International audience; Liver stereotactic body radiotherapy is a developing technique for the treatment of primary tumours and metastases. Its implementation is complex because of the particularities of the treated organ and the comorbidities of the patients. However, this technique is a treatment opportunity for patients otherwise in therapeutic impasse. The scientific evidence of liver stereotactic body radiotherapy has been considered by the French health authority as insufficient for its widespread use outside specialized and experienced centers, despite a growing and important number of retrospective and prospective studies, but few comparative data. This article focuses on the specific features of stereotactic body radiotherapy for liver treatments and the results of published studies of liver stereotactic body radiotherapy performed with classic linear accelerators and dedicated radiosurgery units.; La radiothérapie stéréotaxique hépatique est une technique de traitement en développement pour le traitement des tumeurs primitives et secondaires. Sa mise en œuvre est complexe du fait des particularités de l’organe traité et des comorbidités des patients. Elle représente cependant une opportunité de traitement efficace pour beaucoup de patients en impasse thérapeutique. Son niveau de preuve est jugé insuffisant par la Haute Autorité de Santé (HAS) pour une large diffusion en dehors des centres expérimentés. Il existe pourtant un nombre conséquent et grandissant d’études rétrospectives ou prospectives, mais peu de données comparatives. Cet article résume les particularités de la radiothérapie stéréotaxique pour cette localisation et reprend les résultats des études publiées pour les machines dédiées et non dédiées.

International audience; Breast cancers are highly radiosensitive since the risk of recurrences and of mortality after adjuvant radiotherapy following breast-conserving surgery is decreased by 15.7% and 3.8%, respectively at 10 years. The total dose if irradiation also significantly increases local control: a boost of 16 Gy to the tumour bed after breast-conserving surgery reduces the absolute risk of recurrence by 4% at 10 years. Breast cancers are sensitive to the dose per fraction, as shown by the results from four randomized trials which compared standard irradiation (50 Gy/25 fractions) with a hypofractionated scheme: no statistical difference was observed in locoregional recurrence and overall survival at 10 years. The α/β ratio, which reflects the dose per fraction and is theoretically over 10 Gy for tumour tissues, has been estimated between 2.2 and 4.4 Gy for breast cancers. Molecular abnormalities, such as overexpression of HER1 (especially in triple negative breast cancer) and HER2, induce a higher radioresistance. In vitro studies showed that targeted therapies, which block these receptors, increase breast cancer radiosensitivity. Tumour stem cells have been identified in breast cancers and are characterized by a higher radioresistance. This radioresistance could be related to a better repair of radiation-induced DNA damages and a decrease of reactive oxygen species (ROS), which are involved in their occurrence. In the future, a better understanding of genetics tumour abnormalities will allow to identify new radiosensitivity settings in breast cancers.; Les cancers du sein sont très radiosensibles puisqu’après une chirurgie conservatrice, avec un recul de 10 ans, la radiothérapie permet de diminuer le risque absolu de récidive locorégionale de 15,7 % et le risque absolu de mortalité de 3,8 %. De même, la dose totale influence le contrôle local puisqu’un complément de dose de 16 Gy dans le lit tumoral après chirurgie conservatrice permet de diminuer le risque absolu de récidive locale de 4 % à 10 ans. Le dogme classique de l’absence de sensibilité des cancers à la dose par fraction a été remis en question dans les cancers du sein puisque les quatre essais randomisés ayant comparé une irradiation de 50 Gy en 25 fractions et une irradiation hypofractionnée n’ont pas montré de différence statistiquement significative en termes de récidive locorégionale et de survie globale avec un recul de 10 ans. Le rapport α/β, qui reflète la sensibilité à la dose par fraction et qui est théoriquement de plus de 10 Gy dans les tissus tumoraux, a été estimé entre 2,2 et 4,4 Gy pour les cancers du sein selon les études. Certaines anomalies moléculaires telles qu’une surexpression des récepteurs HER1 (notamment dans les tumeurs triples négatives) et HER2 entraînent une plus grande radiorésistance. Des études in vitro ont montré que des thérapies ciblées bloquant ces récepteurs permettaient d’augmenter la radiosensibilité de ces tumeurs. Des cellules souches tumorales ont été mises en évidence dans les cancers du sein et sont caractérisées par une plus grande radiorésistance. Cette radiorésistance pourrait être liée à une meilleure réparation des lésions de l’ADN et à une diminution d’espèces réactives de l’oxygène (reactive oxygen species [ROS]), qui est impliquée dans les lésions de l’ADN radio-induites. L’exploration du patrimoine génétique tumoral permettra dans les prochaines années de mette en évidence de nouveaux paramètres de radiosensibilité dans les cancers du sein.

Owing to breast cancer screening, breast cancer is more and more diagnosed at early stage. For those breast cancer women, breast conserving treatment (breast conserving surgery followed by whole breast irradiation) is commonly used since many years. New radiation modalities have been recently developed in early breast cancers particularly accelerated partial breast irradiation (APBI). Among all techniques of radiotherapy, 3D-conformal APBI and intraoperative radiotherapy (IORT) are the main modalities of radiotherapy used. The present review states on indications, treatment modalities and updated results of local control and side effects of partial breast irradiation.

In stage IV cancers, locoregional management of primitive tumours as surgery and/or radiotherapy improved both progression-free survival and overall survival. Among metastatic cancer patients, some of them are considered as oligometastatic stage as they present few metastatic sites associated with low tumor aggressiveness. For those patients, metastatic local control, and therefore prolonged time to progression should be reached through local treatments as surgery and/or radiofrequency ablation and/or stereotactic radiotherapy. Here we propose a review of oligometastatic stage and results from extracranial stereotactic radiotherapy in terms of efficacy and tolerance.

International audience; Toll-like receptors are ubiquitous and very well conserved throughout evolution, with important functions mediating innate and adaptative immunological mechanisms. The importance of these receptors and their agonists has been recently pointed out in immunology and cancerology, although the accurate underlying mechanisms are still under investigation. The association of agonists of these receptors with ionizing radiation has been studied in preclinical experiments with promising results. Part of these compounds is flagellin, which seems to be able to modulate the radiosensitivity of both tumors and healthy tissues.
