Abstract

Rationale: Hemodynamically significant patent ductus arteriosus (hsPDA) in premature infants has been associated with bronchopulmonary dysplasia (BPD) and pulmonary hypertension (PH). However, these associations remain incompletely understood. Objectives: To assess the associations between hsPDA duration and clinical outcomes, PH, and phenotypic differences on lung magnetic resonance imaging (MRI). Methods: In this retrospective cohort study, we identified all infants with BPD at <32 weeks' gestation who also underwent research lung MRI at <48 weeks' postmenstrual age (PMA) from 2014 to 2022. Clinical echocardiograms were reviewed for hsPDA and categorized as no hsPDA, hsPDA 1–60 days, and hsPDA >60 days. Outcome variables included BPD severity, PH at 36 weeks' PMA, PH after 36 weeks' PMA in the absence of shunt (PH–pulmonary vascular disease [PVD]), tracheostomy or death, and lung phenotype by MRI via modified Ochiai score, indexed total lung volume, and whole-lung hyperdensity. Logistic regression and ANOVA were used. Measurements and Main Results: In total, 133 infants born at 26.2 ± 1.9 weeks, weighing 776 ± 276 g, were reviewed (47 with no hsPDA, 44 with hsPDA 1–60 days, and 42 with hsPDA >60 d). hsPDA duration > 60 days was associated with BPD severity (P < 0.01), PH at 36 weeks' PMA (adjusted odds ratio [aOR], 9.7 [95% confidence interval (CI), 3.3–28.4]), PH–PVD (aOR, 6.5 [95% CI, 2.3–18.3]), and tracheostomy or death (aOR, 3.0 [95% CI, 1.0–8.8]). Duration of hsPDA > 60 days was associated with higher Ochiai score (P = 0.03) and indexed total lung volume (P = 0.01) but not whole-lung hyperdensity (P = 0.91). Conclusions: In infants with moderate or severe BPD, prolonged exposure to hsPDA is associated with BPD severity, PH–PVD, and increased parenchymal lung disease by MRI. [ABSTRACT FROM AUTHOR]

Abstract

White Oyster Mushroom (Pleurotus florida) is one of the domesticated mushrooms in the Philippines and known to grow in various biodegradable wastes. Low Density Polyethelene (LDPE) plastic is one of the most common non-biodegradable wastes in the environment which causes pollution. At present, biodegradation process is one of the ways to degrade plastics because of its nonpolluting mechanism and cost-effectiveness. Thus, this study aimed to determine if P. florida could grow in LDPE plastic and facilitate its decomposition. Fourteen treatments were used in the study, laid out in Complete Randomized Design, and replicated thrice. The first four (4) treatments were composed of substrates with no LDPE plastics (rice straw, sawdust and cocopeat) while the rests were substrates with certain percentage of LDPE plastics. Observation was done for eight (8) weeks in the PUP Lopez Mushroom Labouratory and the data obtained was analyzed using ANOVA. Result shows that slight decomposition happened in LDPE plastic mixed with lingo-cellulosic wastes. Small spots or dark holes appear in the surface of LDPE plastics observed under microscope which is a sign of decomposition. In terms of mycelial ramification, fruiting bags with LDPE plastics were first to occupy the fruiting bag. There was also a decreased weight of the fruiting bags after 8 weeks. In terms of yield, substrates without LDPE plastics grew mushrooms with the highest weight. However, yield in sawdust and cocopeat substrates were not significantly different with the yield of mushrooms grown in substrates with LDPE plastics. [ABSTRACT FROM AUTHOR]

Abstract

The study describes the manner in which participation in SPU Manila’s Glee Club is instrumental in the lives of its members. Through a focus group discussion (FGD) involving eight members, the results revealed that the experiences of the students affect the physical, financial, political, and social dimensions of their lives, enabling them to achieve personal goals related to exposure, receive assistance, and develop self-confidence. These benefits result in the satisfaction of most of the members and a desire to continue with the group, provided participation does not get in the way of academic priorities. The study also revealed a desire among students to assert instrumentality in the chorale group as they sought more regular evaluations and frustrations over not being heard by their moderator/s.

19. Favourable Tolerability and Drug Survival of Tioguanine Versus Methotrexate After Failure of Conventional Thiopurines in Crohn's Disease.

Abstract

Background and Aims Both methotrexate and tioguanine can be considered as treatment options in patients with Crohn's disease after failure of conventional thiopurines. This study aimed to compare tolerability and drug survival of methotrexate and tioguanine therapy after failure of conventional thiopurines in patients with Crohn's disease. Methods We conducted a retrospective, multicentre study, including patients with Crohn's disease initiating monotherapy methotrexate or tioguanine after failure [all causes] of conventional thiopurines. Follow-up duration was 104 weeks or until treatment discontinuation. The primary outcome was cumulative therapy discontinuation incidence due to adverse events. Secondary outcomes included total number of [serious] adverse events, and ongoing monotherapy. Results In total, 219 patients starting either methotrexate [ n  = 105] or tioguanine [ n  = 114] were included. In all 65 [29.7%] patients (methotrexate 43.8% [46/105 people], tioguanine 16.7% [19/114 people], p  <0.001) discontinued their treatment due to adverse events during follow-up. Median time until discontinuation due to adverse events was 16 weeks (interquartile range [IQR] 7-38, p  = 0.812). Serious adverse events were not significantly different. Patients treated with methotrexate experienced adverse events more often [methotrexate 83%, tioguanine 46%, p  <0.001]. Total monotherapy drug survival after 104 weeks was 22% for methotrexate and 46% for tioguanine [ p  <0.001]. Conclusions We observed a higher cumulative discontinuation incidence due to adverse events for methotrexate [44%] compared with tioguanine [17%] in Crohn's disease patients after failure of conventional thiopurines. The total adverse events incidence during methotrexate use was higher, whereas serious adverse events incidence was similar. These favourable results for tioguanine treatment may guide the selection of immunosuppressive therapy after failure of conventional thiopurines. [ABSTRACT FROM AUTHOR]

Abstract

Surfactants in skin cleansers interact with the skin in several manners. In addition to the desired benefit of providing skin hygiene, surfactants also extract skin components during cleansing and remain in the stratum corneum (SC) after rinsing. These side effects disrupt SC structure and degrade its barrier properties. Recent applications of vibrational spectroscopy and two-photon microscopy in skin research have provided molecular-level information to facilitate our understanding of the interaction between skin and surfactant. In the arena of commercial skin cleansers, technologies have been developed to produce cleansers that both cleanse and respect skin barrier. The main approach is to minimize surfactant interaction with skin through altering its solution properties. Recently, hydrophobically modified polymers (HMPs) have been introduced to create skin compatible cleansing systems. At the presence of HMP, surfactants assemble into larger, more stable structures. These structures are less likely to penetrate the skin, thereby resulting in less aggressive cleansers and the integrity of the skin barrier is maintained. In this paper, we reviewed our recent findings on surfactant and SC interactions at molecular level and provided an overview of the HM technology for developing cleansers that respect skin barrier.

Abstract

A coordinated multiple-substream unequal error-protection and error-concealment algorithm for SPIHT-coded bitstreams transmitted over lossy channels is presented. The simulation results show that the proposed multiple-substream UEP algorithm is simple, fast, and robust in hostile network conditions, and that the proposed error-concealment algorithm can achieve 2-3-dB PSNR gain over the case when error concealment is not used at high packet-loss rates.

Abstract

A study was conducted to analyze the flow associated with a flap edge, one of the three-dimensional properties of flow over a high-lift system. Structured, overset grids were utilized with an incompressible Navier-Stokes to calculate the flow over a two-element high-lift. The full-span and half-span Fowler flap configurations were then determined. Results indicated a number of flow features that significantly influence the performance of high-lift system. These include the flap-edge vortex, flow separation and spanwise lift distribution.
