Background and purpose: Gastric cancer is one of the most common cancers, with over one million new cases annually, and is the second leading cause of cancer-related deaths worldwide, after lung cancer. This disease is highly heterogeneous, with different primary sites, histological types, molecular classifications, and biological behaviors. The Cancer Genome Atlas has identified four distinct subtypes of gastric adenocarcinoma. In recent decades, various molecular studies have been conducted to understand the mechanisms underlying gastric cancer development and progression, as well as to identify new diagnostic and prognostic biomarkers. Several intracellular processes, including changes in gene expression, can contribute to the development and progression of gastric cancer. Gene expression changes are also implicated in other diseases, such as heart disease, autoimmune disorders, and diabetes. The CLU gene, an evolutionarily conserved gene, encodes the protein clusterin, which plays a role in various cellular processes, including cell-cell adhesion and programmed cell death. Clusterin has multiple roles in carcinogenesis and tumor invasion. This gene is classified as a metastasis suppressor gene, meaning it does not undergo mutations in tumors but is often downregulated in expression. The aim of the current study is to investigate the relationship between changes in CLU gene expression and the prognosis of patients with gastric cancer. Materials and methods: In this cross-sectional study, fifty tumor tissues and adjacent normal tissues were collected from gastric cancer patients. The relative gene expression of CLU was assessed using quantitative real-time reverse transcriptase-polymerase chain reaction (qRT-PCR). RNA extraction was performed using RNX-plus solution, and cDNA synthesis was carried out using the BIOFACT kit, following the Random Hexamer and oligo(dT) mixed primer method. Gene expression levels were measured using the StepOnePlus thermocycler, with GAPDH serving as the internal control gene. Additionally, a bioinformatics analysis was conducted to evaluate the expression of the CLU gene in two distinct groups of gastric cancer patients. A Kaplan-Meier survival analysis was performed to assess the impact of CLU expression levels on patient survival rates. Furthermore, a comprehensive analysis of CLU expression across multiple types of cancer was conducted using TCGA data. Results: The analysis of CLU gene expression showed that its expression level in tumor samples was 0.46 compared to adjacent normal tissues. Using data from two additional cohorts, we confirmed that CLU gene expression was consistently lower in tumor samples compared to normal samples. At all stages of gastric cancer, CLU expression was significantly reduced compared to adjacent normal tissue. Survival analysis revealed significant differences in overall survival between patients with high CLU expression and those with lower expression. Lower CLU expression was associated with reduced patient survival time. Further analysis showed that in most cancers studied, CLU expression was generally decreased. Conclusion: The expression of the CLU gene can be used as a prognostic marker in gastric cancer, and CLU gene therapy may be considered as a potential treatment option for gastric cancer.

AbstractEmbedding quality provision in physical education (PE) programmes in school settings remains a perennial concern among stakeholders worldwide. The demise of PE and sports participation in schools elevated the dialogue to comprehend the underpinning issues which erode its progress in Iraq. The study’s three aims are as follows: First, it aims to identify the potential items that define the quality aspects of PE (primary through university level) among professionals. Second, gender perceptions were gauged on the retained factors through an exploratory factor analysis (EFA). Third, perceptions of professionals working in various jobs were measured on the aspects of quality PE (QPE). Except for two statements (Items 6 and 44—low communality), professionals perceived all items to potentially indicate QPE. The items were gauged through an EFA for each subfactor separately. A t-test and analysis of variance (ANOVA) were implemented to measure the differences in gender and job positions on the subfactors of QPE. Male and female professionals reported significant differences in all the subfactors. However, the perception of QPE by job position did not show any significant differences. Further, a 2 (gender) × 3 (job positions: primary school vs. high school vs. university) MANOVA was analysed, which highlighted a significant interaction effect (Wilks’ l = .92) among the professionals; but η2 (=.038) had a medium effect. The study provided a foundation for identifying items for measuring the QPE among professionals in Iraq. Nevertheless, significant gender differences for all QPE subfactors warrant future research to comprehend the underlying issues.

The increasing use of wind resources has led to new challenges in transmission network expansion planning (TNEP). Wind curtailment due to improper expansion of transmission lines prevents the optimal use of these renewable resources. In addition, the uncertainty of these resources puts expansion decisions at risk. In this paper, risk-constrained TNEP in electricity markets in the presence of wind resources is presented with the aim of increasing the utilization of these resources by considering the wind curtailment cost and reducing expected energy not supplied (EENS). For this purpose, a risk-constrained bi-level stochastic model is used. At the upper level, transmission network expansion decisions are made by the independent system operator (ISO) with the aim of minimizing network investment cost and EENS cost. At the lower level, the market-clearing problems are solved maximizing social welfare. Using duality theory, the bi-level problem is converted into a mixed-integer linear programming (MILP) problem. The proposed framework is analyzed using Garver’s six-bus test system and the IEEE 24-bus reliability test system (RTS). The simulation results indicate that considering wind curtailment cost has increased the use of wind units and reduced the expected energy produced by fossil units, which is important from an environmental point of view. However, considering the risk, due to the uncertainty of wind resources, the energy produced by these resources has decreased.

The aim of this paper is to dynamic analyses of rectangular columns using different types of Fibre-reinforced plastic (FRP) rebars (CFRP-AFRP-GFRP) and compare them with steel rebars under impact loading. For this purpose, first, to evaluate the effect of FRP, the 25 cm column with 12mm rebars are placed under three types of impact loads in near the base, middle and the whole column. Then, to evaluate the reinforcement size effect, 12 mm rebars is replaced with 18 mm and the effect of the impact load on the whole column is evaluated. In the next step to evaluate the effect of cross sectional dimensions, the impact load across the column with the rectangular cross sectional with 35 cm dimensions will be examined. Finally, 20 models are prepared and examined by the finite element method and the results related to displacement, stress and energy are calculated. The results show that FRP reinforcements are more resistant to impact load than steel reinforcements and CFRP polymer reinforcement has performed better than other existing reinforcements. In general, it can be said that the use of Fibre-reinforced plastic reinforcement can be a measure to strengthen structures against impact load.

The purpose of this study is to explore the effective factors on the productivity of human capital via an interpretive structural modeling (ISM) in General office of Sports and Youth in Khorasan Razavi Province. The present study is a qualitative research with an analytical nature and it is also an applied one that was conducted via interpretive structural modeling. Literature review, interviews and ISM questionnaires were used for data collection. The research population included professors and experts in sports management, managers, deputies and staff of the General office of Sports and Youth in Khorasan Razavi, of which 35 people were totally selected through purposeful sampling in three stages of this study. According to the research findings, 59 effective indicators were identified under seven major dimensions playing role in improving the productivity of human capital, which were classified into five levels. Motivational factors, organizational culture and work environment, the relationship between managers and employees along with individual factors, the quality of labor force, and finally the technology factor were identified under five levels of the model. Finally, it can be concluded that managers in sports organizations should pay special attention to the technology factor and its indicators, which have been identified as the foundation of improving employees’ productivity, which then provides the ground for exploiting other dimensions at higher levels.

Sports advertising is one of the methods used by most companies to introduce their brands and products and expand their market. This study aimed to investigate the relationship between brand identity transfer and sport commitment with the mediating role of promotional and advertising tools from the perspective of sports consumers in Iran. 500 sports consumers participated in this study and completed the research questionnaire. The results of the structural equation modeling (SEM) indicate that there is a significant relationship between brand identity transfer and sport commitment and their dimensions, with the mediating role of promotional and advertising tools. Further, TV, mobile and social networks should have a priority over other promotional tools when transferring brand identity and sports commitment of consumers. Besides, contributing to the advancement of theory, this study has important practical implications for sport marketers and managers who are searching effective promotional and advertising tools to re-engage existing consumers or to acquire new ones.

Ventilator-associated pneumonia (VAP) is a prevalent nosocomial illness in mechanically ventilated patients. Hence, the aim of this study was to investigate the pattern of antibiotic resistance and biofilm formation of bacterial profiles from Endotracheal Tubes of patients hospitalized in an intensive care unit in southwest Iran. According to the standard operating method, the microbiological laboratory conducts bacteria culture and susceptibility testing on endotracheal Tube samples suspected of carrying a bacterial infection. The Clinical and laboratory standards institute (CLSI) techniques are used to determine the Antimicrobial resistance (AMR) of bacterial isolates to antibiotics using the disk diffusion method. The crystal violet staining method was used to assess the biofilm-forming potential of isolates in a 96-well microtiter plate. In total, (51%) GPBs were included in this study. The isolated GPB were coagulase-negative Staphylococcus (16%), S. aureus (14%). In total, (40%) of GNB were included in this study. The isolated GNB were Klebsiella spp. (36%), A. baumannii (22%), P. aeruginosa (35%). (32%) bacterial strains were MDR and (29%) strains were XDR. The results of biofilm formation showed (72%) were biofilm producers. VAP is a common and severe nosocomial infection in mechanically ventilated patients. Controlling biofilm formation, whether on the ET or in the oropharyngeal cavity, is thus an important technique for treating VAP. Colistin and linezolid are antibiotics that are effective against practically all resistant GNB and GPB isolates.

Translators often translate rhetoric as rumor, semantics, sermon, or eloquence. Romans called oratory and orator as rhetorica and rhetor respectively. As the beauty of an architecture is the product of the exquisiteness of its different parts, rhetoric and aesthetic aspects of poetry is also the result of the aesthetics of its structural units, including the attention to the phonetic structure and the use of techniques and methods that enhance the aesthetic effects of the vocal of speech more and more. Jalal ad-Din Muhammad Rumi (Mawlana)’s skill in expressing concepts and using language is shown in his sonnets by utilizing poetic creations and artistic processing as well as various figures of speech. The mastery of Mawlana on language has a great contribution to his artistic personality and literary creativeness. As one of the most prominent Iranian poets and mystics in Iran, Rumi has skillfully used the phonological elements of the language to musically create his own words and, as a result, has had a greater impact on the addressee. By using his knowledge of language capacities especially in the field of phonology, he has not only made music and formed pictures but also has paid special courtesy to transfer the necessary concepts to the addressee. Hence, Mawlana’s prominent and distinguishing feature in this field is his selection of appropriate formal elements in order to induce his poetic meanings in a way that his sonnets are not only regarded as a suitable tool to transfer the supreme concepts of his mystic thought, but also as a kind of musical tool to impress the aesthetic emotions of the addressee. Results of this research, which are based on a bibliographic and descriptive-analytical method, show that Rumi has used phonetic clusters at all phonological, lexical and syntactical levels appropriately.

This method reported the use of tartaric acid as a green and highly efficient catalyst for the convenient synthesis of polysubstituted quinolines and xanthenes derivatives in excellent yields under solvent-free conditions. The main advantages of this one-pot procedure are the green and economic availability of the catalyst, simple experimental and work-up procedures.

In this work, a simple and economical procedure for the synthesis of spiro-4H-pyranderivatives has been found through the three-component, one-pot condensation of isatin/acenaphthequinone, malononitrile and different reagents including 1, 3-dicrbonyl compounds, naphthol and 4-hydroxycumarin under thermal and solvent-free conditions in the presence ofFe2O3 as an efficient catalyst. The major advantages of this methodology are mild reaction conditions, eco-friendly, environmentally benign, non-toxic and inexpensive catalyst, experimentally simplicity, good yields and short reaction times.

An efficient and environmental friendly methodology for the synthesis of polysubstituted dihydro-2-oxypyrroles via a one-pot four-component domino condensation of dialkyl acetylenedicarboxylate, formaldehyde and amines (aromatic and aliphatic) at present of salicylic acid as an economical and naturally green Brønsted acid catalyst under ambient temperature with excellent yields and short reaction times is described. Green, natural, inexpensive and non-toxic catalyst, easily separated with no column chromatographic separation and highly efficient is an option for the simple synthesis of these rings.

Objective: Medical plants, as rich sources of natural antioxidants with antidiabetic effects, are used worldwide to diminish a variety of symptoms and many diseases. R. ribes L., which belongs to the family of polygonaceae, can provide symptomatic relief and assist in the prevention of the secondary complications of the diabetes. Material and Methods: 36 female adult rats were randomly divided into 6 groups of 6. Normal Control groups treated with normal saline. Positive control groups treated with hydroalcohlic extract of R. ribes root (150 mg/kg) daily by gavages for 4 consecutive weeks. Diabetes was inducedby injection of 120 mg/kg alloxan monohydrate intraperitoneally. Two diabetic groups were treated with different doses of R. ribes root extract. The sixth diabetic groups were treated with glibenclamide (0.6 mg/kg). At the end of 28 days, blood samples were collected and their kidney tissues were processed for light microscopy. Background and Objective: Use of medicinal plants for attenuation of hyperglycemia and restoration of lipids to normal level is clinically very important. We decided to assess the preventive role of aqueous extract of Berberis Integerrima root on the serum levels of glucose, insulin, and lipid profile in Streptozotocin (STZ)-induced diabetic rats.Materials and Methods: Forty male rats were divided into 5 groups as follows: 1- normal (N); 2- normal + barberry (N+B) (they received barberry root extract for 6 weeks); 3- diabetic (D) (they received STZ, 65 mg/kg BW /i.p.); 4- diabetic + barberry before (D+Bb) (they received barberry root extract for 3 weeks before STZ injection and continued for another three weeks); and 5- diabetic + barberry after (D+Ba) (three days after STZ injection, they received barberry root extract for 3 weeks). The experimental groups received barberry root extract (500 mg/kg bw) intra gastric by gavage for 6 weeks and the experimental period for each rat was 6 weeks.Results: Diabetic rats showed a significant increase in serum glucose, total cholesterol, triglycerides, LDL-C, and food intake as well as a decrease in HDL-C, body weight and serum insulin, compared to the other groups. Administration of the barberry root extract in diabetic rats restored these changes towards normal to some extent.Conclusion: In this study, for the first time, we showed that the administration of the barberry root extract before diabetes induction resulted in better amelioration in the serum levels of glucose, insulin, and lipid profile, compared to the group receiving it after induction: this indicates that the barberry root extract can play both a preventive and a therapeutic role in such patients.

Thioredoxins (Trxs) are small heat-stable proteins that participate in dithiol-disulfide exchange reactions. In contrast to other organisms, plants contain six different Trx types: f, m, x, y, o and h. The h-type Trx consists of multiple forms that involved in different processes such as cellular protection against oxidative, biotic and abiotic stresses. Two thioredoxin h-type genes, called VvTrxh4 and VvCxxS2, were isolated of grape (Vitis vinifera L. cv. Askari) berry tissue and were cloned into pUC19 plasmid vector. Reverse transcription was carried out using Polymerase Chain Reaction (RT-PCR), to analyze biochemical, structural and phylogenetic characteristics. Nucleotide sequence analysis revealed that the open reading frame of VvTrxh4 and VvCxxS2 genes are 345 bp and 381 bp long, respectively and encode for proteins of 114 and 126 amino acid residues, respectively. Protein sequence analysis showed that VvTrxh4 gene contains a typical catalytic site WCGPC, whereas VvCxxS2 gene harbors the non-typical active site WCIPS. The calculated molecular mass and the predicted isoelectric point of the deduced polypeptides for both VvTrxh4 and VvCxxS2 were 12.76 kDa and 5.22, and also 14.25 kDa and 4.68, respectively. Structural analysis showed that deduced proteins contain a identical 3D structure, whereas phylogenetic study of cloned genes with thioredoxin from other plants revealed that these genes are belonging to different subgroups. Objective: Patellofemoral pain (PFP) is the most commonly reported musculoskeletal overuse injury in active individuals, such as athletes, and is a multifactorial problem with no definite cause identified to date. Some studies have shown a relationship between impaired core and trunk sensorimotor control and knee disorders, especially PFP. The aim of this study was to evaluate trunk flexion proprioception by comparing the repositioning error between healthy athletes and athletes with PFP. Design: Cross-sectional case-control study. Setting: Rehabilitation sciences research center. Participants: Twenty healthy athletes and 20 athletes with PFP. Main Outcome Measures: To examine proprioception of trunk flexors, the absolute active and passive repositioning error at 30° and 60° trunk flexion were evaluated with isokinetic dynamometry. The results were compared between the two groups. Results: In the PFP group, the active trunk repositioning error at 30° flexion was significantly greater than in the healthy individuals (P < .001). The mean absolute active repositioning error at 30° flexion was 3.04° (1.37°) in the PFP group and 1.50° (0.70°) in the control group. There was no significant difference between groups in the active trunk repositioning error at 60° flexion (P = .066). The mean absolute active repositioning error at 60° flexion was 2.96° (1.26°) in the PFP group and 2.18° (0.99°) in the control group. The passive trunk repositioning error at 30° and 60° flexion was significantly greater in the PFP group (P = .013 and P - .004, respectively). The mean absolute passive repositioning error at 30° and 60° flexion in the PFP group was 2.94° (0.80°) and 3.13° (1.19°), respectively, and was 2.08° (1.08°) and 1.96° (0.71°), respectively, in the control group. The calculated eta-squared value showed that joint repositioning errors had large effect sizes (0.15-0.32). Conclusion: Trunk proprioception in the flexion direction may be impaired in patients with PFP. This finding suggests that trunk proprioception training may be important in rehabilitation for athletes with PFP. [ABSTRACT FROM AUTHOR]

The most important problem of the mystic man is to move away from the original homeland, and his effort to return to the era of carpentry is the subject of all mystical books. Because of his beliefs, Aref cannot live like other people, so he often turns away from people. Considering that there are different types of homelessness and in Masnavi the mystic homelessness in different forms such as homelessness in the world, homelessness of the soul in the material world, homelessness among the apparent in this study examines homelessness among the apparent from Rumi's point of view. . This study shows that Rumi, as a pious mystic, describes the life of the great mystics in seclusion and solitude and considers the presence of the common people as a plague for conduct because often the public and the apparent do not understand the actions of the mystic and in addition When they make wrong judgments, their speech is also useless and sometimes harmful for the mystic, so solitude and traveling and away from people is a good way to cover this homelessness.

The truth of intuitive experiences and how to fulfill them are among the issues that all people, cultures and rituals have faced with and expressed views about. There are two major approaches the essence-oriented notion which has emphasized on the cohesion and commonality of these experiences between individuals and people, and construction-oriented perspective which has considered more important roles for the discovery ideas and thoughts in the development and shaping of these experiences, so there are essential differences of them between people and cultures. Intellectual system of Illumination, which has been developed according to Sohrevardi’s discussions, has unique and valuable view in this regard. It asserts that :::::union::::: with light underlies mystical revelation and the specific self-centeredness is involved in the realization of these experiences along with illumination, observation, :::::union:::::, and inexistence. Although characteristics of illumination and talent are essential in this system of thought, the collection of features regarding spiritual experiences, according to Sohrevardi, represents a kind of essence-orientation in mystical revelation. The personal unity of experiences based on the thinking of each individual does not mean to separate experiences. Rather, they all have common grounds. The present study first, refers to the views expressed in this regard, and then explains what and how of spiritual experiences from the viewpoint of Sohrevardi. It finally, provides answers to some unknown issues.

Objective: Despite an increasing number of studies, there is as yet no definite treatment developed for the coronavirus disease 2019 (COVID-19). In this clinical trial, we examined the efficacy of a novel herbal antiviral preparation in critically ill COVID-19 patients.Materials and Methods: A total number of 120 ICU-admitted patients with a diagnosis of COVID-19 pneumonia were recruited to the trial. Participants were equally randomized to receive either the novel antiviral preparation sublingually, for up to two consecutive weeks or till discharge, or placebo. Clinical and laboratory parameters as well as survival rates were compared between the two groups.Results: The cumulative incidence of death throughout the study period was 8.33% in the intervention group and 60% in the placebo group (risk ratio: 0.14; 95% confidence interval [CI], 0.05 to 0.32; p

Background: Many high-risk behaviors often co-occur during adolescents’ development and can be linked to their life patterns, beliefs, and early maladaptive schemas (EMS). This research aimed to determine the association between EMS and high-risk behaviors among teenagers in Shiraz, Iran.Methods: This cross-sectional study was conducted in 2022, involving 348 adolescents aged 13 to 19 selected through a convenience sampling method in Shiraz, Iran. The Young Schema Questionnaire was utilized to assess EMS, and a researcherdeveloped self-reported checklist was used to collect demographic and behavioral data from the adolescents, including smoking, drug usage, alcohol use, and hookah use. Logistic regression assessed the association between behavioral characteristics and adolescents’ EMS aspects.Results: Among the 348 adolescents, 187 (53.7%) were male. The relative frequencies of hookah, alcohol, and drug use among boys were 31.6%, 40.1%, and 10.2%, respectively, whereas among girls, they were 11.2%, 15.5%, and 1.2%, respectively. A significant difference was observed between the two groups in terms of the relative frequencies of hookah (P=0.001), alcohol (P=0.001), and drug usage (P=0.008). A significant association was found between different domains of schemas and high-risk behaviors among the participants.Conclusion: The findings suggested that EMS may predict high-risk behaviors. Therefore, early-life interventions should be considered in line with factors contributing to controlling the negative consequences of EMS among adolescents. Further research is recommended to evaluate this population’s predisposing factors for EMS and preventive interventions.

Yaghma Jandaghi is one of the most famous and prominent poets of the Qajar era who has written significant sonnets with moral and romantic content. Since the beginning of Persian poetry till now, rhetoric has always been taken into special consideration by poets, and one of the fundamental issues of rhetoric in any language is poetic imagination forms, including four general topics, namely simile, metaphor, irony, and metonymy, the thoughtful and appropriate use of which can add to the imaginative aspect as well as the richness of poems. Yaghmai Jandaghi's sonnets are highly important in terms of the inclusion of rhetoric, especially rhetoric and the use of poetic imagery. Yaghma’s sonnets contain moral and romantic themes, written in different meters and mainly in a style similar to that of Hafez’s poems. Since little attention has been paid to the issue of imagination forms in Yaghmai Jandaghi’s poems, in the present research, an attempt has been made to analyze the imagery techniques used in the sonnets of Yaghmai Jandaghi with an in-depth view and with respect to the poet's taste in using them as an aesthetic approach and expressive and artistic technique so that a manifestation of his poetic merits can be presented to the audience. Introduction Rhetoric is one of the important techniques used by poets in order to add an imaginative aspect to their poetry, and most of them have tried to apply different categories of this artistic technique, especially simile and metaphor. Perhaps, it can be argued that the poets of the Qajar era, including Yaghmai Jandaghi, have created a special innovation from the aesthetic dimension using artistic techniques in poetry. This research attempts to evaluate Yaghmai Jandaghi's sonnets accurately from a rhetorical point of view, and then to discuss the imagination forms used in them, including similes, metaphors, irony, and metonymy so that at the end it would be possible to figure out his intended approach in using poetic imagination forms and provide their frequencies. Therefore, the current study seeks to provide an answer to the following research question: How has Yaghmai Jandaghi used imagination forms in his sonnets? Materials and Methods This research has been carried out using the descriptive-analytical method, examining the content of Yaghmay Jandaghi's sonnets and extracting the examples of imagination forms in them through a library approach. Research Findings Yaghmai Jandaghi's sonnets are considered to be a relatively rich work in terms of using imagination forms, and wherever the poet needed imagination forms to expand the content and also to make his audiences understand the topic under discussion, he used them very appropriately in the layers of his poems. The following table shows the frequency of imagination forms applied in Yaghmai Jandaghi's sonnets. Table 1. The most important imagination forms used in Yaghmai Jandaghi's sonnets are simile, metaphor, irony, and metonymy each of which has been used for specific purposes; for instance, the sensible simile has the greatest contribution to making the topic and content sensible. Due to the closeness of his style to the Indian style, it can be argued that the poet has made the most use of allegorical, implicit, and compound similes in order to make abstract and sensible concepts sensible for the audience and so that his poems could have a double effect on the audience. In Yaghma's poetry, explicit and implicit metaphors stand out in parallel in his poems. Regarding explicit metaphors, it was found that the tools of simile used in his poems mainly include the beloved’s parts of the body, such as their lips, mouth, eyes, and hair. In implicit metaphors, he has mainly given life to things such as death, the sky, the zephyr, and the inanimate phenomena of his surroundings and attributed human and living beings’ characteristics to them.

BackgroundMental disorders are increasingly prevalent among adolescents without appropriate response. There are a variety of reasons for unmet mental health needs, including attitudinal and structural barriers. Accordingly, we investigated perceived mental health needs, using mental health services, and their barriers in adolescents.MethodThis cross-sectional study was conducted in 2022 in Shiraz, Iran. Demographic characteristics, the Adolescent Unmet Needs Checklist, and the Young Schema Questionnaire were administered to 348 adolescents aged 13–19 years. Adolescents were classified as having no needs, fully met needs, partially met needs, or wholly unmet needs. Logistic regression analysis was used to determine factors associated with perceived unmet need and refer participants to healthcare centers.Results193 (55.5%) adolescents reported perceived need for mental healthcare out of whom, 21.6% reported fully and 21.6% partially unmet needs. Noticeably, only 12.4% of needy participants reported met need. “Reluctance to seek mental healthcare” and “asked but not receiving help” were common barriers to using the services.ConclusionThe present study reveals unmet mental healthcare needs as a significant public health concern among the adolescents. To address this significant concern, reorientation of primary care, removing economic barriers from mental healthcare services, and improving health literacy in the community are recommended.

Background: Network Scale Up (NSU) is applied in many settings to estimate the size of hidden populations.The visibility of alcohol consumption - as a hidden behavior - in Iran has not been yet set. Our aim is to estimatethe visibility factor (VF) of alcohol consumption in Iran which is an Islamic country in the Middle East.Methods: Ninety persons who had a history of alcohol consumption were recruited. Relationships in networkwere aligned in three main subgroups: immediate family, extended family, and non-family. According to thegame of contact methodology, participants answered questions about total and aware number of personsthey know in each relationship category. VF was calculated by dividing total number of people aware aboutthe respondent’s alcohol consumption by total number of respondent’s social network. The 95% confidenceintervals (CIs) were computed through bootstrapping.Findings: The mean and standard deviation (SD) of participants’ age was 32.9 ± 10.2, the sex ratio was 3.Overall VF (95% CI) was 40% (33% to 47%). VF was estimated at 44% and 23% among men and women’snetwork, respectively. The immediate family was the highest informed group, followed by non-family andextended family members.Conclusion: The visibility of alcohol consumption in Iran was not high. This is due to religious and legalprohibitions around it

Abstract Objectives This study is designed to evaluate the effect of a workshop about new teaching and learning methods on the response, knowledge, and behavior of healthcare staff working a large city healthcare center. Results Kirkpatrick’s program evaluation model showed that the workshop on new teaching and learning methods significantly improved the healthcare staff’s satisfaction about the teaching environment of workshops, their knowledge about new teaching and learning methods and their behavior in performing workshops for teaching people. It is recommended that this teaching and learning methods workshop should be considered in educational programs for healthcare staff. BACKGROUND: Until now, there was no available study on the knowledge and practice of the people of Iran with regard to the coronavirus disease 2019 (COVID-19) pandemic. OBJECTIVE: This study aimed to investigate the knowledge and preventive practices of Iranians toward the COVID-19 pandemic. METHODS: This is a cross-sectional study of 925 people who completed an online questionnaire in March 2020. The study used 21 and 14 questions, respectively, to assess the knowledge and preventive practices of the population in regard to COVID-19. Cronbach's alpha was 0.75 for the knowledge scale and 0.71 for the practice scale. To determine the importance of each independent variable in explaining the participant's practice, a multiple regression model was applied. KEY RESULTS: The results indicated a moderate level of knowledge and preventive practices in regard to COVID-19 in most of the respondents (56.8% and 56.5%, respectively). According to the multiple regression analysis, knowledge showed the highest effect on the practice of the participants (β = 0.479). The determination coefficient for the model (R2 = 0.509) also showed approximately 51% of the variance in practice was explained by gender, occupational status, knowledge, cost of hand sanitizer, and the belief in the effectiveness of using such necessities. CONCLUSIONS: Acceptable rates of knowledge and practice were observed in most Iranians. However, approximately 10% of the participants were unaware of the effective measures for preventing the infection, which can cause active transmission of the virus. In addition to considering the importance of high community awareness in prevention and isolation measures, the government should provide disinfectants and other materials at a low price to reduce the transmission, as this may lead to effective practice to break the chain of transmission of COVID-19.

Background and Objectives. With the increase in the growth of genetically modified (GM) crops, concerns as to the adverse effects of GM crops have risen in the community. The present systematic review seeks to assess the GM plants’ potential impacts on the sperm parameters, including sperm head, sperm motility, sperm abnormality, and fertility index. Materials and Methods. A structured literature search was independently performed by three authors on Scopus, Web of Science, PubMed, and Embase in January 2019. A total of 1467 publications were obtained by the search strategy after eliminating the duplicates. Moreover, the review only included articles written in English language. Other pertinent peer-reviewed publications were chosen (“snowballing”) from the reference lists in the selected publications. To assess the GM crop effects on infertility, experimental studies designed with the control group were selected. On the basis of abstract screening and full-text reviewing, 39 relevant publications were selected, seven of which were used in our review. To access the quality of articles, we used the Cochrane checklist. The collected articles were scored independently by three authors, and the publications with the eligibility criteria were included in our review. No article was excluded due to quality assessment. Results and Conclusion. Our findings indicated that GM products had no adverse effects on infertility indices such as the sperm head, sperm motility, sperm abnormality, and fertility indices. Long-term research still seems to be strongly necessary to ensure that the use of GM plants does not cause any harm to consumers, especially in infertility area.

AIM: To evaluate correlation of cystatin-C(Cys-C)with severity of diabetic retinopathy(DR)and acute phase reactants, including erythrocyte sedimentation rate(ESR)and high-sensitivity C-reactive protein(hs-CRP).METHODS: All diabetic patients who were referred for diabetic retinopathy(DR)screening during 1mo were enrolled. Demographic data were recorded. All patients have undergone full ophthalmic exam. At the same day, all patients were tested for hemoglobin A1c(HbA1c), ESR, hs-CRP, and Cys-C serum levels.RESULTS: Sixty seven diabetics were enrolled, including 19(28.3%)without retinopathy, 22(32.8%)non-proliferative retinopathy, and 26(38.8%)proliferative retinopathy patients. The mean age, sex distribution, mean duration of diabetes, prevalence of hypertension and dyslipidemia, smoking status and HbA1c levels were not significantly different among the three groups. The mean levels of Cys-C increase significantly as retinopathy progress \〖1.1±0.48; 1.22±0.38; 1.71±0.92(P=0.007), respectively\〗. In multiple regression analysis, just Cys-C was significantly associated with severity of DR(P=0.025).CONCLUSIONS: This study revealed that serum levels of Cys-C increase while DR progress independently of acute phase reactants. Therefore, it could be used as an associated marker by primary care physicians to distinguish patients at higher risk of severe DR. Larger randomized studies are warranted to confirm findings. Reviewing physiological role of the Cys-C, we proposed that the Cys-C may be a protective response to catalytic stress rather than being a pathogenic factor in microangiopathies.

Banking system is of great importance in many countries. This system can provide effectiveness in the economy with the proper circulation of money and in the case of improper functioning, the activities of other economic sectors will face difficulties. Although banking without lucre has progresses in theoretical field, but competitiveness ability and its operational aspects is limited in transnational level. Many factors affect on this issue; lack of alignment with Islamic banking standards and operational rules of Basel Committee, political factors such as sanctions and contract compliance risk are challenges that have limited Iranian banking system in communication with other financial institutions and also has limited the improvement of indicators quality. This paper has conducted using a descriptive-analytical method and collecting library and documentary information, and studied the private banks performance of Islamic republic of Iran and Islamic banks of Persian Gulf region, based on CAMEL indexes. The results show that selected Islamic banks in terms of capital adequacy, credit risks, management quality index and liquidity index are in better situation compared to private banks in Iran.

Background Dual use of tobacco products such as cigarettes and water pipe may be associated with increased risk of nicotine dependence and smoking-related complications. Accurate statistics are not available in this regard in Iran. Thus, this study sought to assess the prevalence of dual cigarette/water pipe smoking and its related factors in Iran. Methods This cross-sectional study was conducted on Tehran residents over 15 years of age selected via cluster, multi-stage randomized sampling from different geographical districts of Tehran in November and December 2014. The data were collected using the water pipe section of the Global Adult Tobacco Survey (GATS) questionnaire. Results A total of 1,830 individuals participated in this study; of which, 243 (13.3%) were exclusive water pipe smokers, 76 (4.2%) were dual smokers of cigarettes and water pipe and 120 (6.6%) were exclusive cigarette smokers. Of dual smokers, 86.8% were males and 13.2% were females (P< 0.001). The mean age was 28.01±8.7 yearsin exclusive water pipe smokers and 33.1±1.1 yearsin dual smokers (P< 0.001). Male sex (adj. OR: 3.8) and older age (adj. OR: 1.06) increased the odds of dual smoking. Conclusions The prevalence of dual smoking and exclusive water pipe smoking was 4.2% and 13.3%, respectively. Exclusive cigarette smoking had a prevalence of 6.1%. The frequency of dual smoking of cigarettes and water pipe was considerably high. Dual smokers had a higher mean age than exclusive water pipe smokers and they were mostly males. The mean age at initiation of water pipe smoking in dual smokers was lower than the mean age at the onset of cigarette smoking. In other words, dual smokers started water pipe smoking sooner than cigarette smoking. Future studies with different methodologies are required to further scrutinize the relationship of water pipe and cigarette smoking.

Background: Shiraz University of Medical Sciences, Iran has developed a new version of health urban posts called Urban Community Health Center (UCHC) with primary aim of improving primary health care (PHC) in urban areas. The aim of present study was to evaluate this newly developed model of UCHC in Ghaleno, a suburban region of Shiraz, Iran in 2014. Methods: Besides descriptive analysis of foregoing model and considering its goal, plan, and dimensions, a qualitative study was carried out using in-depth interview with four managers of this model in Ghaleo region as well as the family physicians (FPs) of the model. Data were analyzed using conventional content analysis. Results: Evaluation of this model in 7-month period after implementation, showed that population coverage was raised from 23% to 84%, a remarkable and increasing achievement of 61%. The universal package is a protocol for providing healthcare services based on PHC. The descriptive study of the model based on physicians view, explored real FP, population's satisfaction, importance of physician's assistant, and payment system reform in the model. Conclusion: Ghaleno model has unique objectives in providing healthcare services in urban areas. The findings of this study call for further evaluation, specifically in quality care services. The success and continuation of this model demand the support of policy makers.

Background: Epigenetic reprogramming of somatic cells into embryonic stem cells has attracted much attention, because of the potential for stem cell transplantation and compatibility with recipient. However, the therapeutic application of either nuclear transfer or nuclear fusion of somatic cell has been hindered by technical complications as well as ethical objections. Recently, a new method is reported whereby ectopic expression of embryonic specific transcription factors was shown to induce fibroblasts to become embryonic like SCs (induced pluripotent stem cells). A major limitation of this method is the use of potentially harmful genome integrating viruses such as reto- or lentivirus. The main aim of this investigation was generation of human hematopoietic stem cells from induced fibroblasts by safe adenovectors carrying embryonically active genes. Material and Methods: Isolated fibroblasts from foreskin were expanded and recombinant adenoviruses carrying human Sox2, Oct4, Klf4, cMyc genes were added to culture. After formation of embryonic like colonies and cell expansion, they were transferred to embryonic media without bFGF, and embryoid bodies were cultured on stromal and non-stromal differentiation media for 14 days. Results: Expression of CD34 gene and antigenic markers, CD34, CD38 & CD133 in stromal culture showed significant difference with non-differentiation and non-stromal media. Conclusion: These findings show high hematopoietic differentiation rate of Adeno-iPS cells in stromal culture and no need to use growth factors. While, there was no difference between non-differentiation and non-stromal media.

Article 140 of the Iranian Civil Code, following Islamic jurisprudence, regards inheritance as the fourth cause of ownership. After death, one of the most important issues raised from the contract is the legal rights that had been given to the deceased. One of the legal rights is a consent to or refusal of a blunt transaction. In other words, the questions are: Is this right subject to inheritance after the right owner dies? If yes, how should heirs act? Iranian Civil Code and other legislation have not dealt with this issue adequately. Article 253 of the Code only states that this consent or refusal can be inherited in unauthorised transactions. In relation to this consent or refusal in blunt transactions, Islamic jurists treat it as a warrant, whereas there is a controversy among lawyers. Some lawyers consider it to be a right; however, others perceive it as a warrant. The prevailing view is the latter. Another question posed here is whether the inheritance of the consent or refusal depends on the subject-matter of the contract or it is an independent issue. In answering this question, it should be said that the inheritance of the consent or refusal is followed by inheritance of the subject-matter of contract (property). In other words, it can be interpreted that the property of the deceased is inherited to each heirs who inherits based on their own ownership. This article considers how this right is inherited, and examines relevant provisions and circumstances.

Background and Objectives: Hospitals are important and vital centers for public services,which their nature of activity and type of provided services, require a calm and silent environment. This study was done to evaluate noise exposure dose among nurses of 6 hospitals in Qom city. Methods: In this descriptive cross-sectional study, 12 nurses working in 6 educational hospitals affiliated to Qom University of Medical Sciences were selected randomly at the first stage. To assess the noise exposure level, two calibrated dosimeters (TES model, Taiwan) were used. In the second stage, demographic data such as age, sex, years of employment, daily working hours, etc. were collected using a questionnaire. Finally, the data were analyzed using central statistical indices, frequency distribution, one-way ANOVA and Duncanchr('39')s test at the significance level of less than 0.05. Results: In this study, in all the studied hospitals, the level of noise exposure was higher than permissible limit. Background: The prevalence of water pipe (WP) smoking was studied using a standard questionnaire. Pulmonary function tests were also compared between WP smokers and non-smokers. Materials and Methods: The prevalence of WP smoking was studied using a standard questionnaire. Pulmonary function tests including forced vital capacity (FVC), forced expiratory volume in one second (FEV1), maximal mid-expiratory flow (MMEF), peak expiratory flow (PEF), maximal expiratory flow at 75%, 50%, and 25% of the FVC (MEF75,50,25) were compared between WP smokers and non-smokers. Results: A total of 673 individuals including 372 males and 301 females were interviewed. The number of WP smokers was 58 (8.6%) including 24 males (6.5%) and 34 females (11.3%). All pulmonary functional test (PFT) values in WP smokers were lower as compared to the non-smokers (P < 0.05 to P < 0.001). The prevalence and severity of respiratory symptoms (RS) in WP smokers were higher than non-smokers (P < 0.05 to P < 0.001). There were negative correlations between PFT values and positive correlation between RS and duration, rate, as well as total smoking (duration X rate) (P < 0.05 to P < 0.001). Conclusion: In this study the prevalence of WP smoking in Mashhad city was evaluated for the first time. The results also showed a significant effect of WP smoking on PFT values and respiratory symptoms.

Purpose: Many of the current brain-computer interface systems rely on the patient`s ability to control voluntary eye movements. Some diseases can lead to defects in the visual system. Due to the intactness of these patients` auditory system, researchers moved towards the auditory paradigms. Attention can modulate the power of auditory steady-state response. As a result, this response is useful in an auditory brain-computer interface. As humans intrinsically enjoy listening to rhythmic sounds, this project was carried out with the aim of extraction and classification of the EEG signal patterns in response to simple and rhythmic auditory stimuli to investigate the possibility of using the rhythmic stimuli in brain-computer interface systems. Methods: Two three-membered simple and rhythmic groups of auditory sinusoidally amplitude-modulated tones were generated as the stimuli. Corresponding EEG signals were recorded and classified by means of five-fold cross-validated naïve Bayes classifier on the basis of power spectral density at message frequencies. Results: There was no significant difference between the classification performances of the responses to each group of the stimuli. All the classification accuracies, even without any noise reduction and artifact rejection, was greater than the acceptable value for being used in brain-computer interface systems (70%). Conclusion: Like the common sinusoidally amplitude-modulated tones, the novel proposed rhythmic stimuli in this project have a promising discrimination for being used in brain-computer interface systems. In addition, Power spectral density has provided an appropriate discrimination for within- and between-subject EEG classification.
