1. Utility of epitope-specific IgE, IgG4, and IgG1 antibodies for the diagnosis of wheat allergy.

Abstract

[Display omitted] The bead-based epitope assay has been used to identify epitope-specific (es) antibodies and successfully used to diagnose clinical allergy to milk, egg, and peanut. We sought to identify es-IgE, es-IgG4, and es-IgG1 of wheat proteins and determine the optimal peptides to differentiate wheat-allergic from wheat-tolerant using the bead-based epitope assay. Children and adolescents who underwent an oral food challenge to confirm their wheat allergy status were enrolled. Seventy-nine peptides from α-/β-gliadin, γ-gliadin, ω-5-gliadin, and high- and low-molecular-weight glutenin were commercially synthesized and coupled to LumAvidin beads (Luminex Corporation, Austin, Tex). Machine learning methods were used to identify diagnostic epitopes, and performance was evaluated using the DeLong test. The analysis included 122 children (83 wheat-allergic and 39 wheat-tolerant; 57.4% male). Machine learning coupled with simulations identified wheat es-IgE, but not es-IgG4 or es-IgG1, to be the most informative for diagnosing wheat allergy. Higher es-IgE binding intensity correlated with the severity of allergy phenotypes, with wheat anaphylaxis exhibiting the highest es-IgE binding intensity. In contrast, wheat-dependent exercise-induced anaphylaxis showed lower es-IgG1 binding intensity than did all the other groups. A set of 4 informative epitopes from ω-5-gliadin and γ-gliadin were the best predictors of wheat allergy, with an area under the curve of 0.908 (sensitivity, 83.4%; specificity, 88.4%), higher than the performance exhibited by wheat-specific IgE (area under the curve = 0.646; P <.001). The predictive ability of our model was confirmed in an external cohort of 71 patients (29 allergic, 42 nonallergic), with an area under the curve of 0.908 (sensitivity, 75.9%; specificity, 90.5%). Abstract

Patients with asthma need uninterrupted supplies of affordable, quality-assured essential medicines. However, access in many low- and middle-income countries (LMICs) is limited. The World Health Organization (WHO) Non-Communicable Disease (NCD) Global Action Plan 2013&ndash, 2020 sets an 80% target for essential NCD medicines&rsquo, availability. Poor access is partly due to medicines not being included on the national Essential Medicines Lists (EML) and/or National Reimbursement Lists (NRL) which guide the provision of free/subsidised medicines. We aimed to determine how many countries have essential asthma medicines on their EML and NRL, which essential asthma medicines, and whether surveys might monitor progress. A cross-sectional survey in 2013&ndash, 2015 of Global Asthma Network principal investigators generated 111/120 (93%) responses&mdash, 41 high-income countries and territories (HICs), 70 LMICs. Patients in HICs with NRL are best served (91% HICs included ICS (inhaled corticosteroids) and salbutamol). Patients in the 24 (34%) LMICs with no NRL and the 14 (30%) LMICs with an NRL, however no ICS are likely to have very poor access to affordable, quality-assured ICS. Many LMICs do not have essential asthma medicines on their EML or NRL. Technical guidance and advocacy for policy change is required. Improving access to these medicines will improve the health system&rsquo, s capacity to address NCDs.

Abstract

Background There are few prospective studies on the natural course of chronic urticaria (CU) in children. Objective We sought to examine the natural history of CU in children and to identify predictors for remission. Methods Children 4 to 15 years of age with CU were investigated with a complete blood cell count, erythrocyte sedimentation rate, antinuclear antibody titer, complement CH 50 level, thyroid studies, autologous serum skin test, skin-prick tests, food challenges, and stool examination for parasites. They were considered to be in remission if symptoms did not recur for at least 12 months without medication. Results In all, 92 children (53.3% female) with CU were recruited and followed up for a median duration of 4.3 years (range 2.5-5.8 years). Chronic autoimmune urticaria (CAU) was identified in 40% of the patients. Food allergy was found in 8.7% and parasitic infestations in 5.4%. Remission rates at 1, 3, and 5 years after the onset of CU symptoms were 18.5%, 54%, and 67.7%, respectively. The remission rate did not differ in CAU compared with non-CAU. No predictor of CU remission was identified. Limitations The basophil histamine release assay was not performed. Conclusion Children with CU have a favorable outcome. Abstract

Background: Nonallergic rhinitis (NAR) is characterized by nasal symptoms similar to allergic rhinitis (AR) without an IgE-mediated immune response. Limited data are available on the natural history of NAR in its progression toward AR, particularly in children. This study evaluates the development of AR in children who were previously diagnosed with NAR. Methods: Children with the diagnosis of NAR during the period of 2005-2007 were reevaluated in 2010. Nasal symptoms, disease severity, comorbidities, rescue medication scores (RMSs), and skin-prick test to aeroallergens were assessed. Results: We recruited 175 children with an early diagnosis of NAR. The median age was 5.7 years, 62.9% were boys and 45.7% had family history of atopy. At réévaluation, 41% of children with previously diagnosed NAR developed sensitization to aeroallergens and were reclassified as having AR. The most frequent aeroattergen sensitization was Dermatophagoides pteronyssinus (59.7%), followed by Dermatophagoides farinae (54.2%) and American cockroach (38.9%). Children who developed AR had more nasal/eye symptoms, higher severity, and RMSs than children who did not develop AR. The predictors of developing AR were persistent nasal symptoms (adjusted odds ratio [OR], 8.9; 95% CI, 3.2-24.6), nasal itching (adjusted OR, 3.4; 95% CI, 1.2-9.5), triggered by house dust (adjusted OR, 4.3; 95% CI, 1.6-11.9) and animal danders (adjusted OR, 15.8; 95% CI, 3.3-76.1), and family history of atopy (adjusted OR, 6.0; 95% CI, 2.3-15.9). Conclusion: Children with NAR who had family history of atopy, persistent nasal symptoms, and symptoms triggered by aeroallergens should be reevaluated periodically for the development of AR. Abstract

Wheat can cause severe immunoglobulin E (IgE)-mediated systemic reactions including anaphylaxis but knowledge on relevant wheat allergens at the molecular level is scanty.Seven children (aged from 6 months to 13 years) experiencing from 2 to 10 anaphylactic reactions in a year after eating food-containing wheat were examined. Purifiedω-5 gliadin was used as an allergen in IgE enzyme-linked immunosorbent assay (ELISA) and in skin prick testing (SPT). Wheat CAP radioallergosorbent test (RAST) and SPT were also examined.All seven anaphylactic children, but none of 15 control subjects had IgE antibodies toω-5 gliadin in ELISA. Five of the six tested anaphylactic children showed positive SPT toω-5 and crude gliadin, and all seven had positive wheat CAP RAST and SPT. One child was challenged with wheat, which caused anaphylaxis. After adherence to a wheat-free diet four children remained symptomless and three experienced one to two anaphylactic reactions.The present results show that wheatω-5 gliadin is a major sensitizing allergen in children with wheat-induced anaphylaxis. 26. A Dose-Finding Study with the Once-Daily Oral β-Agonist Bambuterol in Asian Children with Asthma.

Abstract

Objective: The primary objective was to compare the efficacy of two dosages of bambuterol, a once-daily oral bronchodilator, with placebo in Asian children with asthma. The secondary objective was to assess the tolerability of bambuterol in this population. Design: This was a multicentre, randomised, double-blind, placebo-controlled crossover study, with a run-in period of 2 weeks. The study consisted of three treatment periods of 2 weeks each, separated by washout periods of 2 weeks, with 7 clinic visits in total. Setting: Outpatients were recruited from 10 centres in Indonesia, Malaysia, The Philippines, Singapore and Thailand. Patients: Seventy Asian children aged 6 to 12 years (53 boys and 17 girls) with asthma for at least 6 months were randomised to treatment, and 62 patients completed the study. Interventions: During the three treatment periods, patients received either placebo or 5 or 10mg bambuterol tablets once daily for 2 weeks. Morning peak expiratory flow (PEF) was the primary efficacy variable. Secondary efficacy variables were forced expiratory volume in 1 second (FEV), forced vital capacity (FVC) and diary card data including evening PEF. Results: At baseline, FEV predicted was 82.9% and the reversibility was 22.2%. The study results suggest that the change from baseline in both morning and evening PEF recorded on the diary cards on days 4 to 14 of each treatment period was dose dependent, but only the difference in morning PEF between bambuterol 10mg and placebo was statistically significant (9.1 L/min; 95% confidence interval 0.2 to 18.0 L/min). FEV, FVC, asthma symptom scores and the use of supplementary β-agonists did not differ significantly between placebo and the two bambuterol treatments. Blood pressure and pulse rate remained consistent throughout the study. The number of spontaneously reported adverse events was similar for the three treatment groups. Conclusions: Bambuterol 10mg once daily is an effective bronchodilator in 6- to 12-year-old Asian children in terms of improvement in morning PEF. [ABSTRACT FROM AUTHOR]

Abstract

House dust mites have been shown to be the most important source of allergen in Thailand. The results of our initial mite survey were in conflict with previous data reported 20 years ago. To evaluate the current condition of mite fauna in Thailand, we conducted a larger scale survey of house dust mites in dusts from three different parts of the country. A total of 630 mattress dust samples was collected from various types of housings from central, northern and northeastern parts of Thailand. House dust mites were identified and enumerated by standard techniques and 388 dust samples were analysed further for group 1 antigen by a two-site monoclonal antibody enzyme-linked immunosorbent assay. House dust mites were identified in 555/630 (88.1%) of dust samples collected; 27.6% of these 555 samples contained over 100 mites per gram of fine dust. Dermatophagoides pteronyssinus was the most abundant species found with a mean count of 87.2 mites per gram of dust (range 1–1460). Group I antigen was detected in 380/388 (97.9%) samples analysed and 330 of which (868%) contained over 2 μg/gm of group I antigen. The highest density of mites was found in dusts from northern Thailand whereas dusts with highest amount of antigen were those collected from the northeastern part. Some degree of correlation between mite count and mite allergen was observed (r² = 0.133, P < 0.001). No significant difference in the mite count and mite allergen was found among various types of housings. House dust mites are the source of allergens ubiquitously found all over Thailand. Abstract

BACKGROUND The Global Asthma Network (GAN), by using the International Study of Asthma and Allergies in Childhood (ISAAC) methodology, has updated trends in prevalence of symptoms of childhood allergic diseases, including non-infective rhinitis and conjunctivitis ("rhinoconjunctivitis") which is reported here. METHODS Prevalence and severity of rhinoconjunctivitis were assessed by questionnaire among schoolchildren in GAN Phase I and ISAAC Phases I and III surveys 15-23 years apart. Absolute rates of change in prevalence were estimated for each centre and modelled by multi-level linear regression to compare trends by age group, time period and per-capita national income.
