Context: Adaptations in glenohumeral joint laxity and range of motion (ROM) are prevalent in competitive swimmers. Increased glenohumeral laxity in swimmers has been found to exist in multiple directions. However, it is unclear if swimmers with multidirectional laxity (MDL) possess altered glenohumeral ROM compared with swimmers without MDL. The purpose of this study was to compare the glenohumeral ROM characteristics of external rotation (ER), internal rotation (IR), total arc of motion (ER + IR), and total arc of motion ratio (ER/IR) between swimmers with MDL and without MDL. Our secondary objective was to investigate the effect of MDL on self-reported pain and function. Design: Observational study. Methods: Twenty-nine NCAA Division I swimmers (females: 15 and males: 14; age 19.5 [1.2] y; body mass index 23.9 [2.0] km/m²) participated in a preseason physical assessment including measures of glenohumeral ROM and joint laxity. These measures were used to determine the presence or absence of MDL for both shoulders of each participant. Glenohumeral ROM characteristics were compared between shoulders with MDL and shoulders without MDL with independent t tests. Self-reported pain and function scores were recorded biweekly across the season with a modified Kerlan-Jobe Orthopedic Clinic questionnaire. The seasonal average Kerlan-Jobe Orthopedic Clinic questionnaire scores were compared between swimmers with MDL and swimmers without MDL with analysis of variance, with factors of sex and MDL status. Results: Shoulders with MDL possessed increased glenohumeral IR (P < .001; effect size: 0.65) and total arc of motion (P < .004; effect size: 0.45) compared with shoulders without MDL. There were no differences in ER. There was no difference in self-reported pain and function between groups. Conclusions: Shoulders with MDL possess increased glenohumeral IR and total arc of motion compared with shoulders without MDL. Foreign bodies fully embedded in soft tissues present a unique challenge to surgeons attempting excision. Small fragments can be nonpalpable, and many prove difficult to visualize intra-operatively by means of classic radiologic techniques. A 35-year-old active duty soldier presented requesting excision of ballistic fragment embedded in his chest wall that had previously failed a previous attempt at removal. The metallic foreign body was successfully localized intra-operatively using a handheld magnetometer probe and removed without complication. This case demonstrates the utility and cost-effectiveness of the handheld magnetometry for intra-operative localization of metallic foreign bodies. Metallic foreign bodies may be localized intra-operatively using inexpensive and reusable equipment that does not require radiation or bulky radiographic imaging equipment. [ABSTRACT FROM AUTHOR]

The higher education literature continues to highlight difficulties with efforts to implement and sustain institutional effectiveness activities. This article describes an implementation effectiveness model from the field of organizational management and applies it to the implementation of institutional effectiveness initiatives in higher education. As a predictive or evaluative tool regarding implementation success, the model can assist administrators, faculty and staff in developing targeted approaches to institutional effectiveness work within any institution of higher education. The authors share their experiences at the University of New England, illustrating the importance of key variables in the model. (Contains 1 figure.)

BACKGROUND: The Military Health System (MHS) uses a readiness program that identifies the knowledge, skills, and abilities (KSAs) necessary for surgeons to provide combat casualty care. Operative productivity is assigned an objective score based on case type and complexity and totaled to assess overall readiness. As of 2019, only 10.1% of surgeons met goal readiness threshold. At one tertiary military treatment facility (MTF), leadership has taken an aggressive approach toward increasing readiness by forming military training agreements (MTAs) and allowing Off Duty Employment (ODE). We sought to quantify the efficacy of this approach. STUDY DESIGN: Operative logs from 2021 were obtained from surgeons assigned to the MTF. Operations were assigned CPT codes and processed through the KSA calculator (Deloitte; London, UK). Each surgeon was then surveyed to identify time away from clinical duties for deployment or military training. RESULTS: Nine surgeons were present in 2021 and spent an average of 10.1 weeks (19.5%) abroad. Surgeons performed 2,348 operations (Average [Avg] 261 ± 95) including 1,575 (Avg 175; 67.1%) at the MTF, 606 (Avg 67.3; 25.8%) at MTAs, and 167 (Avg 18.6, 7.1%) during ODE. Adding MTA and ODE caseloads increased KSA scores by 56% (17,765 ± 7,889 vs 11,391 ± 8,355). Using the MHS threshold of 14,000, 3 of 9 (33.3%) surgeons met the readiness threshold from MTF productivity alone. Including all operations, 7 of 9 (77.8%) surgeons met threshold. CONCLUSIONS: Increased use of MTAs and ODE significantly augments average caseloads. These operations provide considerable benefit and result in surgeon readiness far exceeding the MHS average. Organisms have been shifting their timing of life history events (phenology) in response to changes in the emergence of resources induced by climate change. Yet understanding these patterns at large scales and across long time series is often challenging. Here we used the US weather surveillance radar network to collect data on the timing of communal swallow and martin roosts and evaluate the scale of phenological shifts and its potential association with temperature. The discrete morning departures of these aggregated aerial insectivores from ground‐based roosting locations are detected by radars around sunrise. For the first time, we applied a machine learning algorithm to automatically detect and track these large‐scale behaviors. We used 21 years of data from 12 weather surveillance radar stations in the Great Lakes region to quantify the phenology in roosting behavior of aerial insectivores at three spatial levels: local roost cluster, radar station, and across the Great Lakes region. We show that their peak roosting activity timing has advanced by 2.26 days per decade at the regional scale. Similar signals of advancement were found at the station scale, but not at the local roost cluster scale. Air temperature trends in the Great Lakes region during the active roosting period were predictive of later stages of roosting phenology trends (75% and 90% passage dates). Our study represents one of the longest‐term broad‐scale phenology examinations of avian aerial insectivore species responding to environmental change and provides a stepping stone for examining potential phenological mismatches across trophic levels at broad spatial scales. Acinetobacter baumannii is an opportunistic pathogen and an emerging global health threat. Within healthcare settings, major presentations of A. baumannii include bloodstream infections and ventilator-associated pneumonia. The increased prevalence of ventilated patients during the COVID-19 pandemic has led to a rise in secondary bacterial pneumonia caused by multidrug resistant (MDR) A. baumannii. Additionally, due to its MDR status and the lack of antimicrobial drugs in the development pipeline, the World Health Organization has designated carbapenem-resistant A. baumannii to be its priority critical pathogen for the development of novel therapeutics. To better inform the design of new treatment options, a comprehensive understanding of how the host contains A. baumannii infection is required. Here, we investigate the innate immune response to A. baumannii by assessing the impact of infection on host gene expression using NanoString technology. The transcriptional profile observed in the A. baumannii infected host is characteristic of Gram-negative bacteremia and reveals expression patterns consistent with the induction of nutritional immunity, a process by which the host exploits the availability of essential nutrient metals to curtail bacterial proliferation. The gene encoding for lipocalin-2 (Lcn2), a siderophore sequestering protein, was the most highly upregulated during A. baumannii bacteremia, of the targets assessed, and corresponds to robust LCN2 expression in tissues. Lcn2-/- mice exhibited distinct organ-specific gene expression changes including increased transcription of genes involved in metal sequestration, such as S100A8 and S100A9, suggesting a potential compensatory mechanism to perturbed metal homeostasis. In vitro, LCN2 inhibits the iron-dependent growth of A. baumannii and induces iron-regulated gene expression. To elucidate the role of LCN2 in infection, WT and Lcn2-/- mice were infected with A. baumannii using both bacteremia and pneumonia models. LCN2 was not required to control bacterial growth during bacteremia but was protective against mortality. In contrast, during pneumonia Lcn2-/- mice had increased bacterial burdens in all organs evaluated, suggesting that LCN2 plays an important role in inhibiting the survival and dissemination of A. baumannii. The control of A. baumannii infection by LCN2 is likely multifactorial, and our results suggest that impairment of iron acquisition by the pathogen is a contributing factor. Modulation of LCN2 expression or modifying the structure of LCN2 to expand upon its ability to sequester siderophores may thus represent feasible avenues for therapeutic development against this pathogen. The mechanisms employed by A. baumannii to cause disease and the host tactics exercised to constrain infection are not fully understood. Here, we further characterize the innate immune response to A. baumannii infection. We identify nutritional immunity, a process where the availability of nutrient metals is exploited to restrain bacterial growth, as being induced during infection. The gene encoding for lipocalin-2 (Lcn2), a protein that can impede iron uptake by bacteria, is highly upregulated in infected mice, and corresponds to robust LCN2 detection in the tissues. We find that LCN2 is crucial to reducing mortality from A. baumannii bacteremia and inhibits dissemination of the pathogen during pneumonia. In wild-type and Lcn2-deficient mice, broader transcriptional profiling reveals expression patterns consistent with the known response to Gram-negative bacteremia. Although the role of LCN2 in infection is likely multifactorial, we find its antimicrobial effects are at least partly exerted by impairing iron acquisition by A. baumannii.
