Background and methodologyIn districts where lymphatic filariasis (LF) is endemic, the goal is to provide 100% geographical coverage of the essential package of care. Additionally, countries seeking elimination status must document the availability of services for lymphoedema and hydrocele in all endemic areas. To do this, the WHO recommends conducting assessments of the readiness and quality of services provided to identify service delivery and quality gaps. This study used the recommended WHO Direct Inspection Protocol (DIP), which consists of 14 core indicators related to LF case management, medicine and commodities, staff knowledge and patient tracking. The survey was administered in 156 health facilities across Ghana designated and trained to provide LF morbidity management services. Patient and health provider interviews were also conducted to assess challenges and feedback.Principal findingsThe highest performing indicators across the 156 surveyed facilities were related to staff knowledge; 96.6% of health workers correctly identified two or more signs and symptoms. The lowest scoring indicators concerned medication availability, with the two lowest scoring indicators in the survey being availability of antifungals (26.28%) and antiseptics (31.41%). Hospitals performed best with an overall score of 79.9%, followed by health centers (73%), clinics (67.1%) and CHPS compounds (66.8%). The most commonly reported challenge from health worker interviews was lack of medications and supplies, followed by a lack of training or poor motivation.Conclusions and significanceThe findings from this study can help the Ghana NTD Program identify areas of improvement as they seek to achieve LF elimination targets and continue to improve access to care for those with LF-related morbidity as part of overall health systems strengthening. Key recommendations include prioritizing refresher and MMDP training for health workers, ensuring reliable patient tracking systems, and integrating lymphatic filariasis morbidity management into the routine healthcare system to ensure medicine and commodity availably.

Background/aims The COVID-19 pandemic necessitated the use of personal protective equipment for those involved in trachoma survey grading and trichiasis surgery. We sought to determine which configuration of a face shield would be less likely to impact grading accuracy and ability to conduct trichiasis surgery. The research also included assessment of comfort, ease of cleaning and robustness.Methods There were three research phases. In phase 1, assessment of four potential face shield configurations was undertaken with principal trachoma graders and trichiasis surgeon trainers to decide which two options should undergo further testing. In phase 2, clarity of vision and comfort (in a classroom environment) of the two configurations were assessed compared with no face shield (control), while grading trachomatous inflammation—follicular (TF). The second phase also included the assessment of impact of the configurations while performing trichiasis surgery using a training model. In phase 3, face shield ease of use was evaluated during routine surgical programmes.Results In phase 2, 124 trachoma graders and 28 trichiasis surgeons evaluated the 2 face shield configurations selected in phase 1. TF agreement was high (kappa=0.83 and 0.82) for both configurations compared with not wearing a face shield. Comfort was reported as good by 51% and 32% of graders using the two configurations. Trichiasis skill scores were similar for both configurations.Conclusion The face shield configuration that includes a cut-out for mounting the 2.5× magnifying loupes does not appear to impact the ability or comfort of trachoma graders or trichiasis surgeons to carry out their work.

Abstract Trachoma, caused by repeated ocular infection with Chlamydia trachomatis (Ct), is targeted for elimination as a public health problem. Serological testing for antibodies is promising for surveillance; determining useful thresholds will require collection of serological data from settings with different prevalence of the indicator trachomatous inflammation—follicular (TF). Dried blood spots were collected during trachoma mapping in two districts each of Togo and Democratic Republic of the Congo. Anti-Ct antibodies were detected by multiplex bead assay (MBA) and three different lateral flow assays (LFA) and seroprevalence and seroconversion rate (SCR) were determined. The combined use of satellite-derived environmental data and a dynamic energy budget (DEB) model to determine Pacific oyster growth potential was adapted for the South African marine environment. Study areas consisted of the West Coast (high-chlorophyll, low temperature) and the South Coast (variable chlorophyll, higher temperature) ecoregions. Chlorophyll-a and sea surface temperature products from the Moderate Resolution Imaging Spectroradiometer (MODIS) were used to simulate yearly growth for 18 years. Average growth performance at the end of the culture period was mapped and compared for prominent sectors. Industry-relevant growth indicators, “days to reach commercial weight” and “optimal culture period length” were also established.High growth potential was found in eastern nearshore sectors of the South Coast, particularly Plettenberg Bay, where oysters reached a median total weight (TW) of 271 g within 9 months. Other sectors in the region yielded Large commercial size within 150 days. However, South Coast growth hotspots were found to be highly variable, with transient unfavourable growth conditions leading to low flesh mass relative to TW. Growth potential was favourable in northern sectors of the West Coast, where oysters reached a median TW of up to 148 g in 9 months, Large commercial size within 200 days, and contained high flesh mass relative to TW.Current oyster production sites were not found to be optimal for growth. Higher-growth sites coincide with areas in economic decline or with high levels of poverty. However, due to constraints of applying DEB models over large spatial scales, these results are considered preliminary and await in situ verification, as well as a spatial multi-criteria analysis, before investment and development.

The visual system must make predictions to compensate for inherent delays in its processing. Yet little is known, mechanistically, about how prediction aids natural behaviors. Here, we show that despite a 20-30ms intrinsic processing delay, the vertical motion sensitive (VS) network of the blowfly achieves maximally efficient prediction. This prediction enables the fly to fine-tune its complex, yet brief, evasive flight maneuvers according to its initial ego-rotation at the time of detection of the visual threat. Combining a rich database of behavioral recordings with detailed compartmental modeling of the VS network, we further show that the VS network has axonal gap junctions that are critical for optimal prediction. During evasive maneuvers, a VS subpopulation that directly innervates the neck motor center can convey predictive information about the fly's future ego-rotation, potentially crucial for ongoing flight control. These results suggest a novel sensory-motor pathway that links sensory prediction to behavior.

Surface water is a critical resource in semiarid West-African regions that are frequently exposed to droughts. Natural and artificial wetlands are of high importance for different livelihoods, particularly during the dry season, from October/November until May. However, wetlands largely go unmonitored. In this work, remote sensing is used to monitor wetlands in semiarid Burkina Faso over large areal extents along a gradient of different rainfall and land use characteristics. Time series of data from the Moderate Resolution Imaging Spectrometer (MODIS) from 2000 to 2012 is used for near-infrared (NIR)-based water monitoring using a latitudinal threshold gradient approach. The occurrence of 21 new water bodies with a size larger than 0.5 km2 over the 13-year analysis period results from a postclassification change detection. Yearly cumulative spatiotemporal analysis shows lower water extents in the drought seasons of 2000-2001, 2004-2005, and 2011-2012. Multiple wetlands indicate a positive trend toward a larger yearly maximum area, but a negative trend toward shorter flooding duration. Such a negative trend is observed particularly for natural wetlands. The temporal behavior of five selected case studies demonstrates that monthly negative anomalies of watercovered areas coincide with the occurrence of drought seasons. The successful application of remote sensing time series as a tool to monitor wetlands in semiarid regions is presented, and the potential of novel early warning indicators of drought from remote sensing is demonstrated.

BACKGROUND:Burkina Faso is endemic with soil-transmitted helminth infections. Over a decade of preventive chemotherapy has been implemented through annual lymphatic filariasis (LF) mass drug administration (MDA) for population aged five years and over, biennial treatment of school age children with albendazole together with schistosomiasis MDA and biannual treatment of pre-school age children through Child Health Days. Assessments were conducted to evaluate the current situation and to determine the treatment strategy for the future. METHODOLOGY/PRINCIPAL FINDINGS:A cross-sectional assessment was conducted in 22 sentinel sites across the country in 2013. In total, 3,514 school age children (1,748 boys and 1,766 girls) were examined by the Kato-Katz method. Overall, soil-transmitted helminth prevalence was 1.3% (95% CI: 1.0-1.8%) in children examined. Hookworm was the main species detected, with prevalence of 1.2% (95% CI: 0.9-1.6%) and mean egg counts of 2.1 epg (95% CI: 0-4.2 epg). Among regions, the Centre Ouest region had the highest hookworm prevalence of 3.4% (95% CI: 1.9-6.1%) and mean egg counts of 14.9 epg (95% CI: 3.3-26.6 epg). A separate assessment was conducted in the Centre Nord region in 2014 using community-based cluster survey design during an LF transmission assessment survey (TAS). In this assessment, 351 children aged 6-7 years and 345 children aged 10-14 years were examined, with two cases (0.6% (95% CI: 0.2-2.1%)) and seven cases (2.0% (95% CI: 1.0-4.1%)) of hookworm infection was identified respectively. Gene loci are found in nuclear subcompartments that are related to their expression status. For instance, silent genes are often localized to heterochromatin and the nuclear periphery, whereas active genes tend to be found in the nuclear center. Evidence also suggests that chromosomes may be specifically positioned within the nucleus; however, the nature of this organization and how it is achieved are not yet fully understood. To examine whether gene regulation is related to a discernible pattern of genomic organization, we analyzed the linear arrangement of co-regulated genes along chromosomes and determined the organization of chromosomes during the differentiation of a hematopoietic progenitor to erythroid and neutrophil cell types. Our analysis reveals that there is a significant tendency for co-regulated genes to be proximal, which is related to the association of homologous chromosomes and the spatial juxtaposition of lineage-specific gene domains. We suggest that proximity in the form of chromosomal gene distribution and homolog association may be the basis for organizing the genome for coordinate gene regulation during cellular differentiation.
