The ecological role of intertidal microphytobenthos (MPB) is increasingly recognized in coastal production systems. MPB primary production (PP) measured in coastal wetlands of Korea, Cambodia, and Australia confirmed large variability at the global scale. Surprisingly, MPB biomass in mangrove forests almost doubled those measured in nearby bare tidal flats. However, MPB productivity (P b ) in vegetated habitats was significantly reduced (by ~50%) compared to that on bare tidal flats. Extensive measurements of MPB biomass, PP, and P b across 12 Korean tidal flats revealed large spatiotemporal variations, suggesting complex sediment-MPB coupled dynamics. The key factors included sediment type, tide, bed elevation, irradiation, temperature, and vegetation. Winter MPB blooms and the elevated P b seem to be unique characteristics of the Korean intertidal flats. The oxidation of sulfide ores is a common phenomenon. To better understand the current development and status of oxidation studies of sulfide ores (OSSO), a bibliometric analysis of OSSO was conducted by mapping the spatiotemporal distribution of the knowledge domain and the research focus using VOSviewer and Citespace tools. The data were derived from the Web of Science (WOS) core collection database from 2000 to 2018. Study emphases covered publication outputs, countries/regions, organizations, top journals, research focus and keyword co-occurrence network, and theme development. The results include the following findings: (1) The line of the 3-year moving average of publications (3-year MAP), h-index (3-year MAH), and authors (3-year MAA) increased from 2001 to 2018. Conversely, the h-index continuously declined. (2) Asia had the most publications, with 1052, followed by Europe, with 923, and America, with 767. China, the USA, and Australia are the most active countries. (3) The top 10 organizations with the most publications are five Chinese organizations and one organization from each of the following countries: Russia, Australia, France, and India. (4) Hydrometallurgy, Minerals Engineering, Ore Geology Reviews, Geochimica Et Cosmochimica Acta, and Economic Geology are among the top 10 journals that researchers are most concerned about. (5) Cooperation among different organization or different countries is the most effective way to produce the most influential papers. (6) The OSSO is still focused on the process of oxidation by using different methods and techniques. In future work, it is necessary to progress new methods to understand the process of self-heating and prevent spontaneous combustion disaster of sulfide ore which result from OSSO.

Until the latter part of the nineteenth century, there were no domestic animals other than pigs, dogs and poultry in the island of New Guinea. From 1889 onwards, occupying authorities, missionaries and settlers from Germany, the UK, Japan and Australia imported ruminants, pigs and horses. Some of these importations were from Asia. Background: Understanding of sex differences, especially in terms of the influence of sex on therapeutic interventions, can lead to improved treatment and management for all., Aim: We examined temporal and regional trends in female participation and the reporting of sex differences in stroke randomized controlled trials., Methods: Randomized controlled trials from 1990 to 2018 were identified from ClinicalTrials.gov, using keywords "stroke" and "cerebrovascular accidents." Studies were selected if they enrolled ≥100 participants, included both sexes and were published trials (identified using PubMed, Google Scholar, and Scopus)., Results: Of 1700 stroke randomized controlled trials identified, 277 were published and eligible for analysis. Overall, these randomized controlled trials enrolled only 40% females, and in the past 10 years, this percentage barely changed, peaking at 41% in 2008-2009 and 2012-2013. North American randomized controlled trials recruited the most women, at 43%, and Asia the lowest, at 40%. Among the 277 randomized controlled trials, 101 (36%) reported results according to sex, of which 91 (33%) were pre-specified analyses. The increasing trend in the number of studies reporting sex-differentiated results from 2008 to 2018 merely paralleled the increase in the number of papers published during the same time period. North American randomized controlled trials most often reported sex-specific results (42%), and Australia and Europe least often (31%)., Conclusion: Little progress has been made in the inclusion of females and the reporting of sex in stroke randomized controlled trials. This highlights the need for key stakeholders, such as funders and journal editors, to provide clear guidance and effective implementation strategies to researchers in the scientific reporting of sex.

Urothrips is one of a group of 10 genera of fungus-feeding Phlaeothripinae in which species have the tenth abdominal segment greatly elongate and bearing long anal setae (Mound 1972; Okajima 2006). The genus is recorded from Africa, Asia and Australia (ThripsWiki 2019), and currently comprises 11 species. A key to nine of these species was provided by Ulitzka and Mound (2014), to which two further species from China were added subsequently (Tong Zhao 2017; Zhao Tong 2017). The purpose of this paper is to describe a new species of Urothrips. This is the fifth member of the genus recorded from China, the others being U. calvus Tong Zhao, U. lancangensis Zhao Tong, U. gibberosa Kudo and U. tarai Stannard.

During the past two centuries metal loads in the Earth's atmosphere and ecosystems have increased significantly over pre-industrial levels. This has been associated with deleterious effects to ecosystem processes and human health. The magnitude of this toxic metal burden, as well as the spatial and temporal patterns of metal enrichment, is recorded in sedimentary archives across the globe. This paper presents a compilation of selected Pb contamination records from lakes (n = 10), peat mires (n = 10) and ice fields (n = 7) from Europe, North and South America, Asia, Australia and the Northern and Southern Hemisphere polar regions. These records quantify changes in Pb enrichment in remote from source environments. The presence of anthropogenic Pb in the environment has a long history, extending as far back as the early to mid-Holocene in North America, Europe and East Asia. However, results show that Pb contamination in the Earth's environment became globally ubiquitous at the beginning of the Second Industrial Revolution (c.1850-1890 CE), after which the magnitude of Pb contamination increased significantly. This date therefore serves as an effective global marker for the onset of the Anthropocene. Current global average Pb enrichment rates are between 6 and 35 times background, however Pb contamination loads are spatially variable. For example, they are >100 times background in Europe and North America and 5-15 times background in Antarctica. Despite a recent decline in Pb loads in some regions, most notably Europe and North America, anthropogenic Pb remains highly enriched and universally present in global ecosystems, while concentrations are increasing in some regions (Australia, Asia and parts of South America and Antarctica). This study explored national differences in plagiarism and duplicate publication in retracted biomedical literature. The national affiliations of authors and reasons for retraction of papers accessible through PubMed that were published from 2008 to 2012 and subsequently retracted were determined in order to identify countries with the largest numbers and highest rates of retraction due to plagiarism and duplicate publication. Authors from more than fifty countries retracted papers. While the United States retracted the most papers, China retracted the most papers for plagiarism and duplicate publication. Rates of plagiarism and duplicate publication were highest in Italy and Finland, respectively. Unethical publishing practices cut across nations.

Increasingly, students with English as a second language (ESL) are enrolled in nursing degrees in English speaking countries (Wang et al., 2008). However, they may be at risk of clinical practice failure due to communication difficulties associated with unfamiliar linguistic and cultural factors (Guhde, 2003). This paper describes and evaluates an innovation to assist ESL nursing students at an Australian university develop their clinical communication skills and practice readiness by providing online learning resources, using podcast and vodcast technology, that blend with classroom activities and facilitate flexible and independent learning. The innovation builds on an intensive clinical language workshop program called 'Clinically Speaking' which has evolved through a cyclical process of ongoing research to produce resources in response to students' learning needs. Whilst uptake of the resources was modest, students of ESL as well as English speaking backgrounds (ESB) found the resources improved their clinical preparation and confidence by increasing their understanding of expectations, clinical language and communication skills. The innovation, developed with a modest budget, shows potential in developing ESL and ESB students' readiness for clinical communication, enabling them to engage in clinical practice to develop competency standards required of nursing graduates and registration authorities., (Copyright © 2012 Elsevier Ltd. Allergen immunotherapy is a subject widely debated by allergists. Currently, there are controversial discussions focused on the sublingual route. Sublingual immunotherapy (SLIT) has so far been used in Europe, Asia and Australia for the treatment of allergic respiratory diseases. The minimum age to start specific immunotherapy with inhalant allergens in children has not been clearly established, and position papers discourage its use in children younger than 5 years of age. Nevertheless, it is known that SLIT efficacy is higher when SLIT is started at an earlier age. The aim of this review is to focus on studies in preschool children evaluating SLIT safety and efficacy, in order to improve this practice at an earlier age in childhood.

Ethnopharmacological Relevance: GP-TCM is the first EU-funded Coordination Action consortium dedicated to traditional Chinese medicine (TCM) research. One of the key deliverables of the Work Package 7 in GP-TCM was to investigate information of the existing requirements for registration of TCM products listed by global regulatory bodies. The paper aims to collate data and draw comparison of these regulations. Case studies are also presented to illustrate the problems involved in registering TCM products in different regions worldwide., Materials and Methods: A collaborative network task force was established during the early stage of the GP-TCM project and operated through exchanges, teleconferences and focused discussions at annual meetings. The task force involved coordinators, academics who are actively involved with R&D of Chinese herbal medicines, experts on monographic standards of Chinese materia medica, representatives from regulatory agencies, experts from industries in marketing Chinese medicines/herbal medicines and natural products. The co-ordinators took turns to chair teleconferences, led discussions on specific issues at AGM discussion sessions, at joint workshops with other work-packages such as WP1 (quality issues), WP3 (toxicology issues) and WP6 (clinical trial issues). Collectively the authors were responsible for collating discussion outcomes and updating written information., Results: A global overview of regulations on herbal registration has been compiled during the three years of the consortium. The regulatory requirements for registration of herbal products in the EU and China were compared, and this is extended to other regions/countries: Africa, Australia, Brazil, Canada, Japan, Russia, South Korea, Taiwan, and the United States. A wide variation of the regulations for the categories of herbal products exists: food (functional food, novel foods, dietary food for special medical purpose, foods for particular nutritional use, food supplement); cosmetic, traditional herbal medicine products; herbal medicines for human use and veterinary use., Conclusion: The regulatory issues for registration of herbal products are complicated among the countries and regions worldwide. The information summarised in the text is for reference only. Some regulations which are presented in this review are still in legislation process and may change in due course. Before taking any regulatory action, readers are advised to consult current official legislation and guidance and/or to seek appropriate professional advice. The lessons learnt from global regulation of TCM will provide valuable insights for regulation of other traditional medicine such as Ayurveda and Unani medicine, as well as other forms of indigenous medicine. This paper investigates species delimitation within the Lactarius gerardii species complex and explores its taxonomic and geographical extent. A combined molecular phylogeny based on ITS, LSU and rpb2 gene sequences is constructed and morphological characters are evaluated. While L. gerardii was originally described from North America, it has later been reported from all over Asia. Therefore a worldwide sampling range was aimed at, including species exhibiting morphological affinities with L. gerardii. The phylogenetic analyses indicate that intercontinental conspecificity in L. gerardii is absent. Thirty strongly supported clades are retrieved of which 18 are morphologically identifiable species. The group is elevated to Lactarius subg. Gerardii stat. nov. It includes, apart from L. gerardii s.l., L. atrovelutinus, L. bicolor, L. ochrogalactus, L. petersenii, L. reticulatovenosus, L. sepiaceus, L. subgerardii and L. wirrabara, as well as the pleurotoid L. uyedae. The paraphyletic nature of the genus Lactarius is confirmed. Lactarius subg. Gerardii appears not affiliated with L. subg. Plinthogalus and this can be substantiated morphologically. No representatives are known from Europe, Africa or South America. The high frequency of intercontinental sister relationships observed between America, Asia and the Australian region, suggests multiple migration and speciation events have occurred across continents., (Copyright © 2010 The British Mycological Society. As reviewed in this paper, meningococcal disease epidemiology varies substantially by geographic area and time. The disease can occur as sporadic cases, outbreaks, and large epidemics. Surveillance is crucial for understanding meningococcal disease epidemiology, as well as the need for and impact of vaccination. Despite limited data from some regions of the world and constant change, current meningococcal disease epidemiology can be summarized by region. By far the highest incidence of meningococcal disease occurs in the meningitis belt of sub-Saharan Africa. During epidemics, the incidence can approach 1000 per 100,000, or 1% of the population. Serogroup A has been the most important serogroup in this region. However, serogroup C disease has also occurred, as has serogroup X disease and, most recently, serogroup W-135 disease. In the Americas, the reported incidence of disease, in the range of 0.3-4 cases per 100,000 population, is much lower than in the meningitis belt. In addition, in some countries such as the United States, the incidence is at an historical low. The bulk of the disease in the Americas is caused by serogroups C and B, although serogroup Y causes a substantial proportion of infections in some countries and W-135 is becoming increasingly problematic as well. The majority of meningococcal disease in European countries, which ranges in incidence from 0.2 to 14 cases per 100,000, is caused by serogroup B strains, particularly in countries that have introduced serogroup C meningococcal conjugate vaccines. Serogroup B also predominates in Australia and New Zealand, in Australia because of the control of serogroup C disease through vaccination and in New Zealand because of a serogroup B epidemic. Based on limited data, most disease in Asia is caused by serogroup A and C strains. Although this review summarizes the current status of meningococcal disease epidemiology, the dynamic nature of this disease requires ongoing surveillance both to provide data for vaccine formulation and vaccine policy and to monitor the impact of vaccines following introduction.

This paper analyses contemporary Australian newspaper coverage of the threat of pandemic influenza in humans, specifically in the light of recent transformations in biomedical and public health understandings of infectious disease as continuously emerging. Our analysis suggests that the spectre of pandemic influenza is characterised, in newspaper accounts, as invoking a specific form of nation building. The Australian nation is depicted as successfully securing itself in the face of a threat from Asia (and in the absence of an effective international health body). What is described in newspaper accounts reflects a shift in the public health response to infectious disease. This response does not entail a direct focus on protecting either the population or national territory. Instead, it involves the continuous rehearsal of readiness to react to disasters through the networking of government and private agencies responsible for maintaining critical infrastructure. In this way, coverage of pandemic influenza positions health as central to national security, with little reporting of the reasons for or the potential implications of this alliance. Thus, the imperative to 'be prepared' is presented as self-evident.

The primary role of the extensive and complex modern food legislation is to protect the consumer. Providing a framework for industry and enabling free trade are secondary aims. In the EU the 2006 Regulation on nutrition and health claims made on foods was adopted in December 2006. This Regulation defines detailed lists of permitted claims with precise conditions, requires foods making claims to meet specific nutrient profiles and requires the submission of a dossier for approval of new health claims. Nutrient profiles and an initial list of existing health claims will not be agreed until January 2009 and January 2010 respectively. The uncertainty about profiles and the initial list of claims as well as the prescriptive nature of the Regulation will have a major impact, some negative but some positive, on food innovation. Worldwide legislation on nutrition and health claims continues to develop. The current paper also provides an outline of some other key pieces of European legislation that affect food innovation. However, currently, all this legislation remains in development and up-to-date information can be sought from the reference material provided.

The status of oral and maxillofacial radiology in other countries is often of importance when making an application for specialty recognition. This information has not previously been readily available. This paper presents the results of a survey to determine the status of oral and maxillofacial radiology worldwide in 2007.

Registries, collecting observational data, can be of great value in our understanding of clinical questions, since they reflect 'real practice in a real world'. This paper provides an overview of what registry data from different regions of the world have taught us on survival outcomes of peritoneal dialysis patients compared to hemodialysis patients. The majority of the comparative studies show that patient survival on peritoneal dialysis is either similar or even better than on hemodialysis, at least for the first few years after starting dialysis. These results support a policy of peritoneal dialysis as the first dialysis modality in a program of integral care.

Background: The lack of reference sequences from well-identified mycorrhizal fungi often poses a challenge to the inference of taxonomic affiliation of sequences from environmental samples, and many environmental sequences are thus left unidentified. Such unidentified sequences belonging to the widely distributed ectomycorrhizal fungal genus Inocybe (Basidiomycota) were retrieved from GenBank and divided into species that were identified in a phylogenetic context using a reference dataset from an ongoing study of the genus. The sequence metadata of the unidentified Inocybe sequences stored in GenBank, as well as data from the corresponding original papers, were compiled and used to explore the ecology and distribution of the genus. In addition, the relative occurrence of Inocybe was contrasted to that of other mycorrhizal genera., Results: Most species of Inocybe were found to have less than 3% intraspecific variability in the ITS2 region of the nuclear ribosomal DNA. This cut-off value was used jointly with phylogenetic analysis to delimit and identify unidentified Inocybe sequences to species level. A total of 177 unidentified Inocybe ITS sequences corresponding to 98 species were recovered, 32% of which were successfully identified to species level in this study. These sequences account for an unexpectedly large proportion of the publicly available unidentified fungal ITS sequences when compared with other mycorrhizal genera. Eight Inocybe species were reported from multiple hosts and some even from hosts forming arbutoid or orchid mycorrhizae. Furthermore, Inocybe sequences have been reported from four continents and in climate zones ranging from cold temperate to equatorial climate. Out of the 19 species found in more than one study, six were found in both Europe and North America and one was found in both Europe and Japan, indicating that at least many north temperate species have a wide distribution., Conclusion: Although DNA-based species identification and circumscription are associated with practical and conceptual difficulties, they also offer new possibilities and avenues for research. Metadata assembly holds great potential to synthesize valuable information from community studies for use in a species and taxonomy-oriented framework.

Objective: In this paper, we review evidence of HCV prevalence among injecting drug users (IDUs) worldwide., Methods: We undertook a desk-based review of both 'grey' and published literature released between 1998 and 2005., Results: Data on HCV prevalence among IDUs was found in 57 countries and in 152 sub-national areas. We found reports of HCV prevalence of at least 50% among IDUs in 49 countries or territories. Available regional estimates varied widely, from 10 to 96% in Eastern Europe and Central Asia, from 10 to 100% in South and South-East Asia, from 34 to 93% in East-Asia and the Pacific, from 5 to 60% in North Africa and the Middle-East, from 2 to 100% in Latin America, from 8 to 90% in North America, from 25 to 88% in Australia and New Zealand, and from 2 to 93% in Western Europe. Only in Colombia and Lebanon were all HCV prevalence estimates below 20%. In addition, evidence of HIV/HCV co-infection among IDUs was found in 16 countries. In China, Poland, Puerto Rico, Russia, Spain, Switzerland, Thailand and Viet Nam, estimates of the prevalence of HIV/HCV co-infection among IDUs reached 90%., Discussion: Taken together, data suggest high global prevalence of HCV and HIV/HCV co-infection among IDUs. We suggest exploring protective factors in sites of low HCV prevalence.

This paper provides an overview of opportunities for graduate education in Gerontology in the Asia-Oceania Region. It locates emerging demand in a demographic context, highlighting high rates of growth in the aged population within the Region and growing awareness of governments of the need for appropriate planning. An important component of infrastructure development will be professional access to specialised education about ageing. Of postgraduate programs currently available, most are offered by Australian universities. The trend among providers is towards flexible delivery by distance education using a variety of models, including traditional print-based independent study, cross-national partnerships using block or blended teaching models, and predominantly online education. An innovative model developed by a leading local provider, The University of Sydney, is described. StudyAgeOnline is a Web-supported postgraduate coursework program in ageing and aged care for practising health professionals in which short courses can be undertaken as stand-alone modules or build towards a Masters degree. We describe a typical module and discuss how learning and teaching strategies have been designed for practical relevance in a multi-professional and multi-cultural "virtual classroom."

Background: El Niño/Southern Oscillation (ENSO) related climate anomalies have been shown to have an impact on infectious disease outbreaks. The Climate Prediction Center of the National Oceanic and Atmospheric Administration (NOAA/CPC) has recently issued an unscheduled El Niño advisory, indicating that warmer than normal sea surface temperatures across the equatorial eastern Pacific may have pronounced impacts on global tropical precipitation patterns extending into the northern hemisphere particularly over North America. Building evidence of the links between ENSO driven climate anomalies and infectious diseases, particularly those transmitted by insects, can allow us to provide improved long range forecasts of an epidemic or epizootic. We describe developing climate anomalies that suggest potential disease risks using satellite generated data., Results: Sea surface temperatures (SSTs) in the equatorial east Pacific ocean have anomalously increased significantly during July - October 2006 indicating the typical development of El Niño conditions. The persistence of these conditions will lead to extremes in global-scale climate anomalies as has been observed during similar conditions in the past. Positive Outgoing Longwave Radiation (OLR) anomalies, indicative of severe drought conditions, have been observed across all of Indonesia, Malaysia and most of the Philippines, which are usually the first areas to experience ENSO-related impacts. This dryness can be expected to continue, on average, for the remainder of 2006 continuing into the early part of 2007. During the period November 2006 - January 2007 climate forecasts indicate that there is a high probability for above normal rainfall in the central and eastern equatorial Pacific Islands, the Korean Peninsula, the U.S. Gulf Coast and Florida, northern South America and equatorial east Africa. Taking into consideration current observations and climate forecast information, indications are that the following regions are at increased risk for disease outbreaks: Indonesia, Malaysia, Thailand and most of the southeast Asia Islands for increased dengue fever transmission and increased respiratory illness; Coastal Peru, Ecuador, Venezuela, and Colombia for increased risk of malaria; Bangladesh and coastal India for elevated risk of cholera; East Africa for increased risk of a Rift Valley fever outbreak and elevated malaria; southwest USA for increased risk for hantavirus pulmonary syndrome and plague; southern California for increased West Nile virus transmission; and northeast Brazil for increased dengue fever and respiratory illness., Conclusion: The current development of El Niño conditions has significant implications for global public health. Extremes in climate events with above normal rainfall and flooding in some regions and extended drought periods in other regions will occur. Forecasting disease is critical for timely and efficient planning of operational control programs. In this paper we describe developing global climate anomalies that suggest potential disease risks that will give decision makers additional tools to make rational judgments concerning implementation of disease prevention and mitigation strategies.

Helminthosis is one of the major constraints in the successful wool and mutton industry throughout the world. Anthelmintic Resistance (AR) is said to have been established when previously effective drug ceases to kill exposed parasitic population at the therapeutically recommended dosages. Anthelmintic resistance is almost cosmopolitan in distribution and it has been reported in almost all species of domestic animals and even in some parasites of human beings. Some of the most important species of parasites of small ruminants in which AR has been reported include: Haemonchus spp., Trichostrongylus spp. Teladorsagia spp., Cooperia spp. Nematodirus spp., and Oesophagostomum spp. All the major groups of anthelmintics have been reported for development of variable degrees of resistance in different species of gastrointestinal nematodes. This paper describes the global scenario of prevalence and methods used for detection of AR in small ruminants. Different mechanisms and contributory factors for the development of AR are discussed. Various options and alternate strategies for the control and/or delay in the onset of AR are suggested in the light of available information.

Although the epicentres of probiotic research in the past decade have been Japan and Europe, researchers in the Asia-Pacific region have actively contributed to the growing understanding of the intestinal microbial ecosystem, and interactions between gut bacteria, diet and health of the human host. A number of new probiotic strains have been developed in the region that have been demonstrated to have beneficial impacts on health in animal and human trials, including improved protection against intestinal pathogens and modulation of the immune system. Probiotics targeted to animals, including aquaculture, feature heavily in many Asian countries. Developments in probiotic technologies have included microencapsulation techniques, antimicrobial production in fermented meats, and synbiotic combinations. In particular, the impact of resistant starch on the intestinal environment and fermentation by intestinal bacteria has been intensively studied and new probiotic strains selected specifically for synbiotic combinations with resistant starch. This paper provides an overview of probiotic research within Australia, New Zealand and a number of Asian countries, and lists scientists in the Asia-Pacific region involved in various aspects of probiotic research and development.

Background: It is widely acknowledged that there is a global divide on health care and health research known as the 10/90 divide., Methods: A retrospective survey of articles published in the BMJ, Lancet, NEJM, Annals of Internal Medicine & JAMA in a calendar year to examine the contribution of the developing world to medical literature. We categorized countries into four regions: UK, USA, Other Euro-American countries (OEAC) and (RoW). OEAC were European countries other than the UK but including Australia, New Zealand and Canada. RoW comprised all other countries., Results: The average contribution of the RoW to the research literature in the five journals was 6.5%. In the two British journals 7.6% of the articles were from the RoW; in the three American journals 4.8% of articles were from RoW. The highest proportion of papers from the RoW was in the Lancet (12%). An analysis of the authorship of 151 articles from RoW showed that 104 (68.9%) involved authorship with developed countries in Europe or North America. There were 15 original papers in these journals with data from RoW but without any authors from RoW., Conclusions: There is a marked under-representation of countries in high-impact general medical journals. The ethical implications of this inequity and ways of reducing it are discussed.

In this paper we critique two recent studies that have been claimed to disprove the Out of Africa hypothesis for modern human origins (Hawks et al., 2000; Wolpoff et al., 2001). We show that the test prediction employed by Hawks et al. (2000) and Wolpff et al. (2001) is not relevant to many versions of the Out of Africa hypothesis, and that the key specimens they used are problematic in terms of morphological representativeness. We also show that there are significant problems with the character state datasets employed in the studies. Lastly, we highlight evidence that the main method used in the studies (pairwise difference analysis) is not reliable when applied to the type of data employed by Hawks et al. (2000) and Wolpoff et al. (2001). In view of the foregoing, we contend that Hawks et al.'s (2000) and Wolpoff et al.'s (2001) claim to have disproved the Out of Africa hypothesis cannot be sustained.

Approximately one-fourth of all cigarettes sold in the United States are mentholated. An understanding of the consequences, patterns, and correlates of menthol cigarette use can guide the development and implementation of strategies to reduce smoking prevalence and smoking-attributable morbidity and mortality. This paper summarizes the literature on the health effects of mentholated cigarettes and describes various patterns of use as indicated by consumption and survey data from the United States and other nations. The epidemiological literature on menthol cigarettes and cancer risk is inconclusive regarding whether these cigarettes confer a risk for cancer above that of nonmentholated varieties. Available data indicate that mentholated cigarettes are at least as dangerous as their nonmentholated counterparts. In addition, because mentholation improves the taste of cigarettes for a substantial segment of the smoking population and appears to mask disease symptoms, this additive may facilitate initiation or inhibit quitting. Menthol market share is high in the Philippines (60%), Cameroon (35%-40%), Hong Kong (26%), the United States (26%), and Singapore (22%). Newport has become the leading menthol brand in the United States. Surveys from four nations indicate that menthol use among adult smokers is more common among females than males. Among U.S. smokers, 68.9% of Blacks, 29.2% of Hispanics, and 22.4% of Whites reported smoking a mentholated variety. Research is needed to better explain factors that may influence menthol preference, such as marketing, risk perceptions, brand formulation, and taste preferences. Such research would guide the development of potentially more effective programs and policies.

Culture determines the different ways that patients understand cancer, the ways they explain it, and their attitudes towards it. These factors affect the patient's emotional response to the disease and health behaviour in terms of prevention and treatment. In this paper we review the explanatory models for cancer in several cultural groups. These models range from metaphysical to interpersonal and physical. Cultural explanations of health and health behaviour could detract from social and economic factors that might be more important as determinants of these behaviours.

Worldwide, the human immunodeficiency virus exhibits a great genetic variability, with multiple circulating subtypes of the virus. This variability allows study of the movement of HIV strains within and between human populations but also has implications for diagnosis, treatment and monitoring. The type of HIV causing the epidemic in Australia is changing from being homogeneous subtype B, reflecting a greater regional diversity. In this paper the classification of HIV-1 subtypes and their distribution within the Australasian region are reviewed and the implications of these distribution patterns discussed.

Objective: To analyse the prevalence studies of dementia from 1994 to 2000., Methods: The papers were searched on Medline and Lilacs and the inclusion criteria were: to be an original study, that estimated dementia prevalence and investigated community samples., Results: thirty-eight studies were evaluated from all continents. The mean age-specific prevalence rates of dementia were 1,17% for the 65-69 years group, and 54,83% over 95 years. The results were strongly influenced by age. Dementia prevalence rates were higher on women on 75% of the papers reviewed. The Alzheimer's disease/vascular dementia ratio was higher in South America and lower in Asia., Discussion: Methodological advances were found, specially regarding the homogeneity of diagnostic criteria. The effect of age on the rates of dementia prevalence was pronounced, up to 90 years of age.

Aims: To examine the volume and potential impact of gastroenterology research outputs from 1985 to 1998 from 14 developed countries; the overlap with research in cancer, infectious diseases, and genetics; and the funding sources for this research. To determine if countries' research outputs correlated with their burden of corresponding diseases and inputs to their research., Methods: Selective retrieval of papers from the Science Citation Index and manual look up of a sample to determine funding sources. Classification of journals by four categories of research level (clinical/basic) and potential impact (low/high)., Results: Gastroenterology represents about 8% of world biomedical research but over 11% in Italy, Japan, and Spain. Its potential impact is highest (but declining) for the USA. It has increased noticeably in most European countries, particularly in Finland. Gastroenterology research has become more clinical in Japan, Spain, Australia, and the Netherlands but more basic in Canada, Germany, Finland, Israel, and South Africa. Funding comes primarily from national governments, followed by national private non-profit sources and industry but little industrial funding occurs in some countries. There is a strong and positive correlation between reported deaths from gastrointestinal neoplasms and countries' outputs of research in gastrointestinal oncology., Conclusions: Bibliometric analysis can reveal differences between countries in their research in a subject when a common methodology is applied to an international database. Variations in research methods in different countries can plausibly explain some of the variation in the potential impact of the work.

Objective: The aim of this paper is to examine the delivery of mental health care which will be enabled by the various communications technologies over the next 5 to 10 years., Method: A literature review, reviews of multiple Internet websites and the author's personal experience and opinions are combined to provide a commentary on the group of new technologies for communication available within health, and how they will affect the practice of psychiatry and psychiatrists over the next decade. The driving forces for these changes are the rise of consumerism, technological change and financial necessity, and it is evident that patients will have in future much greater choice of access to their therapists than at present. The health-care environment of the future will be different to today, as will the roles of psychiatrists, who will increasingly have to work as members of teams in an expert capacity rather than in traditional one-to-one practice., Conclusions: It is concluded that it is essential for psychiatrists to become involved in online health care, and in particular to join their patients on the Internet, and that there are major opportunities for Australasian psychiatrists to provide high quality psychiatric care across national boundaries, particularly into the Asia-Pacific region.

Objective: This paper summarises a report to the NHMRC the objectives of which were to review research into the epidemiology of youth suicide in Australia and identify gaps in research., Method: Literature searches were conducted. A limited amount of new data analysis was included to shed light on reliability issues of official Australian suicide data., Results: The review examined suicide data systems, including issues to do with coroners, the Australian Bureau of Statistics and alternative systems. The epidemiological areas reviewed included: all ages, youth, age and gender, geographical, socioeconomic, marital, indigenous, migrants, suicides in custody and gay and lesbian suicides., Conclusion: While much is known about the epidemiology of youth suicide, much remains to be clarified. Study of indigenous issues is perhaps the most neglected area; study of family issues may be potentially be the most productive.

This paper discusses the dispersal of facilities where family members migrate to different destination countries. Terminology for internationally dispersed families is proposed, and the term branch migration is suggested for the migration of related people from the same source country to different destination countries. Data from two 1993 surveys of senior secondary students show that 22% of Sydney students and 20% of Hong Kong students have relatives in two or more other countries. The data suggest that many Asian migrant families have branched between the US, Canada, Australia and other migrant-receiving nations. The causes and implications of the international dispersal of families are discussed.

The paper covers the establishment of APACPH in 1984 and its subsequent development and achievements. The paper outlines the mission and objectives of the Consortium and brief comparisons are drawn with similar organizations in the European and North American regions. Significant achievements of the Consortium and its contribution to the public health debate are presented. The paper then explores strategies for the future in meeting the challenges of emerging public health issues through collaborative efforts in education, training, research and leadership development in public health in the first century of a new millennium.

Background: There have been many recent changes to the immigration and educational requirements necessary to obtain both clinical and research positions in the USA. This paper describes these requirements in detail and examines the cost of obtaining a position., Methods: An extensive review of Medline and relevant documents from educational and immigration bodies is undertaken in addition to personal and peer experience of the processes necessary to fulfil educational and immigration requirements for funded medical positions in the USA. A cost analysis is performed based upon the latest charges for exam requirements and visa fees. A 'minimum' cost for fulfilling exam and visa requirements only and a 'likely' cost (which represents a more realistic appraisal of the expenses likely to be incurred) are investigated., Results: This review shows that many recent changes have made the attainment of a position difficult and expensive. Much foresight and planning is required to obtain the necessary requirements for medical licensure and to obtain working visa status in addition to the application for the position itself. The cost analysis reveals that the minimum cost is $4050 (AU$) and the 'likely' cost is $8600 (AU$)., Conclusion: Australasian trainees and Fellows need to be fully aware of the difficulties and expenses likely to be encountered in order to make an informed decision with regard to overseas training in the USA.

Considerable progress has been made in the knowledge of the epidemiology of gestational trophoblastic disease (GTD) in the last few years. There are two main and widely known points related to this disease: its geographical distribution and the different frequency in the various classes of age. GTD is more frequent in South-East Asia, India and Africa, and is rare in European and North American populations. For example, in the United States, the frequency of GTD was 108 per 100,000 pregnancies in the 1970's. In Europe, particularly in Italy, frequencies are lower. In northern Italy, the frequency of hydatidiform mole, in the period 1979-1982, was equal to 62 per 100,000 pregnancies, but in Indonesia and in China, the reported rates were 993 and 667 per 100,000 pregnancies respectively. GTD disease is more frequent in the extreme classes of age (under 20 and over 40 years) and the risk may be more than 100 times greater over 50 years. Besides these risk factors, the possible role of both genetic (familiarity, blood groups) and environmental factors (diet, cigarette smoking, etc.) has been investigated on the onset of GTD. This paper reviews the epidemiologic knowledge on GTD.

The explosion of knowledge coupled with a more diversified society have led to a new set of demands on dentists. For more than 20 years, dental educators and the profession have been concerned about the pressure on the university to ensure that every new graduate is fully competent to enter a dental practice independently and successfully. This paper reviews some of the innovations which are being employed to attempt to deal with these concerns. There is no one formula that can answer these concerns. Postgraduate training in general dentistry is becoming more widespread and may need further investigation. The main focus of this paper is on trends in formal advanced postgraduate training in general dentistry.

Morphological variations of the deciduous dentition are as useful as those of the permanent dentition for determining the biological affinities of human populations. This paper provides material on morphological variations of deciduous teeth of the prehistoric Japanese population from the Late and the Latest Jomon Period (ca. 2000-ca. 300 B.C.). The expression of nonmetric traits of the deciduous teeth in the Jomon sample shows a closer affinity with modern Japanese and Native American samples than with American White, Asiatic Indian, and African samples. However, the frequency of shoveling in deciduous upper incisors in the Jomon sample is lower than those in modern Japanese and Native American samples. The Jomon sample also expresses a much higher frequency of cusp 6 in deciduous lower second molars than seen in modern Japanese, Ainu, and Native American samples. The frequency in the Jomon sample is equal to that in the Australian Aboriginal sample, which shows cusp 6 most frequently among the samples compared. A somewhat low incidence of incisor shoveling in the Jomon sample was also reported in the permanent dentition (Turner [1976] Science 193:911-913, [1979] Am. J. Phys. Anthropol. 51:619-635, [1987] Am. J. Phys. Anthropol. 73:305-321, [1990] Am. J. Phys. Anthropol. 82:295-317; T. Hanihara [1992] Am. J. Phys. Anthropol. 88:163-182, 88:183-196). However, the frequency of cusp 6 in the Jomon sample shows no significant difference from those of Northeast Asian or Native American samples in the permanent dentition (Turner [1987] Am. J. Phys. Anthropol. 73:305-321; T. Hanihara [1992] Am. J. Phys. Anthropol. 88:163-182, 88:183-196). Evidently, some nonmetric traits express an inter-group difference only in the deciduous dentition.

This paper reports the findings of a worldwide survey of the teaching of glass ionomer (polyalkenoate) cements (GICs). A total of 324 questionnaires was returned from the 650 dental schools included in the study (50 per cent response), with the response ranging from 100 per cent (Australasia) to 4 per cent (USSR). The findings indicate that most primary dental degree students in the majority of countries investigated are presently taught the use of GICs in a wide range of applications using techniques which largely conform with those recommended by the manufacturers of such materials.

In these days of rapid transit from continent to continent, and the increasing mobility of people, agents of disease are no longer geographically restricted. Disease contracted half way across the world may become manifest in a country in which the pathogen is not normally found. Thus knowledge of the geographical distribution of pathogens becomes increasingly important when a diagnosis is being made. This is as true of ringworm fungi as of any other group of microorganisms. In the last 12 years, in the Mycological Reference Laboratory, an increasing number of exotic dermatophytes have been seen, related in part at least to the great increase in the number of non-British residents. Not all species of dermatophytes are cosmopolitan in their distribution throughout the world. While some have been recorded from every continent, others have geographically limited areas of greater or lesser extent. Surveys taken at intervals in a country may show a rise and fall in occurrence of several species as habits change, populations move and medical facilities became increasingly well-distributed. There have been few geographical surveys of ringworm fungi that have covered the world. Ajello (1960, 1974) has reviewed the individual species with regard to geographical location, while Vanbreuseghem & de Vroey (1970) attempted to estimate the relative importance of the various species in terms of numbers of isolations reported. This paper therefore reviews the world dermatophyte flora in terms of the dominant agents in the various countries, and some of the changes that have been recorded.

This paper attempts to give an overview of current levels of child mortality prevailing in the world. It also examines trends and socioeconomic differentials in child mortality for selected countries and regions of the world. Lastly it reviews data on causes of child death and related environmental factors. The paper concludes that despite the fact that child deaths are frequently avoidable, mortality differentials between the developed and developing regions of the world are more pronounced in childhood (ages one to under five years) than at any other time of life. While some developing countries have substantially reduced the level of mortality in childhood, in others it remains very high. In contrast, in most developed countries child death rates are now so low, that they no longer serve as useful measures of public health.

As difined in the first part of these papers, the distribution of MS does not really correlate well with broad geoclimatic factors. Specifically there is little evidence for a direct correlation with latitude, and such correlations, to be meaningful, need to consider longitude as well. At latitude 40 degrees north, for example, MS is high frequency in America, medium in Europe, and low in Asia. Accordingly the world-wide distribution of MS is best regarded as comprising three bands or zones of high medium, and low risk or frequency as delineated here. All the high and medium regions are at present found in Europe or in areas colonized by Europeans. It seems likely then that MS originated in western Europe and has spread from there to the United States and Canada as well as to New Zealand and Australia. The disorder may well have "taken" also in South Africa and Hawaii, but at considerably lower levels. Further attention to the character and timing of the depresion of this disease may well provide clues vital to determining its cause.

This paper presents two maps summarising all available published data on the world distribution of gallstones as recorded from autopsy studies. It is suggested that further autopsy studies are required, especially in Asia, Russia, the Middle Eastern and Mediterranean countries and South America. Such studies could be readily carried out and would contribute to our understanding of the aetiology of gallstones.

Whilst before World War II migration to Australia was almost entirely from the British Isles, over 60% of post-War II migrants came from other countries other than Britain. They arrived in Australia in distinct waves: Eastern European refugees were followed by migrants from Western Europe, who were later replaced by migrants from the Mediterranean Basin and finally by Asians, initially from the Indian subcontinent, afterwards from Lebanon and most recently from Indo-China. These patterns of migration were changing the numerical size, age distribution and length of residence in Australia of the different ethnic groups. The paper describes the influence of these changes on physical and mental health of the migrants. Physical morbidity was related to 'imported diseases', to the different physical and social environment and to differences and changes in eating and other habits. Psychiatric disorders were associated with traumatic experiences encountered prior to migration, and with stresses of migration and adjustment to the new country. Thus, in evaluating the health of immigrants, one has to take into account the sources from which the migrants were drawn, their situation in the host society, their numerical growth and age distribution, as well as acculturation to the country settlement.
