Optical brain monitoring, such as near-infrared spectroscopy (NIRS), has facilitated numerous brain studies, including those based on machine learning techniques. A large and diverse dataset is necessary for training machine learning algorithms to avoid overfitting a limited amount of data. However, recruiting sufficient subjects is challenging owing to time and budget constraints. Therefore, we propose an NIRS data generation algorithm that scales NIRS signal components, such as hemodynamic response function, physiological systemic noise, and instrumental spike noise, based on the source-detector distance to augment the training data. Experimental self-paced left- and right-hand motor imagery data were augmented with generated NIRS data to train a convolutional neural network and classify the motor imagery data. Augmenting the training dataset with 1000 generated data increased the classification accuracy to 86.3 ± 4.1%, a 26% increase compared with training on experimental data only. In addition, we applied Guided Gradient-weighted Class Activation Mapping (Grad-CAM) to visualize the class discriminative features of the input data. The peaks of Guided Grad-CAM heatmaps aligned with the oxy-hemoglobin peaks during self-paced motor imagery. We concluded that the increased cerebral oxygenation, especially in the contralateral hemisphere, was the class-discriminative feature for classifying left- and right-hand motor imagery.

Background: The monitoring of brain activity along with genital organ response to sexual stimulation can play an important role in understanding the under-lying mechanisms of sexual arousal as well as diagnosing erectile dysfunction. Several studies have observed brain activity corresponding to sexual stimuli, but only a few studies have shown a simultaneous measurement of brain activation and penile response. Aim: To introduce near-infrared spectroscopy (NIRS) as a portable, easily implemented, and low-cost technique to simultaneously record brain activity and hemodynamics in the genital organ during sexual arousal. Methods: Hemodynamic measurements of 15 healthy men were obtained using a home-built NIRS system. In the initial experiment, hemodynamics in the pre-frontal cortex (N = 10) were measured during visual sexual stimulation (VSS) and neutral visual stimulation (NVS) to identify brain activity related to sexual arousal. In the subsequent experiment, cerebral and penile hemodynamics were simultaneously measured (N = 5) using NIRS during VSS and NVS. Results: The pre-frontal cortex showed activity related to VSS but not to NVS. Simultaneous measurements showed a corresponding increase of penile oxygenated and deoxygenated hemoglobin concentration indicating an increase of blood volume associated with sexual arousal in healthy men. An average response delay of 4 seconds was observed in the hemodynamic changes between the brain and genital organ. Conclusion: In this preliminary study, we presented a NIRS system capable not only of detecting cerebral hemodynamic changes related to sexual arousal but also the simultaneous measurement of penile hemodynamics. The head-up tilt table test (HUT) is one of the primary clinical examinations for evaluating orthostatic intolerance (OI). HUT can be divided into three phases: dynamic tilt phase (supine to tilt up), static tilt phase (remain tilted at 70°), and post tilt phase (tilt down back to supine position). Commonly, blood pressure (BP) and heart rate (HR) are monitored to observe for OI symptoms, but are indirect measurements of cerebral perfusion and can lead to inaccurate HUT evaluation. In this study, we implemented a 108-channel near-infrared spectroscopy (NIRS) probe to characterize HUT performance by monitoring cerebral hemodynamic changes for healthy controls (HCs), OI patients with normal HUT results, and OI patients with positive HUT results: vasovagal syncope (VS), postural orthostatic tachycardia syndrome (POTS), orthostatic hypotension (OH), and orthostatic hypertension (OHT). By the end of the static tilt phase, OI patients typically did not show a complete recovery back to baseline cerebral oxygenation and total blood volume compared to HCs. We characterized the return to cerebral homeostasis by polynomial fitting total blood volume changes and determining the inflection point. The OI patients with normal HUT results, VS, OH, or OHT showed a delay in the return to cerebral homeostasis compared to the HC group during HUT.

An 18-month-old Springer Spaniel was presented for investigation of ascites, exercise intolerance and suspected cor triatriatum dexter. Division of the right atrium into two separate chambers by an anomalous perforated membrane consistent with cor triatriatum dexter was confirmed during echocardiography. However, a routine agitated saline contrast (bubble) study yielded results that could not be explained by the congenital heart defect alone. Computed tomography angiography was performed and revealed a dilated ventral internal vertebral venous plexus and a short, dilated vessel in the midlumbar region redirecting blood flow away from the caudal vena cava and into the azygos vein. Balloon dilatation of the orifice within the cor triatriatum dexter membrane successfully reduced pressure within the caudal chamber of the right atrium and enhanced return via the caudal vena cava with subsequent resolution of clinical signs. This case report highlights the use of cross-sectional imaging in dogs with cor triatriatum dexter and unexpected contrast study results, as a means by which concurrent vascular anomalies may be identified. A combined diffuse speckle contrast analysis (DSCA)-near-infrared spectroscopy (NIRS) system is proposed to simultaneously measure qualitative blood flow and blood oxygenation changes in human tissue. The system employs an optical switch to alternate two laser sources at two different wavelengths and a CCD camera to capture the speckle image. Therefore, an optical density can be measured from two wavelengths for NIRS measurements and a speckle contrast can be calculated for DSCA measurements. In order to validate the system, a flow phantom test and an arm occlusion protocol for arterial and venous occlusion were performed. Shorter exposure times (<1 ms) show a higher drop (between 50% and 66%) and recovery of 1/KS² values after occlusion (approximately 150%), but longer exposure time (3 ms) shows more consistent hemodynamic changes. For four subjects, the 1/KS² values dropped to an average of 82.1 ± 4.0% during the occlusion period and the average recovery of 1/KS² values after occlusion was 109.1 ± 0.8%. There was also an approximately equivalent amplitude change in oxyhemoglobin (OHb) and deoxyhemoglobin (RHb) during arterial occlusion (maxRHb = 0.0085±0.0024mM/DPF, minOHb=-0.0057±0.0044mM/DPF). Background: In this paper, we observed a discrepancy of penile hemodynamics dependent on location by using near infrared spectroscopy (NIRS) sensor, and showcase NIRS as a potentially suitable sensor in supplementing the diagnosis and treatment of erectile dysfunction. Methods: To observe the effect that location has on penile hemodynamics, the NIRS sensor was placed on the top and the side of genital organ, and oxy-(HbO), deoxy-(RHb), and total (HbT) hemoglobin concentration changes were acquired. Our results from 6 healthy subjects show that hemodynamic changes vary depending on where the probe was placed. To observe a statistical difference between the signals, a Wilcoxon signed-rank test was performed. Results: The result shows a significant difference (p < 0.05) between concentration changes of RHb and HbT depending on the probes' location. Moreover, the sensor placed on the top of the organ shows a rise of HbO and HbT concentration while RHb concentration decreased. However, hemodynamics from the side of the organ showed that RHb concentration increased along with HbO. Conclusions: The outcomes demonstrates an ability of NIRS to be sensitive enough to detect the different hemodynamic changes in various locations of a healthy male genital organ during visual sexual stimulation. The results also show the importance of sensor location on the genital organ for the resulting hemodynamic changes. We can foresee our results as a way for clinicians to obtain more accurate hemodynamic measurements from the penis, and also show the likelihood for NIRS enhanced diagnosis tool of male erectile dysfunction over the current standards. Health and disease management (HDM) programs have faced challenges in documenting savings related to their implementation. The objective of this eliminate study was to describe OptumHealth's (Optum) methods for estimating anticipated savings from HDM programs using Value Drivers. Optum's general methodology was reviewed, along with details of 5 high-use Value Drivers. The results showed that the Value Driver approach offers an innovative method for estimating savings associated with HDM programs. The authors demonstrated how real-time savings can be estimated for 5 Value Drivers commonly used in HDM programs: (1) use of beta-blockers in treatment of heart disease, (2) discharge planning for high-risk patients, (3) decision support related to chronic low back pain, (4) obesity management, and (5) securing transportation for primary care. The validity of savings estimates is dependent on the type of evidence used to gauge the intervention effect, generating changes in utilization and, ultimately, costs. The savings estimates derived from the Value Driver method are generally reasonable to conservative and provide a valuable framework for estimating financial impacts from evidence-based interventions. Closure is a problem of defining the convective intensity in a given parameterization. In spite of many years of efforts and progress, it is still considered an overall unresolved problem. The present article reviews this problem from phenomenological perspectives The physical variables that may contribute in defining the convective intensity are listed, and their statistical significances identified by observational data analyses are reviewed. A possibility is discussed for identifying a correct closure hypothesis by performing a linear stability analysis of tropical convectively coupled waves with various different closure hypotheses. Various individual theoretical issues are considered from various different perspectives. The review also emphasizes that the dominant physical factors controlling convection differ between the tropics and extra-tropics, as well as between oceanic and land areas. Both observational as well as theoretical analyses, often focused on the tropics, do not necessarily lead to conclusions consistent with our operational experiences focused on midlatitudes. Though we emphasize the importance of the interplays between these observational, theoretical and operational perspectives, we also face challenges for establishing a solid research framework that is universally applicable. Some observational evidence--such as bimodal drop size distributions, comparatively high concentrations of supercooled drops at upper levels, high concentrations of small ice crystals in cloud anvils leading to high optical depth, and lightning in the eyewalls of hurricanes--indicates that the traditional view of the microphysics of deep tropical maritime clouds requires, possibly, some revisions. In the present study it is shown that the observed phenomena listed above can be attributed to the presence of small cloud condensation nuclei (CCN) with diameters less than about 0.05/zm. An increase in vertical velocity above cloud base can lead to an increase in supersaturation and to activation of the smallest CCN, resulting in production of new droplets several kilometers above the cloud base. A significant increase in supersaturation can be also caused by a decrease in droplet concentration during intense warm rain formation accompanied by an intense vertical velocity. This increase in supersaturation also can trigger in-cloud nucleation and formation of small droplets. Another reason for an increase in supersaturation and in-cloud nucleation can be riming, resulting in a decrease in droplet concentration. It has been shown that successive growth of new nucleated droplets increases supercooled water content and leads to significant ice crystal concentrations aloft. The analysis of the synergetic effect of the smallest CCN and giant CCN on production of supercooled water and ice crystals in cloud anvils allows reconsideration of the role of giant CCN. Significant effects of small aerosols on precipitation and cloud updrafts have been found. The possible role of these small aerosols as well as small aerosols with combination of giant CCN in creating conditions favorable for lightning in deep maritime clouds is discussed. Study design:We analyzed longitudinal data on secondary outcomes from participants in a telerehabilitation study.Objectives:To examine the factors affecting return to productive activities and employment and the time to these events following a spinal cord injury (SCI).Setting:A large southeastern rehabilitation hospital in the United States.Methods:We used hazard regression models to analyze data from newly injured people (n=111) participating in an educational intervention post discharge who were followed for up to 2 years. Outcomes were time to return to productive activities and employment.Results:Increasing age and being on Medicaid significantly decreased the likelihood of returning to productive activities (P<0.01), while being white (P<0.05) and having a higher median income (P<0.001) significantly increased this probability. The same factors, bar being on Medicaid, affected the return to employment. Whites returned to productive activities 2.5 times sooner than African Americans and employment twice as fast (P<0.001). Being in the 75th income percentile compared with the 25th shortened time to employment by 209 days.Conclusion:Findings here suggest that income and race affect the time to return to productivity and employment, while being on Medicaid also has a role in general post injury productivity. Ice in atmospheric clouds undergoes complex physical processes, interacting especially with radiation, which leads to serious impacts on global climate. After their primary production, atmospheric ice crystals multiply extensively by secondary processes. Here, it is shown that a mostly overlooked process of mechanical breakup of ice particles by ice--ice collisions contributes to such observed multiplication. A regime for explosive multiplication is identified in its phase space of ice multiplication efficiency and number concentration of ice particles. Many natural mixed-phase clouds, if they have copious millimeter-sized graupel, fall into this explosive regime. The usual Hallett--Mossop (H--M) process of ice multiplication is shown to dominate the overall ice multiplication when active, as it starts sooner, compared to the breakup ice multiplication process. However, for deep clouds with a cold base temperature where the usual H--M process is inactive, the ice breakup mechanism should play a critical role. Supercooled rain, which may freeze to form graupel directly in only a few minutes, is shown to hasten such ice multiplication by mechanical breakup, with an ice enhancement ratio exceeding 104 approximately 20 min after small graupel first appear. The ascent-dependent onset of subsaturation with respect to liquid water during explosive ice multiplication is predicted to determine the eventual ice concentrations. [ABSTRACT FROM AUTHOR]

A detailed study is presented of multicarrier transport properties in liquid-phase epitaxy (LPE)-grown n-type HgCdTe films using advanced mobility spectrum analysis techniques over the temperature range from 95 K to 300 K. Three separate electron species were identified that contribute to the total conduction, and the temperature-dependent characteristics of carrier concentration and mobility were extracted for each individual carrier species. Detailed analysis allows the three observed contributions to be assigned to carriers located in the bulk long-wave infrared (LWIR) absorbing layer, the wider-gap substrate/HgCdTe transition layer, and a surface accumulation layer. The activation energy of the dominant high-mobility LWIR bulk carrier concentration in the high temperature range gives a very good fit to the Hansen and Schmit expression for intrinsic carrier concentration in HgCdTe with a bandgap of 172 meV. The mobility of these bulk electrons follows the classic μ ~ T−3/2 dependence for the phonon scattering regime. The much lower sheet densities found for the other two, lower-mobility electron species show activation energies of the order of ~20 meV, and mobilities that are only weakly dependent on temperature and consistent with expected values for the wider-bandgap transition layer and a surface accumulation layer. The clinical hallmark of Alzheimer's disease (AD) is impairment of cognition associated with loss of synapses, accumulation of amyloid-beta (Abeta) both within neurons and as extracellular deposits, and neurofibrillary degeneration composed of phospho-tau. Neurons in the hippocampus are among those that are most vulnerable. The purpose of this study was to investigate the expression of genes associated with cognition, synapse, and mitochondrial function in hippocampal neurons of AD compared to normal individuals. Neurons from the hippocampus with intraneuronal Abeta immunoreactivity were captured with laser microdissection; RNA was extracted; and levels of brain-derived neurotrophic factor (BDNF), TrkB (BDNF receptor), dynamin-1 (DYN), and cytochrome C oxidase subunit II (COX2) were assessed with quantitative real-time polymerase chain reaction. We found no significant differences in the expression of these genes in AD between neurons associated with Abeta compared to those lacking Abeta immunoreactivity. Double immunofluorescence microscopy showed the number of hippocampal neurons coexpressing Abeta or phospho-tau and either BDNF, TrkB, or DYN was similar in AD and controls. Our results suggest that neither intraneuronal Abeta nor phospho-tau has obligatory effects on reducing the expression of genes important for memory and cognition in hippocampus of AD. Cloud and aerosol effects on radiation in two contrasting cloud types, a deep mesoscale convective system (MCS) and warm stratocumulus clouds, are simulated and compared. At the top of the atmosphere, 45-81% of shortwave cloud forcing (SCF) is offset by longwave cloud forcing (LCF) in the MCS, whereas warm stratiform clouds show the offset of less than ~20%. 28% of increased negative SCF is offset by increased LCF with increasing aerosols in the MCS at the top of the atmosphere. However, the stratiform clouds show the offset of just around 2-5%. Ice clouds as well as liquid clouds play an important role in the larger offset in the MCS. Lower cloud-top height and cloud depth, characterizing cloud types, lead to the smaller offset of SCF by LCF and the offset of increased negative SCF by increased LCF at high aerosol in stratocumulus clouds than in the MCS. Supplementary simulations show that this dependence of modulation of LCF on cloud depth and cloud-top height is also simulated among different types of convective clouds. [ABSTRACT FROM AUTHOR]

A cloud system resolving model including photochemistry (CSRMC) has been developed based on a prototype version of theWeather Research and Forecasting (WRF) model and is used to study influences of deep convection on chemistry in the TOGA COARE/CEPEX region. Lateral boundary conditions for trace gases are prescribed from global chemistry-transport simulations, and the vertical advection of trace gases by large scale dynamics, which is not reproduced in a limited area cloud system resolving model, is taken into account. The influences of deep convective transport and of lightning on NOx, O3, and HOx(=HO2+OH), in the vicinity of the deep convective systems are investigated in a 7-day 3-D 248×248 km² horizontal domain simulation and several 2-D sensitivity runs with a 500 km horizontal domain. Mid-tropospheric entrainment is more important on average for the upward transport of O3 in the 3-D run than in the 2-D runs, but at the same time undiluted O3- poor air from the marine boundary layer reaches the upper troposphere more frequently in the 3-D run than in the 2-D runs, indicating the presence of undiluted convective cores. In all runs, in situ lightning is found to have only minor impacts on the local O3 budget. Near zero O3 volume mixing ratios due to the reaction with lightning-produced NO are only simulated in a 2-D sensitivity run with an extremely high number of NO molecules per flash, which is outside the range of current estimates. The fraction of NOx chemically lost within the domain varies between 20 and 24% in the 2-D runs, but is negligible in the 3-D run, in agreement with a lower average NOx concentration in the 3-D run despite a greater number of flashes. Stratosphere to troposphere transport of O3 is simulated to occur episodically in thin filaments in the 2-D runs, but on average net upward transport of O3 from below ∼16 km is simulated in association with mean large scale ascent in the region. Ozone profiles in the TOGA COARE/CEPEX region are suggested to be strongly influenced by the intra-seasonal (Madden-Julian) oscillation. [ABSTRACT FROM AUTHOR]

The article provides information related to cloud system resolving model (CSRM). The meteorological component of the CSRM is based on a modified height coordinate prototype version of the nonhydrostatic, compressible Weather and Research and Forecasting (WRF) model. The WRF-base CSRM is designed with the capability of running on distributed memory parallel platforms such as Linux clusters.

A cloud modeling framework is described to simulate ice nucleation by biogenic aerosol particles, as represented by airborne ice-nucleation active (INA) bacteria. It includes the empirical parameterization of heterogeneous ice nucleation. The formation of cloud liquid by soluble material coated on such insoluble aerosols is represented and determines their partial removal from deep convective clouds by accretion onto precipitation. Preliminary simulations are performed for a case of deep convection over Oklahoma. If present at high enough concentrations, as might occur in proximity to land sources, INA bacteria are found to influence significantly: - (1) the average numbers and sizes of crystals in the clouds; (2) the horizontal cloud coverage in the free troposphere; and (3) precipitation and incident solar insolation at the surface, which influence rates of bacterial growth. At lower concentrations, the corresponding responses of cloud fields appear much lower or are ambiguous. In nature, the growth rates of INA bacteria on leaves prior to emission into the at mosphere are known to be highly dependent on temperature, precipitation and plant species. Consequently, the open question emerges of whether emissions of such icenucleating biogenic particles can then be modified by their own effects on clouds and atmospheric conditions, forming a weak feedback in climate or microclimate systems. The role of the retention coefficient (i.e. the fraction of a dissolved trace gas which is retained in hydrometeors during freezing) for the scavenging and redistribution of highly soluble trace gases by deep convective cloud systems is investigated using a modified version of the Weather Research and Forecasting (WRF) model. Results from cloud system resolving model runs (in which deep convection is initiated by small random perturbations in association with so-called "large scale forcings (LSF)") for a tropical oceanic (TOGA COARE) and a mid-latitude continental case (ARM) are compared to two runs in which bubbles are used to initiate deep convection (STERAO, ARM). In the LSF runs, scavenging is found to almost entirely prevent a highly soluble tracer initially located in the lowest 1.5 km of the troposphere from reaching the upper troposphere, independent of the retention coefficient. The release of gases from freezing hydrometeors leads to mixing ratio increases in the upper troposphere comparable to those calculated for insoluble trace gases only in the two runs in which bubbles are used to initiate deep convection. A comparison of the two ARM runs indicates that using bubbles to initiate deep convection may result in an overestimate of the influence of the retention coefficient on the vertical transport of highly soluble tracers. It is, however, found that the retention coefficient plays an important role for the scavenging and redistribution of highly soluble trace gases with a (chemical) source in the free troposphere and also for trace gases for which even relatively inefficient transport may be important. The large difference between LSF and bubble runs is attributed to differences in dynamics and microphysics in the inflow regions of the storms. The dependence of the results on the model setup indicates the need for additional model studies with a more realistic initiation of deep convection, e.g., considering effects of orography in a nested model setup.
