Abstract

The Mal'tsev product of two varieties of the same similarity type is not in general a variety, because it can fail to be closed under homomorphic images. In the previous paper we provided a new sufficient condition for such a product to be a variety. In this paper we extend that result by weakening the assumptions regarding the two varieties. We also explore the various special cases of our new result and provide a number of examples of its application.

19. Implementation of a Telephone-Triage Simulation-Based Experience Into Undergraduate Nursing Curriculum.

Abstract

• This article describes the development of a telephone triage SBE for undergraduate nursing students in a community-based course, including details on scenario design, curriculum integration, prebriefing, debriefing, and evaluation. • The SBE is designed to be delivered remotely without the use of high-fidelity simulators. • The SBE uses Briggs Telephone Triage protocols, which allow students to not only practice triage assessments, but also clinical judgment within their scope of practice. • Students gained experience and confidence communicating, making clinical-decisions, and providing evidence-based assessments for safe patient care. Due to changes in health care delivery, there is an increased emphasis on treating patients in the outpatient setting. Phone triage is widely used by nurses in outpatient settings and Simulation-Based Experience (SBE) offers a number of educational benefits. A telephone-triage SBE was developed to be delivered remotely to teach nursing students best practices, protocols, and competencies specific to telephone triage nursing. Seventy-seven undergraduate nursing students participated in the SBE. Thirty-four students completed an online Simulation Effectiveness Tool-Modified (SET-M) and reported overall positive feedback. 20. Low prevalence of cardiac abnormalities in competitive athletes at return-to-play after COVID-19.

Abstract

to evaluate the prevalence of cardiac involvement after COVID-19 in competitive athletes at return-to-play (RTP) evaluation, following the recommended Italian protocol including cardiopulmonary exercise test (CPET) and 24-Hour Holter monitoring. this is a single centre observational, cross-sectional study. Since October 2020, all competitive athletes (age ≥ 14 years) evaluated in our Institute after COVID-19, prior RTP were enrolled. The protocol dictated by the Italian governing bodies included: 12‑lead ECG, blood test, CPET, 24-h ECG monitoring, spirometry. Cardiovascular Magnetic Resonance (CMR) was performed based on clinical indication. 219 consecutive athletes were examined (59% male), age 23 years (IQR 19–27), 21% asymptomatic, 77% mildly symptomatic, 2% with previous pneumonia. The evaluation was performed after a median of 10 (6–17) days from negative SARS-CoV-2 swab. All athletes showed a good exercise capacity at CPET without cardiovascular and respiratory limitations. Uncommon premature ventricular contractions (PVCs) were found in 9.5% (n = 21) at CPET/Holter ECG monitoring. Two athletes (0.9%) were diagnosed with acute myocarditis (by CMR) and another one with new pericardial effusion. All the three athletes were temporally restricted from sport participation. Myocarditis in competitive athletes screened after COVID-19 resolution was detected in a low minority of the cases (0.9%). However, a non-negligible prevalence of uncommon PVCs (9%) was observed, either at CPET and/or Holter ECG monitoring, including all athletes with COVID-19 related cardiovascular abnormalities. 46. Neuroadaptive Bayesian optimisation to study individual differences in infants' engagement with social cues.

Abstract

Infants' motivation to engage with the social world depends on the interplay between individual brain's characteristics and previous exposure to social cues such as the parent's smile or eye contact. Different hypotheses about why specific combinations of emotional expressions and gaze direction engage children have been tested with group-level approaches rather than focusing on individual differences in the social brain development. Here, a novel Artificial Intelligence-enhanced brain-imaging approach, Neuroadaptive Bayesian Optimisation (NBO), was applied to infant electro-encephalography (EEG) to understand how selected neural signals encode social cues in individual infants. EEG data from 42 6- to 9-month-old infants looking at images of their parent's face were analysed in real-time and used by a Bayesian Optimisation algorithm to identify which combination of the parent's gaze/head direction and emotional expression produces the strongest brain activation in the child. This individualised approach supported the theory that the infant's brain is maximally engaged by communicative cues with a negative valence (angry faces with direct gaze). Infants attending preferentially to faces with direct gaze had increased positive affectivity and decreased negative affectivity. This work confirmed that infants' attentional preferences for social cues are heterogeneous and shows the NBO's potential to study diversity in neurodevelopmental trajectories. 50. Single Center Five Years' Experience of Ozaki Procedure: Midterm Follow-up.

Abstract

The Ozaki procedure is an innovative surgical technique aiming at the reconstruction of the aortic valve with autologous pericardium to overcome the limitations of commercially available prostheses. We retrospectively analyzed 71 patients who underwent the Ozaki operation at our center between October 2014 and February 2020. No in-hospital death occurred. Freedom from major adverse valve-related events was 97%. The aortic gradients and transvalvular velocity were significantly lower at the 3-month echocardiographic control than at the predischarge echocardiography (10.93 ± 5.38, P <.01 vs 16.24 ± 7.67, P <.01, respectively). The median follow-up period was 20.7 months (range, 2 to 47). Four patients showed mild/moderate aortic insufficiency (5.6%), and none showed severe aortic valve insufficiency. No patients underwent reoperation. Midterm outcome follow-up of the Ozaki procedure showed optimal results in terms of mortality, transaortic valve gradients, freedom from major adverse valve-related events, and recurrence of aortic valve insufficiency.
