Background/aim: This study aimed to compare the therapeutic efficacy of conventional physical therapy (CPT) methods for knee osteoarthritis (OA) and transfer energy capacitive and resistive (TECAR) therapy, a relatively new and increasingly used treatment modality, based on patient clinical outcomes assessments. Materials and methods: Two groups of 54 patients, aged 40 to 75, were randomly assigned. CPT was given to both groups. In addition to CPT, Group 2 underwent TECAR therapy for six sessions, three times a week for two weeks. The Western Ontario and McMaster Universities Arthritis Index (WOMAC) was used to measure the disability and pain levels of each patient before and at the end of treatment and at 1 month and 3 months. Additionally, goniometric measurements of each patient's knee joint range of motion and isometric quadriceps muscle strength were taken. Results: Significant improvements were noted in the VAS, WOMAC, and isometric quadriceps strength ratings in both groups between the pre- and posttreatment follow-ups. However, there was no discernible difference between the groups. Conclusion: For the conservative treatment of OA in the knee, TECAR therapy may be a helpful therapeutic approach. Objectives: This study aimed to evaluate the neuropathic component of chronic musculoskeletal pain in post-coronavirus disease 2019 (COVID-19) and examine the relationship between neuropathic pain and clinical and demographic characteristics. Patients and methods: This cross-sectional study included 163 adult patients (85 females, 78 males; mean age: 41.7±4.3 years; range, 22 to 50 years) with post-COVID-19 musculoskeletal pain between February 1, 2021, and April 30, 2021. Demographic and clinical characteristics, including age, sex, affected site, duration, and severity of post-COVID-19 musculoskeletal pain using the Visual Analog Scale (VAS), as well as a neuropathic component of pain using the Leeds assessment of neuropathic symptoms and signs (LANSS), were collected. The most common post-COVID-19 symptoms, presence of hospitalization, and length of hospital stay during active COVID-19 infection were recorded from the patient records. Results: The mean duration and severity of pain were 7.85±1.53 months and 5.09±1.95, respectively. Half of the patients were hospitalized, and the mean length of hospital stay was 12.15±18.06 days. The most common pain sites were upper and lower back pain, followed by leg and arm pain. A total of 92 (56.4%) patients had previously received pharmacological or nonpharmacological treatment for post-COVID-19 musculoskeletal pain. Based on the LANSS (scores >12), 31 (19%) patients had neuropathic pain. There was a significant correlation between the presence of neuropathic pain and pulmonary involvement/symptoms. The presence and length of hospital stay were correlated with LANNS scores (p<0.05). The frequency, LANSS scores, and VAS-pain scores of the patients with and without neuropathic pain were similar between male and female patients (p>0.05). Conclusion: The neuropathic component of chronic musculoskeletal pain may be common, as one-fifth of our patients had neuropathic pain as assessed by the LANNS. Therefore, the awareness of post-COVID-19 chronic neuropathic musculoskeletal pain should be increased. We believe that focusing on the identification of pain phenotypes would provide adequate and tailored chronic neuropathic musculoskeletal pain management in the post-COVID-19 period. Objectives: The study aimed to investigate the effect of yoga-based exercises on functional capacity, dyspnea, quality of life, depression, anxiety, and sleep quality following coronavirus disease 2019 (COVID-19). Patients and methods: Forty-one COVID-19-infected healthcare professionals (35 females, 6 males; mean age: 39.7±6.5 years; range, 28 to 55 years) who were actively working during the subacute period were included in the prospective controlled study between March 2021 and September 2021. The participants were divided into two groups: the yoga-based exercise group (YBEG; n=26) and the nonintervention group (n=15). Besides routine recommendations, the YBEG performed stretching, relaxation, isometric strengthening, breathing, and meditation exercises of 60 min twice a week for eight weeks. Clinical outcome was measured with flexibility tests (shoulder flexibility and sit and reach tests), hand grip strength, 6-min walk test, dyspnea score, Beck Depression and Anxiety Inventory, Short Form 36 (SF-36) quality of life, Pittsburgh sleep quality index (PSQI), and the International Physical Activity Questionnaire (IPAQ) before and after eight weeks in both groups. Results: Beck anxiety scores, IPAQ scores, PSQI, SF-36 pain, and social functioning subparameters were positively statistically significant in the YBEG (p<0.05). The only parameter found to be insignificant in time and between groups was the SF-36 mental health subparameter (p>0.05). In addition, the YBEG had significant improvements in muscle strength, flexibility, functional capacity, physical activity level, quality of life, anxiety, depression levels, and sleep scores between before and after the eight-week intervention period (p<0.05). Conclusion: Yoga programs have led to an increase in functional capacity and physical performance, a decrease in anxiety and depression complaints, and an increase in the quality of life in healthcare professionals who were in the process of returning to work during the post-COVID-19. Objectives: This study aims to examine the effect of high-frequency repetitive transcranial magnetic stimulation (rTMS) on gait parameters and lower extremity motor recovery in a more specific sample of individuals with chronic and traumatic incomplete spinal cord injury (iSCI). Patients and methods: This double-blind, sham-controlled, randomized study included a total of 28 individuals (20 males, 8 females; mean age: 35.7±12.1 years; range, 18 to 45 years) with chronic (>1 year) traumatic iSCI. The participants were randomly allocated to either sham rTMS group (n=14) or real rTMS group (n=14). We compared the groups based on the lower extremity motor scores (LEMS), the temporal-spatial gait measurements using three-dimensional gait analysis, the Walking Index for SCI-II (WISCI-II), and 10-m walking test at baseline, three weeks (post-treatment) and five weeks (follow-up) after the treatment. Results: The real rTMS group revealed a significant improvement in walking speed, LEMS score, and 10-m walking test after the treatment compared to baseline (p=0.001, p=0.002, and p=0.023, respectively). Changes in the LEMS score were significantly increased in the real rTMS group compared to the sham group at both three and five weeks (p=0.001 and p=0.001, respectively). No significant difference was observed in the other variables between the groups (p>0.05). Conclusion: Our study findings support the therapeutic effectiveness of rTMS on motor recovery in chronic iSCI. Objective: The pulmonary rehabilitation program has beneficial effects on Coronavirus Disease 2019 (COVID-19). However, the response of each patient is not the same. The aim of this study is to compare the rehabilitation processes of severe and critical patients with COVID-19 infection and investigate the effects of sarcopenia and nutritional parameters on the rehabilitation course. Materials and Methods: Patients with COVID-19 infection who continued to use oxygen were enrolled and classified into severe or critical disease groups. The modified Medical Research Council scale, Borg Rating of Perceived Exertion scale, and Barthel Index were evaluated. A 6-minute walking test, hand grip strength (HGS), and chair stand test were performed. The thickness of the quadriceps muscle was measured by ultrasound to diagnose sarcopenia. Nutrition risk screening and daily protein and calorie intake were computed. Results: Twenty-two patients were included. The oxygen requirement at discharge was reduced compared to those admitted to the rehabilitation unit (p<0.001). Sarcopenia was present in 17 (77.3%) patients in all subjects, 12 (80%) patients in the severe disease group, and in 5 (71.4%) patients in the critical disease group. HGS and the Barthel Index were lower in the critical disease group (p=0.044 and p=0.037, respectively). The duration of rehabilitation was longer in the critical disease group (p=0.044). Daily protein and calorie consumption per kilogram were similar and low in both groups (both p>0.05). Conclusion: Sarcopenia and malnutrition were common in severe and critical COVID-19 disease patients. To investigate the effects of functional electrical stimulation cycling (FES-C) training in addition to conventional physical therapy on gait, muscle strength, gross motor function, and energy expenditure in ambulatory children with spastic diplegic cerebral palsy. Twenty children with diplegic cerebral palsy were randomly assigned to FES-C group (n = 10) or control group (n = 10). Subjects trained 3 days/week for 8 weeks. Control group received conventional physical therapy. The FES-C group additionally received FES-C training. The functional muscle test was used for muscle strength assessment. Vicon-3D system was used for gait analysis. Gross Motor Function Measure (GMFM-88) was used for motor function assessment and calorimeter was used for energy expenditure. Measurements were performed at the baseline, at the eight week and at the sixteenth week. Functional muscle strength, gross motor function, and energy expenditure improved more in the FES-C group after training and follow up (p < 0.05). There was no significant difference found between the changes in gait parameters of the two groups after treatment and follow up (p > 0.05). Pelvic tilt while walking decreased after training in the FES-C group (p < 0.05). FES-C applied in addition to conventional physical therapy in children with diplegic cerebral palsy is more effective than conventional physical therapy for increasing functional muscle strength, improving gross motor function functions, and reducing energy expenditure. FES-C improves lower extremity functional muscle strength, gross motor function, and energy expenditure in ambulatory children with spastic dCP. Autoimmune rheumatic diseases have their own specific clinical presentation, and can affect multiple systems. Neurological involvement of autoimmune rheumatic diseases may involve both the central and peripheral nervous systems. Inflammation of neural tissue, autoantibody-mediated reactions, and small vessel vasculitis may be effective in the pathogenesis of neuropathy in autoimmune rheumatological diseases. Autoimmune rheumatic disease with pure motor neuron involvement is very rare in the literature. The case is here presented of a 58-year-old female patient who presented with the complaints of increasing pain and weakness in the extremities and was diagnosed with lower motor neuron disease and overlap syndrome. The patient was treated with cyclophosphamide, pulse steroid, hydroxychloroquine and intravenous immunoglobulin. After 3 months of treatment, a significant improvement was observed in the patient's clinical complaints and laboratory parameters. In conclusion, some patients with undiagnosed autoimmune rheumatic diseases may have neurological complaints. Objectives: This study aims to reveal the relationship between serum total antioxidant status (TAS), clinical parameters, and nutrition (dietary total antioxidant capacity [TAC]) in patients with fibromyalgia. Patients and methods: This cross-sectional study was conducted with a total of 60 female participants (mean age: 44.7±9.7 years; range, 18 to 50 years) at Gaziler Physical Medicine and Rehabilitation Hospital between July 2020 and February 2021. Thirty female patients with fibromyalgia were compared with 30 age-, sex-, and body mass index-matched healthy individuals. The short-form McGill Pain Questionnaire, Fibromyalgia Impact Questionnaire (FIQ), and Pittsburg Sleep Quality Index were used. Total antioxidant status, total oxidant status (TOS), and oxidative stress index (OSI) were measured. Dietary TAC was calculated using the ferric reducing ability of plasma according to a food frequency questionnaire. Results: Total antioxidant status showed no significant difference between groups (p=0.080). Total oxidant status and OSI were significantly higher in the patient group (p<0.001 and p=0.005, respectively). The mean dietary TAC was 16.5±6.5 in the patient group and 17.2±6.2 in the control group, and it was similar between groups (p=0.492). Pittsburg Sleep Quality Index global score was significantly higher in the patient group than in the control group (p<0.001). Dietary TAC showed a moderate positive correlation with serum TAS in both groups (r=0.373, p=0.042 for the patient group, and r=0.380, p=0.038 for the control group). In the patient group, TOS and OSI showed a moderate positive correlation with FIQ total scores (r=0.420, p=0.021 and r=0.450, p=0.013, respectively). The mean polyunsaturated fatty acid and omega-6 intake of the patient group was significantly lower than the control group (p=0.025 for both). Dietary antioxidant intake from vegetables (p=0.025), legumes/nuts (p=0.049), and meat (p<0.001) was significantly lower, whereas dietary antioxidant intake from cereal and potatoes was significantly higher in the patient group compared to the control group (p=0.028). Objectives: This study aims to investigate the relationship between disease activity, dietary phytochemical index (DPI), and serum total oxidant status (TOS) and total antioxidant status (TAS) in patients with ankylosing spondylitis (AS). Patients and methods: Between August 2020 and January 2021, a total of 37 patients (23 males, 14 females; mean age: 39.3±9.4 years; range, 21 to 61 years) with AS and 36 age-, sex-, and body mass index-matched healthy individuals (24 males, 12 females; mean age: 37.9±8.9 years; range, 20 to 60 years) were included. Serum TAS (µmoLTroloxEq/L) and TOS (µmoL H2O2Eq/L) measurements were performed and the oxidative stress index (OSI) was calculated. Dietary evaluation was made from a one-day dietary record and DPI was calculated. Results: Serum TAS level in AS patients was significantly lower than the healthy group (p=0.003). Serum TOS level was similar in both groups. The OSI of patients was significantly higher than the controls (p=0.035). The mean DPI, polyunsaturated fatty acid, n-3 fatty acid, and vitamin C intake of patients were significantly lower than controls (p=0.042, p=0.033, and p=0.022, respectively). A moderate positive correlation was found between the TAS level and DPI of the control group (r=0.352, p=0.035). According to medications, no significant difference was seen between the groups in terms of patients' characteristics, DPI, and laboratory tests and there was no correlation between DPI, TAS, TOS, and OSI. Conclusion: Lower DPI and lower n-3 fatty acid and vitamin C intake in patient group demonstrated that patients with AS should pay more attention to their diet to increase serum antioxidant status. Aims: To compare demographic and clinical features and their relation to rehabilitation outcomes in patients impaired swallowing after stroke or traumatic brain injury (TBI), anoxic brain injury, encephalitis and glioma. Methods: This retrospective study evaluated the medical records of patients with brain injury in our university rehabilitation. Patients who received swallowing therapy for 24 sessions (3 days a week for 8 weeks) were included. Swallowing was evaluated with a functional oral intake scale (FOIS) and videofluoroscopic swallowing study before and after rehabilitation. Videofluoroscopy images were scored using the Penetration Aspiration Scale (PAS). Functional status was assessed with the Functional Independence Measure. Results: The study included 271 patients [stroke: 175 (64.6%); TBI: 58 (21.4%); anoxic brain injury: 13 (4.8%); glioma: 18 (6.6%); and encephalitis 7 (2.6%)]. Significant improvement was observed on the FOIS in stroke (pretreatment: 5.9±1.9 vs. posttreatment: 6.1±1.9, p=0.011) and TBI groups (pretreatment: 5.3±2.4 vs. posttreatment: 5.9±7.0, p=0.007). In both groups, significant improvements were observed in all three consistencies (solid, pudding, and liquid) according to PAS after treatment (p<0.05). Stroke patients with the middle cerebral artery (MCA) and posterior cerebral artery syndromes showed statistically significant improvement on FOIS. Patients with the MCA syndrome also showed significant improvement in the liquid score of the PAS. Conclusions: Swallowing therapy was found effective in improving swallowing functions in patients with stroke and TBI. Trunk control in sitting position after stroke is one of the most important determinants of independence in daily living activities and there is no gold standard assessment used to measure sitting postural control. The aim of this study is to determine the reliability and validity of the Turkish version of Function in Sitting Test (FIST-T). After translation was conducted, Function in Sitting Test, Berg Balance Scale, Functional Independent Measure and Trunk Impairment Scale were applied to 72 stroke patients (mean age was 59.26 ± 16.38 years; post-stroke time was 95.93 ± 59.64 days). For reliability and validity analysis the spearman correlation analysis was used. A positive, high correlation was found between the first application and repetition of Function in Sitting Test-T (ICC = 0.97, r = 0.95, p =.001). The internal consistency was high (Cronbach Alpha = 0.97), interrater correlation was high (Cronbach Alpha = 0.98) and a positive, high correlation was found with the scores obtained from the Berg Balance Scale (r = 0.82, p =.001), Functional Independent Measure (r = 0.84, p =.001) and Trunk Impairment Scale (r = 0.80, p =.001). A complication of electrical burns that increase the dysfunction is the peripheral nerve compression syndrome. Here, we presented a seventeen-year-old male patient who had an above-elbow amputation on the left and developed burns and severe median nerve entrapment in the right upper extremity after an electrical shock and underwent ultrasound-guided hydrodissection treatment. After the treatment, the edema detected ultrasonographically in the median nerve regressed, pain decreased and hand functions improved in the right upper extremity. [ABSTRACT FROM AUTHOR]

Aim: To investigate demographic features, treatment response and factors affecting improvement in patients with joint contracture. Methods: Three hundred sixty-one in patients with decreased range of motion were included in this retrospectively observational study. The demographic and clinical characteristics of patients were recorded. According to the physical therapy modalities, patients were divided into three subgroups. All patients received conventional therapy, which consisted of hot pack, therapeutic ultrasound, and stretching exercises. In addition to the conventional therapy, transcutaneous electrical nerve stimulation (TENS) was applied to the patients in group A, hydrotherapy (whirlpool) to the patients in group B, TENS and hydrotherapy to the patients in group C. Results: There were 130 (36%) knee, 66 (18.3%) ankle, 58 (16.1%) elbow, 49 (13.6%) wrist, 40 (11.1%) shoulder, and 18 (5%) hip joint cases. Patients had statistically significant improvements of all decreased joint range of motion (p<0.001). Therapy of more than 30 sessions improved only knee flexion and wrist extension significantly. A negative correlation was detected between age and the difference in wrist extension (r=-0.324, p=0.023). Improvements of knee flexion and elbow flexion were significantly better in the acute group than in subacute and chronic groups (p=0.03, p=0.036). There was no statistically significant difference in range of motion of the elbow between the patients who used and those who did not use an adjustable elbow contracture orthosis. There was no statistically significant difference between the treatment protocols. Conclusion: Therapy of more than 30 sessions is useful for only knee flexion and wrist extension contracture. Objectives: This study aims to compare the effectiveness of the partial body weight-supported treadmill exercise (PBWSTE), robotic-assisted treadmill exercise (RATE), and anti-gravity treadmill exercise (ATE) in children with spastic cerebral palsy (CP). Patients and methods: Between December 01, 2015 and May 01, 2016, a total of 29 children (18 males, 11 females; mean age 9.3±2.3 years; range, 6 to 14 years) with spastic CP were included in the study. The patients were randomly divided into three groups as the PBWSTE group (n=10), RATE group (n=10), and ATE group (n=9). Each group underwent a total of 20 treadmill exercise sessions for 45 min for five days a week for a total of four weeks. The patients were assessed using three-dimensional gait analysis, open-circle indirect calorimeter, six-minute walking test, and Gross Motor Functional Measurement (GMFM) scale before and after treatment and at two months of follow-up. Results: No significant change compared to baseline was found in the walking speed on gait analysis among the groups after the treatment. There was no statistically significant difference among the groups in terms of the GFMF-D, GMFM-E and six-minute walking test (p>0.05). There was a significant improvement in the oxygen consumption in the ATE group (p>0.05) and RATE group (p>0.05), but not in the PBWSTE group (p<0.05). Conclusion: Our study findings indicate that all three treadmill exercises have a positive impact on walking, and RATE and ATE can be used more actively in patients with spastic CP. Background: Review of the literature clearly reveals that little is known about the association between functional and mental status, and Lower Urinary Tract Dysfunction (LUTD) in patients with stroke. Objective: The aim of this study was to assess functional and mental status in stroke patients and to identify possible associations with the prevalence, severity and bother of LUTD. Material and methods: This study was designed as a cross-sectional study and included 260 stroke patients enrolled from six different hospitals in Turkey. The patients were questioned using the Danish Prostatic Symptom Score (DAN-PSS) Questionnaire to evaluate LUTD, and evaluated using the Modified Barthel Index (MBI), Incontinence Quality of Life Questionnaire (I-QoL), and the Mini Mental State Examination (MMSE). Results: At least one LUTD finding was reported in 243 (93.5%) patients; the most commonly encountered complaint in these patients was nocturia (75.8%). The mean MBI, MMSE, and I-QoL scores were found to be significantly lower in LUTD (+) patients compared to LUTD (−) patients (p = 0.000, p = 0.005, and p < 0.01, respectively). Similarly all parameters (MBI, MMSE, and I-QoL scores) assessed were found to be significantly lower for patients with urinary incontinence than those without incontinence (p = 0.000, p = 0.000, and p < 0.01, respectively). Conclusion: LUTD is a common problem in patients with stroke. LUTD is associated with poorer cognitive and functional status and the quality of life in these patients. Aims: The aim of current study was to evaluate the prosthesis use, level of satisfaction, reported problems and quality of life in patients with combat related lower limb amputation. Methods: In this descriptive study, a total of 30 patients with 35 amputations were included. The survey collected data on basic demographics including age, time since amputation, gender, etiology, level and side of the amputation, daily prosthetic use frequency, daily total prosthetic wearing time, satisfaction with prosthesis, causes of dissatisfaction and any problems related to the amputation. Satisfaction with the prosthetic device was measured with a numeric scale (0 to 10).Quality of life was assessed with Short-Form Health Survey (SF-36). Results: The leading reasons for rejection or dissatisfaction with the lower limb prosthesis were excessive perspiration, itching and pain. The mean daily prosthesis wearing time was 11.4±3.8 hours. There was a negative correlation between daily prosthesis wearing time and cosmetic anxiety (r:-0.416) (p<0.05). The mean satisfaction level with prosthesis was 7±3.8 and itching, pain and wound were significantly correlated with satisfaction level (r:-0.491, -0.528 and -0.480, with respectively) (p<0.05). SF-36 scores were found to be lower for amputees when compared to those for the general population in Turkey. Conclusions: Participants reported high levels of use and satisfaction with prosthesis, in spite of excessive perspiration, itching and pain. BACKGROUND: Low back pain (LBP) is the most common type of musculoskeletal pain, thus it is one of the most commonly encountered conditions in Physical and Rehabilitation Medicine. The physicians who are primarily responsible for the nonsurgical management of LBP are physiatrists. OBJECTIVE: The present study aimed to investigate the approaches of physiatrists to low back pain across Europe. Preferences, tendencies, and priorities in the diagnosis, management, and treatment of LBP, as well as the epidemiological data pertaining to LBP in PRM practice were evaluated in this Europe-wide study. METHODS: The study was conducted under the control of the European Society of Physical and Rehabilitation Medicine (ESPRM) Musculoskeletal Disorders Research Committee. A total of 576 physiatrists from most European countries participated in the survey. RESULTS: The results show that physiatrists frequently deal with patients with LBP in their daily practice. Most patients are not referred to other departments and are treated with various conservative methods. Less than one-fifth of patients are primarily referred for surgery. The physiatrists believe that a clear diagnosis to account for cases of low back pain is rarely established. The most common diagnosis is discopathy. History and physical examination remain the most valuable clinical evaluation tools for low back pain according to physiatrists. Less than half the patients require a magnetic resonance imaging. Non-steroidal anti-inflammatory drugs are the most commonly prescribed drugs for low back pain. Exercise, back care information, and physical therapy are the preferred conservative treatments. More than half of the physiatrists offer interventional treatments to patients with low back pain. CONCLUSION: The present study is a preliminary report that presents the attitudes of European physiatrists in the management of low back pain. Objectives: This study aims to investigate the effect of rehabilitation on functional level of traumatic brain injury (TBI) patients and to examine the associated factors on functional gain in this patient population. Patients and methods: Between October 2010 and November 2015, a total of 71 patients (63 males, 8 females; mean age 26.6±8.1 years; range, 18 to 56 years) who were admitted to our rehabilitation clinic with moderate-to-severe TBI were retrospectively analyzed. Functional recovery was assessed using the Functional Independence Measure (FIM) and Functional Ambulation Classification (FAC) scales. The patients were divided into two groups according to time from TBI to the initiation of rehabilitation: early (<6 months) and late (=6 months). Possible predictive factors associated with FIM gain were evaluated. Results: There was a significant improvement in the FIM scores from admission to discharge (p<0.001). There was a statistically significant difference in the FIM gain and FIM efficiency between the patient groups according to the initiation of rehabilitation (p<0.001). The FAC scores increased from admission to discharge, showing statistical significance (p<0.001). Duration of rehabilitation, early rehabilitation, heterotopic ossification, and deep venous thrombosis were found to be significant factors associated with FIM gain (p<0.001). Conclusion: Our study results suggest that rehabilitation is effective for functional gain, particularly in the early period in patients with moderate-to-severe TBI and duration of rehabilitation, early rehabilitation, heterotopic ossification, and deep venous thrombosis are also predictors of functional improvement.
