A renal transplant recipient developed acute renal transplant vein thrombosis following surgery for a total hip replacement. As an alternative to standard surgical therapy, the patient was treated with selective intravenous low-dose infusion of streptokinase at doses of 5,000-20,000 units/h. The infusion was continued for 120 h with constant monitoring of PT, PTT, and fibrinogen levels. Therapy resulted in excellent preservation of renal function without complications. Low-dose selective infusion of streptokinase should be considered a potential alternative therapy for acute transplant renal vein thrombosis. [ABSTRACT FROM AUTHOR]

The postoperative chest radiographic findings in 38 patients undergoing orthotopic (37 patients) and heterotopic (1 patient) cardiac transplantation were evaluated. Findings were correlated with those of echocardiograms, sputum and blood cultures, and lung and heart biopsies. The radiographic manifestations in the chest of these patients are classified in the following three main categories: (1) Newly formed cardiac silhouette findings due to the transplanted heart itself, i.e., changes in size and shape of the new heart and pericardial effusion resulting from the placement of a smaller heart in a larger pericardial sac. (2) Infectious complications due to bacteria, fungal, and other opportunistic agents secondary to immunosuppressive therapy, and (3) Usual postoperative complications following thoracotomy and open-heart surgery. There are several vascular abnormalities that could affect the scrotum. The commonest is a varicocele, and differentiation between this and other lesions is possible using Doppler ultrasound and pelvic angiography. Massive perineal bleeding due to trauma was successfully controlled by superselective embolization of the internal pudendal artery with a stainless steel mini coil. While in the standard selective procedure the internal iliac artery is embolized, the superselective technique allows preservation of the blood supply from the uninjured branches of the internal iliac artery. Thickening of the wall of pulmonary veins in cyanotic congenital heart disease and the presence of hilar vessel anatomic variants may mislead a surgeon into incorrectly anastomosing the subclavian artery to the pulmonary vein when performing a Blalock-Taussig shunt. Introduction Carotid blowout syndrome (CBOS) is a catastrophic emergency associated with high morbidity and mortality. The usual causes are postoperative, inflammation, traumatic, and head and neck tumor invasion. The conventional surgical management is fraught with high morbidity, mortality and complication rates and is ineffective in some cases. This includes carotid artery and/or branch ligation. Carotid artery bypass can be performed with limited success. With the advent of uncovered and covered stents, the emergent management of CBOS has improved and, perhaps, at this time, a desperately fatal disease can be managed with temporary success. Materials and methods During the past 3 years, we have studied five patients (men and women of middle and advanced age) with CBOS of different etiologies, who were considered difficult or not manageable by emergency surgery. Insertion of different types of both uncovered and covered stents, both self-expanding and balloon expandable was performed: Gore Viabahn Endoprosthesis, iCast (Atrium) covered stent, Fluency Plus Stent-Graft (Bard), Wallstent (Boston Scientific) and Protégé (Bard). Results The procedures were done in the interventional radiology suite and were successful in temporarily managing the CBOS. No major complications related to the procedures were encountered. No arterial rupture, occlusion, thrombosis, stroke or infection resulted in a short follow-up. Conclusions Emergent insertion of covered and uncovered stents, self-expanding and balloon expandable, is a safe, effective and easy way to temporarily manage CBOS. A longer follow-up and more patients are necessary to assess the long term role of stenting in CBOS. [ABSTRACT FROM PUBLISHER]
