Circular RNAs (circRNAs) play important roles in many human diseases. However, their role in the development of severe sepsis, a condition that remains one of the main causes of death in intensive care units, has not yet been defined. In this study, we interrogated the molecular mechanisms of circRNAs in severe sepsis. We profiled the expression levels of 5,680 circRNAs in plasma extracted from blood samples of 9 severe sepsis cases or 9 controls (male, age 78 ± 7) using the Human circRNA Array. To enrich protein-coding genes hosting severe sepsis-related circRNAs, we conducted gene ontology and pathways analyses. Treatment of breast cancer with low expression of human epidermal growth factor receptor 2 (HER2; HER2-low) has drawn much attention in recent years. With the proven therapeutic effect of trastuzumab deruxtecan (T-DXd) in patients with HER2-low (immunohistochemistry [IHC] 1+, or IHC2+/in situ hybridization [ISH]-) breast cancer, HER2-low may become a new subtype of targeted therapy for breast cancer. The expert committee formulated this consensus based on the current clinical studies and clinical medication experience. The current consensus is the collaborative work of an interdisciplinary working group, including experts in the fields of pathology and oncology. The purpose of this consensus was to guide the clinical diagnosis and treatment of HER2-low breast cancer, thereby prolonging the overall survival of patients.

BackgroundThe output signal quality of optical encoder in nuclear radiation environment is degraded due to the total ionizing dose (TID) effect of γ radiation.PurposeThis study aims to propose an improved adaptive line enhancer method (IALEM) that considers accuracy and efficiency to minimize the degradation of the output signal quality of photoelectric encoders in nuclear radiation environments caused by TID effect of γ radiation.MethodsFirstly, the Softsign function was introduced into the least mean square (LMS) algorithm to establish the nonlinear relationship between the error and the step size. Then, rapid convergence and small steady-state errors were achieved by introducing the previous step size value in the step-size-updating formula. Furthermore, this improved adaptive line enhancer method (IALEM) was compared with similar algorithms in terms of four aspects: convergence speed, steady-state error, low signal-to-noise input, and computational volume. Finally, the proposed algorithm was implemented in a field programmable gate array (FPGA) chip and verified on a photoelectric encoder platform, and its filtering effect was experimentally validated in a cobalt-60-source γ-radiation environment.ResultsThe results show that the proposed algorithm yields a higher convergence speed, lower steady-state error, and better filtering effect than that of other algorithms for low signal-to-noise ratio signals with less computational effort. Uniformity error and orthogonality error of the photoelectric encoder output signal after filtering by IALEM are reduced by 17.6% and 8.0%, respectively.ConclusionsThe experimental results show that the proposed algorithm can effectively filter out the noise generated through γ radiation and improve the output signal quality of the photoelectric encoder.

In order to explore the distribution characteristics of the overlying rock stress field in the floor roadway at different locations, FLAC3D software was used to simulate and analyze the surrounding rock directly above the floor roadway and the surrounding rock within 15m on both sides of the floor roadway when the distance between the floor roadway and the coal roadway and the horizontal distance were changed. The stress field distribution characteristics are obtained, and the stress field distribution characteristics of different areas directly above and on both sides of the floor roadway are obtained, which provides a theoretical basis for the location selection and support of the floor roadway.

In order to study the temporal and spatial variation of gas flow in different rock pillar extraction boreholes, gas pressure, gas content, gas emission from 100-meter coal holes and coal seam permeability coefficient were measured on site. The site inspected the gas flow in the boreholes at 15m, 7.5m, directly above, 7.5m, and 15m at the lower slab of different rock pillar floor roadways. It analyzed the change law of gas flow in boreholes of 710 floor lanes and 505 floor lanes, which provided a basis for the layout of gas drainage boreholes.

The conventional gathering and transportation mode of heating the produced fluid of oil wells with hot water or steam may result in excessive energy consumption. In order to perform the unheated transportation, the idea of hydraulic suspension transport of the gelled crude oil is proposed based on the actual production of Daqing Oilfield, and the experimental system is established to test characteristics of oil particle distribution which have an important effect on the hydraulic suspension transportation. In the experiment, the image of gelled crude oil particle distribution was obtained in a horizontal pipe with inner diameter of 0.053 m, and then the law of particle distribution was investigated by the theoretical model. The results showed that the gelled crude oil hydraulic suspension transport could be achieved without any chemical reagent when the gelled crude oil was transformed into particles and dispersedly suspended in water. The results also showed that the gelled oil particles of 0–4 mm in size accounted for 92% or more of all particles, and the percentage of gelled crude oil particles of a size of 4 mm gradually increased with the increasing mixed flow rate.

Abstract Background To explore the outcomes and prognostic factors of ovarian metastasectomy intervention on overall survival from extragenital primary cancer. Methods Patients with ovarian metastases from extragenital primary cancer confirmed by laparotomy surgery and ovarian metastases resection were retrospectively collected in a single institution during an 8-year period. A total of 147 cases were identified and primary tumor sites were colorectal region (49.0%), gastric (40.8%), breast (8.2%), biliary duct (1.4%) and liver (0.7%). The pathological and clinical features were evaluated. Patients’ outcome with different primary tumor sites and predictive factors for overall survival were also investigated by univariate and multivariate analysis. Results Metachronous ovarian metastasis occurred in 92 (62.6%) and synchronous in 55 (37.4%) patients. Combined metastases occurred in 40 (27.2%). Bilateral metastasis was found in 97 (66%) patients. The median ovarian metastasis tumor size was 9 cm. There were 39 (26.5%) patients with massive ascites ≥ 1000 mL on intraoperative evaluation. With a median follow-up of 48 months, the median OS after ovarian metastasectomy for all patients was 8.2 months (95% CI 7.2-9.3 months). In univariate analyses, there is significant (8.0 months vs. 41.0 months, P = 0.000) difference in OS between patients with gastrointestinal cancer origin from breast origin, and between patients with gastric origin from colorectal origin (7.4 months vs. 8.8 months, P = 0.036). In univariate analyses, synchronous metastases, locally invasion, massive intraoperative ascites (≥ 1000 mL), and combined metastasis, were identified as significant poor prognostic factors. In multivariate analyses combined metastasis (RR, 1.72; 95% CI, 1.09-2.69, P = 0.018), locally invasion (RR, 1.62; 95% CI, 1.03-2.54, P = 0.038) and massive intraoperative ascites (RR, 1.58; 95% CI, 1.02-2.49, P = 0.04) were independent factors for predicting unfavorable overall survival. Conclusion Ovarian metastases are more commonly originated from primary gastrointestinal tract. The prognosis of ovarian metastasis is dismal and the benefit of ovarian metastatectomy is limited. Combined metastasis outside ovaries, locally invasion and massive intraoperative ascites were independent factors for predicting unfavorable overall survival. The identification of the primary tumor is required to plan for adequate treatment for this group of patients.
