Patients with solid tumors have an increased risk of developing pneumonia during the management of tumor. The prevention, diagnosis, and treatment of pneumonia in these patients have different characteristics compared to the general population, such as the correction of specific risk factors for pneumonia, the rational use of prophylactic antimicrobial drugs, the strategy of invasive sampling diagnostic techniques application, the differential diagnosis from anti-tumor treatment associated pulmonary adverse events, and the consideration of pathogens and drug resistance in empirical antimicrobial treatment. In order to standardize the diagnosis, treatment, and prevention of pneumonia in patients with solid tumors, Oncology Respiratory Disease Committee of China Anti-Cancer Association initiated the "Chinese expert consensus on the diagnosis and treatment of pneumonia in patients with solid tumors (2024 edition)". Bone-modifying agents are a class of drugs that alleviate a series of bone-related events such as pain, pathologic fracture, spinal cord compression, and hypercalcemia caused by bone metastases, and currently include bisphosphonates and RANKL inhibitors. Due to the widespread use of bone-modifying agents, the adverse effects of them are gradually increasing and affecting patients' quality of life. The Breast Cancer Group, Chinese Medical Doctor Association, and the International Medical Society, Chinese Anti-Cancer Association have organized relevant experts to focus on the treatment of bone metastases of advanced malignant tumors based on evidence-based medicine, discuss the management of adverse reactions to bone-modifying agents and form the consensus. Based on the first Expert Consensus on Safety Management of Bone-modifying Agents in China, this consensus added the definition of osteonecrosis of the jaw related to bone-modifying agents, the occurrence of adverse reactions of bone-modifying drugs reported in the literature, and summarized the clinical experience of clinicians in the management of adverse reactions in practice in recent years, and ultimately, the expert group members discussed and proposed reasonable suggestions to guide clinicians in the safety management of bone-modifying agents. The incidence of malignant tumors has steadily increased year by year in China in recent decades. The emergence of innovative anti-cancer drugs has bolstered the prognosis of cancer patients, making it a chronic disease. Kidney injuries are frequent complications of cancer treatment clinically manifested as acute kidney injury (AKI), chronic kidney disease (CKD), proteinuria, and electrolyte disorders, etc. Although kidney injuries affect up to 50% of the patients, there is a lack of professional management guidance for anti-cancer drug dosage adjustment. The clinical pharmacology branch of the Chinese Medical Association has initiated the "Chinese expert consensus on the guidance of anti-tumor drugs for patients with tumor combined chronic kidney disease (2024 edition)". We unified national multidisciplinary experts to analyze clinical evidence with the Delphi method, and conducted expert interviews and seminars focusing on kidney function assessment, anti-tumor drug dosage adjustment for kidney disease patients. Cancer related anemia (CRA) is a common side effect in patients with tumors, the incidence of which is related to tumor type, treatment regimen, the duration of chemotherapy, etc. The pathogenesis of CRA has not been fully defined. CRA may lead to chemotherapy dose reduction or may even delay chemotherapy. Patients with CRA require red blood cell transfusion, thus increasing the treatment cost, reducing the efficiency of chemotherapy and the patient′s quality of life, and shortening the survival time. The main treatments of CRA include red blood cell transfusion, iron supplements, erythropoietin, and so on. Based on recent literature and clinical studies, the expert committee of the China Anti-Cancer Association drew up the consensus on the diagnosis and treatment of anemia related to tumor in China (2023 edition). Poly ADP-ribose polymerase inhibitors (PARPi), which approved in recent years, are recommended for ovarian cancer, breast cancer, pancreatic cancer, prostate cancer and other cancers by The National Comprehensive Cancer Network (NCCN) and Chinese Society of Clinical Oncology (CSCO) guidelines. Chemotherapy-induced neutropenia (CIN) is a common hematological adverse events and dose-limiting toxicities of chemotherapy. Treatment using oncolytic virus as a new tumor immunologic agent has made continuous breakthroughs in basic and clinical research of tumor in recent years, and it is expected to play an important role in cancer immunotherapy in the future. At present, there are 4 oncolytic viruses approved for marketing in the world, among which recombinant human type-5 adenovirus (H101) is the only oncolytic virus approved in China. To further standardize the clinical research practice of oncolytic virus in tumor and improve the clinical application of cancer immunotherapy, this consensus is established based on the "Shanghai expert consensus on clinical application of oncolytic virus in the treatment of malignant tumors (2021 edition)" and guided by evidencebased methodology, focusing on the application scope of oncolytic virus approved for marketing at home and abroad, and the clinical studies of oncolytic virus with high level of evidence but not approved at present, looking forward to the future research and development trend of oncolytic virus, clarifying the specific use method of oncolytic virus (mainly recombinant oncolytic adenovirus), and exploring the new modes of follow-up, efficacy evaluation and other aspects. On the basis of literature retrieval, quality evaluation and evidence summary were carried out, and specific issues and hot topics such as the application scope, combination with antiviral drugs, gene detection and other common concerns in the clinical application of recombinant oncolytic adenovirus in cancer were discussed and investigated through multidisciplinary expert meetings, Delphi methods and other forms. Malignant tumors are the leading causes of death from chronic non-communicable diseases and the most important factors influencing life expectancy in recent year, with poor treatment response and poor prognosis. Metformin, the preferred hypoglycemic drug for diabetes mellitus type 2, has been recognized by more and more peers for its anti-tumor effect. However, there is no independent clinical guideline, consensus and large prospective clinical trial at home and abroad. This expert guideline aimed to provide clinical practice reference for the application of metformin in anti-tumor treatment. This expert consensus recommends that metformin, used in combination for most malignancies with diabetes mellitus type 2, can assist in anti-tumor effect and enhance chemotherapeutic drug sensitivity, and reduce the morbidity and mortality of multiple malignancies. Metformin is not recommended or opposed for a small number of malignancies complicated with diabetes mellitus type 2, such as estrogen receptor (ER)-negative or triple-negative breast cancer. Metformin is not recommended for most malignant tumors without diabetes, such as lung cancer, colorectal cancer and prostate cancer. Neurologic immune-related adverse events (NAEs) are rare complications of immune checkpoint inhibitors (ICI). NAEs can affect the central nervous system, peripheral nervous system and neuroendocrine system, they can lead to death and serious dysfunction. NAEs requires standardized diagnosis, treatment and clinicians' high attention. The diagnosis of NAEs is very challenging due to its complexity, diversity and some non-specific clinical manifestations. It needs to be carefully distinguished from neurological dysfunction caused by other diseases such as tumor, infection, metabolism and iatrogenic (non-immune mediated) complications. Therefore, Committee of Neoplastic Supportive-Care of China Anti- Cancer Association and Cancer Clinical Chemotherapy Committee of China Anti-Cancer Association organized experts to conduct literature analysis and evidence level discussion on the clinical key issues related to NAEs, including the epidemiology, pathogenesis, risk factors, general principles of diagnosis and treatment, clinical manifestations and diagnosis and treatment strategies of specific types of NAEs, and the principles of ICI rechallenge after NAEs. Based on the latest clinical evidence and combined with China' s clinical practice, the expert committee finally formulated a Chinese expert consensus on diagnosis and treatment of nAEs (2022 edition) for the prevention, diagnosis, comprehensive treatment and follow-up of NAEs. Chemotherapy-induced peripheral neuropathy (CIPN) is a serious clinical problem and challenging for oncologists. CIPN is often a persistent adverse consequence of certain chemotherapeutic agents and more cancer survivors will experience CIPN leading to chronic pain and worsening quality of life. However, the available and effective strategies for clinical treatment of CIPN are very limited. Oncologists are frequently obliged to decrease or stop neurotoxic anticancer drugs, with a possible deleterious impact on the oncological prognostic. The challenges faced by CIPN include further study on the pathological mechanism, dose threshold, incidence, risk factors and clinical characteristics of CIPN; lack of diagnostic criteria and tools of CIPN; lack of effective and standardized CIPN prevention and treatment programs. The current update of research results on these challenging issues of CIPN will provide more decision-making evidence for oncologists to diagnose and treat CIPN. Patients with malignant tumors are often combined with dyslipidemia, and dyslipidemia is associated with both malignant tumor disease and treatment. The expert group of the Integrative Cardio-Oncology Society of China Anti-Cancer Association summarized the incidence rate and characteristics of dyslipidemia in patients with malignant tumors, and the characteristics and mechanisms of dyslipidemia induced by common antineoplastic drugs, stratified the risk of dyslipidemia in antineoplastic drugs, then formulated the principles of dyslipidemia treatment and monitoring. The Immune checkpoint inhibitors (ICIs) have represented a major breakthrough in the field of malignant tumor treatment. However, due to the mechanism of this novel therapy, some special forms of response modes, such as pseudoprogression and hyperprogression, have arisen, which bring difficulties and challenges to the therapeutic efficacy evaluation. In 2017, the European association of nuclear medicine reported the advantages of using 18-fluoro-2-deoxy-D-glucose (18F-FDG) PET/CT to evaluate the tumor immunotherapy response and immune-related side effects based on the published clinical trial data. In China, a large amount of research data has been generated since the clinical study of immune checkpoint inhibitors officially started in 2013.Therefore, how to utilize PET/CT reasonably and standardly in tumor immunotherapy has become an urgent clinical and scientific issue. Focusing on the practical questions of examination procedures, PET/CT image interpretation and the evaluation criteria after ICIs treatment, this consensus has been finally formed, which based on the combination of literature, expert experiences, and internal discussion among committee members in the PET Group of Chinese Society of Nuclear Medicine.This consensus is also expected to improve the guide for standardizing the application of PET/CT to evaluate tumor immunotherapy response and promoting the technology in corresponding field.
