Aim: Determination of the potential of cerium chloride to reduce artificial carious mineral loss and lesion depth progression. Methods: A total of 160 enamel samples were prepared from 40 bovine lower central incisors. Crowns were sectioned into four pieces, embedded in acrylic resin, ground flat and allocated to eight groups (S1-S4 and D1-D4; n = 20). Specimens of groups D1-D4 were stored (for 7 days) in a demineralizing buffer solution to induce caries-like lesions. Afterwards, samples were treated for 30 s with one of the following solutions: placebo (S1 and D1), amine fluoride (S2 and D2), cerium chloride (S3 and D3) and a combination of fluoride and cerium chloride (S4 and D4). After another 7 (D1-D4) or 14 (S1-S4) days in demineralizing buffer solution, integrated mineral loss and lesion depth were determined by transversal microradiography and compared by Scheffé's post hoc tests. Results: In groups S1-S4, the highest values for integrated mineral loss and lesion depth were observed for group S1 (placebo), the lowest values for group S4. The results in groups S2-S4 were not significantly different. In groups D1-D4, the highest values for integrated mineral loss and lesion depth were observed for group D1 (placebo), the lowest values in groups D3 and D4. In group D2, integrated mineral loss and lesion depth were significantly lower as compared to D1, but significantly higher compared to groups D3 and D4. BACKGROUND The Bologna reform resulted in a drastic restructuring of pre-clinical training courses at the University of Zurich. The aim of this study was to assess student pre-clinical scaling/root planning skills after just 8.5 hours of manual training. MATERIAL AND METHODS Three consecutive classes of dental students (n = 41; n = 34; n = 48) were tasked with removing lacquer concrement from the maxillary left canine on a typodont using Gracey and universal (Deppeler M23A) curettes. At baseline (prior to instruction), a timed five-minute session of scaling/root planning was undertaken. The second scaling/root planning session was held immediately following training. Eight experienced dental hygienists and eight lay people served as positive and negative controls, using the same instruments and time limit, respectively. Instrumented teeth were collected, scanned and planimetrically analysed for the percentage of tooth surface cleaned. Statistical analyses were performed to assess the dental students' improvement after the training (Wilcoxon signed-rank test) and to compare it to that of laypeople and dental hygienists (Kruskal-Wallis rank sum test followed by Conover's post hoc test). RESULTS At baseline, the dental students' mean scaling scores of the cleaned surfaces were not significantly different than those of laypeople (29.8%, 31.0%, 42% vs 27.9%). However, after 8.5 hours of manual training, the students' ability to clean the maxillary tooth improved significantly and they achieved mean removal values of 61.7%, 79.5% and 76% compared to the 67.4% (P

The objective of the present study is to assess the potential of bull and boar spermatozoa and fluorescent beads to be used as a surrogate cell model to determine the cell occlusive potential in vitro using membranes of standardized porosities. A two-chamber model system consisting of upper and lower chambers, which could be separated by membranes, was constructed. Isopore polycarbonate membranes with different standardized pore diameters were used to assess the mobile cellular penetration behavior of spermatozoa or the more passive non-cellular permeability of fluorescent particles (beads) of different diameter and color. In a first experiment, spermatozoa were placed in the lower chamber, whereas semen extender only was placed in the upper chamber. After 10 min of incubation at 37 °C, the sperm number was assessed in the latter. In a second experiment, a bead solution was drawn through resorbable collagen membranes from the upper into the lower chamber by vacuum using a syringe and bead number and size was analyzed by flow cytometry. All experiments were carried out in triplicates. A non-porous polyester membrane was used as negative control to assess the overall tightness of the setup. Boar and bull spermatozoa had average cell body lengths and widths of 9 × 5 μm and were unable to pass through pores ≤2 μm, whereas they were detectable at pore sizes ≥3 μm. Their number increased with increasing pore diameters, i.e., from minimal concentrations of 0.1 × 106/ml for boar and 0.5 × 106/ml for bull spermatozoa at 3 μm to maximal concentrations of 2.1 × 106/ml for boar and 13.1 × 106/ml for bull spermatozoa at 8 μm. The fluorescent beads followed the expected pattern of permeability reliably correlating bead and pore diameter. Within the limitations of this laboratory study and the xenogeneic cell surrogate material, the model allows to easily assess cell and particle penetration through porous structures like membranes. We hope to further assess, improve, and validate this model, which we aim to use for the screening of dental membranes after being exposed to different degradation methods. Convenient and rapid test procedures to evaluate membranes for regenerative procedures are still warranted.

OBJECTIVES: The purpose of this study was to assess the shear bond strength (SBS) of metal brackets when placed with a caries protective adhesive patch. METHODS: Forty stainless steel brackets (Ormco) were bonded to 40 bovine enamel samples according to the following conditioning/bonding procedures using a resin-based orthodontic luting material (Heliosit Orthodontic, N=10 per group): (A) 35% phosphoric acid (30s), rinse and dry; (B) as in A but additional placement of a prototype adhesive patch (Ivoclar Vivadent) using a bonding agent (Heliobond); (C) application of a two-step self-etch adhesive (AdheSE); (D) as in C but additional patch placement. Samples were stored at 37 degrees C for 24h. SBS was measured with a universal testing machine with a crosshead speed of 0.5mm/min and the adhesive remnant index (ARI) was determined under a stereomicroscope (16x). RESULTS: Mean SBS (standard deviation) values were as follows: (A) 16.6 (6.4)MPa; (B) 12.2 (5.8); (C) 12.9 (5.0); and (D) 10.5 (4.7). Analysis of variance followed by Bonferroni correction revealed no statistically significant differences. In 2 (B) and 4 (D) specimens, complete retention of the adhesive patch was observed. CONCLUSIONS: All treatment groups showed adequate bond strength values. The adhesive patch could therefore be applied in combination with orthodontic brackets and seal the enamel adjacent to the bracket.

OBJECTIVES: To investigate the use of computer-assisted densitometric image analysis (CADIA) to detect incipient approximal carious lesions in vitro and to compare areas of demineralization and mechanically induced lesions by radiographic visual inspection. METHODS: Thirty-six extracted non-carious human molars were demineralized in a defined approximal area of interest (AOI) using a gel system (pH 4.8) applied over 15 weeks. Standardized digital radiographs were taken before and after creation of caries-like lesions and grayscale values were measured within the AOI. For another group of eight teeth, artificial defects were mechanically induced by a diamond bur and densitometric analysis was performed. Statistical significance was tested using the Wilcoxon test and set at p < or = 0.05. In addition, radiographs were interpreted by visual examination by six experienced dentists. The sensitivity and specificity for both study groups were determined in comparison to a baseline group of untreated teeth. Polarized light microscopy and SEM analysis were used to evaluate changes in the subsurface mineralization pattern and surface micromorphology. RESULTS: Densitometric comparison of images from identical regions in control teeth revealed no significant grayscale value differences within the AOI, whereas a significant decrease (p < or = 0.05) was observed in the demineralized group. Mechanically induced lesions also showed a significant densitometric decrease (p < or = 0.05). The sensitivity of visual inspection of radiographs of demineralized areas was 0%. In contrast, the sensitivity for the detection of a mechanical defect of 0.1mm, which was the same subsurface extension as the histologically determined demineralization, increased to 52%. Specificity was 100% for all groups. CONCLUSIONS: CADIA is able to detect very small changes in mineralization of incipient carious approximal lesions in vitro. The findings presented in this investigation may improve the subtraction method used for diagnosis and monitoring of approximal caries, as well as prophylactic measures.

Objective To compare the protective potential of a conventional adhesive, a caries infiltrant and a combination of both against acidic challenge in vitro. Methods One-hundred-and-fifty discs from bovine lower central incisors were fabricated. Seventy-five samples remained untreated, whereas the other half was subjected to a demineralisation process (14 days, acidic buffer, and pH 5) to create artificial enamel lesions. Specimens were then radioactively irradiated, and each 15 sound and demineralised specimens were treated with a caries infiltrant (Icon, DMG), an unfilled adhesive (Heliobond, IvoclarVivadent) or a combination of infiltrant and adhesive. Specimens treated with the adhesive followed by a flowable composite (TetricEvoFlow, IvoclarVivadent) served as positive control, while untreated specimens served as negative control. All samples were then subjected to lactic acid for 3 weeks at pH 4. Loss of apatite was determined using the radiochemical method of liquid scintillation. Data were statistically analysed by Kruskal–Wallis-test, one-way ANOVA and Scheffe's post hoc tests (p ≤ .05). Results In both sound enamel and artificial caries lesions, untreated specimens showed the highest rate of apatite loss, whereas enamel treated with the adhesive and the flowable composite showed almost complete protection surface against dissolution. The caries infiltrant, the adhesive and the combination of both were able to decrease enamel dissolution, but the adhesive and the combination of adhesive and infiltrant were more effective than the infiltrant alone. Conclusion Within the limitations of this in vitro study, the application of an adhesive (alone or in combination with the caries infiltrant) is more effective to protect enamel dissolution than the infiltrant alone.

To assess the bonding potential of a universal composite resin cement and an adhesive/composite system to differently pre-treated PEEK surfaces.One hundred and fifty PEEK disks were embedded in epoxy resin, polished (P4000 grit) and treated as follows (n=30/group): (A) no treatment, (B) acid etching with sulfuric acid (98%) for 1 min, (C) sandblasting for 10s with 50 microm alumina, (D) sandblasting for 10s with 110 microm alumina and (E) silica coating using the Rocatec system (3M ESPE). Polished and sandblasted (50 microm alumina) cp titanium (grade 4) served as a control. Acrylic hollow cylinders were either luted with a universal composite resin cement (RelyX Unicem) or an unfilled resin (Heliobond) and a hybrid composite (Tetric) to the specimens. Bond strength was measured in a shear test and failure modes were assessed. Statistic analysis was performed with one-way ANOVA followed by a post hoc Scheffé test and unpaired t-tests.With the universal composite resin cement, no bond could be established on any PEEK surfaces, except specimens etched with sulfuric acid (19.0+/-3.4MPa). Shear bond strength to titanium was significantly lower (8.7+/-2.8MPa, p0.05). Applying the adhesive/composite system, shear bond strength values on pre-treated PEEK ranged from 11.5+/-3.2MPa (silica coating) to 18.2+/-5.4MPa (acid etched) with no statistically significant differences (p0.05). No bond was obtained on the polished surface.Bonding to PEEK is possible when using a bonding system. No adherence can be achieved with the tested universal composite resin cement except on an etched surface. The results strongly encourage further research in PEEK application in dentistry.
