目的：系统评价慢性阻塞性肺疾病（COPD）患者抑郁的发病情况。方法检索中国生物医学文献数据库（CBM）、中国全文数据总库（CNKI）、万方数据库、维普数据库（VIP）、PubMed 2015年6月前发表的有关COPD患者抑郁患病率的文献。采用随机效应模型进行效应量合并。根据年龄、性别、地区、疾病状态、人群来源、COPD严重程度及病程探讨异质性来源，根据样本量和文献质量进行敏感性分析。结果共纳入27篇文献， COPD患者11940例，合并抑郁4439例，COPD患者抑郁总患病率为42．7％（95％CI：0．379～0．476，I2＝93．4％，P＜0．05）。亚组分析显示异质性主要受COPD病程和疾病严重程度影响，其中病程10年以上者抑郁患病率（50．4％）高于10年以下患者（23．7％），P＜0．05；重度和极重度COPD患者抑郁患病率（51．3％）高于轻中度COPD患者（26．1％）， P＜0．01。不同年龄、性别、地区、人群来源、疾病状态的COPD患者抑郁患病率差异无统计学意义。结论 COPD患者抑郁患病率较高，COPD病程较长、病情较重者抑郁患病率更高。 [ABSTRACT FROM AUTHOR]
