The American Gastroenterological Association (AGA) has published the first evidence-based guideline for the management of pouchitis, a common complication following restorative proctocolectomy for ulcerative colitis. The guideline outlines four types of inflammatory pouch disorders and provides recommendations for their treatment. For patients with intermittent pouchitis, antibiotics are suggested, while chronic antibiotic-dependent pouchitis may require near-continuous or cyclical antibiotic therapy. Advanced immunosuppressive therapies are recommended for chronic antibiotic-refractory pouchitis, and corticosteroids may be used for Crohn's-like disease of the pouch.
