Abstract Androgen deprivation therapy (ADT), a key element of prostate cancer treatment, is associated with increased risk for cardiovascular morbidity and mortality. The underlying mechanisms include adverse metabolic alterations, but further mechanisms are likely. Animal studies suggest increased progression of atherosclerosis in androgen deprived conditions. Based on in vitro studies, lack of androgens may modulate immune cells including monocytes, macrophages, and T-cells towards a pro-inflammatory phenotype and pro-atherogenic function. As a novel aspect, this review summarizes existing data on the effect of androgens and androgen deprivation on platelet activity, which play a major role in inflammation and in the initiation and progression of atherosclerotic lesions. Testosterone modulates platelet aggregation responses which are affected by dose level, source of androgen, and age. Data on the effects of ADT on platelet activity and aggregation are limited and conflicting, as both increased and decreased aggregation responses during ADT have been reported. Gaps in knowledge about the mechanisms leading to increased cardiovascular risk during ADT remain and further research is warranted. Improved understanding of pathogenic pathways linking ADT to cardiovascular risk may help identify clinically useful diagnostic and prognostic biomarkers, and accelerate finding novel therapeutic targets, and thus optimize prostate cancer treatment outcomes.

Abstract The Colorado River drains about 8% of the conterminous United States, provides water for 40 million people, and is one of the most overallocated rivers in the world. As the upper Colorado River Basin (UCOL) contributes an estimated 92% of the total basin natural streamflow, knowledge of the location and amount of surface water withdrawals in the UCOL is important for managing the Colorado River system. Since the UCOL encompasses portions of five states, water use data are dispersed among numerous federal and state agency databases, and there is no centralized dataset that documents surface water use within the entire UCOL at a fine spatial and temporal resolution. This article presents an inventory of 1,358 major structures that divert surface water from and within the UCOL with corresponding daily time series withdrawal records from 1980 through 2022. Data compilation efforts, processing methods, and contents of this diversion database are documented, and summary information is provided.

Abstract Purpose Percentage of positive cores involved on a systemic prostate biopsy has been established as a risk factor for adverse oncologic outcomes and is a National Comprehensive Cancer Network (NCCN) independent parameter for unfavorable intermediate-risk disease. Most data from a radiation standpoint was published in an era of conventional fractionation. We explore whether the higher biological dose delivered with SBRT can mitigate this risk factor. Methods A large single institutional database was interrogated to identify all patients diagnosed with localized prostate cancer (PCa) treated with 5-fraction SBRT without ADT. Pathology results were reviewed to determine detailed core involvement as well as Gleason score (GS). High-volume biopsy core involvement was defined as ≥ 50%. Weighted Gleason core involvement was reviewed, giving higher weight to higher-grade cancer. The PSA kinetics and oncologic outcomes were analyzed for association with core involvement. Results From 2009 to 2018, 1590 patients were identified who underwent SBRT for localized PCa. We review the potential contributions of environmental and resource economics (ERE) to the achievement of sustainable development in developing countries; and highlight the limitations associated with applying ERE within a developing-country context, using examples from South Africa. We fInd that ERE has much to offer in helping to overcome the challenges associated with sustainable development in developing countries, but that the developing-country context needs to be taken into account before applying tools and methods that were designed with the developedcountry context in mind. In particular, the unique and often complex socioecological context of developing countries needs to be considered and integrated into policy and management prescriptions.

As industries move actively towards sustainable operations, communicating the value created as a result of implementing social sustainability practices is still a challenge for researchers and practitioners. Understanding the rationale behind the implementation of sustainability practices can improve adoption and communication amongst supply chain actors. This study aims to investigate the drivers of social sustainability practices in the European Leather Supply chain through the lens of the stakeholder and institutional theory. A qualitative approach is applied in which primary and secondary data are used to achieve the study's aim and objectives in the context of the European leather supply chain. Data was thematically analysed based on the key themes developed in the initial conceptual framework. The study's key findings show that internal and external drivers that serve as antecedents for implementing social sustainability practices can be general or specific, providing another paradigm to Sustainability driver studies. Internal drivers rather than external drivers were observed to clearly relate to and explain sustainability practices implemented. The results from the study show that the application of the intersection between Institutional Theory and Stakeholder Theory can assist in explaining the relationship between social sustainability drivers and practices. The study proposes a framework for managers that can assist leather brands in understanding and explaining the importance of implementing social sustainability practices to key stakeholders whilst actualising short- and long-term business goals.

Abstract Herewith waste collagen was processed using a deep eutectic solvent (DES) obtained by the complexation between choline chloride (ChCl) and ethylene glycol 1:2 (ChCl:2EG) to form a material with properties similar to leather. The material generated using the waste tanned collagen processed with ChCl:2EG followed by mechanical extrusion was found to show characteristics similar to the one obtained from the material using animal hide powder. However, the tensile strength of extruded collagen was substantially higher (7.56 MPa vs. 0.23 MPa). The improved strength and structure may be due in part to traces of iron imparted into the material during extrusion of the acidic collagen sample. Nevertheless, the material thus generated was found to have a lower degree of crystallinity compared to commercial leather samples. Overall, the process demonstrates a facile method preparing a useful material out of waste collagen by the intervention of DESs. Graphical Abstract

INTRODUCTION:Mechanical intestinal obstruction is a disorder associated with intra-abdominal hypertension and abdominal compartment syndrome. As the large intestine intraluminal and intra-abdominal pressures are increased, so the patient's risk for intestinal ischaemia. Previous studies have focused on hypoperfusion and bacterial translocation without considering the concomitant effect of intra-abdominal hypertension. The objective of this study was to design and evaluate a mechanical intestinal obstruction model in pigs similar to the human pathophysiology. MATERIALS AND METHODS:Fifteen pigs were divided into three groups: a control group (n = 5) and two groups of 5 pigs with intra-abdominal hypertension induced by mechanical intestinal obstruction. The intra-abdominal pressures of 20 mmHg were maintained for 2 and 5 hours respectively. Hemodynamic, respiratory and gastric intramucosal pH values, as well as blood tests were recorded every 30 min. RESULTS:Significant differences between the control and mechanical intestinal obstruction groups were noted. The mean arterial pressure, cardiac index, dynamic pulmonary compliance and abdominal perfusion pressure decreased. Mosquito surveillance in large urban areas of the southern USA that border Mexico has become increasingly important due to recent transmission of Zika virus and chikungunya virus in the Americas as well as the continued threat of dengue and West Nile viruses. The vectors of these viruses, Aedes aegypti, Ae. albopictus, and Culex quinquefasciatus, co-occur in residential areas, requiring vector control entities to deploy several different trap types, often expensive and labor-intensive, to surveil these ecologically different species. We evaluated the use of a single trap type, the US Centers for Disease Control and Prevention autocidal gravid ovitraps (AGOs), to monitor all 3 vector species across residential neighborhoods in San Antonio, TX, over 12 wk (epiweeks 24–35). Mosquito abundance was highest early in our surveillance period (epiweek 25) and was driven largely by Cx. quinquefasciatus. The AGOs collected significantly more Cx. quinquefasciatus than both Aedes species, with more Ae. aegypti collected than Ae. albopictus. The average number of Ae. aegypti captured per trap was consistent across most neighborhoods except for 2 areas where one had significantly the highest and the other with the lowest mosquitoes collected per trap. The average number of Ae. albopictus captured per trap varied with no clear pattern, and Cx. quinquefasciatus were trapped most often near forested hill country neighborhoods. These results indicate that AGOs are appropriate for detecting and tracking the relative abundance of Ae. aegypti, Ae. albopictus, and Cx. quinquefasciatus across a large and diverse urban landscape over time and therefore may be an inexpensive and streamlined option for vector surveillance programs in large cities. The article discusses the study which investigates whether mosquito species distribution varied throughout residences in urban and semi-rural gradient in San Antonio, Texas. The study collects adult female mosquitoes from June 1 to August 4, 2015 in about 18 residential backyards in the area. Result suggests that the month of June was the peak season for mosquito wherein several mosquitoes carries certain viruses like Zika, Chikungunya, and dengue.

Rapid infiltration basins (RIBs) are effective tools for wastewater treatment and groundwater recharge, but continuous application of wastewater can increase soil P concentrations and subsequently impact groundwater quality. The objectives of this study were to (1) investigate the effects of reclaimed water infiltration rate and “age” of RIBs on soil P concentrations at various depths, and (2) estimate the degree (percentage) of sorption equilibrium reached between effluent P and soil attained during reclaimed water application to different RIBs. The study was conducted in four contrasting cells of a RIB system with up to a 25 year history of secondary wastewater application. Soil samples were collected from 0 to 300 cm depth at 30 cm intervals and analyzed for water extractable phosphorus (WEP) and oxalate extractable P, Al, and Fe concentrations. Water extractable P and P saturation ratio (PSR) values were generally greater in the cells receiving reclaimed water compared to control soils, suggesting that reclaimed water P application can increase soil P concentrations and the risk of P movement to greater depths. Differences between treatment and control samples were more evident in cells with longer histories of reclaimed water application due to greater P loading. Data also indicated considerable spatial variability in WEP concentrations and PSR values, especially within cells from RIBs characterized by fast infiltration rates. This occurs because wastewater-P flows through surface soils much faster than the minimum time required for sorption equilibrium to occur. Phosphanotriylbenzenecarboxylic acid (ptbcH3; P(C6H4-p-CO2H)3) and its methyl phosphonium iodide derivative (mptbcH3I; {H3CP(C6H4-p-CO2H)3}I) have been used as organic building blocks in reaction with Zn(ii) salts to obtain a series of related two-dimensional coordination polymers with honeycomb-like networks. The variable coordination number and oxidation states available to phosphorus have been exploited to produce a family of related phosphine coordination materials (PCMs) using a single ligand precursor. The phosphine carboxylate trianion, ptbc3−, reacted with Zn(ii) to form 3,3-connected undulating hexagonal sheets based on tetrahedral P and Zn nodes, where Zn–ptbc = 1 : 1. When hydroxide was used as an additional framework ligand, Zn4(OH)2clusters were obtained. The clusters support 6,3-connected bilayers that consist of pairs of fused hexagonal sheets (Zn–ptbc = 2 : 1) with intra-layer pore spaces. The Zn4(OH)2clusters are also coordinated by solvent, which was preferentially displaced when the bilayer material was synthesized in the presence of ethylene diamine. Treatment of ptbc3−with MeI resulted in methylation of the phosphine to give the P(v) phosphonium iodide salt derivative. The formally dianionic methylphosphonium tricarboxylate building block, mptbc2−, has the same trigonal-pyramidal bridging geometry as the parent phosphine. However, mptbc2−reacted with Zn(ii) on a 1 : 1 stoichiometric ratio to give an unusual trilayer sheet polymer that is based exclusively on 3-connected nodes. Aristiform spines of the rodents Niviventer fulvescens, Maxomys surifer, Hoplomys gymnurus and 17 species of Proechimys (representing both recognized subgenera and all nine species groups) were studied qualitatively using scanning electron microscopy (SEM) and quantitatively by measuring seven linear dimensions. SEM was used to examine spine tips, bases, longitudinal furrows and cross sections. Spines of the murid rodents N. fulvescens and M. surifer differed from those of the echimyid rodents H. gymnurus and Proechimys spp. in possessing a smaller base with a longer, narrower neck, scaled rather than ridged longitudinal furrows, and a solid internal core and large lacunae at the spine margins. Spines of H. gymnurus differed from those of Proechimys spp. in being considerably more robust with a stout neck at the base, an abruptly-tapering tip and a dense inner layer with a series of smaller lacunae at the spine margins. A factor analysis of spine measurements revealed major differences among N. fulvescens, M. surifer, H. gymnurus, the Proechimys subgenus Trinomys and the nine Proechimys species groups within the subgenus Proechimys. However, all Proechimys species groups clustered closely together. A discriminant function analysis of the nine Proechimys species groups provided generally limited discriminatory power. Although spines are distinct at the generic and subgeneric levels, spines may possess limited diagnostic structure at the level of species within the subgenus Proechimys. [ABSTRACT FROM AUTHOR]

Objectives: The in vitro activity of a new fluoroquinolone, ABT-492, was determined.Methods: MICs were compared with those of two β-lactams, telithromycin, ciprofloxacin and four later generation fluoroquinolones. The effects of human serum and of inoculum concentration were also investigated.Results: MIC data indicate that ABT-492 has potent activity against Gram-positive organisms with enhanced anti-staphylococcal activity compared with earlier fluoroquinolones, in addition to activity against β-haemolytic streptococci, pneumococci including penicillin- and fluoroquinolone-resistant strains and vancomycin-susceptible and -resistant Enterococcus faecalis but not Enterococcus faecium. ABT-492 was the most active agent tested against Haemophilus influenzae, Moraxella catarrhalis, Neisseria meningitidis, fluoroquinolone-susceptible Neisseria gonorrhoeae and anaerobes. Good activity was observed for ABT-492 amongst the Enterobacteriaceae and anaerobes tested, but ciprofloxacin showed superior activity for species of Proteus, Morganella and Providencia, as well as for Pseudomonas spp. In common with the other fluoroquinolones tested, organisms with reduced susceptibility to ciprofloxacin had raised MIC90s to ABT-492. The one isolate of H. influenzae tested with reduced fluoroquinolone susceptibility had an ABT-492 MIC close to that of the population lacking a mechanism of quinolone resistance. ABT-492 was more active than ciprofloxacin against Chlamydia spp. An inoculum effect was observed with a number of isolates of Staphylococcus aureus, Streptococcus pneumoniae, E. faecium, Klebsiella spp. and Escherichia coli, in addition to moderately raised MICs in the presence of 70% serum protein. The clinical significance of these findings is yet to be determined.Conclusions: ABT-492 is a new fluoroquinolone with excellent activity against both Gram-positive and Gram-negative organisms, with many potential clinical uses. Development of genetically modified (GM) plants is contentious, in part because bacterial antibiotic resistance (AR) genes are used in their construction and often become part of the plant genome. This arouses concern that cultivation of GM plants might provide a reservoir of AR genes that could power the evolution of new drug-resistant bacteria. We have considered bacterial DNA transfer systems (conjugation, transduction and transformation) and mechanisms of recombination (homologous recombination, transposition, site-specific recombination and DNA repair) that together might productively transfer AR genes from GM plants to bacterial cells, but are unable to identify a credible scenario whereby new drug-resistant bacteria would be created. However, we cannot entirely rule out the possibility of rare transfer events that involve novel mechanisms. Hence, we also considered if occasional transfers of AR genes (blaTEM, aph(3'),aadA) from GM plants into bacteria would pose a threat to public health. These AR genes are common in many bacteria and each is found on mobile genetic elements that have moved extensively between DNA molecules and bacterial cells. This gene mobility has already severely compromised clinical use of antibiotics to which resistance is conferred. Accordingly, the argument that occasional transfer of these particular resistance genes from GM plants to bacteria would pose an unacceptable risk to human or animal health has little substance. We conclude that the risk of transfer of AR genes from GM plants to bacteria is remote, and that the hazard arising from any such gene transfer is, at worst, slight. [ABSTRACT FROM AUTHOR]

Tunnelling below water passages is a challenging task in terms of planning, pre-investigation and construction. Fracture zones in the underlying bedrock lead to low rock quality and thus reduced stability. For natural reasons, they tend to be more frequent at water passages. Ground investigations that provide information on the subsurface are necessary prior to the construction phase, but these can be logistically difficult. Geophysics can help close the gaps between local point information by producing subsurface images. An approach that combines seismic refraction tomography and electrical resistivity tomography has been tested at the Äspö Hard Rock Laboratory (HRL). The aim was to detect fracture zones in a well-known but logistically challenging area from a measuring perspective. The presented surveys cover a water passage along part of a tunnel that connects surface facilities with an underground test laboratory. The tunnel is approximately 100 m below and 20 m east of the survey line and gives evidence for one major and several minor fracture zones. The geological and general test site conditions, e.g. with strong power line noise from the nearby nuclear power plant, are challenging for geophysical measurements. Co-located positions for seismic and ERT sensors and source positions are used on the 450 m underwater section of the 700 m profile. Because of a large transition zone that appeared in the ERT result and the missing coverage of the seismic data, fracture zones at the southern and northern parts of the underwater passage cannot be detected by separated inversion. Synthetic studies show that significant three-dimensional (3-D) artefacts occur in the ERT model that even exceed the positioning errors of underwater electrodes. The model coverage is closely connected to the resolution and can be used to display the model uncertainty by introducing thresholds to fade-out regions of medium and low resolution. A structural coupling cooperative inversion approach is able to image the northern fracture zone successfully. In addition, previously unknown sedimentary deposits with a significantly large thickness are detected in the otherwise unusually well-documented geological environment. The results significantly improve the imaging of some geologic features, which would have been undetected or misinterpreted otherwise, and combines the images by means of cluster analysis into a conceptual subsurface model.

Objective: This study was designed to measure the concentrations of linezolid in bronchial mucosa, pulmonary macrophages and epithelial lining fluid and to compare them with simultaneous blood levels. Methods: Ten adult patients undergoing bronchoscopy for diagnostic purposes were given oral linezolid at a dosage of 600 mg twice a day for a total of six doses. Patients with active lung infection were excluded from the study. Flexible bronchoscopy was carried out between 2 and 8 h after the last dose of linezolid. Bronchial biopsies and bronchoalveolar lavage were carried out and a simultaneous blood sample obtained. Linezolid levels were measured using high-performance liquid chromatography (HPLC). Results: Mean concentrations of linezolid were 13.4 mg/L in serum, 10.7 mg/kg in mucosa, 8.1 mg/L in alveolar macrophages and 25.1 mg/L in epithelial lining fluid. The mean site/serum concentration ratios were 0.79 for bronchial mucosa, 0.71 for macrophages and 8.35 for epithelial lining fluid. Conclusions: The MIC90 (≤4 mg/L) of linezolid for Staphylococcus aureus and Streptococcus pneumoniae was exceeded in serum and bronchial mucosa in all subjects, in epithelial lining fluid in nine subjects and in macrophages in six subjects. Objectives A validated microbiological assay was used to measure concentrations of iclaprim (AR-100) in plasma, bronchial mucosa (BM), alveolar macrophages (AM) and epithelial lining fluid (ELF) after a single 1.6 mg/kg intravenous 60 min iv infusion of iclaprim. Methods Male volunteers were randomly allocated to three nominal sampling time intervals 1–2 h (Group A), 3–4 h (Group B) and 5.5–7.0 h (Group C) after the start of the drug infusion. Results Mean iclaprim concentrations in plasma, BM, AM and ELF, respectively, were for Group A 0.59 mg/L (SD 0.18), 0.51 mg/kg (SD 0.17), 24.51 mg/L (SD 21.22) and 12.61 mg/L (SD 7.33); Group B 0.24 mg/L (SD 0.05), 0.35 mg/kg (SD 0.17), 7.16 mg/L (SD 1.91) and 6.38 mg/L (SD 5.17); and Group C 0.14 mg/L (SD 0.05), no detectable level in BM, 5.28 mg/L (SD 2.30) and 2.66 mg/L (SD 2.08). Conclusions Iclaprim concentrations in ELF and AM exceeded the MIC90 for penicillin-susceptible Streptococcus pneumoniae (MIC90 0.06 mg/L), penicillin-intermediate S. pneumoniae (MIC90 2 mg/L), penicillin-resistant S. pneumoniae (MIC90 4 mg/L) for 7, 7 and 4 h, respectively, and Chlamydia pneumoniae (MIC90 0.5 mg/L) for 7 h. Mean iclaprim concentrations in ELF exceeded the MIC90 for Haemophilus influenzae (MIC90 4 mg/L) and Moraxella catarrhalis (MIC90 8 mg/L) for up to 4 and 2 h, respectively; in AM the MIC90 was exceeded for up to 7 h. Furthermore, the MIC90 for methicillin-resistant Staphylococcus aureus of 0.12 mg/L was exceeded at all sites for up to 7 h. Personality is known to affect compliance with health-protective behaviors and it has been shown that effective public health messaging can be informed by an understanding of that relationship. Thus, we aimed to evaluate the role personality might play in implementing personal protective measures (PPMs) that can prevent mosquito-borne diseases. This is the first mosquito-related knowledge, attitudes, and practices (KAP) study to incorporate a measure of personality using the Big Five: openness, conscientiousness, extraversion, agreeableness, and neuroticism. KAP studies in Gulf-coast and Mexican border-states in the U.S. are few. Ours is only the second KAP study to take place in Texas despite known local transmission and established mosquito populations capable of transmitting dengue, zika, chikungunya, and West Nile viruses. The KAP survey was administered in three neighborhoods in San Antonio, a large, Hispanic-majority, urban city that is segregated economically and ecologically. We found that openness, agreeableness, and extraversion predicted certain attitudes and PPMs, and that KAP and personality measures did not differ along ethnic or neighborhood lines. Perceptions toward the city's role in mosquito control and education was an important factor in predicting PPMs, suggesting that city culture (attitudes common throughout the city as opposed to attitudes differing by ethnicity and neighborhood) may be most salient in developing public health messaging in San Antonio.

Abstract In preparing the second edition of ‘Tanning Chemistry. The Science of Leather.’, the literature was updated and the content was revised and reviewed. Here, the new findings are presented and discussed. Notable developments include the necessary rethinking of the mechanism of sulfide unhairing because of new understanding of the aqueous chemistry of sulfide species. Revision upwards of the value of the second pKa for sulfide species ionisation means that S2− cannot exist in an aqueous medium, so the unhairing species in hair burn reactions is HS−. Although the technology remains the same, this means the mechanisms of associated reactions such as immunisation must be revised. Rawstock preservation has benefitted from studies of the potential role of materials from plants which accumulate salt, but which also contribute terpene compounds. There is also further discussion on the continuing issue of chromium (VI) in the leather industry. The application to processing of new solvents, ionic liquids and deep eutectics, is the coming technology, which offers transforming options for new chemistries and products. Renewed interest in vegetable tanning and methods of wet white processing are current trends. Also, within the topic of reagent delivery is processing in a solid medium of plastic beads. Abstract Background Approximately one in four pregnant women undergo an induction of labour. The purpose of this study is to investigate the clinical effectiveness, safety, and cost-effectiveness for mothers and babies of two methods of cervical ripening – inpatient care for women starting induction with vaginal prostaglandin E2 hormones, or allowing women to go home for 18 to 24 h after starting induction with a single-balloon catheter. Methods/design This is a multi-centre randomised controlled trial in New Zealand. Eligible pregnant women, with a live singleton baby in a cephalic presentation who undergo a planned induction of labour at term, will be randomised to outpatient balloon-catheter induction or in-hospital prostaglandin induction. The primary outcome is caesarean section rate. To detect a 24% relative risk reduction in caesarean rate from a baseline of 24.8%, with 80% power and 5% type 1 error, will require 1552 participants in a one to one ratio. Discussion If outpatient balloon-catheter induction reduces caesarean section rates, has additional clinical benefits, and is safe, cost-effective, and acceptable to women and clinicians, we anticipate change in induction of labour practice around the world. We think that home-based balloon-catheter induction will be welcomed as part of a patient-centred labour-induction care package for pregnant women. This study aims to identify and compare the key social, economic and environmental sustainability practices in the leather industry. Content analysis was used to analyse extracted sustainability information from either the website, annual report, sustainability report or corporate social responsibility report of six leather-related companies. Review of existing literature assisted in categorising different practices under social, economic and environmental sustainability, while an identification of patterns among practices followed. Findings reveal that companies are observing a good practice of either dedicating a section of their website to revealing their sustainability activities or utilising their sustainability reports or annual reports. Energy efficiency, waste management and reduction of greenhouse gases emission were the most occurring environmental sustainability practices. Health and safety occurred as the dominant social sustainability practice, while economic sustainability practices have not been well defined, providing an opportunity for future research. The study provides a useful resource for managers and companies in the leather supply chain to learn from brands that have been embarking on sustainability efforts and assist them to a better understanding of the concept, in readiness for strategy formulation, implementation and reporting.

Society and the environment in the arid southwestern United States depend on reliable water availability, yet current water use outpaces supply. Water demand is projected to grow in the future and climate change is expected to reduce supply. To adapt, water managers need robust estimates of future regional water supply to support management decisions. To address this need, we estimate future streamflow in seven water resource regions in the southwestern U.S. using a new SPAtially Referenced Regressions On Watershed attributes (SPARROW) streamflow model. We present streamflow projections corresponding to input data from seven climate models and two greenhouse gas Representative Concentration Pathways (RCP4.5 and 8.5) for three, thirty-year intervals centered on the 2030s, 2050s, and 2080s, and for a historical thirty year interval centered on the 1990s. Across water resource regions, about half of the RCP4.5 models (51%) and two thirds of the RCP8.5 models (67%) indicate decreases in streamflow in the 2080s relative to the historical period. Models project maximum decreases in streamflow of 36–80% in all water resource regions for all periods and RCPs relative to historical streamflow, and maximum streamflow decreases of up to 20–45% in the 2080s at sites along the Colorado River used for measuring compliance with interstate and international water agreements. Headwaters are projected to experience the greatest declines, with substantial downstream implications. Among these estimates, the streamflows from models forced with RCP8.5 tend to be lower than those forced with RCP4.5. Not all climate models, times, and RCPs project widespread streamflow declines. The most ubiquitous streamflow increases are projected to occur in the 2030s under RCP4.5. Later time periods and enhanced greenhouse gas forcings indicate smaller regions of streamflow increase and lower accumulated streamflows, suggesting that limiting or reducing greenhouse gas concentrations could support future water availability. Although some possible streamflow increases are promising, the modest and spatially limited increases in streamflow projected for later time periods are still unlikely to be sufficient to meet the projected water demand. These results inform the likelihood of future water agreement compliance, and support developing strategies to balance water supply and demand.

Stereotactic body radiotherapy (SBRT) is an increasingly used radiation modality for the treatment of both localized and metastatic prostate cancer. Substantial data suggests that prostate cancer may be more sensitive to higher doses of radiation per fraction due to its low α/β ratio. This increased sensitivity raises important questions as to how SBRT should be combined with systemic therapy for clinically significant prostate cancer, including whether androgen deprivation therapy retains its beneficial effects when combined with SBRT. Furthermore, pre-clinical and clinical data suggest pronounced immunomodulatory effects of SBRT, including observed improvements in T cell priming and trafficking. These data support investigational strategies combining SBRT with immunotherapy. Here we aim to review the data for the use of SBRT in both the local and metastatic disease settings as well as ongoing translational and clinical research examining combinations with ADT, immunotherapy and other targeted agents.

Background: Germline testing is recommended for all men with metastatic prostate cancer (PCa), and for some with localized PCa meeting specific histologic or family history criteria. Germline genetic evaluation has important implications for PCa prognosis and management, as well as implications for family members and cancer screening. Despite the importance of germline evaluation, its utilization in urologic practice is unknown. Materials and Methods: We conducted a 32-item survey of U.S. urologists to examine knowledge of germline testing guidelines and practice patterns. It was shared through email to 6 American Urological Association sections, the Veterans Affairs Urology Mailgroup, and social media. Results: Among 132 total respondents from diverse practice settings across the U.S., 12% perform germline testing, 44% refer to a genetic counselor, 11% do both, and 33% do not test/refer. Only 4% had formal education in genetics. While 98% ask about PCa family history, only 76% and 52% ask about breast and ovarian cancer. When presented with hypothetical case scenarios where germline testing is indicated, many respondents indicated they would not offer genetic counseling or testing. Younger age (p = 0,03), academic practice (p = 0.04), and specializing in PCa/oncology (p = 0.007) were significantly associated with performing or referring for germline testing. Specializing in PCa/oncology was significantly associated with recommending germline testing for all case scenarios involving metastatic PCa (p = 0.0009) Conclusion: Our results suggest significant gaps in knowledge of germline testing and alignment of practice with national guidelines among urologists. Germline testing education and facilitation of genetic evaluation in urologic practice is warranted.

Abstract Background Teaching clinical audit skills to nascent health professionals is one strategy to improve frontline care. The undergraduate medical curriculum at the University of Auckland provides improvement science theory and skills in Year 5 teaching, and the opportunity to put this into practice during an Obstetrics and Gynaecology (O&G) clinical attachment in Year 6. In 2015, a revised medical school curriculum at the university resulted in a planned reduction of the O&G attachment from five weeks to four, necessitating revision of the Year 6 Quality Improvement (QI) project. The aim of this study was to evaluate if the revised programme provided an important experiential learning opportunity for medical students without imposing an unsustainable burden on clinical services. Methods Based on a CIPP (Context/Input/Process/Product) evaluation model, the study was conducted in several stages to get a sense of the context as the new programme was being planned (Context evaluation), the feasibility of an alternative approach to meet the educational need (Input evaluation), the implementation of the revised programme (Process evaluation) and finally, the programme outcomes (Product evaluation). We used multiple data sources (supervisors, students, academic administrators, and hospital staff) and data collection methods (questionnaires, focus groups, individual interviews, consultative workshops, student reports and oral presentations). Results The context evaluation revealed the Year 6 QI programme to be valuable and contributed to O&G service improvements, however, the following concerns were identified: time to complete the project, timely topic selection and access to data, recognition of student achievement, and staff workload. The evaluation of the revised QI project indicated improvement in student perceptions of their QI knowledge and skills, and most areas previously identified as challenging, despite the concurrent reduction in the duration of the O&G attachment. Conclusions Applying the CIPP model for evaluation to our revised QI programme enabled streamlining of procedures to achieve greater efficiency without compromising the quality of the learning experience, or increasing pressure on staff. A four week clinical rotation is adequate for medical educators to consider opportunities for including QI projects as part of student experiential learning.

Poly (ADP-ribose) polymerases (PARPs) catalyze the transfer of multiple poly(ADP-ribose) units onto target proteins. Poly(ADP-ribosyl)ation plays a crucial role in a variety of cellular processes including, most prominently, auto-activation of PARP at sites of DNA breaks to activate DNA repair processes. In humans, PARP1 (the founding and most characterized member of the PARP family) accounts for more than 90% of overall cellular PARP activity in response to DNA damage. We have found that, in contrast with animals, in Arabidopsis thaliana PARP2 (At4g02390), rather than PARP1 (At2g31320), makes the greatest contribution to PARP activity and organismal viability in response to genotoxic stresses caused by bleomycin, mitomycin C or gamma-radiation. Plant PARP2 proteins carry SAP DNA binding motifs rather than the zinc finger domains common in plant and animal PARP1 proteins. PARP2 also makes stronger contributions than PARP1 to plant immune responses including restriction of pathogenic Pseudomonas syringae pv. tomato growth and reduction of infection-associated DNA double-strand break abundance. For poly(ADP-ribose) glycohydrolase (PARG) enzymes, we find that Arabidopsis PARG1 and not PARG2 is the major contributor to poly(ADP-ribose) removal from acceptor proteins. The activity or abundance of PARP2 is influenced by PARP1 and PARG1. PARP2 and PARP1 physically interact with each other, and with PARG1 and PARG2, suggesting relatively direct regulatory interactions among these mediators of the balance of poly(ADP-ribosyl)ation. As with plant PARP2, plant PARG proteins are also structurally distinct from their animal counterparts. Hence core aspects of plant poly(ADP-ribosyl)ation are mediated by substantially different enzymes than in animals, suggesting the likelihood of substantial differences in regulation.
