Introduction: Low socioeconomic status (SES) is identified as a pivotal risk factor for mental health. Objective socioeconomic status (OSS) is measured by tangible resources, whereas subjective social status (SSS) reflects an individual's perception of their OSS. A paucity of literature exists that elucidates the specific psychosocial mechanisms or pathways linking OSS with mental illness via SSS. This research aimed to explore the mediating role of SSS in the OSS-mental health association, utilizing data from the Iranian Mental Health Survey (IranMHS). Methods: This study is a secondary analysis of data from IranMHS, a national survey conducted in 2011 through face-to-face interviews with 7,886 Iranian adults aged 15-64. As part of the original survey, the General Health Questionnaire-28 (GHQ-28) was randomly administered to half of these participants. We focused our analysis on data from 3,759 participants who completed all items of the GHQ-28. OSS was evaluated using education, occupation, wealth, and a combined OSS index, while SSS was measured using the MacArthur Scale. We examined how SSS mediates the associations between OSS factors and GHQ scores, including its four dimensions—somatic symptoms, anxiety and insomnia, social dysfunction, and severe depression— while adjusting for demographic variables and conducting gender-stratified analyses. Results: Among the 3759 respondents, 2157 (57.4%) were women, 2110 (56.1%) lived in urban areas, with an average age of 33.1 years (SD=12.1). SSS mediated the associations between wealth and the overall GHQ score (mediation percentage: 92.3%), education (75.4%), OSS index (66.7%), and occupation (34.0%) on the GHQ score. The most significant mediation effects were observed for wealth on the somatic symptoms, social dysfunction, and depression dimensions, with SSS accounting for more than 80% of these associations. Conclusion: The findings indicate a significant mediating role of SSS in the relationship between OSS and mental health. Enhancing our comprehension of the social determinants that moderate the relationship between objective and subjective socioeconomic status may contribute to a more nuanced understanding of the impact of SES on mental health outcomes. Background: Exposure to heavy metals in coffee house air poses notable occupational health risks. This study aimed to quantify heavy metal and particulate pollution levels in Tehran's traditional coffee houses. Materials and Methods: In this study of 57 Tehran coffee houses, samples were collected using personal sampling pumps and 0.8 µm cellulose ester filters following National Institute for Occupational Safety & Health (NIOSH-7300) guidelines. Heavy metal levels were determined using ICP-AES analysis. Results: The results showed that iron and magnesium metals had the lowest concentration of metals in the air within coffee houses, with an average of 10-8 µg/m³, whereas manganese metals had the highest concentration, with an average of 4.507 10-5 µg/m³. Two of the 57 locations under study had greater Threshold Limit Values (TLVs) than the others, whereas other locations had lower TLV mixtures, and these two locations had mixed TLVs of 1.315 and 1.282, respectively. 96.49 percent of the 57 measured locations had a mixed TLV below the standard and 3.51 percent above it. According to the findings of the current study, there are eight heavy metals present in Traditional coffee houses air, ranging in concentration from more to less of Fe, Mg > Pb > Ag > Cu > Ca >Na >Mn. Conclusions: The health of coffee house employees may suffer adverse effects from chronic exposure to these elements. To protect the health of Traditional Coffee Houses employees, it is essential to incorporate engineering controls for indoor air quality, monitor ventilation systems, and ensure compliance with safety regulations. Epidemiological studies on air pollution, diabetes, and hypertension conflict. This study examined air pollution, diabetes, and hypertension in adults in 11 metropolitan areas of Iran (2012-2016). Local environment departments and the Tehran Air Quality Control Company provided air quality data. The VIZIT website and Stepwise Approach to Chronic Disease Risk Factor Surveillance study delivered chronic disease data. Multiple logistic regression and generalized estimating equations evaluated air pollution-related diabetes and hypertension. In Isfahan, Ahvaz, and Tehran, PM2.5 was linked to diabetes. In all cities except Urmia, Yasuj, and Yazd, PM2.5 was statistically related to hypertension. O3 was connected to hypertension in Ahvaz, Tehran, and Shiraz, whereas NO2 was not. BMI and gender predict hypertension and diabetes. Diabetes, SBP, and total cholesterol were correlated. Iran's largest cities' poor air quality may promote diabetes and hypertension. Introduction: Pediatric obsessive-compulsive disorder (OCD), attention-deficit hyperactivity disorder (ADHD), and Tourette syndrome (TS) are often concurrent. This study explores the temperament profile of complex OCD phenotypes. Methods: A clinical registry recorded demographic data, psychiatric diagnoses, and temperament traits, including novelty seeking (exploratory behaviors), harm avoidance (fear of uncertainty), reward dependence (sentimentality), and persistence (perseverance). Temperament data were accrued from the Junior Temperament and Character Inventory (JTCI). Participants were divided into (1) OCD only; (2) OCD+ADHD or TS; and (3) OCD+ADHD+TS to compare temperament. Results: Participants include 126 youths with OCD (61.9% male, 88.9% white) between the ages 6 and 18 years (12.7 ± 3.1). Among the three groups, the complex neurodevelopmental disorder group OCD+ADHD+TS expresses the highest novelty seeking and lowest persistence. Harm avoidance is increased in all groups compared to reference controls, irrespective of concurrent ADHD or TS. For the OCD+ADHD+TS group, contamination and washing symptoms have higher novelty seeking (p < 0.01), while counting and ordering have lower novelty seeking (p < 0.05). Harm avoidance is increased with aggressive, somatic, and checking symptoms in OCD only (p < 0.01), while persistence is increased with repeating and counting symptoms in the comorbid groups (OCD+ADHD or TS, OCD+ADHD+TS). Discussion/Conclusion: The complex subtype, OCD+ADHD+TS, is associated with high novelty seeking and low persistence, while high harm avoidance is linked to pediatric OCD irrespective of ADHD or TS co-occurrence. Introduction: Neonatal hyperbilirubinemia is one of the most important factors affecting the auditory system and can cause sensorineural hearing loss. This study investigated the relationship between behavioral hearing thresholds in children with a history of jaundice and the maximum level of bilirubin concentration in the blood. Materials and Methods: This study was performed on 18 children with a mean age of 5.6 years and with a history of neonatal hyperbilirubinemia. Behavioral hearing thresholds, transient evoked emissions and brainstem evoked responses were evaluated in all children. Results: Six children (33.3 %) had normal hearing thresholds and the remaining (66.7 %) had some degree of hearing loss. There was no significant relationship (r= -0.28, P= 0.09) between the mean total bilirubin levels and behavioral hearing thresholds in all samples. A transient evoked emission was seen only in children with normal hearing thresholds however in eight cases brainstem evoked responses had not detected. Conclusion: Increased blood levels of bilirubin at the neonatal period were potentially one of the causes of hearing loss. There was a lack of a direct relationship between neonatal bilirubin levels and the average hearing thresholds which emphasizes on the necessity of monitoring the various amounts of bilirubin levels.

Neglecting possible growth heterogeneity and focusing only on the overall pattern of prostate cancer mortality rates can result in misunderstandings and incorrect conclusions about the growth process of the outcome. The main goal of this study was to capture the heterogeneity of prostate cancer mortality rates among countries from 1990 to 2019. To accomplish this aim, we performed the Bayesian latent Growth Mixture Model (GMM). In this longitudinal study, Prostate cancer mortality rates data from 1990 to 2019, as well as the Human Development Index (HDI), the Gross National Income (GNI), and the Life Expectancy at Birth (LEB), were obtained from WHO and UNDP platforms. The Bayesian GMM was used to discover homogeneous subgroups and estimate the pattern of prostate cancer mortality rates in each subgroup. The HDI, GNI and life expectancy effects were estimated using a Bayesian conditional Latent Growth Model (LGM). Globally, the intercept and the slope of the Bayesian LGM were equal to 8.77 and 0.19, respectively. The Bayesian GMM classified the 109 countries into four groups, which had significant positive growth patterns with different slopes except for the first class. The effect of HDI was not significant on the overall prostate cancer death rates, but GNI and LEB had a significantly positive effect on the model's intercepts and a significantly negative effect on the slope. Although the prostate cancer mortality rate increased globally, it has four distinct latent subgroups with various patterns. Additionally, the effects of HDI, GNI, and LEB on prostate cancer mortality rates varied significantly among the four subgroups, indicating a need for further investigation to identify causal factors. Cornelia de Lange syndrome (CdLS) is a spectrum disorder due to variants in genes of the cohesin protein complex. The following abstracts are from the Cornelia de Lange Syndrome Scientific and Educational Symposium held virtually in October 2020. Aspects of behavior, including autistic features, impulsivity, adaptive skills, executive function, and anxiety are described. Applied behavioral analysis is a promising approach for autism, and an N‐acetylcysteine trial is proposed. Children below 6 years with CdLS have an increased number of and further travel to medical providers, with insurance type comprising a significant barrier. Speech, language, and feeding abilities fall significantly below expectations for age in CdLS. Augmentative alternative communication can yield potential barriers as well as interesting benefits. Developmentally, studies in animal models further elucidate the mechanisms and roles of cohesin: link with mediator transcriptional complex; facilitation of enhancer‐promoter communication; regulation of gene expression; allocation of cells to germ layers; and repair of spontaneous DNA damage in placental cells. Genome and RNA sequencing can help identify the molecular cause in the 20% of individuals with suspected CdLS and negative testing. Abstract Background Appropriate supervision, along with availability of an effective system for monitoring and evaluation, is a crucial requirement to guarantee sufficient coverage and quality of malaria vector control procedures. This study evaluated the efficacy of self-assessment practice as a possible innovative method towards achieving high coverage and excellent quality of larviciding operation in Iran. Methods The research was conducted on the randomly selected rural health centre of Kanmbel Soliman with 10 staff and 30 villages, in three main steps: (i) assessment of effectiveness of larviciding operations in the study areas before intervention through external assessment by a research team; (ii) self-assessment of larviciding operations (intervention) by staff every quarter for three rounds; and, (iii) determining the effectiveness of applying self-assessment of larviciding operations in the study areas. Two toolkits were used for self-assessment and external evaluation. The impact of self-assessment of larviciding operations was measured by two indicators: percentage of missed breeding habitats and cleaned breeding habitats among randomly selected breeding sites. Moreover, the correlation coefficients were measured between self-assessment measures and scores from external evaluation. The correlation coefficient and Mann Whitney test were used to analyse data. Results Following the utilization of self-assessment, the percentage of missed breeding habitats decreased significantly from 14.23% to 1.91% (P Before intervention, the results of self-assessment practice were compatible with external evaluation in 76.3% of 139 reviewed reports of self-assessment. After intervention, the findings of self-assessment and external evaluation were similar in the vast majority of reviewed reports (95%). Conclusion The self-assessment tool seems to be valid and reliable in improving effectiveness of larviciding operations. Furthermore, the result of self-assessment is more compatible with external evaluation results if it would be applied frequently. Therefore, it can be used as an alternative assessment technique in the evaluation of larviciding operations in addition to traditional assessment methods.

A novel type of turbo coded modulation scheme, called multilevel turbo coded-continuous phase frequency shift keying (MLTC-CPFSK), is designed to improve the overall bit error rate (BER) and bandwidth efficiency. Then, this scheme is combined with a new double decision feedback equalizer (DDFE) to remove the interference and to enhance BER performance for the intersymbol interference (ISI) channels. The entire communication scheme is called multilevel turbo equalization-continuous phase frequency shift keying (MLTEQ-CPFSK). In these schemes, parallel input data sequences are encoded using the multilevel scheme and mapped to CPFSK signals to obtain a powerful code with phase continuity over the air. The performances of both MLTC-CPFSK and MLTEQ-CPFSK systems were simulated over nonfrequency and frequency-selective channels, respectively. The superiority of the two level turbo codes with 4CPFSK modulation is shown against the trellis-coded 4CPFSK, multilevel convolutional coded 4CPFSK, and TTCM schemes. Finally, the bit error rate curve of MLTEQ-CPFSK system over Proakis B channel is depicted and ISI cancellation performance of DDFE equalizer is shown against linear and decision feedback equalizers

Background: Musculoskeletal disorder (MSD) is one of the important problems concerning the staffs' health and productivity in the workplace. Nutritional status and consumption of some foods are also among the determining factors of MSD. So, this study aimed to evaluate the correlation of diet and consumed food groups with MSD. Methods: This cross-sectional study was conducted on 100 office workers. The participants' anthropometric parameters and their dietary information were collected using a semi-quantitative food frequency questionnaire. The findings were categorized into nine levels. The total scores were calculated for all the items per food group and per person. Nordic musculoskeletal questionnaire was also administered to evaluate the MSD symptoms in nine parts of body. Results: The score of consumed food groups was compared between individuals "with pain" and "without pain" in nine parts of the body. The scores of fruit intake in individuals "with pain" and "without pain" were 2.94 ± 1.27 vs. 3.29 ± 1.16 and 2.81 ± 1.10 vs. 3.49 ± 1.38 in terms of neck and wrists, respectively. The difference between the two groups were significant (P < 0.05). Furthermore, the participants with pain in the neck consumed significantly lower amounts of cereals and nuts (P = 0.03, 0.04). In the case of the shoulder pain, consuming legumes and nuts in the "without pain" group was higher than the group of participants who had pain (P = 0.01, P = 0.03). Fat intake was higher in the patients who had pain in their hips (P = 0.02). Conclusion: Less pain was reported in the musculoskeletal system by higher consumption of fruits, nuts, and legumes. Background: Female breast cancer is known as one of the top five cancers in terms of mortality. Regarding contradictory reports about the mortality trend of this cancer and its association with the socio-economic status of the world countries, we aimed at assessing the global trend of female breast cancer mortality rate and investigate the relationship between its mortality rate and development status. Methods: The breast cancer AgeStandardized Mortality Rate (ASMR) per 100,000andHumanDevelopment Index (HDI) for 179 world countries were extracted, respectively from the Global Burden of Disease (GBD) 2017 study and the United Nations Development Programme (UNDP) database, for the period 1990 to 2017. The marginal modeling methodology was employed to analyze the global trend of ASMR and examine the relationship between ASMR and HDI. Results: The results showed a slightly constant curve for the global trend of breast cancer ASMR from 1990 to 2017 (around 17 per 100,000). Moreover, it was indicated that the ASMR is strongly related to development status. While countries with higher levels of HDI have experienced a declining trend of breast cancer mortality rate, countries with lower HDI levels experienced an ascending trend at this period. Conclusions: In general, the findings showed that mortality due to breast cancer is still a major health problem in total world countries. Hence, more efforts should be made to screen the patients in the early stages of the disease and promote the level of care, especially in countries with lower levels of economic development. In this paper, we propose regularized block diagonalization (RBD) precoding using artificial noise (AN) for physical layer security in downlink multi-user MIMO wireless networks. We derive secrecy sum rate and asymptotic secrecy sum rate for the proposed scheme. The optimum power allocation for legitimate users and the AN signal are derived in closed form for optimal asymptotic secrecy sum rate. Our analysis shows that to achieve best performance, it is more efficient to degrade the performance of the eavesdropper than improving legitimate users’ rates. We also study the impact of error in channel estimation on the system and derive closed-form SINR and secrecy sum-rate expressions for this case. Simulations show that the secrecy sum rate of RBD precoding outperforms the secrecy sum rate of regularized channel inversion by 0.5 bits/s/Hz. We also show that in the presence of channel estimation error, our approximate closed forms for secrecy sum rate are very closed to the actual secrecy sum rate over a wide range of SNR and channel estimation error values. Road Traffic Accidents (RTA) are a major worldwide public health problem. The aim of this study was to use the growth mixture model for clustering countries on the basis of the mortality rate patterns of RTAs from 2007 to 2013. We obtained the data on RTA death rates from World Health Organization reports and Human Development Index (HDI) of United Nations Development Programme reports for the years 2007, 2010 and 2013. Simple Latent Growth Models (LGM) in 181 countries were applied to estimate overall RTA mortality rate growth trajectories and the latent growth mixture modeling utilized to cluster them. According to non-linear LGM, the overall mortality rate of RTAs showed a decrease from 2007 to 2010 followed by an increase from 2010 to 2013. The HDI covariate had a significant negative and positive effect on intercept and slope of the LGM, respectively. The extracted mixture model appeared to have seven classes with different trends in RTA mortality rates. The worldwide countries were clustered into seven classes. Further studies on each of the seven classes are suggested to provide recommendations for reducing the mortality rate of the RTAs. Objective: National surveys revealed a high prevalence of psychiatric disorders in Iran. Province-level estimates are needed to manage the resources and focus on preventive efforts more efficiently. The objective of this study was to provide province-level estimates of psychiatric disorders. Method: In this study, Iranian Mental Health Survey (IranMHS) data (n = 7886) was used to produce province-level prevalence estimates of any psychiatric disorders among 15-64 year old males and females. Psychiatric disorders were diagnosed based on structured diagnostic interview of the Persian version of Composite International Diagnostic Interview (CIDI, version, 2.1). The Hierarchical Bayesian (HB) random effect model was used to calculate the estimates. The mental health status of half of the participants was also measured using a 28-item general health questionnaire (GHQ). Results: A wide variation in the prevalence of psychiatric disorders was found among 31 provinces of Iran. The direct estimates ranged from 3.6% to 62.6%, while the HB estimates ranged from 12.6% to 36.5%. The provincial prevalence among men ranged from 11.9% to 34.5%, while it ranged from 18.4% to 38.8% among women. The Pearson correlation coefficient between HB estimates and GHQ scores was 0.73. Conclusion: The Bayesian small area estimation provides estimation with improved precision at local levels. Detecting high-priority communities with small-area approach could lead to a better distribution of limited facilities and more effective mental health interventions. [ABSTRACT FROM AUTHOR]

We derive the asymptotic ergodic secrecy sum rate (ESSR) of block diagonalization precoding for physical-layer security in downlink multi-user wireless networks that use artificial noise. First, we derive the ESSR of the system. Then, we derive the power allocation and the asymptotic ESSR as the number of antennas increases. After, we derive an approximate expression for the optimum ratio of the number of transmit antennas to the number of receive antennas ($\Psi $). We then introduce the user dropping factor (UDF) as a means to derive the optimum number of users in the system under different scenarios and then derive the asymptotic optimum UDF, which gives the optimum number of scheduled users. We also study the effect of imperfect channel state information and derive a closed-form ESSR. Simulation results show that these precoders outperform channel inversion (CI) and regularized CI schemes in terms of the ESSR at all SNRs. Temporal distribution of earthquakes with M > 6 in the Dasht-e-Bayaz region, eastern Iran has been investigated using time-dependent models. Based on these types of models, it is assumed that the times between consecutive large earthquakes follow a certain statistical distribution. For this purpose, four time-dependent inter-event distributions including the Weibull, Gamma, Lognormal, and the Brownian Passage Time (BPT) are used in this study and the associated parameters are estimated using the method of maximum likelihood estimation. The suitable distribution is selected based on logarithm likelihood function and Bayesian Information Criterion. The probability of the occurrence of the next large earthquake during a specified interval of time was calculated for each model. Then, the concept of conditional probability has been applied to forecast the next major ( M > 6) earthquake in the site of our interest. The emphasis is on statistical methods which attempt to quantify the probability of an earthquake occurring within a specified time, space, and magnitude windows.
