the verbal WM load effect. Outcomes & Results: For participants with aphasia, increased verbal WM load significantly reduced accuracy of performance on synonymy and rhyming judgements. Better performance in the low verbal WM load conditions was evident even after correcting for chance. The synonymy task included concrete and abstract word triplets. When these were examined separately the verbal WM load effect was significant for the abstract words, but not the concrete words. The same pattern was observed in the performance of the control participants. Additionally, the second set of analyses revealed that semantic STM and one executive function, inhibition ability, emerged as the strongest predictors of the verbal WM load effect in these judgement tasks for individuals with aphasia. Conclusions: The results of this study have important implications for diagnosis and treatment of aphasia. As the roles of verbal STM capacity, executive functions and verbal WM load in language processing are better understood, measurements of these variables can be incorporated into our diagnostic protocols. Moreover, if cognitive abilities such as STM and executive functions support language processing and their impairment adversely affects language function, treating them directly in the context of language tasks should translate into improved language function. Background: Novel word learning of persons with aphasia is little studied, even though a better understanding of learning processes would inform development of effective treatment strategies. Recent evidence suggests some remaining verbal learning capacity in persons with aphasia. Long-term maintenance of newly learned active vocabulary has not been reported previously in persons with aphasia. Aims: To explore learning and long-term maintenance of novel words in persons with aphasia. Methods & Procedures: Two English-speaking males with chronic anomic aphasia and two age-matched controls were taught novel names of 20 unfamiliar objects. Half of the words were taught with semantic information (definition) and half without. Participants were instructed to learn the names. The experiment included four training sessions, one post-training test and four follow-up tests administered 1 week, 4 weeks, 8 weeks, and 6 months post-training. We tested explicit learning of the new names through visual confrontation naming. In addition, incidental learning of semantic information was probed over the follow-up period. Outcomes & Results: The two participants with aphasia learned 6-8 of the 20 novel names during the training. However, this new vocabulary dissipated during the 6-month follow-up. As expected, the controls showed better performance both in acquisition and in maintenance of the new vocabulary over the follow-up period. As regards the accuracy of semantic information, the aphasic participant with semantic impairment demonstrated worse incidental learning of semantic information than controls and the participant with intact lexical semantics. Conclusions: Some new vocabulary can be acquired even in chronic aphasia but the ability to spontaneously retrieve the newly learned words gradually dissipates over the weeks following learning. Our results also indicate an interaction between the level of lexical-semantic processing skills and incidental learning of new lexical-semantic knowledge in aphasia. [ABSTRACT FROM AUTHOR]

Acquired amusia is a common disorder after damage to the middle cerebral artery (MCA) territory. However, its neurocognitive mechanisms, especially the relative contribution of perceptual and cognitive factors, are still unclear. We studied cognitive and auditory processing in the amusic brain by performing neuropsychological testing as well as magnetoencephalography (MEG) measurements of frequency and duration discrimination using magnetic mismatch negativity (MMNm) recordings. Fifty-three patients with a left (n = 24) or right (n = 29) hemisphere MCA stroke (MRI verified) were investigated 1 week, 3 months, and 6 months after the stroke. Amusia was evaluated using the Montreal Battery of Evaluation of Amusia (MBEA). We found that amusia caused by right hemisphere damage (RHD), especially to temporal and frontal areas, was more severe than amusia caused by left hemisphere damage (LHD). Furthermore, the severity of amusia was found to correlate with weaker frequency MMNm responses only in amusic RHD patients. Additionally, within the RHD subgroup, the amusic patients who had damage to the auditory cortex (AC) showed worse recovery on the MBEA as well as weaker MMNm responses throughout the 6-month follow-up than the non-amusic patients or the amusic patients without AC damage. Furthermore, the amusic patients both with and without AC damage performed worse than the non-amusic patients on tests of working memory, attention, and cognitive flexibility. These findings suggest domain-general cognitive deficits to be the primary mechanism underlying amusia without AC damage whereas amusia with AC damage is associated with both auditory and cognitive deficits. Background: We present an anomia treatment study using a modified contextual priming (CP) technique for two anomic participants. The reason for the modification attempt is that the most recent studies have indicated that the CP procedure is less effective in the case of impairment to lexical-semantic processes than in the case of phonological deficits (see Martin, Fink, & Laine, 2004a; Martin, Fink, Laine, & Ayala, 2004b; Renvall, Laine, & Martin, 2005). Aims: Our aim is to study the within- and post-treatment effects of a modified CP procedure and especially whether additional semantic tasks can increase benefits from the CP treatment. Methods & Procedures: The treatment was conducted for two participants with acquired anomia: LV has primarily a semantic component underlying her anomia, while JP suffers primarily from a phoneme-sequencing deficit. The CP procedure encompassed repeated cycles of spontaneous naming attempts and repetition of target names using arrays of multiple pictures where the targets were either semantically related or unrelated. Our modification was to add picture-to-word matching and phonological "rhyming syllable identification" with the targets to the training sequence. The treatment was carried out in a single-subject multiple-baseline design consisting of several baseline measurements, treatment sessions along with within-treatment measurements, and a post-treatment measurement 1.5 months later. Outcomes & Results: Both participants showed short-term facilitation of naming target items in all treatment conditions. For LV, post-treatment improvement of naming was statistically significant in the semantic condition irrespective of additional task type, even though the improvement was strongest when the semantic condition was coupled with the additional semantic tasks. In the case of JP, post-treatment improvement was observed in the semantic condition coupled with additional semantic tasks, and in the unrelated condition with both semantic and phonological tasks. No strong evidence of generalisation to untreated items was observed for either of the participants. Conclusions: The modified CP procedure can provide longer-term improvement of naming target items than the CP training without additional tasks in the face of a lexical-semantic deficit. With lexical-semantic disturbance, the semantic context provided the best results. We constructed a connectionist model with multilayer perceptron networks for the simulation of word production errors in the Finnish language. Such errors occur as slips of the tongue in healthy subjects and even more often in aphasic patients suffering from brain damage. Word production errors are of theoretical interest as they open an avenue to inner workings of the language production system. Here we present our coding schemes for semantic and phoneme information of words, investigate the general properties of the model, and compare the results of the model against empirical naming error data from ten aphasic patients. Abstract: The present case continues the series of anomia treatment studies with contextual priming (CP), being the second in-depth treatment study conducted for an individual suffering from semantically based anomia. Our aim was to acquire further evidence of the facilitation and interference effects of the CP treatment on semantic anomia. Based on the results of the study of , our hypothesis before the treatment was that our participant would show short-term interference and at most modest and short-term benefit from treatment. To acquire such evidence would not only be important for the choice of anomia treatment methods in individual patients, but would also prompt further development of the CP method. The CP technique used for our participant included cycles of repeating and naming items in three contextual conditions (semantic, phonological, and unrelated). As predicted, the overall improvement of naming was modest and short-term. Interestingly, the contextual condition that corresponded with the nature of our patient’s underlying naming deficit (semantic) elicited immediate interference in the form of contextual naming errors, as well as short-term improvement of naming. Based on this and a recent study by , it appears that despite short-term positive effects, in its current form the CP treatment is not sufficient for those aphasics who have a semantic deficit underlying their anomia. Background : Many therapy techniques for word retrieval disorders use some form of priming to improve access to words. Priming can facilitate or interfere with naming under different circumstances. We examined effects of priming when combined with semantic or phonological context (training words in groups that are semantically or phonologically related) and how these effects interact with the type of naming impairment (semantically or phonologically based). Aims : We addressed three questions (1) Are word retrieval impairments differentially sensitive to priming with semantic or phonological contexts? (2) Would such differences be systematically related to deficits of semantic versus phonological processing? (3) Do effects of priming evolve from immediate interference to short-term facilitation, as predicted by an interactive activation model of word retrieval? Methods & Procedures : A total of 11 chronic English-speaking aphasic subjects with varied types of aphasia participated in this experiment. Background measures of semantic and phonological processing ability were administered to determine the nature of each subject's naming impairment. The experiment involved one-session facilitation treatments for each of three context conditions (semantic, phonological, and unrelated), plus three replications (nine subjects) or one replication (two subjects). Ten pictures in each condition were tested before and after treatment. Five pictures were trained and five served as controls. Participants repeated the name of each picture four times (repetition priming) and then attempted to name each picture individually (naming probe). Repetition priming and naming probes were repeated eight times. We used McNemar tests to compare rates of correct responses before and after priming, and chi square analyses of correct responses and contextual errors on naming probes obtained during the priming sessions. Outcome & results : Our predictions were borne out in the data. Participants varied in their sensitivity to the semantic and phonological contexts. The error data suggest that interference during training is more likely when the context (semantic or phonological) and underlying source of the word processing impairment (semantic or phonological) match. Additionally, we found two sequential effects of contextual priming: immediate interference followed short-term facilitation. Conclusions : These data have theoretical implications regarding the time course of priming effects, but also have important clinical implications. The present contextual priming procedure is relatively short and could be used as a predictor of performance patterns in a long-term treatment protocol that uses this approach or other tasks that employ priming. The authors respond on several commentaries by Friedemann Pulvermüller on their paper "Cognitive neuropsychology has been, is, and will be significant to aphasiology." Pulvermüller opines that cognitive neuropsychology (CNP) should be relevant to cognitive neuroscience only if it includes neurobiological orientation at the theoretical level. However, the authors note that CNP remains relevant and can be applied to the treatment of language disorders.

Supplemental Digital Content is Available in the Text. Objective: Of current interest in aphasia research is the relevance of what we can learn from studying word learning ability in aphasia. In a preliminary study, we addressed 2 issues related to the novel word learning ability of individuals with aphasia. First, as word learning engages large-scale cognitive-linguistic systems (language skills, verbal short-term memory [STM], other memory and executive functions), we probed whether novel word learning practice in 3 people with aphasia could stimulate these language-related systems. Second, as lesion correlates affecting word learning in aphasia remain unclear, we examined whether the structural integrity of the left arcuate fasciculus (AF) in the same 3 individuals is related to outcomes of novel word learning practice. Method: To stimulate word learning systems, our 3 participants practiced for 4 weeks with an explicit novel word—novel referent word learning task, adopted from the Ancient Farming Equipment learning paradigm (Laine & Salmelin, 2010). The participants' progress on receptive and expressive novel word learning was followed up, and their language and verbal STM abilities as well as single-session novel word learning (Learning to Name Aliens by Gupta, Martin, Abbs, Schwartz, & Lipinski, 2006) were tested before and after the practice period. To address the second question, we analyzed the participants' structural magnetic resonance images with respect to the integrity of the left AF and its overlap with the lesion areas. Results: All participants showed some receptive word learning in the trained task, as well as improvements in verbal STM span at posttesting. Two of the 3 participants also showed improved performance on some of the language outcome measures. One participant with a partially spared left AF, especially temporoparietal connections, exhibited better word learning performance than the other 2 who had larger damage and disconnection of the AF. Conclusions: Although the present results are preliminary, they open the possibility that novel word learning practice in aphasia may stimulate remaining word learning mechanisms in aphasia and thereby influence language and verbal STM abilities.
