The integration of Artificial Intelligence (AI) into healthcare has the potential to revolutionize medical diagnostics, particularly in specialized fields such as Ear, Nose, and Throat (ENT) medicine. However, the successful adoption of AI-assisted diagnostic tools in ENT practice depends on the understanding of various factors; these include influences on their effectiveness and acceptance among healthcare professionals. This cross-sectional study aimed to assess the usability and integration of AI tools in ENT practice, determine the clinical impact and accuracy of AI-assisted diagnostics in ENT, measure the trust and confidence of ENT professionals in AI tools, gauge the overall satisfaction and outlook on the future of AI in ENT diagnostics, and identify challenges, limitations, and areas for improvement in AI-assisted ENT diagnostics. A structured online questionnaire was distributed to 600 certified ENT professionals with at least one year of experience in the field. The questionnaire assessed participants' familiarity with AI tools, usability, clinical impact, trust, satisfaction, and identified challenges. A total of 458 respondents completed the questionnaire, resulting in a response rate of 91.7%. The majority of respondents reported familiarity with AI tools (60.7%) and perceived them as generally usable and clinically impactful. However, challenges such as integration with existing systems, user-friendliness, accuracy, and cost were identified. Trust and satisfaction levels varied among participants, with concerns regarding data privacy and support. Geographic and practice setting differences influenced perceptions and experiences. The study highlights the diverse perceptions and experiences of ENT professionals regarding AI-assisted diagnostics. While there is general enthusiasm for these tools, challenges related to integration, usability, trust, and cost need to be addressed for their widespread adoption. Methods: In this retrospective cross-sectional study, lateral cephalometric radiographs of adult patients referred for orthodontic consultation were collected. Cephalometric tracing was performed with dedicated software. For each measure, descriptive statistics were calculated. Cephalometric measurements between the different skeletal classes were compared. Linear regressions were performed between upper airway diameters and cephalometric measurements, sex and age. Significance was predetermined for p < 0.05. Results: Lateral cephalometric radiographs of 120 patients were reviewed. Nasopharynx length (NL) and depth (PD) measurements were significantly shorter in skeletal class III patients (p < 0.05). The superior pharyngeal airway space (SPAS) was found to be significantly shorter in class III patients as compared to class II patients (p < 0.05), and the mean airway space (MAS) of class I patients was found to be significantly shorter compared to class II patients (p < 0.05). Palate length (PL) values were found to be significantly longer in class I (p < 0.05). Linear regressions showed that the sella-nasion-A point angle (SNA) and Riedel's angle between point A, the nasion and point B (ANB) significantly influenced NL and PD (p < 0.05). Conclusions: Class III patients show significantly shorter nasopharynx measurements; clinicians should consider that this sagittal discrepancy could be related to an altered anatomy of the upper respiratory tract. Objective: Noise-induced hearing loss is the most prevalent occupational disease worldwide and is historically associated with the Industrial Revolution. This study analyses early descriptions of this disorder during the pre-industrial period in the work of the Italian physician Bernardino Ramazzini (1633–1714). Method: Primary and secondary historical literature were reviewed. Results: Ramazzini described hearing loss in corn millers and in coppersmiths and recognised that this disorder is irreversible and progressive when exposure to noise continues. He also seemed to describe tinnitus. He further suggested the use of earplugs as a preventive measure for these classes of workers. Ramazzini's anatomical and pathological knowledge appears to be based on ancient authors; he did not discuss contemporaneous medical authors' work on hearing function. Conclusion: Despite some limitations, Ramazzini's work appears pioneering for his time and represents an important milestone in the history of otolaryngology. [ABSTRACT FROM AUTHOR]

To identify differences in inpatient otolaryngology consultations and interventions for patients based on COVID-19. Records were reviewed for all patients for whom otolaryngology was consulted at a high-volume tertiary care hospital from April 30, 2020 to October 1, 2020. Demographic information, length of stay, COVID-19 status, indication for consultation, and otolaryngology interventions were recorded. Statistical analysis was performed using R software. Bleeding composed a significantly higher proportion of otolaryngology consults in COVID-19 positive patients (28% vs. 8.4%, p<0.0001). Management of bleeding was the most common procedure performed in positive patients (n=37, 35%), and they had a higher median number of interventions performed when compared to bleeding patients who tested negative (1, IQR 1-2 vs. 1, IQR 0-1, p=0.04). COVID-19 positive patients with bleeding were more likely to expire than those with other indications for otolaryngology consultation (50% vs. 7%, p<0.001). Bleeding and associated interventions comprised the predominant discrepancy between COVID-19 positive and negative patients in our cohort. Background: International consensus statements now subdivide chronic rhinosinusitis (CRS) into several phenotypes and endotypes, including the presence of polyps (CRSwNP) and eosinophilia (eCRSwNP). Biological treatments aimed at blocking eosinophilic inflammation in CRSwNP via interleukin 5 (IL5) or the interleukin 5 receptor (IL5R) have demonstrated limited efficacy thus far. Objective: To review the pathophysiology of eCRSwNP, the evidence for mepolizumab (anti-IL5) and benralizumab (anti-IL5R) in CRSwNP, and to highlight areas for future research and therapeutic intervention. Methods: Primary and secondary literature search. Results: Clinical trials on mepolizumab and benralizumab in CRSwNP are limited and restricted by trial design which prevents direct comparison with other interventions, including surgery. Both agents would appear to provide some benefit in reducing nasal polyp size but limited clinical patient benefit. Molecular biological research highlights that eCRSwNP can occur in the absence of IL5 and that other cells/cytokines play an important part in the disease's pathophysiology. Conclusion: Blockade of IL5/IL5R alone would appear to provide limited "real life" clinical benefit in patients with CRSwNP due to the complexities of the pathophysiology of the condition. Therapy aimed at several simultaneous cytokine targets has logic but well-designed trials are unlikely to be forthcoming in the short term due to the financial cost and commercial conflicts of interest. Objectives: The aim of this study was to analyse, within a French cohort of workers previously occupationally exposed to asbestos, incidence and mortality from various sites of head and neck cancers (larynx excluded) and to examine the potential link of these cancers with pleural plaques.Methods: A 10-year follow-up study was conducted in the 13 481 male subjects included in the cohort between October 2003 and December 2005. Asbestos exposure was assessed by industrial hygienist analysis of a standardised questionnaire. The final cumulative exposure index (CEI; in equivalent fibres.years/mL) for each subject was calculated as the sum of each employment period's four-level CEI. The number of head and neck cancers recorded by the National Health Insurance fund was collected in order to conduct an incidence study. Complementary analysis was restricted to men who had performed at least one chest CT scan (N=4804). A mortality study was also conducted. We used a Cox model with age as the time axis variable adjusted for smoking, time since first exposure, CEI of exposure to asbestos and pleural plaques on CT scans.Results: We reported a significant dose-response relationship between CEI of exposure to asbestos and head and neck cancers after exclusion of laryngeal cancers, in the mortality study (HR 1.03, 95% CI (1.01 to 1.06) for an increase of 10 f.years/mL) and a close to significant dose-response relationship in the incidence study (HR 1.02, 95% CI (1.00 to 1.04) for an increase of 10 f.years/mL). No statistically significant association between pleural plaques and head and neck cancer incidence was observed.Conclusions: This large-scale study suggests a relationship between asbestos exposure and head and neck cancers, after exclusion of laryngeal cancers, regardless of whether associated pleural plaques were present. Otolaryngology is an important specialty in the field of surgery that deals with the diagnosis and treatment of the ear, nose, throat, trachea, as well as related anatomical structures. Various otolaryngological disorders are difficult to treat using established pharmacological and surgical approaches. The advent of molecular and cellular therapies led to further progress in this respect. This article reviews the therapeutic strategies of using stem cells, immune cells, and chondrocytes in otorhinolaryngology. As the most widely recognized cell derivatives, exosomes were also systematically reviewed for their therapeutic potential in head and neck cancer, otitis media, and allergic rhinitis. Finally, we summarize the limitations of stem cells, chondrocytes, and exosomes, as well as possible solutions, and provide an outlook on the future direction of cell- and derivative-based therapies in otorhinolaryngology, to offer a theoretical foundation for the clinical translation of this therapeutic modality. Common ENT diseases include hearing loss, sinusitis, bronchial defects, head and neck cancer, etc. Cells and their derivatives used in the study and treatment of these diseases mainly include stem cells, chondrocytes, and exosomes, with systemic and local drug administration, in vitro tissue culture, and genetic engineering being the common interventions. The main logic of the research consists of two parts: one is to use the differentiation ability of stem cells to culture them into target organs or target tissues, so as to replace their functions; the other is to use the transport properties of exosomes to deliver drugs or use them as biomarkers to predict disease progression. Abstract Introduction Otitis media is an umbrella term for middle ear inflammation; ranging from acute infection to chronic mucosal disease. It is a leading cause of antimicrobial therapy prescriptions and surgery in children. Despite this, treatments have changed little in over 50 years. Research has been limited by the lack of physiological models of middle ear epithelium. Methods We develop a novel human middle ear epithelial culture using an air‐liquid interface (ALI) system; akin to the healthy ventilated middle ear in vivo. We validate this using immunohistochemistry, immunofluorescence, scanning and transmission electron microscopy, and membrane conductance studies. We also utilize this model to perform a pilot challenge of middle ear epithelial cells with SARS‐CoV‐2. Results We demonstrate that human middle ear epithelial cells cultured at an ALI undergo mucociliary differentiation to produce diverse epithelial subtypes including basal (p63+), goblet (MUC5AC+, MUC5B+), and ciliated (FOXJ1+) cells. Mature ciliagenesis is visualized and tight junction formation is shown with electron microscopy, and confirmed by membrane conductance. Together, these demonstrate this model reflects the complex epithelial cell types which exist in vivo. Following SARS‐CoV‐2 challenge, human middle ear epithelium shows positive viral uptake, as measured by polymerase chain reaction and immunohistochemistry. Conclusion We describe a novel physiological system to study the human middle ear. This can be utilized for translational research into middle ear diseases. We also demonstrate, for the first time under controlled conditions, that human middle ear epithelium is susceptible to SARS‐CoV‐2 infection, which has important clinical implications for safe otological surgery. Gradenigo's syndrome (GS) is a triad (otorrhea, abducens nerve palsy, and pain in the trigeminal nerve distribution) of clinical findings that are caused by contiguous spread of petrous apicitis to the nearby neurovascular structures. Petrous apicitis is usually secondary to otitis media but atypical etiologies and absence of the classical triad pose a diagnostic challenge for physicians. We report a rare case of GS in an afebrile 55-year-old male who presented with unilateral headache, dysphagia and hoarseness (IX and X cranial nerve involvement), and diplopia with lateral gaze palsy (VI nerve involvement) in the absence of trigeminal neuralgia or a history of otitis media. Magnetic Resonance Imaging (MRI) revealed hyperintense lesions in the right petrous apex indicating petrous apicitis, the hallmark of GS. Prompt initiation of broad-spectrum antibiotics led to a marked improvement in dysphagia and voice quality on the 4th post-admission day, and complete resolution of symptoms by the end of the fourth week. This shows that GS can present even in the absence of clinically apparent ear infection and cranial nerve palsies may not be limited to the V and VI nerve in all cases. Physicians should be aware of such atypical manifestations as prompt radiological assessment followed by early antibiotics can prevent life-threatening complications from developing.

Objective: For centuries, the tympanum has remained the only visible structure of the organ of hearing. This study aimed to trace the understanding of the tympanic membrane from antiquity to the early twentieth century. Methods: A review was conducted of primary and secondary historical and scientific literature describing the tympanic membrane anatomy. Results: Although ancient polymaths sensed that sounds were vibrations that could spread in the air and be perceived by the hearing organ, there were numerous misconceptions about the tympanum until human dissections performed during the Renaissance. The tympanum was correctly described only centuries later when technological advances enabled otologists to understand it as a fundamental part of the hearing organ. Conclusion: The tympanic membrane history reflects key stages in medical knowledge; limited for centuries, a great technological leap was possible in the nineteenth century, contributing to the emergence of otologists and laying the foundations of modern otology. Background: Whole‐body hybrid positron emission tomography (PET) imaging is increasingly used for sinonasal tumors. However, only empirical data exist on the additional, clinically relevant information derived from these techniques. Methods: This study included 96 regionalized magnetic resonance imaging (MRI) of the sinonasal tract/neck and separate hybrid FDG‐PET/CT or FDG‐PET/MRI in 74 patients. Additional radiological information (ARI) obtained from each hybrid examination was analyzed and its clinically relevance was determined. Clinically relevant information (CRI) was categorized with regard to primary tumor site, regional lymph node metastases, distant metastases, second primary tumors, and non‐neoplastic findings. Results: A total of 45/96 (46.9%) hybrid PET examinations revealed ARI. CRI was found in 32/96 (33.3%) examinations and concerned the primary tumor site (6.1%), regional lymph node metastases (4.1%), distant metastases (14.3%), second primary tumors (7.3%), and non‐neoplastic findings (5.1%). Conclusions: Hybrid PET imaging yields additional radiological information translating into clinically relevant information in a substantial proportion of patients with sinonasal tumors. Objectives: Sarcoidosis is a multisystemic inflammatory disease with extrathoracic manifestations, most commonly affecting the young and middle‐aged, female and Black populations. Diagnosis usually requires evidence of non‐caseating granulomata and, when treated, prognosis is usually favourable. We aim to establish the incidence, clinical features and optimal treatment of ENT manifestations of this disease. Design: We performed a PubMed literature review to determine the evidence base supporting this. Results: ENT manifestations are present in 5%‐15% of patients with sarcoidosis, often as a presenting feature, and require vigilance for swift recognition and coordinated additional treatment specific to the organ. Laryngeal sarcoidosis presents with difficulty in breathing, dysphonia and cough, and may be treated by speech and language therapy (SLT) or intralesional injection, dilatation or tissue reduction. Nasal disease presents with crusting, rhinitis, nasal obstruction and anosmia, usually without sinus involvement. It is treated by topical nasal or intralesional treatments but may also require endoscopic sinus surgery, laser treatment or even nasal reconstruction. Otological disease is uncommon but includes audiovestibular symptoms, both sensorineural and conductive hearing loss, and skin lesions. Conclusions: The consequences of ENT manifestations of sarcoidosis can be uncomfortable, disabling and even life‐threatening. Effective management strategies require good diagnostic skills and use of specific therapies combined with established treatments such as corticosteroids. Objectives/Hypothesis: Lymphoid neogenesis or the development of organised, de novo lymphoid structures has been described increasingly in chronically inflamed tissues. The presence of tertiary lymphoid organs (TLOs) has already been demonstrated to result in significant consequences for disease pathology, severity, prognosis and patient outcomes. Whilst the wider medical community has embraced TLOs as important markers of disease and potential therapeutic targets, the otolaryngology field has only begun turning to these entities in an academic capacity. This review aims to outline the role of tertiary lymphoid organs in disease and summarise key early findings in the ENT field. We also an overview of TLOs, their developmental process and clinicopathological implications. Study design: Literature review. Methods: A literature search for all relevant peer‐reviewed publications pertaining to TLOs and ENT diseases. Search was conducted using PubMed, Embase and CINAHL databases. Results: A total of 24 studies were identified relevant to the topic. The majority of TLO research in ENT fell into the areas of oral squamous cell carcinoma (SCC) and chronic rhinosinusitis (CRS). Conclusions: Early research into both oral SCC and CRS suggests that TLOs have significant roles within ear, nose and throat (ENT) diseases. At this point in time, however, TLOs remain somewhat a mystery amongst otolaryngologists. As information in this field increases, we may develop a better understanding of how lymphoid neogenesis can influence disease outcomes amongst our patients and, ultimately, how they can be utilised in an immunotherapeutic manner. Objective: This study aimed to assess the quality and readability of websites on chronic rhinosinusitis. Methods: A total of 180 results from 3 different search engines regarding 'chronic rhinosinusitis', 'sinusitis' and 'sinus infections' were analysed for readability using the Flesch–Kincaid Grade Level, Flesch Reading Ease Score and Gunning Fog Index. The Discern tool was used to approximate information quality. Results: From 180 total searches, 69 unique websites were identified. These had an average Flesch–Kincaid Grade Level of 9.75 (95 per cent confidence interval = 9.12–10.4), a Flesch Reading Ease Score of 45.0 (41.0–49.0) and a Gunning Fog Index of 13.7 (12.9–14.4), which equates to the average reading level of a college or university student. Discern scores were variable but consistently showed good-quality information. Conclusion: Chronic rhinosinusitis information is of a high quality but is for a reading level higher than that of the average adult. Objective: The COVID-19 health crisis abruptly disrupted the practice of otolaryngology. This article aims to define the changes needed to operate an academic otolaryngology practice safely and efficiently from within the epicenter of the pandemic. We define the areas of normal patient workflow that have been affected by COVID-19, and we offer mitigation strategies with attention paid to the specific needs of subspecialties. Data Sources: The article includes data specific to the office practice metrics of the Weill Cornell Medicine Department of Otolaryngology–Head and Neck Surgery, as well as publically available data from New York Presbyterian Hospital system and the New York Times. Review Methods: Expert opinion. Conclusions: Through careful planning and execution, it is possible to reestablish safe otolaryngologic patient care during the COVID-19 pandemic. It will require a significant change from prior practice models for successful implementation. Additionally, telemedicine can be positively integrated into the treatment of otolaryngology diseases for new and established patients. Implications for Practice: The information conveyed in this review can be used as a guide by large and small otolaryngology groups to identify aspects of the patient visit that are "at risk" due to COVID-19, and it suggests sensible responses that can be made without a significant disruption to normal practice. Background: A missed or non-diagnosed otitis media with effusion at very early age in life can impair the cognitive developmental milestone, especially hearing. Therefore, early diagnosis and treatment (medical, surgical or physiological) is pertinent. A search for an inexpensive, minimally invasive method led to the development of EarPopper®. Methodology: A prospective interventional study on the use of EarPopper® in the treatment of otitis media with effusion. The EarPopper® uses the Politzer method of pressure equilibration to decongest the middle ear through the Eustachian tube. Result: Seven patients (4 males, 3 females) aged between 11 and 54 years were enrolled. Five were bilateral and 2 were unilateral (12 ears). All had type B curves and mild to moderate conductive hearing loss. The pre-therapy EarPopper Scoring System were 60-98%, while the post therapy result dropped to 0-25%. Background: To assess hybrid positron emission tomography (PET) imaging in the initial staging and outcome prediction of sinonasal malignancies. Methods: Retrospective study on patients with sinonasal malignancies undergoing hybrid PET imaging for initial staging. Results: Complete remission (CR) was achieved in 45 of 65 patients (69.2%). Overall sensitivity for detection of primaries using 18F‐fluoro‐deoxy‐d‐glucose PET (FDG‐PET) was 95.4%, for lymph node metastases 100% and distant metastases (DM) 100%. On univariate analysis, PET parameter total lesion glycolysis (TLG) was associated with achieving CR after primary treatment (176.8 ± 157.2 vs 83.7 ± 110.8, P =.03). Multivariate logistic regression demonstrated that TLG adjusted for the T classification best predicted achievement of CR. Conclusions: Hybrid PET imaging yields an excellent sensitivity in detecting primary tumors, lymph node metastases and DM in sinonasal malignancies. ABSTRACT Glomangiopericytomas are soft tissue tumors showing distinct perivascular myoid differentiation in sinonasal region that correspond to less than 0.5% of neoplasms in this region. We report the case of a 39-year-old patient with intranasal tumor of hemangiopericytoid pattern and immunohistochemistry compatible with glomangiopericytoma. We opted for external and endonasal surgical treatment, with preoperative embolization. Glomangiopericytomas are uncommon and are characterized by frequent recurrence, but metastases are rare. Generally painless, they present with unilateral nasal obstruction and/or epistaxis, with a polypoid, reddish and friable mass, and the diagnosis can be confirmed by histopathological and immunohistochemical examination.

Aim: Rhinoplasty is a surgery which can be done in state hospitals, private hospitals and private clinics. Patients gen-erally go to the most suitable clinic whereas most surgeons prefer to perform surgery in private clinics. Thus, patients often confuse where to attend. Social media plays a crucial role for patients to decide where and who to choose. We compared readability of the information text from internet pages of private clinics and private hos-pitals from both plastic surgeons and otorhinolaryngologists. Material and Methods: The word "Rhinoplasty" was searched through internet and first 30 web sites were taken into account. First 100 words from information texts about rhinoplasty from both private clinic and hospital web sites were copied and studied using a program called LIDA. Gunnig-Fog index, Flesch-Kincaid and Atesman values were noted and evaluated. Results: The readability of both private hospitals and private clinics were found to be hard. Flesch-Kincaid values showed that information texts from private clinics in web sites (23.24 ± 3.45) were significantly different (p=0,035) than private hospitals (25,19 ± 3,5). As believed to be more specific to Turkish, Atesman values in private clinics (48,58 ± 16,71) were found to be more readable than private hospitals (38.81 ± 18.06) (p=0,034). Conclusion: Comparison of information texts between private clinics and private hospitals of plastic surgeons and otorhinolaryngologists showed that Atesman values, which are believed to be more specific to Turkish, and Flesch-Kincaid values were significantly high in private clinics suggesting it is easier to understand. Fundamento: el cáncer de laringe es una enfermedad con incremento de su incidencia en Cuba en los últimos años, en la provincia de Las Tunas se mantiene esta tendencia preocupante.Objetivo: identificar los principales factores de riesgo asociados al cáncer de laringe en pacientes con este diagnóstico, atendidos en el Hospital General Docente “Dr. Ernesto Guevara de la Serna”, de la provincia Las Tunas, en el período comprendido desde enero 2016 a enero 2017.Métodos: se realizó un estudio analítico retrospectivo de tipo caso-control, con los pacientes atendidos en la consulta de otorrinolaringología de la institución y durante el periodo de tiempo declarados en el objetivo. La muestra se dividió en un grupo de estudio, los casos, con 92 pacientes y uno de control con 184 integrantes (1:2). Se analizaron las variables: grupo de edades, sexo, antecedentes patológicos personales, antecedentes patológicos familiares de cáncer y hábitos tóxicos. Se determinó la razón de probabilidades (OR, Odds Ratio) y se realizaron pruebas de significación estadística, para cada factor estudiado.Resultados: la edad comprendida entre los 61 y 70 años, el sexo masculino, antecedente de infecciones virales, leucoplasia, reflujo gastroesofágico, laringitis crónica hiperqueratósica, antecedente familiar de cáncer, exposición a sustancias tóxicas, así como, los hábitos tóxicos (tabaco y alcohol) fueron las variables identificadas como factores de riesgo de padecer cáncer de laringe.Conclusiones: se identificaron los factores de riesgo del cáncer de laringe en la población en estudio.

The airway management of a patient with severe tracheal stenosis depends on its severity, length, location, and type of surgery. Its management is complex and requires the collaboration of an experienced team of anaesthetist and ear, nose, and throat surgeon. We report an innovative combination of Ventrain? and S-Guide? for airway management of a planned endoscopic dilation of a severe subglottic stenosis in an adult patient. Background: In the Southeastern Anatolia Region, two new Ear Nose and Throat (ENT) specialists, one in the provincial center and one in the district center, were evaluated for the number, variety and complications of their operations in the first year of their specialty. A detailed analysis of the surgeries and complications was done and it was aimed to give an idea about the future of the ENT physicians who are still studying assistantship. Methods: The records of surgeries between February 1, 2017 and February 1, 2018 were reviewed. Results: The physician working at the province center performed 205, the other physician performed 165 operations. The operations were grouped according to their types. The rates of the most frequent operations in ENT practice were determined. The ratio of adenoidectomy, tonsillectomy and ventilation tube insertion were determined as 35.5% in the physician working in the province center and 25.6% in the physician working in the district center. The rate of septoplasty was 21% and 15.8%, and the rate of septorhinoplasty was 10.4% and 12.8% respectively. The rate of tympanoplasty was 15% and 9.75%, and the ratio of lymph node, cyst and lipoma excision was determined as 5% and 7.3%, respectively. Conclusion: Every ENT specialist should be able to perform basic operations effectively while graduating. Treatment options and approaches to the complications should be known. Introduction: Chronic suppurative otitis media (CSOM) is considered one of the most common causes of acquired hearing impairment in developing countries. CSOM is a multifactorial persistent inflammatory disease of the middle ear. A distinct pathophysiologic mechanism linking allergic rhinitis (AR) and CSOM remains to evolve. The purpose of this study was to investigate the association between AR and CSOM in adults.This was a case-control study. Materials and Methods: The subjects were 62 adults (23 male, 39 female) with established CSOM and 61 healthy controls.CSOM was diagnosed when there was a history of chronic (persisting for at least 3 months) otorrhea, accumulation of mucopurulent exudates in the external auditory canal or middle ear and/or perforated tympanic membrane on otoscopy. All participants were evaluated for the presence of AR by clinical evaluation of allergic symptoms, and underwent a skin-prick test for 23 common regional allergens. Statistical analysis was performed using SPSS version 16. Results: The prevalence of clinical rhinitis (allergic and non-allergic) was significantly higher among the cases compared with controls (62.5% vs. 37.5%, P=0.02). The prevalence of AR (proven by positive skin-prick test) was also significantly higher among affected adults than controls (24.6% and 13.8%, respectively). Adjusting for age, a logistic regression model showed that there was a significant difference between the two groups. Patients with AR and non-AR were at 3.27- (95% CI=1.15–9.29; P=0.036) and 2.57-(95% CI=1.01–6.57; P=0.048) fold increased risk of developing CSOM, respectively, compared with healthy individuals. Conclusion: The study showed a higher prevalence of AR in CSOM patients than in controls. It may be valuable to evaluate and control this factor in these patients.

Otorhinolaryngology Clinic, which is an integral part of Vojvodina Clinical Centre, is the first teaching base of the Department of Otorhinolaryngology from 1964. It was founded only four years after the establishment of the Faculty of Medicine in Novi Sad, pursuant to the faculty's decision. From the beginning and until now ninety professors and assistants were their consistent part. The Department of Otorhinolaryngology joined the organization of the teaching process at the Faculty of Medicine, according to the Bologna Declaration's guidelines from 2005. Since this year 2017, the teachers of the Department of Otorhinolaryngology will conduct theoretical and practical teaching in ten teaching subjects in the Serbian language as a part of the first degree studies. For the purpose of taking specialist exams from 2016, the Department of Otorhinolaryngology has two specialist commissions, and from the previous period a commission for the examination of narrow specializations in audiology and phoniatry. The Department od Otorhinolaryngology celebrated 50th anniversary since its establishment in 2014 year. The Department of Otorhinolaryngology has been and now is a nursery for the best personnel in the field of Otorhinolaryngology in Serbia and in Vojvodina. Through the decades of its existence success can be seen through numerous student awards and recognitions which are the best proof for continuous work on acquiring new and expanding existing knowledge. Abstract The characteristics of non-obese patients with mild to moderate Obstructive Sleep Apnea Syndrome (OSAS) who will present with a good response to Mandibular Repositioning Appliance (MRA) treatment have not yet been well established in the literature. The aim of this study is to assess whether polysomnographic (PSG), demographic, anthropometric, cephalometric, and otorhinolaryngological parameters predict MRA success in the treatment of OSAS. Forty (40) males with mild and moderate OSAS were assessed pretreatment and 2-months post-treatment after wearing an MRA. Demographic, anthropometric, otorhinolaryngological (ENT), cephalometric, and polysomnographic parameters, including continuous positive airway pressure (CPAP) titrated pressure, dental models, Epworth Sleepiness Scale, quality of life (Short Form SF-36), and mood state (Profile of Mood States – POMS), were assessed. The responders exhibited fewer oropharyngeal alterations, increased upper pharyngeal space, reduced lower airway space, and increased mandibular intercanine width, and they had milder disease. Nevertheless, no predictive factors of MRA success could be found. MRA was more successful among men with a more pervious airway, a larger interdental width and milder OSAS. However, a combined [1] functional and structural assessment is needed to successfully predict the [2] effectiveness of MRA treatment of OSA.

Otorhinolaryngological diseases have a high incidence in the general population; about 40% of the consultations to the health services are for reasons that referal that include alterations of ear, nose and pharyngolaringeal. Such pathologies commonly receive an erroneous approach that makes it difficult to establish the specific diagnosis leading to an inadequate therapeutic approach. Currently, there are different molecular techniques that facilitate the timely identification of some pathogens and offer new alternatives for treatment and management, although many of these still require more rigorous studies, offer the possibility of developing new ways of approaching patient. Introduction: Tourette Syndrome (TS) was described for the first time in 1825, and it is a neuropsychiatry disease, initially begins in childhood, affects more males, characterized by remarkable social and psychological commitment, impacting the lives of patients and family. Until recently, this disease were considered a rare condition, but current studies show that the prevalence rate may vary in 1% to 2,9% in a few groups. Objective: Perform a review of literature over the main aspects related to Tourette syndrome. Data synthesis: The TS is a genetic disorder, associated with neuroanatomical and neurophysiological alterations, characterized by compulsive phenomenon, whose etiology is still unknown. The clinical profile is composed, mostly, by motor tics and vocal tics, that which are divided in simple and complex. Its association with Obssessive Compulsive Disorder and Attention Deficit Hyperactivity Disorder is very commom. The diagnosis is mainly clinical and do not exist. Until now, there is no specific laboratory test that confirm this pathology. The treatment is based mainly on psychological theory. However when there is need for the use of drugs, the most widely used are the antagonists of dopamine receptors. Conclusion: The TS causes many damages as psychosocial and educational for the individual and the family. However, the diagnosis and the early treatment are able to minimize or cancel this damages. This way, knowing the general aspects that guide the TS is of utmost importance to preserve the quality of life for the patients with the disease.

OBJETIVO: Analisar a adequação dos encaminhamentos da atenção primária para a secundária em otorrinolaringologia pediátrica. MÉTODOS: Estudo realizado em Belo Horizonte, estado de Minas Gerais, de março de 2004 a maio de 2005. Foram avaliadas 408 crianças pré-escolares encaminhadas da atenção primária para a secundária do setor de otorrinolaringologia com otite, faringoamigdalite, rinossinusite, rinite alérgica e hipertrofia de amígdala/adenóide. As variáveis analisadas foram: concordância dos diagnósticos na atenção primária e secundária, tempo de espera pela consulta, acompanhamento e especialista (médico de família ou pediatra) que examinou a criança na atenção primária. A concordância dos diagnósticos foi avaliada pela análise estatística de kappa. RESULTADOS: Os pacientes tinham em média cinco anos de idade, dos quais 214 (52,5%) eram meninos, o tempo médio de espera pela consulta foi de 3,7 meses. Os diagnósticos na atenção primária e secundária foram, respectivamente: otite (44%, 49%), hipertrofia de amígdala/adenóide (22%, 33%), faringoamigdalite (18%, 23%), rinossunusite (13%, 21%), rinite alérgica (3%, 33%). Análise de concordância kappa foi 0,15 para otite com efusão, 0,35 para otite recorrente, 0,04 para hipertrofia de amígdala/adenóide, 0,43 para faringoamigdalite, 0,05 para rinite alérgica; 0,2 para rinossinusite. Os diagnósticos na atenção primária para encaminhamento à secundária, definidos pelo médico de família ou pelo pediatra que avaliou a criança foram concordantes. CONCLUSÕES: A inadequação dos encaminhamentos da atenção primária para a secundária em otorrinolaringologia foi expressa pelo longo tempo de espera pela consulta e pela baixa concordância de diagnósticos firmados entre os níveis de atenção para os mesmos pacientes avaliados. A atenção primária poderia se tornar mais eficiente se os profissionais fossem mais bem capacitados em otorrinolaringologia.OBJECTIVE: To assess the suitability of referral from primary to secondary care in pediatric Otolaryngology. METHODS: The study was performed in the city of Belo Horizonte, in the state of Minas Gerais, from March 2004 to May 2005. A total of 408 pre-school children referred from primary care to secondary care in the department of Otolaryngology presenting with otitis, tonsillitis, sinusitis, allergic rhinitis, and tonsillar/adenoidal hypertrophy was assessed. The studied variables were: agreement between diagnoses in primary and secondary care; waiting time for doctor's appointment; follow-up, and professional (pediatrician or family physician) that examined children in primary care. Agreement of diagnoses was assessed using kappa statistics. RESULTS: Patients were five years old on average, 214 (52.5%) were boys, mean waiting time for appointment was 3.7 months. Diagnoses in primary and secondary care were respectively: otitis (44%, 49%), tonsillar/adenoidal hypertrophy (22%, 33%), tonsillitis (18%, 23%), sinusitis (13%, 21%), allergic rhinitis (3%, 33%). Agreement analysis of kappa was 0.15 for otitis with effusion, 0.35 for recurrent otitis, 0.04 for tonsillar/adenoidal hypertrophy, 0.43 for tonsillitis, 0.05 for allergic rhinitis, and 0.2 for sinusitis. Diagnoses in primary care referred to secondary care were in agreement when given either by pediatrician or family physician. CONCLUSIONS: Unsuitability of referrals from primary to secondary care in otolaryngology was expressed by the long time waiting for appointments and by the low agreement between diagnoses in different level of care for the same patients. Primary health care could be more efficient if professionals were better qualified in Otolaryngology.

Objectives:To describe a method of using real patients in teaching ENT to undergraduates and to examine whether being a case patient affected patient satisfaction.Methods:In a cross-sectional study, 68 teaching-involved patients (case patients) with a suspected common ENT illness and 68 matched (in terms of age, sex and region of complaint) control patients evaluated the health service and their encounter with the physician. The students saw the case patients first independently and then saw the patient with the teacher physician. The controls were treated in a normal way.Results:Fifty-eight case patients (84 per cent) and 65 control patients (95 per cent) answered the questionnaire. The median duration of the visit was significantly longer for the case patients than the controls (115 vs 60 minutes). Almost all patients in both groups graded the overall quality of the health service, and the variables describing various aspects of the setting and the encounter with the physician, as either good or excellent.Conclusion:Patients who took part in the undergraduate teaching of ENT diseases were equally content with their primary visit as the control patients, even though their visit took a markedly longer time. This study aims to assess the association between microbial composition, biofilm formation and chronic otorhinolaryngologic disorders in Malaysia. A total of 45 patients with chronic rhinosinusitis, chronic tonsillitis and chronic suppurative otitis media and 15 asymptomatic control patients were studied. Swab samples were obtained from these subjects. Samples were studied by conventional microbiological culturing, PCR-based microbial detection and Confocal Laser Scanning Microscopy (CLSM). Haemophilus influenzae, Staphylococcus aureus, Streptococcus pneumoniae, coagulase-negative staphylococci (CoNS) and other Streptococcus species were detected in subjects of both patient and control groups. Biofilm was observed in approximately half of the smear prepared from swab samples obtained from subjects of the patient group. Most of these were polymicrobial biofilms. S. aureus biofilm was most prevalent among nasal samples while H. influenzae biofilm was more common among ear and throat samples. Results from this study supported the hypothesis that chronic otorhinolaryngologic diseases may be biofilm related. Due to the presence of unculturable bacteria in biofilms present in specimens from ear, nose and throat, the use of molecular methods in combination with conventional microbiological culturing has demonstrated an improvement in the detection of bacteria from such specimens in this study.
