Abstract Objective To investigate stage-specific survival from diagnosis, stage-specific disease recurrence, and post-recurrence survival in patients diagnosed with localized and regionally advanced cutaneous melanoma. Methods A retrospective, observational cohort study was conducted in six Dutch hospitals. We included patients with a first diagnosis of stage I, II, or III melanoma between January 2003 and December 2011. Descriptive statistics were used to summarize time to first recurrence and type of first recurrence. Overall survival (OS) from diagnosis and post-recurrence OS were assessed using the Kaplan-Meier method. Results A total of 3,093 patients had a first diagnosis of stage I (n = 2,299), II (n = 565), or III (n = 229) melanoma. Median OS was not yet reached for patients with stage I, 9.5 years for patients with stage II, and 6.8 years for patients with stage III. Fifty-seven patients (8%) with stage IB, 137 patients (29%) with stage II, and 81 patients (47%) with stage III developed disease recurrence. Median time to first recurrence was 2.8, 1.5, and 1.0 years for patients with stage IB, II, and III, respectively. Most patients (79%) developed regional lymph node or distant metastases as first recurrence. Median post-recurrence OS was 2.8, 3.9, and 0.5 years for patients with intralymphatic, regional lymph node, and distant metastases, respectively. Conclusion A substantial number of patients developed disease recurrence. Of these patients, a considerably high proportion developed distant metastases which had a great impact on survival. This paper describes the working principle and characteristics of a new type of valve with a unique self-actuating principle that actuates based on observed temperature difference, which is intended for use on solar thermal storage tank inlets, where it compares the temperature of the flow coming into the valve with the temperature inside the storage tank, and switches the flow based on the observed temperature difference, in order to achieve maximum thermal stratification inside the storage tank. The novel working principle, based on mass-transfer inside the actuator driven by vapour pressure differences, is explained in detail, and results of tests with the valve are presented, which show that the valve switches very effectively, and is a very useful tool for storage tank stratification. Thermal analysis of Olive and Sunflower Oil is done by Fast DSC to evaluate its potential to replace DSC for adulteration detection. DSC measurements take hours, Fast DSC minutes. Peak temperatures of the crystallisation peak in cooling for different Olive and Sunflower Oils are both comparable to DSC, but not always distinguishable. The heating curves of Olive and Sunflower Oil show more differences. Compared to DSC, Fast DSC shows lower peak temperatures for heating curve peaks. [ABSTRACT FROM AUTHOR]

Abstract: A geomechanical assessment of the Naylor Field, Otway Basin, Australia has been undertaken to investigate the possible geomechanical effects of CO2 injection and storage. The study aims to evaluate the geomechanical behaviour of the caprock/reservoir system and to estimate the risk of fault reactivation. The stress regime in the onshore Victorian Otway Basin is inferred to be strike–slip if the maximum horizontal stress is calculated using frictional limits and DITF (drilling induced tensile fracture) occurrence, or normal if maximum horizontal stress is based on analysis of dipole sonic log data. The NW–SE maximum horizontal stress orientation (142°N) determined from a resistivity image log is broadly consistent with previous estimates and confirms a NW–SE maximum horizontal stress orientation for the Otway Basin. An analytical geomechanical solution is used to describe stress changes in the subsurface of the Naylor Field. The computed reservoir stress path for the Naylor Field is then incorporated into fault reactivation analysis to estimate the minimum pore pressure increase required to cause fault reactivation (ΔP p). The highest reactivation propensity (for critically-oriented faults) ranges from an estimated pore pressure increase (ΔP p) of 1MPa to 15.7MPa (estimated pore pressure of 18.5–33.2MPa) depending on assumptions made about maximum horizontal stress magnitude, fault strength, reservoir stress path and Biot''s coefficient. The critical pore pressure changes for known faults at Naylor Field range from an estimated pore pressure increase (ΔP p) of 2MPa to 17MPa (estimated pore pressure of 19.5–34.5MPa). Geological storage of CO2 in the offshore Gippsland Basin, Australia, is being investigated by the Cooperative Research Centre for Greenhouse Gas Technologies (CO2CRC) as a possible method for storing the very large volumes of CO2 emissions from the nearby Latrobe Valley area. A storage capacity of about 50 million tonnes of CO2 per annum for a 40-year injection period is required, which will necessitate several individual storage sites to be used both sequentially and simultaneously, but timed such that existing hydrocarbon assets will not be compromised. Detailed characterisation focussed on the Kingfish Field area as the first site to be potentially used, in the anticipation that this oil field will be depleted within the period 2015–2025. The potential injection targets are the interbedded sandstones of the Paleocene-Eocene upper Latrobe Group, regionally sealed by the Lakes Entrance Formation. The research identified several features to the offshore Gippsland Basin that make it particularly favourable for CO2 storage. These include: a complex stratigraphic architecture that provides baffles which slow vertical migration and increase residual gas trapping and dissolution; non-reactive reservoir units that have high injectivity; a thin, suitably reactive, lower permeability marginal reservoir just below the regional seal providing mineral trapping; several depleted oil fields that provide storage capacity coupled with a transient production-induced flow regime that enhances containment; and long migration pathways beneath a competent regional seal. This study has shown that the Gippsland Basin has sufficient capacity to store very large volumes of CO2. It may provide a solution to the problem of substantially reducing greenhouse gas emissions from future coal developments in the Latrobe Valley. [ABSTRACT FROM AUTHOR]

Abstract: Aim: A survival benefit has been observed for colorectal cancer patients with peritoneal carcinomatosis treated by cytoreductive surgery with intraoperative hyperthermic intraperitoneal chemotherapy (HIPEC). However, this treatment modality is associated with a considerable morbidity and mortality and in a significant number of patients survival is not improved. We studied whether poor survivors could be identified on preoperative computed tomography (CT), in order to avoid unnecessary surgery. Patients and methods: Films of abdominopelvic CT scans from 25 such patients treated by cytoreductive surgery and HIPEC were retrospectively analysed by two radiologists separately. A simplified peritoneal cancer index (SPCI) was used to determine the extent of peritoneal involvement. Correlation between the on preoperative CT based SPCI-scores as well as number of involved abdominopelvic areas (N) and survival was examined with the log-rank test. The relation between each affected region and survival was evaluated with Cox regression analysis. Results: The preoperative SPCI- and N-scores of one of the radiologists had no statistically significant prognostic value, while for the second radiologist SPCI≥7 and N≥4 were associated with particularly poor outcome. Additionally, the presence of ileocaecal region involvement and, depending on the radiologist, the occurrence of tumour deposits in the left subdiaphragmatic area on CT appeared to be unfavourable prognostic signs. Conclusions: The prognostic value of preoperative conventional CT appeared to be radiologist dependent and may, therefore, be of limited value in selecting colorectal cancer patients with peritoneal carcinomatosis who will not benefit from extensive cytoreductive surgery followed by HIPEC. Abstract: Aims: To report the results of standard therapy for peritoneal carcinomatosis of colorectal origin, which consists of conventional surgery and systemic chemotherapy. Method: In a prospective study 50 patients with proven peritoneal carcinomatosis of colorectal origin were treated with conventional surgery combined with 5-fluorouracil and leucovorin, or irinotecan in patients treated by 5-fluorouracil within 12 months prior to entry. Survival and progression-free survival were studied and prognostic factors were analysed. Results: The median survival time was 12.6 months. The median time to progression was 7.6 months. Location of primary tumour and result of conventional surgery and systemic chemotherapy were prognostic factors related to survival. Conclusion: The survival time of patients with peritoneal carcinomatosis of colorectal origin seems to be increased in patients treated by conventional surgery and systemic chemotherapy when compared to minimal treatment. Abstract Overpressure in ‘old’ sedimentary basins that have not undergone rapid, recent sedimentation cannot be easily explained using traditional burial-driven mechanisms. The last significant burial event in the Cooper Basin, Australia, was the Late Cretaceous deposition of the Winton Formation (98.5–90 Ma). Maximum temperature in the basin was attained during the Late Cretaceous, with cooling beginning prior to 75 Ma. Hence, overpressure related to rapid burial or palaeomaximum temperatures (e.g. hydrocarbon generation) must have developed prior to 75 Ma. Retaining overpressure for 75 Ma in ‘old’ basins such as the Cooper Basin requires extremely low seal permeabilities. An alternative explanation is that overpressure in the Cooper Basin has been generated because of an increase in mean stress associated with an increase in horizontal compressive stress since Late Cretaceous times. Structural observations and contemporary stress data indicate that there has been an increase in mean stress of approximately 50 MPa between Late Cretaceous times to that presently measured at 3780 m. The largest measured overpressure in the Cooper Basin is 14.5 MPa at 3780 m in the Kirby 1 well. Hence, disequilibrium compaction driven by increasing mean stress can explain the magnitude of the observed overpressure in the Cooper Basin. Increases in mean stress (tectonic loading) may be a feasible mechanism for overpressure generation in other ‘old’ basins that have undergone a recent increase in horizontal stress (e.g. Anadarko Basin).
