Aim: Quality of life (QOL) in older adults can be affected by many factors, such as dementia, sarcopenia and frailty. This study sought to identify factors that can lead to global deterioration in QOL among older outpatients, including factors stated in caregiver reports. This study also evaluated the discrepancy between QOL ratings reported by the patients and those by their family or professional caregivers., Methods: The sample comprised 141 patients with a Mini-Mental State Examination score ≥20 and a Clinical Dementia Rating of 0.5-1, indicating mild cognitive impairment to mild dementia. QOL was assessed using both components of the EQ-5D scale, namely, the index score and visual analog scale. In addition, cognitive, neuropsychological, nutritional and physical function tests were carried out, and body measurements were taken., Results: Patients' self-reported QOL was significantly associated with basic activities of daily living, Mini Nutritional Assessment score and Geriatric Depression Scale-15. QOL rated by caregivers was significantly associated with basic activities of daily living. EQ-5D index scores reported by caregivers were lower than those reported by patients. Furthermore, patients achieved a better score for self-care, usual activities, pain/discomfort and anxiety/depression than those assigned by caregivers., Conclusions: As far as the patient's ratings are concerned, the presence of depressive symptoms and a worse nutritional status were indicative of lower QOL, whereas a better functional status was associated with a higher QOL. Aim: The present study aimed to: (i) examine the reliability and validity of the Dementia Assessment Sheet for Community-based Integrated Care System 21-items for classifying patients to the appropriate categories for glycemic targets in older patients; and (ii) develop a short version of the tool and examine its reliability and validity., Methods: A total of 410 older individuals were recruited for this multicenter cross-sectional study. We classified them into three categories used for determining the glycemic target in older patients in Japan based on cognitive functions and activities of daily living. Exploratory factor analyses were used to select the eight items of the shorter version. The reliability and validity of the assessment tools were assessed using Cronbach's alpha coefficients and receiver operating characteristic analyses, respectively., Results: The Dementia Assessment Sheet for Community-based Integrated Care System 21-items had three latent factors: cognitive function, instrumental activities of daily living and basic activities of daily living. The Dementia Assessment Sheet for Community-based Integrated Care System 8-items was developed based on each factor load quantity and was confirmed to have a strong correlation with the original version (r = 0.965, P < 0.001). Both tools significantly discriminated older adults belonging to category I from those belonging to category II or III, and category III from category I or II., Conclusions: Both tools had sufficient internal consistency and validity to classify older patients into the categories for determining the glycemic target in this population based on cognitive and daily functions. Aim: The need for and availability of home medical care for elderly patients with limitations in terms of access to medical facilities has been increasing. We investigated the association between low function, malnutrition, dementia and multicomorbidity with patient prognosis, focusing on unexpected hospital admissions and mortality in elderly non-cancer patients receiving home care., Methods: The study included 124 Japanese patients receiving home medical care in the form of regular visits from doctors and nurses for physical and/or mental disability., Results: Of the patients studied, 36.2% experienced hospital admission. Student's t-test showed that admitted patients had significantly higher Charlson Comorbidity Index scores. Meanwhile, 19.6% of patients died during the course of the study. Student's t-test showed that older patients had a higher risk of mortality, and significantly lower activities of daily living and Mini-Nutritional Assessment Short-Form scores. Cox hazard analysis showed that multicomorbidity was a risk for unexpected hospital admission, and malnutrition was a risk for mortality in frail older adults receiving home medical care., Conclusions: We found that multicomorbidity was a risk for unexpected hospital admission, and malnutrition was a risk for mortality in frail older adults receiving home medical care. Objectives: We investigated the differences in the physical function test results across stages from normal cognition (NC) to moderate Alzheimer disease (AD) and how risk factors of physical function decline are correlated with the physical function test results., Design: A cross-sectional study of outpatients at the Memory Disorder Outpatient Center of Japan's National Center of Geriatrics and Gerontology., Participants: We enrolled 882 individuals aged ≥65 diagnosed with NC (n = 210), amnestic mild cognitive impairment (aMCI; n = 273), mild AD (n = 181) or moderate AD (n = 197)., Measurements: We measured the participants' results for functional reach (FR), the one-leg standing (OLS) test, the Timed Up and Go (TUG) test, tandem gait (TG), and grip strength (GS). A one-way analysis of covariance (ANCOVA) was used to identify significant differences among the groups' results on the physical function tests, controlling for age, sex, educational year, Mini-Nutritional Assessment, senior activity and exercise frequency, low-density lipoprotein, body mass index, free-fat mass index, and assistance for the TUG test. Multiple regression analysis was also used to investigate the correlation between these covariates and physical function tests results., Results: The ANCOVA showed that FR, OLS, and TG were significantly worse among the individuals with aMCI, mild AD, or moderate AD compared with NC. However, TUG was significantly worse only in the moderate AD group compared with the NC, aMCI, and mild AD group. Multiple regression analysis showed that aging was correlated with poorer scores on all physical function tests, women had poorer scores on FR and GS than men, and low frequency of senior activity was significantly correlated with poorer scores on FR, OLS, and TG., Conclusion: Postural impairment and instability on TG was seen in earlier AD stages compared with instability on TUG. Background: Instrumental activities of daily living (IADL) start to decline during the progression of amnestic mild cognitive impairment (aMCI) to Alzheimer disease (AD). Cognitive and physical decline are involved in the loss of functional independence. However, little is known about AD-related neural change that leads to IADL impairment. The purpose of this study was to clarify the effects of regional white matter hyperintensity (WMH) on IADL impairment in persons with AD and aMCI., Methods: The participants were 347 female subjects aged 65-85 years diagnosed with AD (n = 227), aMCI (n = 44) or normal cognition (n = 76). IADL was assessed by the Lawton Index. Cognition, mood and mobility function were evaluated by comprehensive geriatric assessment batteries. WMH and brain atrophy were analyzed with brain magnetic resonance imaging, using an automatic segmentation program. Regional WMH was measured in the frontal, temporal, occipital and parietal lobes., Results: Ability to carry out IADL of shopping, food preparation, mode of transportation, responsibility for own medication, and ability to handle finances was obviously impaired in the early stage of AD. Frontal WMH was specifically associated with disability to do shopping and food preparation even after adjusting for several confounders including brain atrophy., Conclusions: IADL subcategories were differentially impaired along with cognitive status in persons with AD and aMCI. Frontal WMH was an important predictor of impaired ability to do shopping and food preparation. A preventive strategy for WMH might lead to suppression of IADL disability and slow the progression of AD.

Aim: Functional status of those who have very mild cognitive impairment have not been sufficiently investigated. In the current study, we analyzed the characteristics of functional awareness in older adults who had cognitive impairment and were at high risk of requiring support/care (termed as specified elderly at high risk for care needs in the long-term care insurance scheme)., Methods: The answers of a health check, which is provided by the local municipal government for those aged 75 years or older who have not been certified as eligible for care services, were analyzed. The differences of the variables between the two groups regarding yes/no answers to each of three cognition-related questions were analyzed. Then, a multiple logistic analysis was carried out to investigate the association of yes/no answers of the three cognition-related questions and the awareness of functional decline., Results: The participants who had cognitive impairment had greater awareness of functional declines. Multiple logistic regression analysis showed that subjective memory impairment and disorientation were significantly associated with a wider range of awareness of functional decline., Conclusions: Subjective cognitive impairment was associated with a wide range of awareness of functional decline in older adults at high risk for care need., (© 2012 Japan Geriatrics Society.)

Aim: In order to propose rehabilitation strategies for the reduction of falls risk, thereby preventing falls in older women, the present study was designed to explore the physical and functional factors related to actual incidence of falls., Methods: Fifty-eight female patients aged 65 years and older (mean age ± standard deviation, 80.5 ± 5.7), who were attending a geriatric outpatient clinic participated. All the participants were assessed with their activities of daily living, gait and balance using various scales. Their handgrip strength and muscle strength of lower extremities were also measured using dynamometers. Falls of the participants during the 6 months follow-up period were recorded., Results: Correlation analysis investigating associations between the scores of assessment scales and actual measurement of muscle strength and balance showed that there were significant correlations between handgrip strength and Falls Efficacy Scale, Functional Reach test, Timed Up and Go test, Berg Balance Scale, Motor Fitness Scale, motor Functional Independence Measure in fallers and non-fallers. A binary logistic stepwise regression analysis incorporating all the possible variables into the model revealed that only inability to "being able to go up and down the staircase" in the Motor Fitness Scale remained a significant variable to predict falls during the period of observation., Conclusion: The results confirmed that the sub-item in the Motor Fitness Scale has a possibility of being a significant predictor of falls in older women, and therefore might prove useful in considering specific rehabilitation program on falls prevention as well as screening this population at risk of falls., (© 2011 Japan Geriatrics Society.)

Background: The relationship between care recipients' depressive symptoms and those of caregivers remains unknown. We evaluated the association between the depressive status of caregivers and that of community-dwelling disabled care recipients., Methods: A prospective cohort study of 893 care recipients and paired caregivers was conducted. The care recipients were all eligible for a universal-coverage long-term care insurance program and their ages ranged from 65 to 104 years. They and their paired caregivers (age range 31-90 years) completed the 15-item Geriatric Depression Scale (GDS-15, score range: 0-15) assessment at baseline. The GDS-15 was used to measure the depression of caregivers and recipients with a threshold of <6/6+. The data included each care recipient's demographic characteristics, overall health status, basic activities of daily living, and comorbidities. The data also included the caregiver's demographic characteristics, including the caregiver's relationship to the recipient, and the caregiver's subjective burden as assessed by the Japanese version of the Zarit Burden Interview (ZBI)., Results: The mean GDS-15 scores of care recipients and caregivers were 6.7 points and 5.6 points, respectively. There was a positive correlation between the GDS-15 scores of caregivers and care recipients (r = 0.307, p<0.001). Multivariate logistic regression analysis adjusting for potential confounders including ZBI score indicated that the depressive symptoms of caregivers were associated with those whose care recipients were in the groups with moderate and high GDS-15 scores (OR: 1.97, 95% CI: 1.39-2.81, OR: 3.13, 95% CI: 1.87-5.24, respectively)., Conclusion: Caregivers' depressive symptoms are associated with the depressive mood of the care recipients even after adjusting for confounders including caregiver burden.

Although anthropometric parameters have been extensively studied regarding their relationship to physical function status, the association between these parameters and the activity of daily living (ADL) function remains controversial. We investigated whether BMI or mid-upper arm circumference (AC) is an indication of variation in the physical functioning of the frail elderly. The present study was a prospective cohort analysis of 543 community-dwelling frail elderly. Data included the participants' demographic characteristics, basic ADL, comorbidity and anthropometric measurements at baseline and at 2-year follow-up. Logistic regression models were used to investigate the association between ADL status and anthropometric measurements during the study period. Among the 543 participants, 418 maintained or improved their ADL status, while 125 showed an ADL decline during the study period. Multivariate logistic regression analysis showed that BMI and AC levels or ADL status at baseline were not independent predictors of the loss of ADL function or the decline in these anthropometric measurements during the study period, respectively. However, the decline in BMI and AC levels and the loss of ADL function were associated with each other during the study period. There is an association between the negative changes in anthropometric measurements during the follow-up period and the decline in ADL function during a 2-year follow-up in community-dwelling frail elderly.

Objectives: To clarify the association between unmet medication management need and 3-year mortality and hospitalization for community-dwelling older people with various levels of disabilities., Design: Prospective cohort study (the Nagoya Longitudinal Study for Frail Elderly)., Setting: Community-based., Participants: One thousand seven hundred seventy-two community-dwelling elderly subjects (611 men, 1,161 women)., Measurements: Data included the clients' demographic characteristics, a rating for basic and instrumental activities of daily living (ADLs), number of prescribed medications and physician-diagnosed chronic diseases, medication adherence, ability to manage medication, and presence or absence of medication assistance. Cox proportional hazard models and the Kaplan-Meier method were used to assess the association between the medication management at baseline and mortality or hospitalization during a 3-year period., Results: Of 1,772 participants, 681 reported no difficulty with self-medication management, and 1,091 experienced difficulty with self-medication. Of participants with difficulty with self-medication management, 929 had medication assistance, and 162 did not. During a 3-year follow up, 424 participants died, and 758 were admitted to hospitals. The baseline data demonstrated that participants not receiving medication assistance were younger and had better ADL status and fewer comorbidities. Multivariate Cox regression models adjusting for potential confounders showed that the lack of assistance in those who needed medication assistance was associated with hospitalization but not mortality during the study period., Conclusion: In community-dwelling disabled elderly people, lack of medication assistance in those needing medication support was associated with higher risk of hospitalization.

Background & Aims: Although it is not uncommon for there to be frail older people living in the community, who do not know their weight and/or height, the health-related outcomes of those older remains unknown. We examined whether missing these anthropometries are a predictor of mortality or hospitalization during a 2-year follow-up period in community-dwelling older people using various community-based services., Methods: This study was a prospective cohort analysis of 952 community-dwelling elderly. Data included the clients' demographic characteristics, basic activities of daily living (ADL), comorbidity, and anthropometric measurements at baseline. Analysis of mortality and hospitalization over the 2-year period was conducted using multivariate Cox proportional hazards models., Results: Among the 952 participants, 342 and 292 had missing data for height and weight at baseline, respectively. Multivariate Cox proportional hazards models adjusting for potential confounders showed that the lack of data on weight was associated with 2-year mortality (hazard ratio, HR:1.54, 96% CI:1.09-1.79) as well as hospitalization (HR:1.34, 95% CI:1.01-1.79) during the 2-year follow-up, although the lack of height measurement was not associated with these adverse outcomes., Conclusions: Older people living in the community with unavailable weight data appear to be more likely to have a high risk of mortality and hospitalization.

Background & Aims: It remains controversial whether mid-arm anthropometric measurements (MAAMs) are reflected with physical impairment or useful predictors of mortality in the frail elderly. We examined the following hypotheses: (1) MAAMs in frail community-dwelling elderly are lower than those of independent elderly, (2) the lower MAAMs are associated with physical function impairment, and (3) are independent predictors of 2-year mortality., Methods: This study was composed of cross-sectional and prospective cohort analyses of 957 community-dwelling elderly. Data included the clients' demographic characteristics, comorbidity, activities of daily living (ADL), and MAAMs at baseline. The mean scores of MAAMs of participants were compared with Japanese Anthropometric Reference Data. Survival analysis of 2-year mortality was conducted using multivariate Cox proportional hazards models., Results: Significantly lower arm muscle area (AMA) and higher triceps skinfold (TSF) levels were observed in most of the age groups of the study participants than those of the standard Japanese population. ADL function was correlated with AMA but not with TSF, both of which were independent risk factors for 2-year mortality in the participants (highest tertile versus lowest, AMA, HR:2.03, 95%CI:1.36-3.02; TSF, HR:1.89, 95%CI:1.30-2.75)., Conclusions: AMA and TSF were independent risk factors for 2-year mortality in the community-dwelling frail elderly.

Background and Aims: Although serum albumin is well known as a marker of nutritional status, it has remained unclear whether impaired physical function affects serum albumin concentrations in older people. We examined whether hypoalbuminemia can be used as a marker of malnutrition in elderly subjects with various levels of physical impairment., Methods: A total of 262 elderly subjects without acute illness were enrolled from various geriatric settings. For the nutritional assessment, serum albumin, total cholesterol, anthropometric measurements, and subjective global assessment (SGA) were determined. Physical function was evaluated by rating score of activity of daily living (ADL)., Results: As a whole, participants' serum albumin levels correlated with various nutritional parameters including anthropometric measurements and levels of serum total cholesterol as well as the SGA evaluation. However, after adjusting for age and gender, serum albumin levels in participants with a low ADL function did not correlate with nutritional parameters. Approximately 80% participants with low ADL function who were evaluated as being well nourished according to SGA evaluation had serum albumin levels lower than 35 g/l., Conclusions: The utility of serum albumin and the traditional cutoff (35 g/l) in older people with low ADL function is questionable even among those without inflammation.

Objectives: To assess the effects of home massage rehabilitation therapy on the bed-ridden elderly., Design: Alternatively allocated trial., Setting: Subjects' homes, three home nursing stations, 13 visit care stations and one day service centre in Aichi prefecture, Japan., Subjects: Bed-ridden patients who were 65 years and above, no dementia, stable general condition, and receiving no rehabilitation therapy., Intervention: Thirty-minute sessions of home massage rehabilitation therapy by a massage practitioner 2 or 3 days a week for three consecutive months or usual care., Main Measurements: Barthel Index (BI), Subjective Satisfaction and Refreshment Scale, Apathy Scale and Self-rating Depression Score., Results: Fifty-three subjects were recruited, 26 in the home massage rehabilitation group (HMG) and 27 in the routine care group without massage (RCG). The protocol was completed for 40 subjects, 22 in the HMG and 18 in the RCG. There were no significant differences between the baseline characteristics of both groups; age, presence of spouse, diseases associated with disabilities and use of day care rehabilitation (p = 0.76, 0.36, 0.94 and 0.71, respectively). The total BI score of the HMG (15.27+/-4.51) at baseline was nonsignificantly lower (p= 0.03) than those of the RCG (11.44+/-5.90). Subjective Satisfaction and Refreshment Scale, Apathy Scale and Self-rating Depression Score of both groups at baseline were matched (p = 0.12, 0.32 and 0.89, respectively). There were no statistical differences between the intergroup changes over time in BI, Subjective Satisfaction and Refreshment Scale, Apathy Scale and Self-rating Depression Score (p = 0.35, 0.08, 0.70 and 0.55, respectively)., Conclusion: Home massage rehabilitation therapy did not show a positive effect on the bed-ridden elderly, either mentally or physically. We would require large-size trials to determine whether it is effective.

Depression is commonly found in older adult patients and is often associated with handicaps. The authors administered the Comprehensive Geriatric Assessment (CGA), including basic activities of daily living (BADL), instrumental activities of daily living (IADL), Mini-Mental State Examination (MMSE), Geriatric Depression Scale (GDS)-15, and a socioenvironmental questionnaire to 198 patients who were admitted to Nagoya University Hospital, to examine the relationship between depressive mood and various physical and socioenvironmental outcomes. The overall GDS-15 score was correlated with the BADL and IADL. The factor analysis extracted 4 factors from the GDS-15 subscales. The factors labeled "loss of morale and hope" and "memory loss and reduction of social activity" were highly correlated with both ADLs, social variables, and the MMSE score. The results reveal that factor analysis of GDS-15 will help in understanding the etiology of depressive mood, thereby contributing to better therapeutic approaches.
