Purpose: The Minimum Speech Test Battery (MSTB) for adults was introduced in 1996 (Nilsson et al., 1996) and subsequently updated in 2011 (Advanced-Bionics et al., 2011). The MSTB has been widely used by clinicians as a guide for cochlear implant (CI) candidacy evaluations and to document post-operative speech recognition performance. Due to changes in candidacy over the past 10 years, a revision to the MSTB was needed. Method: In 2022, the Institute for Cochlear Implant Training (ICIT) recruited a panel of expert CI audiologists to update and revise the MSTB. This panel utilized a modified Delphi consensus process to revise the test battery and to improve its applicability considering recent changes in CI care. Results: This resulted in the MTSB--Version 3 (MSTB-3), which includes test protocols for identifying not only traditional CI candidates but also possible candidates for electric--acoustic stimulation and patients with single-sided deafness and asymmetric hearing loss. The MSTB-3 provides information that supplements the earlier versions of the MSTB, such as recommendations of when to refer patients for a CI, recommended patient-reported outcome measures, considerations regarding the use of cognitive screeners, and sample report templates for clinical documentation of pre- and post-operative care. Background: No standard schedule for cochlear implant (CI) programming has been developed, and common practices may have CI recipients seen in excess of what is necessary. The objective of this study was to review evidence for a de-escalated, evidence-based schedule for adult CI programming. Methods: Systematic review was undertaken in March 2023 of PubMed, Scopus, and CINAHL databases using the Preferred Reporting Items for Systemic Reviews and Meta-analyses (PRISMA) guidelines. Studies were included if (1) they evaluated an evidence-based programming/follow-up schedule in new adult CI patients or (2) they evaluated programming or outcomes in a longitudinal fashion such that they could inform CI follow-up strategies. Level of evidence was evaluated using the LEGEND evidence assessment tool. Results: Our review identified 940 studies. After screening with a priori inclusion criteria, 18 studies were ultimately included in this review. Of these, 2 demonstrated feasibility of de-escalated approaches to new adult CI programming. The remainder presented longitudinal speech and programming parameter data that demonstrated relative stability of both categories by 3 to 6 months post-activation. Conclusions: Overall, there is a paucity of literature evaluating any form of evidence-based CI programming or follow-up. Most applicable data derive from longitudinal outcomes featured in studies of other CI features, with only a handful of studies directly evaluating CI programming strategies over time. However, stability in outcomes and programming detailed in the available data supports consideration of a de-escalated programming paradigm that could primarily limit programming to the very early post-activation period (before 3 to 6 months) to enhance patient care and reduce operational strains on cochlear implant programs. Children with profound deafness are at risk for serious reading difficulties. Multiple factors affect their development of reading skills, including use of cochlear implants. Further, multiple factors influence the overall success that children experience with their cochlear implants. These factors include the age at which they receive an implant, method of communication, vocabulary skills, preoperative residual hearing, and socioeconomic status. Ninety-one children with prelingual and profound hearing impairments who received cochlear implants at varying ages participated in the study. Structural equation modeling confirmed that multiple factors affected young cochlear implant users' reading comprehension skills and that there were significant associations between the predictors of reading comprehension. Pre-implant vocabulary had an indirect positive effect on reading through postimplant vocabulary, which had a direct positive effect on reading. Overall, children with stronger language skills demonstrated stronger reading outcomes. Age at implantation both directly and indirectly, through postimplant vocabulary, affected reading outcomes, and the total effect was large. Children who were younger when they received their implants tended to have higher reading comprehension scores. Socioeconomic status negatively affected reading. Children who used total communication prior to implantation tended to have stronger pre-implant vocabulary scores, but the total effect of pre-implant communication method on children's reading skills was negligible. Research and educational implications are discussed.

Objective: To characterize the catchment area and patient profile of large cochlear implant (CI) centers in the United States. Study Design: Multi-institutional retrospective case series. Setting: Tertiary referral CI centers. Methods: Patients who underwent CI surgery at 7 participating CI centers between 2015 and 2020 were identified. Patients' residential zip codes were used to approximate travel distances and urban vs rural residential areas. Results: Over the 6-year study period (2015-2020), 6313 unique CI surgical procedures occurred (4529 adult, 1784 pediatric). Between 2015 and 2019, CI procedures increased by 43%. Patients traveled a median 52 miles (interquartile range, 21-110) each way; patients treated at rural CI centers traveled greater distances vs those treated at urban centers (72 vs 46 miles, P <.001). Rural residents represented 61% of the patient population and traveled farther than urban residents (73 vs 24 miles, P <.001). Overall, 91% of patients lived within a 200-mile radius of the institution, while 71% lived within a 100-mile radius. In adults, multiple regression analysis redemonstrated an association between greater travel distances and (1) older age at the time of CI and (2) residential rural setting (both P <.001, r 2 = 0.2). Conclusions: While large CI centers serve geographically dispersed populations, most patients reside within a 200-mile radius. Strategies to expand CI utilization may leverage remote programming, telemedicine, and strategic placement of new centers and satellite clinics to ameliorate travel burden. [ABSTRACT FROM AUTHOR]

Among the various cochlear implant systems approved by the Food and Drug Administration, current labeling for pediatric usage encompasses (1) bilateral profound bilateral sensorineural hearing loss in children aged 9 to 24 months and bilateral severe to profound sensorineural hearing loss in children older than 2 years; (2) use of appropriately fitted hearing aids for 3 months (this can be waived if there is evidence of ossification); and (3) demonstration of limited progress with auditory, speech, and language development. Pediatric guidelines require children to have significantly worse speech understanding before qualifying for cochlear implantation. The safety, efficacy, and success of cochlear implants (CIs) are well established and have led to changes in criteria used by clinicians to determine who should receive a CI. Such changes in clinical decision-making have out-paced the slower-occurring changes that have taken place with regulatory bodies' and insurers' indications. We review the historical development of indications for CIs, including those of the U.S. Food and Drug Administration (FDA), Medicare, Medicaid, and private insurers. We report on expansion to include patients with greater residual hearing, such as those who receive Hybrid and EAS devices, and report on recent FDA approvals that place less emphasis on the patient's best-aided condition and greater emphasis on the ear to be treated. This includes expansion of CIs to patients with single-side deafness and asymmetric hearing loss. We review changes in the test materials used to determine candidacy, including transition from sentences in quiet to sentences in noise to the recent use of monosyllabic words and cognitive screening measures. Importantly, we discuss the recent trend to recommend CIs despite a patient not meeting FDA or insurers' indications (a practice known as "off-label"), which serves as attestation that current indications need to be updated. Objective: Children with significant hearing loss (HL) are at risk for lower self-esteem and lower perceived quality of life (QoL). This study examined how self-reported QoL of adolescents cochlear implant (CI) users compared to that of adolescents with normal hearing, and examined if factors such as socioeconomic status (SES) and communication methodology affect QoL and speech recognition. Methods: Forty-three adolescent CI users completed a 34-item questionnaire that included questions adapted from the QoL of Deaf or Hard-of-Hearing Youth questionnaire (Seattle Quality of Life Group SEAQOL. 2010 Quality of life of deaf or hard-of-hearing youth (YQOL-DHH). Seattle, Washington) and from the Kidscreen-10 Index (The KIDSCREEN Group, Europe, 2006, the KIDSCREEN questionnaires - quality of life questionnaires for children and adolescents handbook. Lengerich: Pabst Science Publishers. All subjects received their first CI prior to the age of 5, and ranged in age from 10-17 years at the time of survey completion. Results: Adolescents with CIs demonstrated self-reported QoL scores similar to children with normal hearing. Lower SES and communication mode appear to influence speech recognition, and also appear to impact self-reported QoL in different ways. Conclusions: Examination of communication outcomes, along with other factors that influence QoL, such as SES, will help clinicians identify children at risk for low QoL. The article presents a study which examined the factors that affect the effectiveness of cochlear implants (CI) in hearing impaired people, particularly the influence of the health of the auditory nerve (AN). The research focused on the use of combined animal and human experiments to better assess the factors that influence CI performance. Also cited is the use of auditory brainstem response (ABR) testing in the objective measurement of hearing.

This study examines the relationship between the teaching method, oral or total communication, used at children&apos;s schools and children&apos;s consonant-production accuracy and vocabulary development over time. Children who participated in the study (N = 147) demonstrated profound sensorineural hearing loss and had used cochlear implants for between 6 months and 10 years. Educational programs that used an oral communication (OC) approach focused on the development of spoken language, whereas educational programs that used a total communication (TC) approach focused on the development of language using both signed and spoken language. Using Hierarchical Linear Modeling (HLM) we compared the consonant-production accuracy, receptive spoken vocabulary, and expressive spoken and/or signed vocabulary skills, over time, of children who were enrolled in schools that used either OC or TC approaches, while controlling for a number of variables. These variables included age at implantation, preoperative aided speech detection thresholds, type of cochlear implant device used, and whether a complete or incomplete active electrode array was implanted. The results of this study indicated that as they used their implants the children demonstrated improved consonant-production accuracy and expressive and receptive vocabulary over time, regardless of whether their school employed a TC or OC teaching method. Furthermore, there appeared to be a complex relationship among children&apos;s performance with the cochlear implant, age at implantation, and communication/teaching strategy employed by the school. Controlling for all variables, children in OC programs demonstrated, on average, superior consonant-production accuracy, with significantly greater rates of improvement in consonant-production accuracy scores over time compared to children in TC programs. However, there was no significant difference between OC and TC groups in performance or rate of growth in consonant-production accuracy when children received their implants before the age of 5 years. There was no significant difference between the OC and TC groups in receptive spoken vocabulary scores or in rate of improvement over time. However, children in the TC group achieved significantly higher receptive spoken vocabulary scores than children in the OC group if they received their implant before the age of 5 years. The TC group demonstrated superior scores and rates of growth on the expressive vocabulary measure (spoken and/or signed) when compared to the OC group if they received their implants during their preschool or early elementary school years. There was no significant difference if the children received their implants during middle elementary school. Regardless of whether children were in the OC or TC group, children who received their implants during preschool demonstrated stronger performance, on average, on all measures over time than children who received their implants during their elementary school years. The results of this study suggest that children may benefit from using cochlear implants regardless of the communication strategy/teaching approach employed by their school program and that other considerations, such as the age at which children receive implants, are more important. Objectives: The objective of the current study is to evaluate how speech recognition performance is affected by the number of active electrodes that are turned off in multichannel cochlear implants. Several recent studies have demonstrated positive effects of deactivating stimulation sites based on an objective measure in cochlear implant processing strategies. Previous studies using an analysis of variance have shown that, on average, cochlear implant listeners' performance does not improve beyond eight active electrodes. We hypothesized that using a generalized linear mixed model would allow for better examination of this question.Methods: Seven peri- and post-lingual adult cochlear implant users (eight ears) were tested on speech recognition tasks using experimental MAPs which contained either 8, 12, 16 or 20 active electrodes. Speech recognition tests included CUNY sentences in speech-shaped noise, TIMIT sentences in quiet as well as vowel (CVC) and consonant (CV) stimuli presented in quiet and in signal-to-noise ratios of 0 and +10 dB.Results: The speech recognition threshold in noise (dB SNR) significantly worsened by approximately 2 dB on average as the number of active electrodes was decreased from 20 to 8. Likewise, sentence recognition scores in quiet significantly decreased by an average of approximately 12%.Discussion/conclusion: Cochlear implant recipients can utilize and benefit from using more than eight spectral channels when listening to complex sentences or sentences in background noise. Objective: To evaluate the use of monosyllabic word recognition versus sentence recognition to determine candidacy and long-term benefit for cochlear implantation.Study Design: Prospective multi-center single-subject design.Methods: A total of 21 adults aged 18 years and older with bilateral moderate to profound sensorineural hearing loss and low monosyllabic word scores received unilateral cochlear implantation. The consonant-nucleus-consonant (CNC) word test was the central measure of pre- and postoperative performance. Additional speech understanding tests included the Hearing in Noise Test sentences in quiet and AzBio sentences in +5 dB signal-to-noise ratio (SNR). Quality of life (QoL) was measured using the Abbreviated Profile of Hearing Aid Benefit and Health Utilities Index.Results: Performance on sentence recognition reached the ceiling of the test after only 3 months of implant use. In contrast, none of the participants in this study reached a score of 80% on CNC word recognition, even at the 12-month postoperative test interval. Measures of QoL related to hearing were also significantly improved following implantation.Conclusion: Results of this study demonstrate that monosyllabic words are appropriate for determining preoperative candidate and measuring long-term postoperative speech recognition performance.Level Of Evidence: 2c. Objectives/Hypothesis To describe the clinical experience and characterize the outcomes of cochlear implantation (CI) in children with isolated enlarged vestibular aqueduct (IEVA) as compared to children with enlarged vestibular aqueduct (EVA) associated with other bony labyrinth abnormalities. Study Design Single, tertiary care, institutional retrospective review over 2 decades. Methods The clinical course and outcomes of 55 children with EVA undergoing CI between 1991 and 2013 were reviewed. Test measures included open and closed set speech perception tests, and various speech and language measures. Results In 18 children (32.7%), IEVA was the only defect present. In 33 children (60%), EVA occurred concomitantly with incomplete partition type 2 (IP 2) bilaterally, and three children with incomplete partition type 1 bilaterally. Ninety-two percent (51 of 55) occurred bilaterally and had matching bony defects. Mean age of CI was 73.4 months. A statistically significant defect-related and linguistic-status pattern was noted, impacting the timing of implantation: IEVA = 112.8 months, IP 2 = 58.4 months ( P < .001), prelingual deafness = 53.8 months, postlingual deafness = 110.8 months ( P < .001). Controlling for implant age and hearing loss severity, IEVA children demonstrated superior performance on speech perception tests (8.2 to 20.3 point differences), though statistical significance was inconsistent ( P = .01-.40. Performance was also superior in speech and language tests, though statistical significance was never reached (2.9-13.9 point differences; P = .14-.69). Conclusions Children with hearing loss secondary to EVA respond meaningfully to cochlear implantation. However, the severity of temporal bone anomalies in these children has clinical relevance. The aim of this study was to identify across-site patterns of modulation detection thresholds (MDTs) in subjects with cochlear implants and to determine if removal of sites with the poorest MDTs from speech processor programs would result in improved speech recognition. Five hundred millisecond trains of symmetric-biphasic pulses were modulated sinusoidally at 10 Hz and presented at a rate of 900 pps using monopolar stimulation. Subjects were asked to discriminate a modulated pulse train from an unmodulated pulse train for all electrodes in quiet and in the presence of an interleaved unmodulated masker presented on the adjacent site. Across-site patterns of masked MDTs were then used to construct two 10-channel MAPs such that one MAP consisted of sites with the best masked MDTs and the other MAP consisted of sites with the worst masked MDTs. Subjects' speech recognition skills were compared when they used these two different MAPs. Results showed that MDTs were variable across sites and were elevated in the presence of a masker by various amounts across sites. Better speech recognition was observed when the processor MAP consisted of sites with best masked MDTs, suggesting that temporal modulation sensitivity has important contributions to speech recognition with a cochlear implant. Over the years, numerous technological advances have taken place in regard to the internal and external components of cochlear implant systems. Such advances include improvements in receiver and electrode design and placement, improvements in external component technology that have brought about downsizing and increased efficiency of externally worn speech processors, and improvements in surgical techniques that have resulted in decreased damage to the cochlea following electrode insertion. These advances have resulted in changes in candidacy criteria over time and improved outcomes for adults and children who receive cochlear implants. This article will provide a brief overview of some of the changes and technological advances that have taken place in the field of cochlear implants since they were first introduced. The article focuses on the study on creating a program for the speech processor of children as young as 18 months of age. Neural Response Telemetry, Neural Response Imaging, and Electrically elicited Stapedius Reflex Thresholds were employed to confirm the accuracy of behaviorally-determined map settings. Other speech strategies and programming parameters are discussed. It shows that majority of the children can provide behavioral responses to sound that facilitate creation of reliable speech processor programs.

Objectives: The present study was performed to evaluate the efficacy and clinical feasibility of using monopolar stimulation with the Clarion Simultaneous Analog Stimulation (SAS) strategy in patients with cochlear implants. Methods: Speech recognition by 10 Clarion cochlear implant users was evaluated by means of 4 different speech processing strategy/electrode configuration combinations; ie, SAS and Continuous Interleaved Sampling (CIS) strategies were each used with monopolar (MP) and bipolar (BP) electrode configurations. The test measures included consonants, vowels, consonant nucleus-consonant words, and Hearing in Noise Test sentences with a +10 dB signal-to-noise ratio. Additionally, subjective judgments of sound quality were obtained for each strategy/configuration combination. Results: All subjects but I demonstrated open-set speech recognition with the SAS/MP combination. The group mean Hearing in Noise Test sentence score for the SAS/MP combination was 31.6% (range, 0% to 92%) correct, as compared to 25.0%, 46.7%, and 37.8% correct for the CIS/BP, CIS/MP, and SAS/BP combinations, respectively, iniersubject variability was high, and there were no significant differences in mean speech recognition scores or mean preference ratings among the 4 strategy/configuration combinations tested. Individually, the best speech recognition performance was with the subject's everyday strategy/configuration combination in 72% of the applicable cases. If the everyday strategy was excluded from the analysis, the subjects performed best with the SAS/MP combination in 37.5% of the remaining cases. Conclusions: The SAS processing strategy with an MP electrode configuration gave reasonable speech recognition in most subjects, even though subjects had minimal previous experience with this strategy/con figuration combination. The SAS/MP combination might be particularly appropriate for patients for whom a full dynamic range of electrical hearing could not be achieved with a BP configuration. [ABSTRACT FROM AUTHOR]

The purpose of this study was to characterize the transtympanically evoked, perioperative electrically evoked auditors brainstem response (EABR) and define its relationship with preoperative hearing, age and hearing loss etiology in 59 children (10-60 months of age) who had received cochlear implants. The results indicate that there was no difference between wave V latency obtained from the younger (10-36 months) and the older (37-60 months) children. There was a statistically significant difference in the preoperative pure-tone average between the higher-EABR threshold group (650 µA and above) and the lower-EABR threshold group (600 µA or less). Patients with post-meningitic deafness exhibited the longest EABR wave V latencies. Perioporative, transtympanic, promontory EABR is an effective clinical procedure which can decrease the likelihood of placing a cochlear implant in a non-stimulable ear, and may provide the clinician with a valuable tool for selecting the most appropriate ear for implantation. • Selecting electrode sites for stimulation using electrically-evoked compound action potential (ECAP) interphase gap (IPG) effect can alter speech recognition in cochlear implant users. • Performance listening to a program using higher ECAP IPG Effect electrode sites was better than when using a program with lower ECAP IPG Effect electrode sites, but equal to performance using subjects' everyday program, with all electrodes active. • An ECAP-based electrode site selection approach was most effective when more electrodes are located within the scala tympani, but did not depend on medial-lateral distance. Studies in cochlear implanted animals show that the IPG Effect for ECAP growth functions (i.e., the magnitude of the change in ECAP amplitude growth function (AGF) slope or peak amplitude when the interphase gap (IPG) is increased) can be used to estimate the densities of spiral ganglion neurons (SGNs) near the electrode stimulation and recording sites. In humans, the same ECAP IPG Effect measures correlate with speech recognition performance. The present study examined the efficacy of selecting electrode sites for stimulation based on the IPG Effect, in order to improve performance of CI users on speech recognition tasks. We measured the ECAP IPG Effect for peak amplitude in adult (>18 years old) CI users (N= 18 ears), and created experimental programs to stimulate electrodes with either the highest or lowest ECAP IPG Effect for peak amplitude. Subjects also listened to a program without any electrodes deactivated. In a subset of subject ears (11/18), we compared performance differences between the experimental programs to post-operative computerized tomography (CT) scans to examine underlying factors that might contribute to the efficacy of an electrode site-selection approach. For sentences-in-noise, average performance was better when subjects listened to the experimental program that stimulated electrodes with the highest rather than the lowest IPG Effect for ECAP peak amplitude. A similar pattern was noted for transmission and perception of consonant place cues in a consonant recognition task. However, on average, performance when listening to a program with higher IPG Effect values was equal to that when listening with all electrodes activated. Results also suggest that scalar location (scala tympani or vestibuli) should be considered when using an ECAP-based electrode site-selection procedure to optimize CI performance. The goal of this study was to investigate the relationship between variation in electrode site of stimulation and the perceptual dimensions along which such stimuli vary. This information may allow more effective use of electrode place when encoding speech information. To achieve this goal, two procedures which measure pitch in subjects implanted with the Nucleus/Cochlear Corporation multichannel device were performed. Estimates of electrode discriminability that can be obtained from these procedures were compared to a more direct measure of electrode discriminability that was obtained in a previous study [Collins et al., Assoc. Res. Otolaryng. Abstracts, No. 642 (1994)]. In the first task, subjects performed a pitch ranking procedure similar to that used in previous studies [Townshend et al., J. Acoust. Soc. Am. 82, 106-115 (1987); Nelson et al., J. Acoust. Soc. Am. 98, 1987-1999 (1995)]. Estimates of the pitch percept elicited by stimulation of each electrode as well as the discriminability of the electrodes were generated from the data using two different statistical analyses. In the second task, subjects performed a pitch scaling procedure similar to one used in a previous study [Busby et al., J. Acoust. Soc. Am. 95, 2658-2669 (1994)]. Again, two different statistical analyses were performed to generate estimates of the pitch percept corresponding to stimulation of each electrode and to generate estimates of electrode discriminability. In general, the estimates of the relationships between the pitch percepts obtained from the two procedures were not identical. In addition, the estimates of electrode discriminability were not equivalent to the electrode discrimination measures obtained from the same subjects during the previous study. Signal detection theory has been used to model the decision processes required by each of the procedures described above [e.g., Jesteadt and Bilger, J. Acoust. Soc. Am. 55, 1266-1276 (1974)]. However, these models do not predict the differences that were observed between the data sets obtained during this study. An alternate model is proposed which may explain the data obtained from these subjects. This model is based on the assumption that the percept that is elicited by electrical stimulation of an electrode is multidimensional, as opposed to unidimensional in nature. Background: The Nucleus ® 6 sound processor is now compatible with the Nucleus ® 22 (CI22M)--Cochlear's first generation cochlear implant. The Nucleus 6 offers three new signal processing algorithms that purportedly facilitate improved hearing in background noise. Purpose: These studies were designed to evaluate listening performance and user satisfaction with the Nucleus 6 sound processor. Research Design: The research design was a prospective, single-participant, repeated measures design Study Sample: A group of 80 participants implanted with various Nucleus internal implant devices (CI22M, CI24M, Freedom ® CI24RE, CI422, and CI512) were recruited from a total of six North American sites. Data Collection and Analysis: Participants had their external sound processor upgraded to the Nucleus 6 sound processor. Final speech perception testing in noise and subjective questionnaires were completed after four or 12 weeks of take-home use with the Nucleus 6. Results: Speech perception testing in noise showed significant improvement and participants reported increased satisfaction with the Nucleus 6. Results from previous experiments with multichannel cochlear implant subjects [Collins et al., Midwinter Research Meeting, Association for Research in Otolaryngology, February, 1994] have shown that removal of indiscriminable electrodes from subjects' maps resulted in significant improvements in speech perception scores in some of the better subjects. The procedure used to evaluate electrode discrimination in this previous study lacked clinical applicability due to the amount of time required for data collection. In the present paper several measures of electrode discrimination, including an adaptive discrimination procedure, a pitch ranking procedure, and a paired comparison procedure are evaluated and compared. The clinical applicability of each procedure will be discussed as it relates to both efficiency and impact on mapping for improved speech perception, as measured by both open and closed set tests. In order to evaluate the effect of remapping on speech perception, information transmission analyses were performed on vowel and consonant confusion data from each subject. These data were obtained while subjects used a standard map employing all active electrodes, and while they used maps based on the results of each electrode discrimination procedure. Contextual cue utilization by two groups of cochlear implant wearers was measured with a gating task. Group 1 consisted of implant wearers who were able to perceive open-set speech (unpracticed, unfamiliar words and sentences), while group 2 consisted of implant wearers who were unable to perceive open-set speech. Gated stimuli consisted of high- and low-predictability sentences taken from the speech perception in noise (SPIN) test. Subjects' responses were evaluated to determine the amount of acoustic information required for phoneme and word recognition as well as the percentage of phonemes and words identified in each type of context. Results indicated that open-set listeners required less information than non-open-set listeners to recognize phonemes and words. More importantly, a predictability-by-listener interaction demonstrated that open-set listeners were better able to make use of contextual cues than were non-open-set listeners. Two adult cochlear implant users, with a history of hearing aid use in their non-implanted ear, underwent speech perception and sound localization testing to investigate whether the use of a hearing aid programmed for low-frequency gain only, would be superior to a hearing aid programmed for both low- and high-frequency gain. Background: While published data exist regarding cochlear implant (CI) outcomes from large academic programs, evidence of benefit based on national, multicenter clinical trials is needed for information regarding typical patient outcomes of devices implanted by U.S. centers representing larger academic to smaller hospital-based programs. Purpose: This nationwide trial evaluated outcomes in a group of newly implanted adult recipients of the Cochlear™ Nucleus® 5 Cl system and SmartSound™ 2 signal processing. Unlike previous clinical trials, the AzBio sentence test was used and represents recent transition in our field to use of more challenging test materials. It was hypothesized that (1) speech perception scores in quiet with SmartSound™ 2 signal processing would not be statistically different from previous-generation devices; (2) speech perception scores in noise with SmartSound™ 2 signal processing would be better with enhanced microphone directionality; (3) speech perception scores in noise will be better with the preferred SmartSound™ 2 program for listening in noise; and (4) cochlear implantation would improve quality of life as assessed by the updated Health Utility Index Mark 3 (HUI3). A secondary purpose was to examine the relationships among the current and previously used speech perception tests of the Minimum Speech Test Battery (MSTB). It was hypothesized that speech perception scores within the same test interval would show predictive relationships. Research Design: Prospective, single-arm, repeated-measures study across 13Cl centers in the United States between February 2010 and June 2012. The participating centers ranged from larger academic to smaller hospital-based programs to accurately represent the diversity of programs in the United States. Study Sample: Participants were 38 postlingually deafened adult Cl candidates. Data Collection and Analysis: Primary measures were Consonant-Nucleus-Consonant (CNC) words in quiet and the AzBio Sentence Test in Quiet (AzBioQ) and in Noise (AzBioN) tested at preoperative, and 3-, 6-, and 12-mo postactivation intervals. Quality of life was measured with the HUI3. For the secondary objective, statistical analyses were performed to investigate the predictive properties between current and previously used MSTB tests. Results: Mean CNC scores were significantly higher compared to the Nucleus® 24 Contour™ at 3 mo (p < 0.05) postactivation and showed no difference compared to the Nucleus® Freedom™ at 6 mo postactivation. Both SmartSound™ 2 FOCUS and NOISE programs provided significant improvements in performance in noise over the EVERYDAY program (p < 0.001), and performance with the FOCUS program was significantly better compared to the NOISE program (p < 0.001).
