Abstract

During the last years PVD coatings has been used for decorative purposes on ceramics and other substrates. Standards layers like TiN, CrN, TiCN, etc, give us the possibly of obtaining a wide range of colours, with a good mechanical, chemical properties. This paper gathers the works carried out in the Center of Advance Surface engineering of AIN during the three last years in the field of the decorative coatings. Different layer have been deposited by means of a METAPLAS 232 PVD equipment. A complete chemical and mechanical characterisation have been carried out on the different deposited layers like ultramicroindentation, scratch tests, wear resistance and GDOES.En los últimos años se ha venido estudiado la aplicabilidad de las técnicas de depósitos mediante métodos físicos en fase vapor (PVD), para la realización de recubrimientos sobre cerámicas y otros substratos con fines decorativos. Los recubrimientos estándar por PVD como el TiN, CrN, TiCN, etc, ofrecen la posibilidad de conseguir acabados de apariencia metálica, de una amplia gama de colores, y con buenas propiedades mecánicas y químicas, como resistencia a la abrasión, estabilidad térmica, o resistencia a la corrosión. En este trabajo se recogen las experiencias realizadas en el recubrimiento y caracterización de cerámicas decorativas, durante los tres últimos años, en el Centro de Ingeniería de Superficies de la Asociación de la Industria Navarra. Los distintos tipos de recubrimientos se han realizado mediante un equipo de PVD modelo METAPLAS 232, con seis evaporadores de arco eléctrico y sistema de limpieza iónica patentado AEGD. Los recubrimientos han sido caracterizados, tanto química como mecánicamente, empleando las más modernas técnicas tribológicas (ultramicrodureza, scrascth tests, resistencia al desgaste...) y espectroscópicas óptica por descarga luminiscente (GDOES). Los resultados obtenidos muestran que las técnicas de PVD son una interesante vía para la obtención de recubrimientos decorativos, de excelente resistencia tanto mecánica como química, y que los métodos de caracterización empleados son adecuados para el control de calidad industrial.

Abstract

This paper gathers the studies of the effects introduced by Nitrogen implantation on Ti and Zr. Atomic and molecular Nitrogen have been implanted on Titanium and Zirconium samples to investigate the role of these elements in the nitrides precipitation. In all cases an increase in microhardness, which in some cases was greater than 80% was found out. Moreover was observed improvements in the wear resistance and a decrease in the friction coefficient. AES and XPS spectroscopy revealed that nitride precipitation in titanium is more significant for the high atomic doses of Nitrogen implantation.En el presente trabajo se estudian los efectos introducidos por la implantación de nitrógeno atómico y molecular en Ti y Zr comercialmente puros. Se relacionan los cambios en la dureza superficial y coeficiente de fricción con los cambios en la composición química superficial. En todos los casos se ha encontrado que la implantación iónica proporciona aumentos de dureza que en algunos de los casos llega hasta el 80%. Se encontraron así mismo mejoras en la resistencia al desgaste y disminuciones del coeficiente de fricción de hasta un 50%. La caracterización AES y XPS pone de manifiesto la aparición de precipitados de nitruros, con mayor concentración para la implantación de altas dosis de nitrógeno atómico sobre titanio.

Abstract

This work gathers a comparative study of the mechanical and tribological behaviour (microhardness under load, Young modulus, elasticplastic properties, friction and wear coefficients) of TiN/CrN and TiN/AlTiN multilayers deposited by conventional industrial PVD techniques. PVD multilayers with about between 50 nm individual layer thickness were deposited on flat samples of Co-cemented WC achieving a total thickness of about 2 microns. Mechanical and Tribological tests include microhardness under load at final loads between 10 mN aand 750 mN, Young modulus calculations and friction and wear tests.El presente trabajo recoge un estudio comparativo de las propiedades mecánicas y tribológicas (microdureza, fluencia, módulo elástico, coeficientes de fricción y desgaste), de multicapas TiN/AlTiN y TiN/CrN depositadas mediante técnicas industriales de PVD. Las capas, de espesores comprendidos entre 25nm y 100nm, se depositaron secuencialmente sobre substratos de WC, hasta obtener un espesor total de unas 2 micras. El estudio mecánico y tribológico comparativo incluyó ensayos de microdureza a cargas entre 10 mN y 750 mN, módulo elástico, comportamiento frente a fluencia y coeficientes de fricción y desgaste.

25. How to reduce the complication rate of multiple burr holes surgery in moyamoya angiopathy.

Abstract

Purpose: This study is aimed at analyzing clinical outcome, absence of stroke recurrence, revascularization, and complications and long-term follow-up in the surgical treatment of moyamoya angiopathy (MMA) using the multiple burr holes (MBH) technique with dura opening and arachnoid preservation as a single procedure. To the best of our knowledge, this is the first to describe an MBH technique with arachnoid preservation. Method: We retrospectively reviewed all patients operated from June 2001 to March 2021, for a symptomatic and progressive MMA operated with opening of the dura but arachnoid preservation. Clinical examinations were obtained in all patients, and radiological monitoring was performed by cerebral 3D-magnetic resonance angiography (MRA) with perfusion or single-photon emission computed tomography (SPECT) with acetazolamide. Results: In total, 21 consecutive patients (6 children and 15 adults) were included with a mean age of 7.4 years in the pediatric group and 36.9 years in the adult group. Initial presentation was permanent ischemic stroke in 15 cases, transient ischemic attack (TIA) in 5 cases, and cerebral hemorrhage in one case. The MBH with dura opening and arachnoid preservation was performed bilaterally in 9 cases (43%) and unilaterally in 12 cases (57%). One patient died due to intraoperative bilateral ischemic stroke. Of the 20 other patients, 30% demonstrated clinical stability and 70% showed partial or complete recovery. Although one patient experienced a perioperative stroke, we did not observe any pseudomeningocele or postoperative ischemic stroke (IS) recurrence in all surviving cases during the average follow-up period of 55.5 months (range: 1–195). These outcomes emphasize the importance of preoperative monitoring to ensure the effectiveness and safety of the intervention. Postoperative angiography studies showed revascularization in 96.3% of treated hemispheres (100% in the adult group vs 80% in the pediatric group). Conclusions: Our results on this small cohort suggest that the MBH technique with opening of the dura and arachnoids preservation can prevent recurrent strokes and reduce the risk of pseudomeningocele. Abstract

Background: No evidence currently exists characterising global outcomes following major cancer surgery, including esophageal cancer. Therefore, this study aimed to characterise impact of high income countries (HIC) versus low and middle income countries (LMIC) on the outcomes following esophagectomy for esophageal cancer.Method: This international multi-center prospective study across 137 hospitals in 41 countries included patients who underwent an esophagectomy for esophageal cancer, with 90-day follow-up. The main explanatory variable was country income, defined according to the World Bank Data classification. The primary outcome was 90-day postoperative mortality, and secondary outcomes were composite leaks (anastomotic leak or conduit necrosis) and major complications (Clavien-Dindo Grade III - V). Multivariable generalized estimating equation models were used to produce adjusted odds ratios (ORs) and 95% confidence intervals (CI95%).Results: Between April 2018 to December 2018, 2247 patients were included. Patients from HIC were more significantly older, with higher ASA grade, and more advanced tumors. Patients from LMIC had almost three-fold increase in 90-day mortality, compared to HIC (9.4% vs 3.7%, p < 0.001). On adjusted analysis, LMIC were independently associated with higher 90-day mortality (OR: 2.31, CI95%: 1.17-4.55, p = 0.015). However, LMIC were not independently associated with higher rates of anastomotic leaks (OR: 1.06, CI95%: 0.57-1.99, p = 0.9) or major complications (OR: 0.85, CI95%: 0.54-1.32, p = 0.5), compared to HIC.Conclusion: Resections in LMIC were independently associated with higher 90-day postoperative mortality, likely reflecting a failure to rescue of these patients following esophagectomy, despite similar composite anastomotic leaks and major complication rates to HIC. These findings warrant further research, to identify potential issues and solutions to improve global outcomes following esophagectomy for cancer.

Abstract

In a previous study, our group detected the cholecystokinin (CCK) protein in the porcine oviduct. This fact, together with the involvement of CCK in the regulation of sperm protein tyrosine phosphorylation by the modulation of HCO3− uptake (in mice and humans) suggests a role for CCK during sperm capacitation. Therefore, on the one hand, the expression of CCK receptors (CCK1R and CCK2R) on boar testes has been investigated and probed; on the other hand, boar spermatozoa (from seminal doses of 1‐day and 5‐day storage) were exposed to different concentrations of CCK (0‐control, 25 or 50 μM) in a medium supporting capacitation supplemented with 0, 5 or 25 mmol/L of HCO3− for 1 h at 38.5°C. Sperm motion (total and progressive motility), kinetic parameters, viability, acrosome status, and mitochondrial activity were determined. No differences between groups (0, 25 or 50 μM of CCK) were observed when HCO3− was absent in the media (p >.05). However, the results showed that when the media was supplemented with 5 mmol/L HCO3− in 1‐day seminal dose storage, the linearity index (LIN, %), straightness index (STR, %) and oscillation index (WOB, %) (sperm kinetics parameters) increased in the presence of CCK regardless the concentration (p <.05). Nevertheless, CCK in sperm from 5‐day storage only increased the WOB parameter in comparison to the control (p <.05). Furthermore, the average amplitude of the lateral displacement of the sperm head (ALH, μm) and curvilinear velocity (VCL, μm/s) decreased when CCK was present, depending on its concentration and sperm aging (1‐day vs. 5‐days) (p <.05). In the case of the media supporting capacitation supplemented with 25 mmol/L HCO3−, any differences were observed except for sperm viability in the 5‐day seminal doses, which increased in the 50 μM‐CCK group compared to the control (p <.05). In conclusion, these data suggest an implication of CCK protein during sperm capacitation under low bicarbonate concentration increasing the sperm linear trajectory. [ABSTRACT FROM AUTHOR]

Abstract

The association between the large breast size and improvement following breast reduction surgery suggests a potential involvement of friction in this condition. Skin conditions affecting the hair follicle have been extensively documented in the medical literature.[1] However, diagnosing such conditions can be challenging, particularly when they involve unusual locations.[2] In this letter, we present five clinical cases of a cutaneous entity that, to the best of our knowledge, has not been described previously. One patient chose to discontinue treatment and instead underwent breast reduction surgery, which resulted in complete resolution of the skin disorder. [Extracted from the article]
