This study identifies clinical vision measures or retinal structural measures associated with the driving performance of diabetic retinopathy patients. Twenty-five licensed drivers with diabetic retinopathy (median age, 53 years; range, 34-72 years) completed clinical tests (visual acuity, letter contrast sensitivity, and Humphrey 30-2 visual fields) and structural examinations (retinal thickness analysis and fundus photograph grading of retinopathy and laser scarring). Driving performance was assessed with an interactive driving simulator and a driving history questionnaire. Increased retinal thickness was significantly correlated with a higher frequency of simulator accidents and near accidents. Laser scar grades significantly correlated with steeper brake-response slopes, increased brake-pressure standard deviation (SD), and longer response times. Subjects with focal laser scars had significantly higher average brake-pedal pressure and brake-pressure SD than subjects without focal laser scars. The National Institutes of Health (NIH) includes visual impairment in the 10 most prevalent causes of disability in America. As rehabilitation programs have the potential to restore independence and improve the quality of life for affected persons, NIH research priorities include evaluating their effectiveness. This paper demonstrates a clinical perspective on the use of the Rasch person-item map to evaluate the range and precision of a new vision function questionnaire in early analysis (prior to full sample). A self-report questionnaire was developed to measure the difficulty that persons with different levels of vision loss have performing daily activities. This 48-item Veterans Affairs Low-Vision Visual Functioning Questionnaire (VA LV VFQ-48) was administered to 117 low-vision patients. Preliminary analysis indicates that the questionnaire items are applicable to persons of differing abilities. The Rasch person-item map demonstrates that the field-test version of the VA LV VFQ-48 has good range and is well centered with respect to the person measure distribution. Purpose. This study was conducted to select a neuropsychological battery that correlated with driving simulator skills, thus enabling practitioners to provide information to older patients and their families about driving risks. Methods. The study was conducted in two phases. In Phase 1, a survey inquiring as to the kinds of neuropsychological tests currently used to screen patients for driving was sent to 292 licensed neuropsychologists. Of these 292 surveys, 125 (43%) were returned. We used the responses to develop a battery of nine tests, including eight neuropsychological tests and one other cognitive measure: (1) the Seashore Rhythm Test, (2) Logical Memory (Immediate [I] and Delayed [II]) of the Wechsler Memory Scale-Revised (WMS-R), (3) WMS-R Visual Reproduction (Immediate [I] and Delayed [II]), (4) Trails A and B, (5) Digit Span, (6) Digit Symbol, (7) Block Design, (8) Visual Form Discrimination, and (9) a Zoo Map Test. The complete battery included 12 measures. In Phase 2, 22 licensed drivers were recruited ranging in age from 67 to 91 years (14 males and 8 females). The Mini-Mental Status Exam (MMSE) was administered to all subjects. Scores on this test served as a criterion cutoff for placement into a group of subjects with suspected dementia (Group 1, MMSE score below 25) or a group of control subjects (Group 2, with a MMSE score of 25 or above). None of the patients had any gross motor difficulties. Following screening, subjects were administered the neuropsychological battery, a driving simulator test, and a Driving Habits Interview. Results. Data revealed a significant difference between the performance of Groups 1 and 2 on the driving simulator test in two distinct areas, staying within one's lane boundaries and speed. The suspected dementia subjects had significantly more lane boundary crossings than the control subjects and drove at significantly slower speeds. Ten neuropsychological measures correlated with driving simulator performance. The number of lane boundary crossings correlated with the greatest number of neuropsychological tests, with more lane boundary crossings correlated with poorer performance on the neuropsychological tests. In particular, Trails A, Trails B, and Logical Memory (Immediate) correlated with the largest number of driving measures. Conclusion. Preliminary findings show that commonly used neuropsychological tests correlated with driving simulator skills as measured with a driving simulator. Because the driving simulator has been shown to be correlated with actual on-road driving, one may hypothesize that these neuropsychological tests may be predictive of on-road driving. The purpose of this study was to determine the relationship between clinical measures of visual function and driving-related skills in patients with glaucoma who had good visual acuity in at least one eye and mild to moderate visual field loss. Methods. Twenty-five patients with glaucoma and twenty-nine age-equivalent normally sighted control subjects were included in the study. We tested each patient on an interactive driving simulator and collected vision data, including Lighthouse visual acuity, Goldmann and Humphrey visual fields, and Pelli-Robson contrast sensitivity. Information about real-world accident history for the previous 5-year period was obtained. Results. The glaucoma patients did not have significantly more simulator or real-world accidents than the normally sighted group. There were no significant differences between the groups in performance on seven of the eight simulator indexes that were measured. This article describes the experimental protocol used to instruct fifteen patients with peripheral visual field loss due to retinitis pigmentosa, choroideremia, or Usher's syndrome Type II how to effectively use bioptic amorphic lenses. The factors that contributed to the successful use of these lenses, as well as difficulties the patients encountered, are discussed. The results of the study (published in detail in Szlyk et al. Use of bioptic amorphic lenses to expand the visual field in patients with peripheral loss. PURPOSE: 1) To evaluate a vision rehabilitation program aimed at training persons with central vision loss to use a bioptic telescope for improving life skills, including driving and 2) to compare the outcomes of subjects who are given bioptic telescopes and training, with subjects who are prescribed telescopic lenses without training. METHODS: Twenty-five subjects ranging in age from 16 to 78 years were included in the study. Each subject was randomized to one of three groups: Group 1 received bioptic telescopes and training during the first approximately 3-month-long period of the approximately 6-month-long study; Group 2 received lenses and training during the second approximately 3-month-long period of the study; and Group 3 received the lenses for approximately 3 months without any training. An assessment battery consisting of clinical vision tests, functional tasks evaluated by an orientation and mobility specialist, driving skills evaluated by a kinesiotherapist specializing in driver's education, and psychophysical measures was administered to Groups 1 and 2 at baseline, and at approximately 3 and 6 months, and to Group 3 at baseline and at approximately 3 months. The tasks were categorized into 6 major functional categories: Recognition, Mobility, Peripheral Identification, Scanning, Tracking, and Visual Memory. Training consisted of 5 weeks of laboratory-based training focusing on skills within these 6 categories, and 8 weeks of on-road driving training. RESULTS: There was significant improvement in all task categories with use of the telescopes. There was improvement in all task groups with training, though a significant difference between the trained and untrained groups existed only in the Recognition, Peripheral Identification, and Scanning Categories, but not in Mobility, Tracking, or Visual Memory. When the tasks involving driving-related skills were analyzed separately, training also had a significant effect. CONCLUSION: There was significant improvement in visual skills with the use of a bioptic telescope. Multidimensional psychophysical and electrophysical maps of the central retina are essential for assessing the functioning of the diseased retina. In this study, grating acuity, contrast sensitivity, duration for letter identification, multifocal electroretinograms, and Humphrey visual field thresholds were measured at equivalent positions throughout the central 20°. We found that the rates of sensitivity loss were not equivalent for all psychophysical measures. The rate of loss in the duration required for letter identification as a function of eccentricity was the steepest, followed by acuity and contrast sensitivity. The rate of loss in luminance sensitivity as measured in the Humphrey visual field was the shallowest. The pattern of losses also varied across meridians. Specifically, the rate of loss as a function of eccentricity was highest in the vertical meridian and lowest in the horizontal meridian. These maps and the correlations among measures as a function of retinal position serve as a baseline so that we can examine disease effects throughout the retina. In addition, the development of vision rehabilitation programs focused on eccentric viewing training should consider the differential sensitivities of the peripheral retina. [ABSTRACT FROM AUTHOR]
